This fixes a crash introduced by recent is_kdump_kernel() check.
The source of the crash is that kdump kernel can be loaded on a
system with already created VFs. But for such VFs, it will follow
a logic path of PF and eventually crash.
Thus, we are partially reverting back previous changes and instead
use is_kdump_kernel is a single init point of PF init, where we
disable SRIOV explicitly.

Since we have kdump kernel(s) running under severe memory constraint
it makes sense to disable the qed SRIOV functionality when running the
kdump kernel as kdump configurations on several distributions don't
support SRIOV targets for saving the vmcore (see [1] for example).
Currently the qed SRIOV functionality ends up consuming memory in
the kdump kernel, when we don't really use the same.
An example log seen in the kdump kernel with the SRIOV functionality
enabled can be seen below (obtained via memstrack tool, see [2]):

This patch disables the SRIOV functionality inside kdump kernel and with
the same applied the memory consumption goes down:

The qed_chain data structure was modified in
commit 1a4a69751f ("qed: Chain support for external PBL") to support
receiving an external pbl (due to iWARP FW requirements).
The pages pointed to by the pbl are allocated in qed_chain_alloc
and their virtual address are stored in an virtual addresses array to
enable accessing and freeing the data. The physical addresses however
weren't stored and were accessed directly from the external-pbl
during free.
Destroy-qp flow, leads to freeing the external pbl before the chain is
freed, when the chain is freed it tries accessing the already freed
external pbl, leading to a use-after-free. Therefore we need to store
the physical addresses in additional to the virtual addresses in a
new data structure.

LL2 queues were a limited resource due to FW constraints.
This FW introduced a new resource which is a context based ll2 queue
(memory on host). The additional ll2 queues are required for RDMA SRIOV.
The code refers to the previous ll2 queues as ram-based or legacy, and the
new queues as ctx-based.
This change decreased the "legacy" ram-based queues therefore the first ll2
queue used for iWARP was converted to the ctx-based ll2 queue.

channel functionality was added to the fw_funcs file, since the code
is very similar to the previously used code, the structures used were
moved to qed_hsi. Due to FW conventions, the names of the flags in the
struct changed. Since this required slight modification in the places
that set the flags the code was modified to use GET/SET FIELD macros.

This patch contains changes in initialization and usage of the QM blocks.
Instead of setting a rate limiter per vport the rate limiters are now a
global resource and set independentaly.

This patch contains register initialization related changes.
- Modifications to the runtime offsets - these are defines used
by the driver or firmware functions to set values that are used
by the initialization functions to set device register values.
- Global window values changes to provide different device register
ranges.
- Additional device registers addresses were added to the register file,
used in later stages.

Current implementation for nvm_attr configuration instructs the management
FW to load/unload the nvm-cfg image for each user-provided attribute in
the input file. This consumes lot of cycles even for few tens of
attributes.
This patch updates the implementation to perform load/commit of the config
for every 50 attributes. After loading the nvm-image, MFW expects that
config should be committed in a predefined timer value (5 sec), hence it's
not possible to write large number of attributes in a single load/commit
window. Hence performing the commits in chunks.

This patch refactors the current llh implementation. It exposes a hw

the number of filters per PF and did not support engine affinity per

enables affinitizing protocol packets to a certain engine which

Pull mmiowb removal from Will Deacon:
"Remove Mysterious Macro Intended to Obscure Weird Behaviours (mmiowb())
Remove mmiowb() from the kernel memory barrier API and instead, for
architectures that need it, hide the barrier inside spin_unlock() when
MMIO has been performed inside the critical section.
The only relatively recent changes have been addressing review
comments on the documentation, which is in a much better shape thanks
to the efforts of Ben and Ingo.
I was initially planning to split this into two pull requests so that
you could run the coccinelle script yourself, however it's been plain
sailing in linux-next so I've just included the whole lot here to keep
things simple"

Separate the overflow handling from the hardware interrupt status analysis.
The interrupt status is a single register and is common for all PFs. The
first PF reading the register is not necessarily the one who overflowed.
All PFs must check their overflow status on every attention.
In this change we clear the sticky indication in the attention handler to
allow doorbells to be processed again as soon as possible, but running
the doorbell recovery is scheduled for the periodic handler to reduce the
time spent in the attention handler.
Checking the need for DORQ flush was changed to "db_bar_no_edpm" because
qed_edpm_enabled()'s result could change dynamically and might have
prevented a needed flush.

When the DORQ (doorbell block) is overflowed, all PFs get attentions at the
same time. If one PF finished handling the attention before another PF even
started, the second PF might miss the DORQ's attention bit and not handle
the attention at all.
If the DORQ attention is missed and the issue is not resolved, another
attention will not be sent, therefore each attention is treated as a
potential DORQ attention.
As a result, the attention callback is called more frequently so the debug
print was moved to reduce its quantity.
The number of periodic doorbell recovery handler schedules was reduced
because it was the previous way to mitigating the missed attention issue.

DB_REC_DRY_RUN (running doorbell recovery without sending doorbells) is
never used. DB_REC_ONCE (send a single doorbell from the doorbell recovery)
is not needed anymore because by running the periodic handler we make sure
we check the overflow status later instead.
This patch is needed because in the next patches, the only doorbell
recovery type being used is DB_REC_REAL_DEAL, and the fixes are much
cleaner without this enum.

mmiowb() is now implied by spin_unlock() on architectures that require
it, so there is no reason to call it from driver code. This patch was
generated using coccinelle:
@mmiowb@
@@
- mmiowb();
and invoked as:
$ for d in drivers include/linux/qed sound; do \
spatch --include-headers --sp-file mmiowb.cocci --dir $d --in-place; done
NOTE: mmiowb() has only ever guaranteed ordering in conjunction with
spin_unlock(). However, pairing each mmiowb() removal in this patch with
the corresponding call to spin_unlock() is not at all trivial, so there
is a small chance that this change may regress any drivers incorrectly
relying on mmiowb() to order MMIO writes between CPUs using lock-free
synchronisation. If you've ended up bisecting to this commit, you can
reintroduce the mmiowb() calls using wmb() instead, which should restore
the old behaviour on all architectures other than some esoteric ia64
systems.
