According to the latest AM65x Data Manual[1], a different output tap
delay value is optimum for a given speed mode. Therefore, deprecate the
ti,otap-del-sel binding and introduce a new binding for each of the
possible MMC/SD speed modes. If the legacy mode is not found, fall back
to old binding to maintain dts compatibility.

Now the MMC read/write stack will always wait for previous request is
completed by mmc_blk_rw_wait(), before sending a new request to hardware,
or queue a work to complete request, that will bring context switching
overhead and spend some extra time to poll the card for busy completion
for I/O writes via sending CMD13, especially for high I/O per second
rates, to affect the IO performance.
Thus this patch introduces MMC software queue interface based on the
hardware command queue engine's interfaces, which is similar with the
hardware command queue engine's idea, that can remove the context
switching. Moreover we set the default queue depth as 64 for software
queue, which allows more requests to be prepared, merged and inserted
into IO scheduler to improve performance, but we only allow 2 requests
in flight, that is enough to let the irq handler always trigger the
next request without a context switch, as well as avoiding a long latency.
Moreover the host controller should support HW busy detection for I/O
operations when enabling the host software queue. That means, the host
controller must not complete a data transfer request, until after the
card stops signals busy.
From the fio testing data in cover letter, we can see the software
queue can improve some performance with 4K block size, increasing
about 16% for random read, increasing about 90% for random write,
though no obvious improvement for sequential read and write.
Moreover we can expand the software queue interface to support MMC
packed request or packed command in future.

It has turned out that some host controllers can't use R1B for CMD6 and
other commands that have R1B associated with them. Therefore invent a new
host cap, MMC_CAP_NEED_RSP_BUSY to let them specify this.
In __mmc_switch(), let's check the flag and use it to prevent R1B responses
from being converted into R1. Note that, this also means that the host are
on its own, when it comes to manage the busy timeout.

With large eMMC cards, it is possible to create general purpose
partitions that are bigger than 4GB. The size member of the mmc_part
struct is only an unsigned int which overflows for gp partitions larger
than 4GB. Change this to a u64 to handle the overflow.

v4.11-rc1 did introduce a patch series that rearranged the
sdio quirks into a header file. Unfortunately this did forget
to handle SDIO_VENDOR_ID_TI differently between wl1251 and
wl1271 with the result that although the wl1251 was found on
the sdio bus, the firmware did not load any more and there was
no interface registration.
This patch defines separate constants to be used by sdio quirks
and drivers.

It have turned out that it's not a good idea to unconditionally do a power
cycle and then to re-initialize the SDIO card, as currently done through
mmc_hw_reset() -> mmc_sdio_hw_reset(). This because there may be multiple
SDIO func drivers probed, who also shares the same SDIO card.
To address these scenarios, one may be tempted to use a notification
mechanism, as to allow the core to inform each of the probed func drivers,
about an ongoing HW reset. However, supporting such an operation from the
func driver point of view, may not be entirely trivial.
Therefore, let's use a more simplistic approach to solve the problem, by
instead forcing the card to be removed and re-detected, via scheduling a
rescan-work. In this way, we can rely on existing infrastructure, as the
func driver's ->remove() and ->probe() callbacks, becomes invoked to deal
with the cleanup and the re-initialization.
This solution may be considered as rather heavy, especially if a func
driver doesn't share its card with other func drivers. To address this,
let's keep the current immediate HW reset option as well, but run it only
when there is one func driver probed for the card.
Finally, to allow the caller of mmc_hw_reset(), to understand if the reset
is being asynchronously managed from a scheduled work, it returns 1
(propagated from mmc_sdio_hw_reset()). If the HW reset is executed
successfully and synchronously it returns 0, which maintains the existing
behaviour.

The commit 38c38cb732 ("mmc: queue: use bigger segments if DMA MAP
layer can merge the segments") always enables the bugger segments
if DMA MAP layer can merge the segments, but some controllers (SDHCI)
have strictly limitation about the segments size, and then the commit
breaks on the controllers.
To fix the issue, this patch adds a new flag MMC_CAP2_MERGE_CAPABLE
into the struct mmc_host and the bigger segments usage is disabled
as default.

For the MMC_CAP2_SDIO_IRQ_NOTHREAD case and when using sdio_signal_irq(),
the ->ack_sdio_irq() is already mandatory, which was not the case for those
host drivers that called sdio_run_irqs() directly.
As there are no longer any drivers calling sdio_run_irqs(), let's clarify
the code by dropping the unnecessary check and explicitly state that the
callback is mandatory in the header file.

To avoid each host driver supporting SDIO IRQs, from keeping track
internally about if SDIO IRQs has been claimed, let's introduce a common
helper function, sdio_irq_claimed().
The function returns true if SDIO IRQs are claimed, via using the
information about the number of claimed irqs. This is safe, even without
any locks, as long as the helper function is called only from
runtime/system suspend callbacks of the host driver.

When the max_segs of a mmc host is smaller than 512, the mmc
subsystem tries to use 512 segments if DMA MAP layer can merge
the segments, and then the mmc subsystem exposes such information
to the block layer by using blk_queue_can_use_dma_map_merging().

We want SDIO drivers to be able to temporarily stop retuning when the
driver knows that the SDIO card is not in a state where retuning will
work (maybe because the card is asleep). We'll move the relevant
functions to a place where drivers can call them.

Normally when the MMC core sees an "-EILSEQ" error returned by a host
controller then it will trigger a retuning of the card. This is
generally a good idea.
However, if a command is expected to sometimes cause transfer errors
then these transfer errors shouldn't cause a re-tuning. This
re-tuning will be a needless waste of time. One example case where a
transfer is expected to cause errors is when transitioning between
idle (sometimes referred to as "sleep" in Broadcom code) and active
state on certain Broadcom WiFi SDIO cards. Specifically if the card
was already transitioning between states when the command was sent it
could cause an error on the SDIO bus.
Let's add an API that the SDIO function drivers can call that will
temporarily disable the auto-tuning functionality. Then we can add a
call to this in the Broadcom WiFi driver and any other driver that
might have similar needs.
NOTE: this makes the assumption that the card is already tuned well
enough that it's OK to disable the auto-retuning during one of these
error-prone situations. Presumably the driver code performing the
error-prone transfer knows how to recover / retry from errors. ...and
after we can get back to a state where transfers are no longer
error-prone then we can enable the auto-retuning again. If we truly
find ourselves in a case where the card needs to be retuned sometimes
to handle one of these error-prone transfers then we can always try a
few transfers first without auto-retuning and then re-try with
auto-retuning if the first few fail.
Without this change on rk3288-veyron-minnie I periodically see this in
the logs of a machine just sitting there idle:

This patch introduces the module_sdio_driver macro which is a convenience
macro for SDIO driver modules similar to module_usb_driver. It is intended
to be used by drivers which init/exit section does nothing but register/
unregister the SDIO driver. By using this macro it is possible to eliminate
a few lines of boilerplate code per SDIO driver.

SD spec v5.1 adds discard support. The flows and commands are similar to
mmc, so just set the discard arg in CMD38.
A host which supports DISCARD shall check if the DISCARD_SUPPORT (b313)
is set in the SD_STATUS register. If the card does not support discard,
the host shall not issue DISCARD command, but ERASE command instead.
Post the DISCARD operation, the card may de-allocate the discarded
blocks partially or completely. So the host mustn't make any assumptions
concerning the content of the discarded region. This is unlike ERASE
command, in which the region is guaranteed to contain either '0's or
'1's, depends on the content of DATA_STAT_AFTER_ERASE (b55) in the scr
register.
One more important difference compared to ERASE is the busy timeout
which we will address on the next patch.

The mmc regulator helper functions, are placed in the extensive core.c
file. In a step towards trying to create a better structure of files,
avoiding too many lines of code per file, let's move these helpers to a new
file, regulator.c.
Moreover, this within this context it makes sense to also drop the export
of mmc_vddrange_to_ocrmask(), but instead let's make it internal to the mmc
core.

The argument "override_active_level" made it possible to
enforce a specific polarity on the write-protect
GPIO line. All callers in the kernel pass "false" to this
call after I have converted all drivers to use GPIO machine
descriptors, so remove the argument and clean out this.
This kind of polarity inversion should be handled by the
GPIO descriptor inside the GPIO library if needed.
This rids us of one instance of the kludgy calls into
the gpiod_get_raw_value() API.

The BCM2835 MMC host driver uses a pointer to get from the private
bcm2835_host structure to the generic mmc_host structure. However the
latter is always immediately preceding the former in memory, so compute
its address with a subtraction (which is cheaper than a dereference) and
drop the superfluous pointer.
No functional change intended.

The kblockd workqueue is created with the WQ_MEM_RECLAIM flag set.
This generates a rescuer thread for that queue that will trigger when
the CPU is under heavy load and collect the uncompleted work.
In the case of mmc, this creates the possibility of a deadlock when
there are multiple partitions on the device as other blk-mq work is
also run on the same queue. For example:
- worker 0 claims the mmc host to work on partition 1
- worker 1 attempts to claim the host for partition 2 but has to wait
for worker 0 to finish
- worker 0 schedules complete_work to release the host
- rescuer thread is triggered after time-out and collects the dangling
work
- rescuer thread attempts to complete the work in order starting with
claim host
- the task to release host is now blocked by a task to claim it and
will never be called
The above results in multiple hung tasks that lead to failures to
mount partitions.
Handling complete_work on a separate workqueue avoids this by keeping
the work completion tasks separate from the other blk-mq work. This
allows the host to be released without getting blocked by other tasks
attempting to claim the host.

To allow mmc host drivers to inform the mmc core about rather using
pm_runtime_put_sync_suspend() instead of pm_runtime_put_autosuspend(),
let's introduce MMC_CAP_SYNC_RUNTIME_PM.
This is especially useful for those mmc host drivers that don't benefit
from using the runtime PM autosuspend feature. Typically this is those that
relies on parent devices to power the card via runtime PM, like some USB
host drivers for example.

All host drivers are converted to look up GPIO descriptors
from device tree, ACPI or machine descriptor tables, so now
we can delete the legacy GPIO handling using hardcoded GPIO
numbers from the kernel.

Some eMMC controllers need specific settings for HS400 mode before the
speed mode can be switched to DDR mode, during the HS400 initialization
sequence. For that reason, let's introduce a new host callback,
->hs400_prepare_ddr() and invoked it just before switching to DDR mode.

This adds two new HS400 tuning operations:
* hs400_downgrade
* hs400_complete
These supplement the existing HS400 operation:
* prepare_hs400_tuning
This is motivated by a requirement of Renesas SDHI for the following:
1. Disabling SCC before selecting to HS if selection of HS400 has occurred.
This can be done in an implementation of prepare_hs400_tuning_downgrade
2. Updating registers after switching to HS400
This can be done in an implementation of complete_hs400_tuning
If hs400_downgrade or hs400_complete are not implemented then they are not
called. Thus means there should be no affect for existing drivers as none
implemt these ops.

R1_STATUS(x) now is only used by ioctl_rpmb_card_status_poll(),
which checks all bits as possible. But according to the spec,
bit 17 and bit 18 should be ignored, as well bit 14 which is
reserved(must be set to 0) quoting from the spec and these rule
apply to all places checking the device status. So change
its checking from 0xFFFFE000 to 0xFFF9A000.
As a bonus, we reuse it for mmc_do_erase() as well as
mmc_switch_status_error().
(1) Currently mmc_switch_status_error() doesn't check bit 25, but
it means device is locked but not unlocked by CMD42 prior to any
operations which need check busy, which is also not allowed.
(2) mmc_do_erase() also forgot to to check bit 15, WP_ERASE_SKIP.
The spec says "Only partial address space was erased due to existing
write protected blocks.", which obviously means we should fail this I/O.
Otherwise, the partial erased data stored in nonvalatile flash violates
the data integrity from the view of I/O owner, which probably confuse
it when further used.
So reusing R1_STATUS for them not only improve the readability but also
slove real problems.

The hard-coded 10ms delay in mmc_power_up came from
commit 79bccc5aef ("mmc: increase power up delay"), which said "The TI
controller on Toshiba Tecra M5 needs more time to power up or the cards
will init incorrectly or not at all." But it's too engineering solution
for a special board but force all platforms to wait for that long time,
especially painful for mmc_power_up for eMMC when booting.
However, it's added since 2009, and we can't tell if other platforms
benefit from it. But in practise, the modern hardware are most likely to
have a stable power supply with 1ms after setting it for no matter PMIC
or discrete power. And more importnatly, most regulators implement the
callback of ->set_voltage_time_sel() for regulator core to wait for
specific period of time for the power supply to be stable, which means
once regulator_set_voltage_* return, the power should reach the the
minimum voltage that works for initialization. Of course, if there
are some other ways for host to power the card, we should allow them
to argue a suitable delay as well.
With this patch, we could assign the delay from firmware, or we could
assigne it via ->set_ios() callback from host drivers.

Add support for the BCM43364 chipset via an SDIO interface, as used in
e.g. the Murata 1FX module.
The BCM43364 uses the same firmware as the BCM43430 (which is already
included), the only difference is the omission of Bluetooth.
However, the SDIO_ID for the BCM43364 is 02D0:A9A4, giving it a MODALIAS
of sdio:c00v02D0dA9A4, which doesn't get recognised and hence doesn't
load the brcmfmac module. Adding the 'A9A4' ID in the appropriate place
triggers the brcmfmac driver to load, and then correctly use the
firmware file 'brcmfmac43430-sdio.bin'.

It's rather common that a firmware is loaded into an SDIO func device
memory, by the corresponding SDIO func driver during ->probe() time.
However, to actually start running the new firmware, sometimes a soft reset
(no power cycle) and a re-initialization of the card is needed. This is for
example the case with the Espressif ESP8089 WiFi chips, when connected to
an SDIO interface.
To cope with this scenario, let's add a new exported function,
mmc_sw_reset(), which may be called when a soft reset and re-initialization
of the card are needed.
The mmc_sw_reset() is implemented on top of a new bus ops callback, similar
to how the mmc_hw_reset() has been implemented.

The SDHCI controller in a SoC might support HS200/HS400 (indicated
using mmc-hs200-1_8v/mmc-hs400-1_8v dt property), but if the board is
modeled such that the IO lines are not connected to 1.8v then
HS200/HS400 cannot be supported. Disable HS200/HS400 if the board
does not have 1.8v connected to the IO lines. Also Disable DDR/UHS in 1.8v
if the IO lines are not connected to 1.8v.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

Currently the host can be claimed by a task. Change this so that the host
can be claimed by a context that may or may not be a task. This provides
for the host to be claimed by a block driver queue to support blk-mq, while
maintaining compatibility with the existing use of mmc_claim_host().

In may, Steven sent a patch deleting the bounce buffer handling
and the CONFIG_MMC_BLOCK_BOUNCE option.
I chose the less invasive path of making it a runtime config
option, and we merged that successfully for kernel v4.12.
The code is however just standing in the way and taking up
space for seemingly no gain on any systems in wide use today.
Pierre says the code was there to improve speed on TI SDHCI
controllers on certain HP laptops and possibly some Ricoh
controllers as well. Early SDHCI controllers lacked the
scatter-gather feature, which made software bounce buffers
a significant speed boost.
We are clearly talking about the list of SDHCI PCI-based
MMC/SD card readers found in the pci_ids[] list in
drivers/mmc/host/sdhci-pci-core.c.
The TI SDHCI derivative is not supported by the upstream
kernel. This leaves the Ricoh.
What we can however notice is that the x86 defconfigs in the
kernel did not enable CONFIG_MMC_BLOCK_BOUNCE option, which
means that any such laptop would have to have a custom
configured kernel to actually take advantage of this
bounce buffer speed-up. It simply seems like there was
a speed optimization for the Ricoh controllers that noone
was using. (I have not checked the distro defconfigs but
I am pretty sure the situation is the same there.)
Bounce buffers increased performance on the OMAP HSMMC
at one point, and was part of the original submission in
commit a45c6cb816 ("[ARM] 5369/1: omap mmc: Add new
omap hsmmc controller for 2430 and 34xx, v3")
This optimization was removed in
commit 0ccd76d4c2 ("omap_hsmmc: Implement scatter-gather
emulation")
which found that scatter-gather emulation provided even
better performance.
The same was introduced for SDHCI in
commit 2134a922c6 ("sdhci: scatter-gather (ADMA) support")
I am pretty positively convinced that software
scatter-gather emulation will do for any host controller what
the bounce buffers were doing. Essentially, the bounce buffer
was a reimplementation of software scatter-gather-emulation in
the MMC subsystem, and it should be done away with.

and the task id (tag), and allow for cmd being NULL which it is for CQE

The intention of this check was to prevent the conflict between
hotplug and removing driver for whatever reason. Currently it
doesn't improve anything and the following rescan process could
still saftly perform the scan flow. So these code seems pointless
now and let's remove them.

The MMC_CAP2_HC_ERASE_SZ is used only by a few mmc host drivers. Its intent
is to enable eMMC's high-capacity erase size, as to improve the behaviour
of the erase operations.
We should strive to avoid software configuration options that aren't
necessary, but instead deploy common behaviours. For these reasons, let's
remove the capability bit for MMC_CAP2_HC_ERASE_SZ and make it the default
behaviour.
Note that this change doesn't affect eMMCs supporting trim/discard, because
these commands operates on sectors and takes precedence over erase
commands.

The mmc_queue_req is a per-request state container the MMC core uses
to carry bounce buffers, pointers to asynchronous requests and so on.
Currently allocated as a static array of objects, then as a request
comes in, a mmc_queue_req is assigned to it, and used during the
lifetime of the request.
This is backwards compared to how other block layer drivers work:
they usally let the block core provide a per-request struct that get
allocated right beind the struct request, and which can be obtained
using the blk_mq_rq_to_pdu() helper. (The _mq_ infix in this function
name is misleading: it is used by both the old and the MQ block
layer.)
The per-request struct gets allocated to the size stored in the queue
variable .cmd_size initialized using the .init_rq_fn() and
cleaned up using .exit_rq_fn().
The block layer code makes the MMC core rely on this mechanism to
allocate the per-request mmc_queue_req state container.
Doing this make a lot of complicated queue handling go away. We only
need to keep the .qnct that keeps count of how many request are
currently being processed by the MMC layer. The MQ block layer will
replace also this once we transition to it.
Doing this refactoring is necessary to move the ioctl() operations
into custom block layer requests tagged with REQ_OP_DRV_[IN|OUT]
instead of the custom code using the BigMMCHostLock that we have
today: those require that per-request data be obtainable easily from
a request after creating a custom request with e.g.:

And this is not possible with the current construction, as the request
is not immediately assigned the per-request state container, but

This option is activated by all multiplatform configs and what
not so we almost always have it turned on, and the memory it
saves is negligible, even more so moving forward. The actual
bounce buffer only gets allocated only when used, the only
thing the ifdefs are saving is a little bit of code.
It is highly improper to have this as a Kconfig option that
get turned on by Kconfig, make this a pure runtime-thing and
let the host decide whether we use bounce buffers. We add a
new property "disable_bounce" to the host struct.
Notice that mmc_queue_calc_bouncesz() already disables the
bounce buffers if host->max_segs != 1, so any arch that has a
maximum number of segments higher than 1 will have bounce
buffers disabled.
The option CONFIG_MMC_BLOCK_BOUNCE is default y so the
majority of platforms in the kernel already have it on, and
it then gets turned off at runtime since most of these have
a host->max_segs > 1. The few exceptions that have
host->max_segs == 1 and still turn off the bounce buffering
are those that disable it in their defconfig.

For hosts not supporting MMC_CAP2_SDIO_IRQ_NOTHREAD but MMC_CAP_SDIO_IRQ,
the SDIO IRQs are processed from a dedicated kernel thread. For these
cases, the host calls mmc_signal_sdio_irq() from its ISR to signal a new
SDIO IRQ.
Signaling an SDIO IRQ makes the host's ->enable_sdio_irq() callback to be
invoked to temporary disable the IRQs, before the kernel thread is woken up
to process it. When processing of the IRQs are completed, they are
re-enabled by the kernel thread, again via invoking the host's
->enable_sdio_irq().
The observation from this, is that the execution path is being unnecessary
complex, as the host driver already knows that it needs to temporary
disable the IRQs before signaling a new one. Moreover, replacing the kernel
thread with a work/workqueue would not only greatly simplify the code, but
also make it more robust.
To address the above problems, let's continue to build upon the support for
MMC_CAP2_SDIO_IRQ_NOTHREAD, as it already implements SDIO IRQs to be
processed without using the clumsy kernel thread and without the ping-pong
calls of the host's ->enable_sdio_irq() callback for each processed IRQ.
Therefore, let's add new API sdio_signal_irq(), which enables hosts to
signal/process SDIO IRQs by using a work/workqueue, rather than using the
kernel thread.
Add also a new host callback ->ack_sdio_irq(), which the work invokes when
the SDIO IRQs have been processed. This informs the host about when it
shall re-enable the SDIO IRQs. Potentially, we could re-use the existing
->enable_sdio_irq() callback instead of adding a new one, however it has
turned out that it's more convenient for hosts to get this information via
a separate callback.
Hosts that wants to use this new method to signal/process SDIO IRQs, must
enable MMC_CAP2_SDIO_IRQ_NOTHREAD and implement the ->ack_sdio_irq()
callback.

eMMC can have multiple internal partitions that are represented as separate
disks / queues. However switching between partitions is only done when the
queue is empty. Consequently the array of mmc requests that are queued can
be shared between partitions saving memory.
Keep a pointer to the mmc request queue on the card, and use that instead
of allocating a new one for each partition.

Certain 64-bit systems (e.g. Amlogic Meson GX) require buffers to be
used for DMA to be 8-byte-aligned. struct sdio_func has an embedded
small DMA buffer not meeting this requirement.
When testing switching to descriptor chain mode in meson-gx driver
SDIO is broken therefore. Fix this by allocating the small DMA buffer
separately as kmalloc ensures that the returned memory area is
properly aligned for every basic data type.

With the coexisting __mmc_start_request(), mmc_start_request()
and __mmc_start_req() it is a bit confusing that mmc_start_req()
actually does not start a normal request, but an asynchronous
request.
Rename it to mmc_start_areq() to make it explicit what the
function is doing, also fix the kerneldoc for this function
while we're at it.

According the JEDEC specification an eMMC card supporting 1.8V vccq in DDR
mode should also be capable of 3.3V. However, it's been reported that some
mmc hosts supports 3.3V, but not 1.8V.
Currently the mmc core implements an error handling when the host fails to
set 1.8V for vccq, by falling back to 3.3V. Unfortunate, this seems to be
insufficient for some mmc hosts. To enable these to use eMMC DDR mode let's
invent a new mmc cap, MMC_CAP_3_3V_DDR, which tells whether they support
the eMMC 3.3V DDR mode.
In case MMC_CAP_3_3V_DDR is set, but not MMC_CAP_1_8V_DDR, let's change to
remain on the 3.3V, as it's the default voltage level for vccq, set by the
earlier power up sequence.
As this change introduces MMC_CAP_3_3V_DDR, let's take the opportunity to
do some re-formatting of the related defines in the header file.

Ideally the public mmc header file, core.h, shouldn't contain interfaces
particularly intended to be used by host drivers. Instead those should
remain in the host.h header file. Therefore, let's move a couple functions
from core.h to host.h.

This is the first step in cleaning up the public mmc header files. In this
change we makes sure each header file builds standalone, as that helps to
resolve dependencies.
While changing this, it also seems reasonable to stop including other
headers from inside a header itself which it don't depend upon.
Additionally, in some cases such dependencies are better resolved by
forward declaring the needed struct.

There are currently no external users of the public dw_mmc header file,
except the dw_mmc driver itself. Therefore let's move the definitions from
the public dw_mmc header file into the existing private dw_mmc header file
and then remove the public one.

There are currently no users of the clk_ctrl2_present member from the
platform data, so let's remove it.
Note, as some of the sh_mmcif variants may support clk_ctrl2, let's keep
the current code in the driver, which deals with this. For future support,
we should invent a DT binding instead, but let's leave that until it's
needed.

Removing the callback also enables us to remove the sh_mmcif_get_cd()
altogether, as we convert to use mmc_gpio_get_cd() to the same kind of
work.
