Remove stale ide.h "configuration options":
* INITIAL_MULT_COUNT - always defined to 0
* SUPPORT_SLOW_DATA_PORTS - unused
* OK_TO_RESET_CONTROLLER - always defined to 1
* DISABLE_IRQ_NOSYNC - always defined to 0
Leave SUPPORT_VLB_SYNC (defined to 0 for CRIS and FRV, otherwise to 1)
for now but disallow overriding it by <asm/ide.h>.
There should be no functionality changes caused by this patch.

"extern inline" generates a warning with -Wmissing-prototypes and I'm
currently working on getting the kernel cleaned up for adding this to the
CFLAGS since it will help us to avoid a nasty class of runtime errors.
If there are places that really need a forced inline, __always_inline would be
the correct solution.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
