Add a function to get regulators, supplying card's Vdd and Vccq on a
specific host. If a Vdd supplying regulator is found, the function checks,
whether a valid OCR mask can be obtained from it. The Vccq regulator is
optional. A failure to get it is not fatal.

mmc_execute_hpi should send the HPI command only once, and only
if the card is in PRG state.
According to eMMC spec, the command's completion time is
not dependent on OUT_OF_INTERRUPT_TIME. Only the transition
out of PRG STATE is guarded by OUT_OF_INTERRUPT_TIME - which
is defined to begin at the end of sending the command itself.
Specify the default timeout for the actual sending of HPI
command, and then use OUT_OF_INTERRUPT_TIME to wait for
the transition out of PRG state.

When mmc_host is not spi mode, mmc/sd is doing mmc_deselect_cards().
mmc_deselect_cards could be returned error.

SDIO_CCCR_IF[1:0] in SDIO card is used for card data bus width
setting as below:
00b: 1-bit bus
01b: Reserved
10b: 4-bit bus
11b: 8-bit bus (only for embedded SDIO)
And sdio_enable_wide is for setting data bus width as 4-bit.
But currently, it first reads the register, second OR' 1b with
SDIO_CCCR_IF[1], and then writes it back.
As we can see, this is based on such assumption that the
SDIO_CCCR_IF[0] is always 0. Apparently, this is not right.

This patch adds support for large sector size of 4KB by disabling
emulation. This patch passes eMMC DATA_SECTOR_SIZE as the logical
block size during mmc_blk_alloc_req.
In order to use this patch for 4KB sector size, ensure that
USE_NATIVE_SECTOR is enabled, partition table is 4KB sector size
aligned and file system block and sector size are 4KB multiples.

bus_width is passed to the function and when 0 (MMC_BUS_WIDTH_1)
will cause the function to return. So in in the second test it
definitely is different from 0, and the third test is redundant.

For eMMC cards that has been initialized from a bootloader,
the VCC voltage supply must not be cut in an uncontrolled
manner, without first sending SLEEP or POWEROFF_NOTIFY.
The regulator_init_complete late initcall, may cut the VCC
regulator if it's reference counter is zero. To be able to
prevent the regulator from being cut, mmc_start_host, which
should execute at device init and thus before late init,
calls mmc_power_up. Then the host driver is able to increase
the reference to the regulator.

Currently only 1.2V is treated for HS200 mode. If the host has only
1.8V I/O capability not 1.2V, mmc_set_signal_voltage can't be called
for 1.8V HS200. EXT_CSD_CARD_TYPE_SDR_1_8V needs to be considered.

Current implementation decides the card type exclusively. Even though
eMMC device can support both HS200 and DDR mode, card type will be
set only for HS200. If the host doesn't support HS200 but has DDR
capability, then DDR mode can't be selected.

Commit 06e8935feb ("optimized SDIO IRQ handling for single irq")
introduced some spurious calls to SDIO function interrupt handlers,
such as when the SDIO IRQ thread is started, or the safety check
performed upon a system resume. Let's add a flag to perform the
optimization only when a real interrupt is signaled by the host
driver and we know there is no point confirming it.

MMC bus is using legacy suspend/resume method, which is not compatible if
runtime pm callbacks are used. In this scenario, MMC bus suspend/resume
callbacks cannot be called when system entering S3. So change to use the
new defined dev_pm_ops for system sleeping mode.
Tested on AM335x Platform. Solves major issue/crash reported at

eMMC v4.5 sanitize operation erases all copies of unmapped
data. However trim or erase operations must be used first
to unmap the required sectors. That was not being done.

eMMC v4.5 discard operation is significantly different from the
existing trim operation because it is not guaranteed to work with
the new sanitize operation. Consequently mmc_can_trim() is
separated from mmc_can_discard().
Also the new discard operation does not result in the sectors being
set to all-zeros, so discard_zeroes_data must not be set.
In addition, the new discard has the same timeout as trim, but from
v4.5 trim is defined to use the hc timeout. The timeout calculation
is adjusted accordingly.

Several people have noticed that crappy SD cards take much longer to
complete multiple block writes than the 300ms that Linux specifies.
Try to work around this by using a three second write timeout instead.
This is a generalized version of a patch from Chase Maupin
<Chase.Maupin@ti.com>, whose patch description said:
* With certain SD cards timeouts like the following have been seen
due to an improper calculation of the dto value:
mmcblk0: error -110 transferring data, sector 4126233, nr 8,
card status 0xc00
* By removing the dto calculation and setting the timeout value
to the maximum specified by the SD card specification part A2
section 2.2.15 these timeouts can be avoided.
* This change has been used by beagleboard users as well as the
Texas Instruments SDK without a negative impact.

mmc_select_powerclass() function returns error if eMMC
VDD level supported by host is between 2.7v to 3.2v.
According to eMMC specification, valid voltage for high
voltage cards is 2.7v to 3.6v. This patch ensures that
2.7v to 3.6v VDD range is treated as valid range.
Also, failure to set the power class shouldn't be treated
as fatal error because even if setting the power class
fails, card can still work in default power class.
If mmc_select_powerclass() returns error, just print
the warning message and go ahead with rest of the card
initialization.

Neil Brown reports that commit 35cd133c
PM: Run the driver callback directly if the subsystem one is not there
breaks suspend for his libertas wifi, because SDIO has a protocol
where the suspend method can return -ENOSYS and this means "There is
no point in suspending, just turn me off". Moreover, the suspend
methods provided by SDIO drivers are not supposed to be called by
the PM core or bus-level suspend routines (which aren't presend for
SDIO). Instead, when the SDIO core gets to suspend the device's
ancestor, it calls the device driver's suspend function, catches the
ENOSYS, and turns the device off.
The commit above breaks the SDIO core's assumption that the device
drivers' callbacks won't be executed if it doesn't provide any
bus-level callbacks. If fact, however, this assumption has never
been really satisfied, because device class or device type suspend
might very well use the driver's callback even without that commit.
The simplest way to address this problem is to make the SDIO core
tell the PM core to ignore driver callbacks, for example by providing
no-operation suspend/resume callbacks at the bus level for it,
which is implemented by this change.

Someone could use send_hpi_cmd() on a card that doesn't advertise support
for HPI. Then maybe didn't work fine. Because card->ext_csd.hpi_cmd
didn't set. So if card didn't support hpi, return the warning message.
And CMD12's flags is MMC_RSP_R1B.

This patch fixes following issues when HS200 is enabled:
1. If executing_tuning() host ops is called without mmc_host_clk_hold(),
card clocks might get turned off (if MMC_CLK_GATING is enabled)
while execute_tuning() is in progress. So this patch makes sure
that execute_tuning() is called with mmc_host_clk_hold().
2. If host timing mode is set to HS200 mode, there should not be
any communication with the card until execute_tuning() is completed.
But there is a chance that CMD6 might be sent to enable set HPI_EN
(of HPI_MGMT field in EXT_CSD) before execute_tuning() is called.
So this patch moves this operation after execute_tuning() is completed.

To prevent I/O as soon as possible at card removal, a new detect work is
re-scheduled without a delay to let a rescan remove the card device as
soon as possible.
Additionally, MMC_CAP2_DETECT_ON_ERR can now be used to handle "slowly"
removed cards that a scheduled detect work did not detect as removed.
To prevent further I/O requests for these lingering removed cards,
check if card has been removed and then schedule a detect work to
properly remove it.

Even if cards supports 1.8V I/O voltage those should anyway be
initialized at 3.3V I/O according to (e)MMC, SD and SDIO specs.
Some eMMC and embedded SDIO devices are able to be initialized
at 1.8V as well, but it is better to be safe.
Do note that initialization in this context means that the card
has been completely powered off, otherwise the card will remain
at the last I/O voltage level that were negotitiated.
Due to the above being taken care of the suspend/resume issues
for UHS-I SD-cards has been fixed.

A recent commit "mmc: core: Use delayed work in clock gating framework"
(597dd9d79c) introduced a default 200ms delay before clock gating
actually takes place. This means that every time an MMC interface
becomes idle it first stays on for 200ms before gating its clock. This
leads to increased power consumption and is therefore a clear regression.
This patch restores the original behaviour by setting the default delay
to 0. Users prioritising throughput over power efficiency can still
modify the delay via sysfs.

Modified the mmc_poweroff to resume before sending the poweroff
notification command. In sleep mode only AWAKE and RESET commands are
allowed, so before sending the poweroff notification command resume from
sleep mode and then send the notification command.
PowerOff Notify is tested on a Synopsis Designware Host Controller
(eMMC 4.5). The suspend to RAM and resume works fine.

There is an understood mismatch between the voltage the host controller is
set to and the voltage supplied to the card by a fixed voltage regulator.
Teaching the driver to accept the mismatch is overly complicated. Instead
just accept the regulator's voltage.
This patch adds MMC_CAP2_BROKEN_VOLTAGE.
If the voltage didn't satisfy between min_uV and max_uV, try to change
the voltage in core.c. When changing the voltage, maybe use
regulator_set_voltage().
In regulator_set_voltage(), check the below condition.

If some board should use the fixed-regulator, always return -EINVAL.

A UHS sdio card that fails initialization at 1.8v signaling is not in
UHS mode. We cannot use the speed in the the cis to reflect the bus
speed as this is the maxiumum value and will not reflect the fact
that the host is operating at a lower (non uhs) bus speed.

Fix up conflicts in scripts/mod/file2alias.c due to the new linker-

Performing MMC block IO with simultaneous STR can lead to a deadlock: the
mmc_pm_notify() function claims the host and then calls bus .remove()
method, which lands in mmc_blk_remove(), which calls mmc_blk_remove_req()
then it goes to -> mmc_cleanup_queue() -> kthread_stop(), which waits for
the mmc-block thread to stop. If the mmc-block thread at that time is
processing block requests, it will also try to claim the host in
mmc_blk_issue_rq() and block there. This patch fixes the problem by
calling .remove() before claiming the host.

Turning the cache off implies flushing cache which doesn't define
maximum timeout unlike cache-on. This patch will apply the generic
CMD6 timeout only for cache-on. Additionally the kernel message is
added for checking failure case of cache-on.

Host may now use MMC_CAP2_NO_SLEEP_CMD to disable the use
of eMMC sleep/awake command.
This option can be used when your platform has a buggy
kernel crash dump software, which is supposed to store
the dump on the eMMC, but is not able to wake up the eMMC
from sleep state.
In particular, failures have been seen with u-boot; even if
it is fixed there, platforms will be slow to update their
bootloader binaries.

This patch fixes the wrong comparison before setting the interface
voltage in DDR mode.
The assignment to the variable ddr before comaprison is either
ddr = MMC_1_2V_DDR_MODE; or ddr == MMC_1_8V_DDR_MODE. But the comparison
is done with the extended csd value if ddr == EXT_CSD_CARD_TYPE_DDR_1_2V.

Add a function mmc_detect_card_removed() which upper layers can use to
determine immediately if a card has been removed. This function should
be called after an I/O request fails so that all queued I/O requests
can be errored out immediately instead of waiting for the card device
to be removed.

This patch adds support for sdio UHS cards per the version 3.0
spec.
UHS mode is only enabled for version 3.0 cards when both the
host and the controller support UHS modes.
1.8v signaling support is removed if both the card and the
host do not support UHS. This is done to maintain
compatibility and some system/card combinations break when
1.8v signaling is enabled when the host does not support UHS.

Current clock gating framework disables the MCI clock as soon as the
request is completed and enables it when a request arrives. This aggressive
clock gating framework, when enabled, cause following issues:
When there are back-to-back requests from the Queue layer, we unnecessarily
end up disabling and enabling the clocks between these requests since 8MCLK
clock cycles is a very short duration compared to the time delay between
back to back requests reaching the MMC layer. This overhead can effect the
overall performance depending on how long the clock enable and disable
calls take which is platform dependent. For example on some platforms we
can have clock control not on the local processor, but on a different
subsystem and the time taken to perform the clock enable/disable can add
significant overhead.
Also if the host controller driver decides to disable the host clock too
when mmc_set_ios function is called with ios.clock=0, it adds additional
delay and it is highly possible that the next request had already arrived
and unnecessarily blocked in enabling the clocks. This is seen frequently
when the processor is executing at high speeds and in multi-core platforms
thus reduces the overall throughput compared to if clock gating is
disabled.
Fix this by delaying turning off the clocks by posting request on
delayed workqueue. Also cancel the unscheduled pending work, if any,
when there is access to card.
sysfs entry is provided to tune the delay as needed, default
value set to 200ms.

This patch is to expose the actual SDCLK frequency in
/sys/kernel/debug/mmcX/ios entry.
For example, if the max clk for a normal speed card is 20MHz this
is reported in /sys/kernel/debug/mmcX/ios. Unfortunately the actual
SDCLK frequency (i.e. Baseclock / divisor) is not reported at all:
for example, in that case, on Arasan HC, it should be 48/4=12 (MHz).

This patch allows any block size to be set on the SDIO link,
and still have an arbitrary sized packet (adjusted in size by
using sdio_align_size) transferred in an optimal way
(preferably one transfer).
Previously if the block size was larger than the default of
512 bytes and the transfer size was exactly one block size
(possibly thanks to using sdio_align_size to get an optimal
transfer size), it was sent as a number of byte transfers instead
of one block transfer. Also if the number of blocks was
(max_blocks * N) + 1, the tranfer would be conducted with a number
of blocks and finished off with a number of byte transfers.
When doing this change it was also possible to break out the quirk
for broken byte mode in a much cleaner way, and collect the logic of
when to do byte or block transfer in one function instead of two.

When SDIO runtime PM was originally introduced, we immediately faced
two regressions with two different chipsets, and in response decided
not to enable it by default.
With the recent work on the 8686 we hoped we found all the gotchas,
so 08da834 did make sense (at least experimentally).
Unfortunately we now see that some setups out there still refuse to
work when SDIO runtime PM is enabled by default (see
http://www.spinics.net/lists/linux-mmc/msg11161.html), and obviously
we can't live with these kind of regressions.
This reverts commit 08da834a24.

mmc_suspend_host() tries to claim host during suspend
and release it only when the bus suspend operation is
compeleted. If CONFIG_MMC_UNSAFE_RESUME is defined and
the host is flagged as removable, mmc_suspend_host()
tries to remove the card. In this process, the file system
sync can get blocked trying to acquire host which is already
claimed by mmc_suspend_host() causing deadlock.
Fix this deadlock by releasing host before ->remove() is called.

The eMMC 4.5 devices respond to only RESET and AWAKE command in the
sleep state. Hence the mmc switch command to notify power off state
should be sent before the device enters sleep state.

Adds a quirk that sets the data read timeout to a fixed value instead
of relying on the information in the CSD. The timeout value chosen
is 300ms since that has proven enough for the problematic cards found,
but could be increased if other cards require this.
This patch also enables this quirk for certain Micron cards known to
have this problem.

While trying to suspend the mmc host there could still be
ongoing requests that we need to wait for. At the same time
a device driver must respond to a suspend request rather quickly.
Instead of potentially wait "forever" by claiming the host we now
"try" to claim the host instead. If it fails, -EBUSY is returned.

Here is Essential conditions to indicate Version 3.00 Card
(SD_SPEC=2 and SD_SPEC3=1) :

(4) User area capacity shall be up to 2GB (SDSC) or 32GB (SDHC)

So even if SD card doesn't support any of the newly defined
UHS-I bus speed mode, it can advertise itself as SD3.0 cards
as long as it supports all the essential conditions of
SD3.0 cards. Given this, these type of cards should atleast
run in High Speed mode @50MHZ if it supports HS.
But current initialization sequence for SD3.0 cards is
such that these non-UHS-I SD3.0 cards runs in Default
Speed mode @25MHz.
This patch makes sure that these non-UHS-I SD3.0 cards run
in High Speed Mode @50MHz.
Tested this patch with SanDisk Extreme SDHC 8GB Class 10 card.

This patch adds support for the power off notify feature, available in
eMMC 4.5 devices. If the host has support for this feature, then the
mmc core will notify the device by setting the POWER_OFF_NOTIFICATION
byte in the extended csd register with a value of 1 (POWER_ON).
For suspend mode short timeout is used, whereas for the normal poweroff
long timeout is used.

EXT_CSD[248] includes the default maximum timeout for CMD6.
This field is added at eMMC4.5 Spec. And it can be used for default
timeout except for some operations which don't define the timeout
(i.e. background operation, sanitize, flush cache) in eMMC4.5 Spec.

mmc_request_done() is sometimes called from interrupt or other atomic
context. Mostly all mmc_request_done() does is complete(), however it
contains code to retry on error, which uses ->request(). As the error
path is certainly not performance critical, this may be moved to the
waiting function mmc_wait_for_req_done().
This allows ->request() to use runtime PM get_sync() and guarantee it
is never in an atomic context.

This patch adds the power class selection feature available for mmc
versions 4.0 and above. During the enumeration stage before switching
to the lower data bus, check if the power class is supported for the
current bus width. If the power class is available then switch to the
power class and use the higher data bus. If power class is not supported
then switch to the lower data bus in a worst case.

For cards that support hardware reset (just eMMC), try a reset and
retry before returning an I/O error. However this is not done for
ECC errors and is never done twice for the same operation type
(READ, WRITE, DISCARD, SECURE DISCARD) until that type of operation
again succeeds.

eMMC's may have a hardware reset line. This patch provides a
host controller operation to implement hardware reset and
a function to reset and reinitialize the card. Also, for MMC,
the reset is always performed before initialization.
The host must set the new host capability MMC_CAP_HW_RESET
to enable hardware reset.

Earlier all cards where initiated with bus mode set as OPENDRAIN, and then
later switched to PUSHPULL. According to the MMC/SD/SDIO specifications
only MMC cards use OPENDRAIN during init. For both SD and SDIO the bus
mode shall be PUSHPULL before attempting to init the card.
The consequence of having incorrect bus mode can lead to not being able
to detect the card. Therefore the default behavior have now been changed
to PUSHPULL in mmc_power_up, and will only be temporarily switched when
trying to attach or init a MMC card.

Adds a quirk which can be turned on for SDIO devices that do not support
512 byte requests in byte mode during CMD53. These requests will always
be sent in block mode instead.
This patch also enables this quirk for ST-Ericsson CW1200 WLAN device.

During a rescan operation mmc_attach(sd|mmc|sdio) functions are
called. The error handling in these function can trigger a detach
of the bus, which also meant a power off. This is not notified by
the rescan operation which then continues to the next attach function.
If a power off has been done, the framework must never send any
new commands to the host driver, without first doing a new power up.
This will most likely trigger any host driver to hang.
Moving power off out of detach and instead handle power off
separately when it is actually needed, solves the issue.

Put MMC to sleep if it supports SLEEP/AWAKE (CMD5) in the mmc suspend
so that Vcc (NAND core) can be cut to minimize power consumption.
eMMC put into SLEEP can respond to CMD0 or H/W reset or CMD5.
Current implemention on resume from suspend relies on CMD0 in
mmc_init_card to get out of SLEEP mode.

Stress-testing the runtime power management of libertas_sdio
through a rmmod/insmod loop revealed that it is quite easy to
cause an ETIMEDOUT failure in mmc_sdio_power_restore() leading to:
libertas_sdio: probe of mmc1:0001:1 failed with error -16
Experimentation shows that a very short delay (100us) is needed in
the power down path before the card can be successfully booted again.
We know that this setup is lacking poweroff clamps on the card's power
lines, but as only a short delay is needed, apply this unconditionally.
Also bump up to 1ms sleep for extra legroom.

Now that we have improved the runtime power management powerup/powerdown
code, we believe that MMC_CAP_POWER_OFF_CARD is no longer necessary:
runtime PM should now work everywhere.
The only hard evidence for introducing MMC_CAP_POWER_OFF_CARD was the
Marvell sd8686 wifi chip, which was believed to require external gpio
manipulation which wasn't supported by some boards.
After further investigation it was realized (and confirmed by Marvell
folks) that sd8686 requirements can be fulfilled by changing the reset
sequence itself, even if no external gpio is manipulated.

Enable this trivially for a release or two. If no problems are reported,
we will follow up with a more extensive patch to remove this flag
altogether. If problems are reported, we can look at whitelist/blacklist

This adds support to inject data errors after a completed host transfer.
The mmc core will return error even though the host transfer is successful.
This simple fault injection proved to be very useful to test the
non-blocking error handling in the mmc_blk_issue_rw_rq().
Random faults can also test how the host driver handles pre_req()
and post_req() in case of errors.

mmc_sd_init_uhs_card function sets the driver type, current limit
and bus speed mode on card as well as on host controller side.
Currently bus speed mode is set by sending CMD6 to card and
immediately setting the timing mode in host controller. But
then before initiating tuning sequence, it also tries to set
current limit by sending CMD6 to card which results in data
timeout errors in controller if bus speed mode is SDR50/SDR104 mode.
So basically bus speed mode should be set only after current limit
is set in the card and immediately after setting the bus speed mode,
tuning sequence should be initiated.

The default multithread workqueue can cause the same work to be executed
concurrently on a different CPUs. This isn't really suitable for clock
gating as it might already gated the clock and gating it twice results both
host->clk_old and host->ios.clock to be set to 0.
To prevent this from happening we use system_nrt_wq instead.

We have seen at least two different races when clock gating kicks in in a
middle of ios structure update.
First one happens when ios->clock is changed outside of aggressive clock
gating framework, for example via mmc_set_clock(). The race might happen
when we run following code:
mmc_set_ios():

mmc_set_ungated(host);
Now if gating kicks in right after the condition check we end up setting
host->clk_gated to false even though we have just gated the clock. Next
time a request is started we try to ungate and restore the clock in
mmc_host_clk_hold(). However since we have host->clk_gated set to false the
original clock is not restored.
This eventually will cause the host controller to hang since its clock is
disabled while we are trying to issue a request. For example on Intel
Medfield platform we see:

This BUG() comes from the fact that ios.power_mode was still in previous
value (MMC_POWER_ON) and ios.vdd was set to zero.
We prevent these by inhibiting the clock gating while we update the ios
structure.
Both problems can be reproduced by simply running the device in a reboot
loop.

fixed a bug where mmc's runtime power management functions were not being
called. We have now also made improvements to the SDIO powerup routine
which could possibly mask this kind of issue in future.
Add debug messages to the runtime PM hooks so that it is easy to verify
if and when runtime PM is happening.

In the case where a driver returns -ENOSYS from its suspend handler
to indicate that the device should be powered down over suspend, the
remove routine of the driver was not being called, leading to lots of
confusion during resume.
The problem is that runtime PM is disabled during this process,
and when we reach mmc_sdio_remove, calling the runtime PM functions here
(validly) return errors, and this was causing us to skip the remove
function.
Fix this by ignoring the error value of pm_runtime_get_sync(), which
can return valid errors. This also matches the behaviour of
pci_device_remove().

Non default Drive Strength cannot be set automatically. It is a function
of the board design and only if there is a specific platform handler can
it be set. The platform handler needs to take into account the board
design. Pass to the platform code the necessary information.
For example: The card and host controller may indicate they support HIGH
and LOW drive strength. There is no way to know what should be chosen
without specific board knowledge. Setting HIGH may lead to reflections
and setting LOW may not suffice. There is no mechanism (like ethernet
duplex or speed pulses) to determine what should be done automatically.
If no platform handler is defined -- use the default value.

Previously there has only been one function mmc_wait_for_req()
to start and wait for a request. This patch adds:
* mmc_start_req() - starts a request wihtout waiting
If there is on ongoing request wait for completion
of that request and start the new one and return.
Does not wait for the new command to complete.
This patch also adds new function members in struct mmc_host_ops
only called from core.c:
* pre_req - asks the host driver to prepare for the next job
* post_req - asks the host driver to clean up after a completed job
The intention is to use pre_req() and post_req() to do cache maintenance
while a request is active. pre_req() can be called while a request is
active to minimize latency to start next job. post_req() can be used after
the next job is started to clean up the request. This will minimize the
host driver request end latency. post_req() is typically used before
ending the block request and handing over the buffer to the block layer.
Add a host-private member in mmc_data to be used by pre_req to mark the
data. The host driver will then check this mark to see if the data is
prepared or not.

Some host controllers will not operate without a hardware
timeout that is limited in value. However large discards
require large timeouts, so there needs to be a way to
specify the maximum discard size.
A host controller driver may now specify the maximum discard
timeout possible so that max_discard_sectors can be calculated.
However, for eMMC when the High Capacity Erase Group Size
is not in use, the timeout calculation depends on clock
rate which may change. For that case Preferred Erase Size
is used instead.

On reading the ext_csd for the first time (in 1 bit mode), save the
ext_csd information needed for bus width compare.
On every pass we make re-reading the ext_csd, compare the data
against the saved ext_csd data.
This fixes a regression introduced in 3.0-rc1 by 08ee80cc39
("mmc: core: eMMC bus width may not work on all platforms"), which
incorrectly assumed we would be re-reading the ext_csd at resume-
time.

mmc_sdio_power_restore() skips some steps that are performed in other
power-related codepaths which are necessary to fully reset the card.
Without this, runtime PM fails for SD8686 SDIO wifi on OLPC XO-1.5.

After commit e1866b3 "PM / Runtime: Rework runtime PM handling
during driver removal" was introduced, the driver core stopped
incrementing the runtime PM usage counter of the device during
the invocation of the ->remove() callback.
This indirectly broke SDIO's runtime PM path during driver removal,
because no one calls _put_sync() anymore after ->remove() completes.
This means that the power of runtime-PM-managed SDIO cards is kept
high after their driver is removed (even if it was powered down
beforehand).
Fix that by directly calling _put_sync() when the last usage
counter is downref'ed by the SDIO bus.

CMD19 -- The offical way to validate bus widths from the JEDEC spec
does not work on all platforms. Some platforms that use PCI/PCIe
to connect their SD controllers are known to fail.
If the quirk MMC_BUS_WIDTH_TEST is not defined we try to figure out
the bus width by reading the ext_csd at different bus widths and
compare this against the ext_csd read in 1 bit mode. If no ext_csd
is available we default to 1 bit operations.
Code has been tested on mmp2 against 8 bit eMMC and Transcend 2GB
card that is known to not work in 4 bit mode. The physical pins
on the card are not present to support 4 bit operation.

mmc_rescan_try_freq() tries to init two times with the last frequency.
For example, if host->f_min is 400KHz, we see the message below:

Andy Ross says that he didn't try this code on a board with an f_min
that exactly matches one of the table entries, which explains why the

Since the MMC_PM_KEEP_POWER flag should be set on each suspend,
it should also cleared on each resume.
Upon resuming, we have to know if power was kept
(for re-initialization, etc.), so clear it just after resuming.

According to the Host Controller spec v3.00, setting Preset Value Enable
in the Host Control2 register lets SDCLK Frequency Select, Clock Generator
Select and Driver Strength Select to be set automatically by the Host
Controller based on the UHS-I mode set. This patch enables this feature.
Since Preset Value Enable makes sense only for UHS-I cards, we enable this
feature after successfull UHS-I initialization. We also reset Preset Value
Enable next time before initialization.
Tested by Zhangfei Gao with a Toshiba uhs card and general hs card,
on mmp2 in SDMA mode.

Host Controller needs tuning during initialization to operate SDR50
and SDR104 UHS-I cards. Whether SDR50 mode actually needs tuning is
indicated by bit 45 of the Host Controller Capabilities register.
A new command CMD19 has been defined in the Physical Layer spec
v3.01 to request the card to send tuning pattern.
We enable Buffer Read Ready interrupt at the very begining of tuning
procedure, because that is the only interrupt generated by the Host
Controller during tuning. We program the block size to 64 in the
Block Size register. We make sure that DMA Enable and Multi Block
Select in the Transfer Mode register are set to 0 before actually
sending CMD19. The tuning block is sent by the card to the Host
Controller using DAT lines, so we set Data Present Select (bit 5) in
the Command register. The Host Controller is responsible for doing
the verfication of tuning block sent by the card at the hardware
level. After sending CMD19, we wait for Buffer Read Ready interrupt.
In case we don't receive an interrupt after the specified timeout
value, we fall back on fixed sampling clock by setting Execute
Tuning (bit 6) and Sampling Clock Select (bit 7) of Host Control2
register to 0. Before exiting the tuning procedure, we disable Buffer
Read Ready interrupt and re-enable other interrupts.

Since only UHS-I cards respond with S18A set in response to ACMD41,
we set the card as ultra-high-speed after successfull initialization.
We need to decide whether a card is SDXC based on the C_SIZE field
of CSDv2.0 register. According to Physical Layer spec v3.01, the
minimum value of C_SIZE for SDXC card is 00FFFFh.

We decide on the current limit to be set for the card based on the
Capability of Host Controller to provide current at 1.8V signalling,
and the maximum current limit of the card as indicated by CMD6
mode 0. We then set the current limit for the card using CMD6 mode 1.
As per the Physical Layer Spec v3.01, the current limit switch is
only applicable for SDR50, SDR104, and DDR50 bus speed modes. For
other UHS-I modes, we set the default current limit of 200mA.

This patch adds support for setting UHS-I bus speed mode during UHS-I
initialization procedure. Since both the host and card can support
more than one bus speed, we select the highest speed based on both of
their capabilities. First we set the bus speed mode for the card using
CMD6 mode 1, and then we program the host controller to support the
required speed mode. We also set High Speed Enable in case one of the
UHS-I modes is selected. We take care to reset SD clock before setting
UHS mode in the Host Control2 register, and then re-enable it as per
the Host Controller spec v3.00. We then set the clock frequency for
the UHS-I mode selected.

This patch adds support for setting driver strength during UHS-I
initialization procedure. Since UHS-I cards set S18A (bit 24) in
response to ACMD41, we use this as a base for UHS-I initialization.
We modify the parameter list of mmc_sd_get_cid() so that we can
save the ROCR from ACMD41 to check whether bit 24 is set.
We decide whether the Host Controller supports A, C, or D driver
type depending on the Capabilities register. Driver type B is
suported by default. We then set the appropriate driver type for
the card using CMD6 mode 1. As per Host Controller spec v3.00, we
set driver type for the host only if Preset Value Enable in the
Host Control2 register is not set. SDHCI_HOST_CONTROL has been
renamed to SDHCI_HOST_CONTROL1 to conform to the spec.

SD cards which conform to Physical Layer Spec v3.01 can support
additional Bus Speed Modes, Driver Strength, and Current Limit
other than the default values. We use CMD6 mode 0 to read these
additional card functions. The values read here will be used
during UHS-I initialization steps.

Host Controller v3.00 adds another Capabilities register. Apart
from other things, this new register indicates whether the Host
Controller supports SDR50, SDR104, and DDR50 UHS-I modes. The spec
doesn't mention about explicit support for SDR12 and SDR25 UHS-I
modes, so the Host Controller v3.00 should support them by default.
Also if the controller supports SDR104 mode, it will also support
SDR50 mode as well. So depending on the host support, we set the
corresponding MMC_CAP_* flags. One more new register. Host Control2
is added in v3.00, which is used during Signal Voltage Switch
procedure described below.
Since as per v3.00 spec, UHS-I supported hosts should set S18R
to 1, we set S18R (bit 24) of OCR before sending ACMD41. We also
need to set XPC (bit 28) of OCR in case the host can supply >150mA.
This support is indicated by the Maximum Current Capabilities
register of the Host Controller.
If the response of ACMD41 has both CCS and S18A set, we start the
signal voltage switch procedure, which if successfull, will switch
the card from 3.3V signalling to 1.8V signalling. Signal voltage
switch procedure adds support for a new command CMD11 in the
Physical Layer Spec v3.01. As part of this procedure, we need to
set 1.8V Signalling Enable (bit 3) of Host Control2 register, which
if remains set after 5ms, means the switch to 1.8V signalling is
successfull. Otherwise, we clear bit 24 of OCR and retry the
initialization sequence. When we remove the card, and insert the
same or another card, we need to make sure that we start with 3.3V
signalling voltage. So we call mmc_set_signal_voltage() with
MMC_SIGNAL_VOLTAGE_330 set so that we are back to 3.3V signalling
voltage before we actually start initializing the card.

Allows appropriately-privileged applications to send CMD (normal) and ACMD
(application-specific; preceded with CMD55) commands to cards/devices on
the mmc bus. This is primarily useful for enabling the security
functionality built in to every SD card.
It can also be used as a generic passthrough (e.g. to enable virtual
machines to control mmc bus devices directly). However, this use case has
not been tested rigorously. Generic passthrough testing was only conducted
for a few non-security opcodes to prove the feasibility of the passthrough.
Since any opcode can be sent using this passthrough, it is very possible to
render the card/device unusable. Applications that use this ioctl must
have CAP_SYS_RAWIO.

TI OMAP3621/OMAP3630 SoC, Samsung S5PC110 SoC, Qualcomm MSM7200A SoC.

The wl12xx device supports disconnecting the pull-up resistor on
CD/DAT[3] (pin 1) of the card.
Tell SDIO core to disconnect that resistor during card init,
since we don't need it at that point (and anyway all
hosts shall provide pull-up resistors on all data lines DAT[3:0]
as described in section 6 of the SD physical specification).
As a result, this may save some power, but it's also generally healthy
since it prevents both ends from pulling up that pin, which
results in undesirable asymmetric physical bus.

Introduce MMC_QUIRK_NONSTD_FUNC_IF to ignore the "SDIO Standard Function
interface code" as indicated by the card's FBR, and instead treat all
functions as non-standard interfaces.
This is required to prevent standard drivers from facing
errors when trying to communicate with SDIO cards that erroneously
indicate standard function interface codes.

6b5eda36 followed SDIO spec part E1 section 8, which states that
in case SDIO interrupts are being used to wake up a suspended host,
then it is required to switch to 1-bit mode before stopping the clock.
Before switching to 1-bit mode (or back to 4-bit mode on resume),
make sure that SDIO interrupts are really being used to wake the host.
This is helpful for devices which have an external irq line (e.g.
wl1271), and do not use SDIO interrupts to wake up the host.
In this case, switching to 1-bit mode (and back to 4-bit mode on resume)
is not necessary.

Currently there is a race in the MMC core between a card-detect
rescan work and the clock-gating work, scheduled from a command
completion. Fix it by removing the dedicated clock-gating mutex
and using the MMC standard locking mechanism instead.

At power off, reset OCR mask to be the highest possible voltage
supported for the current mmc host.
This solves the re-initialization during the power up sequence.
The voltage may have been decreased due to the card accepts a lower
voltage than the voltage used during the initialization sequence.
We need to reset the voltage to by the host highest possible value
since according to specification the initialization must always be
done at high voltage.

During redetection of a SDIO card, a request for a new card RCA
was submitted to the card, but was then overwritten by the old RCA.
This caused the card to be deselected instead of selected when using
the incorrect RCA. This bug's been present since the "oldcard"
handling was introduced in 2.6.32.

Some TOSHIBA MMC cards only support sector addressing even though the
size is < 2GB. According to JEDEC Spec JESD84-A441-1 the ocr register
(bits 30, 29) determine byte/sector mode. Use them.

sdio_reset sends a CMD52 to reset the sdio card. This is highly
recommended for sdio cards being reinitialized. Since we do not
know if the card is being reinitialized we just send the command.
SD/eMMC cards are supposed to ignore the CMD before the CMD0.
Document why we are doing this.

Some sdio card are not following sdio standard, and do not work
when the sdio bus's clock is gated.
To keep functionnality for all legacy driver, we turn this quirk on
for every sdio card.
Drivers needs to disable the quirk manually when someone verifies that
their supported card works with clock gating.

Some cards have quirks valid for every platforms using current
platform quirk hooks leads to a lot of code and debug duplication.
So we inspire a bit from what exists in PCI subsystem and do our own
per vendorid/deviceid quirk. We still drop the complexity of the pci
quirk system (with special section tables, and so on).
That can be added later if needed.

Enhanced area feature is a new feature defined in eMMC4.4 standard. This
user data area provides higher performance/reliability, at the expense
of using twice the effective media space due to the area using SLC.
The MMC driver now reads out the enhanced area offset and size and adds
them to the device attributes in sysfs. Enabling the enhanced area can
only be done once, and should be done in manufacturing. To use this
feature, bit ERASE_GRP_DEF should also be set.
Documentation/ABI/testing/sysfs-devices-mmc describes the two new
attributes.

30201e7f3 ("mmc: skip detection of nonremovable cards on rescan")
allowed skipping detection of nonremovable cards on mmc_rescan().
The intention was to only skip detection of hardwired cards that
cannot be removed, so make sure this is indeed the case by directly
checking for (lack of) MMC_CAP_NONREMOVABLE, instead of using
mmc_card_is_removable(), which is overloaded with
CONFIG_MMC_UNSAFE_RESUME semantics.
The user-visible symptom of the bug this patch fixes is that no
"mmc: card XXXX removed" message appears in dmesg when a card is
removed and CONFIG_MMC_UNSAFE_RESUME=y.

This fixes a bug introduced by 807e8e4067 ("mmc: Fix sd/sdio/mmc
initialization frequency retries") that prevented SDIO drivers from
performing SDIO commands in their probe routines -- the above patch
called mmc_claim_host() before sdio_add_func(), which causes a deadlock
if an external SDIO driver calls sdio_claim_host().
Fix tested on an OLPC XO-1.75 with libertas on SDIO.

The card is not always clocked and the clock frequency zero is perfectly
legal, thus this code in mmc_set_data_timeout() may cause a division by
zero. It will be triggered more often if you're using software clock
gating but can be triggered under other conditions too.

Rewrite and clean up mmc_rescan() to properly retry frequencies lower
than 400kHz. Failures can happen both in sd_send_* calls and
mmc_attach_*. Break out "mmc_rescan_try_freq" from the frequency
selection loop. Symmetrize claim/release logic in mmc_attach_* API,
and move the sd_send_* calls there to make mmc_rescan easier to read.

With the bus-width test patch, mmc_set_bus_width*() isn't called properly
when the driver doesn't set MMC_CAP_BUS_WIDTH and no DDR mode.
This patch fixes the regression by moving the call up before the cap test.

Some old MMC devices fail with the 4/8 bits the driver tries to use
exclusively. This patch adds a test for the given bus setup and falls
back to the lower bit mode (until 1-bit mode) when the test fails.

Upon system resume, SDIO core must reinitialize cards that were
powered off during suspend.
If the card had its power kept during suspend (and thus it is
'powered-resumed'), SDIO core performs only a limited reinitializing,
mainly needed to make sure that the card wasn't removed/replaced.
If a __nonremovable__ card is powered-resumed, we can safely skip the
reinitializing phase.
Note: 9b966aa (mmc: sdio: fully reconfigure oldcard on resume) removed
the bus width reconfiguration since mmc_sdio_init_card already does it.
It is brought back now in case mmc_sdio_init_card is skipped.

Initial SDIO runtime PM implementation took a conservative approach
of powering up cards (and fully reinitializing them) on system suspend,
just before the suspend handlers of the relevant drivers were executed.
To avoid redundant power and reinitialization cycles, this patch removes
this behavior: if a card is already powered off when system suspend kicks
in, it is left at that state.
If a card is active when a system sleep starts, everything is
straightforward and works exactly like before. But if the card was
already suspended before the sleep began, then when the MMC core powers
it back up on resume, its run-time PM status has to be updated to reflect
the actual post-system sleep status.
The technique to do that is borrowed from the I2C runtime PM
implementation (for more info see Documentation/power/runtime_pm.txt).

mmc_rescan() checks whether registered cards are still present before
skipping them, by calling the bus-specific ->detect() handler.
With buses that support runtime PM, the card may be powered off at
this point, so they need to be powered on and fully reinitialized before
->detect() executes.
This whole process is redundant with nonremovable cards; in those cases,
we can safely skip calling ->detect() and implicitly assume its success.

JMicron 388 SD/MMC combo controller supports the 1.8V low-voltage for
SD, but MMC doesn't work with the low-voltage, resulting in an error
at probing.
This patch adds the support for multiple voltage mask per device type,
so that SD works with 1.8V while MMC forces 3.3V. Here new ocr_avail_*
fields for each device are introduced, so that the actual OCR mask is
switched dynamically.
Also, the restriction of low-voltage in core/sd.c is removed when the
bit is allowed explicitly via ocr_avail_sd mask.
This patch was rewritten from scratch based on Aries' original code.

This patch modifies the MMC core code to optionally call the set_ios()
operation on the driver with the clock frequency set to 0 (gate) after
a grace period of at least 8 MCLK cycles, then restore it (ungate)
before any new request. This gives the driver the option to shut down
the MCI clock to the MMC/SD card when the clock frequency is 0, i.e.
the core has stated that the MCI clock does not need to be generated.
It is inspired by existing clock gating code found in the OMAP and
Atmel drivers and brings this up to the host abstraction. Gating is
performed before and after any MMC request.
This patchset implements this for the MMCI/PL180 MMC/SD host controller,
but it should be simple to switch OMAP/Atmel over to using this instead.
mmc_set_{gated,ungated}() add variable protection to the state holders
for the clock gating code. This is particularly important when ordinary
.set_ios() calls would race with the .set_ios() call resulting from a
delayed gate operation.

In the error-path where PM notifies PM_POST_RESTORE, the rescan-blockage
should be cleared as well. Otherwise it'll be never re-probed.
Also, as a bonus, this fixes a bug in S4 with user-mode suspend in the
current code, as it sends PM_POST_RESTORE instead of
PM_POST_HIBERNATION wrongly.

Some board/card/host configurations are not capable of powering off the
card after boot.
To support such configurations, and to allow smoother transition to
runtime PM behavior, MMC_CAP_POWER_OFF_CARD is added, so hosts need to
explicitly indicate whether it's OK to power off their cards after boot.
SDIO core will enable runtime PM for a card only if that cap is set.
As a result, the card will be powered down after boot, and will only
be powered up again when a driver is loaded (and then it's up to the
driver to decide whether power will be kept or not).
This will prevent sdio_bus_probe() failures with setups that do not
support powering off the card.

Power off the card in mmc_sdio_detect __before__ a potential error
handler, which completely removes the card, executes, and only if the
card was successfully powered on beforehand.
While we're at it, use the _sync variant of the runtime PM put API, in
order to ensure that the card is left powered off in case an error
occurred, and the card is going to be removed.
Reproduced and tested on the OLPC XO-1.5.

MMC hosts that poll for card detection by defining the MMC_CAP_NEEDS_POLL
flag have a race on rmmod, where the delayed work is cancelled without
waiting for completed polling. To prevent this a _sync version of the work
cancellation has to be used.

We should not call mmc_card_set_ddr_mode() if we are in single data
mode. This sets DDR and causes the kernel log to say the card is DDR
when it is not.
Explicitly set ddr to 0 rather then rely on MMC_SDR_MODE being 0 when
doing the checks.

For debugging power management features it is convenient to have the
possibility of changing the MMC host controller clock at runtime. This
patch adds a 'clock' file for this under the MMC host root of debugfs.
Usage is as follows:
# cat /sys/kernel/debug/mmc0/clock
52000000
# echo "1000000000" > /sys/kernel/debug/mmc0/clock
# cat /sys/kernel/debug/mmc0/clock
52000000

Bring SDIO devices back to full power before their suspend
handler is invoked.
Doing so ensures that SDIO suspend/resume semantics are
maintained (drivers still get to decide whether their
card should be removed or kept during system suspend,
and at what power state), and that SDIO suspend/resume
execution paths are unchanged.
This is achieved by resuming a runtime-suspended SDIO device
in its ->prepare() PM callback (similary to the PCI subsystem).
Since the PM core always increments the run-time usage
counter before calling the ->prepare() callback and decrements
it after calling the ->complete() callback, it is guaranteed
that when the system will come out of suspend, our device's
power state will reflect its runtime PM usage counter.

To prevent an erroneous removal of the card, make sure
the device is powered when it is mmc_sdio_detect()ed.
This is required since mmc_sdio_detect may be invoked
while the device is runtime suspended (e.g., MMC core
is rescanning when system comes out of suspend).

Enable runtime PM for SDIO functions.
SDIO functions are initialized with a disabled runtime PM state,
and are set active (and their usage count is incremented)
only before potential drivers are probed.
SDIO function drivers that support runtime PM should call
pm_runtime_put_noidle() in their probe routine, and
pm_runtime_get_noresume() in their remove routine (very
similarly to PCI drivers).
In case a matching driver does not support runtime PM, power will
always be kept high (since the usage count is positive).

Assign the generic runtime PM handlers for SDIO.
These handlers invoke the relevant SDIO function drivers'
handlers, if exist, otherwise they just return success
(so SDIO drivers don't have to define any runtime PM handlers
unless they need to).
Runtime PM is still disabled by default, so this patch alone
has no immediate effect.

On resume, let mmc_sdio_init_card go all the way, instead
of skipping the reconfiguration of the card's speed and width.
This is needed to ensure cards wake up with their clock
reconfigured (otherwise it's kept low).
This patch also removes the explicit bus width reconfiguration
on resume, since now this is part of mmc_sdio_init_card.

The DDR support patch needs the following fixes:
- The block driver does not need to know about DDR, any more
than it needs to know about bus width.
- Not only the card must be switched to DDR mode. The host
controller must also be configured, which is done through
the 'set_ios()' function.
- Do not set the DDR mode state until after the switch command
is successful.
- Setting block length is not supported in DDR mode. Make that
a core function and change the other place it is used (mmc_test)
also.

After discovering a problem in regulator reference counting I took Mark
Brown's advice to move the reference count into the MMC core by making the
regulator status a member of struct mmc_host.
I took this opportunity to also implement NULL versions of
the regulator functions so as to rid the driver code from
some ugly #ifdef CONFIG_REGULATOR clauses.

mmc_rescan() includes a pr_info which prints 4 lines each second for
hosts configured with MMC_CAP_NEEDS_POLL. This patch enables the message
only if CONFIG_MMC_DEBUG is selected. Tested on i.MX51's sdhci-esdhc.

In the latest releases of the mmc driver, the freq during initialization
is set to a fixed 400 Khz. This was reportedly too fast for several
users. As there doesn't seem to be an ideal frequency
which-works-for-all, Pierre suggested to let the driver try several
frequencies.
This patch implements that idea. It will try mmc-initialization using
several frequencies from an array 400, 300, 200 and 100.
In case SDIO is broken, it'll still try to detect SDMEM, also at different
freqs.

There are two checks that need to be made when determining whether a
card is removable. A host controller may set MMC_CAP_NONREMOVABLE if the
controller does not support removing cards (e.g. eMMC), in which case
the card is physically non-removable. Also the 'mmc_assume_removable'
module parameter can be configured at module load time, in which case
the card may be logically non-removable.
A helper function keeps the logic in one place so that code always
checks both conditions.
Because this new function is likely to be called from modules we now
need to export the mmc_assume_removable symbol.

The EXTRA_CFLAGS assignment in mmc/Makefile was not accomplishing
anything because this flag only has effect on sources at the same level
as the makefile (i.e., per directory). Since card/, core/, and host/
rely on MMC_DEBUG, the subdir-ccflags-y variant seems to be the
appropriate choice.

4c2ef25fe0 moved the card removal/insertion mechanism out of MMC's
suspend/resume path and into pm notifiers (mmc_pm_notify), and that
broke SDIO's expectation that mmc_suspend_host() will remove the card,
and squash the error, in case -ENOSYS is returned from the bus suspend
handler (mmc_sdio_suspend() in this case).
mmc_sdio_suspend() is using this whenever at least one of the card's SDIO
function drivers does not have suspend/resume handlers - in that case
it is agreed to force removal of the entire card.
This patch fixes this regression by trivially bringing back that part of
mmc_suspend_host(), which was removed by 4c2ef25fe0.

All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.
The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.
New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time. Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.
The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was

The introduction of support for SD combo cards breaks the initialization
of all CSR SDIO chips. The GO_IDLE (CMD0) in mmc_sd_get_cid() causes CSR
chips to be reset (this is non-standard behavior).
When initializing an SDIO card check for a combo card by using the memory
present bit in the R4 response to IO_SEND_OP_COND (CMD5). This avoids the
call to mmc_sd_get_cid() on an SDIO-only card.

SD/MMC cards tend to support an erase operation. In addition, eMMC v4.4
cards can support secure erase, trim and secure trim operations that are
all variants of the basic erase command.
SD/MMC device attributes "erase_size" and "preferred_erase_size" have been
added.
"erase_size" is the minimum size, in bytes, of an erase operation. For
MMC, "erase_size" is the erase group size reported by the card. Note that
"erase_size" does not apply to trim or secure trim operations where the
minimum size is always one 512 byte sector. For SD, "erase_size" is 512
if the card is block-addressed, 0 otherwise.
SD/MMC cards can erase an arbitrarily large area up to and
including the whole card. When erasing a large area it may
be desirable to do it in smaller chunks for three reasons:
1. A single erase command will make all other I/O on the card
wait. This is not a problem if the whole card is being erased, but
erasing one partition will make I/O for another partition on the
same card wait for the duration of the erase - which could be a
several minutes.
2. To be able to inform the user of erase progress.
3. The erase timeout becomes too large to be very useful.
Because the erase timeout contains a margin which is multiplied by
the size of the erase area, the value can end up being several
minutes for large areas.
"erase_size" is not the most efficient unit to erase (especially for SD
where it is just one sector), hence "preferred_erase_size" provides a good
chunk size for erasing large areas.
For MMC, "preferred_erase_size" is the high-capacity erase size if a card
specifies one, otherwise it is based on the capacity of the card.
For SD, "preferred_erase_size" is the allocation unit size specified by
the card.
"preferred_erase_size" is in bytes.

There are some chips (like TI WL12xx series) that can be interfaced over
SDIO but don't support the SDIO specification, meaning that they are
missing CIA (Common I/O Area) with all it's registers. Current Linux SDIO
implementation relies on those registers to identify and configure the
card, so non-standard cards can not function and cause lots of warnings
from the core when it reads invalid data from non-existent registers.
After this patch, init_card() host callback can now set new quirk
MMC_QUIRK_NONSTD_SDIO, which means that SDIO core should not try to access
any standard SDIO registers and rely on init_card() to fill all SDIO
structures instead. As those cards are usually embedded chips, all the
required information can be obtained from machine board files by the host
driver when it's called through init_card() callback.

A non-zero value of SEC_COUNT does not indicate that the card is sector
addressed. According to the MMC specification, cards with a density
greater than 2GiB are sector addressed.

If you don't use CONFIG_MMC_UNSAFE_RESUME, as soon as you attempt to
suspend, the card will be removed, therefore this patch doesn't change the
behavior of this option.
However the removal will be done by pm notifier, which runs while
userspace is still not frozen and thus can freely use del_gendisk, without
the risk of deadlock which would happen otherwise.
Card detect workqueue is now disabled while userspace is frozen, Therefore
if you do use CONFIG_MMC_UNSAFE_RESUME, and remove the card during
suspend, the removal will be detected as soon as userspace is unfrozen,
again at the moment it is safe to call del_gendisk.
Tested with and without CONFIG_MMC_UNSAFE_RESUME with suspend and hibernate.

This series adds support for SD combo cards to MMC/SD driver stack.
SD combo consists of SD memory and SDIO parts in one package. Since the
parts have a separate SD command sets, after initialization, they can be
treated as independent cards on one bus.
Changes are divided into two patches. First is just moving initialization
code around so that SD memory part init can be called from SDIO init.
Second patch is a proper change enabling SD memory along SDIO. I tried to
move as much no-op changes to the first patch so that it's easier to
follow the required changes to initialization flow for SDIO cards.
This is based on Simplified SDIO spec v.2.00. The init sequence is
slightly modified to follow current SD memory init implementation.
Command sequences, assuming SD memory and SDIO indeed ignore unknown
commands, are the same as before for both parts.
This patch:
Prepare for SD-combo (IO+mem) support by splitting SD memory
card init and related functions.

SDIO specification allows RAW (Read after Write) operation using
IO_RW_DIRECT command (CMD52) by setting the RAW bit. This operation is
similar to ordinary read/write commands, except that both write and read
are performed using single command/response pair. The Linux SDIO layer
already supports this internaly, only external function is missing for
drivers to make use, which is added by this patch.
This type of command is required to implement proper power save mode
support in wl1251 wifi driver.
Android has similar patch for G1 in it's tree for the same reason:

Even though many mmc host drivers pass a pm_message_t argument to
mmc_suspend_host() that argument isn't used the by MMC core. As host
drivers are converted to dev_pm_ops they'll have to construct
pm_message_t's (as they won't be passed by the PM subsystem any more) just
to appease the mmc suspend interface.
We might as well just delete the unused paramter.

The clearing of mrq via a memset at the top of the for loop in
mmc_wait_for_app_cmd() is not required as mrq is not used and there is
another clearing of mrq just below. We remove the first memset since if
the initial tests in the for loop fail the memset is not required.

MX3 SoCs have a silicon bug which corrupts CRC calculation of
multi-block transfers when connected SDIO peripheral doesn't drive the
BUSY line as required by the specs.
One way to prevent this is to only allow 1-bit transfers.
Another way is playing tricks with the DMA engine, but this isn't
mainline yet. So for now, we live with the performance drawback of 1-bit
transfers until a nicer solution is found.
This patch introduces a new host controller callback 'init_card' which
is for now only called from mmc_sdio_init_card().

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

In the extended CSD register the CARD_TYPE is an 8-bit value of which the
upper 6 bits were reserved in JEDEC specifications prior to version 4.4.
In version 4.4 two of the reserved bits were designated for identifying
support for the newly added High-Speed Dual Data Rate. Unfortunately the
mmc_read_ext_csd() function required that the reserved bits be zero
instead of ignoring them as it should.
This patch makes mmc_read_ext_csd() ignore the CARD_TYPE bits that are
reserved or not yet supported. It also stops the function jumping to the
end as though an error occurred, when it is only warns that the CARD_TYPE
bits (that it does interpret) are invalid.

SDIO Simplified Specification V2.00 states that it is strongly recommended
that the host executes either a power reset or issues a CMD52 (I/O Reset)
to re-initialize an I/O only card or the I/O portion of a combo card.
Additionally, the CMD52 must be issued first because it cannot be issued
after a CMD0.
With this patch the Nintendo Wii SDIO-based WLAN card is detected after a
system reset, without requiring a complete system powercycle.

Some SDIO cards may suspend while keeping function interrupts active
especially in the powered suspend case. Upon resume we need to kick the
SDIO interrupt thread to check for pending interrupts and to restart card
IRQ detection at the host controller level.

Seen on a Marvell 8686 SDIO card and Via VX855 controller: we must avoid
sending CMD3/5/7 on a resume where power has been maintained, because the
8686 will refuse to respond to them and the MMC stack will give up on the
card.

This patch series provides the core changes needed to allow SDIO cards to
remain powered and active while the host system is suspended, and let them
wake up the host system when needed. This is used to implement
wake-on-lan with SDIO wireless cards at the moment. Patches to add that
support to the libertas driver will be posted separately.
This patch:
Some SDIO cards have the ability to keep on running autonomously when the
host system is suspended, and wake it up when needed. This however
requires that the host controller preserve power to the card, and
configure itself appropriately for wake-up.
There is however 4 layers of abstractions involved: the host controller
driver, the MMC core code, the SDIO card management code, and the actual
SDIO function driver. To make things simple and manageable, host drivers
must advertise their PM capabilities with a feature bitmask, then function
drivers can query and set those features from their suspend method. Then
each layer in the suspend call chain is expected to act upon those bits
accordingly.

transfer size. Add a quirk to that effect.

We need to accurately track how many SDIO functions have been initialised
(and keep card->sdio_funcs in sync) so that we don't try to remove more
functions than we initialised if we hit the error path in
mmc_attach_sdio().
Without this patch if we hit the error path in mmc_attach_sdio() we run
the risk of deferencing invalid memory in sdio_remove_func(), leading to a
crash.

Some people run general-purpose distribution kernels on netbooks with
a card that is physically non-removable or logically non-removable
(e.g. used for /home) and cannot be cleanly unmounted during suspend.
Add a module parameter to set whether cards are assumed removable or
non-removable, with the default set by CONFIG_MMC_UNSAFE_RESUME.
In general, it is not possible to tell whether a card present in an MMC
slot after resume is the same that was there before suspend. So there are
two possible behaviours, each of which will cause data loss in some cases:
CONFIG_MMC_UNSAFE_RESUME=n (default): Cards are assumed to be removed
during suspend. Any filesystem on them must be unmounted before suspend;
otherwise, buffered writes will be lost.
CONFIG_MMC_UNSAFE_RESUME=y: Cards are assumed to remain present during
suspend. They must not be swapped during suspend; otherwise, buffered
writes will be flushed to the wrong card.
Currently the choice is made at compile time and this allows that to be
overridden at module load time.

Rework the current CIS tuple parsing code, reusing the existing
infrastructure and providing an easy way to add new CISTPL_FUNCE parsers
by TPLFE_TYPE.
Valid known CIS tuples are now silently queued for the SDIO function
driver when not parsed/processed (-EILSEQ) by the SDIO core. Unknown CIS
tuples (-ENOENT) are queued too for the SDIO function driver without
aborting the initialization, but emit a warning in the kernel log.
CISTPL_FUNCE tuples can be "whitelisted" now by adding a matching entry to
the cis_tpl_funce_list table.

The PC Card 8.0 specification (vol. 4, section 3.2.10) says the
TPLLV1_INFO field of the CISTPL_VERS_1 tuple must contain 4 strings. Some
cards don't have all 4 so just parse as many as we can.

Some manufacturers provide vendor information in non-vendor specific CIS
tuples. For example, Broadcom uses an Extended Function tuple to provide
the MAC address on some of their network cards, as in the case of the
Nintendo Wii WLAN daughter card.
This patch allows passing whitelisted FUNCE tuples unknown to the SDIO
core to a matching SDIO driver instead of rejecting them and failing.

Especially for SDIO drivers which may have special conditions/errors to
report, it is a good thing to relay the returned error code back to upper
layers.
This also allows for the rationalization of the resume path where code to
"remove" a no-longer-existing or replaced card was duplicated into the
MMC, SD and SDIO bus drivers.
In the SDIO case, if a function suspend method returns an error, then all
previously suspended functions are resumed and the error returned. An
exception is made for -ENOSYS which the core interprets as "we don't
support suspend so just kick the card out for suspend and return success".
When resuming SDIO cards, the core code only validates the manufacturer
and product IDs to make sure the same kind of card is still present before
invoking functions resume methods. It's the function driver's
responsibility to perform further tests to confirm that the actual same
card is present (same MAC address, etc.) and return an error otherwise.

Currently, all SDIO cards are virtually removed upon a suspend, and
completely reprobed upon a resume. This adds the suspend and resume
methods to the SDIO bus driver so to be able to dispatch those events to
the actual SDIO function drivers for real suspend/resume instead.
All active functions on a card must have a driver with both a suspend and
a resume method though. Failing that, we fall back to the current
behavior of simply "removing" the card when suspending.
When resuming, we make sure the same card is still inserted by comparing
the vendor and product IDs. If there is a mismatch, or if there is simply
no card anymore in the slot, then the previous card is "removed" and the
new card is detected. This is further enhanced with the next patch.

Some time ago, I have send a patch to the mmc_spi subsystem changing the
error codes. This was after a discussion with Pierre about using EINVAL
only for non-recoverable errors. This patch was accepted as
http://git.kernel.org/linus/fdd858db7113ca64132de390188d7ca00701013d
Unfortunately, several weeks later, I realized that this patch has opened
a little can of worms because there are SD cards on the market which
a) claim that they support the switch command
AND
b) refuse to execute this command if operating in SPI mode.
So, such a card would get unusuable in an embedded linux system in SPI
mode, because the init sequence terminates with an error.
This patch adds the missing error codes to the caller of the switch
command and restores the old behaviour to fail gracefully if these
commands can not execute.

Normally writes to SDIO function 0 outside the vendor specific CCCR
registers are prohibited.
To support embedded devices that require writes to SDIO function 0 outside
this range (e.g. TI WL127x embedded sdio wifi device),
MMC_QUIRK_LENIENT_FN0 is introduced.
A card quirks field is added to `struct mmc_card' to support non-standard
devices (e.g. embedded sdio devices).

Add support to disconnect the pull-up resistor on CD/DAT[3] (pin 1)
of the card. This may be desired on certain setups of boards,
controllers and embedded sdio devices which do not need the card's
pull-up. As a result, card detection is disabled and power is saved.

Power can be saved by powering off cards that are not in use. This is
similar to suspend / resume except it is under the control of the driver,
and does not require any power management support. It can only be used
when the driver can monitor whether the card is removed, otherwise it is
unsafe. This is possible because, unlike suspend, the driver still
receives card detect and / or cover switch interrupts.

With this patch, mmc_rescan can detect the removal of an mmc card and
the insertion of (possibly another) card in the same run. This means
that a card change can be detected without having to call
mmc_detect_change multiple times.
This change generalises the core such that it can be easily used by
hosts which provide a mechanism to detect only the presence of a card
reader cover, which has to be taken off in order to insert a card. Other
hosts ("card detect" or "MMC_CAP_NEEDS_POLL") each receive an event when
a card is removed and when a card is inserted, so it is sufficient for
them if mmc_rescan handles only one event at a time. "Cover detect"
hosts, however, only receive events about the cover status. This means
that between 2 subsequent events, both a card removal and a card
insertion can occur. In this case, the pre-patch version of mmc_rescan
would only detect the removal of the previous card but not the insertion
of the new card.

Some cards are not able to calculate a valid CRC16 value
for CID and CSD reads (CRC for 512 byte data blocks is OK).
By moving the CRC enable after the read of CID and CSD, these
cards can be used. This patch was tested with a faulty 8 GByte
takeMS Class 6 SDHC card. This patch was suggested by
Pierre Ossman.

Glue between MMC and regulator stacks ... verified with
some OMAP3 boards using adjustable and configured-as-fixed
regulators on several MMC controllers.
These calls are intended to be used by MMC host adapters
using at least one regulator per host. Examples include
slots with regulators supporting multiple voltages and
ones using multiple voltage rails (e.g. DAT4..DAT7 using a
separate supply, or a split rail chip like certain SDIO
WLAN or eMMC solutions).

Extended CSD is a MMC card register. As increasingly interesting
fields are being added to Extended CSD, it is helpful to see its
value. Note that SD cards do not have an Extended CSD
register, so it is MMC only.

Currently we are using an explicit udev rule to trigger loading of the
mmc-block module when an MMC or SD card is detected:
SUBSYSTEM=="mmc", RUN+="/sbin/modprobe -Qba mmc-block"
It makes much more sense for the mmc bus driver and the mmc-block module to
share MODALIAS information so that they are linked automatically.
There is no real information of use in the MMC system at the current time.
All devices inserted require us to load the mmc-block device. Until such
time as useful parameters exist simply reflect the module linkage via
the module alias below:

The delayed work item mmc_host.detect is now cancelled before flushing
the work queue. This takes care of cases when delayed_work was scheduled
for mmc_host.detect, but not yet placed in the work queue.

"MMC: CSD and CID timeout values" inadvertently broke
the timeout for the MMC command SEND_EXT_CSD.
This patch puts it back again.
Depending on the characteristics of the controller,
this bug may prevent the use of MMC cards.

Get rid of a silent failure mode when the MMC/SD host doesn't
support the voltages needed to operate a given card, by
adding a warning. A 3.3V host and a 3.0V card, for example,
no longer need to mysteriously just not work at all.
This isn't the best diagnostic; ideally it would also tell
what voltage the card and host support (and not just by
dumping the bitmasks).

It seems that some cards are slightly out of spec and occasionally
will not be able to complete a write in the alloted 250 ms [1].
Incease the timeout slightly to allow even these cards to function
properly.

Make sure we can be woken from the forced sleep that is done on errors.
Removing a card often results in -ENOMEDIUM or -EILSEQ so we previously
locked up the removal process for a second.
We could completely exit on -ENOMEDIUM, but it might be a transient
glitch so treat it like any other error.

For each card successfully added to the bus, create a subdirectory under
the host's debugfs root with information about the card.
At the moment, only a single file is added to the card directory for
all cards: "state". It reflects the "state" field in struct mmc_card,
indicating whether the card is present, readonly, etc.
For MMC and SD cards (not SDIO), another file is added: "status".
Reading this file will ask the card about its current status and
return it. This can be useful if the card just refuses to respond to
any commands, which might indicate that the card state is not what the
MMC core thinks it is (due to a missing stop command, for example.)

When CONFIG_DEBUG_FS is set, create a few files under /sys/kernel/debug
containing information about an mmc host's internal state. Currently,
just a single file is created, "ios", which contains information about
the current operating parameters for the bus (clock speed, bus width,
etc.)
Host drivers can add additional files and directories under the host's
root directory by passing the debugfs_root field in struct mmc_host as
the 'parent' parameter to debugfs_create_*.

Make sure that the maximum size for a byte mode transfer is identical
in all places. Also tweak the transfer helper so that a single byte
mode transfer is preferred over (possibly multiple) block mode
request(s).

There are a lot of crappy controllers out there that cannot handle
all the request sizes that the MMC/SD/SDIO specifications require.
In case the card driver can pad the data to overcome the problems,
this commit adds a helper that calculates how much that padding
should be.
A corresponding helper is also added for SDIO, but it can also deal
with all the complexities of splitting up a large transfer efficiently.

Now get_ro() callback must return 0/1 values for its logical states, and
negative errno values in case of error. If particular host instance doesn't
support RO/WP switch, it should return -ENOSYS.
This patch changes some hosts in two ways:
1. Now functions should be smart to not return negative values in
"RO asserted" case (particularly gpio_ calls could return negative
values for the outermost GPIOs).
Also, board code usually passes get_ro() callbacks that directly return
gpioreg & bit result, so at91_mci, imxmmc, pxamci and mmc_spi's get_ro()
handlers need take special care when returning platform's values to the
mmc core.
2. In case of host instance didn't implement get_ro() callback, it should
really return -ENOSYS and let the mmc core decide what to do about it
(mmc core thinks the same way as the hosts, so it isn't functional
change).

Some hosts (and boards that use mmc_spi) do not use interrupts on the CD
line, so they can't trigger mmc_detect_change. We want to poll the card
and see if there was a change. 1 second poll interval seems resonable.
This patch also implements .get_cd() host operation, that could be used
by the hosts that are able to report card-detect status without need to
talk MMC.

That way host drivers can use that name for resource allocations and

Suppressing uevents turned out to be a bad idea as it screws up the
order of events, making user space very confused. Change the system to
use sysfs groups instead.
This is a regression that, for some odd reason, has gone unnoticed for
some time. It confuses hal so that the block devices (which have the
mmc device as a parent) are not registered. End result being that
desktop magic when cards are inserted won't work.

MMC over SPI sends the CID and CSD registers as data, not responses,
which means that the host driver won't do the necessary byte flipping
for us.

This changes the uevent buffer functions to use a struct instead of a
long list of parameters. It does no longer require the caller to do the
proper buffer termination and size accounting, which is currently wrong
in some places. It fixes a known bug where parts of the uevent
environment are overwritten because of wrong index calculations.

Currently we print a message about some bad states wrt function IRQ
handlers but return 0 from process_sdio_pending_irqs() nevertheless.
This can lead to an infinite loop as nothing might have cleared the
condition for the pending card interrupt from the host controller by
the time host->ops->enable_sdio_irq(host, 1) is called.

The interrupt polling frequency is a compromise between power usage and
interrupt latency. Unfortunately, it affects throughput rather severely
for devices which require an interrupt for every chunk of data.
By making the polling frequency adaptive, we get better throughput with
those devices without sacficing too much power. Polling will quickly
increase when there is an actual interrupt, and slowly fall back to the
idle frequency when the interrupts stop coming.

Teach the MMC/SD/SDIO core about using SPI mode.
- Use mmc_host_is_spi() so enumeration works through SPI signaling
and protocols, not just the native versions.
- Provide the SPI response type flags with each request issued,
including requests from the new lock/unlock code.
- Understand that cmd->resp[0] and mmc_get_status() results for SPI
return different values than for "native" MMC/SD protocol; this
affects resetting, checking card lock status, and some others.
- Understand that some commands act a bit differently ... notably:
* OP_COND command doesn't return the OCR
* APP_CMD status doesn't have an R1_APP_CMD analogue
Those changes required some new and updated primitives:
- Provide utilities to access two SPI-only requests, and one
request that wasn't previously needed:
* mmc_spi_read_ocr() ... SPI only
* mmc_spi_set_crc() ... SPI only (override by module parm)
* mmc_send_cid() ... for use without broadcast mode
- Updated internal routines:
* Previous mmc_send_csd() modified into mmc_send_cxd_native();
it uses native "R2" responses, which include 16 bytes of data.
* Previous mmc_send_ext_csd() becomes new mmc_send_cxd_data()
helper for command-and-data access
* Bugfix to that mmc_send_cxd_data() code: dma-to-stack is
unsafe/nonportable, so kmalloc a bounce buffer instead.
- Modified mmc_send_ext_csd() now uses mmc_send_cxd_data() helper
- Modified mmc_send_csd(), and new mmc_spi_send_cid(), routines use
those helper routines based on whether they're native or SPI
The newest categories of cards supported by the MMC stack aren't expected
to work yet with SPI: MMC or SD cards with over 4GB data, and SDIO.
All those cards support SPI mode, so eventually they should work too.

writing function 0 registers. Writes outside the vendor specific CCCR

Extend sdio_readsb(), sdio_writesb(), sdio_memcpy_fromio(), and
sdio_memcpy_toio() to handle any length of buffer by splitting the transfer
into several IO_RW_EXTENDED commands. Typically, a transfer would be split
into a single block mode transfer followed by a byte mode transfer for the
remainder but we also handle lack of block mode support and the block size
being greater than 512 (the maximum byte mode transfer size).
host->max_seg_size <= host->max_req_size so there's no need to check both
when determining the maximum data size for a single command.

Before a driver is probed, set the function's block size to the default so the
driver is sure the block size is something sensible and it needn't explicitly
set it.
The default block size is the largest that's supported by both the card and
the host, with a maximum of 512 to ensure aribitrarily sized transfer use the
optimal (least) number of commands.

The problem is that the sdio_bus must be registered before any SDIO
drivers are registered against it otherwise the kernel sulks. Because
the sdio_bus registration happens through module_init (equivalent to
device_initcall), then any SDIO
drivers linked before the SDIO core code in the kernel will be initialized
first.
Upcoming SDIO function drivers are likely to be located outside the
drivers/mmc directory as it is common practice to group drivers according
to their function rather than the bus they use. SDIO drivers are therefore
likely to appear at random location in the kernel link.
To make sure the sdio_bus is always initialized before any SDIO drivers,
let's move the MMC init to the subsys_initcall level.

When the card has been added to the device model, it might be bound
to a card driver. Therefore, we have to release the host lock when
trying to remove it as we otherwise might deadlock with the driver.

In the normal case, the host lock can be claimed directly.
When it cannot, the caller will sleep. Make sure we don't
have any latent bugs by always calling might_sleep().

Since many have the system root on MMC/SD we must allow some foot
shooting when it comes to resume.
We cannot detect if a card is removed and reinserted during suspend,
so the safe approach would be to assume it was, avoiding potential
filesystem corruption. This will of course not work if you cannot
release the card before suspend.
This commit adds a compile time option that makes the MMC layer
assume the card wasn't touched if it is redetected upon resume.

Fix handling of low voltage MMC cards.
The latest MMC and SD specs both agree that support for
low-voltage operations is indicated by bit 7 in the OCR.
The MMC spec states that the low voltage range is
1.65-1.95V while the SD spec leaves the actual voltage
range undefined - meaning that there is still no such
thing as a low voltage SD card.
However, an old Sandisk spec implied that bits 7.0
represented voltages below 2.0V in 1V or 0.5V increments,
and the code was accordingly written with that expectation.
This confusion meant that host drivers attempting to support
the typical low voltage (1.8V) would set the wrong bits in
the host OCR mask (usually bits 5 and/or 6) resulting in the
the low voltage mode never being used.
This change corrects the low voltage range and adds sanity
checks on the reserved bits (0-6) and for SD cards that
claim to support low-voltage operations.

Delegate protocol handling to "bus handlers". This allows the core to
just handle the task of arbitrating the bus. Initialisation and
pampering of cards is now done by the different bus handlers.
This design also allows MMC and SD (and later SDIO) to be more cleanly
separated, allowing easier maintenance.
