Add support for USB serial interface provided by ViVOtech ViVOpay devices via
new driver vivopay-serial. Currently only the ViVOpay 8800 device is supported,
but support for similar devices can be added by adding the appropriate device
IDs to the driver.

Adding vbus_pin_inverted so that the usb detect pin can be active high
or low depending on HW implementation also replaced the
gpio_get_value(udc->vbus_pin); with a call to vbus_is_present(udc); This
allows the driver to be loaded and save about 0,15W on the consumption.

A bunch of USB gadget drivers where never ported from the linux 2.4
series to 2.6 kernels. However there's some code still in the tree for
them which isn't used and is probably untested for ages.
As the chance of these drivers being forward ported is probably quite
small now it might be time to get rid of them.

When the HDC driver writes the data to the transfer buffers it pollutes
the D-cache (unlike DMA drivers where the device writes the data). If
the corresponding pages get mapped into user space, there are no
additional cache flushing operations performed and this causes random
user space faults on architectures with separate I and D caches
(Harvard) or those with aliasing D-cache.

As Alan Cox have pinpointed the driver still required protection against
parallels calls to the config ioctl(). If lock is still necessary the use of
BKL is abused here. So replace BKL with a more convenient mutex.

The mass storage function responded needlessly to a set
configuration packet. This was a leftover from converting
gadget (file storage gadget) into a composite function.
Moreover, it has failed to respond to get max LUN request.
Adding request queueing made the function work better.

Adds a fallback which forces all LUNs ejection (including
non-removable and with prevent_medium_removal flag) when mass storage
function (MSF) worker thread exits and gadget fails to handle the
situation.
Previously, if thread_exits was not specified mass storage function
(MSF) did nothing when exiting thread as it's unclear for *function*
what to do when it's thread terminates so responsibility of handling
this situation was left to the *gadget* using the function.
The g_mass_storage handled the situation by unregistering itself (the
same thing that file storage gadget does). However, g_multi did
nothing and so MSF did not eject LUNs which prevented file system
unmounting.

All current Blackfin parts are using RTL v1.9, but they don't expose the
hardware registers to probe this dynamically. So hardcode the version to
v1.9 for now.
Need to move the local hwvers related defines higher up in the header so
that sub-musb headers may utilize them.

As reported by Antonio, there are cases where the ep->lock
can be taken twice, triggering a deadlock.
The typical sequence is :
irq_handler
\
-> gadget.complete()
\
-> pxa27x_udc.pxa_ep_queue() : ep->lock is taken
\
-> gadget.complete()
\
-> pxa27x_udc.pxa_ep_queue() : ep->lock is taken
==> *deadlock*
The patch fixes this by :
- releasing the lock each time gadget.complete() is called
- adding a check in handle_ep() to detect a recursive call,
in which case the function becomes on no-op.
The patch is still not good enough for ep0. For this unique
endpoint, another well thought over patch will be needed.

Add support for the serial port on devices based on the MosChip 7715,
which provides a serial and parallel port on a single usb interface.
This is added to the existing driver for the Moschip 7720 dual serial
port device. The 7715 is very similiar to the 7720, requiring only the
addition of a calc_num_ports() function, a separate interrupt-in
endpoint callback, and some manipulation of the port pointers added to
the attach() function to correct the fact that the usbserial core
erroneously assigns the first bulk in/out endpoint pair to the serial
port (the 7715 uses these for its parallel port). There is no support
for the 7715's parallel port yet.

Johan Hovold points out that get_string() is basically just a
re-implimentation of usb_string(). It is also buggy. It does DMA on
the stack and it doesn't handle negative returns from
usb_get_descriptor(). Plus unicode_to_ascii() is a rubbish function and
moving to usb_string() avoids using it.
Let's eliminate get_string() entirely.

The problem with Ethernet based networking devices is to clearly
identify what's their usage. Special interfaces like bridges, WiFi,
Bluetooth, WiMAX or WWAN are already using DEVTYPE identification.
This patch marks the Ethernet functions of gadgets from the device
type "gadget". Automatic setup of these interfaces can now happen
from userspace without the need of hardcoding the network interface
name.

The Barry project's userspace program, bcharge, can better handle this
device and functionality, and it also works with the latest phones,
which this driver does not support. So remove it, as the userspace code
should be used instead.

This driver had used BKL to guard against disconnect but
was incorrectly converted leaving an SMP race.
BKL was added to disconnect() to fix this race
BKL was removed from ioctl() as the mutex is sufficient
on its own.

The 4G XS Stick W14 seems to not understand RTS/DTR setting in
option_send_setup causing long timeouts on any open() which disturbs a
lot of well-known userspace applications like minicom or ModemManager.
Therefore, we enable OPTION_BLACKLIST_SENDSETUP blacklisting for it.

As suggested by Matthias Urlichs, this patch adds a somehow generic
mechanism for special handling of devices which don't support all bits
expected by this driver.
The blacklisting code is heavily stolen from sierra.c, but extended to
support different special cases.
For now, one case is implemented (OPTION_BLACKLIST_SENDSETUP), targeted
at the 4G W14 device: devices which don't understand the setting of
RTS/DTR in option_send_setup() causing a USB timeout of 5 s in any
userspace open() which leads to errors in most userspace applications.
In addition, I prepared another case for devices with interfaces which
shall not be accessed by this driver (targeted at the D-Link DWM 652).
However, OPTION_BLACKLIST_RESERVED_IF is not fully implemented yet as I
have no device to test this. Anyone volunteering to help here? If not,
I'll contact the guys who added D-Link DWM 652 support soon.

USB: Move hcd free_dev call into usb_disconnect
I found a way to oops the kernel:
1. Open a USB device through devio.
2. Remove the hcd module in the host kernel.
3. Close the devio file descriptor.
The problem is that closing the file descriptor does usb_release_dev
as it is the last reference. usb_release_dev then tries to invoke
the hcd free_dev function (or rather dereferencing the hcd driver
struct). This causes an oops as the hcd driver has already been
unloaded so the struct is gone.
This patch tries to fix this by bringing the free_dev call earlier
and into usb_disconnect. I have verified that repeating the
above steps no longer crashes with this patch applied.

This occurs because an interrupt scans uhci->skelqh, which is
being freed. We do the right thing: disable the interrupts in the
device, and do not do any processing if the interrupt is shared
with other source, but it's possible that another CPU gets
delayed somewhere (e.g. loops) until we started freeing.
The agreed-upon solution is to wait for interrupts to play out
before proceeding. No other bareers are neceesary.
A backport of this patch was tested on a 2.6.18 based kernel.
Testing of 2.6.32-based kernels is under way, but it takes us
forever (months) to turn this around. So I think it's a good
patch and we should keep it.

This patch (as1329) converts the USB stack over to the PM core's
runtime PM framework. This involves numerous changes throughout
usbcore, especially to hub.c and driver.c. Perhaps the most notable
change is that CONFIG_USB_SUSPEND now depends on CONFIG_PM_RUNTIME
instead of CONFIG_PM.
Several fields in the usb_device and usb_interface structures are no
longer needed. Some code which used to depend on CONFIG_USB_PM now
depends on CONFIG_USB_SUSPEND (requiring some rearrangement of header
files).
The only visible change in behavior should be that following a system
sleep (resume from RAM or resume from hibernation), autosuspended USB
devices will be resumed just like everything else. They won't remain
suspended. But if they aren't in use then they will naturally
autosuspend again in a few seconds.

This patch (as1327) changes the way negative autosuspend delays
prevent device from autosuspending. The current code checks for
negative values explicitly in the autosuspend_check() routine. The
updated code keeps things from getting that far by using
usb_autoresume_device() to increment the usage counter when a negative
delay is set, and by using usb_autosuspend_device() to decrement the
usage counter when a non-negative delay is set.
This complicates the set_autosuspend() attribute method code slightly,
but it will reduce the overall power management overhead.

This patch (as1326) adds usb_enable_autosuspend() and
usb_disable_autosuspend() routines for use by drivers. If a driver
knows that its device can handle suspends and resumes correctly, it
can enable autosuspend all by itself. This is equivalent to the user
writing "auto" to the device's power/level attribute.
The implementation differs slightly from what it used to be. Now
autosuspend is disabled simply by doing usb_autoresume_device() (to
increment the usage counter) and enabled by doing
usb_autosuspend_device() (to decrement the usage counter).
The set_level() attribute method is updated to use the new routines,
and the USB Power-Management documentation is updated.
The patch adds a usb_enable_autosuspend() call to the hub driver's
probe routine, allowing the special-case code for hubs in quirks.c to
be removed.

This patch (as1324) makes a small change to the code used for remote
wakeup of root hubs. hcd_resume_work() now calls the hub driver's
remote-wakeup routine instead of implementing its own version.
The patch is complicated by the need to rename remote_wakeup() to
usb_remote_wakeup(), make it non-static, and declare it in a header
file. There's also the additional complication required to make
everything work when CONFIG_PM isn't set; the do-nothing inline
routine had to be moved into the header file.

This patch (as1323) changes the locking requirements for
usb_autosuspend_device(), usb_autoresume_device(), and
usb_try_autosuspend_device(). This isn't a very important change;
mainly it's meant to make the locking more uniform.
The most tricky part of the patch involves changes to usbdev_open().
To avoid an ABBA locking problem, it was necessary to reduce the
region protected by usbfs_mutex. Since that mutex now protects only
against simultaneous open and remove, this posed no difficulty -- its
scope was larger than necessary.
And it turns out that usbfs_mutex is no longer needed in
usbdev_release() at all. The list of usbfs "ps" structures is now
protected by the device lock instead of by usbfs_mutex.

USB devices do not have to sort interfaces in their descriptors based on
the interface number, and they may choose to skip interface numbers. The
USB bandwidth allocation code for installing a new configuration assumes
the for loop variable will match the interface number. Make it use the
interface number (bInterfaceNumber) in the descriptor instead.

The original code was passing a stack variable as a dma buffer, so I
made it an allocated variable. Instead of adding a bunch of kfree()
calls, I changed all the error return paths to gotos.
Also I noticed that the error checking wasn't correct because
usb_get_descriptor() can return negative values.
While I was at it, I made an unrelated white space change by moving
the unicode_to_ascii() on to one line.

OMAP3 has three HS USB ports so it can have three different regulator
for each PHY connected to each port.
Currently these regulators are assumed to be optional and driver doesn't
fail but continue with the initialization if it doesn't get any regulators.
Regulator supply names has to be mapped in board files as 'hsusbN' where
'N' is port number and can be {0, 1 ,2}.

Some of the board might use external Vbus power supply on musb
interface which would require to program ULPI_BUSCONTROL register.
Adding 'extvbus' flag which can be set from such boards which will
be checked at musb driver files before programming ULPI_BUSCONTROL.

This patch fixes the Tx abort/teardown logic. We now wait for the teardown
completion interrupt and acknowledge the same by setting the tx_complete
register to 0.
This change is needed to ensure that abort processing works on DM365 platform.
Without this change after completion of abort processing the system is
overwhelmed with continuous stream of abort interrupts.
This change has been tested on all CPPI3.x platforms (DM644x, DM646x, DM35x,
DM36x).

This patch implements the work around for a Mentor controller related
bug where it's observed a BULK Tx toggle error on the bus when a
BULK IO gets scheduled on an endpoint that was earlier used for
handling ISOC transaction and needed to start on 1 toggle. When such
a situation arises even if the TXCSR toggle bits are programmed
correctly by the musb driver the data gets transmitted with 0 toggle
which leads to toggle error on the bus and the BULK transaction fails.
In case of MSC write, the device gets reset by the Host.
This Mentor bug is observed on almost all Mentor versions (1.3, 1.5,
1.8). Confirmed on DM644x, DM355, DM365, OMAPL13x platforms.

MUSB DMA_INTR register may sometimes read zero when infact there
was a pending interrupt. Workaround this by reading the DMA_COUNT
values for all enabled channels when this condition occurs.
Flag these channels as the ones needing to be serviced.
Additionally, the absence of a debug print meant we would never
catch a spurious DMA interrupt in MUSB. So this patch adds a
debug print in the IRQ handler.

We have observed MSC data read corruption when USB LAN device is
also connected and it's interface is up.
Silicon team has confirmed an errata where in all the active
transfers should use FIFO space either in first 8K or next 8K.
So far we have observed the issue in above use case scenario.
As a workaround to it, adding a new FIFO config (5) fitting well
within first 8K which can be used for such use cases.

Return values are being initialised to zero only to be unconditionally
assigned to a few instructions later. This may give the impression that
zero is returned on success, which is not the case.

Currently a non-root-hub USB device's wakeup settings are initialized when the
device is set to a configured state using device_init_wakeup(), but this is not
correct as wakeup is split into "capable" (can_wakeup) and "enabled"
(should_wakeup). The settings should be initialized instead in the device
initialization (usb_new_device) with the "capable" setting disabled and the
"enabled" setting enabled. The "capable" setting should be set based on the
device being configured or unconfigured, and "enabled" setting set based on
the sysfs power/wakeup control.
This patch retains the sysfs power/wakeup setting of a non-root-hub USB device
over a USB device re-configuration, which can happen (for example) after a
suspend/resume cycle.

The pbLua console port is known to not be a modem, so it is
unnecessary to be told this when it is plugged in.
Add NOT_A_MODEM quirk to tell the driver that we know this already

The pbLua firmware (http://pblua.com/) for the Lego Mindstorms NXT
provides a CDC ACM port for it's serial console.
This used to be detected automatically, but this support has been
dropped, probably for sensible reasons.
Explicitly add support for this device by adding an item to the
device ID table.

Due to a simple oversight when bus zero was added, the text API fails to
deliver the bus number in 'E' messages (which are equivalent of 'C'
messages, only for error case). This makes it harder, for instance,
use a search-and-highlight in a text editor. So fix it.
Also, Alan Stern requested adding timestamps to 'E' messages. This is
purely cosmetic, but makes it easier to read the trace. This is done
for both text and binary APIs.

Normally, the musb uses ep1 as the bidirectional bulk endpoint. This won't
work on the Blackfin musb as all endpoints (except ep0) are unidirectional.
Further, ep1-ep4 have a small 128 byte FIFO which makes them undesirable
for bulk endpoints (which need more like a 512 byte FIFO). This leaves us
with ep5-ep7 which have 1024 byte FIFOs and can be configured as either
in/out and bulk/interrupt/iso on the fly.

baud rates craps out on throughput, it works fine with lower ones.
While it'd be nice to simply use a lower baud rate, not all devices
can be configured this way, and it is possible to (slowly) interact
at higher rates by sending a byte at a time. So let people force
higher rates when they need it via a module parameter.

Some devices which use mode switching revert to their
primary mode as they are reset. They must not be reset for
error handling. As user spaces makes the switch it also
has to tell the kernel that a device is quirky.

Some devices must be switched to a new mode to fully use them.
A reset would make them revert to the old mode. Therefore a reset

An incorrect sizeof() resulted in only 4 (or 8) octets of the CHID being
checked instead of all 16 octets. A randomly generated CHID had a
probability of being unable to start a WUSB host of less than 1 in
2 billion.

EHCI FSL controller preserve its state during sleep mode, so nothing
fancy needs to be done.
Though, during 'deep sleep' mode (as found in MPC831x CPUs) the
controller turns off and needs to be reinitialized upon resume.
This patch adds support for hibernation and resuming after deep sleep.
Based on Dave Liu and Jerry Huang's work[1].

This patch fix audio record functionality for some Full speed sound blaster devices.
Issue: Sometimes transaction complete indication is coming from HW one frame later.
Solution: If scan_periodic process now frame or previous frame now-1 and sitd transaction
is not finished yet, exit scan_periodic function and check the same transaction in the next frame.

Add a new host controller driver method, reset_device(), that the USB core
will use to notify the host of a successful device reset. The call may
fail due to out-of-memory errors; attempt the port reset sequence again if
that happens. Update hub_port_init() to allow resetting a configured
device.

When a USB device is reset, the xHCI hardware must know, in order to match
the device state and disable all endpoints except control endpoint 0.
Issue a Reset Device command after a USB device is successfully reset.
Wait on the command to finish, and then cache or free the disabled
endpoint rings.
There are four different USB device states that the xHCI hardware tracks:
- disabled/enabled - device connection has just been detected,
- default - the device has been reset and has an address of 0,
- addressed - the device has a non-zero address but no configuration has
been set,
- configured - a set configuration succeeded.
The USB core may issue a port reset when a device is in any state, but the
Reset Device command will fail for a 0.96 xHC if the device is not in the
addressed or configured state. Don't consider this failure as an error,
but don't free any endpoint rings if this command fails.
A storage driver may request that the USB device be reset during error
handling, so use GPF_NOIO instead of GPF_KERNEL while allocating memory
for the Reset Device command.

Add the hub emulation code to allow ports on an xHCI root hub to be
disabled. Add the code to clear the port enabled/disabled bit, and clear
the port enabled/disabled change bit. Like EHCI, the port cannot be
enabled by setting the port enabled/disabled bit. Instead, a port is
enabled by the host controller after a reset.

Refactor the code to clear the port change bits in the port status
register. All port status change bits are write one to clear.
Remove a redundant port status read that was supposed to unblock any
posted writes. We read the port after the write to get the updated status
for debugging, so the port read after that is unnecessary.

All commands that can be issued to the xHCI hardware can come back with
vendor-specific "informational" completion codes. These are to be treated
like a successful completion code. Refactor out the code to test for the
range of these codes and print debugging messages.

The xhci_command structure is the basic structure for issuing commands to
the xHCI hardware. It contains a struct completion (so that the issuing
function can wait on the command), command status, and a input context
that is used to pass information to the hardware. Not all commands need
the input context, so make it optional to allocate. Allow
xhci_free_container_ctx() to be passed a NULL input context, to make
freeing the xhci_command structure simple.

If we fail to queue an evaluate context command or a configure endpoint
command to the command ring in xhci_configure_endpoint(), we need to
remove the xhci_command structure from the device's command list before
returning. If the command is left on the command list, it will sit there
indefinitely, blocking commands submitted after this fails.

This patch (as1316) adds some error checking to usb_submit_urb().
It's conditional on CONFIG_USB_DEBUG, so it won't affect normal users.
The new check makes sure that the actual type of the endpoint
described by urb->pipe agrees with the type encoded in the pipe value.
The USB error code documentation is updated to include the code
returned by the new check, and the usbfs SUBMITURB handler is updated
to use the correct pipe type when legacy user code tries to submit a
bulk transfer to an interrupt endpoint.

My distro kernel (Fedora Rawhide) started throwing warnings from DMA API
checker, so I have no choice but band-aid it quick. There's no attempt
to reuse DMA buffers. Control messages are only sent rarely anyway.

On various mxc boards, the intial ULPI reads resulted in a timeout
which prevented the transceiver to be identified and thus the ehci
device to be probed.
Initializing the hardware lines connected to the transceiver (through
pdata->init call) before actually enabling clocks and configuring
registers in the devices fixes this problem.

Latency timeout was read but never stored on port probe. When
ASYNC_LOW_LATENCY was cleared the device timeout would get set to 0

This has never worked properly because wsize passed to
cxacru_cm() is incorrectly set to the number of values
instead of the data bytes. The maximum number of values
that can be set at once is 7 which means the device will
not get enough data to work with and none of the
configuration values will be used.
At least one existing cxacru-cf.bin file contains invalid
data which will prevent the modem from syncing properly.
Fixing it is likely to break existing systems, and the
new sysfs interface for setting configuration parameters
can provide the same functionality. A script is provided
to convert from the original format.

The modem can be configured using CM_REQUEST_CARD_DATA_SET,
although CM_REQUEST_CARD_DATA_GET does not return any data.
Tested by setting the modulation (0x0a) option.
There is a list of parameters in the following archive,
but the meaning of many of them is not well documented:

This source also indicates that the highest parameter set
is 0x4a but this varies by model so an arbitrary limit of
0x7f has been used (the index is a 32-bit integer).

Firmware writing takes 256ms per 4KB with OHCI, which
is very slow compared to 7ms per 4KB with UHCI.
Until I have access to a hardware USB analyser it may
not be possible to determine why this happens.
Instead of appearing to do nothing, log progress when
writing firmware and then log the ATM device information
when finished. Remove an unnecessary 4 second delay.

It is possible for usb_get_intfdata() to return NULL if
sysfs is accessed while the module is being unloaded or
the device is being removed.
Move the access code to an inline function in usbatm.h,
and return -ENODEV if any of the pointers are NULL.
It should not be possible for the instance data or atm
device to be invalid until after unbind() completes and
the sysfs attributes have been removed.

When attempting to read data that is not actually
an array of values, the length may be negative
which causes an Oops due to a likely access off
the end of the data array.
This bug should not occur under normal use unless
the device returns an invalid response.

Set power.async_suspend for USB devices, endpoints and interfaces,
allowing them to be suspended and resumed asynchronously during
system sleep transitions.
The power.async_suspend flag is also set for devices that don't have
suspend or resume callbacks, because otherwise they would make the
main suspend/resume thread wait for their "asynchronous" children
(during suspend) or parents (during resume), effectively negating the
possible gains from executing these devices' suspend and resume
callbacks asynchronously.

This patch (as1331) adds non-tree ordering constraints needed for
proper resume of PCI USB host controllers from hibernation. The main
issue is that non-high-speed devices must not be resumed before the
high-speed root hub, because it is the ehci_bus_resume() routine which
takes care of handing the device connection over to the companion
controller. If the device resume is attempted before the handover
then the device won't be found and it will be treated as though it had
disconnected.
The patch adds a new field to the usb_bus structure; for each
full/low-speed bus this field will contain a pointer to the companion
high-speed bus (if one exists). It is used during normal device
resume; if the hs_companion pointer isn't NULL then we wait for the
root-hub device on the hs_companion bus.
A secondary issue is that an EHCI controlller shouldn't be resumed
before any of its companions. On some machines I have observed
handovers failing if the companion controller is reinitialized after
the handover. Thus, the EHCI resume routine must wait for the
companion controllers to be resumed.
The patch also fixes a small bug in usb_hcd_pci_probe(); an error path
jumps to the wrong label, causing a memory leak.

The five-second delay can be rather annoying, and makes the system
appear much less responsive when you connect a USB drive.
It's also not entirely clear that it is needed - the settling delay has
at least historically been an issue on some Apple iPods, for example,
and some devices have been reported to need even more than the old 5s
delay.
But before we penalize them all, let's see how bad it really is. Some
of the reasons for long delays seem to be actual historical kernel bugs
that should probably never have been papered over with a delay in the
first place (there's a Ubuntu bug report for 2.6.20 about a NULL pointer
dereference unless 'delay_use' is 8 or more, for example).
It also looks like some distros have already shipped with delay_use=0,
so the five second default may well be totally historical.
In other words: "Let's see if anybody screams".

The block layer calling convention is blk_queue_<limit name>.
blk_queue_max_sectors predates this practice, leading to some confusion.
Rename the function to appropriately reflect that its intended use is to
set max_hw_sectors.
Also introduce a temporary wrapper for backwards compability. This can
be removed after the merge window is closed.

part of version 1.0 of the USB audio standard. Follow these changes

eem_wrap() is sending a sentinel CRC, but it didn't indicate that to
the host, it should zero bit 14 (bmCRC) in the EEM packet header,
instead of setting it.

With CONFIG_USB_ULPI=y, CONFIG_USB<=m, CONFIG_PCI=n and
CONFIG_USB_OTG_UTILS=n, which is the default used for mx31moboard,
the build for all mx3 platforms fails because drivers/usb/otg/ulpi.c
where otg_ulpi_create is defined is not compiled.

g_multi used CONFIG_USB_ETH_RNDIS to check if RNDIS option was requested
where it should check for CONFIG_USB_G_MULTI_RNDIS. As a result, RNDIS
was never present in g_multi regardless of configuration.

This patch adds support for Dell Computer Corp. Wireless 5720 VZW Mobile
Broadband (EVDO Rev-A) Minicard GPS Port. I stole the name from lsusb,
but my card does not have a GPS on it (at least not that I can make
function). I'm sure the patch is whitespace damaged but the one line
addition should be fairly straightforward nonetheless.

added new device pid (PAPOUCH_AD4USB_PID) to ftdi_sio.h and ftdi_sio.c
AD4USB measuring converter is a 4-input A/D converter which enables the
user to measure to four current inputs ranging from 0(4) to 20 mA or
voltage between 0 and 10 V. The measured values are then transferred to
a superior system in digital form. The AD4USB communicates via USB.

I notice that the processcompl_compat() function seems to be leaking the
'struct async *as' in the error paths.
I think that the calling convention is fundamentally buggered. The
caller is the one that did the "reap_as()" to get the as thing, the
caller should be the one to free it too.
Freeing it in the caller also means that it very clearly always gets
freed, and avoids the need for any "free in the error case too".

the whole kernel buffer, which can contain "stale" data.

The current root hub polling code exhibits a spinlock recursion on the
private controller lock. r8a66597_root_hub_control() is called from
r8a66597_timer() which grabs the lock and disables IRQs. The following
chain emerges:

usb_hcd_poll_rh_status() <-- acquires the same lock

The entire chain requires IRQs to be disabled, so we just unlock and

This implements the same D-cache flushing logic for r8a66597-hcd as
Catalin's isp1760 (http://patchwork.kernel.org/patch/76391/) change,
with the same note applying here as well:
When the HDC driver writes the data to the transfer buffers it
pollutes the D-cache (unlike DMA drivers where the device writes
the data). If the corresponding pages get mapped into user space,
there are no additional cache flushing operations performed and
this causes random user space faults on architectures with
separate I and D caches (Harvard) or those with aliasing D-cache.
This fixes up crashes during USB boot on SH7724 and others:

This patch improves disable_controller() in the r8a66597-hdc
driver to disable all interrupts and clear status flags. It
also makes sure that disable_controller() is called during
probe(). This fixes the relatively rare case of unexpected
pending interrupts after kexec reboot.

Resolves kernel.org bug 14914.
Remove entry for 2770:915d (usb digital camera with mass storage
support) from unusual_devs.h. The fix triggered by the entry causes
the file system on the camera to be completely inaccessible (no
partition table, the device is not mountable).
The patch works, but let me clarify a few things about it. All the
patch does is remove the entry for this device from the
drivers/usb/storage/unusual_devs.h, which is supposed to help with a
problem with the device's reported size (I think). I'm pretty sure it
was originally added for a reason, so I'm not sure removing it won't
cause other problems to reappear. Also, I should note that this
unusual_devs.h entry was present (and activating workarounds) in
2.6.29, but in that version everything works fine. Starting with
2.6.30, things no longer work.

The problem is caused by a mismatch between the USB core's view of the
device state and the USB device and xHCI host's view of the device state.
After the device reset and re-configuration, the device and the xHCI host
think they are using alternate setting 0 of all interfaces. However, the
USB core keeps track of the old state, which may include non-zero
alternate settings. It uses intf->cur_altsetting to keep the endpoint
sysfs files for the old state across the reset.
The bandwidth allocation functions need to know what the xHCI host thinks
the current alternate settings are, so original patch set
intf->cur_altsetting to the alternate setting 0. This caused duplicate
endpoint files to be created.
The solution is to not set intf->cur_altsetting before calling
usb_set_interface() in usb_reset_and_verify_device(). Instead, we add a
new flag to struct usb_interface to tell usb_hcd_alloc_bandwidth() to use
alternate setting 0 as the currently installed alternate setting.

This patch (as1330) fixes a bug in khbud's handling of remote
wakeups. When a device sends a remote-wakeup request, the parent hub
(or the host controller driver, for directly attached devices) begins
the resume sequence and notifies khubd when the sequence finishes. At
this point the port's SUSPEND feature is automatically turned off.
However the device needs an additional 10-ms resume-recovery time
(TRSMRCY in the USB spec). Khubd does not wait for this delay if the
SUSPEND feature is off, and as a result some devices fail to behave
properly following a remote wakeup. This patch adds the missing
delay to the remote-wakeup path.
It also extends the resume-signalling delay used by ehci-hcd and
uhci-hcd from 20 ms (the value in the spec) to 25 ms (the value we use
for non-remote-wakeup resumes). The extra time appears to help some
devices.

This patch (as1321) fixes a problem with EHCI and UHCI root-hub
suspends: If the suspend occurs while a port is trying to resume, the
resume doesn't finish and simply gets lost. When remote wakeup is
enabled, this is undesirable behavior.
The patch checks first to see if any port resumes are in progress, and
if they are then it fails the root-hub suspend with -EBUSY.

This patch (as1320) fixes two problems related to interrupt-URB
scheduling in ehci-hcd.
URBs with an interval of 2 or 4 microframes aren't handled.
For the time being, the patch reduces to interval to 1 uframe.
URBs are constrained to have an interval no larger than 1024
frames by usb_submit_urb(). But some EHCI controllers allow
use of a schedule as short as 256 frames; for these
controllers we may have to decrease the interval to the
actual schedule length.
The second problem isn't very significant since few devices expose
interrupt endpoints with an interval larger than 256 frames. But the
first problem is critical; it will prevent the kernel from working
with devices having interrupt intervals of 2 or 4 uframes.

Memory allocations with GFP_KERNEL can cause IO to a storage
device which can fail resulting in a need to reset the device.
Therefore GFP_KERNEL cannot be safely used between usb_lock_device()

brightness status is reported by the Apple Cinema Displays as an
'unsigned char' (u8) value, but the code used 'char' instead.
Note that he driver was developed on the PowerPC architecture,
where the two types are synonymous, which is not always the case.
Fixed that. Otherwise the driver will interpret brightness
levels > 127 as negative, and fail to load.

Some of these workarounds are already in place, but labeled as affecting
all BF52x parts. Since we have official anomaly numbers now, use those
defines. And since writing to the FIFO has a similar hang issue as reading
from the FIFO, implement the workaround there too when necessary.

Buffer overflow, array index of 'aInfo' may be out of
bounds. Array 'aInfo' of size 78 may use index value(s) 6..84
The data stored in 'aInfo' array exceeds the array size of 78.
This patch increases the size of this array to hold the string
correctly without any memory corruption.

Race condition exists between the cppi_interrupt handler and
davinci_interrupt handler w.r.t completing a TX IO. Since DM646x
has seperate DMA and USB endpoint interrupts cppi_interrupt handler
needs to hold the lock while operating on the endpoint.
Update over previous patch to avoid taking the lock if already
taken. Tested on DM644x, DM355 and DM646x platforms.

When registering gadget driver, the state of the transceiver
must be set from undefined (no gadget) to b_idle.
Module unload sets the transceiver state to undefined state.
After the first load/unload pair, the reset irq will be lost.

This is a (almost) sort-only patch to sort FTDI device
product ID definitions in new ftdi_sio_ids.h header.
Advantage is that new device ID submissions will now have a specific (sorted)
position - less future merge conflicts.
Compile-tested, based on _current_ mainline git.
Minor checkpatch.pl warnings were eliminated whereever it made sense,
very minor text changes.

This is a strictly move-only patch to relocate all FTDI device
product ID definitions to their own ftdi_sio_ids.h header
(following the usual *_ids.h kernel tree convention, too),
thus correcting the slightly too messy appearance
(crucial driver defines were stuck somewhere in the decaying middle swamp
of the huge existing header).
Compile-tested, based on latest mainline git.

While converting emi62 to use request_firmware(), the driver was also
changed to use the ihex helper functions. However, this broke the loading
of the FPGA firmware because the code tries to access the addr field of
the EOF record which works with a plain array that has an empty last
record but not with the ihex helper functions where the end of the data is
signaled with a NULL record pointer, resulting in:

This can be fixed by changing the loop condition to test the return value
of ihex_next_binrec() directly (like in emi26.c).

This patch corrects a problem where drivers/usb/otg is linked twice
if CONFIG_USB_ULPI is selected, resulting in a build error (symbol
conflict). The files in that directory are properly linked already
as part of CONFIG_USB, and need not be indicated specifically for
CONFIG_USB_ULPI.

This patch (as1315) fixes some bugs in the USB core authorization
code:
usb_deauthorize_device() should deallocate the device strings
instead of leaking them, and it should invoke
usb_destroy_configuration() (which does proper reference
counting) instead of freeing the config information directly.
usb_authorize_device() shouldn't change the device strings
until it knows that the authorization will succeed, and it should
autosuspend the device at the end (having autoresumed the
device at the start).
Because the device strings can be changed, the sysfs routines
to display the strings must protect the string pointers by
locking the device.

This patch (as1314) renames usb_configure_device() and
usb_configure_device_otg() in the hub driver. Neither name is
appropriate because these routines enumerate devices, they don't
configure them. That's handled by usb_choose_configuration() and
usb_set_configuration().

tree do not actually use a spinlock, so the few exceptions now have to
call kfifo_{get,put}_locked, which takes an extra argument to a
spinlock.

This is a new generic kernel FIFO implementation.
The current kernel fifo API is not very widely used, because it has to
many constrains. Only 17 files in the current 2.6.31-rc5 used it.
FIFO's are like list's a very basic thing and a kfifo API which handles
the most use case would save a lot of development time and memory
resources.
I think this are the reasons why kfifo is not in use:
- The API is to simple, important functions are missing
- A fifo can be only allocated dynamically
- There is a requirement of a spinlock whether you need it or not
- There is no support for data records inside a fifo
So I decided to extend the kfifo in a more generic way without blowing up
the API to much. The new API has the following benefits:
- Generic usage: For kernel internal use and/or device driver.
- Provide an API for the most use case.
- Slim API: The whole API provides 25 functions.
- Linux style habit.
- DECLARE_KFIFO, DEFINE_KFIFO and INIT_KFIFO Macros
- Direct copy_to_user from the fifo and copy_from_user into the fifo.
- The kfifo itself is an in place member of the using data structure, this save an
indirection access and does not waste the kernel allocator.
- Lockless access: if only one reader and one writer is active on the fifo,
which is the common use case, no additional locking is necessary.
- Remove spinlock - give the user the freedom of choice what kind of locking to use if
one is required.
- Ability to handle records. Three type of records are supported:
- Variable length records between 0-255 bytes, with a record size
field of 1 bytes.
- Variable length records between 0-65535 bytes, with a record size
field of 2 bytes.
- Fixed size records, which no record size field.
- Preserve memory resource.
- Performance!
- Easy to use!
This patch:
Since most users want to have the kfifo as part of another object,
reorganize the code to allow including struct kfifo in another data
structure. This requires changing the kfifo_alloc and kfifo_init
prototypes so that we pass an existing kfifo pointer into them. This
patch changes the implementation and all existing users.

USB drivers that create character devices call usb_register_dev in their
probe function. This associates the usb_interface device with that minor
number and creates the character device and announces it to the world.
However, the driver's probe function is called before the new
usb_interface is added to the driver's klist_devices.
This is a problem because userspace will respond to the character device
creation announcement by opening the character device. The driver's open
function will the call usb_find_interface to find the usb_interface
associated with that minor number. usb_find_interface will walk the
driver's list of devices and find the usb_interface with the matching
minor number.
Because the announcement happens before the usb_interface is added to the
driver's klist_devices, a race condition exists. A straightforward fix
is to walk the list of devices on usb_bus_type instead since the device
is added to that list before the announcement occurs.
bus_find_device calls get_device to bump the reference count on the found
device. It is arguable that the reference count should be dropped by the
caller of usb_find_interface instead of usb_find_interface, however,
the current users of usb_find_interface do not expect this.
The original version of this patch only matched against minor number
instead of driver and minor number. This version matches against both.

The upcoming TWL6030 is companion chip for OMAP4 like the current TWL4030
for OMAP3. The common modules like RTC, Regulator creates opportunity
to re-use the most of the code from twl4030.

As Alan Stern pointed out - now we have tty_port_open the shutdown method
and locking allow us to whack the other bits into the full helper methods
and provide a shutdown op which the tty port code will synchronize with
setup for us.

If g_ether and g_multi are both built CONFIG_USB_ETH_RNDIS symbol
may be redefined in the later and, whats even worse, g_ether's settings
may affect g_multi's. This adds a USB_ETH_RNDIS symbol defined at the
beginning of ether.c and multi.c according toproper KConfig settings.

Make the USB core check the bandwidth when switching from one
interface alternate setting to another. Also check the bandwidth
when resetting a configuration (so that alt setting 0 is used). If
this check fails, the device's state is unchanged. If the device
refuses the new alt setting, re-instate the old alt setting in the
host controller hardware.
If a USB device doesn't have an alternate interface setting 0, install
the first alt setting in its descriptors when a new configuration is
requested, or the device is reset.
Add a mutex per root hub to protect bandwidth operations:
adding/reseting/changing configurations, and changing alternate interface
settings. We want to ensure that the xHCI host controller and the USB
device are set up for the same configurations and alternate settings.
There are two (possibly three) steps to do this:
1. The host controller needs to check that bandwidth is available for a
different setting, by issuing and waiting for a configure endpoint
command.
2. Once that returns successfully, a control message is sent to the
device.
3. If that fails, the host controller must be notified through another
configure endpoint command.
The mutex is used to make these three operations seem atomic, to prevent
another driver from using more bandwidth for a different device while
we're in the middle of these operations.
While we're touching the bandwidth code, rename usb_hcd_check_bandwidth()
to usb_hcd_alloc_bandwidth(). This function does more than just check
that the bandwidth change won't exceed the bus bandwidth; it actually
changes the bandwidth configuration in the xHCI host controller.

Refactor out the code to find alternate interface settings into
usb_find_alt_setting(). Print a debugging message and return null if the
alt setting is not found.
While we're at it, correct a bug in the refactored code. The interfaces
in the configuration's interface cache are not necessarily in numerical
order, so we can't just use the interface number as an array index. Loop
through the interface caches, looking for the correct interface.

The xHCI driver issues a Configure Endpoint command for two reasons:
- a new configuration or alternate interface setting is selected
- a quirky Fresco Logic prototype requires the command after a Reset
Endpoint command.
The xHCI driver only waits on the command in the first case.
When a configure endpoint command completes, the driver needs to know why
the command was generated. When the driver only supported selecting an
initial configuration, the check was simple. Unfortunately that check
doesn't work now that the driver supports alternate interfaces. If an
endpoint must be dropped (because it's not in the new alternate setting)
and no new endpoints are added, the math involving
xhci_last_valid_endpoint() will assign -1 to an unsigned integer and cause
an out-of-bounds array access.
Move the check for the quirky hardware sooner and avoid the bad array
access.

When a driver wants to switch to a different alternate setting for an
interface, the USB core will (soon) check whether there is enough
bandwidth. Once the new alternate setting is installed in the xHCI
hardware, the USB core will send a USB_REQ_SET_INTERFACE control
message. That can fail in various ways, and the USB core needs to be
able to reinstate the old alternate setting.
With the old code, reinstating the old alt setting could fail if the
there's not enough memory to allocate new endpoint rings. Keep
around a cache of (at most 31) endpoint rings for this case. When we
successfully switch the xHCI hardware to the new alt setting, the old
alt setting's rings will be stored in the cache. Therefore we'll
always have enough rings to satisfy a conversion back to a previous
device setting.

Make the USB Test & Measurement driver use usb_clear_halt() instead of
usb_control_msg() to clear a stalled endpoint. This will allow devices to
be tested under an xHCI host controller. The endpoint stall will not be
cleared in the internal xHCI hardware state unless usb_clear_halt() is
used.

Accroding commit 0994375e, which is adding remove_id sysfs attr
for pci drivers, for management tools dynamically bind/unbind
a pci/usb devices to a specified drivers; with this patch,
the management tools can be simplied.
And the original code didn't handle the failure of
usb_create_newid_file, fixed in this patch.

When building qTDs (sTDs) from a scatter-gather list, the length of the
qTD must be a multiple of wMaxPacketSize if the transfer continues into
another qTD.
This also fixes a link failure on configurations for 32 bit processors
with 64 bit dma_addr_t (e.g., CONFIG_HIGHMEM_64G).

This patch (as1312) fixes a minor bug in usb-storage. The
fill_inquiry() routine neglects to pre-load the inquiry data buffer
with spaces. As a result, if the vendor name is shorter than 8
characters or the product name is shorter than 16, the remainder will
be filled with garbage.
The patch also removes some unnecessary calls to strlen().

This patch (as1311) fixes a problem in usb-storage: Some devices are
pretty broken when it comes to reporting sense data. The information
they send back indicates that they have more than 18 bytes of sense
data available, but when the system asks for more than 18 they fail or
hang. The symptom is that probing fails with multiple resets.
The patch adds a new BAD_SENSE flag to indicate that usb-storage
should never ask for more than 18 bytes of sense data. The flag can
be set in an unusual_devs entry or via the "quirks=" module parameter,
and it is set automatically whenever a REQUEST SENSE command for more
than 18 bytes fails or times out.
An unusual_devs entry is added for the Agfa photo frame, which uses a
Prolific chip having this bug.

This patch (as1303) revises the USB Power Management infrastructure to
make it compatible with the new driver-model Runtime PM framework:
Drivers are no longer allowed to access intf->pm_usage_cnt
directly; the PM framework manages its own usage counters.
usb_autopm_set_interface() is eliminated, because it directly
sets intf->pm_usage_cnt.
usb_autopm_enable() and usb_autopm_disable() are eliminated,
because they call usb_autopm_set_interface().
usb_autopm_get_interface_no_resume() and
usb_autopm_put_interface_no_suspend() are added. They
correspond to pm_runtime_get_noresume() and
pm_runtime_put_noidle() in the PM framework.
The power/level attribute no longer accepts "suspend", only
"on" and "auto". The PM framework doesn't allow devices to be
forced into a suspended mode.
The hub driver contains the only code that violates the new
guidelines. It is updated to use the new interface routines instead.

For TWL family of power management ICs, USB charging works only
when USB regulators are in enabled state: 3v1, 1v5, 1v8
On a USB cable attach, twl4030_phy_resume(twl) function enables
the regulators. Enable USB charging, only after all regulators
are enabled.
Its observed that enabling USB charging before regulators are
enabled, causes USB charging to fail.
Tested on: Zoom2: omap3430: ES3.1 + TWL5030
Needs T2-MADC and T2-BCI drivers which are still not upstreamed.

usb_bulk_msg() transfers only bytes up to the maximum packet size.
It must be repeated by the usbtmc driver until all bytes of a TMC message
are transfered.
Without this patch, ETIMEDOUT is reported when writing TMC messages
larger than the maximum USB bulk size and the transfer remains incomplete.
The user will notice that the device hangs and must be reset by either closing
the application or pulling the plug.

Commit a5073b5283 (musb_gadget: fix
unhandled endpoint 0 IRQs) somehow missed its key change:
"The gadget EP0 code routinely ignores an interrupt at end of
the data phase because of musb_g_ep0_giveback() resetting the
state machine to "idle, waiting for SETUP" phase prematurely."
So, the majority of the cases of unhandled IRQs is still unfixed...

MUSB: save hardware revision at init
This can be used later to flag workarounds for issues affecting
particular revisions. Saving this at init avoids having to

If we clear the interrupt pending bits at the end, we sometimes return too
fast and have the same interrupt assert itself. There is no way in a
Blackfin system to force a sync of this state, so the hardware manual
instructs people to clear interrupt flags early in their ISR.

The xHCI 0.95 and 0.96 specification defines several transfer buffer
request completion codes that indicate a USB transaction error occurred.
When a stall, babble, transaction, or split transaction error completion code
is set, the xHCI has halted that endpoint ring. Software must issue a
Reset Endpoint command and a Set Transfer Ring Dequeue Pointer command
to clean up the halted ring.
The USB device driver is supposed to call into usb_reset_endpoint() when
an endpoint stalls. That calls into the xHCI driver to issue the proper
commands. However, drivers don't call that function for the other
errors that cause the xHC to halt the endpoint ring. If a babble,
transaction, or split transaction error occurs, check if the endpoint
context reports a halted condition, and clean up the endpoint ring if it
does.

The transfer descriptor (TD) is a series of transfer request buffers
(TRBs) that describe the buffer pointer, length, and other
characteristics. The xHCI controllers want to know an estimate of how
long the TD is, for caching reasons. In each TRB, there is a "TD size"
field that provides a rough estimate of the remaining buffers to be
transmitted, including the buffer pointed to by that TRB.
The TD size is 5 bits long, and contains the remaining size in bytes,
right shifted by 10 bits. So a remaining TD size less than 1024 would get
a zero in the TD size field, and a remaining size greater than 32767 would
get 31 in the field.
This patches fixes a bug in the TD_REMAINDER macro that is triggered when
the URB has a scatter gather list with a size bigger than 32767 bytes.
Not all host controllers pay attention to the TD size field, so the bug
will not appear on all USB 3.0 hosts.

The Multifunction Composite Gadget has two configurations
consisting of Ethernet (RNDIS in first and CDC Ethernet in
second configuration), CDC Serial and File-backed Storage
functions.
When connected to a Windows host, the first configuration
is chosen thus gadget provides RNDIS Ethernet, serial and
mass storage whereas when connected to Linux host, second
configuration is chosen thus providing CDC Ethernet,
serial and mass storage.
Which configurations are built can be configured via
KConfig options.

This callback is called when MSF's thread exits (is terminated

Most of the data from fsg_dev have been moved to fsg_common
structure. The fsg_dev structure holds only endpoint dependent
data. The fsg_common structure has a fsg pointer which points
to active fsg_dev structure -- endpoints are referenced via this
pointer.
This fixes the problem of several threads created when a single
instance of MSF is used in several USB configurations.

A two fsg_config fields were added:
* lun_name_format which lets one specify format of a name
used when registering LUN devices. It is useful if there
would be ever need for two MSFs to be used in a single
composite gadget (as opposed to single MSF in two
configuration); and
* thread_name which lets one specify the name of a kernel
thread used by MSF. This is not required since two or more
threads can have the same name but nevertheless it's here
for consistency.

Removed all references to mod_data in f_mass_storage.c and
instead created fsg_config structure fsg_common_init() takes
as an argument -- it stores all configuration options that
were previously taken from mod_data.
Moreover, The fsg_config structure allows per-LUN
configuration of removable and CD-ROM emulation.
Module parameters are handled by defining an object of
fsg_module_parameters structure and then declaring module
parameters via FSG_MODULE_PARAMETERS() macro. It adds proper
declarations to the code making specified object be populated
from module parameters.
To use values stored there one may use either
fsg_config_from_params() which will will a fsg_config structure
with values taken from fsg_module_parameters structure or
fsg_common_from_params() which will initialise fsg_common
structure directly.

It's not surprising that the transfer request buffer (TRB) physical to
virtual address translation function has bugs in it, since I wrote most of
it at 4am last October. Add a test suite to check the TRB math. This
runs at memory initialization time, and causes the driver to fail to load
if the TRB math fails.
Please excuse the excessively long lines in the test vectors; they can't
really be made shorter and still be readable.

Add D-Link DWM-162-U5 device id 1e0e:ce16 into option driver. The device
has 4 interfaces, of which 1 is handled by storage and the other 3 by

USB drivers that create character devices call usb_register_dev in their
probe function. This associates the usb_interface device with that minor
number and creates the character device and announces it to the world.
However, the driver's probe function is called before the new
usb_interface is added to the driver's klist_devices.
This is a problem because userspace will respond to the character device
creation announcement by opening the character device. The driver's open
function will the call usb_find_interface to find the usb_interface
associated with that minor number. usb_find_interface will walk the
driver's list of devices and find the usb_interface with the matching
minor number.
Because the announcement happens before the usb_interface is added to the
driver's klist_devices, a race condition exists. A straightforward fix
is to walk the list of devices on usb_bus_type instead since the device

bus_find_device calls get_device to bump the reference count on the found
device. It is arguable that the reference count should be dropped by the
caller of usb_find_interface instead of usb_find_interface, however,
the current users of usb_find_interface do not expect this.

However in drivers/usb/host/isp1362-hcd.c:1103:

Nokia S60 phones expose two ACM channels. The first is a modem and is picked
up by the standard AT-command interface information in the CDC-ACM driver. The
second is marked as having a vendor-specific protocol. Normally, we don't
expose those as ttys. (On some other devices, they may be claimed by the
rndis_host driver and used as a network interface).
But on S60 this second ACM channel is the way that third-party S60 application
developers are expected to communicate over USB. It acts as a serial device
at the S60 end, and so it should on Linux too.

In map_urb_for_dma(), the DMA address returned by dma_map_single()
is not checked to determine if it is legal. This lack of checking
contributed to a problem with the libertas wireless driver
(http://marc.info/?l=linux-wireless&m=125695331205062&w=2). The
difficulty was not detected until the buffer was unmapped. By this time
memory corruption had occurred.
The situation is fixed by testing the returned DMA address, and
returning -EAGAIN if the address is invalid.

This patch (as1301) adds support to usbmon for scatter-gather URBs.
The text interface looks at only the first scatterlist element, since
it never copies more than 32 bytes of data anyway. The binary
interface copies as much data as possible up to the first
non-addressable buffer.

This patch deals with reducing the memory footprint for sierra driver.
This optimization is aimed for embedded software customers.
Some sierra modems can expose upwards of 7 USB interfaces, each possibly
offering different services. In general, interfaces used for the
exchange of wireless data require much higher throughput, hence require
more memory (i.e. more URBs) than lower performance interfaces. URBs
used for the IN direction are pre-allocated by the driver and this patch
introduces a way to configure the number of IN URBs allocated on a
per-interface basis. Interfaces with lower throughput requirements
receive fewer URBs, thereby reducing the RAM memory consumed by the
driver.
NOTE1: This driver has always pre-allocated URBs for the IN direction.
NOTE2: The number of URBs pre-allocated for the low-performance
interfaces has already been extensively tested in previous versions of
this driver.
We also added the capability to log function calls by adding DEBUG flag.
Please note that this flag is commented out because this is the default
state
for it.

Without Interface Association Descriptor, the CDC serial and
RNDIS functions did not work correctly when added to a
composite gadget with other functions. This is because, it
defined two interfaces and some hosts tried to treat each
interface separatelly.

Moved code initialising fsg_common structure to fsg_common_init()
function which is called from fsg_bind(). Moreover, changed
reference counting mechanism: fsg_common has a reference counter
which counts how many fsg_dev structures uses it. When this
reaches zero fsg_common_release() is run which unregisters
LUN devices and frees memory.
fsg_common_init() takes pointer to fsg_common structure as an
argument. If it is NULL function allocates storage otherwise
uses pointed to memory (handy if fsg_common is a field of another
structure or a static variable).
fsg_common_release() will free storage only if
free_storage_on_release is set -- it is initialised by
fsg_common_init(): set if allocation was done, unset
otherwise (one may overwrite it of course).

array of characters with predefined size. Since mass storage
function does not define changing buffer size on run-time it is

In the final version, many fsg_dev structures will (be able to)
refer to a single fsg_common structure and so it is required
to move common data to another object which can be shared.
Situation where many fsg_dev structures refer single fsg_common
structure is when a single instance of MSF is used in several
USB configurations.

Removed code that was included when CONFIG_USB_FILE_STORAGE_TEST
was defined. If this functionality is required one may still use
the original File-backed Storage Gadget. It has been agreed that
testing functionality is not required in the composite function.
Also removed fsg_suspend() and fsg_resume() which were no
operations.
Moreover, storage_common.c has been modified in such a way that
defining certain macros skips parts of the file. Those macros
are:
* FSG_NO_INTR_EP -- skips interrupt endpoint descriptors
* FSG_NO_DEVICE_STRINGS -- skips certain strings
* FSG_NO_OTG -- skips OTG descriptor

Since storage_common.c no longer references mod_data object
it is now possible to include it before mod_data object is
defined. This makes it possible to move some defines that
are used as default values of mod_data fields to be defined
in storage_common.c file (where they should be set from
the beginning).
Also, show_ro(), show_file(), store_ro() and store_file()
have been moved to storage_common.c with LUN's device's
drvdata changed from pointing to fsg_dev to pointing to
rw_semaphore (&fsg->filesem).

removable and cdrom flag has been added to the fsg_lun structure
removing any references to mod_data object from storage_common.c.
As of read-only flag, previously it was set if a read-only
backing file was specified (which is good) and remained set
even after the file has been closed (which may be considered an
issue). Currently, the initial read-only flag is preserved so
if it was unset each time file is opened code will try to open
it read-write even if previous file was opened read-only.

The Freescale MX27 and MX31 SoCs have a EHCI controller onboard.
The controller is capable of USB on the go. This patch adds
a driver to support all three of them.
Users have to pass details about serial interface configuration in the
platform data.
The USB OTG core used here is the ARC core, so the driver should
be renamed and probably be merged with ehci-fsl.c eventually.

facility. If an URB uses a scatterlist then it should not have the

This patch (as1297) adds a "remove" attribute to each USB device's
directory in sysfs. Writing to this attribute causes the device to be
deconfigured (the same as writing 0 to the "bConfigurationValue"
attribute) and then tells the hub driver to disable the device's
upstream port. The device remains locked during these activities so
there is no possibility of it getting reconfigured in between. The
port will remain disabled until after the device is unplugged.
The purpose of this is to provide a means for user programs to imitate
the "Safely remove hardware" applet in Windows. Some devices do
expect their ports to be disabled before they are unplugged, and they
provide visual feedback to users indicating when they can safely be
unplugged.
The security implications are minimal. Writing to the "remove"
attribute is no more dangerous than writing to the
"bConfigurationValue" attribute.

This patch (as1296) gets rid of the fixed DMA-buffer mapping used by
the hub driver for its status URB. This URB doesn't get used much --
mainly when a device is plugged in or unplugged -- so the dynamic
mapping overhead is minimal. And most systems have many fewer
external hubs than root hubs, which don't need a mapped buffer anyway.

The EHCI specification says that an EHCI host controller may cache part of
the isochronous schedule. The EHCI controller must advertise how much it
caches in the schedule through the HCCPARAMS register isochronous
scheduling threshold (IST) bits.
In theory, adding new iTDs within the IST should be harmless. The HW will
follow the old cached linked list and miss the new iTD. SW will notice HW
missed the iTD and return 0 for the transfer length.
However, Intel ICH9 chipsets (and some later chipsets) have issues when SW
attempts to schedule a split transaction within the IST. All transfers
will cease being sent out that port, and the drivers will see isochronous
packets complete with a length of zero. Start of frames may or may not
also disappear, causing the device to go into auto-suspend. This "bus
stall" will continue until a control or bulk transfer is queued to a
device under that roothub.
Most drivers will never cause this behavior, because they use multiple
URBs with multiple packets to keep the bus busy. If you limit the number
of URBs to one, you may be able to hit this bug.
Make sure the EHCI driver does not schedule full-speed transfers within
the IST under an Intel chipset. Make sure that when we fall behind the
current microframe plus IST, we schedule the new transfer at the next
periodic interval after the IST.
Don't change the scheduling for new transfers, since the schedule slop will
always be greater than the IST. Allow high speed isochronous transfers to
be scheduled within the IST, since this doesn't trigger the Intel chipset
bug.
Make sure that if the host caches the full frame, the EHCI driver's
internal isochronous threshold (ehci->i_thresh) is set to
8 microframes + 2 microframes wiggle room. This is similar to what is done in
the case where the host caches less than the full frame.

In order to giveback a canceled URB, we must ensure that the xHCI
hardware will not access the buffer in an URB. We can't modify the
buffer pointers on endpoint rings without issuing and waiting for a stop
endpoint command. Since URBs can be canceled in interrupt context, we
can't wait on that command. The old code trusted that the host
controller would respond to the command, and would giveback the URBs in
the event handler. If the hardware never responds to the stop endpoint
command, the URBs will never be completed, and we might hang the USB
subsystem.
Implement a watchdog timer that is spawned whenever a stop endpoint
command is queued. If a stop endpoint command event is found on the
event ring during an interrupt, we need to stop the watchdog timer with
del_timer(). Since del_timer() can fail if the timer is running and
waiting on the xHCI lock, we need a way to signal to the timer that
everything is fine and it should exit. If we simply clear
EP_HALT_PENDING, a new stop endpoint command could sneak in and set it
before the watchdog timer can grab the lock.
Instead we use a combination of the EP_HALT_PENDING flag and a counter
for the number of pending stop endpoint commands
(xhci_virt_ep->stop_cmds_pending). If we need to cancel the watchdog
timer and del_timer() succeeds, we decrement the number of pending stop
endpoint commands. If del_timer() fails, we leave the number of pending
stop endpoint commands alone. In either case, we clear the
EP_HALT_PENDING flag.
The timer will decrement the number of pending stop endpoint commands
once it obtains the lock. If the timer is the tail end of the last stop
endpoint command (xhci_virt_ep->stop_cmds_pending == 0), and the
endpoint's command is still pending (EP_HALT_PENDING is set), we assume
the host is dying. The watchdog timer will set XHCI_STATE_DYING, try to
halt the xHCI host, and give back all pending URBs.
Various other places in the driver need to check whether the xHCI host
is dying. If the interrupt handler ever notices, it should immediately
stop processing events. The URB enqueue function should also return
-ESHUTDOWN. The URB dequeue function should simply return the value
of usb_hcd_check_unlink_urb() and the watchdog timer will take care of
giving the URB back. When a device is disconnected, the xHCI hardware
structures should be freed without issuing a disable slot command (since
the hardware probably won't respond to it anyway). The debugging
polling loop should stop polling if the host is dying.
When a device is disconnected, any pending watchdog timers are killed
with del_timer_sync(). It must be synchronous so that the watchdog
timer doesn't attempt to access the freed endpoint structures.

xhci_quiesce() is basically a no-op right now. It's only called if
HC_IS_RUNNING() is true, and the body of the function consists of a
BUG_ON if HC_IS_RUNNING() is false. For the new xHCI watchdog timer, we
need a new function that clears the xHCI running bit in the command
register, but doesn't wait for the halt status to show up in the status
register. Re-purpose xhci_quiesce() to do that.

In the old code, there was a race condition between the stop endpoint
command and the URB submission process. When the stop endpoint command is
handled by the event handler, the endpoint ring is assumed to be stopped.
When a stop endpoint command is queued, URB submissions are to not ring
the doorbell. The old code would check the number of pending URBs to be
canceled, and would not ring the doorbell if it was non-zero.
However, the following race condition could occur with the old code:
1. Cancel an URB, add it to the list of URBs to be canceled, queue the stop
endpoint command, and increment ep->cancels_pending to 1.
2. The URB finishes on the HW, and an event is enqueued to the event ring
(at the same time as 1).
3. The stop endpoint command finishes, and the endpoint is halted. An
event is queued to the event ring.
4. The event handler sees the finished URB, notices it was to be
canceled, decrements ep->cancels_pending to 0, and removes it from the to
be canceled list.
5. The event handler drops the lock and gives back the URB. The
completion handler requeues the URB (or a different driver enqueues a new
URB). This causes the endpoint's doorbell to be rung, since
ep->cancels_pending == 0. The endpoint is now running.
6. A second URB is canceled, and it's added to the canceled list.
Since ep->cancels_pending == 0, a new stop endpoint command is queued, and
ep->cancels_pending is incremented to 1.
7. The event handler then sees the completed stop endpoint command. The
handler assumes the endpoint is stopped, but it isn't. It attempts to
move the dequeue pointer or change TDs to cancel the second URB, while the
hardware is actively accessing the endpoint ring.
To eliminate this race condition, a new endpoint state bit is introduced,
EP_HALT_PENDING. When this bit is set, a stop endpoint command has been
queued, and the command handler has not begun to process the URB
cancellation list yet. The endpoint doorbell should not be rung when this
is set. Set this when a stop endpoint command is queued, clear it when
the handler for that command runs, and check if it's set before ringing a
doorbell. ep->cancels_pending is eliminated, because it is no longer
used.
Make sure to ring the doorbell for an endpoint when the stop endpoint
command handler runs, even if the canceled URB list is empty. All
canceled URBs could have completed and new URBs could have been enqueued
without the doorbell being rung before the command was handled.

Control requests targeted at an endpoint (that is sent to EP0 but
specifying the target endpoint address in wIndex) are dispatched to the
current configuration's setup callback, requiring all gadget drivers to
dispatch the requests to the correct function driver.
To avoid this, record which endpoints are used by each function in the
composite driver SET CONFIGURATION handler and dispatch requests
targeted at endpoints to the correct function.

Support urbs with scatter-gather lists by trying to fit sg list elements
into page lists in one or more qTDs. qTDs must end on a wMaxPacketSize
boundary so if this isn't possible the urb's sg list must be copied into
bounce buffers.

The WHCI HCD will also support urbs with scatter-gather lists. Add a
usb_bus field to indicated how many sg list elements are supported by
the HCD. Use this to decide whether to pass the scatter-list to the HCD
or not.

Add bus glue driver for Xilinx USB host controller. The controller can be
configured as HS only or HS/FS hybrid. The driver uses the device tree file
to configure the driver according to the setting in the hardware system.
This driver has been tested with usbtest using the NET2280 PCI card.

Use dev_dbg() instead of an unconditional printk(KERN_DEBUG). This has
two benefits; one is that it identifies the USB device which the messages
related to, and the other is that the messages won't be produced unless
debug is turned on.

Unfortunately, one cannot hold on to the struct firmware
that request_firmware_nowait() hands off, which is needed
in some cases. Allow this by requiring the callback to
free it (via release_firmware).
Additionally, give it a gfp_t parameter -- all the current
users call it from a GFP_KERNEL context so the GFP_ATOMIC
isn't necessary. This also marks an API break which is
useful in a sense, although that is obviously not the
primary purpose of this change.

While Linux provided an O_SYNC flag basically since day 1, it took until
Linux 2.4.0-test12pre2 to actually get it implemented for filesystems,
since that day we had generic_osync_around with only minor changes and the
great "For now, when the user asks for O_SYNC, we'll actually give
O_DSYNC" comment. This patch intends to actually give us real O_SYNC
semantics in addition to the O_DSYNC semantics. After Jan's O_SYNC
patches which are required before this patch it's actually surprisingly
simple, we just need to figure out when to set the datasync flag to
vfs_fsync_range and when not.
This patch renames the existing O_SYNC flag to O_DSYNC while keeping it's
numerical value to keep binary compatibility, and adds a new real O_SYNC
flag. To guarantee backwards compatiblity it is defined as expanding to
both the O_DSYNC and the new additional binary flag (__O_SYNC) to make
sure we are backwards-compatible when compiled against the new headers.
This also means that all places that don't care about the differences can
just check O_DSYNC and get the right behaviour for O_SYNC, too - only
places that actuall care need to check __O_SYNC in addition. Drivers and
network filesystems have been updated in a fail safe way to always do the
full sync magic if O_DSYNC is set. The few places setting O_SYNC for
lower layers are kept that way for now to stay failsafe.
We enforce that O_DSYNC is set when __O_SYNC is set early in the open path
to make sure we always get these sane options.
Note that parisc really screwed up their headers as they already define a
O_DSYNC that has always been a no-op. We try to repair it by using it for
the new O_DSYNC and redefinining O_SYNC to send both the traditional
O_SYNC numerical value _and_ the O_DSYNC one.

This patch adds the vendor and device id for the Mobilcom Debitel UMTS surf
stick (a.k.a. 4G Systems XSStick W14, MobiData MBD-200HU, ...).
To see these ids, you need to switch the stick to modem operation first
with the help of usb_modeswitch. This makes it switch from 1c9e:f000 to
1c9e:9603 and thus be recognized by the option driver.

On tx channel abort a cppi interrupt is generated for a short time by
setting the lowest bit of the TCPPICOMPPTR register. It is then reset
immediately by clearing the bit. When the interrupt handler is run,
it does not detect an interrupt in the TCPPIMSKSR or RCPPIMSKSR
registers and thus exits early without writing the TCPPIEOIR register.
It appears that this inhibits further cppi interrupts until the handler
is called by chance, f.ex. from davinci_interrupt().
By moving the unmasking of the interrupt below the writes to
TCPPICOMPPTR, no interrupt is generated and no write to TCPPIEOIR is
necessary.

In gadget mode the answer to a control request should be followed by
a zero-length packet if the amount transferred is an exact multiple of
the endpoint's packet size and the requests has its "zero" flag set.
This patch prevents the request from being immediately removed from the
queue when a control IN transfer ends on a full packet and "zero" is set.
The next time ep0_txstate is entered, a zero-length packet is queued and
the request is removed as fifo_count is 0.

The use of urb->actual_length to update tx_outstanding_bytes
implicitly assumes that the number of bytes actually written is the
same as the number of bytes we tried to write. On error that
assumption is violated so just use transfer_buffer_length the number
of bytes we intended to write to the device.
If an error occurs we need to fall through and call
usb_serial_port_softint to wake up processes waiting in
tty_wait_until_sent.

The driver incorrectly cancels the mass-storage device CSW request
(which leads to device reset) due to giving back URB at the head of
endpoint's queue after sending each STALL handshake; stop doing that
and start checking for the queue being non-empty before stalling an
endpoint and disallowing stall in such case in musb_gadget_set_halt()
like the other gadget drivers do.
Moreover, the driver starts Rx request despite of the endpoint being
halted -- fix this by moving the SendStall bit check from musb_g_rx()
to rxstate(). And we also sometimes get into rxstate() with DMA still
active after clearing an endpoint's halt (not clear why), so bail out
in this case, similarly to what txstate() does...
While at it, also do the following changes :
- in musb_gadget_set_halt(), remove pointless Tx FIFO flushing (the
driver does not allow stalling with non-empty Tx FIFO anyway);
- in rxstate(), stop pointlessly zeroing the 'csr' variable;
- in musb_gadget_set_halt(), move the 'done' label to a more proper
place;
- in musb_g_rx(), eliminate the 'done' label completely...

This patch (as1304) fixes a regression in ehci-hcd. Evidently some
hubs don't handle Clear-TT-Buffer requests correctly, so we should
avoid sending them when they don't appear to be absolutely necessary.
The reported symptom is that output on a downstream audio device cuts
out because the hub stops relaying isochronous packets.
The patch prevents Clear-TT-Buffer requests from being sent following
a STALL handshake. In theory a STALL indicates either that the
downstream device sent a STALL or that no matching TT buffer could be
found. In either case, the transfer is completed and the TT buffer
does not remain busy, so it doesn't need to be cleared.
Also, the patch fixes a minor flaw in the code that actually sends the
Clear-TT-Buffer requests. Although the pipe direction isn't really
used for control transfers, it should be a Send rather than a Receive.

Most of the irq_req_t typedef'd struct can be re-worked quite
easily:
(1) IRQInfo2 was unused in any case, so drop it.
(2) IRQInfo1 was used write-only, so drop it.
(3) Instance (private data to be passed to the IRQ handler):
Most PCMCIA drivers using pcmcia_request_irq() to actually
register an IRQ handler set the "dev_id" to the same pointer
as the "priv" pointer in struct pcmcia_device. Modify the two
exceptions (ipwireless, ibmtr_cs) to also work this waym and
set the IRQ handler's "dev_id" to p_dev->priv unconditionally.
(4) Handler is to be of type irq_handler_t.
(5) Handler != NULL already tells whether an IRQ handler is present.
Therefore, we do not need the IRQ_HANDLER_PRESENT flag in
irq_req_t.Attributes.

The l3_ocpi_ck clock is needed on omap7xx processors for USB.
Additionally, bit 8 of the SOFT_REQ_REG needs to be enabled for
the usb_dc_ck on omap7xx, which is a different bit than that
of the omap16xx-defined clock of the same name.
I added a provision for the usb_dc_ck and l3_ocpi_ck clocks as
dc_clk and hhc_clk, respectively, for omap7xx CPUs. Additionally,
I added a check in machine_without_vbus_sense for all omap7xx
devices, as presently I know of no omap7xx-based devices that
have vbus sense, and it made more sense to me to use a cpu check
here than to spell out each machine one at a time. Finally, DMA
is disabled for omap7xx, as it causes problems with these chips.

Add D-Link DWM-162-U5 device id 1e0e:ce16 into option driver. The device
has 4 interfaces, of which 1 is handled by storage and the other 3 by
option driver.
The device appears first as CD-only 05c6:2100 device and must be

This patch (as1299b) fixes a bug in an error-handling path of usbmon's
binary interface. The storage area for URB data is divided into
fixed-size blocks. If an URB's data can't be copied, the area
reserved for it should be decreased to the size of the truncated
information (rounded up to a block boundary). Rounding up the amount
to be removed and subtracting it from the reserved size is definitely
the wrong thing to do.
Also, when the data for an isochronous URB can't be copied, we can
still copy the isoc packet descriptors. In fact the current code does
copy the descriptors, but then sets the capture length to 0 so they
remain inaccessible. The capture length should be reduced to the
length of the descriptors, not set to 0.

The scratchpad_free() function uses xhci->page_size to free some memory
with pci_free_consistent(). However, the page_size is set to zero before
the call, causing kernel oopses on driver unload. Call scratchpad_free()
before setting xhci->page_size to zero.

The trb_in_td() function in the xHCI driver is supposed to translate a
physical transfer buffer request (TRB) into a virtual pointer to the ring
segment that TRB is in.
Unfortunately, a mistake in this function may cause endless loops as the
driver searches through the linked list of ring segments over and over
again. Fix a couple bugs that may lead to loops or bad output:
1. Bail out if we get a NULL pointer when translating the segment's
private structure and the starting DMA address of the segment chunk. If
this happens, we've been handed a starting TRB pointer from a different
ring.
2. Make sure the function works when there's multiple segments in the
ring. In the while loop to search through the ring segments, use the
current segment variable (cur_seg), rather than the starting segment
variable (start_seg) that is passed in.
3. Stop searching the ring if we've run through all the segments in the
ring.

I have the same problem with cdc-acm (I'm using a Samsung SGH-U900): when I
unplug it from the USB port during a PPP connection, the ppp daemon gets the
hangup correctly (and closes the device), but the struct acm corresponding to
the device disconnected is not freed. Hence reconnecting the device results in
creation of /dev/ttyACM(x+1). The same happens when the system is hibernated
during a PPP connection.
This memory leak is due to the fact that when the tty is hung up,
tty_port_close_start() returns always zero, and acm_tty_close() never reaches
the point where acm_tty_unregister() is called.
Here is a fix for this.

cp210x setup which needed the carrier lines set up (non CLOCAL) which did
not make a call which set the termios bits left the lines down even if
CLOCAL was not asserted.
Fix this not by reverting but by adding the proper dtr_rts and
carrier_raised methods. This both sets the modem lines properly and also
implements the correct blocking semantics for the port as required by
POSIX.

Convert PCMCIA drivers to use the dynamic debug infrastructure, instead of
requiring manual settings of PCMCIA_DEBUG.
Also, remove all usages of the CS_CHECK macro and replace them with proper
Linux style calling and return value checking. The extra error reporting may
be dropped, as the PCMCIA core already complains about any (non-driver-author)
errors.

fsl_udc_release() calls dma_free_coherent() with an inappropriate

This patch presents fixes for the autosuspend feature implementation in
sierra usb serial driver in functions sierra_open(), sierra_close() and
stop_read_write_urbs().
The patch "sierra_close() must resume the device before it notifies it
of a closure" submitted by Oliver Neukum on Wed, October 14 has been
merged as fix in sierra_close() function.
The bug fix in sierra_open() function restores the autopm interface
state on error condition.
The bug fix in in stop_read_write_urbs() function assures that both
receive and interrupt urbs are recycled.

This patch presents a fix for the autosuspend feature implementation in
sierra usb serial driver for function sierra_send_setup(). Because it
is possible to call sierra_send_setup() before sierra_open() or after
sierra_close() we added a get/put interface activity to assure that the
usb control can happen even when the device is autosuspended.

usb-storage: Workaround devices with bogus sense size
Some devices, such as Huawei E169, advertise more than the standard
amount of sense data, causing us to set US_FL_SANE_SENSE, assuming
they support it. However, they subsequently fail the request sense
with that size.
This works around it generically. When a sense request fails due to
a device returning an error, US_FL_SANE_SENSE was set, and that sense
request used a larger sense size, we retry with a smaller size before
giving up.

When the EHCI driver falls behind in its scheduling, the active stream's
first empty microframe may be in the past with respect to the current
microframe. The code attempts to move the starting microframe ("start") N
number of microframes forward, where N is the interval of endpoint.
However, stream->interval is a copy of the endpoint's bInterval, which is
designated in frames for FS devices, and microframes for HS devices.
Convert stream->interval to microframes before using it to move the
starting microframe forward.

If an endpoint is deleted before it's been fully added to the hardware
list, the associated qset will not be fully initialized and an oops will
occur when complete(&qset->remove_complete) is called. This can happen
if a queued URB is cancelled.
Fix this by only removing the qset from the hardware list if the
cancelled URB had qTDs.

The MUSB code relies on platform implementations that currently only
exists for Arm and Blackfin processors, so have the MUSB Kconfig depend
upon those arches.
This should prevent other arches from building MUSB via randconfig.

We simply can't do the USB handoff at FIXUP_HEADER time, since it will
often require us to have valid IO mappings etc. But that in turn
requires a whole different approach, not this trivial one-liner.
Maybe we could teach all the USB quirk handoff handlers to only do the
quirk if the device has all its registers set up (since if it isn't
initialized, it's unlikely to be active), but regardless that will need
a whole lot more code than just saying "let's do it really early".
The proper fix is almost certainly to just leave the legacy IOMMU
mappings active until after all devices have been initialized.
