Tomas Henzl f06aa52cb2 scsi: core: scsi_logging: Fix a BUG
The request_queue may be NULL in a request, for example when it comes from
scsi_ioctl_reset(). Check it before use.

Christoph Hellwig ce70fd9a55 scsi: core: Remove the cmd field from struct scsi_request
Now that each scsi_request is backed by a scsi_cmnd, there is no need to
indirect the CDB storage.  Change all submitters of SCSI passthrough
requests to store the CDB information directly in the scsi_cmnd, and while
doing so allocate the full 32 bytes that cover all Linux supported SCSI
hosts instead of requiring dynamic allocation for > 16 byte CDBs.  On
64-bit systems this does not change the size of the scsi_cmnd at all, while
on 32-bit systems it slightly increases it for now, but that increase will
be made up by the removal of the remaining scsi_request fields.

Bart Van Assche aa8e25e500 scsi: core: Use scsi_cmd_to_rq() instead of scsi_cmnd.request
Prepare for removal of the request pointer by using scsi_cmd_to_rq()
instead. Cast away constness where necessary when passing a SCSI command
pointer to scsi_cmd_to_rq(). This patch does not change any functionality.

Bart Van Assche dccc96abfb scsi: core: Reduce memory required for SCSI logging
The data structure used for log messages is so large that it can cause a
boot failure. Since allocations from that data structure can fail anyway,
use kmalloc() / kfree() instead of that data structure.

Hannes Reinecke 9e5ed2a5b3 scsi: use external buffer for command logging
Use an external buffer for __scsi_print_command() and move command
logging over to use the per-cpu logging buffer.  With that we can
guarantee the command always will always be formatted in one line.
So we can even print out a variable length command correctly across
several lines. Finally rename __scsi_print_command() to
__scsi_format_comment() to better reflect the functionality.

Hannes Reinecke ded85c193a scsi: Implement per-cpu logging buffer
Implement a per-cpu buffer for formatting messages to avoid line breaks
up under high load.  This patch implements scmd_printk() and
sdev_prefix_printk() using the per-cpu buffer and makes sdev_printk() a
wrapper for sdev_prefix_printk().
