To allow creating more than one netdev over the same PCI function, we
change the driver such that global NIC resources are created once and
later be shared amongst all the mlx5e netdevs running over that port.
Move the CQ UAR, PD (pdn), Transport Domain (tdn), MKey resources from
being kept in the mlx5e priv part to a new resources structure
(mlx5e_resources) placed under the mlx5_core device.
This patch doesn't add any new functionality.

Add a new namespace (MLX5_FLOW_NAMESPACE_OFFLOADS) to be populated
with flow steering rules that deal with rules that have have to
be executed before the EN NIC steering rules are matched.
The namespace is located after the bypass name-space and before the
kernel name-space. Therefore, it precedes the HW processing done for
rules set for the kernel NIC name-space.
Under SRIOV, it would allow us to match on e-switch missed packet
and forward them to the relevant VF representor TIR.

The current implementation does not handle timeout in case of command
with callback request, and this can lead to deadlock if the command
doesn't get fw response.
Add delayed callback timeout work before posting the command to fw.
In case of real fw command completion we will cancel the delayed work.
In case of fw command timeout the callback timeout handler will be
called and it will simulate fw completion with timeout error.

Previous to this patch auto negotiation was reported off although it was
on by default in hardware. This patch reports the correct information to
ethtool and allows the user to toggle it on/off.
Added another parameter to set port proto function in order to pass
the auto negotiation field to the hardware.

Add a dedicated function to toggle port link. It should be called only
after setting a port register.
Toggle will set port link to down and bring it back up in case that it's
admin status was up.

Configuring and managing HW rate limit tables.
The HW holds a table of rate limits, each rate is
associated with an index in that table.
Later a Send Queue uses this index to set the rate limit.
Multiple Send Queues can have the same rate limit, which is
represented by a single entry in this table.
Even though a rate can be shared, each queue is being rate
limited independently of others.
The SW shadow of this table holds the rate itself,
the index in the HW table and the refcount (number of queues)
working with this rate.
The exported functions are mlx5_rl_add_rate and mlx5_rl_remove_rate.
Number of different rates and their values are derived
from HW capabilities.

Currently, the SRQ API uses the obsolete mlx5_*_srq_mbox_{in,out}
structs which limit the ability to pass the SRQ attributes between
net and IB parts of the driver.
This patch changes the SRQ API so as to use auto-generated structs
and provides a better way to pass attributes which will be in use by
coming features.

The driver exposes interfaces that directly relate to HW state.
Upon fatal error, consumers of these interfaces (ULPs) that rely
on completion of all their posted work-request could hang, thereby
introducing dependencies in shutdown order. To prevent this from
happening, we manage the relevant resources (CQs, QPs) that are used
by the device. Upon a fatal error, we now generate simulated
completions for outstanding WQEs that were not completed at the
time the HW was reset.
It includes invoking the completion event handler for all involved
CQs so that the ULPs will poll those CQs. When polled we return
simulated CQEs with IB_WC_WR_FLUSH_ERR return code enabling ULPs
to clean up their resources and not wait forever for completions
upon receiving remove_one.
The above change requires an extra check in the data path to make
sure that when device is in error state, the simulated CQEs will
be returned and no further WQEs will be posted.

If the caller specified IB_SEND_FENCE in the send flags of the work
request and no previous work request stated that the successive one
should be fenced, the work request would be executed without a fence.
This could result in RDMA read or atomic operations failure due to a MR
being invalidated. Fix this by adding the mlx5 enumeration for fencing
RDMA/atomic operations and fix the logic to apply this.

In RoCE, the RDMA-CM needs the node guid to establish connection
between nodes.
Today, the node guid exposed to mlx5 Ethernet VFs is zero, therefore
RDMA-CM on the VF is broken.
Whenever the administrator sets a MAC for a VF, derive the node guid
from it and set it as well in the following way:

Flow steering infrastructure is currently used only on link layer
ethernet, therefore the driver should initialize the flow steering
when the device link layer is ethernet.
In addition, add missing capability check before initializing the
namespace of NIC RX flow tables.

Having MLX5_CMD_OP_MAX on another file causes us to repeatedly miss
accounting new commands added to the driver and hence there're no entries
for them in debugfs. To solve that, we integrate it into the commands enum
as the last entry.

Pkey index fields in the QP context path record are extended to 16
bits, as required by IB spec (version 1.3).
This change affects all QP commands which include path records.
To enable this change, moved the free adaptive routing flag bit
(free_ar) to the most significant byte of the QP path record.

Previously, we've fired all our completion callbacks straight from
our ISR.
Some of those callbacks were lightweight (for example, mlx5 Ethernet
napi callbacks), but some of them did more work (for example,
the user-space RDMA stack uverbs' completion handler). Besides that,
doing more than the minimal work in ISR is generally considered wrong,
it could even lead to a hard lockup of the system. Since when a lot
of completion events are generated by the hardware, the loop over
those events could be so long, that we'll get into a hard lockup by
the system watchdog.
In order to avoid that, add a new way of invoking completion events
callbacks. In the interrupt itself, we add the CQs which receive
completion event to a per-EQ list and schedule a tasklet. In the
tasklet context we loop over all the CQs in the list and invoke the
user callback.

If a counter has the aging flag set when created, it is added to a list
of counters that will be queried periodically from a workqueue. query
result and last use timestamp are cached.
add/del counter must be very efficient since thousands of such
operations might be issued in a second.
There is only a single reference to counters without aging, therefore
no need for locks.
But, counters with aging enabled are stored in a list. In order to make
code as lockless as possible, all the list manipulation and access to
hardware is done from a single context - the periodic counters query
thread.
The hardware supports multiple counters per FTE, however currently we
are using one counter for each FTE.

When adding a flow steering rule with a counter, need to supply a
destination of type MLX5_FLOW_DESTINATION_TYPE_COUNTER, with a pointer
to a struct mlx5_fc.
Also, MLX5_FLOW_CONTEXT_ACTION_COUNT bit should be set in the action.

In-order to avoid possible future conflicts between rdma and
net-next we added all expected updates to this file for this release.
If more changes will be submitted, we plan to do it only through
one of the subsystems, probably net-next.

the up-coming submissions to net-next and rdma trees.

CQE compression feature is meant to save PCIe bandwidth by
compressing few CQEs into smaller amount of bytes on PCIe.
CQE compression can be selectively enabled per CQ. By default
is disabled for now and will be enabled later on.

Currently, consumers of the flow steering infrastructure can't
choose their own flow table levels and are limited to one
flow table per level. This just waste levels.
Instead, we introduce here the possibility to use multiple
flow tables in a level. The user is free to connect these
flow tables, while following the rule (FTEs in FT of level x
could only point to FTs of level y where y > x).
In addition this patch switch the order of the create/destroy
flow tables of the NIC(vlan and main).

Now as rx-vlan offload can be disabled, packets can be received
with vlan tag not stripped, which means is_first_ethertype_ip will
return false, for that we need to check if the hardware reported
csum OK so we will report CHECKSUM_UNNECESSARY for those packets.

Introduce new access register named Ports Check Mask Register (PCMR) to
control all HW checks on port. With this register, the driver can
enable/disable Hardware FCS validation.

This counter is read from PPort statistics, then proccessed and stored as
a special handling software counter.
This counter is stored along software counters since it is the only PPort
counter that it's size is not 64 bits.

Redesign ethtool statistics handling and reporting in the driver:
1. Move counters to a separate file (en_stats.h).
2. Remove unnecessary dependencies between stats and strings.
3. Use counter descriptors which hold a name and offset for each counter,
and will be used to decide which counters will be exposed.
For example when adding a new software counter to ethtool, instead of:
1. Add to stats struct.
2. Add to strings struct in the same order.
3. Change macro defining number of software counters.
The only thing needed is to link the new counter to a counter descriptor.
VPort counters are a set of hardware traffic counters created automatically
for each virtual port opened.
PPort counters are a set of counters describing per physical port
performance statistics.
These counters are gathered from hardware register and divided to groups
according to different protocols.

This patch introduces kexec support for mlx5.
When switching kernels, kexec() calls shutdown, which unloads
the driver and cleans its resources.
In addition, remove unregister netdev from shutdown flow. This will
allow a clean shutdown, even if some netdev clients did not release their
reference from this netdev. Releasing The HW resources only is enough as
the kernel is shutting down

Set and report vport MTU rather than physical MTU,
Driver will set both vport and physical port mtu and will
rely on the query of vport mtu.
SRIOV VFs have to report their MTU to their vport manager (PF),
and this will allow them to work with any MTU they need
without failing the request.
Also for some cases where the PF is not a port owner, PF can
work with MTU less than the physical port mtu if set physical
port mtu didn't take effect.

Introduce the feature of multi-packet WQE (RX Work Queue Element)
referred to as (MPWQE or Striding RQ), in which WQEs are larger
and serve multiple packets each.
Every WQE consists of many strides of the same size, every received
packet is aligned to a beginning of a stride and is written to
consecutive strides within a WQE.
In the regular approach, each regular WQE is big enough to be capable
of serving one received packet of any size up to MTU or 64K in case of
device LRO is enabled, making it very wasteful when dealing with
small packets or device LRO is enabled.
For its flexibility, MPWQE allows a better memory utilization
(implying improvements in CPU utilization and packet rate) as packets
consume strides according to their size, preserving the rest of
the WQE to be available for other packets.
MPWQE default configuration:
Num of WQEs = 16
Strides Per WQE = 2048
Stride Size = 64 byte
The default WQEs memory footprint went from 1024*mtu (~1.5MB) to
16 * 2048 * 64 = 2MB per ring.
However, HW LRO can now be supported at no additional cost in memory
footprint, and hence we turn it on by default and get an even better
performance.
Performance tested on ConnectX4-Lx 50G.
To isolate the feature under test, the numbers below were measured with
HW LRO turned off. We verified that the performance just improves when
LRO is turned back on.
* Netperf single TCP stream:
- BW raised by 10-15% for representative packet sizes:
default, 64B, 1024B, 1478B, 65536B.
* Netperf multi TCP stream:
- No degradation, line rate reached.
* Pktgen: packet rate raised by 2-10% for traffic of different message
sizes: 64B, 128B, 256B, 1024B, and 1500B.
* Pktgen: packet loss in bursts of small messages (64byte),
single stream:

As expected as the driver can receive as many small packets (<=64B) as

A queue counter can collect several statistics for one or more
hardware queues (QPs, RQs, etc ..) that the counter is attached to.
For Ethernet it will provide an "out of buffer" counter which
collects the number of all packets that are dropped due to lack
of software buffers.
Here we add device commands to alloc/query/dealloc queue counters.

Implement the IB defined callbacks used to manipulate the policy for the

Add a vf parameter to mlx5_core_query_vport_counter so we can call it to
query counters of virtual functions. Also update current users of the
API.
PFs may call mlx5_core_query_vport_counter with other_vport set to
indicate that they are querying a virtual function. The virtual
function to be queried is given by the vf parameter. Virtual function
numbering is zero based so the first VF is 0 and so on. When a PF
queries its own function, the other_vport parameter is cleared.

Device capability function was called similar in all places.
It was called twice for every queried parameter, while the
difference between calls was in HCA capability mode only.
The change proposed unify these calls into one function.

Each bypass flow steering priority will be split into two priorities:
1. Priority for don't trap rules.
2. Priority for normal rules.
When user creates a flow using IB_FLOW_ATTR_FLAGS_DONT_TRAP flag, the
driver creates two flow rules, one used for receiving the traffic and
the other one for forwarding the packet to continue matching in lower
or equal priorities.

Add support to create flow rule that forward packets
to the first flow table in the next priority (next priority
could be the first priority in the next namespace or the
next priority in the same namespace).
This feature could be used for DONT_TRAP rules or rules
that only want to mark the packet with flow tag.
In order to do it optimally, each flow table has list
of all rules that point to this flow table,
when a flow table is destroyed/created, we update the list
head correspondingly.
This kind of rule is created when destination is NULL and
action is MLX5_FLOW_CONTEXT_ACTION_FWD_NEXT_PRIO.

Avoid double mapping of io mapped memory, Device page may be
mapped to non-cached(NC) or to write-combining(WC).
The code before this fix tries to map it both to WC and NC
contrary to what stated in Intel's software developer manual.
Here we remove the global WC mapping of all UARS
"dev->priv.bf_mapping", since UAR mapping should be decided
per UAR (e.g we want different mappings for EQs, CQs vs QPs).
Caller will now have to choose whether to map via
write-combining API or not.
mlx5e SQs will choose write-combining in order to perform
BlueFlame writes.

The command timeout is terribly long, whole two hours. Make it 60s so if
things do go wrong, the user gets feedback in relatively short time, so
they can take corrective actions and/or investigate using tools and such.

Mlx5's mkey mechanism is also used for memory windows.
The current code base uses MR (memory region) naming, which is
inaccurate. Changing MR to mkey in order to represent its different
usages more accurately.

The HW can supply several offloads for UD QP, added offloads for
checksumming for both TX and RX and LSO for TX.
Two new bits were added in order to expose and enable these offloads:
1. HCA capability bit: declares the support for IPoIB basic offloads.
2. QPC bit which will be used in the QP creation flow, which set these
abilities in the QP.

Modify mlx5_ib_process_mad to use PPCNT and query_vport commands
instead of MAD_IFC, as MAD_IFC is deprecated on new firmware
versions (and doesn't support RoCE anyway).
Traffic counters exist in both 32-bit and 64-bit forms.
Declaring support of extended coutners results in traffic counters
to be read in their 64-bit form only via the query_vport command.
Error counters exist only in 32-bit form and read via PPCNT command.
This commit also adds counters support in RoCE.

mlx5_ifc.h is a header file representing the API and ABI between
the driver to the firmware and hardware. This file is used from
both the mlx5_ib and mlx5_core drivers.
Previously, this file used incrementing counter to indicate
reserved fields, for example:

If one developer implements through net-next feature A that uses
reserved_0, they replace it with featureA and renames reserved_1 to
reserved_0. In the same kernel cycle, a 2nd developer could implement
feature B through the rdma tree, that uses reserved_1 and split it to
featureB and a smaller reserved_1 field. This will cause a conflict
when the two trees are merged.
The source of this conflict is that the 1st developer changed *all*
reserved fields.
As Linus suggested, we change the layout of structs to:

When modifying a QP, the desired operation was determined in
the mlx5_core using a transition table that takes the current
state, the final state, and returns the desired operation.
Since this logic will be used for Raw Packet QP, move the
operation table to the mlx5_ib.

When the user changes the Address Vector(AV) in the modify QP, he
provides an SL. This SL should be translated to Ethernet Priority
by taking the 3 LSB bits, and modify the QP's TIS according to this
Ethernet priority.

Since Raw Packet QP is composed of RQ and SQ, the IB QP's
state is derived from the sub-objects. Therefore we need
to query each one of the sub-objects, and decide on the
IB QP's state.

RQ/SQ will be used to implement IB verbs QPs, so the IB QP affiliated
events are affiliated also with SQs and RQs.
Since SQ, RQ and QP resource numbers do not share the same name
space, a queue type field was added to the event data to specify
the SW object that the event is affiliated with.

With several ConnectX-4 cards installed on a server, one may receive
irqn > 255 from the kernel API, which we mistakenly trim to 8bit.

Adding flow steering support by creating a flow-table per
priority (if rules exist in the priority). mlx5_ib uses
autogrouping and thus only creates the required destinations.
Also includes adding of these flow steering utilities
1. Parsing verbs flow attributes hardware steering specs.
2. Check if flow is multicast - this is required in order to decide
to which flow table will we add the steering rule.
3. Set outer headers in flow match criteria to zeros.

When the driver is loaded, we create flow steering namespace
for kernel bypass with nine priorities and another namespace
for leftovers(in order to catch packets that weren't matched).
Verbs applications will use these priorities.
we found nine as a number that balances the requirements from the
user and retains performance.
The bypass namespace is used by verbs applications that want to bypass
the kernel networking stack. The leftovers namespace is used by verbs
applications and the sniffer in order to catch packets that weren't
handled by any preceding rules.

Introduce the modify flow table command. This command is used when
we want to change the next flow table of an existing flow table.
The next flow table is defined as the table we search (in order
to find a match), if we couldn't find a match in any of the flow table
entries in the current flow table.

The root Flow Table for each Flow Table Type is defined,
by default, as the Flow Table with level 0.
In order not to use an empty flow tables and introduce new hops,
but still preserve space for flow-tables that have a priority
greater(lower number) than the current flow table, we introduce this
new set root flow table command.
This command tells the HW to start matching packets from the
assigned root flow table.
This command is used when we create new flow table with level lower than the
current lowest flow table or it is the first flow table.

When user add rule to autogrouped flow table, we search
for flow group with the same match criteria, if we don't
find such group then we create new flow group with the
required match criteria and insert the rule to this group.
We divide the flow table into required_groups + 1,
in order to reserve a part of the flow table for rules
which don't match any existing group.

HW is capable of 2 requestor endianness modes for standard 8 Bytes
atomic: BE (0x0) and host endianness (0x1). Read the supported modes
from hca atomic capabilities and configure HW to host endianness mode if
supported.

Add support of cross-channel functionality to mlx5
driver. This includes ability to ignore overrun for CQ
which intended for cross-channel, export device capability and
configure the QP to be sync master/slave queues.
The cross-channel enabled QP supports combination of
three possible properties:
* WQE processing on the receive queue of this QP
* WQE processing on the send queue of this QP
* WQE are supported on the send queue

Pass hca_core_clock_offset to user-space is mandatory in order to
let the user-space read the free-running clock register from the
right offset in the memory mapped page.
Passing this value is done by changing the vendor's command
and response of init_ucontext to be in extensible form.

Reporting the hca_core_clock (in kHZ) and the timestamp_mask in
query_device extended verb. timestamp_mask is used by users in order
to know what is the valid range of the raw timestamps, while
hca_core_clock reports the clock frequency that is used for
timestamps.

A mlx5 Ethernet port must be explicitly enabled for RoCE.
When RoCE is not enabled on the port, the NIC will refuse to create
QPs attached to it and incoming RoCE packets will be considered by the
NIC as plain Ethernet packets.

Expose the new flow steering API and remove the old
one.
Few changes are required:
1. The Ethernet flow steering follows the existing implementation, but uses
the new steering API. The old flow steering implementation is removed.
2. Move the E-switch FDB management to use the new API.
3. When driver is loaded call to mlx5_init_fs which initialize
the flow steering tree structure, open namespaces for NIC receive
and for E-switch FDB.
4. Call to mlx5_cleanup_fs when the driver is unloaded.

Flow steering initialization is based on static tree which
illustrates the flow steering tree when the driver is loaded. The
initialization considers the max supported flow table level of the device,
a minimum of 2 kernel flow tables(vlan and mac) are required to have
kernel flow table functionality.
The tree structures when the driver is loaded:

In the following patches, When the EN driver will use the flow steering
API, it create two flow tables and their flow groups under

E-Switch vport context is unlike NIC vport context, managed by the
E-Switch manager or vport_group_manager and not by the NIC(VF) driver.
The E-Switch manager can access (read/modify) any of its vports
E-Switch context.
Currently E-Switch vport context includes only clietnt and server
vlan insertion and striping data (for later support of VST mode).

Enabling E-Switch SRIOV for nvfs+1 vports.
Create E-Switch FDB for L2 UC/MC mac steering between VFs/PF and
external vport (Uplink).
FDB contains forwarding rules such as:
UC MAC0 -> vport0(PF).
UC MAC1 -> vport1.
UC MAC2 -> vport2.
MC MACX -> vport0, vport2, Uplink.
MC MACY -> vport1, Uplink.
For unmatched traffic FDB has the following default rules:
Unmached Traffic (src vport != Uplink) -> Uplink.
Unmached Traffic (src vport == Uplink) -> vport0(PF).
FDB rules population:
Each NIC vport (VF) will notify E-Switch manager of its UC/MC vport
context changes via modify vport context command, which will be
translated to an event that will be handled by E-Switch manager (PF)
which will update FDB table accordingly.

E-Switch is the software entity that represents and manages ConnectX4
inter-HCA ethernet l2 switching.
E-Switch has its own Virtual Ports, each Vport/vNIC/VF can be
connected to the device through a vport of an e-switch.
Each e-switch is managed by one vNIC identified by
HCA_CAP.vport_group_manager (usually it is the PF/vport[0]),
and its main responsibility is to forward each packet to the
right vport.
e-Switch needs to manage its own l2-table and FDB tables.
L2 table is a flow table that is managed by FW, it is needed for
Multi-host (Multi PF) configuration for inter HCA switching between
PFs.
FDB table is a flow table that is totally managed by e-Switch driver,
its main responsibility is to switch packets between e-Swtich internal
vports and uplink vport that belong to the same.
This patch introduces only e-Swtich l2 table management, FDB managemnt
will come later when ethernet SRIOV/VFs will be enabled.

In preparation for SR-IOV we add here an API to enable each e-switch
client (PF/VF) to configure its L2 MAC addresses and for the e-switch
manager (usually the PF) to access them in order to be able to
configure them into the e-switch.
Therefore we now pass vport num parameter to
mlx5_query_nic_vport_context, so PF can access other vports contexts.

This patch adds SRIOV base support for mlx5 supported devices. The same
driver is used for both PFs and VFs; VFs are identified by the driver
through the flag MLX5_PCI_DEV_IS_VF added to the pci table entries.
Virtual functions are created as usual through writing a value to the
sriov_numvs sysfs file of the PF device. Upon instantiating VFs, they will
all be probed by the driver on the hypervisor. One can gracefully unbind
them through /sys/bus/pci/drivers/mlx5_core/unbind.
mlx5_wait_for_vf_pages() was added to ensure that when a VF dies without
executing proper teardown, the hypervisor driver waits till all of the
pages that were allocated at the hypervisor to maintain its operation
are returned.
In order for the VF to be operational, the PF needs to call enable_hca
for it. This can be done before the VFs are created through a call to
pci_enable_sriov.
If the there are VFs assigned to a VMs when the driver of the PF is
unloaded, all the VF will experience system error and PF driver unloads
cleanly; in this case pci_disable_sriov is not called and the devices
will show when running lspci. Once the PF driver is reloaded, it will
sync its data structures which maintain state on its VFs.

Prevent outgoing multicast frames from looping back to the RX queue.
By introducing new HW capability self_lb_en_modifiable, which indicates
the support to modify self_lb_en bit in modify_tir command.
When this capability is set we can prevent TIRs from sending back
loopback multicast traffic to their own RQs, by "refreshing TIRs" with
modify_tir command, on every time new channels (SQs/RQs) are created at
device open.
This is needed since TIRs are static and only allocated once on driver
load, and the loopback decision is under their responsibility.
Fixes issues of the kind:
"IPv6: eth2: IPv6 duplicate address fe80::e61d:2dff:fe5c:f2e9 detected!"
The issue is seen since the IPv6 solicitations multicast messages are
loopedback and the network stack thinks they are coming from another host.

On device initialization, wait till firmware indicates that that it is done
with initialization before proceeding to initialize the device.
Also update initialization segment layout to match driver/firmware
interface definitions.

This patch implement the pci_error_handlers for mlx5_core which allow the
driver to recover from PCI error.
Once an error is detected in the PCI, the mlx5_pci_err_detected is called
and it:
1) Marks the device to be in 'Internal Error' state.
2) Dispatches an event to the mlx5_ib to flush all the outstanding cqes
with error.
3) Returns all the on going commands with error.
4) Unloads the driver.
Afterwards, the FW is reset and mlx5_pci_slot_reset is called and it
enables the device and restore it's pci state.
If the later succeeds, mlx5_pci_resume is called, and it loads the SW
stack.

Use a single threaded work queue for each device in the system instead of
using one thread for any device. This is required so we can concurrently
process system error handling for all the devices that need that.

In preparation to handling system errors at the mlx5_core level, change the
interface of cmd_work_handler to accept a 64 bit argument for the vector.
This allows to encode a flag that signifies when the handler is called
as a result of a driver logic that wishes to terminate commands that
the hardware may not be able to terminate. Such command completions
are detected at the handler and proper return status is encoded.
To be able to terminate page handler commands, we make sure to set
the corresponding bit in the bitmask.

Commit 96249d70dd ("IB/core: Guarantee that a local_dma_lkey
is available") allows ULPs that make use of the local dma key to keep
working as before by allocating a DMA MR with local permissions and
converted these consumers to use the MR associated with the PD
rather then device->local_dma_lkey.
ConnectIB has some known issues with memory registration
using the local_dma_lkey (SEND, RDMA, RECV seems to work ok).
Thus don't expose support for it (remove device->local_dma_lkey
setting), and take advantage of the above commit such that no regression
is introduced to working systems.
The local_dma_lkey support will be restored in CX4 depending on FW
capability query.

The mlx5 driver exposes device capability IB_DEVICE_LOCAL_DMA_LKEY
but does not set the the device local_dma_lkey. This breaks
rpcrdma drivers.
Query and set this lkey when creating the device resources.

- Port speed settings are applied by the device only upon
port admin status transition from DOWN to UP.
So we enforce this transition regardless of the port's
current operation state (which may be occasionally DOWN if
for example the network cable is disconnected).
- Fix the PORT_UP/DOWN device interface enum
- Set the local_port bit in the device PAOS register
- EXPORT the PAOS (Port Administrative and Operational Status)
register set/query access functions.

Create/destroy TIRs, TISs and flow tables upon PCI probe/remove rather
than upon the netdev ndo_open/stop.
Upon ndo_stop(), redirect all RX traffic to the (lately introduced)
"Drop RQ" and then close only the RX/TX rings, leaving the TIRs,
TISs and flow tables alive.

A regular TX WQE execution involves two or more DMA reads -
one to fetch the WQE, and another one per WQE gather entry.
These DMA reads obviously increase the TX latency.
There are two mlx5 mechanisms to bypass these DMA reads:
1) Inline WQE
2) Blue Flame (BF)
An inline WQE contains a whole packet, thus saves the DMA read/s
of the regular WQE gather entry/s. Inline WQE support was already
added in the previous commit.
A BF WQE is written directly to the device I/O mapped memory, thus
enables saving the DMA read that fetches the WQE.
The BF WQE I/O write must be in cache line granularity, thus uses
the CPU write combining mechanism.
A BF WQE I/O write acts also as a TX doorbell for notifying the
device of new TX WQEs.
A BF WQE is written to the same I/O mapped address as the regular TX
doorbell, thus this address is being mapped twice - once by ioremap()
and once by io_mapping_map_wc().
While both mechanisms reduce the TX latency, they both consume more CPU
cycles than a regular WQE:
- A BF WQE must still be written to host memory, in addition to being
written directly to the device I/O mapped memory.
- An inline WQE involves copying the SKB data into it.
To handle this tradeoff, we introduce here a heuristic algorithm that
strives to avoid using these two mechanisms in case the TX queue is
being back-pressured by the device, and limit their usage rate otherwise.
An inline WQE will always be "Blue Flamed" (written directly to the
device I/O mapped memory) while a BF WQE may not be inlined (may contain
gather entries).
Preliminary testing using netperf UDP_RR shows that the latency goes down
from 17.5us to 16.9us, while the message rate (tested with pktgen) stays
the same.

By affinity hints and XPS, each mlx5e channel is assigned a CPU
core.
Channel DMA coherent memory that is written by the NIC and read
by SW (e.g CQ buffer) is allocated on the NUMA node of the CPU
core assigned for the channel.
Channel DMA coherent memory that is written by SW and read by the
NIC (e.g SQ/RQ buffer) is allocated on the NUMA node of the NIC.
Doorbell record (written by SW and read by the NIC) is an
exception since it is accessed by SW more frequently.

To make it act as XOR we re-order the HW RSS indirection table.
Set XOR to be the default RSS hash function and add ethtool API to

Previously we configured HW MTU to be netdev->mtu, actually we
need to configure netdev->mtu + (ETH_HLEN + VLAN_HLEN + ETH_FCS_LEN).
Also, query MTU can not fail, hence make the relevant helper a
void functionm, add mlx5e_set_dev_port_mtu, helper function to
handle MTU setting.

Until now, mlx5_query_port_ptys always queried port number one.
Added new argument in the function's prototype so we can also query
the second port. This will be needed when thr helper will be invoked
from the IB driver on non FPP (Function-Per-Port) devices.

Extend the function prototypes for max and operational mtu to take the
local port number. In the Ethernet driver is this hard coded to one,
since ConnectX4 Ethernet devices are always function-per-port.
The IB driver also serves older devices (ConnectIB) which isn't such,
and hence the part can vary.

When working in ISSI > 0 mode, the model exposed by the device for
XRCs and SRQs is different. XRCs use XRC SRQs and plain SRQs are based
on RPM (Receive Memory Pool).

This is the Ethernet part of the driver for the Mellanox ConnectX(R)-4
Single/Dual-Port Adapter supporting 100Gb/s with VPI. The driver
extends the existing mlx5 driver with Ethernet functionality.
This patch contains the driver entry points but does not include
transmit and receive (see the previous patch in the series) routines.
It also adds the option MLX5_CORE_EN to Kconfig to enable/disable the
Ethernet functionality. Currently, Kconfig is programmed to make
Ethernet and Infiniband functionality mutally exclusive.
Also changed MLX5_INFINIBAND to be depandant on MLX5_CORE instead of
selecting it, since MLX5_CORE could be selected without MLX5_INFINIBAND
being selected.

This patch contains the resource handling files:
- flow_table.c: This file contains the code to handle the low level API
to configure hardware flow table. It is separated from
the flow_table_en.c, because it will be used in the
future by Raw Ethernet QP in mlx5_ib too.
- en_flow_table.[ch]: Ethernet flow steering handling. The flow table
object contain a mapping between flow specs and TIRs.
This mechanism will be used also to configure e-switch
in the future, when SR-IOV support will be added.
- transobj.[ch] - Low level functions to create/modify/destroy the
transport objects: RQ/SQ/TIR/TIS
- vport.[ch] - Handle attributes of a virtual port (vPort) in the
embedded switch. Currently this switch is a passthrough, until SR-IOV
support will be added.

mlx5_ifc.h was heavily modified here since it is now generated by a
script from the device specification (PRM rev 0.25). This specification
is backward compatible to existing hardware.
Some structures/fields were added here in order to enable the Ethernet
functionality of the driver.

As David Daney pointed in mlx4_core driver [1], mlx5_core is also
misusing the DMA-API.
This patch is removing the code that vmap() memory allocated by
dma_alloc_coherent().
After this patch, users of this drivers might fail allocating resources
on memory fragmeneted systems. This will be fixed later on.

This patch implement a page fault handler (leaving the pages pinned as
of time being). The page fault handler handles initiator and responder
page faults for UD/RC transports, for send/receive operations, as well
as RDMA read/write initiator support.

The new function allows updating the page tables of a memory region
after it was created. This can be used to handle page faults and page
invalidations.
Since mlx5_ib_update_mtt will need to work from within page invalidation,
so it must not block on memory allocation. It employs an atomic memory
allocation mechanism that is used as a fallback when kmalloc(GFP_ATOMIC) fails.
In order to reuse code from mlx5_ib_populate_pas, the patch splits
this function and add the needed parameters.

This patch wraps together several changes needed for on-demand paging support
in the mlx5_ib_populate_pas function, and when registering memory regions.
* Instead of accepting a UMR bit telling the function to enable all
access flags, the function now accepts the access flags themselves.
* For on-demand paging memory regions, fill the memory tables from the
correct list, and enable/disable the access flags per-page according
to whether the page is present.
* A new bit is set to enable writing of access flags when using the
firmware create_mkey command.
* Disable contig pages when on-demand paging is enabled.
In addition the patch changes the UMR code to use PTR_ALIGN instead of
our own macro.

The current UMR interface doesn't allow partial updates to a memory
region's page tables. This patch changes the interface to allow that.
It also changes the way the UMR operation validates the memory
region's state. When set, IB_SEND_UMR_FAIL_IF_FREE will cause the UMR
operation to fail if the MKEY is in the free state. When it is
unchecked the operation will check that it isn't in the free state.

Rather than using the basic BSF layout which utilizes a pre-configured
signature settings (sufficient for current DIF implementation), we use
the extended BSF layout to expose advanced signature settings. These
settings will also be exposed to the user later.

This patch puts a common part as the first field of mlx5_core_qp. This field is
used to identify which resource generated an event. This is required since upcoming
new resource types such as DC targets are allocated for the same numerical space
as regular QPs and may generate the same events. By searching the resource in the
same table we can then look at the common field to identify the resource.

Add an auto generated header file that describes hardware registers along with
set of macros that set/get values. The macros do static checks to avoid
overflow, handle endianess, and overall provide a clean way to code commands.
Currently the header file is small and we will add structs as we make use of
the macros.
A few commands were removed from the commands enum since they are not supported
currently and will be added when support is available.

Rearrange struct mlx5_caps so it has a "gen" field to represent the current
capabilities configured for the device. Max capabilities can also be queried
from the device. Also update capabilities struct to contain more fields as per
the latest revision if firmware specification.

In the event flow, we currently pass only a port number in the
void *data argument. Rather than pass a pointer to the event handlers,
we should use an "unsigned long" parameter, and pass the port number
value directly.
In the future, if necessary for some events, we can use the unsigned long
parameter to pass a pointer.

There were many places where parameters which should be u8/u16 were
integer type.
Additionally, in 2 places, a check for a non-null pointer was added

In preparation for a new mlx5 device which is VPI (i.e., ports can be
either IB or ETH), move the pci device functionality from mlx5_ib
to mlx5_core.
This involves the following changes:
1. Move mlx5_core_dev struct out of mlx5_ib_dev. mlx5_core_dev
is now an independent structure maintained by mlx5_core.
mlx5_ib_dev now has a pointer to that struct.
This requires changing a lot of places where the core_dev
struct was accessed via mlx5_ib_dev (now, this needs to
be a pointer dereference).
2. All PCI initializations are now done in mlx5_core. Thus,
it is now mlx5_core which does pci_register_device (and not
mlx5_ib, as was previously).
3. mlx5_ib now registers itself with mlx5_core as an "interface"
driver. This is very similar to the mechanism employed for
the mlx4 (ConnectX) driver. Once the HCA is initialized
(by mlx5_core), it invokes the interface drivers to do
their initializations.
4. There is a new event handler which the core registers:
mlx5_core_event(). This event handler invokes the
event handlers registered by the interfaces.

This commit takes care of the generated signature error CQE generated
by the HW (if happened). The underlying mlx5 driver will handle
signature error completions and will mark the relevant memory region
as dirty.
Once the consumer gets the completion for the transaction, it must
check for signature errors on signature memory region using a new
lightweight verb ib_check_mr_status().
In case the user doesn't check for signature error (i.e. doesn't call
ib_check_mr_status() with status check IB_MR_CHECK_SIG_STATUS), the
memory region cannot be used for another signature operation
(REG_SIG_MR work request will fail).

This patch implements IB_WR_REG_SIG_MR posted by the user.
Baisically this WR involves 3 WQEs in order to prepare and properly
register the signature layout:
1. post UMR WR to register the sig_mr in one of two possible ways:
* In case the user registered a single MR for data so the UMR data segment
consists of:
- single klm (data MR) passed by the user
- BSF with signature attributes requested by the user.
* In case the user registered 2 MRs, one for data and one for protection,
the UMR consists of:
- strided block format which includes data and protection MRs and
their repetitive block format.
- BSF with signature attributes requested by the user.
2. post SET_PSV in order to set the memory domain initial
signature parameters passed by the user.
SET_PSV is not signaled and solicited CQE.
3. post SET_PSV in order to set the wire domain initial
signature parameters passed by the user.
SET_PSV is not signaled and solicited CQE.
* After this compound WR we place a small fence for next WR to come.
This patch also introduces some helper functions to set the BSF correctly
and determining the signature format selectors.

If user requested signature enable we initialize relevant mlx5_ib_qp
members. We mark the qp as sig_enable and we increase the effective
SQ size, but still limit the user max_send_wr to original size
computed. We also allow the create_qp routine to accept sig_enable
create flag.

Support create_mr and destroy_mr verbs. Creating ib_mr may be done
for either ib_mr that will register regular page lists like
alloc_fast_reg_mr routine, or indirect ib_mrs that can register other
(pre-registered) ib_mrs in an indirect manner.
In addition user may request signature enable, that will mean that the
created ib_mr may be attached with signature attributes (BSF, PSVs).
Currently we only allow direct/indirect registration modes.

On some architectures (for example, arm), we don't end up indirectly
pulling in the declaration of kzalloc() and kfree(), and so building
anything that includes <linux/mlx5/driver.h> breaks. Fix this by adding
an explicit include to get the declaration.

Commit c1be5232d2 ("Fix micro UAR allocator") broke binary compatibility
between libmlx5 and mlx5_ib since it defines a different value to the number
of micro UARs per page, leading to wrong calculation in libmlx5. This patch
defines struct mlx5_ib_alloc_ucontext_req_v2 as an extension to struct
mlx5_ib_alloc_ucontext_req. The extended size is determined in mlx5_ib_alloc_ucontext()
and in case of old library we use uuarn 0 which works fine -- this is
acheived due to create_user_qp() falling back from high to medium then to
low class where low class will return 0. For new libraries we use the
more sophisticated allocation algorithm.

The micro UAR (uuar) allocator had a bug which resulted from the fact
that in each UAR we only have two micro UARs avaialable, those at
index 0 and 1. This patch defines iterators to aid in traversing the
list of available micro UARs when allocating a uuar.
In addition, change the logic in create_user_qp() so that if high
class allocation fails (high class means lower latency), we revert to
medium class and not to the low class.

Connect-IB firmware requires 4K pages to be communicated with the
driver. This patch breaks larger pages to 4K units to enable support
for architectures utilizing larger page size, such as PowerPC. This
patch also fixes several places that referred to PAGE_SHIFT instead of
explicit 12 which is the inherent page shift on Connect-IB.

Use asynchronous commands to execute up to eight concurrent create MR

In the previous QUERY_PAGES command version we used one command to get the
required amount of boot, init and post init pages. The new version uses the
op_mod field to specify whether the query is for the required amount of boot,
init or post init pages. In addition the output field size for the required
amount of pages increased from 16 to 32 bits.
In MANAGE_PAGES command the input_num_entries and output_num_entries fields
sizes changed from 16 to 32 bits and the PAS tables offset changed to 0x10.
In the pages request event the num_pages field also changed to 32 bits.
In the HCA-capabilities-layout the size and location of max_qp_mcg field has
been changed to support 24 bits.
This patch isn't compatible with firmware versions < 5; however, it turns out that the
first GA firmware we will publish will not support previous versions so this should be OK.

Introduce enbale_hca and disable_hca commands to signify when the
driver starts or ceases to operate on the device.
In addition the driver will use boot and init pages count; boot pages
is required to allow firmware to complete boot commands and the other
to complete init hca. Command interface revision is bumped to 4 to
enforce using supported firmware.
This patch breaks compatibility with old versions of firmware (< 4);

Sparse reported an endianness bug in the assignment to hca_cap.uar_page_sz.
Fix the declaration of this field to be __be16 (which is what is in
the firmware spec), renaming the field to log_uar_pg_size to conform
to the spec, which fixes the endianness bug reported by sparse.

The driver is comprised of two kernel modules: mlx5_ib and mlx5_core.
This partitioning resembles what we have for mlx4, except that mlx5_ib
is the pci device driver and not mlx5_core.
mlx5_core is essentially a library that provides general functionality
that is intended to be used by other Mellanox devices that will be
introduced in the future. mlx5_ib has a similar role as any hardware
device under drivers/infiniband/hw.
