After m68k's task_thread_info() doesn't refer to current,
it's possible to remove sched.h from interrupt.h and not break m68k!

Normally In HBA reset path MPT driver will flush existing work in current work
queue (mpt/0) . This is just a dummy activity for MPT driver point of
view, since HBA reset will turn off Work queue events.
It means we will simply returns from work queue without doing anything.
But for the case where Work is already done (half the way), we have to have
that work to be done.
Considering above condition we stuck forever since Deadlock in scsi midlayer
and MPT driver. sd_sync_cache() will wait forever since HBA is not in
Running state, and it will never come into Running state since
sd_sync_cache() is called from HBA reset context.
Now new code will not wait for half cooked work to be finished
before returning from HBA reset.
Once we are out of HBA reset, EH thread will change host state to running from
recovery and work waiting for running state of HBA will be finished.
New code is turning ON firmware event from another special work called
Rescan toplogy.

Do not set max_id value received from FW. Once SAS transport layer is

Convert magic values 1 and -1 to NETDEV_TX_BUSY and NETDEV_TX_LOCKED respectively.
0 (NETDEV_TX_OK) is not changed to keep the noise down, except in very few cases

Firmware is able to handle Broadcast primitives, but upstream driver does not

Device add interrupt from Firmware, we will not receive it as part of Reset
is going ON. After Reset we will do special Rescan of SAS topology.

SendEventNotification was handled through FIFO, now it is using doorbell to
communicate with hardware. Added Sleep Flag as an extra argument to support

sas_discovery_quiesce_io flag is used to control IO start/resume functionality.
IO will be stoped while doing discovery of topology. Once discovery is completed

The reason for this change is there is a data corruption when four different
physical memory regions in the 36GB to 37GB region are
accessed. This is only affecting 1078.
The solution is we need to use different addressing when filling in
the scatter gather table for the effected memory regions. So instead
of snooping on all four different memory holes, we treat any physical
addresses in the 36GB address with the same algorithm.
The fix is explained below
1) Ensure that the message frames are NOT located in the trouble
region. There is no remapping available for message frames, they must
be allocated outside the problem region.
2) Ensure that Sense buffers are NOT in the trouble region. There is
no remapping available.
3) Walk through the SGE entries and if any are inside the trouble region
then they need to be remapped as discussed below.
1) Set the Local Address bit in the SGE Flags field.
MPI_SGE_FLAGS_LOCAL_ADDRESS
2) Ensure we are using 64-bit SGEs
3) Set MSb (Bit 63) of the 64-bit address, this will indicate buffer
location is Host Memory.

Until now we have had a 1:1 mapping between storage device physical
block size and the logical block sized used when addressing the device.
With SATA 4KB drives coming out that will no longer be the case. The
sector size will be 4KB but the logical block size will remain
512-bytes. Hence we need to distinguish between the physical block size
and the logical ditto.
This patch renames hardsect_size to logical_block_size.

In commit c3a4d78c58, while introducing
rq->resid_len, the default value of residue count was changed from
full count to zero. The conversion was done under the assumption that
when a request fails residue count wasn't defined. However, Boaz and
James pointed out that this wasn't true and the residue count should
be preserved for failed requests too.
This patchset restores the original behavior by setting rq->resid_len
to blk_rq_bytes(rq) on request start and restoring explicit clearing
in affected drivers. While at it, take advantage of the fact that
rq->resid_len is set to full count where applicable.

Till now block layer allowed two separate modes of request execution.
A request is always acquired from the request queue via
elv_next_request(). After that, drivers are free to either dequeue it
or process it without dequeueing. Dequeue allows elv_next_request()
to return the next request so that multiple requests can be in flight.
Executing requests without dequeueing has its merits mostly in
allowing drivers for simpler devices which can't do sg to deal with
segments only without considering request boundary. However, the
benefit this brings is dubious and declining while the cost of the API
ambiguity is increasing. Segment based drivers are usually for very
old or limited devices and as converting to dequeueing model isn't
difficult, it doesn't justify the API overhead it puts on block layer
and its more modern users.
Previous patches converted all block low level drivers to dequeueing
model. This patch completes the API transition by...
* renaming elv_next_request() to blk_peek_request()
* renaming blkdev_dequeue_request() to blk_start_request()
* adding blk_fetch_request() which is combination of peek and start
* disallowing completion of queued (not started) requests
* applying new API to all LLDs
Renamings are for consistency and to break out of tree code so that
it's apparent that out of tree drivers need updating.

plat-omap/mailbox, floppy, viocd, mspro_block, i2o_block and
mmc/card/queue are already pretty close to dequeueing model and can be
converted with simple changes. Convert them.

With recent cleanups, there is no place where low level driver
directly manipulates request fields. This means that the 'hard'
request fields always equal the !hard fields. Convert all
rq->sectors, nr_sectors and current_nr_sectors references to
accessors.
While at it, drop superflous blk_rq_pos() < 0 test in swim.c.

Implement accessors - blk_rq_pos(), blk_rq_sectors() and
blk_rq_cur_sectors() which return rq->hard_sector, rq->hard_nr_sectors
and rq->hard_cur_sectors respectively and convert direct references of
the said fields to the accessors.
This is in preparation of request data length handling cleanup.

rq->data_len served two purposes - the length of data buffer on issue
and the residual count on completion. This duality creates some
headaches.
First of all, block layer and low level drivers can't really determine
what rq->data_len contains while a request is executing. It could be
the total request length or it coulde be anything else one of the
lower layers is using to keep track of residual count. This
complicates things because blk_rq_bytes() and thus
[__]blk_end_request_all() relies on rq->data_len for PC commands.
Drivers which want to report residual count should first cache the
total request length, update rq->data_len and then complete the
request with the cached data length.
Secondly, it makes requests default to reporting full residual count,
ie. reporting that no data transfer occurred. The residual count is
an exception not the norm; however, the driver should clear
rq->data_len to zero to signify the normal cases while leaving it
alone means no data transfer occurred at all. This reverse default
behavior complicates code unnecessarily and renders block PC on some
drivers (ide-tape/floppy) unuseable.
This patch adds rq->resid_len which is used only for residual count.
While at it, remove now unnecessasry blk_rq_bytes() caching in
ide_pc_intr() as rq->data_len is not changed anymore.

end_request() has been kept around for backward compatibility;
however, it's about time for it to go away.
* There aren't too many users left.
* Its use of @updtodate is pretty confusing.
* In some cases, newer code ends up using mixture of end_request() and
[__]blk_end_request[_all](), which is way too confusing.
So, add [__]blk_end_request_cur() and replace end_request() with it.
Most conversions are straightforward. Noteworthy ones are...
* paride/pcd: next_request() updated to take 0/-errno instead of 1/0.
* paride/pf: pf_end_request() and next_request() updated to take
0/-errno instead of 1/0.
* xd: xd_readwrite() updated to return 0/-errno instead of 1/0.
* mtd/mtd_blkdevs: blktrans_discard_request() updated to return
0/-errno instead of 1/0. Unnecessary local variable res
initialization removed from mtd_blktrans_thread().

Setting ->owner as done currently (pde->owner = THIS_MODULE) is racy
as correctly noted at bug #12454. Someone can lookup entry with NULL
->owner, thus not pinning enything, and release it later resulting
in module refcount underflow.
We can keep ->owner and supply it at registration time like ->proc_fops
and ->data.
But this leaves ->owner as easy-manipulative field (just one C assignment)
and somebody will forget to unpin previous/pin current module when
switching ->owner. ->proc_fops is declared as "const" which should give
some thoughts.
->read_proc/->write_proc were just fixed to not require ->owner for
protection.
rmmod'ed directories will be empty and return "." and ".." -- no harm.
And directories with tricky enough readdir and lookup shouldn't be modular.
We definitely don't want such modular code.
Removing ->owner will also make PDE smaller.
So, let's nuke it.

A bug in the fusion driver was exposed by the switch to block timeout.
Basically, drivers are supposed to terminate commands once error
handling begins on them. The fusion apparently wasn't doing this.
Under the old timeout regime, completions on terminated commands would
by and large get ignored because of the way command timeouts used to
work. The new block timers are very intolerant to this, though,
becuase the request gets cleaned and freed.

As it is, all instances of ->release() for files that have ->fasync()
need to remember to evict file from fasync lists; forgetting that
creates a hole and we actually have a bunch that *does* forget.
So let's keep our lives simple - let __fput() check FASYNC in
file->f_flags and call ->fasync() there if it's been set. And lose that
crap in ->release() instances - leaving it there is still valid, but we
don't have to bother anymore.

To keep the size of changesets sane we split the switch by drivers;
to keep the damn thing bisectable we do the following:
1) rename the affected methods, add ones with correct
prototypes, make (few) callers handle both. That's this changeset.
2) for each driver convert to new methods. *ALL* drivers
are converted in this series.
3) kill the old (renamed) methods.
Note that it _is_ a flagday; all in-tree drivers are converted and by the
end of this series no trace of old methods remain. The only reason why
we do that this way is to keep the damn thing bisectable and allow per-driver
debugging if anything goes wrong.

The I2O ioctls assume 32bits. In itself that is fine as they are old
cards and nobody uses 64bit. However on LKML it was noted this
assumption is also made for allocated memory and is unsafe on 64bit
systems.
Fixing this is a mess. It turns out there is tons of crap buried in a
header file that does racy 32/64bit filtering on the masks.
So we:
- Verify all callers of the racy code can sleep (i2o_dma_[re]alloc)
- Move the code into a new i2o/memory.c file
- Remove the gfp_mask argument so nobody can try and misuse the function
- Wrap a mutex around the problem area (a single mutex is easy to do and
none of this is performance relevant)
- Switch the remaining problem kmalloc holdout to use i2o_dma_alloc

Kobjects do not have a limit in name size since a while, so stop

When the firmware is in Fault state it will be identifed only when the next time
the driver access the IOC state.
This patch includes a polling function in the driver which will be executed in
regular interval to check the status of the firmware and if it is in Fault
state, then the firmware will be reset by the driver.

There's a fault on the FC controllers that makes them not respond
correctly to MSI. The SPI controllers are fine, but are likely to be
onboard on older motherboards which don't handle MSI correctly, so
default both these cases to disabled. Enable by setting the module
parameter mpt_msi_enable=1.
For the SAS case, enable MSI by default, but it can be disabled by
setting the module parameter mpt_msi_enable=0.

The problem here is that if the ioc faults too early in the bring up
sequence (as it usually does for an irq routing problem), ioc_reset gets
called before the scsi host is even allocated. This causes an oops when
it later schedules a renegotiation. Fix this by checking ioc->sh before
trying to renegotiate.

Following a hard reset of a SAS raid, one of the raid targets is occasionally
missing. I tracked this down to a pretty obscure little bug.
The LSI fusion drivers for SAS and Fibre Channel both use their respective
transport layers. Those transport layers increment the target number
assigned to new targets.
The routine __scsi_scan_target uses the "this_id" element of the Scsi_Host
structure to avoid scanning the scsi host adapter. Both fusion drivers set
"this_id" from a value returned in a firmware PortFacts response. For my
particular test case (SAS) the firmware id assigned to the initiator was
173. After enough raid resets to cause the raid targets to go and come a
sufficient number of times, the id assigned by the transport to a raid
target would match the id assigned by the host adapter to the "this_id"
field, resulting in that target not being scanned.
Fix by not assigning this_id and not checking it in slave_configure.

While performing hardware raid reset testing via the raid's client, I
noticed that sometimes, following the reset, that there would be more
raid targets in the lsscsi output than there actually were raid
targets. I tracked this down to the following issue.
Fusion cannot always find the mptsas_portinfo structure for the hba
because it uses the handle stored in ioc->handle to locate it. The
problem is that the firmware can change the handle associated with the
hba when h/w raid is reset (via the raid client). When this happens,
the driver will allocate another mptsas_portinfo structure and link it
into the chain of said structures. This ultimately causes confusion
within the driver resulting in targets not being removed when they
should be.
Eric Moore pointed out that the hba's portinfo structure is always the
first structure on the sas_topology list. This patch modifies
mptsas.c to access the hba's portinfo structure by taking the first
structure on said list.

This patch modifies the I/O resource allocation behavior of FUSION
driver. The current version of driver allocates the I/O resources
even if they are not required and this creates trouble in low resource
environments. This driver now uses
pci_enable_device_mem/pci_enable_device functions to differentiate the
resource allocations.

This patch fixes the module unload problem in flash less 1030
controller environment where firmware download boot functionality is
invoked. The problem is due to the firmware download is being done in
the reverse order, which this patch solves by insureing the download
occurs to the last controller being reset.

NB = number of blocks. This represents the number of blocks to
transfer. The block size is based on the message frame size provided
in the ioc_facts. A value of zero indicates the entire message frame
should be copied. This is two bit value. So by setting this to
non-zero vaule, you increase performance by reducing amount of data
needing to be dma'd. The value that is stored in ioc->ReqeustNB is
sometimes a non-zero vaule, which creates a bug in mptlan, where not
the entire message frame is getting transfer to firware, resulting in
corruption. This fix sets the default to zero, thus entire message
frame is copied.

Cleaning up prints that use the xxx_printk API, in that the fusion
preamble "mptbase: iocX" follows the info provided by the print API.
The way its currently coded, the [H:C:T] print in sdev_printk will be
inbetween "mptbase" and "iocX", instead of before.

ScsiLookup is an array of pending scmd pointers that the scsi lld
maintains. This array is touched from queuecommand, eh threads, and
interrupt context. This array should put under locks, hence this patch
to synchronize its access. I've added some nice little function
wrappers for this, and moved the ScsiLookup array over to MPT_ADAPTER
struct.

The Coverity checker spotted two potential memory leaks in
drivers/message/fusion/mptbase.c::mpt_attach().
There are two returns that may leak the storage allocated for 'ioc'
(sizeof(MPT_ADAPTER) bytes).
A simple fix would be to simply add two kfree() calls before the return
statements, but a better fix (that this patch implements) is to reorder the
code so that if we hit the first return condition we don't have to do the
allocation at all and then just add a kfree() call for the second case.

This patch adds a new file mptdebug.h in the fusion source directory,
which contains different debug macros. The existing debug macros and
flags are removed from the mptbase.h and Makefile In Kconfig a new
configuration parameter FUSION_LOGGING is added to enable/disable the
logging support during compile time.

Slab destructors were no longer supported after Christoph's
c59def9f22 change. They've been
BUGs for both slab and slub, and slob never supported them
either.
This rips out support for the dtor pointer from kmem_cache_create()
completely and fixes up every single callsite in the kernel (there were
about 224, not including the slab allocator definitions themselves,
or the documentation references).

This patch fixes access to memory that has not been allocated:
i2o_msg_get_wait() can returns errors different from I2O_QUEUE_EMPTY. But the
result is checked only against this code. If it is not I2O_QUEUE_EMPTY then
we dereference the error code as the pointer later.

This patch fixes a number of issues in i2o_cfg_passthru{,32}:
- i2o_msg_get_wait() return vaile is not checked;
- i2o_message memory leaks on error paths;
- infinite loop to sg_list_cleanup in passthru32
It's important issue because of i2o_cfg_passthru is used by raidutils for
monitorig controllers state, and in case of memory shortage it leads to the
node crash or disk IO stall.

If there is IO going to the volume while a hidden disk is being torn down,
there is a case where we would return a DID_NO_CONNECT for IO sent to the
volume. The end result is the volume goes read-only. This problem is due to the
fact the firmware mapped target ids saved in per device object is phys_disk_num
for hidden raid components, and target_id for the volume. There is a single
case when both phys_disk_num and target_id are equal, so enters this issue. We
fix this issue by checking the tflags when the device is torned down,
insuring the IO being completed is meant for hidden raid component, not the
volume. In addition to this fix, there are a couple other cases to address
hidden raid components. For instance task_abort and device reset are not
supported by mpt fw for hidden raid components, a bus reset would be required
or target reset to volume.

inadvertently change the order some code was called. This caused
a massive slowdown (of 150x to 300x) on the CD/DVD drive on the

Maximum number of I2O drivers which could be registered is configurable by
max_drivers module parameter.
But the module parameter is ignored and default value (I2O_MAX_DRIVERS = 8)
is used in the loops to notify all registered drivers.

i2o_driver_register() initalizes event queue for driver only when
drv->event is set. So similarly the event queue should be destroyed only
when drv->event is set in the error path. Otherwise destroy_workqueue()
will called with NULL.

After host reset, the device are programmed to default asyn narrow nego.
We need to reprogram the parameter back to previous values. If the host
reset is called as a result of spi_dv_device() commands timing out, its
possible to get into an infinite loop of dv to host reset. This will
prevent that case, as we merely program old values. If host reset is
called outside context of domain validation, then we can call
spi_dv_device.

For the places where we need a pointer to the mac header, it is still legal to
touch skb->mac.raw directly if just adding to, subtracting from or setting it
to another layer header.
This one also converts some more cases to skb_reset_mac_header() that my
regex missed as it had no spaces before nor after '=', ugh.

For the common, open coded 'skb->mac.raw = skb->data' operation, so that we can
later turn skb->mac.raw into a offset, reducing the size of struct sk_buff in
64bit land while possibly keeping it as a pointer on 32bit.
This one touches just the most simple case, next will handle the slightly more
"complex" cases.

I2O subsystem has been broken in mainstream several months ago (after
2.6.18). Commit 4aff5e2333 from Jens
Axboe split struct request ->flags into two parts: cmd_type and
cmd_flags.
In i2o layer this patch has replaced flag REQ_SPECIAL by the according
cmd_type. However i2o has used REQ_SPECIAL not as command type but as
driver-specific flag for the debug purposes. As result all i2o requests
have type "special" now, are not processed to the hardware and fail with
I/O error:

This address the issue of VMWare guest OS being remounted as read-only
becuase the underlying device was held busy too long and at the
same time address Engenio MPP driver concerns over infinite retries.
This patch removes the code that snoops the SAM STATUS on busy, which
would be returning DID_BUS_BUSY, instead we return the status as is.
Retry hanlding seems to be properly handled in scsi_softirq_done,
where a busy sam status would only occurr for the time specified by
(cmd->allowed +1) * cmd->timeout_per_command.

This fix's an oops during driver load time. mptsas_probe calls
mpt_attach(over in mptbase.c). Inside that call, we read some
manufacturing config pages to setup some defaults. While reading the
config pages, the firmware doesn't complete the reply in time, and we
have a timeout. The timeout results in hardreset handler being called.
The hardreset handler calls all the fusion upper layer driver reset
callback handlers. The mptsas_ioc_reset function is the callback
handler in mptsas.c. So where I'm getting to, is mptsas_ioc_reset is
getting called before scsi_host_alloc is called, and the pointer ioc->sh
is NULL as well as the hostdata.

After Al Viro (finally) succeeded in removing the sched.h #include in module.h
recently, it makes sense again to remove other superfluous sched.h includes.
There are quite a lot of files which include it but don't actually need
anything defined in there. Presumably these includes were once needed for
macros that used to live in sched.h, but moved to other header files in the
course of cleaning it up.
To ease the pain, this time I did not fiddle with any header files and only
removed #includes from .c-files, which tend to cause less trouble.
Compile tested against 2.6.20-rc2 and 2.6.20-rc2-mm2 (with offsets) on alpha,
arm, i386, ia64, mips, powerpc, and x86_64 with allnoconfig, defconfig,
allmodconfig, and allyesconfig as well as a few randconfigs on x86_64 and all
configs in arch/arm/configs on arm. I also checked that no new warnings were
introduced by the patch (actually, some warnings are removed that were emitted
by unnecessarily included header files).

Fusion firmware requires target reset following hotplug removal event,
with purpose to flush target outstanding request in fw. Current implementation
does the target resets from delayed work tasks, that in heavy load
conditions, take too long to be invoked, resulting in command time outs
This patch will issue target reset immediately from ISR context, and will
queue remaining target resets to be issued after the previous one completes.
The delayed work tasks are spawned during the target reset completion.

This patch makes the mptctl pass through available if
the mptsas driver is selected. Without this patch
if mptsas is the only fusion driver chosen, then
the mptctl is not presented as an option.
smp_utils uses the mptctl driver to pass SAS SMP
functions through a MPT SAS HBA.

The fibre channel IOC may kill a request for a variety of
reasons, some of which may be recovered by a retry, some of
which are unlikely to be recovered. Return DID_ERROR
instead of DID_RESET to permit retry of the command,
just not an infinite number of them.

This avoids device offline scenarios due to errors in the error handler.
Also verify that VirtDevice is available before issuing scsi command.
VirtDevice is removed when fc transport removes a target.

Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.
The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around. On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).
Where appropriate, an arch may override the generic storage facility and do
something different with the variable. On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.
Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions. Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller. A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.
I've build this code with allyesconfig for x86_64 and i386. I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.
This will affect all archs. Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:

Don't pass regs through to generic_handle_irq() or __do_IRQ().
In timer_interrupt(), this sort of change will be necessary:
- update_process_times(user_mode(regs));
- profile_tick(CPU_PROFILING, regs);
+ update_process_times(user_mode(get_irq_regs()));
+ profile_tick(CPU_PROFILING);
I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().
Some notes on the interrupt handling in the drivers:
(*) input_dev() is now gone entirely. The regs pointer is no longer stored in
the input_dev struct.
(*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking. It does
something different depending on whether it's been supplied with a regs
pointer or not.
(*) Various IRQ handler function pointers have been moved to type
irq_handler_t.

Make it possible to disable the block layer. Not all embedded devices require
it, some can make do with just JFFS2, NFS, ramfs, etc - none of which require
the block layer to be present.
This patch does the following:
(*) Introduces CONFIG_BLOCK to disable the block layer, buffering and blockdev
support.
(*) Adds dependencies on CONFIG_BLOCK to any configuration item that controls
an item that uses the block layer. This includes:
(*) Block I/O tracing.
(*) Disk partition code.
(*) All filesystems that are block based, eg: Ext3, ReiserFS, ISOFS.
(*) The SCSI layer. As far as I can tell, even SCSI chardevs use the
block layer to do scheduling. Some drivers that use SCSI facilities -
such as USB storage - end up disabled indirectly from this.
(*) Various block-based device drivers, such as IDE and the old CDROM
drivers.
(*) MTD blockdev handling and FTL.
(*) JFFS - which uses set_bdev_super(), something it could avoid doing by
taking a leaf out of JFFS2's book.
(*) Makes most of the contents of linux/blkdev.h, linux/buffer_head.h and
linux/elevator.h contingent on CONFIG_BLOCK being set. sector_div() is,
however, still used in places, and so is still available.
(*) Also made contingent are the contents of linux/mpage.h, linux/genhd.h and
parts of linux/fs.h.
(*) Makes a number of files in fs/ contingent on CONFIG_BLOCK.
(*) Makes mm/bounce.c (bounce buffering) contingent on CONFIG_BLOCK.
(*) set_page_dirty() doesn't call __set_page_dirty_buffers() if CONFIG_BLOCK
is not enabled.
(*) fs/no-block.c is created to hold out-of-line stubs and things that are
required when CONFIG_BLOCK is not set:
(*) Default blockdev file operations (to give error ENODEV on opening).
(*) Makes some /proc changes:
(*) /proc/devices does not list any blockdevs.
(*) /proc/diskstats and /proc/partitions are contingent on CONFIG_BLOCK.
(*) Makes some compat ioctl handling contingent on CONFIG_BLOCK.
(*) If CONFIG_BLOCK is not defined, makes sys_quotactl() return -ENODEV if
given command other than Q_SYNC or if a special device is specified.
(*) In init/do_mounts.c, no reference is made to the blockdev routines if
CONFIG_BLOCK is not defined. This does not prohibit NFS roots or JFFS2.
(*) The bdflush, ioprio_set and ioprio_get syscalls can now be absent (return
error ENOSYS by way of cond_syscall if so).
(*) The seclvl_bd_claim() and seclvl_bd_release() security calls do nothing if
CONFIG_BLOCK is not set, since they can't then happen.

Right now ->flags is a bit of a mess: some are request types, and
others are just modifiers. Clean this up by splitting it into
->cmd_type and ->cmd_flags. This allows introduction of generic
Linux block message types, useful for sending generic Linux commands
to block devices.

This flag denotes local attachment of the phy. There are two problems
with it:
1) It's actually redundant ... you can get the same information simply
by seeing whether a host is the phys parent
2) we condition a lot of phy parameters on it on the false assumption
that we can only control local phys. I'm wiring up phy resets in the
aic94xx now, and it will be able to reset non-local phys as well.
I fixed 2) by moving the local check into the reset and stats function
of the mptsas, since that seems to be the only HBA that can't
(currently) control non-local phys.

This patch corrects a problem in mptfc which can result in targets
being removed after executing an "lsiutil 99" reset of the fibre
channel ports.
The last rescan event was being processed before the setup reset work
due to an inappropriate optimization in the event processing logic.
Every rescan event is now queued for execution and the setup reset
work now executes in the proper sequence.

One of the current problems the mptsas driver has is that of "ghost"
devices (these are devices the firmware reports as existing, but what
they actually represent are the parents of a lower device), so for
example in my dual expander configuration, three expanders actually show
up, two for the real expanders but a third is created because the
firmware reports that the lower expander also has another expander
connected (which is simply the port going back to the upper expander).
The attached patch eliminates all these ghosts by not allocating any
devices for them if the SAS address is the SAS address of the parent.

Fixed up trivial conflict in drivers/ide/pci/cmd64x.c (the printk that
was changed by the 64-bit resources had been deleted in the meantime ;)

* Adding 1078 ROC (Raid On Chip) Support - New host adapter
* Moving all PCI Vendor/Device ids to using internal defines; a request
from Christoph/James B. some time ago for when the next chip was added.
* Removing SAS 1066/1066E Vendor/Device IDs, as there are no plans to
manufacture that controller.

* Wide port support added - using James Bottomley's new SAS wide port API.
(There is a known problem in sas transport layer reported yesterday to
James. The Kobject dev.bus_ids for end devices are not unique across
expanders. I have added a work around in this patch, where I asigning
an unique port identifier for every port within the host - this solves
the problem, but I expect a fix from James in the sas transport).
* Adding target_alloc and target_destroy entry points, and moving code over
from the slave entry points.
* The renaming of some mptscsih_xxx functions declared in mptsas.c,
to mptsas_xxx.
* Target Reset moved from slave_destroy to hotplug work thread
handling (with regard to device removal). Also inhibit IO to end device
while device is being broken down . Talked to James Smart about this
at Linux Expo (with questions of how the fc transport handles this).
* Cleaning up the kzalloc's, and kfree's
