Daniel pointed me towards this function and there are multiple obvious problems
in the implementation.
First of all the retry loop is not working as intended. In general the retry
makes only sense if you grab the reference first and then check the sequence
values.
Then we should always also wait for the exclusive fence.
It's also good practice to keep the reference around when installing callbacks
to fences you don't own.
And last the whole implementation was unnecessary complex and rather hard to
understand which could lead to probably unexpected behavior of the IOCTL.
Fix all this by reworking the implementation from scratch. Dropping the
whole RCU approach and taking the lock instead.
Only mildly tested and needs a thoughtful review of the code.
Pushing through drm-misc-next to avoid merge conflicts and give the code
another round of testing.
v2: fix the reference counting as well
v3: keep the excl fence handling as is for stable
v4: back to testing all fences, drop RCU
v5: handle in and out separately
v6: add missing clear of events
v7: change coding style as suggested by Michel, drop unused variables

Specifically document the new/clarified rules around how the shared
fences do not have any ordering requirements against the exclusive
fence.
But also document all the things a bit better, given how central
struct dma_resv to dynamic buffer management the docs have been very
inadequat.
- Lots more links to other pieces of the puzzle. Unfortunately
ttm_buffer_object has no docs, so no links :-(
- Explain/complain a bit about dma_resv_locking_ctx(). I still don't
like that one, but fixing the ttm call chains is going to be
horrible. Plus we want to plug in real slowpath locking when we do
that anyway.
- Main part of the patch is some actual docs for struct dma_resv.
Overall I think we still have a lot of bad naming in this area (e.g.
dma_resv.fence is singular, but contains the multiple shared fences),
but I think that's more indicative of how the semantics and rules are
just not great.
Another thing that's real awkard is how chaining exclusive fences
right now means direct dma_resv.exclusive_fence pointer access with an
rcu_assign_pointer. Not so great either.

It is expected from the clients to follow the below steps on an imported
dmabuf fd:
a) dmabuf = dma_buf_get(fd) // Get the dmabuf from fd
b) dma_buf_attach(dmabuf); // Clients attach to the dmabuf
o Here the kernel does some slab allocations, say for
dma_buf_attachment and may be some other slab allocation in the
dmabuf->ops->attach().
c) Client may need to do dma_buf_map_attachment().
d) Accordingly dma_buf_unmap_attachment() should be called.
e) dma_buf_detach () // Clients detach to the dmabuf.
o Here the slab allocations made in b) are freed.
f) dma_buf_put(dmabuf) // Can free the dmabuf if it is the last
reference.
Now say an erroneous client failed at step c) above thus it directly
called dma_buf_put(), step f) above. Considering that it may be the last
reference to the dmabuf, buffer will be freed with pending attachments
left to the dmabuf which can show up as the 'memory leak'. This should
at least be reported as the WARN().

Since there has been a reported a performance regression due to the

Each add_fence() call does a dma_fence_get() on the relevant fence. In
the error path, we weren't calling dma_fence_put() so all those fences
got leaked. Also, in the krealloc_array failure case, we weren't

Christian and me realized we have a pretty massive disconnect about
different interpretations of what dma_resv is used for by different
drivers. The discussion is much, much bigger than this change here,
but this is an important one:
Non-dynamic exporters must guarantee that the memory they return is
ready for use. They cannot expect importers to wait for the exclusive
fence. Only dynamic importers are required to obey the dma_resv fences
strictly (and more patches are needed to define exactly what this
means).
Christian has patches to update nouvea, radeon and amdgpu. The only
other driver using both ttm and supporting dma-buf export is qxl,
which only uses synchronous ttm_bo_move.
v2: To hammer this in document that dynamic importers _must_ wait for

Overview
========
The patch adds DMA-BUF statistics to /sys/kernel/dmabuf/buffers. It
allows statistics to be enabled for each DMA-BUF in sysfs by enabling
the config CONFIG_DMABUF_SYSFS_STATS.
The following stats will be exposed by the interface:
/sys/kernel/dmabuf/buffers/<inode_number>/exporter_name
/sys/kernel/dmabuf/buffers/<inode_number>/size
/sys/kernel/dmabuf/buffers/<inode_number>/attachments/<attach_uid>/device
/sys/kernel/dmabuf/buffers/<inode_number>/attachments/<attach_uid>/map_counter
The inode_number is unique for each DMA-BUF and was added earlier [1]
in order to allow userspace to track DMA-BUF usage across different
processes.
Use Cases
=========
The interface provides a way to gather DMA-BUF per-buffer statistics
from production devices. These statistics will be used to derive DMA-BUF
per-exporter stats and per-device usage stats for Android Bug reports.
The corresponding userspace changes can be found at [2].
Telemetry tools will also capture this information(along with other
memory metrics) periodically as well as on important events like a
foreground app kill (which might have been triggered by Low Memory
Killer). It will also contribute to provide a snapshot of the system
memory usage on other events such as OOM kills and Application Not
Responding events.
Background
==========
Currently, there are two existing interfaces that provide information
about DMA-BUFs.
1) /sys/kernel/debug/dma_buf/bufinfo
debugfs is however unsuitable to be mounted in production systems and
cannot be considered as an alternative to the sysfs interface being
proposed.
2) proc/<pid>/fdinfo/<fd>
The proc/<pid>/fdinfo/<fd> files expose information about DMA-BUF fds.
However, the existing procfs interfaces can only provide information
about the buffers for which processes hold fds or have the buffers
mmapped into their address space. Since the procfs interfaces alone
cannot provide a full picture of all DMA-BUFs in the system, there is
the need for an alternate interface to provide this information on
production systems.
The patch contains the following major improvements over v1:
1) Each attachment is represented by its own directory to allow creating
a symlink to the importing device and to also provide room for future
expansion.
2) The number of distinct mappings of each attachment is exposed in a
separate file.
3) The per-buffer statistics are now in /sys/kernel/dmabuf/buffers
inorder to make the interface expandable in future.
All of the improvements above are based on suggestions/feedback from
Daniel Vetter and Christian König.
A shell script that can be run on a classic Linux environment to read
out the DMA-BUF statistics can be found at [3](suggested by John
Stultz).

Allocate a new private stub fence in drm_syncobj_assign_null_handle,
instead of using a static stub fence.
When userspace creates a fence with DRM_SYNCOBJ_CREATE_SIGNALED or when
userspace signals a fence via DRM_IOCTL_SYNCOBJ_SIGNAL, the timestamp
obtained when the fence is exported and queried with SYNC_IOC_FILE_INFO
should match when the fence's status was changed from the perspective of
userspace, which is during the respective ioctl.
When a static stub fence started being used in by these ioctls, this
behavior changed. Instead, the timestamp returned by SYNC_IOC_FILE_INFO
became the first time anything used the static stub fence, which has no
meaning to userspace.

By default dma_buf_export() sets the exporter name to be
KBUILD_MODNAME. Unfortunately this may not be identical to the
string used as the heap name (ie: "system" vs "system_heap").
This can cause some minor confusion with tooling, and there is
the future potential where multiple heap types may be exported
by the same module (but would all have the same name).
So to avoid all this, set the exporter exp_name to the heap name.

We have too many people abusing the struct page they can get at but
really shouldn't in importers. Aside from that the backing page might
simply not exist (for dynamic p2p mappings) looking at it and using it
e.g. for mmap can also wreak the page handling of the exporter
completely. Importers really must go through the proper interface like
dma_buf_mmap for everything.
I'm semi-tempted to enforce this for dynamic importers since those
really have no excuse at all to break the rules.
Unfortuantely we can't store the right pointers somewhere safe to make
sure we oops on something recognizable, so best is to just wrangle
them a bit by flipping all the bits. At least on x86 kernel addresses
have all their high bits sets and the struct page array is fairly low
in the kernel mapping, so flipping all the bits gives us a very high
pointer in userspace and hence excellent chances for an invalid
dereference.

It is observed 'use-after-free' on the dmabuf's file->f_inode with the
race between closing the dmabuf file and reading the dmabuf's debug
info.
Consider the below scenario where P1 is closing the dma_buf file
and P2 is reading the dma_buf's debug info in the system:
P1 P2
dma_buf_debug_show()
dma_buf_put()
__fput()
file->f_op->release()
dput()
....
dentry_unlink_inode()
iput(dentry->d_inode)
(where the inode is freed)
mutex_lock(&db_list.lock)
read 'dma_buf->file->f_inode'
(the same inode is freed by P1)
mutex_unlock(&db_list.lock)
dentry->d_op->d_release()-->
dma_buf_release()
.....
mutex_lock(&db_list.lock)
removes the dmabuf from the list
mutex_unlock(&db_list.lock)
In the above scenario, when dma_buf_put() is called on a dma_buf, it
first frees the dma_buf's file->f_inode(=dentry->d_inode) and then
removes this dma_buf from the system db_list. In between P2 traversing
the db_list tries to access this dma_buf's file->f_inode that was freed
by P1 which is a use-after-free case.
Since, __fput() calls f_op->release first and then later calls the
d_op->d_release, move the dma_buf's db_list removal from d_release() to
f_op->release(). This ensures that dma_buf's file->f_inode is not
accessed after it is released.

This patch is basically a port of Ørjan Eide's similar patch for ION
https://lore.kernel.org/lkml/20200414134629.54567-1-orjan.eide@arm.com/
Only sync the sg-list of dma-buf heap attachment when the attachment
is actually mapped on the device.
dma-bufs may be synced at any time. It can be reached from user space
via DMA_BUF_IOCTL_SYNC, so there are no guarantees from callers on when
syncs may be attempted, and dma_buf_end_cpu_access() and
dma_buf_begin_cpu_access() may not be paired.
Since the sg_list's dma_address isn't set up until the buffer is used
on the device, and dma_map_sg() is called on it, the dma_address will be
NULL if sync is attempted on the dma-buf before it's mapped on a device.
Before v5.0 (commit 55897af630 ("dma-direct: merge swiotlb_dma_ops
into the dma_direct code")) this was a problem as the dma-api (at least
the swiotlb_dma_ops on arm64) would use the potentially invalid
dma_address. How that failed depended on how the device handled physical
address 0. If 0 was a valid address to physical ram, that page would get
flushed a lot, while the actual pages in the buffer would not get synced
correctly. While if 0 is an invalid physical address it may cause a
fault and trigger a crash.
In v5.0 this was incidentally fixed by commit 55897af630 ("dma-direct:
merge swiotlb_dma_ops into the dma_direct code"), as this moved the
dma-api to use the page pointer in the sg_list, and (for Ion buffers at
least) this will always be valid if the sg_list exists at all.
But, this issue is re-introduced in v5.3 with
commit 449fa54d68 ("dma-direct: correct the physical addr in
dma_direct_sync_sg_for_cpu/device") moves the dma-api back to the old
behaviour and picks the dma_address that may be invalid.
dma-buf core doesn't ensure that the buffer is mapped on the device, and
thus have a valid sg_list, before calling the exporter's
begin_cpu_access.

In preparation for some patches to optmize the system
heap code, rework the dmabuf exporter to utilize sgtables rather
then pageslists for tracking the associated pages.
This will allow for large order page allocations, as well as
more efficient page pooling.
In doing so, the system heap stops using the heap-helpers logic
which sadly is not quite as generic as I was hoping it to be, so
this patch adds heap specific implementations of the dma_buf_ops
function handlers.

The new type struct dma_buf_map represents a mapping of dma-buf memory
into kernel space. It contains a flag, is_iomem, that signals users to
access the mapped memory with I/O operations instead of regular loads
and stores.
It was assumed that DMA buffer memory can be accessed with regular load
and store operations. Some architectures, such as sparc64, require the
use of I/O operations to access dma-map buffers that are located in I/O
memory. Providing struct dma_buf_map allows drivers to implement this.
This was specifically a problem when refreshing the graphics framebuffer
on such systems. [1]
As the first step, struct dma_buf stores an instance of struct dma_buf_map
internally. Afterwards, dma-buf's vmap and vunmap interfaces are be
converted. Finally, affected drivers can be fixed.
v3:
* moved documentation into separate patch
* test for NULL pointers with !<ptr>

NULL pointer dereference is observed while exporting the dmabuf but
failed to allocate the 'struct file' which results into the dropping of
the allocated dentry corresponding to this file in the dmabuf fs, which
is ending up in dma_buf_release() and accessing the uninitialzed

GPU drivers need this in their shrinkers, to be able to throw out
mmap'ed buffers. Note that we also need dma_resv_lock in shrinkers,
but that loop is resolved by trylocking in shrinkers.
So full hierarchy is now (ignore some of the other branches we already
have primed):
mmap_read_lock -> dma_resv -> shrinkers -> i_mmap_lock_write
I hope that's not inconsistent with anything mm or fs does, adding
relevant people.

The Documentation/DMA-API-HOWTO.txt states that the dma_map_sg() function
returns the number of the created entries in the DMA address space.
However the subsequent calls to the dma_sync_sg_for_{device,cpu}() and
dma_unmap_sg must be called with the original number of the entries
passed to the dma_map_sg().
struct sg_table is a common structure used for describing a non-contiguous
memory buffer, used commonly in the DRM and graphics subsystems. It
consists of a scatterlist with memory pages and DMA addresses (sgl entry),
as well as the number of scatterlist entries: CPU pages (orig_nents entry)
and DMA mapped pages (nents entry).
It turned out that it was a common mistake to misuse nents and orig_nents
entries, calling DMA-mapping functions with a wrong number of entries or
ignoring the number of mapped entries returned by the dma_map_sg()
function.
To avoid such issues, lets use a common dma-mapping wrappers operating
directly on the struct sg_table objects and use scatterlist page
iterators where possible. This, almost always, hides references to the
nents and orig_nents entries, making the code robust, easier to follow
and copy/paste safe.

- Untangle the header spaghetti which causes build failures in various
situations caused by the lockdep additions to seqcount to validate that
the write side critical sections are non-preemptible.
- The seqcount associated lock debug addons which were blocked by the
above fallout.
seqcount writers contrary to seqlock writers must be externally
serialized, which usually happens via locking - except for strict per
CPU seqcounts. As the lock is not part of the seqcount, lockdep cannot
validate that the lock is held.
This new debug mechanism adds the concept of associated locks.
sequence count has now lock type variants and corresponding
initializers which take a pointer to the associated lock used for
writer serialization. If lockdep is enabled the pointer is stored and
write_seqcount_begin() has a lockdep assertion to validate that the
lock is held.
Aside of the type and the initializer no other code changes are
required at the seqcount usage sites. The rest of the seqcount API is
unchanged and determines the type at compile time with the help of
_Generic which is possible now that the minimal GCC version has been
moved up.
Adding this lockdep coverage unearthed a handful of seqcount bugs which
have been addressed already independent of this.
While generaly useful this comes with a Trojan Horse twist: On RT
kernels the write side critical section can become preemtible if the
writers are serialized by an associated lock, which leads to the well
known reader preempts writer livelock. RT prevents this by storing the
associated lock pointer independent of lockdep in the seqcount and
changing the reader side to block on the lock when a reader detects
that a writer is in the write side critical section.
- Conversion of seqcount usage sites to associated types and initializers.

A sequence counter write side critical section must be protected by some
form of locking to serialize writers. If the serialization primitive is
not disabling preemption implicitly, preemption has to be explicitly
disabled before entering the sequence counter write side critical
section.
The dma-buf reservation subsystem uses plain sequence counters to manage
updates to reservations. Writer serialization is accomplished through a
wound/wait mutex.
Acquiring a wound/wait mutex does not disable preemption, so this needs
to be done manually before and after the write side critical section.
Use the newly-added seqcount_ww_mutex_t instead:
- It associates the ww_mutex with the sequence count, which enables
lockdep to validate that the write side critical section is properly
serialized.
- It removes the need to explicitly add preempt_disable/enable()
around the write side critical section because the write_begin/end()
functions for this new data type automatically do this.
If lockdep is disabled this ww_mutex lock association is compiled out
and has neither storage size nor runtime overhead.

Commit 3c3b177a93 ("reservation: add support for read-only access
using rcu") introduced a sequence counter to manage updates to
reservations. Back then, the reservation object initializer
reservation_object_init() was always inlined.
Having the sequence counter initialization inlined meant that each of
the call sites would have a different lockdep class key, which would've
broken lockdep's deadlock detection. The aforementioned commit thus
introduced, and exported, a custom seqcount lockdep class key and name.
The commit 8735f16803 ("dma-buf: cleanup reservation_object_init...")
transformed the reservation object initializer to a normal non-inlined C
function. seqcount_init(), which automatically defines the seqcount
lockdep class key and must be called non-inlined, can now be safely used.
Remove the seqcount custom lockdep class key, name, and export. Use
seqcount_init() inside the dma reservation object initializer.

Two in one go:
- it is allowed to call dma_fence_wait() while holding a
dma_resv_lock(). This is fundamental to how eviction works with ttm,
so required.
- it is allowed to call dma_fence_wait() from memory reclaim contexts,
specifically from shrinker callbacks (which i915 does), and from mmu
notifier callbacks (which amdgpu does, and which i915 sometimes also
does, and probably always should, but that's kinda a debate). Also
for stuff like HMM we really need to be able to do this, or things
get real dicey.
Consequence is that any critical path necessary to get to a
dma_fence_signal for a fence must never a) call dma_resv_lock nor b)
allocate memory with GFP_KERNEL. Also by implication of
dma_resv_lock(), no userspace faulting allowed. That's some supremely
obnoxious limitations, which is why we need to sprinkle the right
annotations to all relevant paths.
The one big locking context we're leaving out here is mmu notifiers,
added in
commit 23b68395c7

mm/mmu_notifiers: add a lockdep map for invalidate_range_start/end
that one covers a lot of other callsites, and it's also allowed to
wait on dma-fences from mmu notifiers. But there's no ready-made
functions exposed to prime this, so I've left it out for now.
v2: Also track against mmu notifier context.
v3: kerneldoc to spec the cross-driver contract. Note that currently
i915 throws in a hard-coded 10s timeout on foreign fences (not sure
why that was done, but it's there), which is why that rule is worded
with SHOULD instead of MUST.
Also some of the mmu_notifier/shrinker rules might surprise SoC
drivers, I haven't fully audited them all. Which is infeasible anyway,
we'll need to run them with lockdep and dma-fence annotations and see
what goes boom.
v4: A spelling fix from Mika
v5: #ifdef for CONFIG_MMU_NOTIFIER. Reported by 0day. Unfortunately
this means lockdep enforcement is slightly inconsistent, it won't spot
GFP_NOIO and GFP_NOFS allocations in the wrong spot if
CONFIG_MMU_NOTIFIER is disabled in the kernel config. Oh well.
v5: Note that only drivers/gpu has a reasonable (or at least
historical) excuse to use dma_fence_wait() from shrinker and mmu
notifier callbacks. Everyone else should either have a better memory
manager model, or better hardware. This reflects discussions with

Design is similar to the lockdep annotations for workers, but with
some twists:
- We use a read-lock for the execution/worker/completion side, so that
this explicit annotation can be more liberally sprinkled around.
With read locks lockdep isn't going to complain if the read-side
isn't nested the same way under all circumstances, so ABBA deadlocks
are ok. Which they are, since this is an annotation only.
- We're using non-recursive lockdep read lock mode, since in recursive
read lock mode lockdep does not catch read side hazards. And we
_very_ much want read side hazards to be caught. For full details of
this limitation see
commit e914985897
Author: Peter Zijlstra <peterz@infradead.org>
Date: Wed Aug 23 13:13:11 2017 +0200
locking/lockdep/selftests: Add mixed read-write ABBA tests
- To allow nesting of the read-side explicit annotations we explicitly
keep track of the nesting. lock_is_held() allows us to do that.
- The wait-side annotation is a write lock, and entirely done within
dma_fence_wait() for everyone by default.
- To be able to freely annotate helper functions I want to make it ok
to call dma_fence_begin/end_signalling from soft/hardirq context.
First attempt was using the hardirq locking context for the write
side in lockdep, but this forces all normal spinlocks nested within
dma_fence_begin/end_signalling to be spinlocks. That bollocks.
The approach now is to simple check in_atomic(), and for these cases
entirely rely on the might_sleep() check in dma_fence_wait(). That
will catch any wrong nesting against spinlocks from soft/hardirq
contexts.
The idea here is that every code path that's critical for eventually
signalling a dma_fence should be annotated with
dma_fence_begin/end_signalling. The annotation ideally starts right
after a dma_fence is published (added to a dma_resv, exposed as a
sync_file fd, attached to a drm_syncobj fd, or anything else that
makes the dma_fence visible to other kernel threads), up to and
including the dma_fence_wait(). Examples are irq handlers, the
scheduler rt threads, the tail of execbuf (after the corresponding
fences are visible), any workers that end up signalling dma_fences and
really anything else. Not annotated should be code paths that only
complete fences opportunistically as the gpu progresses, like e.g.
shrinker/eviction code.
The main class of deadlocks this is supposed to catch are:
Thread A:
mutex_lock(A);
mutex_unlock(A);
dma_fence_signal();
Thread B:
mutex_lock(A);
dma_fence_wait();
mutex_unlock(A);
Thread B is blocked on A signalling the fence, but A never gets around
to that because it cannot acquire the lock A.
Note that dma_fence_wait() is allowed to be nested within
dma_fence_begin/end_signalling sections. To allow this to happen the
read lock needs to be upgraded to a write lock, which means that any
other lock is acquired between the dma_fence_begin_signalling() call and
the call to dma_fence_wait(), and still held, this will result in an
immediate lockdep complaint. The only other option would be to not
annotate such calls, defeating the point. Therefore these annotations
cannot be sprinkled over the code entirely mindless to avoid false
positives.
Originally I hope that the cross-release lockdep extensions would
alleviate the need for explicit annotations:
https://lwn.net/Articles/709849/
But there's a few reasons why that's not an option:
- It's not happening in upstream, since it got reverted due to too
many false positives:
commit e966eaeeb6
Author: Ingo Molnar <mingo@kernel.org>
Date: Tue Dec 12 12:31:16 2017 +0100
locking/lockdep: Remove the cross-release locking checks
This code (CONFIG_LOCKDEP_CROSSRELEASE=y and CONFIG_LOCKDEP_COMPLETIONS=y),
while it found a number of old bugs initially, was also causing too many
false positives that caused people to disable lockdep - which is arguably
a worse overall outcome.
- cross-release uses the complete() call to annotate the end of
critical sections, for dma_fence that would be dma_fence_signal().
But we do not want all dma_fence_signal() calls to be treated as
critical, since many are opportunistic cleanup of gpu requests. If
these get stuck there's still the main completion interrupt and
workers who can unblock everyone. Automatically annotating all
dma_fence_signal() calls would hence cause false positives.
- cross-release had some educated guesses for when a critical section
starts, like fresh syscall or fresh work callback. This would again
cause false positives without explicit annotations, since for
dma_fence the critical sections only starts when we publish a fence.
- Furthermore there can be cases where a thread never does a
dma_fence_signal, but is still critical for reaching completion of
fences. One example would be a scheduler kthread which picks up jobs
and pushes them into hardware, where the interrupt handler or
another completion thread calls dma_fence_signal(). But if the
scheduler thread hangs, then all the fences hang, hence we need to
manually annotate it. cross-release aimed to solve this by chaining
cross-release dependencies, but the dependency from scheduler thread
to the completion interrupt handler goes through hw where
cross-release code can't observe it.
In short, without manual annotations and careful review of the start
and end of critical sections, cross-relese dependency tracking doesn't
work. We need explicit annotations.
v2: handle soft/hardirq ctx better against write side and dont forget
EXPORT_SYMBOL, drivers can't use this otherwise.
v3: Kerneldoc.
v4: Some spelling fixes from Mika
v5: Amend commit message to explain in detail why cross-release isn't
the solution.

There exists a sleep-while-atomic bug while accessing the dmabuf->name
under mutex in the dmabuffs_dname(). This is caused from the SELinux
permissions checks on a process where it tries to validate the inherited
files from fork() by traversing them through iterate_fd() (which
traverse files under spin_lock) and call
match_file(security/selinux/hooks.c) where the permission checks happen.
This audit information is logged using dump_common_audit_data() where it
calls d_path() to get the file path name. If the file check happen on
the dmabuf's fd, then it ends up in ->dmabuffs_dname() and use mutex to
access dmabuf->name. The flow will be like below:
flush_unauthorized_files()
iterate_fd()
spin_lock() --> Start of the atomic section.
match_file()
file_has_perm()
avc_has_perm()
avc_audit()
slow_avc_audit()
common_lsm_audit()
dump_common_audit_data()
audit_log_d_path()
d_path()
dmabuffs_dname()
mutex_lock()--> Sleep while atomic.

So, use spinlock to access dmabuf->name to avoid sleep-while-atomic.
