A Gentoo bug report [1] showed that as of 2.6.31 lpfc only uses INTx interrupts.
This patch restores lpfc's ability to support MSI-X/MSI interrupts that the
"Addition of SLI4 Interface - Base Support" patch [2] broke.
It reestablishes MSI-X as the default interrupt method and in case MSI-X is not
supported lpfc_sli{4,}_enable_intr fallbacks to MSI and then to INTx.

Background:
Nothing Broke. This was intended.
We had originally enabled MSI-X by default, but in qualification within the
last 12 months, we encountered a major catch-22:
There were at least 4 platforms, from 2 major OEMs, that :
- Say they support MSI-X - platform routines work and act as if they do.
- We enable it, generate a test interrupt to check they really do deliver it,
and it works.
- But shortly after attachment, the system hangs or loses interrupts,
resulting in a bad system behavior.
Given the distro's picking up the 2.6.32 kernel, we had to stick with a
default of MSI-X off, with user-enabled MSI-X as these platforms couldn't get
fixed.
However, we're also now encountering platforms that require MSI-X and never
INTx, so we must change. It's desired also for also for performance reasons.
So - now (2.6.33) is the right time to re-enable MSI-X by default.

In testing 2.6.31 on one of our ia64 platforms I've encountered a hang
due to the driver using hardware ATEs which are a limited resource.
This is because the driver does not set the dma consistent mask to
64 bits.

Some of our virtual SCSI hosts don't have a proper bus parent at the
top, which can be a problem for doing DMA on them
This patch makes the host device cache a pointer to the physical bus
device and provides an extra API for setting it (the normal API picks
it up from the parent). This patch also modifies the qla2xxx and lpfc
vport logic to use the new DMA host setting API.

The checksum format is orthogonal to whether the protection information
is being passed on beyond the HBA or not. It is perfectly valid to use
a non-T10 CRC with WRITE_STRIP and READ_INSERT.
Consequently it no longer makes sense to explicitly refer to the
conversion in the protection operation. Update sd_dif and lpfc
accordingly.

Consistently implement persistent port disable.
Ability was to be managed in the adapter via firmware via flash settings.

Somewhere, it never get merged. The patch restructures the task mgmt
routines, commonizing like behavior. Then the patch changes device

Adds support for the new queues in the SLI-4 interface. There are :
- Work Queues - host-to-adapter for fast-path traffic
- Mailbox Queues - host-to-adapter for control (slow-path)
- Buffer Queues - host-to-adapter for posting buffers for async receive
- Completion Queues - adapter-to-host for posting async events,
completions for fast or slow patch work, receipt of async
receive traffic
- Event Queues - tied to MSI-X vectors, binds completion queues with
interrupts
These patches add the all the support code to tie into command submission
and response paths, updates the interrupt handling, etc.

Preps the organization of the driver so that the bottom half, which
interacts with the hardware, can share common code sequences for
attachment, detachment, initialization, teardown, etc with new hardware.
For very common code sections, which become specific to the interface
type, the driver uses an indirect function call. The function is set at
initialization. For less common sections, such as initialization, the
driver looks at the interface type and calls the routines relative to
the interface.

With recent cleanups, there is no place where low level driver
directly manipulates request fields. This means that the 'hard'
request fields always equal the !hard fields. Convert all
rq->sectors, nr_sectors and current_nr_sectors references to
accessors.
While at it, drop superflous blk_rq_pos() < 0 test in swim.c.

Fix lpfc_parse_bg_err()'s use of do_div(). It should be passing a 64-bit
variable as the first parameter. However, since it's only using a 32-bit
variable, it doesn't need to use do_div() at all, but can instead use the
division operator.
This deals with the following warnings:

Fixed up trivial conflict due to moving the async part of sd_probe
around in the async probes vs using dev_set_name() in naming.
In each case, if the NULL test is necessary, then the dereference should be
moved below the NULL test.

Per the recent discussions at the Linux Plumbers Conference, when
enabling MSI or MSI-X, generate a test interrupt to verify the
interrupt routing is working properly. If the test interrupt fails,
fall back to MSI first, and if that fails as well, to INTx. If the
interrupt test fails with INTx, log an error and fail the PCI probe.
Also changed the use of spin_(lock|unlock) to the _irq(save|restore)
variants in the interrupt handlers because with multi-message MSI-X,
both interrupt handlers can now run in parallel.

The phba hbq_get address was setup before the HBA reported support for
Interrupt Enable Block (INB). This caused loopback tests to fail
after all HBQs had been used once. Moved setting of hba_get until
after config port mailbox command completes.

- Fix lock up on rmmod with vports defined by having
lpfc_pci_remove_one() invoke fc_vport_terminate() to remove all the
vports before invoking fc_remove_host() for the physical port
- Fix echotest failure when NPIV is enabled
- Add the vport_disable function to the physical port's transport
template to make the vport disable attribute visible
- Set the vport state to DISABLE on create if the disable flag is
true
- Call lpfc_alloc_sysfs_attr() for vports so that statistical data
collection works on them
- Support setting a vport's symbolic name via sysfs by writing to
/sys/class/fc_vport/vportX/symbolic_name
- Fix create vport fails when link is down or in loop mode. Should be
able to be create vports any time NPIV is enabled
- Fix slow vport deletes when deleting multiple vports at once
