Chauhan, Vijay 8f03226358 [SCSI] scsi_dh_rdac: Retry for NOT_READY(02/04/01) in rdac device handler
During device discovery read capacity fails with 0x020401 and sets the
device size to 0. As a reason any I/O submitted to this path gets
killed at sd_prep_fn with BLKPREP_KILL. This patch is to retry for
0x020401. NEED_RETRY in scsi_decide_disposition does not give
sufficient time for the device to become ready.

Chauhan, Vijay 7687fb9209 [SCSI] scsi_dh_rdac: Retry mode select for NO_SENSE, ABORTED_COMMAND, UNIT_ATTENTION, NOT_READY(02/04/01)
This patch is to add retry for mode select if mode select command is
returned with sense NO_SENSE, UNIT_ATTENTION, ABORTED_COMMAND,
NOT_READY(02/04/01). This patch reorganise the sense keys from if-else
to switch-case format for better maintainability.

Chandra Seetharaman c85f8cb925 [SCSI] scsi_dh: Retry mode select in rdac device handler
When the mode select sent to the controller fails with the retryable
error, it is better to retry the mode_select from the hardware handler
itself, instead of propagating the failure to dm-multipath.

Alan D. Brunelle febd7a5c13 Commands needing to be retried require a complete re-initialization.
The test-unit-ready portion of this patch was causing boots to fail on
my test machine (as in http://lkml.org/lkml/2008/12/5/161). With this
patch in place, the system is booting reliably.

Mike Anderson found the same problem in the hp_hw_start_stop code,
and I applied the same solution in cdrom_read_cdda_bpc.

Chandra Seetharaman 5a36756b8b [SCSI] scsi_dh: Initialize path state to be passive when path is not owned
Set the path state to be passive when we learn that the controller does
not own the path to the LUN.

This will avoid sending even a single i/o thru the passive path at the
probe time.

Mike Christie 64f84bc1cf [SCSI] scsi_dh_alua: remove REQ_NOMERGE
We do not need to set REQ_NOMERGE because when the module calls
blk_execute_rq -> blk_execute_rq_nowait, blk_execute_rq_nowait sets
it for us. This brings all the modules in sync for those bits.

Mike Christie 6000a368cd [SCSI] block: separate failfast into multiple bits.
Multipath is best at handling transport errors. If it gets a device
error then there is not much the multipath layer can do. It will just
access the same device but from a different path.

This patch breaks up failfast into device, transport and driver errors.
The multipath layers (md and dm mutlipath) only ask the lower levels to
fast fail transport errors. The user of failfast, read ahead, will ask
to fast fail on all errors.

Note that blk_noretry_request will return true if any failfast bit
is set. This allows drivers that do not support the multipath failfast
bits to continue to fail on any failfast error like before. Drivers
like scsi that are able to fail fast specific errors can check
for the specific fail fast type. In the next patch I will convert
scsi.

Create a cache of devices that are seen in a system. This will avoid

Hannes Reinecke ae11b1b36d [SCSI] scsi_dh: attach to hardware handler from dm-mpath
multipath keeps a separate device table which may be
more current than the built-in one.
So we should make sure to always call ->attach whenever
a multipath map with hardware handler is instantiated.
And we should call ->detach on removal, too.

Hannes Reinecke b6ff1b14cd [SCSI] scsi_dh: Update EMC handler
This patch converts the EMC device handler to use a proper
state machine. We now also parse the extended INQUIRY
information to determine if long trespass commands are
supported. And we're now using the long trespass command
correctly. And finally there's now an check at init time
to refuse to attach to devices not supporting EMC-specific
VPD pages.

Hannes Reinecke 4c05ae52fc [SCSI] scsi_dh: Add 'dh_state' sysfs attribute
Implement a 'dh_state' sdev attribute for dynamic device handler
manipulation. A read on the attribute will return the name of
the currently attached device handler or 'detached' if no handler
is attached.
The attribute allows the following strings to be written:
- The name of the device handler to be attached if the state is
  'detached'.
- 'activate' to trigger path activation if a device handler
  is attached.
- 'detach' to detach the currently attached device handler.

Hannes Reinecke 765cbc6dad [SCSI] scsi_dh: Implement common device table handling
Instead of having each and every driver implement its own
device table scanning code we should rather implement a common
routine and scan the device tables there.
This allows us also to implement a general notifier chain
callback for all device handler instead for one per handler.

Chandra Seetharaman 33af79d12e scsi_dh: Verify "dev" is a sdev before accessing it.
Before accessing the device data structure in hardware handlers,
make sure it is a indeed a sdev device.

Yinghai Lu <yhlu.kernel@gmail.com> found the bug on Jul 16, 2008,

Chandra Seetharaman a6a8d9f87e [SCSI] scsi_dh: add infrastructure for SCSI Device Handlers
Some of the storage devices (that can be accessed through multiple paths),
do need some special handling for
	1. Activating the passive path of the storage access.
	2. Decode and handle the special sense codes returned by the devices.
	3. Handle the I/Os being sent to the passive path, especially
           during the device probe time.
when accessed through multiple paths.

As of today this special device handling is done at the dm-multipath
layer using dm-handlers. That works well for (1); for (2) to be handled
at dm layer, scsi sense information need to be exported from SCSI to dm-layer,
which is not very attractive; (3) cannot be done at all at the dm layer.

Device handler has been moved to SCSI mainly to handle (2) and (3) properly.
