The crash handler calls hv_synic_cleanup() to shutdown the
Hyper-V synthetic interrupt controller. But if the CPU
that calls hv_synic_cleanup() has a VMbus channel interrupt
assigned to it (which is likely the case in smaller VM sizes),
hv_synic_cleanup() returns an error and the synthetic
interrupt controller isn't shutdown. While the lack of
being shutdown hasn't caused a known problem, it still
should be fixed for highest reliability.
So directly call hv_synic_disable_regs() instead of
hv_synic_cleanup(), which ensures that the synic is always
shutdown.

At a slight footprint cost (24 vs 32 bytes), mutexes are more optimal
than semaphores; it's also a nicer interface for mutual exclusion,
which is why they are encouraged over binary semaphores, when possible.
Replace the hyperv_mmio_lock, its semantics implies traditional lock
ownership; that is, the lock owner is the same for both lock/unlock
operations. Therefore it is safe to convert.

Hyper-V assumes page size to be 4K. This might not be the case for
ARM64 architecture. Hence use hyper-v specific page size and page
shift definitions to avoid conflicts between different host and guest
page sizes on ARM64.
Also, remove some old and incorrect comments and redefine ballooning
granularities to handle larger page sizes correctly.

VMbus ring buffers are sized based on the 4K page size used by
Hyper-V. The Linux guest page size may not be 4K on all architectures

The recv_buffer is used to retrieve data from the VMbus ring buffer.
VMbus ring buffers are sized based on the guest page size which
Hyper-V assumes to be 4KB. But it may be different on some
architectures. So use the Hyper-V page size to allocate the
recv_buffer and set the maximum size to receive.

Currently, Linux guests negotiate the VMBus version with Hyper-V
and use the highest available VMBus version they can connect to.
This has some drawbacks: by using the highest available version,
certain code paths are never executed and can not be tested when
the guest runs on the newest host.
Add the module parameter "max_version", to upper-bound the VMBus
versions guests can negotiate.

Hyper-V has added VMBus protocol versions 5.1 and 5.2 in recent release
versions. Allow Linux guests to negotiate these new protocol versions
on versions of Hyper-V that support them. While on this, also allow
guests to negotiate the VMBus protocol version 4.1 (which was missing).

The technique used to get the next VMBus version seems increasisly
clumsy as the number of VMBus versions increases. Performance is
not a concern since this is only done once during system boot; it's
just that we'll end up with more lines of code than is really needed.
As an alternative, introduce a table with the version numbers listed
in order (from the most recent to the oldest). vmbus_connect() loops
through the versions listed in the table until it gets an accepted
connection or gets to the end of the table (invalid version).

Hyper-V has historically initialized stimer-based clockevents late in the
process of onlining a CPU because clockevents depend on stimer
interrupts. In the original Hyper-V design, stimer interrupts generate a
VMbus message, so the VMbus machinery must be running first, and VMbus
can't be initialized until relatively late. On x86/64, LAPIC timer based
clockevents are used during early initialization before VMbus and
stimer-based clockevents are ready, and again during CPU offlining after
the stimer clockevents have been shut down.
Unfortunately, this design creates problems when offlining CPUs for
hibernation or other purposes. stimer-based clockevents are shut down
relatively early in the offlining process, so clockevents_unbind_device()
must be used to fallback to the LAPIC-based clockevents for the remainder
of the offlining process. Furthermore, the late initialization and early
shutdown of stimer-based clockevents doesn't work well on ARM64 since there
is no other timer like the LAPIC to fallback to. So CPU onlining and
offlining doesn't work properly.
Fix this by recognizing that stimer Direct Mode is the normal path for
newer versions of Hyper-V on x86/64, and the only path on other
architectures. With stimer Direct Mode, stimer interrupts don't require any
VMbus machinery. stimer clockevents can be initialized and shut down
consistent with how it is done for other clockevent devices. While the old
VMbus-based stimer interrupts must still be supported for backward
compatibility on x86, that mode of operation can be treated as legacy.
So add a new Hyper-V stimer entry in the CPU hotplug state list, and use
that new state when in Direct Mode. Update the Hyper-V clocksource driver
to allocate and initialize stimer clockevents earlier during boot. Update
Hyper-V initialization and the VMbus driver to use this new design. As a
result, the LAPIC timer is no longer used during boot or CPU
onlining/offlining and clockevents_unbind_device() is not called. But
retain the old design as a legacy implementation for older versions of
Hyper-V that don't support Direct Mode.

When the host re-offers the primary channels upon resume, the host only
guarantees the Instance GUID doesn't change, so vmbus_bus_suspend()
should invalidate channel->offermsg.child_relid and figure out the
number of primary channels that need to be fixed up upon resume.
Upon resume, vmbus_onoffer() finds the old channel structs, and maps
the new offers to the old channels, and fixes up the old structs,
and finally the resume callbacks of the VSC drivers will re-open
the channels.

Before suspend, Linux must make sure all the hv_sock channels have been
properly cleaned up, because a hv_sock connection can not persist across
hibernation, and the user-space app must be properly notified of the
state change of the connection.
Before suspend, Linux also must make sure all the sub-channels have been
destroyed, i.e. the related channel structs of the sub-channels must be
properly removed, otherwise they would cause a conflict when the
sub-channels are recreated upon resume.
Add a counter to track such channels, and vmbus_bus_suspend() should wait
for the counter to drop to zero.

Fake RESCIND_CHANNEL messages to clean up hv_sock channels by force for
hibernation. There is no better method to clean up the channels since
some of the channels may still be referenced by the userspace apps when
hibernation is triggered: in this case, with this patch, the "rescind"
fields of the channels are set, and the apps will thoroughly destroy
the channels after hibernation.

Before Linux enters hibernation, it sends the CHANNELMSG_UNLOAD message to
the host so all the offers are gone. After hibernation, Linux needs to
re-negotiate with the host using the same vmbus protocol version (which
was in use before hibernation), and ask the host to re-offer the vmbus
devices.

When the VM resumes, the host re-sends the offers. We should not add the
offers to the global vmbus_connection.chn_list again.
This patch assumes the RELIDs of the channels don't change across
hibernation. Actually this is not always true, especially in the case of
NIC SR-IOV the VF vmbus device's RELID sometimes can change. A later patch
will address this issue by mapping the new offers to the old channels and
fixing up the old channels, if necessary.

Break out synic enable and disable operations into separate
hv_synic_disable_regs() and hv_synic_enable_regs() functions for use by a
later patch to support hibernation.
There is no functional change except the unnecessary check
"if (sctrl.enable != 1) return -EFAULT;" which is removed, because when
we're in hv_synic_cleanup(), we're absolutely sure sctrl.enable must be 1.

There is no particular reason to not enable TSC page clocksource on
32-bit. mul_u64_u64_shr() is available and despite the increased
computational complexity (compared to 64bit) TSC page is still a huge win
compared to MSR-based clocksource.
In-kernel reads:
MSR based clocksource: 3361 cycles
TSC page clocksource: 49 cycles
Reads from userspace (utilizing vDSO in case of TSC page):
MSR based clocksource: 5664 cycles
TSC page clocksource: 131 cycles
Enabling TSC page on 32bits allows to get rid of CONFIG_HYPERV_TSCPAGE as
it is now not any different from CONFIG_HYPERV_TIMER.

In the case of X86_PAE, unsigned long is u32, but the physical address type
should be u64. Due to the bug here, the netvsc driver can not load
successfully, and sometimes the VM can panic due to memory corruption (the
hypervisor writes data to the wrong location).

Move the code that negotiates with the host to a new function
balloon_connect_vsp() and improve the error handling.
This makes the code more readable and paves the way for the
support of hibernation in future.

In the sysctl code the proc_dointvec_minmax() function is often used to
validate the user supplied value between an allowed range. This
function uses the extra1 and extra2 members from struct ctl_table as
minimum and maximum allowed value.
On sysctl handler declaration, in every source file there are some
readonly variables containing just an integer which address is assigned
to the extra1 and extra2 members, so the sysctl range is enforced.
The special values 0, 1 and INT_MAX are very often used as range
boundary, leading duplication of variables like zero=0, one=1,
int_max=INT_MAX in different source files:

Add a const int array containing the most commonly used values, some
macros to refer more easily to the correct array member, and use them
instead of creating a local one for every object file.

Hyper-V clock/timer code and data structures are currently mixed
in with other code in the ISA independent drivers/hv directory as
well as the ISA dependent Hyper-V code under arch/x86.
Consolidate this code and data structures into a Hyper-V clocksource driver
to better follow the Linux model. In doing so, separate out the ISA
dependent portions so the new clocksource driver works for x86 and for the
in-process Hyper-V on ARM64 code.
To start, move the existing clockevents code to create the new clocksource
driver. Update the VMbus driver to call initialization and cleanup routines
since the Hyper-V synthetic timers are not independently enumerated in
ACPI.
No behavior is changed and no new functionality is added.

Add a special Kconfig symbol X86_HV_CALLBACK_VECTOR so that the guests
using the hypervisor interrupt callback counter can select and thus
enable that counter. Select it when xen or hyperv support is enabled. No
functional changes.

Fix a race condition that can result in a ring buffer pointer being set
to null while a "_show" function is reading the ring buffer's data. This
problem was discussed here: https://lkml.org/lkml/2018/10/18/779
To fix the race condition, add a new mutex lock to the
"hv_ring_buffer_info" struct. Add a new function,
"hv_ringbuffer_pre_init()", where a channel's inbound and outbound
ring_buffer_info mutex locks are initialized.
Acquire/release the locks in the "hv_ringbuffer_cleanup()" function,
which is where the ring buffer pointers are set to null.
Acquire/release the locks in the four channel-level "_show" functions
that access ring buffer data. Remove the "const" qualifier from the
"vmbus_channel" parameter and the "rbi" variable of the channel-level
"_show" functions so that the locks can be acquired/released in these
functions.
Acquire/release the locks in hv_ringbuffer_get_debuginfo(). Remove the
"const" qualifier from the "hv_ring_buffer_info" parameter so that the
locks can be acquired/released in this function.

The chan->state "if statement" was introduced in commit 6712cc9c22
("vmbus: don't return values for uninitalized channels"). That commit
states that the purpose of the chan->state "if statement" is to prevent
returning garbage or causing a kernel OOPS when the channel ring buffer
is not initialized. The changes in this patch provide the same
protection.
Refactor the chan->state “if statement” in vmbus_chan_attr_show():
- Instead of checking the channel state in the "if statement", check
whether the channel ring buffer pointer is NULL. Checking the
ring buffer pointer makes this code consistent with
hv_ringbuffer_get_debuginfo().
- Move the "if statement" to the four "_show" functions that access a
channel ring buffer. Only four of the channel-level "_show" functions
access a ring buffer. The ring buffer pointer does not need to be
checked before calling the other "_show" functions, and moving the
ring buffer pointer "if statement" to the "_show" functions that
access a ring buffer makes the purpose of the "if statement" clear.

There are two methods for signaling the host: the monitor page mechanism
and hypercalls. The monitor page mechanism is used by performance
critical channels (storage, networking, etc.) because it provides
improved throughput. However, latency is increased. Monitor pages are
allocated to these channels.
Monitor pages are not allocated to channels that do not use the monitor
page mechanism. Therefore, these channels do not have a valid monitor id
or valid monitor page data. In these cases, some of the "_show"
functions return incorrect data. They return an invalid monitor id and
data that is beyond the bounds of the hv_monitor_page array fields.
The "channel->offermsg.monitor_allocated" value can be used to determine
whether monitor pages have been allocated to a channel.
Add "is_visible()" callback functions for the device-level and
channel-level attribute groups. These functions will hide the monitor
sysfs files when the monitor mechanism is not used.
Remove ".default_attributes" from "vmbus_chan_attrs" and create a
channel-level attribute group. These changes allow the new
"is_visible()" callback function to be applied to the channel-level
attributes.
Call "sysfs_create_group()" in "vmbus_add_channel_kobj()" to create the
channel's sysfs files. Add a new function,
“vmbus_remove_channel_attr_group()”, and call it in "free_channel()" to
remove the channel's sysfs files when the channel is closed.

Here is the big char/misc driver patch pull request for 5.1-rc1.
The largest thing by far is the new habanalabs driver for their AI
accelerator chip. For now it is in the drivers/misc directory but will
probably move to a new directory soon along with other drivers of this
type.
Other than that, just the usual set of individual driver updates and
fixes. There's an "odd" merge in here from the DRM tree that they asked
me to do as the MEI driver is starting to interact with the i915 driver,
and it needed some coordination. All of those patches have been
properly acked by the relevant subsystem maintainers.
All of these have been in linux-next with no reported issues, most for
quite some time.

When freeing pages are done with higher order, time spent on coalescing
pages by buddy allocator can be reduced. With section size of 256MB,
hot add latency of a single section shows improvement from 50-60 ms to
less than 1 ms, hence improving the hot add latency by 60 times. Modify
external providers of online callback to align with the change.

Counter values for per-channel interrupts and ring buffer full
conditions are useful for investigating performance.
Expose counters in sysfs for 2 types of guest to host interrupts:
1) Interrupts caused by the channel's outbound ring buffer transitioning
from empty to not empty
2) Interrupts caused by the channel's inbound ring buffer transitioning
from full to not full while a packet is waiting for enough buffer space to
become available
Expose 2 counters in sysfs for the number of times that write operations
encountered a full outbound ring buffer:
1) The total number of write operations that encountered a full
condition
2) The number of write operations that were the first to encounter a
full condition
Increment the outbound full condition counters in the
hv_ringbuffer_write() function because, for most drivers, a full
outbound ring buffer is detected in that function. Also increment the
outbound full condition counters in the set_channel_pending_send_size()
function. In the hv_sock driver, a full outbound ring buffer is detected
and set_channel_pending_send_size() is called before
hv_ringbuffer_write() is called.
I tested this patch by confirming that the sysfs files were created and
observing the counter values. The values seemed to increase by a
reasonable amount when the Hyper-v related drivers were in use.

The changes to split ring allocation from open/close, broke
the cleanup of subchannels. This resulted in problems using
uio on network devices because the subchannel was left behind
when the network device was unbound.
The cause was in the disconnect logic which used list splice
to move the subchannel list into a local variable. This won't
work because the subchannel list is needed later during the
process of the rescind messages (relid2channel).
The fix is to just leave the subchannel list in place
which is what the original code did. The list is cleaned
up later when the host rescind is processed.
Without the fix, we have a lot of "hang" issues in netvsc when we
try to change the NIC's MTU, set the number of channels, etc.

Hyper-V memory hotplug protocol has 2M granularity and in Linux x86 we use
128M. To deal with it we implement partial section onlining by registering
custom page onlining callback (hv_online_page()). Later, when more memory
arrives we try to online the 'tail' (see hv_bring_pgs_online()).
It was found that in some cases this 'tail' onlining causes issues:

Turns out that we now have deferred struct page initialization for memory
hotplug so e.g. memory_block_action() in drivers/base/memory.c does
pages_correctly_probed() check and in that check it avoids inspecting
struct pages and checks sections instead. But in Hyper-V balloon driver we
do PageReserved(pfn_to_page()) check and this is now wrong.
Switch to checking online_section_nr() instead.

fc96df16a1 is good and can already fix the "return stack garbage" issue,
but let's also improve hv_ringbuffer_get_debuginfo(), which would silently
return stack garbage, if people forget to check channel->state or
ring_info->ring_buffer, when using the function in the future.
Having an error check in the function would eliminate the potential risk.
Add a Fixes tag to indicate the patch depdendency.

use of it in containerized systems. This was discussed at the
Plumbers conference a few months ago and knocked into mergable shape

another binder maintainer, showing a distinct lack of good judgement :)

This series converts totalram_pages, totalhigh_pages and
zone->managed_pages to atomic variables.
totalram_pages, zone->managed_pages and totalhigh_pages updates are
protected by managed_page_count_lock, but readers never care about it.
Convert these variables to atomic to avoid readers potentially seeing a
store tear.
Main motivation was that managed_page_count_lock handling was complicating
things. It was discussed in length here,

vmbus_process_offer() mustn't call channel->sc_creation_callback()
directly for sub-channels, because sc_creation_callback() ->
vmbus_open() may never get the host's response to the
OPEN_CHANNEL message (the host may rescind a channel at any time,
e.g. in the case of hot removing a NIC), and vmbus_onoffer_rescind()
may not wake up the vmbus_open() as it's blocked due to a non-zero
vmbus_connection.offer_in_progress, and finally we have a deadlock.
The above is also true for primary channels, if the related device
drivers use sync probing mode by default.
And, usually the handling of primary channels and sub-channels can
depend on each other, so we should offload them to different
workqueues to avoid possible deadlock, e.g. in sync-probing mode,
NIC1's netvsc_subchan_work() can race with NIC2's netvsc_probe() ->
rtnl_lock(), and causes deadlock: the former gets the rtnl_lock
and waits for all the sub-channels to appear, but the latter
can't get the rtnl_lock and this blocks the handling of sub-channels.
The patch can fix the multiple-NIC deadlock described above for
v3.x kernels (e.g. RHEL 7.x) which don't support async-probing
of devices, and v4.4, v4.9, v4.14 and v4.18 which support async-probing
but don't enable async-probing for Hyper-V drivers (yet).
The patch can also fix the hang issue in sub-channel's handling described
above for all versions of kernels, including v4.19 and v4.20-rc4.
So actually the patch should be applied to all the existing kernels,
not only the kernels that have 8195b1396e.

This is a longstanding issue: if the vmbus upper-layer drivers try to
consume too many GPADLs, the host may return with an error
0xC0000044 (STATUS_QUOTA_EXCEEDED), but currently we forget to check
the creation_status, and hence we can pass an invalid GPADL handle
into the OPEN_CHANNEL message, and get an error code 0xc0000225 in
open_info->response.open_result.status, and finally we hang in
vmbus_open() -> "goto error_free_info" -> vmbus_teardown_gpadl().
With this patch, we can exit gracefully on STATUS_QUOTA_EXCEEDED.

Commit d86adf482b ("scsi: storvsc: Enable multi-queue support") removed
the usage of the API in Jan 2017, and the API is not used since then.
netvsc and storvsc have their own algorithms to determine the outgoing
channel, so this API is useless.
And the API is potentially unsafe, because it reads primary->num_sc without
any lock held. This can be risky considering the RESCIND-OFFER message.
Let's remove the API.

Currently we are replicating state in struct hv_context that is unnecessary -
this state can be retrieved from the hypervisor. Furthermore, this is a per-cpu
state that is being maintained as a global state in struct hv_context.
Get rid of this state in struct hv_context.

In kvp_send_key(), we do need call process_ib_ipinfo() if
message->kvp_hdr.operation is KVP_OP_GET_IP_INFO, because it turns out
the userland hv_kvp_daemon needs the info of operation, adapter_id and
addr_family. With the incorrect fc62c3b197, the host can't get the
VM's IP via KVP.
And, fc62c3b197 added a "break;", but actually forgot to initialize
the key_size/value in the case of KVP_OP_SET, so the default key_size of
0 is passed to the kvp daemon, and the pool files
/var/lib/hyperv/.kvp_pool_* can't be updated.
This patch effectively rolls back the previous fc62c3b197, and
correctly fixes the "this statement may fall through" warnings.
This patch is tested on WS 2012 R2 and 2016.

We don't need to call process_ib_ipinfo() if message->kvp_hdr.operation is
KVP_OP_GET_IP_INFO in kvp_send_key(), because here we just need to pass on
the op code from the host to the userspace; when the userspace returns
the info requested by the host, we pass the info on to the host in
kvp_respond_to_host() -> process_ob_ipinfo(). BTW, the current buggy code
actually doesn't cause any harm, because only message->kvp_hdr.operation
is used by the userspace, in the case of KVP_OP_GET_IP_INFO.
The patch also adds a missing "break;" in kvp_send_key(). BTW, the current
buggy code actually doesn't cause any harm, because in the case of
KVP_OP_SET, the unexpected fall-through corrupts
message->body.kvp_set.data.key_size, but that is not really used: see
the definition of struct hv_kvp_exchg_msg_value.

With CONFIG_DEBUG_PREEMPT=y, I always see this warning:
BUG: using smp_processor_id() in preemptible [00000000]
Fix the false warning by using get/put_cpu().
Here vmbus_connect() sends a message to the host and waits for the
host's response. The host will deliver the response message and an
interrupt on CPU msg->target_vcpu, and later the interrupt handler
will wake up vmbus_connect(). vmbus_connect() doesn't really have
to run on the same cpu as CPU msg->target_vcpu, so it's safe to
call put_cpu() just here.

The UIO driver needs the ring buffer to be persistent(reused)
across open/close. Split the allocation and setup of ring buffer
out of vmbus_open. For normal usage vmbus_open/vmbus_close there
are no changes; only impacts uio_hv_generic which needs to keep
ring buffer memory and reuse when application restarts.

If hv_synic_alloc() errors out, the state of the per-cpu context
for some CPUs is unknown since the zero'ing is done as each
CPU is iterated over. In such case, hv_synic_cleanup() may try to
free memory based on uninitialized values. Fix this by zero'ing
the per-cpu context for all CPUs before doing any memory
allocations that might fail.

For unsupported device types, the vmbus channel ringbuffer is never
initialized, and therefore reading the sysfs files will return garbage

clk_evt memory is not being freed when the synic is shutdown
or when there is an allocation error. Add the appropriate
kfree() call, along with a comment to clarify how the memory
gets freed after an allocation error. Make the free path
consistent by removing checks for NULL since kfree() and
free_page() already do the check.

slow_virt_to_phys() is only implemented for arch/x86.
Remove its use in arch independent Hyper-V drivers, and
replace with test for vmalloc() address followed by
appropriate v-to-p function. This follows the typical
pattern of other drivers and avoids the need to implement
slow_virt_to_phys() for Hyper-V on ARM64.

Before setting channel->rescind in vmbus_rescind_cleanup(), we should make
sure the channel callback won't run any more, otherwise a high-level
driver like pci_hyperv, which may be infinitely waiting for the host VSP's
response and notices the channel has been rescinded, can't safely give
up: e.g., in hv_pci_protocol_negotiation() -> wait_for_response(), it's
unsafe to exit from wait_for_response() and proceed with the on-stack
variable "comp_pkt" popped. The issue was originally spotted by
Michael Kelley <mikelley@microsoft.com>.
In vmbus_close_internal(), the patch also minimizes the range protected by
disabling/enabling channel->callback_event: we don't really need that for
the whole function.

The code to support panic control message was checking the return was
checking the return value from kmsg_dump_get_buffer as error value, which

In the VM mode on Hyper-V, currently, when the kernel panics, an error
code and few register values are populated in an MSR and the Hypervisor
notified. This information is collected on the host. The amount of
information currently collected is found to be limited and not very
actionable. To gather more actionable data, such as stack trace, the
proposal is to write one page worth of kmsg data on an allocated page
and the Hypervisor notified of the page address through the MSR.

The Hyper-V feature and hint flags in hyperv-tlfs.h are all defined
with the string "X64" in the name. Some of these flags are indeed
x86/x64 specific, but others are not. For the ones that are used
in architecture independent Hyper-V driver code, or will be used in
the upcoming support for Hyper-V for ARM64, this patch removes the
"X64" from the name.
This patch changes the flags that are currently known to be
used on multiple architectures. Hyper-V for ARM64 is still a
work-in-progress and the Top Level Functional Spec (TLFS) has not
been separated into x86/x64 and ARM64 areas. So additional flags
may need to be updated later.
This patch only changes symbol names. There are no functional
changes.

In architecture independent code for manipulating Hyper-V synthetic timers
and synthetic interrupts, pass in an ordinal number identifying the timer
or interrupt, rather than an actual MSR register address. Then in
x86/x64 specific code, map the ordinal number to the appropriate MSR.
This change facilitates the introduction of an ARM64 version of Hyper-V,
which uses the same synthetic timers and interrupts, but a different
mechanism for accessing them.

This is mostly updates to the usual drivers: ufs, qedf, mpt3sas, lpfc,
xfcp, hisi_sas, cxlflash, qla2xxx. In the absence of Nic, we're also
taking target updates which are mostly minor except for the tcmu
refactor. The only real core change to worry about is the removal of
high page bouncing (in sas, storvsc and iscsi). This has been well
tested and no problems have shown up so far.

With VMBus protocol 5.0, we're able to better support new features, e.g.
running two or more VMBus drivers simultaneously in a single VM -- note:
we can't simply load the current VMBus driver twice, instead, a secondary
VMBus driver must be implemented.
This patch adds the support for the new VMBus protocol, which is available
on new Windows hosts, by:
1) We still use SINT2 for compatibility;
2) We must use Connection ID 4 for the Initiate Contact Message, and for
subsequent messages, we must use the Message Connection ID field in
the host-returned VersionResponse Message.
Notes for developers of the secondary VMBus driver:
1) Must use VMBus protocol 5.0 as well;

Netvsc has a function to calculate how much ring buffer in percentage is
available to write. This function is also useful for storvsc and other
vmbus devices.
Define a similar function in vmbus to be used by other vmbus devices.

hyperv.h is not part of uapi, there are no (known) users outside of kernel.
We are making changes to this file to match current Hyper-V Hypervisor
Top-Level Functional Specification (TLFS, see:
https://docs.microsoft.com/en-us/virtualization/hyper-v-on-windows/reference/tlfs)
and we don't want to maintain backwards compatibility.
Move the file renaming to hyperv-tlfs.h to avoid confusing it with
mshyperv.h. In future, all definitions from TLFS should go to it and
all kernel objects should go to mshyperv.h or include/linux/hyperv.h.

The pci-hyperv driver's channel callback hv_pci_onchannelcallback() is not
really a hot path, so we don't need to mark it as a perf_device, meaning
with this patch all HV_PCIE channels' target_cpu will be CPU0.

Hyper-V balloon driver makes non-trivial calculations to convert Linux's
representation of free/used memory to what Hyper-V host expects to see. Add

Our num_pages_onlined accounting is buggy:
1) In case we're offlining a memory block which was present at boot (e.g.
when there was no hotplug at all) we subtract 32k from 0 and as
num_pages_onlined is unsigned get a very big positive number.
2) Commit 6df8d9aaf3 ("Drivers: hv: balloon: Correctly update onlined
page count") made num_pages_onlined counter accurate on onlining but
totally incorrect on offlining for partly populated regions: no matter
how many pages were onlined and what was actually added to
num_pages_onlined counter we always subtract the full region (32k) so
again, num_pages_onlined can wrap around zero. By onlining/offlining
the same partly populated region multiple times we can make the
situation worse.
Solve these issues by doing accurate accounting on offlining: walk HAS
list, check for covered range and gaps.

The 2016 version of Hyper-V offers the option to operate the guest VM
per-vcpu stimer's in Direct Mode, which means the timer interupts on its
own vector rather than queueing a VMbus message. Direct Mode reduces
timer processing overhead in both the hypervisor and the guest, and
avoids having timer interrupts pollute the VMbus interrupt stream for
the synthetic NIC and storage. This patch enables Direct Mode by
default on stimer0 when running on a version of Hyper-V that supports
it.
In prep for coming support of Hyper-V on ARM64, the arch independent
portion of the code contains calls to routines that will be populated
on ARM64 but are not needed and do nothing on x86.

The kzalloc function is called with GFP_ATOMIC.
But according to driver call graph, it is not in atomic context,
namely no spinlock is held nor in an interrupt handler.
This GFP_ATOMIC is unnecessary, and replace with GFP_KERNEL.

Fix bugs in signaling the Hyper-V host when freeing space in the
host->guest ring buffer:
1. The interrupt_mask must not be used to determine whether to signal
on the host->guest ring buffer
2. The ring buffer write_index must be read (via hv_get_bytes_to_write)
*after* pending_send_sz is read in order to avoid a race condition
3. Comparisons with pending_send_sz must treat the "equals" case as
not-enough-space
4. Don't signal if the pending_send_sz feature is not present. Older
versions of Hyper-V that don't implement this feature will poll.

The next patch from Al will sort out the final differences, and we
should be all done.

Pull poll annotations from Al Viro:
"This introduces a __bitwise type for POLL### bitmap, and propagates
the annotations through the tree. Most of that stuff is as simple as
'make ->poll() instances return __poll_t and do the same to local
variables used to hold the future return value'.
Some of the obvious brainos found in process are fixed (e.g. POLLIN
misspelled as POLL_IN). At that point the amount of sparse warnings is
low and most of them are for genuine bugs - e.g. ->poll() instance
deciding to return -EINVAL instead of a bitmap. I hadn't touched those
in this series - it's large enough as it is.
Another problem it has caught was eventpoll() ABI mess; select.c and
eventpoll.c assumed that corresponding POLL### and EPOLL### were
equal. That's true for some, but not all of them - EPOLL### are
arch-independent, but POLL### are not.
The last commit in this series separates userland POLL### values from
the (now arch-independent) kernel-side ones, converting between them
in the few places where they are copied to/from userland. AFAICS, this
is the least disruptive fix preserving poll(2) ABI and making epoll()
work on all architectures.
As it is, it's simply broken on sparc - try to give it EPOLLWRNORM and
it will trigger only on what would've triggered EPOLLWRBAND on other
architectures. EPOLLWRBAND and EPOLLRDHUP, OTOH, are never triggered
at all on sparc. With this patch they should work consistently on all
architectures"

hv_is_hypercall_page_setup() is used to check if Hyper-V is
initialized, but a 'hypercall page' is an x86 implementation detail
that isn't necessarily present on other architectures. Rename to the
architecture independent hv_is_hyperv_initialized() and add check
that x86_hyper is pointing to Hyper-V. Use this function instead of
direct references to x86-specific data structures in vmbus_drv.c,
and remove now redundant call in hv_init(). Also remove 'x86' from
the string name passed to cpuhp_setup_state().

Without the patch, a device can't be thoroughly destroyed, because
vmbus_device_register() -> kset_create_and_add() still holds a reference
to the hv_device's device.kobj.

The last use of hv_get_ringbuffer_availbytes in drivers is now
gone. Only used by the debug info routine so make it static. Also, add

The current rescind processing code will not correctly handle
the case where the host immediately rescinds a channel that has
been offerred. In this case, we could be blocked in the open call and
since the channel is rescinded, the host will not respond and we could
be blocked forever in the vmbus open call.i Fix this problem.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

Hyper-V allows the guest to report panic and the guest can pass additional
information. All this is logged on the host. Currently Linux is passing back
information that is not particularly useful. Make the following changes:
1. Windows uses crash MSR P0 to report bugcheck code. Follow the same
convention for Linux as well.
2. It will be useful to know the gust ID of the Linux guest that has
paniced. Pass back this information.
These changes will help in better supporting Linux on Hyper-V

Without the patch, vmbus_hvsock_device_unregister() can destroy the device
prematurely when close() is called, and can cause NULl dereferencing or
potential data loss (the last portion of the data stream may be dropped
prematurely).

This patch addresses the following bugs in the current rescind handling code:
1. Fixes a race condition where we may be invoking hv_process_channel_removal()
on an already freed channel.
2. Prevents indefinite wait when rescinding sub-channels by correctly setting
the probe_complete state.
I would like to thank Dexuan for patiently reviewing earlier versions of this
patch and identifying many of the issues fixed here.

This extends existing vmbus related sysfs structure to provide per-channel
state information. This is useful when diagnosing issues with multiple
queues in networking and storage.
The existing sysfs only displayed information about the primary
channel. The one place it reported multiple channels was the
channel_vp_mapping file which violated the sysfs convention
of one value per file.

Till recently the expected length of bytes read by the
daemon did depend on the context. It was either hv_start_fcopy or
hv_do_fcopy. The daemon had a buffer size of two pages, which was much
larger than needed.
Now the expected length of bytes read by the
daemon changed slightly. For START_FILE_COPY it is still the size of
hv_start_fcopy. But for WRITE_TO_FILE and the other operations it is as
large as the buffer that arrived via vmbus. In case of WRITE_TO_FILE
that is slightly larger than a struct hv_do_fcopy. Since the buffer in
the daemon was still larger everything was fine.
Currently, the daemon reads only what is actually needed.
The new buffer layout is as large as a struct hv_do_fcopy, for the
WRITE_TO_FILE operation. Since the kernel expects a slightly larger
size, hvt_op_read will return -EINVAL because the daemon will read
slightly less than expected. Address this by restoring the expected
buffer size in case of WRITE_TO_FILE.

Due to commit 54a66265d6 ("Drivers: hv: vmbus: Fix rescind handling"),
we need this patch to resolve the below deadlock:
after we get the mutex in vmbus_hvsock_device_unregister() and call
vmbus_device_unregister() -> device_unregister() -> ... -> device_release()
-> vmbus_device_release(), we'll get a deadlock, because
vmbus_device_release() tries to get the same mutex.

This patch handles the following issues that were observed when we are
handling racing channel offer message and rescind message for the same
offer:
1. Since the host does not respond to messages on a rescinded channel,
in the current code, we could be indefinitely blocked on the vmbus_open() call.
2. When a rescinded channel is being closed, if there is a pending interrupt on the
channel, we could end up freeing the channel that the interrupt handler would run on.

There's a bug which passes the output buffer size as MAX_IP_ADDR_SIZE,
when converting the adapter_id field to UTF16. This is much larger than
the actual size (MAX_ADAPTER_ID_SIZE). Fix this by passing the proper
size.
Fortunately, the translation is limited by the length of the input. This
explains why we haven't seen output buffer overflow conditions.

When left uninitialized, this sometimes fails the following check in
post_status():

return;
}
This causes unnecessary delays in reporting memory pressure to host after
booting up.

Previously we were only showing max number of pages. We should make it
more clear that this value is the max amount of dynamic memory that the
Hyper-V host is willing to assign to this guest.

Previously, num_pages_onlined was updated using value from memory online
notifier. This is incorrect because they assume that all hot-added pages
are online, even though we only online the amount that's backed by the
host. We should update num_pages_onlined only when the balloon driver
marks a page as online.

Hyper-V host can suggest us to use hypercall for doing remote TLB flush,
this is supposed to work faster than IPIs.
Implementation details: to do HvFlushVirtualAddress{Space,List} hypercalls
we need to put the input somewhere in memory and we don't really want to
have memory allocation on each call so we pre-allocate per cpu memory areas
on boot.
pv_ops patching is happening very early so we need to separate
hyperv_setup_mmu_ops() and hyper_alloc_mmu().
It is possible and easy to implement local TLB flushing too and there is
even a hint for that. However, I don't see a room for optimization on the
host side as both hypercall and native tlb flush will result in vmexit. The
hint is also not set on modern Hyper-V versions.

We need to pass only 8 bytes of input for HvSignalEvent which makes it a
perfect fit for fast hypercall. hv_input_signal_event_buffer is not needed
any more and hv_input_signal_event is converted to union for convenience.

We have only three call sites for hv_do_hypercall() and we're going to
change HVCALL_SIGNAL_EVENT to doing fast hypercall so we can inline this
function for optimization.
Hyper-V top level functional specification states that r9-r11 registers
and flags may be clobbered by the hypervisor during hypercall and with
inlining this is somewhat important, add the clobbers.

This problem shows up in 4.11 when netvsc driver is removed and reloaded.
The problem is that the channel is closed during module removal and the
tasklet for processing responses is disabled. When module is reloaded
the channel is reopened but the tasklet is marked as disabled.
The fix is to re-enable tasklet at the end of close which gets it back
to the initial state.
The issue is less urgent in 4.12 since network driver now uses NAPI
and not the tasklet; and other VMBUS devices are rarely unloaded/reloaded.

In storvsc driver, inbound messages do not go through inbound lock. The
only effect of this lock was is to provide a barrier for connect and
remove logic.

Commit c0bb03924f ("Drivers: hv: vmbus: Raise retry/wait limits in
vmbus_post_msg()") increased the retry/wait limits of vmbus_post_msg()
to address the new DoS protection policies in WS2016.
Increase the time between retries to make the

It was found that ICTIMESYNCFLAG_SYNC packets are handled incorrectly
on WS2012R2, e.g. after the guest is paused and resumed its time is set
to something different from host's time. The problem is that we call
adj_guesttime() with reftime=0 for these old hosts and we don't account
for that in 'if (adj_flags & ICTIMESYNCFLAG_SYNC)' branch and
hv_set_host_time().
While we could've solved this by adding a check like
'if (ts_srv_version > TS_VERSION_3)' to hv_set_host_time() I prefer
to do some refactoring. We don't need to have two separate containers
for host samples, struct host_ts which we use for PTP is enough.
Throw away 'struct adj_time_work' and create hv_get_adj_host_time()
accessor to host_ts to avoid code duplication.

The host may send multiple negotiation packets
(due to timeout) before the KVP user-mode daemon
is connected. KVP user-mode daemon is connected.
We need to defer processing those packets
until the daemon is negotiated and connected.
It's okay for guest to respond
to all negotiation packets.
In addition, the host may send multiple staged
KVP requests as soon as negotiation is done.
We need to properly process those packets using one
tasklet for exclusive access to ring buffer.

Fix the rescind handling. This patch addresses the following rescind
scenario that is currently not handled correctly:
If a rescind were to be received while the offer is still being
peocessed, we will be blocked indefinitely since the rescind message
is handled on the same work element as the offer message. Fix this
issue.

The change to reschedule tasklet if more data arrives in ring buffer
can cause performance regression if host timing is such that the
next response happens in small window.
Go back to a modified version of the original looping behavior.
If the race occurs in a small time, then loop. But if the tasklet
has been running for a long interval due to flood, then reschedule
the tasklet to allow migration to ksoftirqd.

When we close a channel that has been rescinded, we will leak memory since
vmbus_teardown_gpadl() returns an error. Fix this so that we can properly
cleanup the memory allocated to the ring buffers.

Waiting for release_event in all three drivers introduced issues on release
as on_reset() hook is not always called. E.g. if the device was never
opened we will never get the completion.
Move the waiting code to hvutil_transport_destroy() and make sure it is
only called when the device is open. hvt->lock serialization should
guarantee the absence of races.

The first 2 issues can be handled by Stephen's recent patch
"vmbus: use rcu for per-cpu channel list", and the third issue
can be handled by calling tasklet_disable in vmbus_close_internal here.
We don't need the original hv_event_tasklet_disable/enable since we
now use per-channel tasklet instead of the previous per-CPU tasklet,
and actually we must remove them due to the side effect now:
vmbus_process_offer -> hv_event_tasklet_enable -> tasklet_schedule will
start the per-channel callback prematurely, cauing NULL dereferencing
(the channel may haven't been properly configured to run the callback yet).

The per-cpu channel list is now referred to in the interrupt
routine. This is mostly safe since the host will not normally generate
an interrupt when channel is being deleted but if it did then there
would be a use after free problem.
To solve, this use RCU protection on ther per-cpu list.

We are going to split <linux/sched/task_stack.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.
Create a trivial placeholder <linux/sched/task_stack.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.
Include the new header in the files that are going to need it.

All current usage of vmbus write uses the acquire_lock flag, therefore
having it be optional is unnecessary. This also fixes a sparse warning
since sparse doesn't like when a function has conditional locking.

Change the simple boolean batched_reading into a tri-value.
For future NAPI support in netvsc driver, the callback needs to
occur directly in interrupt handler.
Batched mode is also changed to disable host interrupts immediately
in interrupt routine (to avoid unnecessary host signals), and the
tasklet is rescheduled if more data is detected.

Use standard kernel operations for find first set bit to traverse
the channel bit array. This has added benefit of speeding up
lookup on 64 bit and because it uses find first set instruction.

With TimeSync version 4 protocol support we started updating system time
continuously through the whole lifetime of Hyper-V guests. Every 5 seconds
there is a time sample from the host which triggers do_settimeofday[64]().
While the time from the host is very accurate such adjustments may cause
issues:
- Time is jumping forward and backward, some applications may misbehave.
- In case an NTP server runs in parallel and uses something else for time
sync (network, PTP,...) system time will never converge.
- Systemd starts annoying you by printing "Time has been changed" every 5
seconds to the system log.
Instead of doing in-kernel time adjustments offload the work to an
NTP client by exposing TimeSync messages as a PTP device. Users may now
decide what they want to use as a source.
I tested the solution with chrony, the config was:
refclock PHC /dev/ptp0 poll 3 dpoll -2 offset 0
The result I'm seeing is accurate enough, the time delta between the guest
and the host is almost always within [-10us, +10us], the in-kernel solution
was giving us comparable results.
I also tried implementing PPS device instead of PTP by using not currently
used Hyper-V synthetic timers (we use only one of four for clockevent) but
with PPS source only chrony wasn't able to give me the required accuracy,
the delta often more that 100us.

Previously, we were assuming that each IC protocol version was tied to a
specific host version. For example, some Windows 10 preview hosts only
support v3 TimeSync even though driver assumes v4 is supported by all
Windows 10 hosts.
The guest will stop trying to negotiate even though older supported
versions may still be offered by the host.
Make IC version negotiation more robust by going through all versions
that are supported by the guest.

Coverity scan gives a warning when there is fall through in a switch
without a comment. This fall through is intentional as ol_waitevent needs
to be completed to unblock hv_mem_hot_add() allowing it to process next
requests regardless of the result of if we were able to online this block.

We need to cleanup the hypercall page before doing kexec/kdump or the new
kernel may crash if it tries to use it. Reuse the now-empty hv_cleanup
function renaming it to hyperv_cleanup and moving to the arch specific
code.

Commit a389fcfd2c ("Drivers: hv: vmbus: Fix signaling logic in
hv_need_to_signal_on_read()")
added the proper mb(), but removed the test "prev_write_sz < pending_sz"
when making the signal decision.
As a result, the guest can signal the host unnecessarily,
and then the host can throttle the guest because the host
thinks the guest is buggy or malicious; finally the user
running stress test can perceive intermittent freeze of
the guest.
This patch brings back the test, and properly handles the
in-place consumption APIs used by NetVSC (see get_next_pkt_raw(),
put_pkt_raw() and commit_rd_index()).
