Yang Yang e9ea874a8f mm/vmstat: add events for THP max_ptes_* exceeds
There are interfaces to adjust max_ptes_none, max_ptes_swap,
max_ptes_shared values, see
  /sys/kernel/mm/transparent_hugepage/khugepaged/.

But system administrator may not know which value is the best.  So Add
those events to support adjusting max_ptes_* to suitable values.

For example, if default max_ptes_swap value causes too much failures,
and system uses zram whose IO is fast, administrator could increase
max_ptes_swap until THP_SCAN_EXCEED_SWAP_PTE not increase anymore.

Pasha Tatashin 08d5b29eac mm: ptep_clear() page table helper
We have ptep_get_and_clear() and ptep_get_and_clear_full() helpers to
clear PTE from user page tables, but there is no variant for simple
clear of a present PTE from user page tables without using a low level
pte_clear() which can be either native or para-virtualised.

Add a new ptep_clear() that can be used in common code to clear PTEs
from page table.  We will need this call later in order to add a hook
for page table check.

Matthew Wilcox (Oracle) 6b24ca4a1a mm: Use multi-index entries in the page cache
We currently store large folios as 2^N consecutive entries.  While this
consumes rather more memory than necessary, it also turns out to be buggy.
A writeback operation which starts within a tail page of a dirty folio will
not write back the folio as the xarray's dirty bit is only set on the
head index.  With multi-index entries, the dirty bit will be found no
matter where in the folio the operation starts.

This does end up simplifying the page cache slightly, although not as
much as I had hoped.

Liangcai Fan bd3400ea17 mm: khugepaged: recalculate min_free_kbytes after stopping khugepaged
When initializing transparent huge pages, min_free_kbytes would be
calculated according to what khugepaged expected.

So when transparent huge pages get disabled, min_free_kbytes should be
recalculated instead of the higher value set by khugepaged.

Pull memory folios from Matthew Wilcox:

  The point of all this churn is to allow filesystems and the page cache
  to manage memory in larger chunks than PAGE_SIZE. The original plan
  was to use compound pages like THP does, but I ran into problems with
  some functions expecting only a head page while others expect the
  precise page containing a particular byte.

  The folio type allows a function to declare that it's expecting only a
  head page. Almost incidentally, this allows us to remove various calls
  to VM_BUG_ON(PageTail(page)) and compound_head().

  This converts just parts of the core MM and the page cache. For 5.17,
  we intend to convert various filesystems (XFS and AFS are ready; other
  filesystems may make it) and also convert more of the MM and page
  cache to folios. For 5.18, multi-page folios should be ready.

  The multi-page folios offer some improvement to some workloads. The
  80% win is real, but appears to be an artificial benchmark (postgres
  startup, which isn't a serious workload). Real workloads (eg building
  the kernel, running postgres in a steady state, etc) seem to benefit
  between 0-10%. I haven't heard of any performance losses as a result
  of this series. Nobody has done any serious performance tuning; I
  imagine that tweaking the readahead algorithm could provide some more
  interesting wins. There are also other places where we could choose to
  create large folios and currently do not, such as writes that are
  larger than PAGE_SIZE.

Yang Shi a4aeaa06d4 mm: khugepaged: skip huge page collapse for special files
The read-only THP for filesystems will collapse THP for files opened
readonly and mapped with VM_EXEC.  The intended usecase is to avoid TLB
misses for large text segments.  But it doesn't restrict the file types
so a THP could be collapsed for a non-regular file, for example, block
device, if it is opened readonly and mapped with EXEC permission.  This
may cause bugs, like [1] and [2].

This is definitely not the intended usecase, so just collapse THP for
regular files in order to close the attack surface.

Currently collapse_file does not explicitly check PG_writeback, instead,
page_has_private and try_to_release_page are used to filter writeback
pages.  This does not work for xfs with blocksize equal to or larger
than pagesize, because in such case xfs has no page->private.

This makes collapse_file bail out early for writeback page.  Otherwise,
xfs end_page_writeback will panic as follows.

Hugh Dickins acdd9f8e0f huge tmpfs: SGP_NOALLOC to stop collapse_file() on race
khugepaged's collapse_file() currently uses SGP_NOHUGE to tell
shmem_getpage() not to try allocating a huge page, in the very unlikely
event that a racing hole-punch removes the swapped or fallocated page as
soon as i_pages lock is dropped.

We want to consolidate shmem's huge decisions, removing SGP_HUGE and
SGP_NOHUGE; but cannot quite persuade ourselves that it's okay to regress
the protection in this case - Yang Shi points out that the huge page would
remain indefinitely, charged to root instead of the intended memcg.

collapse_file() should not even allocate a small page in this case: why
proceed if someone is punching a hole?  SGP_READ is almost the right flag
here, except that it optimizes away from a fallocated page, with NULL to
tell caller to fill with zeroes (like a hole); whereas collapse_file()'s
sequence relies on using a cache page.  Add SGP_NOALLOC just for this.

There are too many consecutive "if (page"s there in shmem_getpage_gfp():
group it better; and fix the outdated "bring it back from swap" comment.

Collin Fijalkovich eb6ecbed0a mm, thp: relax the VM_DENYWRITE constraint on file-backed THPs
Transparent huge pages are supported for read-only non-shmem files, but
are only used for vmas with VM_DENYWRITE.  This condition ensures that
file THPs are protected from writes while an application is running
(ETXTBSY).  Any existing file THPs are then dropped from the page cache
when a file is opened for write in do_dentry_open().  Since sys_mmap
ignores MAP_DENYWRITE, this constrains the use of file THPs to vmas
produced by execve().

Systems that make heavy use of shared libraries (e.g.  Android) are unable
to apply VM_DENYWRITE through the dynamic linker, preventing them from
benefiting from the resultant reduced contention on the TLB.

This patch reduces the constraint on file THPs allowing use with any
executable mapping from a file not opened for write (see
inode_is_open_for_write()).  It also introduces additional conditions to
ensure that files opened for write will never be backed by file THPs.

Restricting the use of THPs to executable mappings eliminates the risk
that a read-only file later opened for write would encounter significant
latencies due to page cache truncation.

The ld linker flag '-z max-page-size=(hugepage size)' can be used to
produce executables with the necessary layout.  The dynamic linker must
map these file's segments at a hugepage size aligned vma for the mapping
to be backed with THPs.

Comparison of the performance characteristics of 4KB and 2MB-backed
libraries follows; the Android dex2oat tool was used to AOT compile an
example application on a single ARM core.

Miaohe Lin e6be37b2e7 mm/huge_memory.c: add missing read-only THP checking in transparent_hugepage_enabled()
Since commit 99cb0dbd47 ("mm,thp: add read-only THP support for
(non-shmem) FS"), read-only THP file mapping is supported.  But it forgot
to add checking for it in transparent_hugepage_enabled().  To fix it, we
add checking for read-only THP file mapping and also introduce helper
transhuge_vma_enabled() to check whether thp is enabled for specified vma
to reduce duplicated code.  We rename transparent_hugepage_enabled to
transparent_hugepage_active to make the code easier to follow as suggested
by David Hildenbrand.

Dave Hansen 202e35db5e mm/vmscan: replace implicit RECLAIM_ZONE checks with explicit checks
RECLAIM_ZONE was assumed to be unused because it was never explicitly
used in the kernel.  However, there were a number of places where it was
checked implicitly by checking 'node_reclaim_mode' for a zero value.

These zero checks are not great because it is not obvious what a zero
mode *means* in the code.  Replace them with a helper which makes it
more obvious: node_reclaim_enabled().

This helper also provides a handy place to explicitly check the
RECLAIM_ZONE bit itself.  Check it explicitly there to make it more
obvious where the bit can affect behavior.

This should have no functional impact.

Rik van Riel cd89fb0650 mm,thp,shmem: make khugepaged obey tmpfs mount flags
Currently if thp enabled=[madvise], mounting a tmpfs filesystem with
huge=always and mmapping files from that tmpfs does not result in
khugepaged collapsing those mappings, despite the mount flag indicating
that it should.

Fix that by breaking up the blocks of tests in hugepage_vma_check a little
bit, and testing things in the correct order.

Currently we use struct per_cpu_nodestat to cache the vmstat counters,
which leads to inaccurate statistics especially THP vmstat counters.  In
the systems with hundreds of processors it can be GBs of memory.  For
example, for a 96 CPUs system, the threshold is the maximum number of 125.
And the per cpu counters can cache 23.4375 GB in total.

The THP page is already a form of batched addition (it will add 512 worth
of memory in one go) so skipping the batching seems like sensible.
Although every THP stats update overflows the per-cpu counter, resorting
to atomic global updates.  But it can make the statistics more accuracy
for the THP vmstat counters.

So we convert the NR_SHMEM_THPS account to pages.  This patch is
consistent with 8f182270df ("mm/swap.c: flush lru pvecs on compound page
arrival").  Doing this also can make the unit of vmstat counters more
unified.  Finally, the unit of the vmstat counters are pages, kB and
bytes.  The B/KB suffix can tell us that the unit is bytes or kB.  The
rest which is without suffix are pages.

As huge page usage in the page cache and for shmem files proliferates in
our production environment, the performance monitoring team has asked for
per-cgroup stats on those pages.

We already track and export anon_thp per cgroup.  We already track file
THP and shmem THP per node, so making them per-cgroup is only a matter of
switching from node to lruvec counters.  All callsites are in places where
the pages are charged and locked, so page->memcg is stable.

Vijay Balakrishna 4aab2be098 mm: khugepaged: recalculate min_free_kbytes after memory hotplug as expected by khugepaged
When memory is hotplug added or removed the min_free_kbytes should be
recalculated based on what is expected by khugepaged.  Currently after
hotplug, min_free_kbytes will be set to a lower default and higher
default set when THP enabled is lost.

This change restores min_free_kbytes as expected for THP consumers.

Hugh Dickins 033b5d7755 mm/khugepaged: fix filemap page_to_pgoff(page) != offset
There have been elusive reports of filemap_fault() hitting its
VM_BUG_ON_PAGE(page_to_pgoff(page) != offset, page) on kernels built
with CONFIG_READ_ONLY_THP_FOR_FS=y.

Suren has hit it on a kernel with CONFIG_READ_ONLY_THP_FOR_FS=y and
CONFIG_NUMA is not set: and he has analyzed it down to how khugepaged
without NUMA reuses the same huge page after collapse_file() failed
(whereas NUMA targets its allocation to the respective node each time).
And most of us were usually testing with CONFIG_NUMA=y kernels.

An initial patch replaced __SetPageLocked() by lock_page(), which did
fix the race which Suren illustrates above.  But testing showed that it's
not good enough: if the racing task's __lock_page() gets delayed long
after its find_get_page(), then it may follow collapse_file(new start)'s
successful final unlock_page(), and crash on the same VM_BUG_ON_PAGE.

It could be fixed by relaxing filemap_fault()'s VM_BUG_ON_PAGE to a
check and retry (as is done for mapping), with similar relaxations in
find_lock_entry() and pagecache_get_page(): but it's not obvious what
else might get caught out; and khugepaged non-NUMA appears to be unique
in exposing a page to page cache, then revoking, without going through
a full cycle of freeing before reuse.

Instead, non-NUMA khugepaged_prealloc_page() release the old page
if anyone else has a reference to it (1% of cases when I tested).

Although never reported on huge tmpfs, I believe its find_lock_entry()
has been at similar risk; but huge tmpfs does not rely on khugepaged
for its normal working nearly so much as READ_ONLY_THP_FOR_FS does.

syzbot crashes on the VM_BUG_ON_MM(khugepaged_test_exit(mm), mm) in
__khugepaged_enter(): yes, when one thread is about to dump core, has set
core_state, and is waiting for others, another might do something calling
__khugepaged_enter(), which now crashes because I lumped the core_state
test (known as "mmget_still_valid") into khugepaged_test_exit().  I still
think it's best to lump them together, so just in this exceptional case,
check mm->mm_users directly instead of khugepaged_test_exit().

Joonsoo Kim b518154e59 mm/vmscan: protect the workingset on anonymous LRU
In current implementation, newly created or swap-in anonymous page is
started on active list.  Growing active list results in rebalancing
active/inactive list so old pages on active list are demoted to inactive
list.  Hence, the page on active list isn't protected at all.

Following is an example of this situation.

Assume that 50 hot pages on active list.  Numbers denote the number of
pages on active/inactive list (active | inactive).

1. 50 hot pages on active list
50(h) | 0

2. workload: 50 newly created (used-once) pages
50(uo) | 50(h)

3. workload: another 50 newly created (used-once) pages
50(uo) | 50(uo), swap-out 50(h)

This patch tries to fix this issue.  Like as file LRU, newly created or
swap-in anonymous pages will be inserted to the inactive list.  They are
promoted to active list if enough reference happens.  This simple
modification changes the above example as following.

1. 50 hot pages on active list
50(h) | 0

2. workload: 50 newly created (used-once) pages
50(h) | 50(uo)

3. workload: another 50 newly created (used-once) pages
50(h) | 50(uo), swap-out 50(uo)

As you can see, hot pages on active list would be protected.

Note that, this implementation has a drawback that the page cannot be
promoted and will be swapped-out if re-access interval is greater than the
size of inactive list but less than the size of total(active+inactive).
To solve this potential issue, following patch will apply workingset
detection similar to the one that's already applied to file LRU.

Move collapse_huge_page()'s mmget_still_valid() check into
khugepaged_test_exit() itself.  collapse_huge_page() is used for anon THP
only, and earned its mmget_still_valid() check because it inserts a huge
pmd entry in place of the page table's pmd entry; whereas
collapse_file()'s retract_page_tables() or collapse_pte_mapped_thp()
merely clears the page table's pmd entry.  But core dumping without mmap
lock must have been as open to mistaking a racily cleared pmd entry for a
page table at physical page 0, as exit_mmap() was.  And we certainly have
no interest in mapping as a THP once dumping core.

Hugh Dickins 18e77600f7 khugepaged: retract_page_tables() remember to test exit
Only once have I seen this scenario (and forgot even to notice what forced
the eventual crash): a sequence of "BUG: Bad page map" alerts from
vm_normal_page(), from zap_pte_range() servicing exit_mmap();
pmd:00000000, pte values corresponding to data in physical page 0.

The pte mappings being zapped in this case were supposed to be from a huge
page of ext4 text (but could as well have been shmem): my belief is that
it was racing with collapse_file()'s retract_page_tables(), found *pmd
pointing to a page table, locked it, but *pmd had become 0 by the time
start_pte was decided.

In most cases, that possibility is excluded by holding mmap lock; but
exit_mmap() proceeds without mmap lock.  Most of what's run by khugepaged
checks khugepaged_test_exit() after acquiring mmap lock:
khugepaged_collapse_pte_mapped_thps() and hugepage_vma_revalidate() do so,
for example.  But retract_page_tables() did not: fix that.

The fix is for retract_page_tables() to check khugepaged_test_exit(),
after acquiring mmap lock, before doing anything to the page table.
Getting the mmap lock serializes with __mmput(), which briefly takes and
drops it in __khugepaged_exit(); then the khugepaged_test_exit() check on
mm_users makes sure we don't touch the page table once exit_mmap() might
reach it, since exit_mmap() will be proceeding without mmap lock, not
expecting anyone to be racing with it.

Hugh Dickins 119a5fc161 khugepaged: collapse_pte_mapped_thp() protect the pmd lock
When retract_page_tables() removes a page table to make way for a huge
pmd, it holds huge page lock, i_mmap_lock_write, mmap_write_trylock and
pmd lock; but when collapse_pte_mapped_thp() does the same (to handle the
case when the original mmap_write_trylock had failed), only
mmap_write_trylock and pmd lock are held.

That's not enough.  One machine has twice crashed under load, with "BUG:
spinlock bad magic" and GPF on 6b6b6b6b6b6b6b6b.  Examining the second
crash, page_vma_mapped_walk_done()'s spin_unlock of pvmw->ptl (serving
page_referenced() on a file THP, that had found a page table at *pmd)
discovers that the page table page and its lock have already been freed by
the time it comes to unlock.

Follow the example of retract_page_tables(), but we only need one of huge
page lock or i_mmap_lock_write to secure against this: because it's the
narrower lock, and because it simplifies collapse_pte_mapped_thp() to know
the hpage earlier, choose to rely on huge page lock here.

Hugh Dickins 723a80dafe khugepaged: collapse_pte_mapped_thp() flush the right range
pmdp_collapse_flush() should be given the start address at which the huge
page is mapped, haddr: it was given addr, which at that point has been
used as a local variable, incremented to the end address of the extent.

Found by source inspection while chasing a hugepage locking bug, which I
then could not explain by this.  At first I thought this was very bad;
then saw that all of the page translations that were not flushed would
actually still point to the right pages afterwards, so harmless; then
realized that I know nothing of how different architectures and models
cache intermediate paging structures, so maybe it matters after all -
particularly since the page table concerned is immediately freed.

Much easier to fix than to think about.
