"[SCSI] zfcp: Use status_read_buf_num provided by FCP channel"
started using a value returned by the channel but only evaluated the value
if the fabric link is up.

"[SCSI] zfcp: Update status read mempool"
introduced mempool resizings based on the above value.
On setting an FCP device online for the very first time since boot, a new
zeroed adapter object is allocated. If the link is down, the number of
status read requests remains zero. Since just the config data exchange is
incomplete, we proceed with adapter open recovery. However, we
unconditionally call mempool_resize with adapter->stat_read_buf_num == 0 in
this case.
This causes a kernel message "kernel BUG at mm/mempool.c:131!" in process
"zfcperp<FCP-device-bus-ID>" with last function mempool_resize in Krnl PSW
and zfcp_erp_thread in the Call Trace.
Don't evaluate channel values which are invalid on link down. The number of
status read requests is always valid, evaluated, and set to a positive
minimum greater than zero. The adapter open recovery can proceed and the
channel has status read buffers to inform us on a future link up event.
While we are not aware of any other code path that could result in mempool
resize attempts of size zero, we still also initialize the number of status
read buffers to be posted to a static minimum number on adapter object
allocation.

This patch removes an interface that was used to manage access control
tables within the HBA. The patch consequently removes the handling
for conditions related to those access control tables, too.
That initiator-based access control feature was only needed until the
introduction of NPIV and was withdrawn with z10 years ago.
It's time to cleanup the corresponding device driver code.

So far, we could only increase the s390dbf log level after an FCP
device has been initially set online for it to create the dbf entries
required to adjust the level.
Introduce zfcp.dbflevel as counterpart to the already existing
zfcp.dbfsize to enable debugging of e.g. setting an FCP device online.

"[SCSI] zfcp: support for hardware data router"
reduced the initial block queue limits in the scsi_host_template to the
absolute minimum and adjusted them later on. However, the adjustment was
too late for the BSG devices of Scsi_Host and fc_host.
Therefore, ioctl(..., SG_IO, ...) with request or response size > 4kB to a
BSG device of an fc_host or a Scsi_Host fails with EINVAL. As a result,
users of such ioctl such as HBA_SendCTPassThru() in libzfcphbaapi return
with error HBA_STATUS_ERROR.
Initialize the block queue limits in zfcp_scsi_host_template to the
greatest common denominator (GCD).
While we cannot exploit the slightly enlarged maximum request size with
data router, this should be neglectible. Doing so also avoids running into
trouble after live guest relocation (LGR) / migration from a data router
FCP device to an FCP device that does not support data router. In that
case, zfcp would figure out the new limits on adapter recovery, but the
fc_host and Scsi_Host (plus in fact all sdevs) still exist with the old and
now too large queue limits.
It should also OK, not to use half the size as in the DIX case, because
fc_host and Scsi_Host do not transport FCP requests including SCSI commands
using protection data.

FCP device remains in status ERP_FAILED when device is switched online
or adapter recovery is triggered while link to SAN is down.
When Exchange Configuration Data command returns the FSF status
FSF_EXCHANGE_CONFIG_DATA_INCOMPLETE it aborts the exchange process.
The only retries are done during the common error recovery procedure
(i.e. max. 3 retries with 8sec sleep between) and remains in status
ERP_FAILED with QDIO down.
This commit reverts the commit 0df138476c
(zfcp: Fix adapter activation on link down).
When FSF status FSF_EXCHANGE_CONFIG_DATA_INCOMPLETE is received the
adapter recovery will be finished without any retries. QDIO will be
up now and status changes such as LINK UP will be received now.

The bus rescan process was called simultaneously on
every device failure. This finally leads into race
conditions (double device add/remove actions).
This patch protects the rescan area by mutual exclusion
and improves ap_config_timer handling

Given that the memory block size is always larger or equal to
the memory increment size there is no need for a usecount anymore.
Each memory increment belongs to exactly one memory block.
So get rid of the usecount.

The current memory detection loop will detect all present memory of
a machine. This is true even if the user specified the "mem=" parameter
on the kernel command line.
This can be a problem since the memory detection may cause a fully
populated host page table for the guest, even for those parts of the
memory that the guest will never use afterwards.
So fix this and only detect memory up to a user supplied "mem=" limit
if specified.

The current location for mapping Virtio devices does not take
into consideration of the standby memory. This causes the failure
of mapping standby memory since the location for the mapping is
already taken by the Virtio devices. To fix the problem, we move
the location to beyond the end of standby memory.

The zcore device driver makes use of the global visible real_memory_size
variable with its odd semantics.
Since the zcore device driver already has code in place which calculates
the memory size at module load time, use that code to calculate the current
memory end value.
One user less of the odd real_memory_size variable.

When we fetch an interrupt on the CCW console using tsch (via
ccw_device_wait_idle formerly known as wait_cons_dev) we increment
the irq count for the affected interruption class but it is not
accounted as an IO interrupt.
This is broken since commit b603d258a4
"s390: remove superfluous tpi from wait_cons_dev"
Fix it so that the sum of the individual interrupts per class matches
the number of IO interrupts again.

In case a machine supports memory hotplug all active memory increments
present at IPL time have been initialized with a "usecount" of 1.
This is wrong if the memory increment size is larger than the memory
section size of the memory hotplug code. If that is the case the
usecount must be initialized with the number of memory sections that
fit into one memory increment.
Otherwise it is possible to put a memory increment into standby state
even if there are still active sections.
Afterwards addressing exceptions might happen which cause the kernel
to panic.
However even worse, if a memory increment was put into standby state
and afterwards into active state again, it's contents would have been
zeroed, leading to memory corruption.
This was only an issue for machines that support standby memory and
have at least 256GB memory.
This is broken since commit fdb1bb15 "[S390] sclp/memory hotplug: fix
initial usecount of increments".

The e{uid,gid} --> {uid,gid} credentials fix conflicted with the
cleanup in net-next to now pass cred structs around.
The be2net driver had a bug fix in 'net' that overlapped with the VLAN
interface changes by Patrick McHardy in net-next.
An IGB conflict existed because in 'net' the build_skb() support was
reverted, and in 'net-next' there was a comment style fix within that
code.
Several batman-adv conflicts were resolved by making sure that all
calls to batadv_is_my_mac() are changed to have a new bat_priv first
argument.
Eric Dumazet's TS ECR fix in TCP in 'net' conflicted with the F-RTO
rewrite in 'net-next', mostly overlapping changes.
Thanks to Stephen Rothwell and Antonio Quartulli for help with several
of these merge resolutions.

qeth_hdr_chk_and_bounce() can possibly shift the skb->data
pointer. However, the existing code didn't update the hdr pointer,
which should point to skb->data, accordingly.
Symptoms of this issue are sporadic recoveries.

Rename the hardware VLAN acceleration features to include "CTAG" to indicate
that they only support CTAGs. Follow up patches will introduce 802.1ad
server provider tagging (STAGs) and require the distinction for hardware not
supporting acclerating both.

If too many ccw requests are pre-build before a suspend/resume cycle
the device might not get enough memory to do path verification
during resume.
Requeue requests to the block device request queue on suspend and free
pre-build ccw requests.

cio_validate_subchannel is used to do some basic checks to find out
if it's worth to further investigate a subchannel. Move the allocation
and initialization of the subchannels locks to css_alloc_subchannel.
Clean up the functions involved while at it.

Via ccw_device_init_count we keep track of how many devices are in
asynchronous device recognition/initialization. For early devices this
variable was not only used prior to its initialization but used
incorrectly (incremented but never decremented). Fix this by using static
initialization for this variable (and friends), make them visible to
device.c only, and decrement the counter after recognition of early
devices is finished.

Use cio_register_early_subchannels to register early subchannels
which are already in use. Call this function before we do the
actual subchannel scanning loop. This helps us to get rid of some
more special cases regarding the console subchannel.

allocation for these structures. With this change the console
device is treated (mostly) like any other ccw device and we can
remove some special cases.

Split the subchannel registration in device_initialize and device_add
and move the initialization part inside the allocation function. With
this change we can use refcounting during the complete lifespan of a
subchannel which is important for devices where we do the actually
registration at a later time.

force_console is used to wake up the CCW based console device to
print a panic message in case something goes wrong in a suspend
or resume cycle. Stop using the static console_subchannel and add
a parameter to this function to specify which ccw device we have
to wake up.

wait_cons_dev is used to busy wait for an interrupt on the console
ccw device. Stop using the static console_subchannel and add a
parameter to this function to specify on which ccw device/subchannel
we have to do the polling.
While at it rename the function to ccw_device_wait_idle and
move it to device.c
