This patch adds a capability check before enabling DCBX.
In addition, it re-organizes the relevant data structures,
and fixes a typo in a define.

mlx4_en_dcbnl_set_state() returns u8, the return value from
mlx4_en_setup_tc() could be negative in case of failure, so fix that.

Create two new files fw_qos.h and fw_qos.c in mlx4_core module.
It gathers all relevant QoS firmware related commands etc, thus improving
encapsulation of the mlx4_core module. For now it contains the QoS existing
commands: mlx4_SET_PORT_SCHEDULER and mlx4_SET_PORT_PRIO2TC.

Flow control per priority and Global pause counters are now visible via
ethtool. The counters shows statistics regarding pauses in the device.

Fix a bug when FC and PFC are enabled/disabled at the same time.
According to ConnectX-3 Programmer Manual these two features are mutial
exclusive. So make sure when enabling PFC to turn off global FC and
vise versa. Otherwise it hurts the performance.

Enable the DCB ETS ops only when supported by the firmware. For older firmware/cards
which don't support ETS, advertize only PFC DCB ops.

introduced 64 bit math operations into mlx4_en_dcbnl_ieee_setmaxrate()
causing the following final link failure on an x86_32 allmodconfig
ERROR: "__udivdi3" [drivers/net/ethernet/mellanox/mlx4/mlx4_en.ko] undefined!
Convert it to use div_u64() instead.
