When updating the error capture code and introducing vma snapshots,
we introduced code to hold the vma in memory while capturing it,
calling i915_active_acquire_if_busy(). Now that function isn't relevant
for perma-pinned vmas and caused important vmas to be dropped from the
coredump. Like for example the GuC log.
Fix this by instead requiring those vmas to be pinned while capturing.
Tested by running the initial subtests of the gem_exec_capture igt test
with GuC submission enabled and verifying that a GuC log blob appears
in the output.

With asynchronous migrations, the vma state may be several migrations
ahead of the state that matches the request we're capturing.
Address that by introducing an i915_vma_snapshot structure that
can be used to snapshot relevant state at request submission.
In order to make sure we access the correct memory, the snapshots take
references on relevant sg-tables and memory regions.
Also move the capture list allocation out of the fence signaling
critical path and use the CONFIG_DRM_I915_CAPTURE_ERROR define to
avoid compiling in members and functions used for error capture
when they're not used.
Finally, Introduce lockdep annotation.

With both integrated and discrete Intel GPUs in a system, the current
global check of intel_iommu_gfx_mapped, as done from intel_vtd_active()
may not be completely accurate.
In this patch we add i915 parameter to intel_vtd_active() in order to
prepare it for multiple GPUs and we also change the check away from Intel
specific intel_iommu_gfx_mapped (global exported by the Intel IOMMU
driver) to probing the presence of IOMMU on a specific device using
device_iommu_mapped().
This will return true both for IOMMU pass-through and address translation
modes which matches the current behaviour. If in the future we wanted to
distinguish between these two modes we could either use
iommu_get_domain_for_dev() and check for __IOMMU_DOMAIN_PAGING bit
indicating address translation, or ask for a new API to be exported from
the IOMMU core code.

The gpu coredump typically takes place in a dma_fence signalling
critical path, and hence can't use GFP_KERNEL allocations, as that
means we might hit deadlocks under memory pressure. However
changing to __GFP_KSWAPD_RECLAIM which will be done in an upcoming
patch will instead mean a lower chance of the allocation succeeding.
In particular large contigous allocations like the coredump page
vector.
Remove the page vector in favor of a linked list of single pages.
Use the page lru list head as the list link, as the page owner is
allowed to do that.

On Xe_HP and beyond the SFC unit may be fused off, even if the
corresponding media engines are present. Check the SFC-specific fusing
before trying to dump the SFC_DONE instances.

The SFC_DONE register lives within the corresponding VD0/VD2/VD4/VD6
forcewake domain and is not accessible if the vdbox in that domain is
fused off and the forcewake is not initialized.
This mistake went unnoticed because until recently we were using the
wrong register offset for the SFC_DONE register; once the register
offset was corrected, we started hitting errors like

Xe_HPG adds some additional INSTDONE_GEOM debug registers; the Mesa team
has indicated that having these reported in the error state would be
useful for debugging GPU hangs. These registers are replicated per-DSS

We no longer have traditional slices on Xe_HP platforms, but the
INSTDONE registers are replicated according to gslice representation
which is similar. We can mostly re-use the existing instdone code with
just a few modifications:
* Create an alternate instdone loop macro that will iterate over the
flat DSS space, but still provide the gslice/dss steering values for
compatibility with the legacy code.
* We should allocate INSTDONE storage space according to the maximum
number of gslices rather than the maximum number of legacy slices to
ensure we have enough storage space to hold all of the values. XeHP
design has 8 gslices, whereas older platforms never had more than 3
slices.

We receive notification of an engine reset from GuC at its
completion. Meaning GuC has potentially cleared any HW state
we may have been interested in capturing. GuC resumes scheduling
on the engine post-reset, as the resets are meant to be transparent,
further muddling our error state.
There is ongoing work to define an API for a GuC debug state dump. The
suggestion for now is to manually disable FW initiated resets in cases
where debug state is needed.

The object ops i915_GEM_OBJECT_HAS_IOMEM and the object
I915_BO_ALLOC_STRUCT_PAGE flags are considered immutable by
much of our code. Introduce a new mem_flags member to hold these
and make sure checks for these flags being set are either done
under the object lock or with pages properly pinned. The flags
will change during migration under the object lock.

I doubt anyone has used the display error state since CS flips
went the way of the dodo. Just nuke it.
It might be semi interesting to have something like this for
FIFO underruns and the like, but as it stands this wouldn't
provide a sufficient amount of information. So would need
an extensive rewrite anyway.
The lockless power well handling is also racy, so this could
just be contributing noise to test results if we end up
accessing something with the relevant power well already
disabled.

io_mapping_map_wc() expects the offset to be relative to the iomapping
base address. Currently we just pass in the physical address for the
page which only works if the region.start starts at zero.

Pull the GT clock information [used to derive CS timestamps and PM
interval] under the GT so that is it local to the users. In doing so, we
consolidate the two references for the same information, of which the
runtime-info took note of a potential clock source override and scaling
factors.

Instead of printing out the internal engine mask, which can change between
kernel versions making it difficult to map to actual engines, present a
bitmask of hanging engines ABI classes. For example:
[drm] GPU HANG: ecode 9:8:24dffffd, in gem_exec_schedu [1334]
Engine ABI class is useful to quickly categorize render vs media etc hangs
in bug reports. Considering virtual engine even more so than the current
scheme.

Between events which trigger engine and GPU resets and capturing the error
state we lose information on which engine triggered the reset. Improve
this by passing in the hung engine mask down to error capture.
Result is that the list of engines in user visible "GPU HANG: ecode
<gen>:<engines>:<ecode>, <process>" is now a list of hanging and not just
active engines. Most importantly the displayed process is now the one
which was actually hung.

As the error capture will compress user buffers as directed to by the
user, it can take an arbitrary amount of time and space. Break up the
compression loops with a call to cond_resched(), that will allow other
processes to schedule (avoiding the soft lockups) and also serve as a
warning should we try to make this loop atomic in the future.

Shrink the hold time for the error capture mutex to just around the
acquire/release of the PTE used for reading back the object via the
Global GTT. For platforms that do not need the GGTT read back, we can
skip the mutex entirely and allow concurrent error capture. Where we do
use the GGTT, by restricting the hold time around the slow readback and
compression, we are more resilient against softlockups (khungtaskd) as
the heartbeat may well also trigger an error while the first is on
going, and this allows the heartbeat reset to skip past the capture and
not be stalled.

We try not to assume that we captured any information, and so have to
check that error->gt exists before reporting. This check was missed in
err_print_capabilities, so lets break up the capability info and push it
into the GT dump.
We are still a long way from yamlifying this output!

Since the engines belong to the GT, move the runtime-updated list of
available engines to the intel_gt struct. The original mask has been
renamed to indicate it contains the maximum engine list that can be
found on a matching device.
In preparation for other info being moved to the gt in follow up patches
(sseu), introduce an intel_gt_info structure to group all gt-related
runtime info.

Start using device specific parameters instead of module parameters for
most things. The module parameters become the immutable initial values
for i915 parameters. The device specific parameters in i915->params
start life as a copy of i915_modparams. Any later changes are only
reflected in the debugfs.
The stragglers are:
* i915.force_probe and i915.modeset. Needed before dev_priv is
available. This is fine because the parameters are read-only and never
modified.
* i915.verbose_state_checks. Passing dev_priv to I915_STATE_WARN and
I915_STATE_WARN_ON would result in massive and ugly churn. This is
handled by not exposing the parameter via debugfs, and leaving the
parameter writable in sysfs. This may be fixed up in follow-up work.
* i915.inject_probe_failure. Only makes sense in terms of the module,
not the device. This is handled by not exposing the parameter via
debugfs.

We need to keep the default context state around to instantiate new
contexts (aka golden rendercontext), and we also keep it pinned while
the engine is active so that we can quickly reset a hanging context.
However, the default contexts are large enough to merit keeping in
swappable memory as opposed to kernel memory, so we store them inside
shmemfs. Currently, we use the normal GEM objects to create the default
context image, but we can throw away all but the shmemfs file.
This greatly simplifies the tricky power management code which wants to
run underneath the normal GT locking, and we definitely do not want to
use any high level objects that may appear to recurse back into the GT.
Though perhaps the primary advantage of the complex GEM object is that
we aggressively cache the mapping, but here we are recreating the
vm_area everytime time we unpark. At the worst, we add a lightweight
cache, but first find a microbenchmark that is impacted.
Having started to create some utility functions to make working with
shmemfs objects easier, we can start putting them to wider use, where
GEM objects are overkill, such as storing persistent error state.

We only hold the active spinlock while dumping the error state, and this
does not prevent another thread from retiring the request -- as it is
quite possible that despite us capturing the current state, the GPU has
completed the request. As such, it is dangerous to dereference state
below the request as it may already be freed, and the simplest way to
avoid the danger is not include it in the error state.

GPU saves accumulated context runtime (in CS timestamp units) in PPHWSP
which will be useful for us in cases when we are not able to track context
busyness ourselves (like with GuC). Keep a copy of this in struct
intel_context from where it can be easily read even if the context is not
pinned.

Now that we have offline error capture and can reset an engine from
inside an atomic context while also preserving the GPU state for
post-mortem analysis, it is time to handle error interrupts thrown by
the command parser.
This provides a much, much faster mechanism for us to detect known
problems than using heartbeats/hangchecks, and also provides a mechanism
for when those are disabled. However, it is limited to problems the HW
can detect in the CS and so not a complete solution for detecting lockups.
