The driver is getting hit with 100s of RSCNs during remote port address
changes. Each of those RSCN's ends up generating UNREG_RPI and REG_PRI
mailbox commands. The discovery engine within the driver doesn't wait for
the mailbox command completions. Instead it sets state flags and moves
forward. At some point, there's a massive backlog of mailbox commands which
take time for the adapter to process. Additionally, it appears there were
duplicate events from the switch so the driver generated duplicate mailbox
commands for the same remote port. During this window, failures on PLOGI
and PRLI ELS's are see as the adapter is rejecting them as they are for
remote ports that still have pending mailbox commands.
Streamline the discovery engine so that PLOGI log checks for outstanding
UNREG_RPIs and defer the processing until the commands complete. This
better synchronizes the ELS transmission vs the RPI registrations.
Filter out multiple UNREG_RPIs being queued up for the same remote port.
Beef up log messages in this area.

The driver data structure for managing a mailbox command contained two
context fields. Unfortunately, the context were considered "generic" to be
used at the whim of the command code. Of course, one section of code used
fields this way, while another did it that way, and eventually there were
mixups.
Refactored the structure so that the generic contexts become a node context
and a buffer context and all code standardizes on their use.

The switches seem to respond faster to GID_PT vs GID_FT NameServer
queries. Add support for GID_PT to be used over GID_FT to enable
faster storage failover detection. Includes addition of new module
parameter to select between GID_PT and GID_FT (GID_FT is default).

An address change for a remote port cause PRLI for the wrong protocol
to be sent. The node copy done in the discovery code skipped copying
the fc4 protocols supported as well.
Fix the copy logic for the address change. Beefed up log messages in
this area as well.

After a LOGO in response to an ABTS timeout, a PLOGI wasn't issued to
re-establish the login. An nlp_type check in the LOGO completion
handler failed to restart discovery for NVME targets. Revised the
nlp_type check for NVME as well as SCSI.
While reviewing the LOGO handling a few other issues were seen and
were addressed:
- Better lock synchronization around ndlp data types
- When the ABTS times out, unregister the RPI before sending the LOGO
so that all local exchange contexts are cleared and nothing received
while awaiting LOGO/PLOGI handling will be accepted.
- LOGO handling optimized to:
Wait only R_A_TOV for a response.
It doesn't need to be retried on timeout. If there wasn't a
response, a PLOGI will be sent, thus an implicit logout
applies as well when the other port sees it.
If there is a response, any kind of response is considered "good"
and the XRI quarantined for a exchange qualifier window.
- PLOGI is issued as soon a LOGO state is resolved.

Driver only sends NVME PRLI to a device that also supports FCP. This resuls
in remote ports that don't have fc_remote_ports created for them. The driver
is clearing the nlp_fc4_type for a ndlp at the wrong time.
Fix by moving the nlp_fc4_type clearing to the discovery engine in the
DEVICE_RECOVERY state. Also ensure that rport registration is done for all
nlp_fc4_types.

Performance is affected when target queue depth is tracked. An atomic
counter is incremented on the submission path which competes with it being
decremented on the completion path. In addition, multiple CPUs can
simultaniously be manipulating this counter for the same ndlp.
Reduce the overhead by only performing the target increment/decrement when
the target queue depth is less than the overall adapter depth, thus is
actually meaningful.

For ABORT_XRI_CN command, firmware identifies XRI to abort by IOTAG and RPI
combination. For ELS aborts, driver specifies IOTAG correctly but RPI is
not specified.

Under large configurations, the driver would start to log message 6065 -
NVME out of buffers (exchanges).
The driver is using the ndlp cmd_qdepth value when determining the max
outstanding ios for an adapter. This value, by default, is set to 65536,
which exceeds the maximum exchange counts supported on an adapter. The ndlp
cmd_qdepth has no relevance and outstanding io count should be capped at
the max exchange count with IO requests beyond that level getting bounced
back with an EBUSY status so that they are retried by the block layer.

Nodelist entry for SCSI array ends up in UNMAPPED state. This is due to
illegal discovery State machine transition because of two PRLIs and the
first one failing with LS_RJT. Also, the error path was designed
assuming the PRLIs complete in the order they were sent, FCP first, then
NVME. In a failing case, the array thinks about the first PRLI (FCP),
but issues LS_RJT for the 2nd PRLI immediately.
Fix PRLI completion error path for the ordering expectation. Ensure the
discovery state machine update is not set until all outstanding PRLIs
are complete.

In the lpfc discovery engine, when as a nvme target, where the driver
was performing mailbox io with the adapter for port login when a NVME
PRLI is received from the host. Rather than queue and eventually get
back to sending a response after the mailbox traffic, the driver
rejected the io with an error response.
Turns out this particular initiator didn't like the rejection values
(unable to process command/command in progress) so it never attempted a
retry of the PRLI. Thus the host never established nvme connectivity
with the lpfc target.
By changing the rejection values (to Logical Busy/nothing more), the
initiator accepted the response and would retry the PRLI, resulting in
nvme connectivity.

When enabled for both SCSI and NVME support, and connected pt2pt to a
SCSI only target, the driver nodelist entry for the remote port is left
in PRLI_ISSUE state and no SCSI LUNs are discovered. Works fine if only
configured for SCSI support.
Error was due to some of the prli points still reflecting the need to
send only 1 PRLI. On a lot of fabric configs, targets were NVME only,
which meant the fabric-reported protocol attributes were only telling
the driver one protocol or the other. Thus things worked fine. With
pt2pt, the driver must send a PRLI for both protocols as there are no
hints on what the target supports. Thus pt2pt targets were hitting the
multiple PRLI issues.
Complete the dual PRLI support. Track explicitly whether scsi (fcp) or
nvme prli's have been sent. Accurately track protocol support detected
on each node as reported by the fabric or probed by PRLI traffic.

Handling a rcv'ed PRLI incorrectly can cause the ndlp to end up in the
wrong state or the driver to ACC and PRLI when it should send LS_RJT.
The cause was due to the driver not properly looking at the PRLI type
and taking the multiple protocol support into consideration.
Resolved by adding checks in the various PRLI receive points to validate
PRLI type and reject if not valid for the enabled protocols and mode
(host vs target).

The driver crashes when attempting to use a freed ndpl pointer.
The pci_remove_one handler runs on a separate kernel thread. The order
of the removal is starting by freeing all of the ndlps and then
disabling interrupts. In between these two events the driver can still
receive an ELS and process it. When it tries to use the ndlp pointer
will be NULL
Change the order of the pci_remove_one vs disable interrupts so that
interrupts are disabled before the ndlp's are freed.

After link bounce in a NVME Pt2Pt config, the driver managed to map the
same nport twice, resulting in multiple device nodes for the same
namespace.
In Pt2Pt, the driver must send PRLI's for both (scsi) FCP and NVME
rather than using fabric aids. The driver was inconsistent on handling
various PRLI completions, especially rejects, which had reject codes
cross the different protocol PRLI completions.
Fixed to perform the following: if nvmet mode (fc port can only be a
nvme target) - rejects all unsolicitly FCP PRLI's. Never issues a FCP
PRLI.
The multiple protocol PRLI's are sent simultaneously. However, driver
will now only state transition after both PRLI's are complete. New flags
were added to aid tracking the responses from the different PRLI's.

When the switch blade is pulled out then plugged back in, the driver
does not issue a PLOGI to the target
When the switch blade is pulled out, it does not reset the link. The
driver ends up issuing a LOGO to the target, and finally sees devloss.
Since the driver believes that a LOGO is outstanding, it does not issue
a PLOGI to the target upon link up
Correct by placing the ndlp in UNUSED state When devloss happens in
LOGO_ISSUE state.

The driver does not discover targets when in loop mode.
The NLP type is correctly getting set when a fabric connection is
detected but, not for loop. The unknown NLP type means that the driver
does not issue a PRLI when in loop topology. Thus target discovery
fails.
Fix by checking the topology during discovery. If it is loop, set the
NLP FC4 type to FCP.

We might have a NULL pring in lpfc_els_abort(), for example on error
recovery path, since queues are destroyed during error recovery
mechanism.
In this case, we should just drop the abort since the queues will be
recreated anyway. This patch just verifies for NULL pointer and stop the
abortion of the queue in case of a NULL pring.
Also, this patch converts return type of lpfc_els_abort() from int to
void, since it's not checked anywhere.

Code review of NVMEI's FC_PORT_ROLE_NVME_DISCOVERY looked wrong.
Discussions with storage architecture team clarified NVMEI's audit of
the PRLI response port roles. Following up discussion with code review
showed a few minor corrections were required - especially in
anticipation of NVME auto discovery.
During PRLI, NVMEI should sent prli_init - which it it does. NVMET
should send prli_tgt and prli_disc - which it does. When NVMEI receives
a PRLI Response now, it audits the incoming target bits and stores the
attributes in the corresponding NDLP. Later, when NVMEI registers the
NVME rport, it uses the stored ndlp attributes to set the rport
port_roles correctly.

NVMET didn't have any RSCN handling at all and
would not execute implicit LOGO when receiving a PLOGI
from an rport that NVMET had in state UNMAPPED.
Clean up the logic in lpfc_nlp_state_cleanup for
initiators (FCP and NVME). NVMET should not respond to
RSCN including allocating new ndlps so this code was
conditionalized when nvmet_support is true. The check
for NLP_RCV_PLOGI in lpfc_setup_disc_node was moved
below the check for nvmet_support to allow the NVMET
to recover initiator nodes correctly. The implicit
logo was introduced with lpfc_rcv_plogi when NVMET gets
a PLOGI on an ndlp in UNMAPPED state. The RSCN handling
was modified to not respond to an RSCN in NVMET. Instead
NVMET sends a GID_FT and determines if an NVMEP_INITIATOR
it has is UNMAPPED but no longer in the zone membership.

NVME Initiator: Base modifications
This patch adds base modifications for NVME initiator support.
The base modifications consist of:
- Formal split of SLI3 rings from SLI-4 WQs (sometimes referred to as
rings as well) as implementation now widely varies between the two.
- Addition of configuration modes:
SCSI initiator only; NVME initiator only; NVME target only; and
SCSI and NVME initiator.
The configuration mode drives overall adapter configuration,
offloads enabled, and resource splits.
NVME support is only available on SLI-4 devices and newer fw.
- Implements the following based on configuration mode:
- Exchange resources are split by protocol; Obviously, if only
1 mode, then no split occurs. Default is 50/50. module attribute
allows tuning.
- Pools and config parameters are separated per-protocol
- Each protocol has it's own set of queues, but share interrupt
vectors.
SCSI:
SLI3 devices have few queues and the original style of queue
allocation remains.
SLI4 devices piggy back on an "io-channel" concept that
eventually needs to merge with scsi-mq/blk-mq support (it is
underway). For now, the paradigm continues as it existed
prior. io channel allocates N msix and N WQs (N=4 default)
and either round robins or uses cpu # modulo N for scheduling.
A bunch of module parameters allow the configuration to be
tuned.
NVME (initiator):
Allocates an msix per cpu (or whatever pci_alloc_irq_vectors
gets)
Allocates a WQ per cpu, and maps the WQs to msix on a WQ #
modulo msix vector count basis.
Module parameters exist to cap/control the config if desired.
- Each protocol has its own buffer and dma pools.

After a link bounce, when a remote port issues a LOGO while a REGLOGIN
is pending on that port, the driver does not clean up the ndlp
structure. May result in stack traces in the console log.

Fix the FLOGI discovery logic to comply with T11 standards
We weren't properly setting fabric parameters, such as R_A_TOV and E_D_TOV,
when we registered the vfi object in default configs and pt2pt configs.
Revise to now pass service params with the values to the firmware and
ensure they are reset on link bounce. Required reworking the call sequence
in the discovery threads.

The domain controller PLOGI's concurrent with prior LOGO's/unreg_rpi's
completing created a race condition where driver rpi ref count can
inadvertantly hit 0 and the rpi attempted to be freed. This error
sometimes resulted in Warning messages indicating kref.h via
lfpc_nlp_get+0x128.
Correct by dropping any new PLOGI until the prior nport state has settled.
