Dominik Brodowski 5fa9167a1b pcmcia: rework the irq_req_t typedef
Most of the irq_req_t typedef'd struct can be re-worked quite
easily:

(1) IRQInfo2 was unused in any case, so drop it.

(2) IRQInfo1 was used write-only, so drop it.

(3) Instance (private data to be passed to the IRQ handler):
	Most PCMCIA drivers using pcmcia_request_irq() to actually
	register an IRQ handler set the "dev_id" to the same pointer
	as the "priv" pointer in struct pcmcia_device. Modify the two
	exceptions (ipwireless, ibmtr_cs) to also work this waym and
	set the IRQ handler's "dev_id" to p_dev->priv unconditionally.

(4) Handler is to be of type irq_handler_t.

(5) Handler != NULL already tells whether an IRQ handler is present.
	Therefore, we do not need the IRQ_HANDLER_PRESENT flag in
	irq_req_t.Attributes.

Dominik Brodowski 2caff14713 pcmcia: use dynamic debug infrastructure, deprecate CS_CHECK (wireless)
Convert PCMCIA drivers to use the dynamic debug infrastructure, instead of
requiring manual settings of PCMCIA_DEBUG.

Also, remove all usages of the CS_CHECK macro and replace them with proper
Linux style calling and return value checking. The extra error reporting may
be dropped, as the PCMCIA core already complains about any (non-driver-author)
errors.

David Kilroy 6415f7df10 orinoco: Handle suspend/restore in core driver
Each device does almost exactly the same things on suspend and resume
when upping and downing the interface. So move this logic into a common
routine.

David Kilroy 5381956b78 orinoco: move netdev interface creation to main driver
With the move to cfg80211 it's nice to keep the hardware operations
distinct from the interface, even though we can only support a single
interface.

This also means the driver resembles other cfg80211 drivers.

David Kilroy a2608362b2 orinoco: Replace net_device with orinoco_private in driver interfaces
Move away from using net_device as the main structure in orinoco
function calls. Use orinoco_private instead.

This makes more sense when we move to cfg80211, and we get wiphys as
well.

David Kilroy ac7cafd722 orinoco: Resume spectrum_cs in the same way as orinoco_cs
Retrieval of external firmware has been resolved, and should work with
the standard orinoco resume algorithm.

This fixes an issue where priv->hw_unavailable indicates the card is
ready when firmware has not been loaded.
