Dan Williams b7b3c01b19 mm/memremap_pages: support multiple ranges per invocation
In support of device-dax growing the ability to front physically
dis-contiguous ranges of memory, update devm_memremap_pages() to track
multiple ranges with a single reference counter and devm instance.

Convert all [devm_]memremap_pages() users to specify the number of ranges
they are mapping in their 'struct dev_pagemap' instance.

Dan Williams a4574f63ed mm/memremap_pages: convert to 'struct range'
The 'struct resource' in 'struct dev_pagemap' is only used for holding
resource span information.  The other fields, 'name', 'flags', 'desc',
'parent', 'sibling', and 'child' are all unused wasted space.

This is in preparation for introducing a multi-range extension of
devm_memremap_pages().

The bulk of this change is unwinding all the places internal to libnvdimm
that used 'struct resource' unnecessarily, and replacing instances of
'struct dev_pagemap'.res with 'struct dev_pagemap'.range.

P2PDMA had a minor usage of the resource flags field, but only to report
failures with "%pR".  That is replaced with an open coded print of the
range.

Dan Williams 9ffc1d19fc mm/memremap_pages: Introduce memremap_compat_align()
The "sub-section memory hotplug" facility allows memremap_pages() users
like libnvdimm to compensate for hardware platforms like x86 that have a
section size larger than their hardware memory mapping granularity.  The
compensation that sub-section support affords is being tolerant of
physical memory resources shifting by units smaller (64MiB on x86) than
the memory-hotplug section size (128 MiB). Where the platform
physical-memory mapping granularity is limited by the number and
capability of address-decode-registers in the memory controller.

While the sub-section support allows memremap_pages() to operate on
sub-section (2MiB) granularity, the Power architecture may still
require 16MiB alignment on "!radix_enabled()" platforms.

In order for libnvdimm to be able to detect and manage this per-arch
limitation, introduce memremap_compat_align() as a common minimum
alignment across all driver-facing memory-mapping interfaces, and let
Power override it to 16MiB in the "!radix_enabled()" case.

The assumption / requirement for 16MiB to be a viable
memremap_compat_align() value is that Power does not have platforms
where its equivalent of address-decode-registers never hardware remaps a
persistent memory resource on smaller than 16MiB boundaries. Note that I
tried my best to not add a new Kconfig symbol, but header include
entanglements defeated the #ifndef memremap_compat_align design pattern
and the need to export it defeats the __weak design pattern for arch
overrides.

Aneesh Kumar K.V cf387d9644 libnvdimm/altmap: Track namespace boundaries in altmap
With PFN_MODE_PMEM namespace, the memmap area is allocated from the device
area. Some architectures map the memmap area with large page size. On
architectures like ppc64, 16MB page for memap mapping can map 262144 pfns.
This maps a namespace size of 16G.

When populating memmap region with 16MB page from the device area,
make sure the allocated space is not used to map resources outside this
namespace. Such usage of device area will prevent a namespace destroy.

Add resource end pnf in altmap and use that to check if the memmap area
allocation can map pfn outside the namespace. On ppc64 in such case we fallback
to allocation from memory.

Christoph Hellwig 3ed2dcdf54 memremap: validate the pagemap type passed to devm_memremap_pages

Dan Williams 50f44ee724 mm/devm_memremap_pages: fix final page put race
Logan noticed that devm_memremap_pages_release() kills the percpu_ref
drops all the page references that were acquired at init and then
immediately proceeds to unplug, arch_remove_memory(), the backing pages
for the pagemap.  If for some reason device shutdown actually collides
with a busy / elevated-ref-count page then arch_remove_memory() should
be deferred until after that reference is dropped.

As it stands the "wait for last page ref drop" happens *after*
devm_memremap_pages_release() returns, which is obviously too late and
can lead to crashes.

Fix this situation by assigning the responsibility to wait for the
percpu_ref to go idle to devm_memremap_pages() with a new ->cleanup()
callback.  Implement the new cleanup callback for all
devm_memremap_pages() users: pmem, devdax, hmm, and p2pdma.

Dan Williams a95c90f1e2 mm, devm_memremap_pages: fix shutdown handling
The last step before devm_memremap_pages() returns success is to allocate
a release action, devm_memremap_pages_release(), to tear the entire setup
down.  However, the result from devm_add_action() is not checked.

Checking the error from devm_add_action() is not enough.  The api
currently relies on the fact that the percpu_ref it is using is killed by
the time the devm_memremap_pages_release() is run.  Rather than continue
this awkward situation, offload the responsibility of killing the
percpu_ref to devm_memremap_pages_release() directly.  This allows
devm_memremap_pages() to do the right thing relative to init failures and
shutdown.

Without this change we could fail to register the teardown of
devm_memremap_pages().  The likelihood of hitting this failure is tiny as
small memory allocations almost always succeed.  However, the impact of
the failure is large given any future reconfiguration, or disable/enable,
of an nvdimm namespace will fail forever as subsequent calls to
devm_memremap_pages() will fail to setup the pgmap_radix since there will
be stale entries for the physical address range.

An argument could be made to require that the ->kill() operation be set in
the @pgmap arg rather than passed in separately.  However, it helps code
readability, tracking the lifetime of a given instance, to be able to grep
the kill routine directly at the devm_memremap_pages() call site.

Logan Gunthorpe 977196b8c5 PCI/P2PDMA: Add PCI p2pmem DMA mappings to adjust the bus offset
The DMA address used when mapping PCI P2P memory must be the PCI bus
address.  Thus, introduce pci_p2pmem_map_sg() to map the correct addresses
when using P2P memory.

Memory mapped in this way does not need to be unmapped and thus if we
provided pci_p2pmem_unmap_sg() it would be empty.  This breaks the expected
balance between map/unmap but was left out as an empty function doesn't
really provide any benefit.  In the future, if this call becomes necessary
it can be added without much difficulty.

For this, we assume that an SGL passed to these functions contain all P2P
memory or no P2P memory.

Logan Gunthorpe 52916982af PCI/P2PDMA: Support peer-to-peer memory
Some PCI devices may have memory mapped in a BAR space that's intended for
use in peer-to-peer transactions.  To enable such transactions the memory
must be registered with ZONE_DEVICE pages so it can be used by DMA
interfaces in existing drivers.

Add an interface for other subsystems to find and allocate chunks of P2P
memory as necessary to facilitate transfers between two PCI peers:

The new interface requires a driver to collect a list of client devices
involved in the transaction then call pci_p2pmem_find() to obtain any
suitable P2P memory.  Alternatively, if the caller knows a device which
provides P2P memory, they can use pci_p2pdma_distance() to determine if it
is usable.  With a suitable p2pmem device, memory can then be allocated
with pci_alloc_p2pmem() for use in DMA transactions.

Depending on hardware, using peer-to-peer memory may reduce the bandwidth
of the transfer but can significantly reduce pressure on system memory.
This may be desirable in many cases: for example a system could be designed
with a small CPU connected to a PCIe switch by a small number of lanes
which would maximize the number of lanes available to connect to NVMe

The code is designed to only utilize the p2pmem device if all the devices
involved in a transfer are behind the same PCI bridge.  This is because we
have no way of knowing whether peer-to-peer routing between PCIe Root Ports
is supported (PCIe r4.0, sec 1.3.1).  Additionally, the benefits of P2P
transfers that go through the RC is limited to only reducing DRAM usage
and, in some cases, coding convenience.  The PCI-SIG may be exploring
adding a new capability bit to advertise whether this is possible for
future hardware.

This commit includes significant rework and feedback from Christoph
Hellwig.

Dan Williams e763848843 mm: introduce MEMORY_DEVICE_FS_DAX and CONFIG_DEV_PAGEMAP_OPS
In preparation for fixing dax-dma-vs-unmap issues, filesystems need to
be able to rely on the fact that they will get wakeups on dev_pagemap
page-idle events. Introduce MEMORY_DEVICE_FS_DAX and
generic_dax_page_free() as common indicator / infrastructure for dax
filesytems to require. With this change there are no users of the
MEMORY_DEVICE_HOST designation, so remove it.

The HMM sub-system extended dev_pagemap to arrange a callback when a
dev_pagemap managed page is freed. Since a dev_pagemap page is free /
idle when its reference count is 1 it requires an additional branch to
check the page-type at put_page() time. Given put_page() is a hot-path
we do not want to incur that check if HMM is not in use, so a static
branch is used to avoid that overhead when not necessary.

Now, the FS_DAX implementation wants to reuse this mechanism for
receiving dev_pagemap ->page_free() callbacks. Rework the HMM-specific
static-key into a generic mechanism that either HMM or FS_DAX code paths
can enable.

For ARCH=um builds, and any other arch that lacks ZONE_DEVICE support,
care must be taken to compile out the DEV_PAGEMAP_OPS infrastructure.
However, we still need to support FS_DAX in the FS_DAX_LIMITED case
implemented by the s390/dcssblk driver.

Christoph Hellwig e8d5134833 memremap: change devm_memremap_pages interface to use struct dev_pagemap
This new interface is similar to how struct device (and many others)
work. The caller initializes a 'struct dev_pagemap' as required
and calls 'devm_memremap_pages'. This allows the pagemap structure to
be embedded in another structure and thus container_of can be used. In
this way application specific members can be stored in a containing
struct.

This will be used by the P2P infrastructure and HMM could probably
be cleaned up to use it as well (instead of having it's own, similar
'hmm_devmem_pages_create' function).

Greg Kroah-Hartman b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

Jérôme Glisse 5042db43cc mm/ZONE_DEVICE: new type of ZONE_DEVICE for unaddressable memory
HMM (heterogeneous memory management) need struct page to support
migration from system main memory to device memory.  Reasons for HMM and
migration to device memory is explained with HMM core patch.

This patch deals with device memory that is un-addressable memory (ie CPU
can not access it).  Hence we do not want those struct page to be manage
like regular memory.  That is why we extend ZONE_DEVICE to support
different types of memory.

A persistent memory type is define for existing user of ZONE_DEVICE and a
new device un-addressable type is added for the un-addressable memory
type.  There is a clear separation between what is expected from each
memory type and existing user of ZONE_DEVICE are un-affected by new
requirement and new use of the un-addressable type.  All specific code
path are protect with test against the memory type.

Because memory is un-addressable we use a new special swap type for when a
page is migrated to device memory (this reduces the number of maximum swap
file).

The main two additions beside memory type to ZONE_DEVICE is two callbacks.
First one, page_free() is call whenever page refcount reach 1 (which
means the page is free as ZONE_DEVICE page never reach a refcount of 0).
This allow device driver to manage its memory and associated struct page.

The second callback page_fault() happens when there is a CPU access to an
address that is back by a device page (which are un-addressable by the
CPU).  This callback is responsible to migrate the page back to system
main memory.  Device driver can not block migration back to system memory,
HMM make sure that such page can not be pin into device memory.

If device is in some error condition and can not migrate memory back then
a CPU page fault to device memory should end with SIGBUS.

Dan Williams 5c2c2587b1 mm, dax, pmem: introduce {get|put}_dev_pagemap() for dax-gup
get_dev_page() enables paths like get_user_pages() to pin a dynamically
mapped pfn-range (devm_memremap_pages()) while the resulting struct page
objects are in use.  Unlike get_page() it may fail if the device is, or
is in the process of being, disabled.  While the initial lookup of the
range may be an expensive list walk, the result is cached to speed up
subsequent lookups which are likely to be in the same mapped range.

devm_memremap_pages() now requires a reference counter to be specified
at init time.  For pmem this means moving request_queue allocation into
pmem_alloc() so the existing queue usage counter can track "device
pages".

ZONE_DEVICE pages always have an elevated count and will never be on an
lru reclaim list.  That space in 'struct page' can be redirected for
other uses, but for safety introduce a poison value that will always
trip __list_add() to assert.  This allows half of the struct list_head
storage to be reclaimed with some assurance to back up the assumption
that the page count never goes to zero and a list_add() is never
attempted.

Dan Williams 4b94ffdc41 x86, mm: introduce vmem_altmap to augment vmemmap_populate()
In support of providing struct page for large persistent memory
capacities, use struct vmem_altmap to change the default policy for
allocating memory for the memmap array.  The default vmemmap_populate()
allocates page table storage area from the page allocator.  Given
persistent memory capacities relative to DRAM it may not be feasible to
store the memmap in 'System Memory'.  Instead vmem_altmap represents
pre-allocated "device pages" to satisfy vmemmap_alloc_block_buf()
requests.

Dan Williams 9476df7d80 mm: introduce find_dev_pagemap()
There are several scenarios where we need to retrieve and update
metadata associated with a given devm_memremap_pages() mapping, and the
only lookup key available is a pfn in the range:

1/ We want to augment vmemmap_populate() (called via arch_add_memory())
   to allocate memmap storage from pre-allocated pages reserved by the
   device driver.  At vmemmap_alloc_block_buf() time it grabs device pages
   rather than page allocator pages.  This is in support of
   devm_memremap_pages() mappings where the memmap is too large to fit in
   main memory (i.e. large persistent memory devices).

2/ Taking a reference against the mapping when inserting device pages
   into the address_space radix of a given inode.  This facilitates
   unmap_mapping_range() and truncate_inode_pages() operations when the
   driver is tearing down the mapping.

3/ get_user_pages() operations on ZONE_DEVICE memory require taking a
   reference against the mapping so that the driver teardown path can
   revoke and drain usage of device pages.
