   recovery code has specific enabling for several possible page states

Pull libnvdimm memory-failure update from Dave Jiang:
 "As it stands, memory_failure() gets thoroughly confused by dev_pagemap
  backed mappings. The recovery code has specific enabling for several
  possible page states and needs new enabling to handle poison in dax
  mappings.

  In order to support reliable reverse mapping of user space addresses:

   1/ Add new locking in the memory_failure() rmap path to prevent races
      that would typically be handled by the page lock.

   2/ Since dev_pagemap pages are hidden from the page allocator and the
      "compound page" accounting machinery, add a mechanism to determine
      the size of the mapping that encompasses a given poisoned pfn.

   3/ Given pmem errors can be repaired, change the speculatively
      accessed poison protection, mce_unmap_kpfn(), to be reversible and
      otherwise allow ongoing access from the kernel.

  A side effect of this enabling is that MADV_HWPOISON becomes usable
  for dax mappings, however the primary motivation is to allow the
  system to survive userspace consumption of hardware-poison via dax.
  Specifically the current behavior is:

Dave Jiang e1fb4a0864 dax: remove VM_MIXEDMAP for fsdax and device dax
This patch is reworked from an earlier patch that Dan has posted:
https://patchwork.kernel.org/patch/10131727/

VM_MIXEDMAP is used by dax to direct mm paths like vm_normal_page() that
the memory page it is dealing with is not typical memory from the linear
map.  The get_user_pages_fast() path, since it does not resolve the vma,
is already using {pte,pmd}_devmap() as a stand-in for VM_MIXEDMAP, so we
use that as a VM_MIXEDMAP replacement in some locations.  In the cases
where there is no pte to consult we fallback to using vma_is_dax() to
detect the VM_MIXEDMAP special case.

Now that we have explicit driver pfn_t-flag opt-in/opt-out for
get_user_pages() support for DAX we can stop setting VM_MIXEDMAP.  This
also means we no longer need to worry about safely manipulating vm_flags
in a future where we support dynamically changing the dax mode of a
file.

DAX should also now be supported with madvise_behavior(), vma_merge(),
and copy_page_range().

This patch has been tested against ndctl unit test.  It has also been
tested against xfstests commit: 625515d using fake pmem created by
memmap and no additional issues have been observed.

Stefan Hajnoczi b7751410c1 device-dax: avoid hang on error before devm_memremap_pages()
dax_pmem_percpu_exit() waits for dax_pmem_percpu_release() to invoke the
dax_pmem->cmp completion.  Unfortunately this approach to cleaning up
the percpu_ref only works after devm_memremap_pages() was successful.

If devm_add_action_or_reset() or devm_memremap_pages() fails,
dax_pmem_percpu_release() is not invoked.  Therefore
dax_pmem_percpu_exit() hangs waiting for the completion:

Huaisheng Ye e0b401e3fe dax/super: Do not request a pointer kaddr when not required
Function __bdev_dax_supported doesn't need to get local pointer kaddr
from direct_access. Using NULL instead of having to pass in a useless
local pointer that caller then just throw away.

Dan Williams 35de299547 device-dax: Set page->index
In support of enabling memory_failure() handling for device-dax
mappings, set ->index to the pgoff of the page. The rmap implementation
requires ->index to bound the search through the vma interval tree.

The ->index value is never cleared. There is no possibility for the
page to become associated with another pgoff while the device is
enabled. When the device is disabled the 'struct page' array for the
device is destroyed and ->index is reinitialized to zero.

In support of enabling memory_failure() handling for device-dax
mappings, set the ->mapping association of pages backing device-dax
mappings. The rmap implementation requires page_mapping() to return the
address_space hosting the vmas that map the page.

The ->mapping pointer is never cleared. There is no possibility for the
page to become associated with another address_space while the device is
enabled. When the device is disabled the 'struct page' array for the
device is destroyed / later reinitialized to zero.

Dan Williams 226ab56107 device-dax: Convert to vmf_insert_mixed and vm_fault_t
Use new return type vm_fault_t for fault and huge_fault handler. For
now, this is just documenting that the function returns a VM_FAULT value
rather than an errno.  Once all instances are converted, vm_fault_t will
become a distinct type.

Commit 1c8f422059 ("mm: change return type to vm_fault_t")

Previously vm_insert_mixed() returned an error code which driver mapped into
VM_FAULT_* type. The new function vmf_insert_mixed() will replace this
inefficiency by returning VM_FAULT_* type.

Ross Zwisler 15256f6cc4 dax: check for QUEUE_FLAG_DAX in bdev_dax_supported()
Add an explicit check for QUEUE_FLAG_DAX to __bdev_dax_supported().  This
is needed for DM configurations where the first element in the dm-linear or
dm-stripe target supports DAX, but other elements do not.  Without this
check __bdev_dax_supported() will pass for such devices, letting a
filesystem on that device mount with the DAX option.

* DAX broke a fundamental assumption of truncate of file mapped pages.
   The truncate path assumed that it is safe to disconnect a pinned page
   from a file and let the filesystem reclaim the physical block. With DAX
   the page is equivalent to the filesystem block. Introduce
   dax_layout_busy_page() to enable filesystems to wait for pinned DAX
   pages to be released. Without this wait a filesystem could allocate
   blocks under active device-DMA to a new file.
 
 * DAX arranges for the block layer to be bypassed and uses
   dax_direct_access() + copy_to_iter() to satisfy read(2) calls.
   However, the memcpy_mcsafe() facility is available through the pmem
   block driver. In order to safely handle media errors, via the DAX
   block-layer bypass, introduce copy_to_iter_mcsafe().
 
 * Fix cache management policy relative to the ACPI NFIT Platform
   Capabilities Structure to properly elide cache flushes when they are not
   necessary. The table indicates whether CPU caches are power-fail
   protected. Clarify that a deep flush is always performed on

     pinned page from a file and let the filesystem reclaim the physical
     block. With DAX the page is equivalent to the filesystem block.
     Introduce dax_layout_busy_page() to enable filesystems to wait for
     pinned DAX pages to be released. Without this wait a filesystem
     could allocate blocks under active device-DMA to a new file.

Kees Cook acafe7e302 treewide: Use struct_size() for kmalloc()-family
One of the more common cases of allocation size calculations is finding
the size of a structure that has a zero-sized array at the end, along
with memory for some number of elements for that array. For example:

Change bdev_dax_supported so it takes a bdev parameter.  This enables
multi-device filesystems like xfs to check that a dax device can work for
the particular filesystem.  Once that's in place, actually fix all the
parts of XFS where we need to be able to distinguish between datadev and
rtdev.

This patch fixes the problem where we screw up the dax support checking
in xfs if the datadev and rtdev have different dax capabilities.

Dan Williams e763848843 mm: introduce MEMORY_DEVICE_FS_DAX and CONFIG_DEV_PAGEMAP_OPS
In preparation for fixing dax-dma-vs-unmap issues, filesystems need to
be able to rely on the fact that they will get wakeups on dev_pagemap
page-idle events. Introduce MEMORY_DEVICE_FS_DAX and
generic_dax_page_free() as common indicator / infrastructure for dax
filesytems to require. With this change there are no users of the
MEMORY_DEVICE_HOST designation, so remove it.

The HMM sub-system extended dev_pagemap to arrange a callback when a
dev_pagemap managed page is freed. Since a dev_pagemap page is free /
idle when its reference count is 1 it requires an additional branch to
check the page-type at put_page() time. Given put_page() is a hot-path
we do not want to incur that check if HMM is not in use, so a static
branch is used to avoid that overhead when not necessary.

Now, the FS_DAX implementation wants to reuse this mechanism for
receiving dev_pagemap ->page_free() callbacks. Rework the HMM-specific
static-key into a generic mechanism that either HMM or FS_DAX code paths
can enable.

For ARCH=um builds, and any other arch that lacks ZONE_DEVICE support,
care must be taken to compile out the DEV_PAGEMAP_OPS infrastructure.
However, we still need to support FS_DAX in the FS_DAX_LIMITED case
implemented by the s390/dcssblk driver.

Dan Williams 569d0365f5 dax: require 'struct page' by default for filesystem dax
If a dax buffer from a device that does not map pages is passed to
read(2) or write(2) as a target for direct-I/O it triggers SIGBUS. If
gdb attempts to examine the contents of a dax buffer from a device that
does not map pages it triggers SIGBUS. If fork(2) is called on a process
with a dax mapping from a device that does not map pages it triggers
SIGBUS. 'struct page' is required otherwise several kernel code paths
break in surprising ways. Disable filesystem-dax on devices that do not
map pages.

In addition to needing pfn_to_page() to be valid we also require devmap
pages.  We need this to detect dax pages in the get_user_pages_fast()
path and so that we can stop managing the VM_MIXEDMAP flag. For DAX
drivers that have not supported get_user_pages() to date we allow them
to opt-in to supporting DAX with the CONFIG_FS_DAX_LIMITED configuration
option which requires ->direct_access() to return pfn_t_special() pfns.
This leaves DAX support in brd disabled and scheduled for removal.

Note that when the initial dax support was being merged a few years back
there was concern that struct page was unsuitable for use with next
generation persistent memory devices. The theoretical concern was that
struct page access, being such a hotly used data structure in the
kernel, would lead to media wear out. While that was a reasonable
conservative starting position it has not held true in practice. We have
long since committed to using devm_memremap_pages() to support higher
order kernel functionality that needs get_user_pages() and
pfn_to_page().

Christoph Hellwig e8d5134833 memremap: change devm_memremap_pages interface to use struct dev_pagemap
This new interface is similar to how struct device (and many others)
work. The caller initializes a 'struct dev_pagemap' as required
and calls 'devm_memremap_pages'. This allows the pagemap structure to
be embedded in another structure and thus container_of can be used. In
this way application specific members can be stored in a containing
struct.

This will be used by the P2P infrastructure and HMM could probably
be cleaned up to use it as well (instead of having it's own, similar
'hmm_devmem_pages_create' function).

alignment, the same constraints apply to munmap().  Implement ->split()

Greg Kroah-Hartman b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

Mikulas Patocka c3ca015fab dax: remove the pmem_dax_ops->flush abstraction
Commit abebfbe2f7 ("dm: add ->flush() dax operation support") is
buggy. A DM device may be composed of multiple underlying devices and
all of them need to be flushed. That commit just routes the flush
request to the first device and ignores the other devices.

It could be fixed by adding more complex logic to the device mapper. But
there is only one implementation of the method pmem_dax_ops->flush - that
is pmem_dax_flush() - and it calls arch_wb_cache_pmem(). Consequently, we
don't need the pmem_dax_ops->flush abstraction at all, we can call
arch_wb_cache_pmem() directly from dax_flush() because dax_dev->ops->flush
can't ever reach anything different from arch_wb_cache_pmem().

It should be also pointed out that for some uses of persistent memory it
is needed to flush only a very small amount of data (such as 1 cacheline),
and it would be overkill if we go through that device mapper machinery for
a single flushed cache line.

Fix this by removing the pmem_dax_ops->flush abstraction and call
arch_wb_cache_pmem() directly from dax_flush(). Also, remove the device
mapper code that forwards the flushes.

Currently dm_dax_flush() is not being called, even if underlying dax
device supports write cache, because DAXDEV_WRITE_CACHE is not being
propagated up to the DM dax device.

If the underlying dax device supports write cache, set
DAXDEV_WRITE_CACHE on the DM dax device.  This will cause dm_dax_flush()
to be called.

  errors to userland. Most applications that care about data integrity
  will periodically call fsync/fdatasync/msync to ensure that their
  writes have made it to the backing store.

  For a very long time, we have tracked writeback errors using two flags
  in the address_space: AS_EIO and AS_ENOSPC. Those flags are set when a
  writeback error occurs (via mapping_set_error) and are cleared as a
  side-effect of filemap_check_errors (as you noted yesterday). This
  model really sucks for userland.

  Only the first task to call fsync (or msync or fdatasync) will see the
  error. Any subsequent task calling fsync on a file will get back 0
  (unless another writeback error occurs in the interim). If I have
  several tasks writing to a file and calling fsync to ensure that their
  writes got stored, then I need to have them coordinate with one
  another. That's difficult enough, but in a world of containerized
  setups that coordination may even not be possible.

  But wait...it gets worse!

  The calls to filemap_check_errors can be buried pretty far down in the
  call stack, and there are internal callers of filemap_write_and_wait
  and the like that also end up clearing those errors. Many of those
  callers ignore the error return from that function or return it to
  userland at nonsensical times (e.g. truncate() or stat()). If I get
  back -EIO on a truncate, there is no reason to think that it was
  because some previous writeback failed, and a subsequent fsync() will
  (incorrectly) return 0.

  This pile aims to do three things:

   1) ensure that when a writeback error occurs that that error will be
      reported to userland on a subsequent fsync/fdatasync/msync call,
      regardless of what internal callers are doing

   2) report writeback errors on all file descriptions that were open at
      the time that the error occurred. This is a user-visible change,
      but I think most applications are written to assume this behavior
      anyway. Those that aren't are unlikely to be hurt by it.

   3) document what filesystems should do when there is a writeback
      error. Today, there is very little consistency between them, and a
      lot of cargo-cult copying. We need to make it very clear what
      filesystems should do in this situation.

  To achieve this, the set adds a new data type (errseq_t) and then
  builds new writeback error tracking infrastructure around that. Once
  all of that is in place, we change the filesystems to use the new
  infrastructure for reporting wb errors to userland.

  Note that this is just the initial foray into cleaning up this mess.
  There is a lot of work remaining here:

   1) convert the rest of the filesystems in a similar fashion. Once the
      initial set is in, then I think most other fs' will be fairly
      simple to convert. Hopefully most of those can in via individual
      filesystem trees.

   2) convert internal waiters on writeback to use errseq_t for
      detecting errors instead of relying on the AS_* flags. I have some
      draft patches for this for ext4, but they are not quite ready for
      prime time yet.

Jeff Layton 5660e13d2f fs: new infrastructure for writeback error handling and reporting
Most filesystems currently use mapping_set_error and
filemap_check_errors for setting and reporting/clearing writeback errors
at the mapping level. filemap_check_errors is indirectly called from
most of the filemap_fdatawait_* functions and from
filemap_write_and_wait*. These functions are called from all sorts of
contexts to wait on writeback to finish -- e.g. mostly in fsync, but
also in truncate calls, getattr, etc.

The non-fsync callers are problematic. We should be reporting writeback
errors during fsync, but many places spread over the tree clear out
errors before they can be properly reported, or report errors at
nonsensical times.

If I get -EIO on a stat() call, there is no reason for me to assume that
it is because some previous writeback failed. The fact that it also
clears out the error such that a subsequent fsync returns 0 is a bug,
and a nasty one since that's potentially silent data corruption.

This patch adds a small bit of new infrastructure for setting and
reporting errors during address_space writeback. While the above was my
original impetus for adding this, I think it's also the case that
current fsync semantics are just problematic for userland. Most
applications that call fsync do so to ensure that the data they wrote
has hit the backing store.

In the case where there are multiple writers to the file at the same
time, this is really hard to determine. The first one to call fsync will
see any stored error, and the rest get back 0. The processes with open
fds may not be associated with one another in any way. They could even
be in different containers, so ensuring coordination between all fsync
callers is not really an option.

One way to remedy this would be to track what file descriptor was used
to dirty the file, but that's rather cumbersome and would likely be
slow. However, there is a simpler way to improve the semantics here
without incurring too much overhead.

This set adds an errseq_t to struct address_space, and a corresponding
one is added to struct file. Writeback errors are recorded in the
mapping's errseq_t, and the one in struct file is used as the "since"
value.

This changes the semantics of the Linux fsync implementation such that
applications can now use it to determine whether there were any
writeback errors since fsync(fd) was last called (or since the file was
opened in the case of fsync having never been called).

Note that those writeback errors may have occurred when writing data
that was dirtied via an entirely different fd, but that's the case now
with the current mapping_set_error/filemap_check_error infrastructure.
This will at least prevent you from getting a false report of success.

The new behavior is still consistent with the POSIX spec, and is more
reliable for application developers. This patch just adds some basic
infrastructure for doing this, and ensures that the f_wb_err "cursor"
is properly set when a file is opened. Later patches will change the
existing code to use this new infrastructure for reporting errors at
fsync time.

Dan Williams 6e0c90d691 libnvdimm, pmem, dax: export a cache control attribute
The dax_flush() operation can be turned into a nop on platforms where
firmware arranges for cpu caches to be flushed on a power-fail event.
The ACPI 6.2 specification defines a mechanism for the platform to
indicate this capability so the kernel can select the proper default.
However, for other platforms, the administrator must toggle this setting
manually.

Given this flush setting is a dax-specific mechanism we advertise it
through a 'dax' attribute group hanging off a host device. For example,
a 'pmem0' block-device gets a 'dax' sysfs-subdirectory with a
'write_cache' attribute to control response to dax cache flush requests.
This is similar to the 'queue/write_cache' attribute that appears under
block devices.

For configurations that do not enable DAX filesystems or drivers, do not
require the DAX core to be built.

Given that the 'direct_access' method has been removed from
'block_device_operations', we can also go ahead and remove the
block-related dax helper functions from fs/block_dev.c to
drivers/dax/super.c. This keeps dax details out of the block layer and
lets the DAX core be built as a module in the FS_DAX=n case.

Filesystems need to include dax.h to call bdev_dax_supported().

Dan Williams 7138970383 mm, zone_device: Replace {get, put}_zone_device_page() with a single reference to fix pmem crash
The x86 conversion to the generic GUP code included a small change which causes
crashes and data corruption in the pmem code - not good.

The root cause is that the /dev/pmem driver code implicitly relies on the x86
get_user_pages() implementation doing a get_page() on the page refcount, because
get_page() does a get_zone_device_page() which properly refcounts pmem's separate
page struct arrays that are not present in the regular page struct structures.
(The pmem driver does this because it can cover huge memory areas.)

But the x86 conversion to the generic GUP code changed the get_page() to
page_cache_get_speculative() which is faster but doesn't do the
get_zone_device_page() call the pmem code relies on.

One way to solve the regression would be to change the generic GUP code to use
get_page(), but that would slow things down a bit and punish other generic-GUP
using architectures for an x86-ism they did not care about. (Arguably the pmem
driver was probably not working reliably for them: but nvdimm is an Intel
feature, so non-x86 exposure is probably still limited.)

So restructure the pmem code's interface with the MM instead: get rid of the
get/put_zone_device_page() distinction, integrate put_zone_device_page() into
__put_page() and and restructure the pmem completion-wait and teardown machinery:

Kirill points out that the calls to {get,put}_dev_pagemap() can be
removed from the mm fast path if we take a single get_dev_pagemap()
reference to signify that the page is alive and use the final put of the
page to drop that reference.

This does require some care to make sure that any waits for the
percpu_ref to drop to zero occur *after* devm_memremap_page_release(),
since it now maintains its own elevated reference.

This speeds up things while also making the pmem refcounting more robust going
forward.

Dan Williams b0686260fe dax: introduce dax_direct_access()
Replace bdev_direct_access() with dax_direct_access() that uses
dax_device and dax_operations instead of a block_device and
block_device_operations for dax. Once all consumers of the old api have
been converted bdev_direct_access() will be deleted.

Given that block device partitioning decisions can cause dax page
alignment constraints to be violated this also introduces the
bdev_dax_pgoff() helper. It handles calculating a logical pgoff relative
to the dax_device and also checks for page alignment.

Dan Williams 6568b08b77 dax: introduce dax_operations
Track a set of dax_operations per dax_device that can be set at
alloc_dax() time. These operations will be used to stop the abuse of
block_device_operations for communicating dax capabilities to
filesystems. It will also be used to replace the "pmem api" and move
pmem-specific cache maintenance, and other dax-driver-specific
filesystem-dax operations, to dax device methods. In particular this
allows us to stop abusing __copy_user_nocache(), via memcpy_to_pmem(),
with a driver specific replacement.

This is a standalone introduction of the operations. Follow on patches
convert each dax-driver and teach fs/dax.c to use ->direct_access() from
dax_operations instead of block_device_operations.

for it to lookup the dax_device associated with a block_device. Add a
'host' property of a dax_device that can be used for this purpose. It is
a free form string, but for a dax_device associated with a block device
it is the bdev name.

This is a stop-gap until filesystems are able to mount on a dax-inode
directly.

Dan Williams 7b6be8444e dax: refactor dax-fs into a generic provider of 'struct dax_device' instances
We want dax capable drivers to be able to publish a set of dax
operations [1]. However, we do not want to further abuse block_devices
to advertise these operations. Instead we will attach these operations
to a dax device and add a lookup mechanism to go from block device path
to a dax device. A dax capable driver like pmem or brd is responsible
for registering a dax device, alongside a block device, and then a dax
capable filesystem is responsible for retrieving the dax device by path
name if it wants to call dax_operations.

For now, we refactor the dax pseudo-fs to be a generic facility, rather
than an implementation detail, of the device-dax use case. Where a "dax
device" is just an inode + dax infrastructure, and "Device DAX" is a
mapping service layered on top of that base 'struct dax_device'.
"Filesystem DAX" is then a mapping service that layers a filesystem on
top of that same base device. Filesystem DAX is associated with a
block_device for now, but perhaps directly to a dax device in the
future, or for new pmem-only filesystems.

Pushkar Jambhlekar 54eafcc9e3 device-dax: fix dax_dev_huge_fault() unknown fault size handling
The default case for dax_dev_huge_fault() fault size handling mistakenly
returns when it should unlock. This is not a problem in practice since
the only three possible fault sizes are handled. Going forward, if the
core mm adds a new fault size beyond pte, pmd, or pud device-dax should
abort VM_FAULT_SIGBUS requests not VM_FAULT_FALLBACK since device-dax
guarantees a configured fault granularity for all faults.

If device_add() fails, cleanup the cdev. Otherwise, we leak a kobj_map()
with a stale device number.

As Jason points out, there is a small possibility that userspace has

any established mappings.

Dave Jiang c791ace1e7 mm: replace FAULT_FLAG_SIZE with parameter to huge_fault
Since the introduction of FAULT_FLAG_SIZE to the vm_fault flag, it has
been somewhat painful with getting the flags set and removed at the
correct locations.  More than one kernel oops was introduced due to
difficulties of getting the placement correctly.

Remove the flag values and introduce an input parameter to huge_fault
that indicates the size of the page entry.  This makes the code easier
to trace and should avoid the issues we see with the fault flags where
removal of the flag was necessary in the fallback paths.

Dave Jiang a2d581675d mm,fs,dax: change ->pmd_fault to ->huge_fault
Patch series "1G transparent hugepage support for device dax", v2.

The following series implements support for 1G trasparent hugepage on
x86 for device dax.  The bulk of the code was written by Mathew Wilcox a
while back supporting transparent 1G hugepage for fs DAX.  I have
forward ported the relevant bits to 4.10-rc.  The current submission has
only the necessary code to support device DAX.

Comments from Dan Williams: So the motivation and intended user of this
functionality mirrors the motivation and users of 1GB page support in
hugetlbfs.  Given expected capacities of persistent memory devices an
in-memory database may want to reduce tlb pressure beyond what they can
already achieve with 2MB mappings of a device-dax file.  We have
customer feedback to that effect as Willy mentioned in his previous
version of these patches [1].

There are applications which have a process model; and if you assume
10,000 processes attempting to mmap all the 6TB memory available on a
server; we are looking at the following:

processes         : 10,000
memory            :    6TB
pte @ 4k page size: 8 bytes / 4K of memory * #processes = 6TB / 4k * 8 * 10000 = 1.5GB * 80000 = 120,000GB
pmd @ 2M page size: 120,000 / 512 = ~240GB
pud @ 1G page size: 240GB / 512 = ~480MB

As you can see with 2M pages, this system will use up an exorbitant
amount of DRAM to hold the page tables; but the 1G pages finally brings
it down to a reasonable level.  Memory sizes will keep increasing; so
this number will keep increasing.

An argument can be made to convert the applications from process model
to thread model, but in the real world that may not be always practical.
Hopefully this helps explain the use case where this is valuable.

In preparation for adding the ability to handle PUD pages, convert
vm_operations_struct.pmd_fault to vm_operations_struct.huge_fault.  The
vm_fault structure is extended to include a union of the different page
table pointers that may be needed, and three flag bits are reserved to

Dave Jiang 11bac80004 mm, fs: reduce fault, page_mkwrite, and pfn_mkwrite to take only vmf
->fault(), ->page_mkwrite(), and ->pfn_mkwrite() calls do not need to
take a vma and vmf parameter when the vma already resides in vmf.

Remove the vma parameter to simplify things.

Dave Jiang f42003917b mm, dax: change pmd_fault() to take only vmf parameter
pmd_fault() and related functions really only need the vmf parameter since
the additional parameters are all included in the vmf struct.  Remove the
additional parameter and simplify pmd_fault() and friends.

Dave Jiang d8a849e1bc mm, dax: make pmd_fault() and friends be the same as fault()
Instead of passing in multiple parameters in the pmd_fault() handler,
a vmf can be passed in just like a fault() handler. This will simplify
code and remove the need for the actual pmd fault handlers to allocate a
vmf. Related functions are also modified to do the same.

Dan Williams d7fe1a67f6 dax: add region 'id', 'size', and 'align' attributes
While this information is available by looking at the nvdimm parent
device that may not always be the case when/if we add support for other
memory regions. Tooling should not depend on walking a given ancestor
topology that is not guaranteed by the device's class. For example, a
device-dax instance will always have a dax_region parent, but it may not
always have a libnvdimm "dax" device as a grandparent.

Jan Kara 1a29d85eb0 mm: use vmf->address instead of of vmf->virtual_address
Every single user of vmf->virtual_address typed that entry to unsigned
long before doing anything with it so the type of virtual_address does
not really provide us any additional safety.  Just use masked
vmf->address which already has the appropriate type.

Dan Williams 325896ffdf device-dax: fix private mapping restriction, permit read-only
Hugh notes in response to commit 4cb19355ea "device-dax: fail all
private mapping attempts":

  "I think that is more restrictive than you intended: haven't tried, but I
  believe it rejects a PROT_READ, MAP_SHARED, O_RDONLY fd mmap, leaving no
  way to mmap /dev/dax without write permission to it."

Indeed it does restrict read-only mappings, switch to checking
VM_MAYSHARE, not VM_SHARED.
