Add support for flow steering counters action with a non-base counter
ID (offset) for bulk counters.
When creating a flow counter object, save the bulk value. This value is
used when a flow action with a non-base counter ID is requested - to
validate that the required offset is in the range of the allocated bulk.

Enable subscription for device events over DEVX.
Each subscription is added to the two level xarray data structure
according to its event number and the DEVX object information in case was
given with the given target fd.
Those events will be reported over the given fd once will occur.
Downstream patches will mange the dispatching to any subscription.

This patch intoruduces a new mlx5_ib driver attribute to the DM allocation
method - the DM type.
In order to allow addition of new types in downstream patches this patch
also refactors the allocation, deallocation and registration handlers to
consider the requested type and perform the necessary actions according to
it.
Since not all future device memory types will be such that are mapped to
user memory, the mandatory page index output attribute is modified to be
optional.

Allow this only via mlx5 raw create flow API, legacy verbs are not
supported. To accommodate that, we add a new attribute to matcher creation
to indicate the type of flow table to be used.
MLX5_IB_ATTR_FLOW_MATCHER_FT_TYPE
With this new attribute MLX5_IB_ATTR_FLOW_MATCHER_FLOW_FLAGS is no longer
needed, we keep it for compatibility but at most only a single attribute can
be passed of the two.
When inserting a flow rule to the FDB we require that a DEVX FT is
provided as a destination, no other configuration is allowed.

A user can provide a hint which will be attached to the packet and written
to the CQE on receive. This can be used as a way to offload operations
into the HW, for example parsing a packet which is a tunneled packet, and
if so, pass 0x1 as the hint. The software can use that hint to decapsulate
the packet and parse only the inner headers thus saving CPU cycles.

Currently a matcher can only be created and attached to a NIC RX flow
table. Extend it to allow it on NIC TX flow tables as well.
In order to achieve that, we:
1) Expose a new attribute: MLX5_IB_ATTR_FLOW_MATCHER_FLOW_FLAGS.
enum ib_flow_flags is used as valid flags. Only
IB_FLOW_ATTR_FLAGS_EGRESS is supported.
2) Remove the requirement to have a DEVX or QP destination when creating a
flow. A flow added to NIC TX flow table will forward the packet outside
of the vport (Wire or E-Switch in the SR-iOV case).

Return a device UAR index for a given user index via the DEVX interface.
Security note:
The hardware protection mechanism works like this: Each device object that
is subject to UAR doorbells (QP/SQ/CQ) gets a UAR ID (called uar_page in
the device specification manual) upon its creation. Then upon doorbell,
hardware fetches the object context for which the doorbell was rang, and
validates that the UAR through which the DB was rang matches the UAR ID
of the object.
If no match the doorbell is silently ignored by the hardware. Of
course, the user cannot ring a doorbell on a UAR that was not mapped to
it.
Now in devx, as the devx kernel does not manipulate the QP/SQ/CQ command
mailboxes (except tagging them with UID), we expose to the user its UAR
ID, so it can embed it in these objects in the expected specification
format. So the only thing the user can do is hurt itself by creating a
QP/SQ/CQ with a UAR ID other than his, and then in this case other users
may ring a doorbell on its objects.
The consequence of that will be that another user can schedule a QP/SQ
of the buggy user for execution (just insert it to the hardware schedule
queue or arm its CQ for event generation), no further harm is expected.
