Seokmann Ju b9b12f73df [SCSI] qla2xxx: Correct several PCI-EEH issues.
In addition to checking for potentially unnecessary iomem
readX()/writeX() operations, a pci_channel_io_perm_failure should
not trigger a full internal removal.  Found during additional
testing with pSeries blade systems.

Andrew Vasquez 24a081386e [SCSI] qla2xxx: Explicitly set the execution-throttle with recent ISPs.
Firmware semantics changed for 24xx and above ISPs in their
handling of the specified execution-throttle passed during
firmware initialization.  The original codes use of a theoretical
maximum (0xffff, as carried over from earlier ISPs) could in fact
act as a throttle in some circumstances.  Now set the value based
of the firmware's own 'resource' (exchange IOCBs) capabilities.

Andrew Vasquez d743de6675 [SCSI] qla2xxx: Reduce request queue-size overhead with recent ISPs.
The original code to 'resize request-queues' based on iocb-counts
and employed during early ISP23xx testing was too
overly-pessimistic with regards to latencies in the firmware
pulling requests.  Recent ISPs can easily keep up processing a
stream of commands from an abbreviated (effectively, half the
original size) queue.

Giridhar Malavali d1b1bef4c9 [SCSI] qla2xxx: Drop use of IRQF_DISABLE.
When IRQs are shared by multiple controllers and if the first one
to register does not disable the IRQ, then IRQ will be enabled
for all other controllers by default, irrespective of their
setting. With IRQF_DISABLED registration, the driver interrupt
routine was called with interrupt enabled always. Disbaling the
registration with IRQF_DISABLED, since driver code is re-entrant
safe and all critical sections are guarded with interrupt safe
locks.

Lalit Chandivade d0c3eefae0 [SCSI] qla2xxx: Remove implicit ISP-abort after a flash update.
For ISP24xx and above the ISP-abort after flash update is not
needed, as the only purpose it was serving was to update the boot
code and firmware versions in the scsi_qla_host_t structure.  Now
an update of the versions will be done in the write-vpd path.

Lalit Chandivade 2533cf671d [SCSI] qla2xxx: Correct ISP abort semantics for NVRAM, VPD, and flash update.
Ensure that an ISP-abort has completed before performing any
update.  After the update do not wait for an ISP-abort completion,
instead just wait until the ISP is reset.  This avoids long
delays due to waiting for loop ready in qla2x00_abort_isp().

Joe Carnuccio 1d2874de80 [SCSI] qla2xxx: Add Flash-Access-Control support for recent ISPs.
Given the low-level interface varies from one flash-part
manufacturer to the next, the Flash-Access-Control (FAC) mailbox
command makes the specific flash type transparent to the driver
by encapsulating a basic set of accessor and update routines.
Use these new routines where applicable by querying FAC opcode
get-sector-size at init-time.

Andrew Vasquez b997876987 [SCSI] qla2xxx: Ensure the timer and DPC routines complete prior to midlayer tear-down.
Since the routines can/will use resources such as devices and
rports that aren't valid after midlayer tear-down, correct this
potential race, by stopping the offending during the early stages
of the remove() callback.

Andrew Vasquez be67e6530b [SCSI] qla2xxx: Preserve an fcport's loop-id after terminate_rport_io().
Since in some circumstances, login-retries may be occuring in the
background via the DPC routine.  This race, in the inadvertant
setting of the loop-id to 'NONE' breaks the existing retry logic.

Andrew Vasquez dda772e8e3 [SCSI] qla2xxx: Always (re)read firmware version/capabilities information.
With recent ISPs loading firmware from flash, a flash-update to
the firmware-image region with a follow-on reset will reload the
new image.

Original caching of data only made sense when firmware was bound
with the driver.

Original code used an overabundance of indirect pointers to
function helpers.  Instead, the driver can exploit the immutable
properties of a queue's ISP-association and ID, which are both
known at queue initialization-time.

Andrew Vasquez c6b2fca820 [SCSI] qla2xxx: Correct truncation in return-code status checking.
QLA_* return codes are 'int' in size.  There were still several
legacy check-points which assumed a return-code width of 8-bits.
This could cause incorrect assumptions of 'good' status if a
return of QLA_FUNCTION_TIMEOUT.

Andrew Vasquez 8a659571ec [SCSI] qla2xxx: Properly acknowledge IDC notification messages.
To ensure smooth operations amongst the FCoE and NIC side
components of the ISP81xx chip, the FCoE driver (qla2xxx) must
ensure the 10gb NIC driver (qlge) does not timeout waiting for
IDC (Inter-Driver Communication) acknowledgments.  The
acknowledgment requirements are trivial -- a simple mirroring of
incoming mailbox registers during the AEN to a process-context
capable mailbox command.

Anirban Chakraborty 618a752319 [SCSI] qla2xxx: Remove interrupt request bit check in the response processing path in multiq mode.
Correct response-queue-0 processing by instructing the firmware
to run with interrupt-handshaking disabled, similarly to what is
now done for all non-0 response queues.  Since all
response-queues now run in the same mode, the driver no longer
needs the hot-path 'is-disabled-HCCR' test.

Seokmann Ju 3c01b4f9fb [SCSI] qla2xxx: Add checks for a valid fcport in dev-loss-tmo/terminate_rport_io callbacks.
Commit f78badb1ae ([SCSI] fc
transport: pre-emptively terminate i/o upon dev_loss_tmo timeout)
changed the callback semantics of dev_loss_tmo and
terminate_rport_io such that repeated calls could be made.  This
could result in the the driver using stale (NULLed-out, in
dev_loss_tmo) data from the rport.  Correct this by addint a
simple check to ensure a valid fcport is attached.

was not setting the proper consistent and streaming DMA masks
prior to memory allocations.  Correct this and remove the
unnecessary prototype.

Joe Carnuccio b872ca4081 [SCSI] qla2xxx: Correct descriptions in flash manipulation routines.
When clearing the flash device's SR, the comment is incorrect...
clearing the SR is 2 steps:

1. the SR protect bit is 1, so the first write zero clears only
   that bit,

2. the SR protect bit is now 0, so the next write zero clears the
   remaining bits.

The sector erase debug print more correctly identifies that the erase failed.

Andrew Vasquez eaac30be26 [SCSI] qla2xxx: Modify firmware-load order precedence for ISP81XX parts.
Pre-ISP81XX parts (including ISP24xx and ISP25xx) could contain a
firmware image within a segment of flash, driver would fallback
to loading this firmware if the request-firmware interface failed
(userspace .bin file).  Moving forward, all ISP81XX parts will
ship with a suggested-to-be-used firmware image within flash
which all driver should first attempt to load.  If the flash
firmware load fails, the driver will then fallback to loading
firmware via the request-firmware interface (ql8100_fw.bin).

Andrew Vasquez 8eca3f39c4 [SCSI] qla2xxx: Always serialize mailbox command execution.
Original code would incorrectly bypass serialization if the DPC
thread were performing a big-hammer operation (ISP abort).  This
short circuit, though rare, would subsequently stomp on a
secondary thread's mailbox command execution.  Found during
ISP81XX testing.

Anirban Chakraborty 73208dfd7a [SCSI] qla2xxx: add support for multi-queue adapter
Following changes have been made.
1. qla_hw_data structure holds an array for request queue pointers,
and an array for response queue pointers.
2. The base request and response queues are created by default.
3. Additional request and response queues are created at the time of vport
creation. If queue resources are exhausted during vport creation, newly
created vports use the default queue.
4. Requests are sent to the request queue that the vport was assigned
in the beginning.
5. Responses are completed on the response queue with which the request queue
is associated with.

Following changes have been made to the qla2xxx FC driver in
preparation for the multi- queue and future SR IOV hardware.

1. scsi_qla_host structure has been changed to contain scsi host
   specific data only.

2. A new structure, qla_hw_data is created to contain HBA specific
   hardware data.

3. Request and response IO specific data strucures are created.

4. The global list of fcports for the hba is not maintained anymore,
   instead a fcport list is construted on per scsi_qla_host.

Michael Reed 5bff55db3d [SCSI] qla2xxx: Return a FAILED status when abort mailbox-command fails.
Mike Reed noted
(https://bugzilla.novell.com/show_bug.cgi?id=421330) that the
driver was incorrectly returning a SUCCESS status if the driver's
request to the firmware to abort a command failed.  By doing so,
the mid-layer believed, incorrectly, that the command has
completed and has been returned (ultimately clearing
scsi_cmnd.request_buffer) yet the driver still has the command.
What should correctly happen is a mid-layer escalation
(device-reset, etc.) of recovery during which the driver will
eventually return the outstanding commands to the mid-layer.

Mike Christie 056a448349 [SCSI] qla2xxx: use new host byte transport errors.
This has qla2xxx use the new transport error values instead of
DID_BUS_BUSY. I am not sure if all the errors
in qla_isr.c I changed are transport related. We end up blocking/deleting
the rport for all of them so it is better to use the new transport error since
the fc classs will decide when to fail the IO.

With this patch if I pull a cable then IO that had reached
the driver, will be failed with DID_TRANSPORT_DISRUPTED (not including
tape). The fc class will then fail the IO when the fast io fail tmo
has fired, and the driver will flush any other commands running.

Mike Christie 7b594131c4 [SCSI] qla2xxx: return SCSI_MLQUEUE_TARGET_BUSY when driver has detected rport error or race
If the fcport is not online then we do not want to block IO to all ports on
the host. We just want to stop IO on port not online, so we should be using
the SCSI_MLQUEUE_TARGET_BUSY return value.

For the case where we race with the rport memset initialization
we do not want the queuecommand to be called again so we can just use
SCSI_MLQUEUE_TARGET_BUSY for this.

Andrew Vasquez c795c1e4b6 [SCSI] qla2xxx: Explicitly tear-down vports during PCI remove_one().
During internal testing, we've seen issues (hangs) with the
'deferred' vport tear-down-processing typically accompanied with
the fc_remove_host() call.  This is due to the current
implementation's back-end vport handling being performed by the
physical-HA's DPC thread where premature shutdown could lead to
latent vport requests without a processor.

This should also address a problem reported by Gal Rosen
(http://marc.info/?l=linux-scsi&m=121731664417358&w=2) where the
driver would attempt to awaken a previously torn-down DPC thread
from interrupt context by implicitly calling wake_up_process()
rather than the driver's qla2xxx_wake_dpc() helper.  Rather, than
reshuffle the remove_one() device-removal code, during unload,
depend on the driver's timer to wake-up the DPC process, by
limiting wake-ups based on an 'unloading' flag.

* Use correct 'ha' to mark a device lost from ISR.
  I/Os will always be returned on the physical-HA.
  qla2x00_mark_device_lost() should be called with the HA bound
  to the fcport.
* Mark *all* devices lost during ISP-ABORT (bighammer).

These fixes correct issues discovered locally where during
link-perturbation and heavy vport-I/O fcport/rport states would
stray and an rport's scsi-target lost (timed-out).

Andrew Vasquez 6390d1f33f [SCSI] qla2xxx: Correct synchronization of software/firmware fcport states.
Greg Wettstein (greg@enjellic.com) noted:

on a reboot of a previously recognized SCST target, the initiator
driver would be unable to re-recognize the device as a target.
It turns out that prior to the SCST software reloading and
returning it's "target-capable" abilities in the PRLI payload,
the HBA would be re-initialized as an initiator-only type port.
Since initiators typically classify themselves as an FCP-2
capable device, both software and firmware do not perform an
explicit logout during port-loss.  Unfortunately, as can be seen
by the failure case, when the port (now target-capable) returns,
firmware performs an ADISC without a follow-on PRLI, leaving
stale 'initiator-only' data in the firmware's port database.

Correct the discrepancy by performing the explicit logout during
the transport's request to terminate-rport-io, thus synchronizing
port states and ensuring a follow-on PRLI is performed.

Andrew Vasquez 8201e20799 [SCSI] qla2xxx: Issue proper ISP callbacks during stop-firmware.
As the original code would incorrectly call the non-ISP24xx/25xx
callbacks during recovery, a stop-firmware failure could result
in improper bit-banging of the RISC and in some cases manifest in
a NMI-watchdog trigger due to the RISC not coming out of its
reset state.

Seokmann Ju 5de1f70f41 [SCSI] qla2xxx: Correct rport/fcport visibility-state handling during loop-resync.
There were several issues here, one, during RSCN handling if a
follow-on RSCN occurred (within interrupt context) the DPC thread
could inadvertantly leave the fcport in a stale lost state.
Secondly, scheduled rport removal is handled exclusively by the
'parent' DPC thread, so wake up the proper thread.  Finally,
process vport loop-resync's only when the vport has in an
"active" state (ID acquired).

The firmware group has suggested that FCE (Fibre Channel Event)
tracing be enabled prior to EFT (Extended Firmware Tracing) to
maximize the capturing of data on the wire.  This change has no
real semantic effect on driver operation, as it's mostly a
shuffling of code.

Andrew Vasquez e5896bd5dc [SCSI] qla2xxx: Allow the user the option of disabling iIDMA.
iIDMA support requires the driver issue several additional
fabric-managegment (FM) commands per port discovered during SNS
scanning -- GFPN (Get Fabric Port Name) and GPSC (Get Port Speed
Capabilities).  It has been found during testing that some
switches do not respond as *well* as expected to these commands
(silence -- no ACC nor BS_RJT).  So, to handle such conditions,
allow the user the ability to indirectly disable the FM commands
by disabling iIDMA with the ql2xiidmaenable module-parameter.

with irqs-enabled in the former case.

During init-time and error-recovery (after a RISC reset), the
driver disables interrupts and 'polls' for completions by calling

regression where a successful relogin would result in an fcport's

The driver is incorrectly assuming that the 'sp' reference held
in qla2[x00|4xx]_abort_command() is valid after the mailbox
command is issued to abort the exchange.  It is *not*, as the

is returned to the mailbox caller.

Use the more efficient read-DMA'ble-buffer mailbox commands

Seokmann Ju 221726d41a [SCSI] qla2xxx: Correct vport configuration-change handling.
Upon having configuration changes on vports only, the driver
handles SCR regardless physical port state and, in turn, it
results mailbox error as below:

With the changes, driver checks physical port loop_state and make
sure the port is ready to take commands.

Andrew Vasquez 523ec773b8 [SCSI] qla2xxx: Add midlayer target/device reset support.
Now that infrastructure is present within the midlayer and there
is a clear distinction between what is expected from a device and
target reset, convert the current device-reset codes to a
target-reset, and add codes to perform a proper device-reset (LUN
reset).

In the process of adding reset support, collapse and consolidate
large sections of mailbox-command (TMF issuance) codes,
generalize the two 'wait-for-commands-to-complete' functions, and
add a generic-reset routine for use by midlayer reset functions.

Andrew Vasquez 7d232c745e [SCSI] qla2xxx: Add Flash Descriptor Table layout support.
The Flash Descriptor Table (FDT) present on many recent HBAs
encodes flash accessing characteristics of the flash-part used on
the HBA.  Use this information during flash manipulation (writes)
rather than using specific hard-coded values based on queried
manufacturer and device IDs.

Supported events include LIP, LIP reset, RSCN, link up, and link
down.

To support AEN (and additional forthcoming features), we also
introduce a simple deferred-work construct to manage events which
require a non-atomic sleeping-capable context.  This work-list is
processed as part of the driver's standard DPC routine.

*_irq[save|restore]() locking primitives within the driver's
interrupt handlers, interrupts are guaranteed to be

Andrew Vasquez 00a537b820 [SCSI] qla2xxx: Correct usage of inconsistent timeout values while issuing ELS commands.
The original code would incorrectly hardcode ELS timeout values
rather than using the traditional '2 * r_a_tov' value.  In some
cases, the hardcoded values would be larger than the
mailbox-command-timeout and result in a needless BIG_HAMMER (ISP
reset), the typical recovery mechanism employed in such cases.

The second defect in the original code was in the assignment of
the default 'ha->r_a_tov' to twice the traditional value.
Correct this by setting the value to 10 seconds.

Andrew Vasquez 3e8ce320cf [SCSI] qla2xxx: Correct discrepancies during OVERRUN handling on FWI2-capable cards.
For recent ISPs, software must detect OVERRUN conditions by
checking the SS_RESIDUAL_OVER bit during CS_COMPLETE handling.
Update the driver to perform this check, which is consistent with
what earlier firmwares did by explicitly cracking open the
FCP_RSP statuses and returning an CS_DATA_OVERRUN.

Andrew Vasquez 7c7f1f299b [SCSI] qla2xxx: Correct needless clean-up resets during shutdown.
There's no point in hitting the RISC with what will most
assuredly be an unsucessful reset of the RISC hardware if the
initial stop-firmware mailbox command fails with a time-out
status.  Instead, to avoid what could amount to a lengthy
stop-firmware/detect-failure/reset-risc loop, continue with
driver unloading and discard the stop-firmware requirement.

BIT_2 of the firmware attributes is only valid on FW-interface-2
type HBAs.  Code in commit
c48339decc would cause the
incorrect initialize-firmware mailbox command to be issued for
non-NPIV capable ISPs.  Correct this by reverting to previously
used (and correct) pre-condition 'if' check.

Seokmann Ju 99363ef81c [SCSI] qla2xxx: Correct issue where vport-state was not updated during an ISP_ABORT_NEEDED requst.
While running IO simultaneously through physical port and virtual
port, if user changes Data Rate (from scli utility), IO through
virtual port fails.  It failed because the vport had not received

- Properly unwind deallcations of memory during failures.

  the callers.
- Defer DMA pool allocation of SFP data until requested.

Andrew Vasquez 948882f6b7 [SCSI] qla2xxx: Correct resource_size_t usages.
Hmm, it looks like the conversion to resource_size_t usage
(3776541d8a) requires some additional
fixups to cleanup the structure-pointer castings used during IO mapped
accesses to the chip.

There's only a small number of locations, where the driver uses IO
mapped accesses to the hardware, the patch below should take care of
it without introducing to many structural changes to code flow.

Benjamin Herrenschmidt 0948391641 PCI: Remove users of pci_enable_device_bars()
This patch converts users of pci_enable_device_bars() to the new
pci_enable_device_{io,mem} interface.

The new API fits nicely, except maybe for the QLA case where a bit of
code re-organization might be a good idea but I prefer sticking to the
simple patch as I don't have hardware to test on.

I'll also need some feedback on the cs5520 change.

James Bottomley d3f46f39b7 [SCSI] remove use_sg_chaining
With the sg table code, every SCSI driver is now either chain capable
or broken (or has sg_tablesize set so chaining is never activated), so
there's no need to have a check in the host template.

Also tidy up the code by moving the scatterlist size defines into the
SCSI includes and permit the last entry of the scatterlist pools not
to be a power of two.

Seokmann Ju c48339decc [SCSI] qla2xxx: Issue correct MBC_INITIALIZE_FIRMWARE command.
There is a case where 54xx HBA loads MID firmware as it use 24xx
firmware. In this case, the driver should issue
MBC_MID_INITIALIZE FIRMWARE even though the HBA doesn't support
NPIV.  This patch make changes in the driver so that could behave
accordingly.

Andrew Vasquez df613b9607 [SCSI] qla2xxx: Add Fibre Channel Event (FCE) tracing support.
FCE support enables the firmware to record FC extended link
services and basic link services frames which have been
transmitted and received by the ISP.  This allows for a limited
view of the FC traffic through the ISP without using a FC
analyzer.  This can be useful in situations where a physical
connection to the FC bus is not possible.

The driver exports this information in two ways -- first, via a
debugfs node exported for all supported ISPs under:

	<debugfs_mount_point>/qla2xxx/qla2xxx_<host_no>/fce

where a read of the 'fce' file will provide a snapshot of the
firmware's FCE buffer; and finally, the FCE buffer will be
extracted during a firmware-dump scenario.

Andrew Vasquez a4722cf24d [SCSI] qla2xxx: Don't schedule the DPC routine to perform an issue-lip request.
As the driver depends on the DPC routine to handle bottom-half
loop resynchronization in order to recover from the issue-lip
request.  The issue_lip call is sleeping context capable, so just
issue the reset function there.

Andrew Vasquez 6f13fd57cd [SCSI] qla2xxx: Wait for FLASH write-protection to complete after a write.
Some flash parts have a slow enable write-protection (WP)
operation whereby subsequent FLASH accesses would fail if the WP
operation had not completed.  Software now polls the SPI's
status-register for WP completion.

Andrew Vasquez 3776541d8a [SCSI] qla2xxx: Fix for 32-bit platforms with 64-bit resources.
The driver stores the contents of PCI resources into unsigned
long's before ioremapping. This breaks on 32-bit platforms which
support 64-bit MMIO resources.

Correct code by removing the temporary variables used during MMIO
PIO mapping and using resource_size_t where applicable.  Also
correct a small typo in a printk() where the wrong region number
was displayed.

Shyam Sundar f4a8dbc7f6 [SCSI] qla2xxx: Properly handle Vport state-change-notifications.
Drivers do SCRs for each Vport. When something changes in the
fabric, firmware generates one interrupt for each RSCN. Based on
the current implementation, in each case, we make recursive calls
to handle RSCN for physical and each subsequent virtual ports.
The fix is to also take into consideration the vp_idx, which is
set by the firmware to indicate the vport the RSCN was meant for.

Andrew Vasquez 06e23b7470 [SCSI] qla2xxx: Don't explicitly read mbx registers while processing a system-error.
Callers of qla2x00_async_event() already populate the mb[] array
upon invocation, doing so via the appropriate mailbox register
accessors.  The stale codes removed are leftover-bits kept during
the FWI2 transition.  Though relatively benign, the extra-reads
are not valid for FWI2 boards (ISP24xx and above) and peek into
the incorrect regions of registers.

Andrew Vasquez 6acf819002 [SCSI] qla2xxx: Correct residual-count handling discrepancies during UNDERRUN handling.
For recent ISPs, software during CS_UNDERRUN handling must
determine if the two residuals, firmware-calculated and FCP_RSP,
are different to recognize if a frame has been dropped.  Update
the driver to catch this condition, and clear the
SS_RESIDUAL_UNDER and lscsi_status bits.  This logic is
consistent with what earlier firmwares did by explicitly
cracking open the FCP_RSP statuses and clearing
SS_RESIDUAL_UNDER.

Andrew Vasquez 285d0321d1 [SCSI] qla2xxx: Make driver (mostly) legacy I/O port free.
Recent ISPs need only the single MMIO BAR to manipulate HW
registers.  Unfortunately, ISP21xx, ISP22xx, ISP23xx, and ISP63xx
type cards still require the I/O mapped region to manipulate the
FLASH via the two HW flash-registers (flash_address and
flash_data).

Andrew Vasquez 87f27015b8 [SCSI] qla2xxx: Rework MSI-X handlers.
Since MSI-X vectors do not require a clearing "handshake" from
the system perspective, and the registered handler will not be
called more than once for one occurrence of receipt of a vector,
there is no requirement to flush the risc register write clearing
the interrupt condition in the risc. Also, since the msi-x
registered handlers are optimised for a particular vector, it is
preferable to handle the one vector received per invocation of
the handler.

Paul Mundt 20c2df83d2 mm: Remove slab destructors from kmem_cache_create().
Slab destructors were no longer supported after Christoph's
c59def9f22 change. They've been
BUGs for both slab and slub, and slob never supported them
either.

This rips out support for the dtor pointer from kmem_cache_create()
completely and fixes up every single callsite in the kernel (there were
about 224, not including the slab allocator definitions themselves,
or the documentation references).

Zhang Rui 91a6902958 sysfs: add parameter "struct bin_attribute *" in .read/.write methods for sysfs binary attributes
Well, first of all, I don't want to change so many files either.

What I do:
Adding a new parameter "struct bin_attribute *" in the
.read/.write methods for the sysfs binary attributes.

In fact, only the four lines change in fs/sysfs/bin.c and
include/linux/sysfs.h do the real work.
But I have to update all the files that use binary attributes
to make them compatible with the new .read and .write methods.
I'm not sure if I missed any. :(

Why I do this:
For a sysfs attribute, we can get a pointer pointing to the
struct attribute in the .show/.store method,
while we can't do this for the binary attributes.
I don't know why this is different, but this does make it not
so handy to use the binary attributes as the regular ones.
So I think this patch is reasonable. :)

Who benefits from it:
The patch that exposes ACPI tables in sysfs
requires such an improvement.
All the table binary attributes share the same .read method.
Parameter "struct bin_attribute *" is used to get
the table signature and instance number which are used to
distinguish different ACPI table binary attributes.

Without this parameter, we need to offer different .read methods
for different ACPI table binary attributes.
This is impossible as there are various ACPI tables on different
platforms, and we don't know what they are until they are loaded.

Tejun Heo 7b595756ec sysfs: kill unnecessary attribute->owner
sysfs is now completely out of driver/module lifetime game.  After
deletion, a sysfs node doesn't access anything outside sysfs proper,
so there's no reason to hold onto the attribute owners.  Note that
often the wrong modules were accounted for as owners leading to
accessing removed modules.

This patch kills now unnecessary attribute->owner.  Note that with
this change, userland holding a sysfs node does not prevent the
backing module from being unloaded.

Auke Kok 44c10138fd PCI: Change all drivers to use pci_device->revision
Instead of all drivers reading pci config space to get the revision
ID, they can now use the pci_device->revision member.

This exposes some issues where drivers where reading a word or a dword
for the revision number, and adding useless error-handling around the
read. Some drivers even just read it for no purpose of all.

In devices where the revision ID is being copied over and used in what
appears to be the equivalent of hotpath, I have left the copy code
and the cached copy as not to influence the driver's performance.

Compile tested with make all{yes,mod}config on x86_64 and i386.

David Miller 4e08df3f91 [SCSI] qla2xxx: fix regression on sparc64
Some sparc64 boxes don't have a valid NVRAM (from which the driver
takes its WWPN) try to extract this from open firmware instead and if
that fails, fall back to a default, which would be invalid if more
than one machine on the same SAN does this, since two machines with
the same WWPN would be illegal, so warn when taking this potentially
invalid default.

Tested on SunBlade-1000:

Bernhard Walle 665db93b8a [SCSI] qla2xxx: Remove duplicate pci_disable_device() call
On the path qla2x00_probe_one() -> probe_failed -> qla2x00_free_device(),
pci_disable_device() is executed twice, once in qla2x00_free_device()
and once in qla2x00_probe_one().

This patch removes the unnecessary call.

Andrew Vasquez 27d9403528 [SCSI] qla2xxx: Allow the extended-error-logging flag to be dynamic.
The module parameter, ql2xextended_error_logging, can now be
set dynamically by writing to the following sysfs entry:

	/sys/module/qla2xxx/parameters/ql2xextended_error_logging

This alleviates the need for the driver to be unloaded and
reloaded in order to enable logging.

Andrew Vasquez fecf97882a [SCSI] qla2xxx: Drop acquisition of hardware_lock during flash manipulations.
There's no need given, I/O has been quiesced, RISC
interrupts have been disabled, and finally the RISC has been
paused.  Flash manipulation on ISP21xx, ISP22xx, and ISP23xx
parts requires the RISC to go through a full reset to
recover.

Andrew Vasquez d88021a671 [SCSI] qla2xxx: Handle IRQ-0 assignments by the system.
No restriction should be placed on the IRQ number assigned
to a given ISP.  Original code incorrectly assumed a
non-zero IRQ number assignment by the system.  In these
circumstances the proper freeing of the IRQ (via free_irq())
would not take place.

Seokmann Ju befede3dab [SCSI] qla2xxx: correct locking while call starget_for_each_device()
Removed spin_unlock_irq()/spin_lock_irq() pairs surrounding
starget_for_each_device() calls.
As Matthew W. pointed out, starget_for_each_device() can be called under
a spinlock being held.
The change has been tested and verified on qla2xxx.ko module.
