Andy Honig fda4e2e855 KVM: x86: Convert vapic synchronization to _cached functions (CVE-2013-6368)
In kvm_lapic_sync_from_vapic and kvm_lapic_sync_to_vapic there is the
potential to corrupt kernel memory if userspace provides an address that
is at the end of a page.  This patches concerts those functions to use
kvm_write_guest_cached and kvm_read_guest_cached.  It also checks the
vapic_address specified by userspace during ioctl processing and returns
an error to userspace if the address is not a valid GPA.

This is generally not guest triggerable, because the required write is
done by firmware that runs before the guest.  Also, it only affects AMD
processors and oldish Intel that do not have the FlexPriority feature
(unless you disable FlexPriority, of course; then newer processors are
also affected).

Andy Honig b963a22e6d KVM: x86: Fix potential divide by 0 in lapic (CVE-2013-6367)
Under guest controllable circumstances apic_get_tmcct will execute a
divide by zero and cause a crash.  If the guest cpuid support
tsc deadline timers and performs the following sequence of requests
the host will crash.
- Set the mode to periodic
- Set the TMICT to 0
- Set the mode bits to 11 (neither periodic, nor one shot, nor tsc deadline)
- Set the TMICT to non-zero.
Then the lapic_timer.period will be 0, but the TMICT will not be.  If the
guest then reads from the TMCCT then the host will perform a divide by 0.

This patch ensures that if the lapic_timer.period is 0, then the division
does not occur.

Andy Honig 338c7dbadd KVM: Improve create VCPU parameter (CVE-2013-4587)
In multiple functions the vcpu_id is used as an offset into a bitfield.  Ag
malicious user could specify a vcpu_id greater than 255 in order to set or
clear bits in kernel memory.  This could be used to elevate priveges in the
kernel.  This patch verifies that the vcpu_id provided is less than 255.
The api documentation already specifies that the vcpu_id must be less than
max_vcpus, but this is currently not checked.

Elie De Brauwer 2212a8529e i2c: mux: Inherit retry count and timeout from parent for muxed bus
If a muxed i2c bus gets created the default retry count and
timeout of the muxed bus is zero. Hence it it possible that you
end up with a situation where the parent controller sets a default
retry count and timeout which gets applied and used while the muxed
bus (using the same controller) has a default retry count of zero
and a default timeout of 1s (set in i2c_add_adapter()). This can be
solved by initializing the retry count and timeout of the muxed
bus with the values used by the the parent at creation time.

Paul Durrant a3314f3d40 xen-netback: fix gso_prefix check
There is a mistake in checking the gso_prefix mask when passing large
packets to a guest. The wrong shift is applied to the bit - the raw skb
gso type is used rather then the translated one. This leads to large packets
being handed to the guest without the GSO metadata. This patch fixes the
check.

The mistake manifested as errors whilst running Microsoft HCK large packet
offload tests between a pair of Windows 8 VMs. I have verified this patch
fixes those errors.

Sebastian Siewior a0a9663dd2 net: make neigh_priv_len in struct net_device 16bit instead of 8bit
neigh_priv_len is defined as u8. With all debug enabled struct
ipoib_neigh has 200 bytes. The largest part is sk_buff_head with 96
bytes and here the spinlock with 72 bytes.
The size value still fits in this u8 leaving some room for more.

On -RT struct ipoib_neigh put on weight and has 392 bytes. The main
reason is sk_buff_head with 288 and the fatty here is spinlock with 192
bytes. This does no longer fit into into neigh_priv_len and gcc
complains.

This patch changes neigh_priv_len from being 8bit to 16bit. Since the
following element (dev_id) is 16bit followed by a spinlock which is
aligned, the struct remains with a total size of 3200 (allmodconfig) /
2048 (with as much debug off as possible) bytes on x86-64.
On x86-32 the struct is 1856 (allmodconfig) / 1216 (with as much debug
off as possible) bytes long. The numbers were gained with and without
the patch to prove that this change does not increase the size of the
struct.

Stephen Warren f1d6ff7939 ASoC: tegra: update module reset list for Tegra124
Tegra124 adds a number of extra modules into the configlink bus, which
must be taken out of reset before the bus is used. Update the AHUB
driver to know about these extra modules (the AHUB HW module hosts the
configlink bus).

This patch depends on "ASoC: tegra: use reset framework" to compile,
which is ack'd and slated to go through a (large) topic branch in the
Tegra tree. So, we can either:

a) Merge that Tegra topic branch into the ASoC tree, then apply this.
   Note that I haven't created the topic branch yet, since I'm still
   waiting for DMA dependencies to be applied.

b) Apply this change to the Tegra tree too. This change isn't directly
   related to the changes in the Tegra tree; it just makes use of the new
   reset controller feature that's introduced there.

When CPSW and Davinci MDIO are build as modules, CPSW crashes when
accessing CPSW registers in CPSW probe. The same is working in built-in
as the CPSW clocks are enabled in Davindi MDIO probe, SO Enabling the
clocks before accessing the version register and moving out the other
register access to cpsw device open.

Will Deacon 11ec50caed word-at-a-time: provide generic big-endian zero_bytemask implementation
Whilst architectures may be able to do better than this (which they can,
by simply defining their own macro), this is a generic stab at a
zero_bytemask implementation for the asm-generic, big-endian
word-at-a-time implementation.

On arm64, a clz instruction is used to implement the fls efficiently.

Will Deacon a5c21dcefa dcache: allow word-at-a-time name hashing with big-endian CPUs
When explicitly hashing the end of a string with the word-at-a-time
interface, we have to be careful which end of the word we pick up.

On big-endian CPUs, the upper-bits will contain the data we're after, so
ensure we generate our masks accordingly (and avoid hashing whatever
random junk may have been sitting after the string).

This patch adds a new dcache helper, bytemask_from_count, which creates
a mask appropriate for the CPU endianness.

Paul Durrant d9601a36ff xen-netback: napi: don't prematurely request a tx event
This patch changes the RING_FINAL_CHECK_FOR_REQUESTS in
xenvif_build_tx_gops to a check for RING_HAS_UNCONSUMED_REQUESTS as the
former call has the side effect of advancing the ring event pointer and
therefore inviting another interrupt from the frontend before the napi
poll has actually finished, thereby defeating the point of napi.

The event pointer is updated by RING_FINAL_CHECK_FOR_REQUESTS in
xenvif_poll, the napi poll function, if the work done is less than the
budget i.e. when actually transitioning back to interrupt mode.

Paul Durrant 10574059ce xen-netback: napi: fix abuse of budget
netback seems to be somewhat confused about the napi budget parameter. The
parameter is supposed to limit the number of skbs processed in each poll,
but netback has this confused with grant operations.

This patch fixes that, properly limiting the work done in each poll. Note
that this limit makes sure we do not process any more data from the shared
ring than we intend to pass back from the poll. This is important to
prevent tx_queue potentially growing without bound.

     Unfortunately, the garbage collector still calls big_key_destroy()
     when sees one of the pointers with a weird value in it (and not
     NULL) which it then tries to clean up.

   - Three patches to fix the keyring type:

     * A patch to fix the hash function to correctly divide keyrings off
       from keys in the topology of the tree inside the associative
       array.  This is only a problem if searching through nested
       keyrings - and only if the hash function incorrectly puts the a
       keyring outside of the 0 branch of the root node.

     * A patch to fix keyrings' use of the associative array.  The
       __key_link_begin() function initially passes a NULL key pointer
       to assoc_array_insert() on the basis that it's holding a place in
       the tree whilst it does more allocation and stuff.

       This is only a problem when a node contains 16 keys that match at
       that level and we want to add an also matching 17th.  This should
       easily be manufactured with a keyring full of keyrings (without
       chucking any other sort of key into the mix) - except for (a)
       above which makes it on average adding the 65th keyring.

     * A patch to fix searching down through nested keyrings, where any
       keyring in the set has more than 16 keyrings and none of the
       first keyrings we look through has a match (before the tree
       iteration needs to step to a more distal node).

   - A patch to fix the big_key type's use of a shmem file as its
     backing store causing audit messages and LSM check failures.  This
     is done by setting S_PRIVATE on the file to avoid LSM checks on the
     file (access to the shmem file goes through the keyctl() interface
     and so is gated by the LSM that way).

     This isn't normally a problem if a key is used by the context that
     generated it - and it's currently only used by libkrb5.

Linus Torvalds 5cdec2d833 futex: move user address verification up to common code
When debugging the read-only hugepage case, I was confused by the fact
that get_futex_key() did an access_ok() only for the non-shared futex
case, since the user address checking really isn't in any way specific
to the private key handling.

Now, it turns out that the shared key handling does effectively do the
equivalent checks inside get_user_pages_fast() (it doesn't actually
check the address range on x86, but does check the page protections for
being a user page).  So it wasn't actually a bug, but the fact that we
treat the address differently for private and shared futexes threw me
for a loop.

Just move the check up, so that it gets done for both cases.  Also, use
the 'rw' parameter for the type, even if it doesn't actually matter any
more (it's a historical artifact of the old racy i386 "page faults from
kernel space don't check write protections").

Filipe David Borba Manana 639eefc8af Btrfs: don't miss skinny extent items on delayed ref head contention
Currently extent-tree.c:btrfs_lookup_extent_info() can miss the lookup
of skinny extent items. This can happen when the execution flow is the
following:

* We do an extent tree lookup and fail to find a skinny extent item;

* As a result, we attempt to see if a non-skinny extent item exists,
  either by looking at previous item in the leaf or by doing another
  full extent tree search;

* We have a transaction and then we check for a matching delayed ref
  head in the transaction's delayed refs rbtree;

* We find such delayed ref head and then we try to lock it with a
  call to mutex_trylock();

* The lock was contended so we jump to the label "again", which repeats
  the extent tree search but for a non-skinny extent item, because we set
  previously metadata variable to 0 and the search key to look for a
  non-skinny extent-item;

* After the jump (and after releasing the transaction's delayed refs
  lock), a skinny extent item might have been added to the extent tree
  but we will miss it because metadata is set to 0 and the search key
  is set for a non-skinny extent-item.

The fix here is to not reset metadata to 0 and to jump to the initial search
key setup if the delayed ref head is contended, instead of jumping directly
to the extent tree search label ("again").

This issue was found while investigating the issue reported at Bugzilla 64961.

David Sterba suspected this function was missing extent items, and that
this could be caused by the last change to this function, which was made
in the following patch:

    (commit 74be951087)

But in fact this issue already existed before, because after failing to find
a skinny extent item, the code set the search key for a non-skinny extent
item, and on contention of a matching delayed ref head it would not search
the extent tree for a skinny extent item anymore.

It is because we cleared the default compression type when setting the
nodatacow. In fact, we needn't do it because we have used COMPRESS flag to
indicate if we need compressed the file data or not, needn't use the

use it to hold the default compression type. Or we would get a wrong compress
type for a file whose own compress flag is set but the compress flag of its
filesystem is not set.

Paulo Zanoni 566b734a19 drm/i915: split intel_ddi_pll_mode_set in 2 pieces
The first piece, intel_ddi_pll_select, finds a PLL and assigns it to
the CRTC, but doesn't write any register. It can also fail in case it
doesn't find a PLL.

The second piece, intel_ddi_pll_enable, uses the information stored by
intel_ddi_pll_select to actually enable the PLL by writing to its
register. This function can't fail. We also have some refcount sanity
checks here.

The idea is that one day we'll remove all the functions that touch
registers from haswell_crtc_mode_set to haswell_crtc_enable, so we'll
call intel_ddi_pll_select at haswell_crtc_mode_set and then call
intel_ddi_pll_enable at haswell_crtc_enable. Since I'm already
touching this code, let's take care of this particular split today.

Mika Kuoppala 47e9766df0 drm/i915: Fix timeout with missed interrupts in __wait_seqno
Commit 094f9a54e3 ("drm/i915: Fix __wait_seqno to use true infinite
timeouts") added support for __wait_seqno to detect missing interrupts and
go around them by polling. As there is also timeout detection in
__wait_seqno, the polling and timeout detection were done with the same
timer.

When there has been missed interrupts and polling is needed, the timer is
set to trigger in (now + 1) jiffies in future, instead of the caller
specified timeout.

Now when io_schedule() returns, we calculate the jiffies left to timeout
using the timer expiration value. As the current jiffies is now bound to be
always equal or greater than the expiration value, the timeout_jiffies will
become zero or negative and we return -ETIME to caller even tho the
timeout was never reached.

Fix this by decoupling timeout calculation from timer expiration.

The bug I'm seeing can be reproduced with:
  - Have vgacon configured/enabled
  - Make sure the power well gets disabled, then enabled. You can
    check this by seeing the messages print by hsw_set_power_well
  - Stop your display manager
  - echo 0 > /sys/class/vtconsole/vtcon1/bind

I can easily reproduce this by blacklising snd_hda_intel and booting
with eDP+HDMI.

If you do this and then look at dmesg, you'll see we're printing
infinite "Unclaimed register" messages. This is happening because
we're stuck on an infinite loop inside console_unlock(), which is
calling many functions from vgacon.c. And the code that's triggering
the error messages is from vgacon_set_cursor_size().

After we re-enable the power well, every time we read/write the VGA
address 0x3d5 we get an "unclaimed register" interrupt (ERR_INT) and
print error messages. If we write anything to the VGA MSR register (it
doesn't really matter which value you write to bit 0), any
reads/writes to 0x3d5 _don't_ trigger the "unclaimed register" errors
anymore (even if MSR bit 0 is zero). So what happens with the current
code is that when we unbind i915 and bind vgacon, we call
console_unlock(). Function console_unlock() is responsible for
printing any messages that were supposed to be print when the console
was locked, so it calls the TTY layer, which calls the console layer,
which calls vgacon to print the messages. At this point, vgacon
eventually calls vgacon_set_cursor_size(), which touches 0x3d5, which
triggers unclaimed register interrupts. The problem is that when we
get these interrupts, we print the error messages, so we add more work
to console_unlock(), which will try to print it again, and then call
vgacon again, trigger a new interrupt, which will put more stuff to
the buffer, and then we'll be stuck at console_unlock() forever.

If you patch intel_uncore.c to not print anything when we detect
unclaimed registers, we won't get into the console_unlock() infinite
loop and the driver unbind will work just fine. We will still be
getting interrupts every time vgacon touches those registers, but we
will survive. This is a valid experiment, but IMHO it's not the real
fix: if we don't print any error messages we will still keep getting
the interrupts, and if we disable ERR_INT we won't get the interrupt
anymore, but we will also stop getting all the other error interrupts.

I talked about this problem with the HW engineer and his
recommendation is "So don't do any VGA I/O or memory access while the
power well is disabled, and make to re-program MSR after enabling the
power well and before using VGA I/O or memory accesses.".

Notice that this is just a partial fix to fd.o #67813. This fixes the
case where the power well is already enabled when we unbind, not when
it's disabled when we unbind.

Guenter Roeck 3d489ac07e hwmon: (lm90) Unregister hwmon device if interrupt setup fails
Commit 109b1283fb (hwmon: (lm90) Add support to handle IRQ) introduced
interrupt support. Its error handling code fails to unregister the already
registered hwmon device.

Yang Yingliang d55d282e6a sch_tbf: use do_div() for 64-bit divide
It's doing a 64-bit divide which is not supported
on 32-bit architectures in psched_ns_t2l(). The
correct way to do this is to use do_div().

It's introduced by commit cc106e441a
("net: sched: tbf: fix the calculation of max_size")

Eric Dumazet 9750223102 udp: ipv4: must add synchronization in udp_sk_rx_dst_set()
Unlike TCP, UDP input path does not hold the socket lock.

Before messing with sk->sk_rx_dst, we must use a spinlock, otherwise
multiple cpus could leak a refcount.

This patch also takes care of renewing a stale dst entry.
(When the sk->sk_rx_dst would not be used by IP early demux)

Maxime Ripard 6f97dc8d46 ARM: sun6i: dt: Fix interrupt trigger types
The Allwinner A31 uses the ARM GIC as its internal interrupts controller. The
GIC can work on several interrupt triggers, and the A31 was actually setting it
up to use a rising edge as a trigger, while it was actually a level high
trigger, leading to some interrupts that would be completely ignored if the
edge was missed.

Shawn Guo 2a82f95c3f MAINTAINERS: merge IMX6 entry into IMX
I have been co-maintaining IMX sub-architecture for a couple of years,
and collecting IMX sub-architecture patches rather than IMX6 only ones
for a few release cycles.  It makes sense to officially add myself as
the co-maintainer for IMX sub-architecture now.  Consequently, IMX6
entry can just be merged into IMX.

While at it, add a 'F:' entry for IMX DTS files.

Sergei Ianovich ff88b4724f ARM: pxa: prevent PXA270 occasional reboot freezes
Erratum 71 of PXA270M Processor Family Specification Update
(April 19, 2010) explains that watchdog reset time is just
8us insead of 10ms in EMTS.

If SDRAM is not reset, it causes memory bus congestion and
the device hangs. We put SDRAM in selfresh mode before watchdog
reset, removing potential freezes.

Without this patch PXA270-based ICP DAS LP-8x4x hangs after up to 40
reboots. With this patch it has successfully rebooted 500 times.
