This adds nowait support to the RAID10 driver. Very similar to
raid1 driver changes. It makes RAID10 driver return with EAGAIN
for situations where it could wait for eg:
- Waiting for the barrier,
- Reshape operation,
- Discard operation.
wait_barrier() and regular_request_wait() fn are modified to return bool
to support error for wait barriers. They returns true in case of wait
or if wait is not required and returns false if wait was required
but not performed to support nowait.

commit 021a24460d ("block: add QUEUE_FLAG_NOWAIT") added support
for checking whether a given bdev supports handling of REQ_NOWAIT or not.
Since then commit 6abc49468e ("dm: add support for REQ_NOWAIT and enable
it for linear target") added support for REQ_NOWAIT for dm. This uses
a similar approach to incorporate REQ_NOWAIT for md based bios.
This patch was tested using t/io_uring tool within FIO. A nvme drive
was partitioned into 2 partitions and a simple raid 0 configuration
/dev/md0 was created.

Before patch:

Running top while the above runs:

We can see iou-wrk-38397 io worker thread created which gets created
when io_uring sees that the underlying device (/dev/md0 in this case)
doesn't support nowait.
After patch:
$ ./t/io_uring /dev/md0 -p 0 -a 0 -d 1 -r 100
Running top while the above runs:
$ ps -eL | grep $(pidof io_uring)
38341 38341 pts/2 00:10:22 io_uring
38341 38342 pts/2 00:10:37 io_uring
After running this patch, we don't see any io worker thread
being created which indicated that io_uring saw that the
underlying device does support nowait. This is the exact behaviour
noticed on a dm device which also supports nowait.
For all the other raid personalities except raid0, we would need
to train pieces which involves make_request fn in order for them
to correctly handle REQ_NOWAIT.

As suggested by Neil Brown[1], this limitation seems to be
deprecated.
With plugging in use, writes are processed behind the raid thread
and conf->pending_count is not increased. This limitation occurs only
if caller doesn't use plugs.
It can be avoided and often it is (with plugging). There are no reports
that queue is growing to enormous size so remove queue limitation for
non-plugged IOs too.

raid_run_ops() relies on the implicitly disabled preemption for
its percpu ops, although this is really about CPU locality. This
breaks RT semantics as it can take regular (and thus sleeping)
spinlocks, such as stripe_lock.
Add a local_lock such that non-RT does not change and continues
to be just map to preempt_disable/enable, but makes RT happy as
the region will use a per-CPU spinlock and thus be preemptible
and still guarantee CPU locality.

There are currently 2 ways to create a set of sysfs files for a
kobj_type, through the default_attrs field, and the default_groups
field. Move the dm sysfs code to use default_groups field which has
been the preferred way since aa30f47cf6 ("kobject: Add support for
default attribute groups to kobj_type") so that we can soon get rid of
the obsolete default_attrs field.

These methods indirect the actual DAX read/write path. In the end pmem
uses magic flush and mc safe variants and fuse and dcssblk use plain ones
while device mapper picks redirects to the underlying device.
Add set_dax_nocache() and set_dax_nomc() APIs to control which copy
routines are used to remove indirect call from the read/write fast path
as well as a lot of boilerplate code.

adding the biovec's bv_offset. As such using bvec_kmap_local() caused

Commit 0259d4498b ("bcache: move calc_cached_dev_sectors to proper
place on backing device detach") tries to fix calc_cached_dev_sectors
when bcache device detaches, but now we have:

Upper codeflows shows how bug happens, this patch fix the problem by
caching dc->disk.c beforehand, and cache_set won't be freed under us
because c->caching closure at least holds a reference count and closure
callback __cache_set_unregister only being called by bch_cache_set_stop
which using closure_queue(&c->caching), that means c->caching closure
callback for destroying cache_set won't be trigger by previous
closure_put(&d->c->caching).
So at this stage(while cached_dev_detach_finish is calling) it's safe to
access cache_set dc->disk.c.

In driver/md/md.c, if the function autorun_array() is called,
the problem of double free may occur.
In function autorun_array(), when the function do_md_run() returns an
error, the function do_md_stop() will be called.
The function do_md_run() called function md_run(), but in function
md_run(), the pointer mddev->private may be freed.
The function do_md_stop() called the function __md_stop(), but in
function __md_stop(), the pointer mddev->private also will be freed
without judging null.
At this time, the pointer mddev->private will be double free, so it
needs to be judged null or not.

The superblock of version 1.0 doesn't get moved to the new position on a
device size change. This leads to a rdev without a superblock on a known
position, the raid can't be re-assembled.
The line was removed by mistake and is re-added by this patch.

The device mapper DAX support is all hanging off a block device and thus
can't be used with device dax. Make it depend on CONFIG_FS_DAX instead
of CONFIG_DAX_DRIVER. This also means that bdev_dax_pgoff only needs to
be built under CONFIG_FS_DAX now.

All modern drivers can support extra partitions using the extended
dev_t. In fact except for the ioctl method drivers never even see
partitions in normal operation.
So remove the GENHD_FL_EXT_DEVT and allow extra partitions for all
block devices that do support partitions, and require those that
do not support partitions to explicit disallow them using
GENHD_FL_NO_PART.

The above commit replaces page_address(bv->bv_page) by bvec_virt(bv) to
avoid directly access to bv->bv_page, but in situation bv->bv_offset is
not zero and page_address(bv->bv_page) is not equal to bvec_virt(bv). In
such case a memory corruption may happen because memory in next page is
tainted by following line in do_btree_node_write(),
memcpy(bvec_virt(bv), addr, PAGE_SIZE);
This patch reverts the mentioned commit to avoid the memory corruption.

In bcache_device_free(), pointer disk is referenced still in
ida_simple_remove() after blk_cleanup_disk() gets called on this
pointer. This may cause a potential panic by use-after-free on the
disk pointer.
This patch fixes the problem by calling blk_cleanup_disk() after
ida_simple_remove().

For fixing queue quiesce race between driver and block layer(elevator
switch, update nr_requests, ...), we need to support concurrent quiesce
and unquiesce, which requires the two call to be balanced.
__bind() is only called from dm_swap_table() in which dm device has been
suspended already, so not necessary to stop queue again. With this way,
request queue quiesce and unquiesce can be balanced.

We never checked for errors on add_disk() as this function returned
void. Now that this is fixed, use the shiny new error handling.
There are two calls to dm_setup_md_queue() which can fail then, one on
dm_early_create() and we can easily see that the error path there
calls dm_destroy in the error path. The other use case is on the ioctl
table_load case. If that fails userspace needs to call the
DM_DEV_REMOVE_CMD to cleanup the state - similar to any other
failure.

The header file include/uapi/linux/bcache.h is not really a user space
API heaer. This file defines the ondisk format of bcache internal meta
data but no one includes it from user space, bcache-tools has its own
copy of this header with minor modification.
Therefore, this patch moves include/uapi/linux/bcache.h to bcache code
directory as drivers/md/bcache/bcache_ondisk.h.

Since dm-crypt target can be stacked on dm-integrity targets to
provide authenticated encryption, integrity violations are recognized
here during aead computation. We use the dm-audit submodule to
signal those events to user space, too.
The construction and destruction of crypt device mappings are also
logged as audit events.

dm-integrity signals integrity violations by returning I/O errors
to user space. To identify integrity violations by a controlling
instance, the kernel audit subsystem can be used to emit audit
events to user space. We use the new dm-audit submodule allowing
to emit audit events on relevant I/O errors.
The construction and destruction of integrity device mappings are
also relevant for auditing a system. Thus, those events are also
logged as audit events.

blk_keyslot_manager is misnamed because it doesn't necessarily manage
keyslots. It actually does several different things:
- Contains the crypto capabilities of the device.
- Provides functions to control the inline encryption hardware.
Originally these were just for programming/evicting keyslots;
however, new functionality (hardware-wrapped keys) will require new
functions here which are unrelated to keyslots. Moreover,
device-mapper devices already (ab)use "keyslot_evict" to pass key
eviction requests to their underlying devices even though
device-mapper devices don't have any keyslots themselves (so it
really should be "evict_key", not "keyslot_evict").
- Sometimes (but not always!) it manages keyslots. Originally it
always did, but device-mapper devices don't have keyslots
themselves, so they use a "passthrough keyslot manager" which
doesn't actually manage keyslots. This hack works, but the
terminology is unnatural. Also, some hardware doesn't have keyslots
and thus also uses a "passthrough keyslot manager" (support for such
hardware is yet to be upstreamed, but it will happen eventually).
Let's stop having keyslot managers which don't actually manage keyslots.
Instead, rename blk_keyslot_manager to blk_crypto_profile.
This is a fairly big change, since for consistency it also has to update
keyslot manager-related function names, variable names, and comments --
not just the actual struct name. However it's still a fairly
straightforward change, as it doesn't change any actual functionality.

In preparation for renaming struct blk_keyslot_manager to struct
blk_crypto_profile, rename the keyslot-manager.h and keyslot-manager.c
source files. Renaming these files separately before making a lot of
changes to their contents makes it easier for git to understand that
they were renamed.

We never checked for errors on add_disk() as this function
returned void. Now that this is fixed, use the shiny new
error handling.
This driver doesn't do any unwinding with blk_cleanup_disk()
even on errors after add_disk() and so we follow that
tradition.

We never checked for errors on add_disk() as this function
returned void. Now that this is fixed, use the shiny new
error handling.
There are two calls to dm_setup_md_queue() which can fail then,
one on dm_early_create() and we can easily see that the error path
there calls dm_destroy in the error path. The other use case is on
the ioctl table_load case. If that fails userspace needs to call
the DM_DEV_REMOVE_CMD to cleanup the state - similar to any other
failure.

Using local kmaps slightly reduces the chances to stray writes, and
the bvec interface cleans up the code a little bit.
Also switch from page_address to bvec_kmap_local for cbv to be on the
safe side and to avoid pointlessly poking into bvec internals.

Calculation of cache_set's cached sectors is done by travelling
cached_devs list as shown below:

it to its right place.

In register_bcache(), there are several cases we didn't set
correct error info (return value and/or error message):
- if kzalloc() fails, it needs to return ENOMEM and print
"cannot allocate memory";
- if register_cache() fails, it's better to propagate its
return value rather than using default EINVAL.

Commit 6607cd319b ("raid1: ensure write
behind bio has less than BIO_MAX_VECS sectors") tried to guarantee the
size of behind bio is not bigger than BIO_MAX_VECS sectors.
Unfortunately the same calltrace still could happen since an array could
enable write-behind without write mostly device.
To match the manpage of mdadm (which says "write-behind is only attempted
on drives marked as write-mostly"), we need to check WriteMostly flag to
avoid such unexpected behavior.

Replace the blk_poll interface that requires the caller to keep a queue
and cookie from the submissions with polling based on the bio.
Polling for the bio itself leads to a few advantages:
- the cookie construction can made entirely private in blk-mq.c
- the caller does not need to remember the request_queue and cookie
separately and thus sidesteps their lifetime issues
- keeping the device and the cookie inside the bio allows to trivially
support polling BIOs remapping by stacking drivers
- a lot of code to propagate the cookie back up the submission path can
be removed entirely.

DM uses blk-mq's quiesce/unquiesce to stop/start device mapper queue.
But blk-mq's unquiesce may come from outside events, such as elevator
switch, updating nr_requests or others, and request may come during
suspend, so simply ask for blk-mq to requeue it.
Fixes one kernel panic issue when running updating nr_requests and
dm-mpath suspend/resume stress test.

Without FEC, dm-verity won't call verity_handle_err() when I/O fails,
but with FEC enabled, it currently does even if an I/O error has
occurred.
If there is an I/O error and FEC correction fails, return the error
instead of calling verity_handle_err() again.

Commit b0140891a8 ("md: Fix race when creating a new md device.")
not only moved assigning mddev->gendisk before calling add_disk, which
fixes the races described in the commit log, but also added a
mddev->open_mutex critical section over add_disk and creation of the
md kobj. Adding a kobject after add_disk is racy vs deleting the gendisk
right after adding it, but md already prevents against that by holding
a mddev->active reference.
On the other hand taking this lock added a lock order reversal with what
is not disk->open_mutex (used to be bdev->bd_mutex when the commit was
added) for partition devices, which need that lock for the internal open
for the partition scan, and a recent commit also takes it for
non-partitioned devices, leading to further lockdep splatter.

The functions get_online_cpus() and put_online_cpus() have been
deprecated during the CPU hotplug rework. They map directly to
cpus_read_lock() and cpus_read_unlock().
Replace deprecated CPU-hotplug functions with the official version.
The behavior remains unchanged.

There is no point in trying to finding the dax device if the DAX flag is
not set on the queue as none of the users of the device mapper exported

In the first loop of function raid10_handle_discard. It already
determines which disk need to handle discard request and add the

Certain DM targets ('integrity', 'multipath', 'verity') need to update the
way their attributes are recorded in the ima log, so that the attestation
servers can interpret the data correctly and decide if the devices
meet the attestation requirements. For instance, the "mode=%c" attribute
in the 'integrity' target is measured twice, the 'verity' target is
missing the attribute "root_hash_sig_key_desc=%s", and the 'multipath'
target needs to index the attributes properly.
Update 'integrity' target to remove the duplicate measurement of
the attribute "mode=%c". Add "root_hash_sig_key_desc=%s" attribute
for the 'verity' target. Index various attributes in 'multipath'
target. Also, add "nr_priority_groups=%u" attribute to 'multipath'
target to record the number of priority groups.

The end-users of DM devices/targets may remove and re-create the same
device multiple times. IMA does not measure such duplicate events if the
configuration CONFIG_IMA_DISABLE_HTABLE is set to 'n'.
To avoid confusion, the end-users need some indication on the client
if that configuration option is disabled.
Add a one-time warning during dm_init() if CONFIG_IMA_DISABLE_HTABLE
is set to 'n', to notify the end-users that duplicate events will not
be measured in the ima log. Also cleanup some whitespace in dm_init().

The DM events present in the ima log contain various attributes in the
key=value format. The attributes' names/values may change in future,
and new attributes may also get added. The attestation server needs
some versioning to determine which attributes are supported and are
expected in the ima log.
Add version information to the DM events present in the ima log to
help attestation servers to correctly process the attributes across
different versions.

The active/inactive table hashes measured in the ima log do not contain
the information about hash algorithm. This information is useful for the
attestation servers to recreate the hashes and compare them with the ones
present in the ima log to verify the table contents.
Prefix the table hashes in various DM events in ima log with the hash
algorithm used to compute those hashes.

On systems with many cores using dm-crypt, heavy spinlock contention in
percpu_counter_compare() can be observed when the page allocation limit
for a given device is reached or close to be reached. This is due
to percpu_counter_compare() taking a spinlock to compute an exact
result on potentially many CPUs at the same time.
Switch to non-exact comparison of allocated and allowed pages by using
the value returned by percpu_counter_read_positive() to avoid taking
the percpu_counter spinlock.
This may over/under estimate the actual number of allocated pages by at
most (batch-1) * num_online_cpus().
Currently, batch is bounded by 32. The system on which this issue was
first observed has 256 CPUs and 512GB of RAM. With a 4k page size, this
change may over/under estimate by 31MB. With ~10G (2%) allowed dm-crypt
allocations, this seems an acceptable error. Certainly preferred over
running into the spinlock contention.
This behavior was reproduced on an EC2 c5.24xlarge instance with 96 CPUs
and 192GB RAM as follows, but can be provoked on systems with less CPUs
as well.

lowered to ~7% and write throughput on the loop device increases
to ~2.7GB/s. perf report shows mempool_alloc() as ~8% rather than ~62%
in the profile and not hitting the percpu_counter() spinlock anymore.

For device mapper targets to take advantage of IMA's measurement
capabilities, the status functions for the individual targets need to be
updated to handle the status_type_t case for value STATUSTYPE_IMA.
Update status functions for the following target types, to log their
respective attributes to be measured using IMA.
01. cache
02. crypt
03. integrity
04. linear
05. mirror
06. multipath
07. raid
08. snapshot
09. striped
10. verity
For rest of the targets, handle the STATUSTYPE_IMA case by setting the
measurement buffer to NULL.
For IMA to measure the data on a given system, the IMA policy on the
system needs to be updated to have the following line, and the system
needs to be restarted for the measurements to take effect.

measure func=CRITICAL_DATA label=device-mapper template=ima-buf
The measurements will be reflected in the IMA logs, which are located at:
/sys/kernel/security/integrity/ima/ascii_runtime_measurements
/sys/kernel/security/integrity/ima/binary_runtime_measurements
These IMA logs can later be consumed by various attestation clients
running on the system, and send them to external services for attesting
the system.
The DM target data measured by IMA subsystem can alternatively
be queried from userspace by setting DM_IMA_MEASUREMENT_FLAG with
DM_TABLE_STATUS_CMD.

A given block device is identified by it's name and UUID. However, both
these parameters can be renamed. For an external attestation service to
correctly attest a given device, it needs to keep track of these rename
events.
Update the device data with the new values for IMA measurements. Measure
both old and new device name/UUID parameters in the same IMA measurement
event, so that the old and the new values can be connected later.

For a given block device, an inactive table slot contains the parameters
to configure the device with. The inactive table can be cleared
multiple times, accidentally or maliciously, which may impact the
functionality of the device, and compromise the system. Therefore it is
important to measure and log the event when a table is cleared.
Measure device parameters, and table hashes when the inactive table slot
is cleared.

Presence of an active block-device, configured with expected parameters,
is important for an external attestation service to determine if a system
meets the attestation requirements. Therefore it is important for DM to
measure the device remove events.

A given block device can load a table multiple times, with different
input parameters, before eventually resuming it. Further, a device may
be suspended and then resumed. The device may never resume after a
table-load. Because of the above valid scenarios for a given device,
it is important to measure and log the device resume event using IMA.
Also, if the table is large, measuring it in clear-text each time the
device changes state, will unnecessarily increase the size of IMA log.
Since the table clear-text is already measured during table-load event,
measuring the hash during resume should be sufficient to validate the
table contents.
Measure the device parameters, and hash of the active table, when the
device is resumed.

DM configures a block device with various target specific attributes
passed to it as a table. DM loads the table, and calls each target’s
respective constructors with the attributes as input parameters.
Some of these attributes are critical to ensure the device meets
certain security bar. Thus, IMA should measure these attributes, to
ensure they are not tampered with, during the lifetime of the device.
So that the external services can have high confidence in the
configuration of the block-devices on a given system.
Some devices may have large tables. And a given device may change its
state (table-load, suspend, resume, rename, remove, table-clear etc.)
many times. Measuring these attributes each time when the device
changes its state will significantly increase the size of the IMA logs.
Further, once configured, these attributes are not expected to change
unless a new table is loaded, or a device is removed and recreated.
Therefore the clear-text of the attributes should only be measured
during table load, and the hash of the active/inactive table should be
measured for the remaining device state changes.
Export IMA function ima_measure_critical_data() to allow measurement
of DM device parameters, as well as target specific attributes, during
table load. Compute the hash of the inactive table and store it for
measurements during future state change. If a load is called multiple
times, update the inactive table hash with the hash of the latest
populated table. So that the correct inactive table hash is measured
when the device transitions to different states like resume, remove,
rename, etc.

device mapper is currently the only outlier that tries to call
register_disk after add_disk, leading to fairly inconsistent state
of these block layer data structures. Instead change device-mapper
to just register the gendisk later now that the holder mechanism
can cope with that.
Note that this introduces a user visible change: the dm kobject is
now only visible after the initial table has been loaded.

Commit 95b88f4d71 ("dm writecache: pause
writeback if cache full and origin being written directly") introduced a
code that pauses cache flushing if we are issuing writes directly to the
origin.
Improve that initial commit by making the timeout code configurable
(via the option "pause_writeback"). Also change the default from 1s to
3s because it performed better.

Implementation reuses dm_io_tracker, that until now was only used by
dm-cache, to track if any writes were issued directly to the origin
(due to cache being full) within the last second. If so writeback is
paused for a second.
This change improves performance for when the cache is full and IO is
issued directly to the origin device (rather than through the cache).

remove_raw() in dm_btree_remove() may fail due to IO read error
(e.g. read the content of origin block fails during shadowing),
and the value of shadow_spine::root is uninitialized, but
the uninitialized value is still assign to new_root in the
end of dm_btree_remove().
For dm-thin, the value of pmd->details_root or pmd->root will become
an uninitialized value, so if trying to read details_info tree again
out-of-bound memory may occur as showed below:

Make sure that the zone write pointer offset array is allocated with a
vmalloc in dm_zone_revalidate_cb() by passing GFP_KERNEL gfp flag to
kvcalloc(). However, since we do not want to trigger IOs while
revalidating zones, change dm_revalidate_zones() to have the zone scan
done in GFP_NOIO context using memalloc_noio_save/restore calls.

Add a "metadata_only" parameter that when present: only metadata is
promoted to the cache. This option improves performance for heavier
REQ_META workloads (e.g. device-mapper-test-suite's "git clone and
checkout" benchmark improves from 341s to 312s).

Commit d53f1fafec ("dm writecache: do
direct write if the cache is full") changed dm-writecache, so that it
writes directly to the origin device if the cache is full.
Unfortunately, it doesn't forward flush requests to the origin device,
so that there is a bug where flushes are being ignored.
Fix this by adding missing flush forwarding.
For PMEM mode, we fix this bug by disabling direct writes to the origin
device, because it performs better.

There is a lock contention on device_lock in read_one_chunk().
device_lock is taken to sync conf->active_aligned_reads and
conf->quiesce.
read_one_chunk() takes the lock, then waits for quiesce=0 (resumed)
before incrementing active_aligned_reads.
raid5_quiesce() takes the lock, sets quiesce=2 (in-progress), then waits
for active_aligned_reads to be zero before setting quiesce=1
(suspended).
Introduce a fast (lockless) path in read_one_chunk(): activate aligned
read without taking device_lock. In case quiesce starts while
activating the aligned-read in fast path, deactivate it and revert to
old behavior (take device_lock and wait for quiesce to finish).
Add smp store/load in raid5_quiesce()/read_one_chunk() respectively to
gaurantee that read_one_chunk() does not miss an ongoing quiesce.
My setups:
1. 8 local nvme drives (each up to 250k iops).
2. 8 ram disks (brd).
Each setup with raid6 (6+2), 1024 io threads on a 96 cpu-cores (48 per
socket) system. Record both iops and cpu spent on this contention with
rand-read-4k. Record bw with sequential-read-128k. Note: in most cases
cpu is still busy but due to "new" bottlenecks.

The bio_set (io_acct_set) is used by personalities to clone bio and
trace the timestamp of bio. Some personalities such as raid1/10 don't
need the bio_set, so add check to not create it unconditionally.
Also update the comment for md_account_bio to make it more clear.

After enable io accounting, chunk read bio could be cloned twice which
is not good. To avoid such inefficiency, let's clone align_bio from
io_acct_set too, then we need only call md_account_bio in make_request
unconditionally.

We introduce a new bioset (io_acct_set) for raid0 and raid5 since they
don't own clone infrastructure to accounting io. And the bioset is added
to mddev instead of to raid0 and raid5 layer, because with this way, we
can put common functions to md.h and reuse them in raid0 and raid5.
Also struct md_io_acct is added accordingly which includes io start_time,
the origin bio and cloned bio. Then we can call bio_{start,end}_io_acct
to get related io status.

The commit 41d2d848e5 ("md: improve io stats accounting") could cause
double fault problem per the report [1], and also it is not correct to
change ->bi_end_io if md don't own it, so let's revert it.
And io stats accounting will be replemented in later commits.

In the cache missing code path of cached device, if a proper location
from the internal B+ tree is matched for a cache miss range, function
cached_dev_cache_miss() will be called in cache_lookup_fn() in the
following code block,

Here s->d->cache_miss() is the call backfunction pointer initialized as
cached_dev_cache_miss(), the last parameter 'sectors' is an important
hint to calculate the size of read request to backing device of the
missing cache data.
Current calculation in above code block may generate oversized value of
'sectors', which consequently may trigger 2 different potential kernel
panics by BUG() or BUG_ON() as listed below,

All the above panics are original from cached_dev_cache_miss() by the
oversized parameter 'sectors'.
Inside cached_dev_cache_miss(), parameter 'sectors' is used to calculate
the size of data read from backing device for the cache missing. This
size is stored in s->insert_bio_sectors by the following lines of code,

The oversized parameter 'sectors' may trigger panic 2) by BUG() from the
agove code block.
Now let me explain how the panics happen with the oversized 'sectors'.
In code block 5, replace_key is generated by macro KEY(). From the
definition of macro KEY(),

Here 'size' is 16bits width embedded in 64bits member 'high' of struct
bkey. But in code block 1, if "KEY_START(k) - bio->bi_iter.bi_sector" is
very probably to be larger than (1<<16) - 1, which makes the bkey size
calculation in code block 5 is overflowed. In one bug report the value
of parameter 'sectors' is 131072 (= 1 << 17), the overflowed 'sectors'
results the overflowed s->insert_bio_sectors in code block 4, then makes
size field of s->iop.replace_key to be 0 in code block 5. Then the 0-
sized s->iop.replace_key is inserted into the internal B+ tree as cache
missing check key (a special key to detect and avoid a racing between
normal write request and cache missing read request) as,

Then the 0-sized s->iop.replace_key as 3rd parameter triggers the bkey
size check BUG_ON() in code block 2, and causes the kernel panic 1).
Another kernel panic is from code block 6, is by the bvecs number
oversized value s->insert_bio_sectors from code block 4,
min(sectors, bio_sectors(bio) + reada)
There are two possibility for oversized reresult,
- bio_sectors(bio) is valid, but bio_sectors(bio) + reada is oversized.
- sectors < bio_sectors(bio) + reada, but sectors is oversized.
From a bug report the result of "DIV_ROUND_UP(s->insert_bio_sectors,
PAGE_SECTORS)" from code block 6 can be 344, 282, 946, 342 and many
other values which larther than BIO_MAX_VECS (a.k.a 256). When calling
bio_alloc_bioset() with such larger-than-256 value as the 2nd parameter,
this value will eventually be sent to biovec_slab() as parameter
'nr_vecs' in following code path,
bio_alloc_bioset() ==> bvec_alloc() ==> biovec_slab()
Because parameter 'nr_vecs' is larger-than-256 value, the panic by BUG()
in code block 3 is triggered inside biovec_slab().
From the above analysis, we know that the 4th parameter 'sector' sent
into cached_dev_cache_miss() may cause overflow in code block 5 and 6,
and finally cause kernel panic in code block 2 and 3. And if result of
bio_sectors(bio) + reada exceeds valid bvecs number, it may also trigger
kernel panic in code block 3 from code block 6.
Now the almost-useless readahead size for cache missing request back to
backing device is removed, this patch can fix the oversized issue with
more simpler method.
- add a local variable size_limit, set it by the minimum value from
the max bkey size and max bio bvecs number.
- set s->insert_bio_sectors by the minimum value from size_limit,
sectors, and the sectors size of bio.
- replace sectors by s->insert_bio_sectors to do bio_next_split.
By the above method with size_limit, s->insert_bio_sectors will never
result oversized replace_key size or bio bvecs number. And split bio
'miss' from bio_next_split() will always match the size of 'cache_bio',
that is the current maximum bio size we can sent to backing device for
fetching the cache missing data.
Current problmatic code can be partially found since Linux v3.13-rc1,
therefore all maintained stable kernels should try to apply this fix.

For read cache missing, bcache defines a readahead size for the read I/O
request to the backing device for the missing data. This readahead size
is initialized to 0, and almost no one uses it to avoid unnecessary read
amplifying onto backing device and write amplifying onto cache device.
Considering upper layer file system code has readahead logic allready
and works fine with readahead_cache_policy sysfile interface, we don't
have to keep bcache self-defined readahead anymore.
This patch removes the bcache self-defined readahead for cache missing
request for backing device, and the readahead sysfs file interfaces are
removed as well.
This is the preparation for next patch to fix potential kernel panic due
to oversized request in a simpler method.

Zone append BIOs (REQ_OP_ZONE_APPEND) always specify the start sector
of the zone to be written instead of the actual sector location to
write. The write location is determined by the device and returned to
the host upon completion of the operation. This interface, while simple
and efficient for writing into sequential zones of a zoned block
device, is incompatible with the use of sector values to calculate a
cypher block IV. All data written in a zone end up using the same IV
values corresponding to the first sectors of the zone, but read
operation will specify any sector within the zone resulting in an IV
mismatch between encryption and decryption.
To solve this problem, report to DM core that zone append operations are
not supported. This result in the zone append operations being emulated
using regular write operations.

For zoned targets that cannot support zone append operations, implement
an emulation using regular write operations. If the original BIO
submitted by the user is a zone append operation, change its clone into
a regular write operation directed at the target zone write pointer
position.
To do so, an array of write pointer offsets (write pointer position
relative to the start of a zone) is added to struct mapped_device. All
operations that modify a sequential zone write pointer (writes, zone
reset, zone finish and zone append) are intersepted in __map_bio() and
processed using the new functions dm_zone_map_bio().
Detection of the target ability to natively support zone append
operations is done from dm_table_set_restrictions() by calling the
function dm_set_zones_restrictions(). A target that does not support
zone append operation, either by explicitly declaring it using the new
struct dm_target field zone_append_not_supported, or because the device
table contains a non-zoned device, has its mapped device marked with the
new flag DMF_ZONE_APPEND_EMULATED. The helper function
dm_emulate_zone_append() is introduced to test a mapped device for this
new flag.
Atomicity of the zones write pointer tracking and updates is done using
a zone write locking mechanism based on a bitmap. This is similar to
the block layer method but based on BIOs rather than struct request.
A zone write lock is taken in dm_zone_map_bio() for any clone BIO with
an operation type that changes the BIO target zone write pointer
position. The zone write lock is released if the clone BIO is failed
before submission or when dm_zone_endio() is called when the clone BIO
completes.
The zone write lock bitmap of the mapped device, together with a bitmap
indicating zone types (conv_zones_bitmap) and the write pointer offset
array (zwp_offset) are allocated and initialized with a full device zone
report in dm_set_zones_restrictions() using the function
dm_revalidate_zones().
For failed operations that may have modified a zone write pointer, the
zone write pointer offset is marked as invalid in dm_zone_endio().
Zones with an invalid write pointer offset are checked and the write
pointer updated using an internal report zone operation when the
faulty zone is accessed again by the user.
All functions added for this emulation have a minimal overhead for
zoned targets natively supporting zone append operations. Regular
device targets are also not affected. The added code also does not
impact builds with CONFIG_BLK_DEV_ZONED disabled by stubbing out all
dm zone related functions.

Move the definitions of struct dm_target_io, struct dm_io and the bits
of the flags field of struct mapped_device from dm.c to dm-core.h to
make them usable from dm-zone.c. For the same reason, declare
dec_pending() in dm-core.h after renaming it to dm_io_dec_pending().
And for symmetry of the function names, introduce the inline helper
dm_io_inc_pending() instead of directly using atomic_inc() calls.

A target map method requesting the requeue of a bio with
DM_MAPIO_REQUEUE or completing it with DM_ENDIO_REQUEUE can cause
unaligned write errors if the bio is a write operation targeting a
sequential zone. If a zoned target request such a requeue, warn about
it and kill the IO.
The function dm_is_zone_write() is introduced to detect write operations
to zoned targets.
This change does not affect the target drivers supporting zoned devices
and exposing a zoned device, namely dm-crypt, dm-linear and dm-flakey as
none of these targets ever request a requeue.

To simplify the implementation of the report_zones operation of a zoned
target, introduce the function dm_report_zones() to set a target
mapping start sector in struct dm_report_zones_args and call
blkdev_report_zones(). This new function is exported and the report
zones callback function dm_report_zones_cb() is not.
dm-linear, dm-flakey and dm-crypt are modified to use dm_report_zones().

Fix dm_accept_partial_bio() to actually check that zone management
commands are not passed as explained in the function documentation

The dm-zoned target cannot support zoned block devices with zones that
have a capacity smaller than the zone size (e.g. NVMe zoned namespaces)
due to the current chunk zone mapping implementation as it is assumed
that zones and chunks have the same size with all blocks usable.
If a zoned drive is found to have zones with a capacity different from
the zone size, fail the target initialization.

If dm-writecache overwrites existing cached data, it splits the
incoming bio into many block-sized bios. The I/O scheduler does merge
these bios into one large request but this needless splitting and
merging causes performance degradation.
Fix this by avoiding bio splitting if the cache target area that is
being overwritten is contiguous.

The disk space map stores it's index entries in a btree, these are
accessed very frequently, so having a few cached makes a big difference
to performance.
With this change provisioning a new block takes roughly 20% less cpu.

When we break sharing on btree nodes we typically need to increment
the reference counts to every value held in the node. This can
cause a lot of repeated calls to the space maps. Fix this by changing
the interface to the space map inc/dec methods to take ranges of
adjacent blocks to be operated on.
For installations that are using a lot of snapshots this will reduce
cpu overhead of fundamental operations such as provisioning a new block,
or deleting a snapshot, by as much as 10 times.

Current commit code resets the place where the search for free blocks
will begin back to the start of the metadata device. There are a couple
of repercussions to this:
- The first allocation after the commit is likely to take longer than
normal as it searches for a free block in an area that is likely to
have very few free blocks (if any).
- Any free blocks it finds will have been recently freed. Reusing them
means we have fewer old copies of the metadata to aid recovery from
hardware error.
Fix these issues by leaving the cursor alone, only resetting when the
search hits the end of the metadata device.

This commit improves the residency of btrees built in the metadata for
dm-thin and dm-cache.
When inserting a new entry into a full btree node the current code
splits the node into two. This can result in very many half full nodes,
particularly if the insertions are occurring in an ascending order (as
happens in dm-thin with large writes).
With this commit, when we insert into a full node we first try and move
some entries to a neighbouring node that has space, failing that it
tries to split two neighbouring nodes into three.
Results are given below. 'Residency' is how full nodes are on average
as a percentage. Average instruction counts for the operations
are given to show the extra processing has little overhead.

The third parameter of module_param() is permissions for the sysfs node
but it looks like it is being used as the initial value of the parameter
here. In fact, false here equates to omitting the file from sysfs and
does not affect the value of require_signatures.
Making the parameter writable is not simple because going from
false->true is fine but it should not be possible to remove the
requirement to verify a signature. But it can be useful to inspect the
value of this parameter from userspace, so change the permissions to
make a read-only file in sysfs.

Revert the commit 7a5b96b478 ("dm integrity:
use discard support when recalculating").
There's a bug that when we write some data beyond the current recalculate
boundary, the checksum will be rewritten with the discard filler later.
And the data will no longer have integrity protection. There's no easy
fix for this case.
Also, another problematic case is if dm-integrity is used to detect
bitrot (random device errors, bit flips, etc); dm-integrity should
detect that even for unused sectors. With commit 7a5b96b478 it can
happen that such change is undetected (because discard filler is not a
valid checksum).

The following commands will crash the kernel:

The reason is that when we test for zero chunk size, we jump to the label
bad_read_metadata without setting the "r" variable. The function
snapshot_ctr destroys all the structures and then exits with "r == 0". The
kernel then crashes because it falsely believes that snapshot_ctr
succeeded.
In order to fix the bug, we set the variable "r" to -EINVAL.

My UEK-derived config has 1030 files depending on pagemap.h before this
change. Afterwards, just 326 files need to be rebuilt when I touch
pagemap.h. I think blkdev.h is probably included too widely, but
untangling that dependency is harder and this solves my problem. x86
allmodconfig builds, but there may be implicit include problems on other
architectures.

Kernel panic - not syncing: Fatal exception: panic_on_oops
When allocation/initialization of the blk_mq_tag_set fails in
dm_mq_init_request_queue(), it is uninitialized/freed, but the pointer
is not reset to NULL; so when dev_remove() later gets into
dm_mq_cleanup_mapped_device() it sees the pointer and tries to
uninitialize and free it again.
Fix this by setting the pointer to NULL in dm_mq_init_request_queue()
error-handling. Also set it to NULL in dm_mq_cleanup_mapped_device().

Pull CFI on arm64 support from Kees Cook:
"This builds on last cycle's LTO work, and allows the arm64 kernels to
be built with Clang's Control Flow Integrity feature. This feature has
happily lived in Android kernels for almost 3 years[1], so I'm excited

The wide diffstat is mainly due to the treewide fixing of mismatched
list_sort prototypes. Other things in core kernel are to address
various CFI corner cases. The largest code portion is the CFI runtime
implementation itself (which will be shared by all architectures
implementing support for CFI). The arm64 pieces are Acked by arm64
maintainers rather than coming through the arm64 tree since carrying
this tree over there was going to be awkward.
CFI support for x86 is still under development, but is pretty close.
There are a handful of corner cases on x86 that need some improvements
to Clang and objtool, but otherwise works well.

This patch addresses a data corruption bug in raid1 arrays using bitmaps.
Without this fix, the bitmap bits for the failed I/O end up being cleared.
Since we are in the failure leg of raid1_end_write_request, the request
either needs to be retried (R1BIO_WriteError) or failed (R1BIO_Degraded).

If fast table reloads occur during an ongoing reshape of raid4/5/6
devices the target may race reading a superblock vs the the MD resync
thread; causing an inconclusive reshape state to be read in its
constructor.
lvm2 test lvconvert-raid-reshape-stripes-load-reload.sh can cause
BUG_ON() to trigger in md_run(), e.g.:
"kernel BUG at drivers/md/raid5.c:7567!".
Scenario triggering the bug:
1. the MD sync thread calls end_reshape() from raid5_sync_request()
when done reshaping. However end_reshape() _only_ updates the
reshape position to MaxSector keeping the changed layout
configuration though (i.e. any delta disks, chunk sector or RAID
algorithm changes). That inconclusive configuration is stored in
the superblock.
2. dm-raid constructs a mapping, loading named inconsistent superblock
as of step 1 before step 3 is able to finish resetting the reshape
state completely, and calls md_run() which leads to mentioned bug
in raid5.c.
3. the MD RAID personality's finish_reshape() is called; which resets
the reshape information on chunk sectors, delta disks, etc. This
explains why the bug is rarely seen on multi-core machines, as MD's
finish_reshape() superblock update races with the dm-raid
constructor's superblock load in step 2.
Fix identifies inconclusive superblock content in the dm-raid
constructor and resets it before calling md_run(), factoring out
identifying checks into rs_is_layout_change() to share in existing
rs_reshape_requested() and new rs_reset_inclonclusive_reshape(). Also
enhance a comment and remove an empty line.

bio in the above stack is a bitmap write whose completion is invoked after
the tear down sequence sets the mddev structure to NULL in rdev.
During tear down, there is an attempt to flush the bitmap writes, but for
external bitmaps, there is no explicit wait for all the bitmap writes to
complete. For instance, md_bitmap_flush() is called to flush the bitmap
writes, but the last call to md_bitmap_daemon_work() in md_bitmap_flush()
could generate new bitmap writes for which there is no explicit wait to
complete those writes. The call to md_bitmap_update_sb() will return
simply for external bitmaps and the follow-up call to md_update_sb() is
conditional and may not get called for external bitmaps. This results in a
kernel panic when the completion routine, super_written() is called which
tries to reference mddev in the rdev that has been set to
NULL(in unbind_rdev_from_array() by tear down sequence).
The solution is to call md_super_wait() for external bitmaps after the
last call to md_bitmap_daemon_work() in md_bitmap_flush() to ensure there
are no pending bitmap writes before proceeding with the tear down.

commit df7b59ba92 ("dm verity: fix FEC for RS roots unaligned to
block size") introduced the possibility for misaligned roots IO
relative to the underlying device's logical block size. E.g. Android's
default RS roots=2 results in dm_bufio->block_size=1024, which causes

In bch_cached_dev_run(), free(env[1])|free(env[2])|free(buf)
show up three times. This patch introduce out tag in
which free(env[1])|free(env[2])|free(buf) are only called
one time. If we need to call free() when errors occur,
we can set error code to ret, and then goto out tag directly.

list_sort() internally casts the comparison function passed to it
to a different type with constant struct list_head pointers, and
uses this pointer to call the functions, which trips indirect call
Control-Flow Integrity (CFI) checking.
Instead of removing the consts, this change defines the
list_cmp_func_t type and changes the comparison function types of
all list_sort() callers to use const pointers, thus avoiding type
mismatches.

Split mddev_find into a simple mddev_find that just finds an existing
mddev by the unit number, and a more complicated mddev_find that deals
with find or allocating a mddev.
This turns out to fix this bug reported by Zhao Heming.

speical bug. To stop md array in mdcluster env will do more jobs than
non-cluster array, which will leave enough time/gap to allow kernel to

--monitor" by default. When "mdadm -Ss" is running, the stop action will
wakeup "mdadm --monitor". The "--monitor" daemon will immediately get
info from /proc/mdstat. This time mddev in kernel still exist, so
/proc/mdstat still show md device, which makes "mdadm --monitor" to open
/dev/md0.
The previously "mdadm -Ss" is removing action, the "mdadm --monitor"
open action will trigger md_open which is creating action. Racing is
happening.

mddev_delayed_delete which was created in <thread 1> also can't be
schedule.
In preempt kernel, it can also trigger above racing. But kernel doesn't
allow one thread running on a CPU all the time. after <thread 2> running
some time, the later "mdadm -A" (refer above script line 13) will call
md_alloc to alloc a new gendisk for mddev. it will break md_open
statement "if (mddev->gendisk != bdev->bd_disk)" and return 0 to caller,
the soft lockup is broken.

md_open shouldn't create mddev when all_mddevs list doesn't contain
mddev. With currently code logic, there will be very easy to trigger
soft lockup in non-preempt env.
This patch changes md_open returning from -ERESTARTSYS to -EBUSY, which
will break the infinitely retry when md_open enter racing area.
This patch is partly fix soft lockup issue, full fix needs mddev_find
is split into two functions: mddev_find & mddev_find_or_alloc. And
md_open should call new mddev_find (it only does searching job).
For more detail, please refer with Christoph's "split mddev_find" patch
in later commits.

Add a new flag "reset_recalculate" that will restart recalculating
from the beginning of the device. It can be used if we want to change
the hash function. Example:

If we set non-empty param->name or param->uuid on the DM_LIST_DEVICES_CMD
ioctl, the set values are considered filter prefixes. The ioctl will only
return entries with matching name or uuid prefix.

When LVM needs to find a device with a particular UUID it needs to ask for
UUID for each device. This patch returns UUID directly in the list of
devices, so that LVM doesn't have to query all the devices with an ioctl.
The UUID is returned if the flag DM_UUID_FLAG is set in the parameters.
Returning UUID is done in backward-compatible way. There's one unused
32-bit word value after the event number. This patch sets the bit
DM_NAME_LIST_FLAG_HAS_UUID if UUID is present and
DM_NAME_LIST_FLAG_DOESNT_HAVE_UUID if it isn't (if none of these bits is
set, then we have an old kernel that doesn't support returning UUIDs). The
UUID is stored after this word. The 'next' value is updated to point after
the UUID, so that old version of libdevmapper will skip the UUID without
attempting to interpret it.

For high numbers of DM devices the 64-entry hash table has non-trivial
overhead. Fix this by replacing the hash table with a red-black tree.
