Arjan van de Ven bb4e3b5ac8 usbmon: fix tiny race exposed by the fastboot patches
usbmon registers the notifier chain, takes the bus lock and then goes to
scan the existing devices for hooking up.

Unfortunately, if usb_mon gets initialized while USB bus discovery is
going on, it's possible that usbmon gets a notifier on one cpu (which runs
without USB locks), and the scan is going on and also finds the new bus,
resulting in a double sysfs registration, which then produces a WARNING.

Alan Stern 8520f38099 USB: change hub initialization sleeps to delayed_work
This patch (as1137) changes the hub_activate() routine, replacing the
power-power-up and debounce delays with delayed_work calls.  The idea
is that on systems where the USB stack is compiled into the kernel
rather than built as modules, these delays will no longer block the
boot thread.  At least 100 ms is saved for each root hub, which can
add up to a significant savings in total boot time.

Arjan van de Ven was very pleased to see that this shaved 700 ms off
his computer's boot time.  Since his total boot time is on the order
of two seconds, the improvement is considerable.

Ming Lei 3c4bb71f96 USB: ehci-dbg: fix reading less content of periodic file
This patch fix 2 problems about reading periodic file:

1. The "..." after a interrupt qh is missed because buffer pointer is
   not moved.

2. After setting p.ptr as NULL, its next qh or itd will be omited and
   can't be stored in debug buffer.

Ming Lei 3c04e20ea9 USB: ehci-dbg: increase debug buffer size for periodic file
This patch is based on the following ideas:

1. Some usb devices (such as usb video class) have endpoints of high
   interval attribute, so reading "periodic" file need more debug buffer
   to accommodate the qh or itd schedule information.  For example, 4KB
   buffer is not enough for a single interrupt qh of 2ms period.

2. print a %p need 16 byte buffer on 64-bits arch, but 8 byte on 32-bits
   arch. Add a extra bonus for 64-bits arch.

Oliver Neukum 399d31da4e USB: RESET_RESUME needs to block autosuspend when remote wakeup is needed
Reset upon resumption will wipe the input buffer and is therefore
a reason to not suspend if remote wakeup is requested because
the driver needs that data.

Alan Stern 4a511bc3f5 OHCI: Allow broken controllers to auto-stop
This patch (as1134) attempts to improve the way we handle OHCI
controllers with broken Root Hub Status Change interrupt support.  In
these controllers the RHSC interrupt bit essentially never turns off,
making RHSC interrupts useless -- they have to remain permanently
disabled.

Such controllers should still be allowed to turn off their root hubs
when no devices are attached.  Polling for new connections can
continue while the root hub is suspended.  The patch implements this
feature.  (It won't have much effect unless CONFIG_PM is enabled and
CONFIG_USB_SUSPEND is disabled, but since the overhead is very small
we may as well do it.)

Alan Stern 8bfa247270 usb-storage: report underflow with no sense data
This patch (as1118) addresses a problem with certain USB mass-storage
devices.  These devices sometimes return less data than asked for and
then provide no sense data to explain the problem.  Currently
usb-storage leaves it up to the SCSI layer to decide how this should
be handled, and the SCSI layer interprets the lack of sense data to
mean that nothing went wrong.  But if we got less data than required
then something definitely _did_ go wrong, and we should say so.

The patch tells the SCSI layer to retry the command when this sort of
thing happens.  Retrying may not solve the underlying problem, but
it's better than believing that data was transferred when it wasn't.

Li Yang 3948f0e0c9 usb: add Freescale QE/CPM USB peripheral controller driver
Some of Freescale SoC chips have a QE or CPM co-processor which
supports full speed USB.  The driver adds device mode support
of both QE and CPM USB controller to Linux USB gadget.  The
driver is tested with MPC8360 and MPC8272, and should work with
other models having QE/CPM given minor tweaks.

David Brownell 2121427836 usb gadget: defer obex enumeration
Some USB peripheral controller drivers support software control
over the data pullup.  Use those controls to prevent the OBEX
function from enumerating until the userspace server has opened
the /dev/ttyGS* node it will use to implement protocol chitchat
with the USB host.

David Brownell 60beed95e3 usb gadget: function activation/deactivation
Add a new mechanism to the composite gadget framework, letting
functions deactivate (and reactivate) themselves.  Think of it
as a refcounted wrapper for the software pullup control.

A key example of why to use this mechanism involves functions that
require a userspace daemon.  Those functions shuld use this new
mechanism to prevent the gadget from enumerating until those daemons
are activated.  Without this mechanism, hosts would see devices that
malfunction until the relevant daemons start.

Marc Zyngier 2714021937 USB: Let some USB host controllers get IRQ flags from resource
[This version fixes a thinko in the r8a66597 driver]

This patch let a few discrete USB host controllers drivers (isp116x-hcd,
r8a66597-hcd and sl811-hcd) obtain IRQ flags from their IORESOURCE_IRQ
resource if configured as such, much like it's been done for the smc91x
driver.

It spares people writing support for specific boards the burden to
configure the interrupt controller independantly, and keeps all IRQ
related information in a single resource.

HCD that are integrally part of a SoC have been left aside, as there
is probably no "wiring" options...

Tested on an Xscale PXA-255 based platform with isp116x-hcd.

Oliver Neukum 55b447bf79 USB: kill URBs permanently
looking at usb_kill_urb() it seems to me that it is unnecessarily lenient.
In the use case of disconnect() you never want to use the URB again
(for the same device) But leaving urb->reject elevated will make it easier
to avoid races between read/write and disconnect.

Vladimir Vukicevic c3d36c453c USB: Add Oceanic product ID to ftdi_sio
Add Oceanic PID to ftdi_sio driver

Oceanic dive computers (such as the VT3 --
http://www.oceanicworldwide.com/p_computers_vt3.html) all use an onboard
FTDI serial converter, with the FTDI vid and a  PID of 0xf460.  The
attached patch adds that pid to ftdi_sio; driver  connects to my VT3
after that.

Steven Whitehouse a447c09324 vfs: Use const for kernel parser table
This is a much better version of a previous patch to make the parser
tables constant. Rather than changing the typedef, we put the "const" in
all the various places where its required, allowing the __initconst
exception for nfsroot which was the cause of the previous trouble.

This was posted for review some time ago and I believe its been in -mm
since then.

Alan Cox 4a90f09b20 tty: usb-serial krefs
Use kref in the USB serial drivers so that we don't free tty structures
from under the URB receive handlers as has historically been the case if
you were unlucky. This also gives us a framework for general tty drivers to
use tty_port objects and refcount.

Contains two err->dev_err changes merged together to fix clashes in the
-next tree.

Jason Wessel 4ad6de0b3f usb: fix pl2303 initialization
This patch removes the private check for the termios_initialized for
the pl2303 usb driver.  It forced the baud to 9600 on the first call
to pl2303_set_termios()

Based on the tty changes in the 2.6.27 kernel, the termios passed to
the *_set_termios functions is always populated the first time.

This means there is no need to privately initialize the settings the
first time, and doing so will not allow the use of the kernel
parameter "console=ttyUSB0,115200" as an example.

Scott Ashcroft 7aef709bf6 Fix oti6858 debug level
For some reason the oti6858 driver undefines and redefines the dbg
macro. This makes it spew debugging messages at KERN_INFO instead of
KERN_DEBUG.

This patch removes the undef and define making the driver log like every
other USB serial driver.

Direct access to USB host controller registers is considered to be not
portable, and is usually a bad sign for poorly abstracted interface.
Introduce .flags and .power_on_delay to "struct pxaohci_platform_data"
so that most platforms don't bother to write their own .init/.exit()
sequences.

Alan Stern 5257d97a21 USB: revert recovery from transient errors
This patch (as1135) essentially reverts the major parts of two earlier
patches to usbcore, because they ended up causing a regression.

Trying to recover from transient communication errors can lead to
other problems, because operations that failed during the error period
are not always retried.  The simplest example is the initial
Set-Config request sent after device enumeration; if it gets lost then
it will not be retried and the device will remain unconfigured.

This patch restores the old behavior in which any port disconnect or
port disable causes the entire device structure to be removed, fixing a
reported regression.

Chris Adams 2bcbe4c199 usb serial: ti_usb_3410_5052 obviously broken by firmware changes
While making some other changes to ti_usb_3410_5052, I noticed that the
changes made to move the firmware loading to a separate function are
broken (in ti_download_firmware(), status is set to -ENOMEM and never
changed).  This means the driver will never initialize the device
properly.  It looks like status was supposed to get the result of
ti_do_download().

David Brownell 01c1714265 USB: fix EHCI periodic transfers
As noted by Stefan Neis <Stefan.Neis@kobil.com>, we had a recent
regression with EHCI periodic transfers, in some (seemingly not
all that common) cases.

The root cause was that the schedule activation was only loosely
coupled to the addition or removal of transfers, so two different
execution contexts could both think they had to deactivate (or
conversely activate) the schedule.  So this fix tightens that
coupling, managing it more like a refcount.

Geoff Levand 83a7982073 USB: fix hcd interrupt disabling
Commit de85422b94, 'USB: fix interrupt
disabling for HCDs with shared interrupt handlers' changed usb_add_hcd()
to strip IRQF_DISABLED from irqflags prior to calling request_irq()
with the justification that such a removal was necessary for shared
interrupts to work properly.  Unfortunately, the change in that commit
unconditionally removes the IRQF_DISABLED flag, causing problems on
platforms that don't use a shared interrupt but require IRQF_DISABLED.
This change adds a check for IRQF_SHARED prior to removing the
IRQF_DISABLED flag.

Fixes the PS3 system startup hang reported with recent Fedora and
OpenSUSE kernels.

Note that this problem is hidden when CONFIG_LOCKDEP=y (ps3_defconfig),
as local_irq_enable_in_hardirq() is defined as a null statement for
that config.

David Brownell 0bcfeb3ec9 USB: ehci: fix some ehci hangs and crashes
I noticed that the "Refactor "if (handshake()) state = HC_STATE_HALT"
patch from earlier this year perpetuated a potential problem:  it can
mark the controller as halted when it's still running (but not acting
as, perhaps wrongly, expected).

That caused some hangs and crashes, rather than more polite failure
modes of a truly halted controller.  This patch forces a true halt,
and emits a (previously missing) diagnostic.

Alan Stern b5fb454f69 USB: automatically enable RHSC interrupts
This patch (as1069c) changes the way OHCI root-hub status-change
interrupts are enabled.  Currently a special HCD method,
hub_irq_enable(), is called when the hub driver is finished using a
root hub.  This approach turns out to be subject to races, resulting
in unnecessary polling.

The patch does away with the method entirely.  Instead, the driver
automatically enables the RHSC interrupt when no more status changes
are present.  This scheme is safe with controllers using
level-triggered semantics for their interrupt flags.

Alan Stern 5096aedcd2 USB: Don't rebind before "complete" callback
This patch (as1130) fixes an incompatibility between the new PM
infrastructure and USB power management.  We are not allowed to call
drivers' probe routines during a system sleep transition between the
"prepare" and "complete" callbacks, but that's exactly what we do when
a driver doesn't have full suspend/resume support.  Such drivers are
unbound during the "suspend" call and reprobed during the "resume" call.

The patch causes the reprobe step to be skipped if the "complete"
callback hasn't been issued yet, i.e., if the interface's
dev.power.status field is not equal to DPM_ON.  Thus during the
"resume" callback nothing bad will happen, and during the final
"complete" callback the reprobing will occur as desired.

This fixes the problem reported in Bugzilla #11263.

Alan Stern f2189c477c USB: Add new PM callback methods for USB
This patch (as1129) adds support for the new PM callbacks to usbcore.
The new callbacks merely invoke the same old USB power management
routines as the old ones did.

A minor improvement is that the callbacks are present only in the
"USB-device" device_type structure, rather than in the bus_type
structure.  This way they will be invoked only for USB devices, not
for USB interfaces.  The core USB PM routines automatically handle
suspending and resuming interfaces along with their devices.

Alan Stern 55151d7dab USB: Defer Set-Interface for suspended devices
This patch (as1128) fixes one of the problems related to the new PM
infrastructure.  We are not allowed to register new child devices
during the middle of a system sleep transition, but unbinding a USB
driver causes the core to automatically install altsetting 0 and
thereby create new endpoint pseudo-devices.

The patch fixes this problem (and the related problem that installing
altsetting 0 will fail if the device is suspended) by deferring the
Set-Interface call until some later time when it is legal and can
succeed.  Possible later times are: when a new driver is being probed
for the interface, and when the interface is being resumed.

Anand Gadiyar fb85d99161 MUSB: Fix index register corruption seen with g_ether and Windows host
If Indexed Mode register accesses are enabled, the ep0_rxstate()
function calls musb_g_ep0_giveback() before writing to the CSR
register. When control returns to this ep0_rxstate, the index
register contents are over-written. This causes the CSR register
write to fail.

Fixed by writing the correct value into the index register before
writing to the CSR.

This was observed only in ep0_rxstate() with g_ether loaded and
the device connected to a MS Windows host PC. Anticipatively fixed
ep0_txstate() as well.

Robert Jarzmik bf31338bfd USB: Fix pxa27x_udc usb speed handling.
The new composite framework revealed a weakness in the
pxa27x_udc driver gadget register function. Instead of
checking if speed asked for was USB_LOW_SPEED upon
usb_gadget_register() to deny service, it checked only
for USB_FULL_SPEED, thus denying service to usb high
speed capable gadgets (like g_ether).

On some AMD 700 series southbridges, ISO OUT transfers (such as audio
playback through speakers) on the USB OHCI controller may be corrupted
when an A-Link express power saving feature is active.

PLL power down mode in conjunction with link power management feature
L1 being enabled is the bad combination ... this patch prevents them
from being enabled when ISO transfers are pending.

David Brownell e5fbab51b4 usb: cdc-acm: drain writes on close
Add a mechanism to let the write queue drain naturally before
closing the TTY, rather than always losing that data.  There
is a timeout, so it can't wait too long.

Provide missing locking inside acm_wb_is_avail(); it matters
more now.  Note, this presumes an earlier patch was applied,
removing a call to this routine where the lock was held.

Slightly improved diagnostics on write URB completion, so we
can tell when a write URB gets killed and, if so, how much
data it wrote first ... and so that I/O path is normally
silent (and can't much change timings).

David Brownell 934da4635c usb: cdc-acm: stop dropping tx buffers
The "increase cdc-acm write throughput" patch left in place two
now-obsolete mechanisms, either of which can make the cdc-acm
driver drop TX data (nasty!).  This patch removes them:

  - The write_ready flag ... if an URB and buffer were found,
    they can (and should!) always be used.

  - TX path acm_wb_is_used() ... used when the buffer was just
    allocated, so that check is pointless.

Also fix a won't-yet-matter leak of a write buffer on a disconnect path.

David Brownell 1f1ba11b64 usb gadget: issue notifications from ACM function
Update the CDC-ACM gadget code to support the peripheral-to-host
notifications when the tty is opened or closed, or issues a BREAK.
The serial framework code calls new generic hooks; right now only
CDC-ACM uses those hooks.  This resolves several REVISIT comments
in the code.  (Based on a patch from Felipe Balbi.)

Note that this doesn't expose USB_CDC_CAP_BRK to the host, since
this code still rejects USB_CDC_REQ_SEND_BREAK control requests
for host-to-peripheral BREAK signaling (received via /dev/ttyGS*).

According to Alan Stern, short packets are quite normal under
certain circumstances. This printk was triggered by usb to
serial converters on every packet and some usb sticks triggered
a few of those while plugging the stick.

Enrico Scholz 3f02a957d5 usb: ISP1760: improve pre-fetch timing
ISP1760 requires a delay of 90ns between programming the address and
reading the data.  Current driver solves this by a mdelay(1) which is
very heavy weighted and slow.  This patch applies the workaround from
the ISP1760 FAQ by using two different banks for PTD and payload data
and using a common wait for them.  This wait is done by an additional
ISP1760 access (whose timing constraints guarantee the 90ns delay).
This improves speed when reading from an USB stick from:

  $ time dd if=/dev/sda of=/dev/zero bs=65536 count=1638
  real    1m 15.43s
  user    0m 0.44s
  sys     0m 39.46s

to

  $ time dd if=/dev/sda of=/dev/zero bs=65536 count=1638
  real    0m 18.53s
  user    0m 0.16s
  sys     0m 12.97s

David Brownell 937ef73d50 USB: serial gadget: rx path data loss fixes
Update RX path handling in new serial gadget code to cope better with
RX blockage:  queue every RX packet until its contents can safely be
passed up to the ldisc.  Most of the RX path work is now done in the
RX tasklet, instead of just the final "push to ldisc" step.  This
addresses some cases of data loss:

  - A longstanding serial gadget bug: when tty_insert_flip_string()
    didn't copy the entire buffer, the rest of the characters were
    dropped!  Now that packet stays queued until the rest of its data
    is pushed to the ldisc.

  - Another longstanding issue:  in the unlikely case that an RX
    transfer returns data and also reports a fault, that data is
    no longer discarded.

  - In the recently added RX throttling logic:  it needs to stop
    pushing data into the TTY layer, instead of just not submitting
    new USB read requests.  When the TTY is throttled long enough,
    backpressure will eventually make the OUT endpoint NAK.

Also: an #ifdef is removed (no longer necessary); and start switching
to a better convention for debug messages (prefix them with tty name).

Kevin Lloyd 32fe5e3934 USB Storage Sierra: TRU-Install feature update
This patch upgrades the support for the Sierra Wireless TRU-Install
feature (i.e. zeroCD) to allow for future support of Linux enabled
TRU-Install devices.

By default all devices that do not have a Linux enabled TRU-Install
device (i.e. the device does not have a Linux package on the virtual CD
partition) will be switched into "modem mode." Devices that do contain a
Linux package in the TRU-Install virtual CD will be allowed to enumerate
as a CD-Rom so that either (a) a user can install the packaged software
or (b) a user-space application (e.g. udev) can switch it to modem mode.

This patch does allow for manual override by adding a usb-storage module
parameter 'swi_tru_install' which can force the modem into either mode
regardless of what packages it contains.

This patch (as1115) adds unusual_devs entries with the IGNORE_RESIDE
flag for the iRiver T10 and the Simple Tech/Datafab CF+SM card
reader.  Apparently these devices provide reasonable residue values
for READ and WRITE operations, but not for others like INQUIRY or READ
CAPACITY.

This fixes the iRiver T10 problem reported in Bugzilla #11125.

Alan Stern 59f4ff2ecf usb-storage: automatically recognize bad residues
This patch (as1119) will help to reduce the clutter of usb-storage's
unusual_devs file by automatically detecting some devices that need
the IGNORE_RESIDUE flag.  The idea is that devices should never return
a non-zero residue for an INQUIRY or a READ CAPACITY command unless
they failed to transfer all the requested data.  So if one of these
commands transfers a standard amount of data but there is a positive
residue, we know that the residue is bogus and we can set the flag.

This fixes the problems reported in Bugzilla #11125.

is unregistered, the core will give back its minors -- even if the

Alan Stern 38b375d961 USB: OHCI: fix system hang caused by earlier patch
This patch (as1114) fixes a problem that was revealed by an earlier
patch (as1069b).  Some broken controllers seem never to turn off their
RHCS interrupt status bit, even when told to do so.  As a result they
generate an interrupt storm and hang the system.

The patch avoids enabling RHSC interrupt requests when the RHCS status
bit is already set.  This should have no adverse affects on normal
controllers, since they won't set the status bit until a root-hub
status change actually occurs, in which case we wouldn't enable RHSC
interrupt requests anyway -- we would wait until the status change had
been processed and cleared.

David Brownell c728df70ab USB: fix USB boot crash, ecm_do_notify(), list_add corruption. prev->next should be next (ffff88003b8f82f8)
This fixes a BUG() turned up by Ingo via randconfig testing, where
CONFIG_LIST_DEBUG turned up list corruption.  The corruption was
caused by the dummy_hcd (single-machine test harness for gadget and
HCD code) trashing the request queue when driven by the new CDC
composite gadget an I/O pattern that was previously uncommon.
Fix suggested by Alan Stern.

Wolfgang Mües 88d987d6db usb: auerswald: remove driver (obsolete)
This patch removes the auerswald USB driver from the linux kernel
2.6.26.

This driver was included into the kernel mainly to connect to the ISDN
framework. This was done in linux 2.4.x. For 2.6.x, due to the fragile
and moving ISDN support, this connection was never realized, and the
only use of this driver was for device configuration. In the age of DSL,
the demand of ISDN support is getting very low.

Meanwhile, with the advent of libusb, an userspace driver was done for
the device configuration which works fine for linux and mac. (Thanks to
the libusb developers!). The userspace driver is downloadable from the
auerswald web site.

So this driver is obsolete now and has to be removed. Many thanks to all
developers which helped me to bring this driver up and working.

Luis R. Rodriguez 7d283aee50 list.h: Add list_splice_tail() and list_splice_tail_init()
If you are using linked lists for queues list_splice() will not do what
you would expect even if you use the elements passed reversed. We need
to handle these differently. We add list_splice_tail() and
list_splice_tail_init().

Alan Stern 580da34847 Fix USB storage hang on command abort
Okay, I found the cause of the hang.  It is a simple bug in the USB
scatter-gather library, caused by changes added in response to the S-G
chaining modification.

This patch (as1125) fixes a bug in the USB scatter-gather library.
Early exit from the S-G initialization loop does not reset the count of
outstanding URBs.

Kmem cache passed to constructor is only needed for constructors that are
themselves multiplexeres.  Nobody uses this "feature", nor does anybody uses
passed kmem cache in non-trivial way, so pass only pointer to object.

Non-trivial places are:

	arch/powerpc/mm/hugetlbpage.c

Arjan van de Ven b6c6393700 Rename WARN() to WARNING() to clear the namespace
We want to use WARN() as a variant of WARN_ON(), however a few drivers are
using WARN() internally.  This patch renames these to WARNING() to avoid the
namespace clash.  A few cases were defining but not using the thing, for those
cases I just deleted the definition.

Linus Torvalds 6069fb2ef5 Re-delete zombie 'drivers/usb/serial/airprime.c' file
Stephen Rothwell points out that this file got deleted (on purpose) by
commit 640c1bce86 ("USB: delete airprime
driver"), but then almost immediately incorrectly resurrected by commit
95da310e66 ("usb_serial: API all change").

Delete it again.  If it comes back, we'll need to drive a stake through
its heart.
