Cropping should be configured on the pipeline subdev nodes, not through
the video nodes. Remove crop support on all video nodes.

Commit 813f5c0ac5 ("media: Change media device link_notify behaviour")
modified the media controller link setup notification API and updated the
OMAP3 ISP driver accordingly. As a side effect it introduced a bug by
turning power on after setting the link instead of before. This results in
sub-devices not being powered down in some cases when they should be. Fix
it.

The async notifier was registered before the v4l2_device was registered and
before the notifier callbacks were set. This could lead to missing the
bound() and complete() callbacks and to attempting to spin_lock() and
uninitialised spin lock.
Also fix unregistering the async notifier in the case of an error --- the
function may not fail anymore after the notifier is registered.

Added support for reduced blanking version 2 (RB v2) in cvt timings.
Standard specifies a fixed vsync pulse of 8 lines to indicate RB v2
timings. Vertical back porch is fixed at 6 lines and vertical front
porch is remainder of vertical blanking time.
For RB v2, horizontal blanking is fixed at 80 pixels. Horizontal sync
is fixed at 32. All horizontal timing counts (active pixels, front,
back porches) can be specified upto a precision of 1.
RB v2 allows for non standard aspect ratios. In RB v2 vsync does not
indicate aspect ratio. In absence of aspect ratio v4l2_detect_cvt()
cannot calculate image width from image height. Hence extending the
v4l2_detect_cvt() to pass image width in case of RB v2.

The conversion of opal events to a proper irqchip means that handlers
are called until the relevant opal event has been cleared by
processing it. Events that queue work should therefore use a threaded
handler to mask the event until processing is complete.

Ville noticed that the PLL HW readout code parsed the fractional
divider value as if the fractional divider was always enabled. This may
result in a port clock state check mismatch if the preceeding modeset
disabled the fractional divider, but left a non-zero divider value in
the register.

If it takes longer than 12us to read the PIT counter lsb/msb,
then the error margin will never fall below 500ppm within 50ms,
and Fast TSC calibration will always fail.
This patch detects when that will happen and fails fast. Note
the failure message is not printed in that case because:
1. it will always happen on that class of hardware
2. the absence of the message is more informative than its
presence

opal-prd driver will mmap() firmware code/data area as private
mapping to prd user space daemon. Write to this page will
trigger COW faults. The new COW pages are normal kernel RAM
pages accounted by the kernel and are not special.
vma->vm_page_prot value will be used at page fault time
for the new COW pages, while pgprot_t value passed in
remap_pfn_range() is used for the initial page table entry.
Hence:
* Do not add _PAGE_SPECIAL in vma, but only for remap_pfn_range()
* Also remap_pfn_range() will add the _PAGE_SPECIAL flag using
pte_mkspecial() call, hence no need to specify in the driver
This fix resolves the page accounting warning shown below:
BUG: Bad rss-counter state mm:c0000007d34ac600 idx:1 val:19
The above warning is triggered since _PAGE_SPECIAL was incorrectly
being set for the normal kernel COW pages.

We were using the include/linux/rbtree.h directly from the kernel,
which broke the build as soon as it started using rcupdate.h, to
avoid dragging the rcu header files into tools/, for which there is
no use so far, grab a copy of rbtree.h.
This is the minimal fix, later patches will copy as well lib/rbtree.c
and move rbtree.h into tools/include/, etc.

ext4_free_blocks is looping around the allocation request and mimics
__GFP_NOFAIL behavior without any allocation fallback strategy. Let's
remove the open coded loop and replace it with __GFP_NOFAIL. Without the
flag the allocator has no way to find out never-fail requirement and
cannot help in any way.

Commit 835a6a2f86 ("Bluetooth: Stop sabotaging list poisoning")
thought that the code was sabotaging the list poisoning when NULL'ing
out the list pointers and removed it.
But what was going on was that the bluetooth code was using NULL
pointers for the list as a way to mark it empty, and that commit just
broke it (and replaced the test with NULL with a "list_empty()" test on
a uninitialized list instead, breaking things even further).
So fix it all up to use the regular and real list_empty() handling
(which does not use NULL, but a pointer to itself), also making sure to
initialize the list properly (the previous NULL case was initialized
implicitly by the session being allocated with kzalloc())

An.

"high performance" NTB clients.
