If we were not able to map the io bar in device init, don't attempt
to unmap it in device fini. All radeons should have a io bar, so
I doubt this would ever trigger, but just to be on the safe side...

This interface allows userspace to request hyperz support, it probably
needs more locking, and really reporting that you can have hyperz is racy
since someone else might get it before you do.

We used to limit the rs4xx gart aperture to 32 MB, but I suspect
that was due to not meeting the alignment requirements of the
aperture. This patch should only be applied after:
"drm/radeon/kms: fix gtt MC base alignment on rs4xx/rs690/rs740 asics"
has been applied.
This patch should probably soak for a bit in d-r-t.

The "error" variable is unsigned so it's never less than zero. I
changed it to check if (freq < current_freq) directly.
"best_error" is also unsigned so "best_error - 100" could be a large
number instead of a negative. Since "error" is unsigned it is never
less than a negative and so the cases where "best_error" is less than or
equal to 100 are false.

On systems using kexec, the new kernel is booted straight from the old kernel, without any warning to the graphics driver. So the GPU is basically left as-is in a running state, however the CPU side is completly reset.
Without stating the saneness of anyone using kexec on live systems, we should at least try not to crash the GPU. This patch resets 3 registers to 0 that could cause bad things to happen to the running system.
This allows kexec to work on a Power6/RN50 system.

This is required for the NB_MISC regs on rs780/rs880 which
means HDMI/DVI/DP ports using PCIEPHY won't work without
it. It might also help with s/r (asic init) issues on other
atombios cards.

We will need method of selecting encoder that should receive HDMI block. For
now we assign HDMI block to first enabled encoder. Hopefully there are not many
RS6x0 chips with two digital encoders.

decides to disable audio (module parameter) we still will try to use timer in
r600_audio_enable_polling. This would lead to BUG in kernel/timer.c.

rv6xx/rv7xx/evergreen families supported; older asics did
not have an internal thermal sensor.
Note, not all oems use the internal thermal sensor, so it's
only exposed in cases where it is used.
Note also, that most laptops use an oem specific ACPI solution for
GPU thermal information rather than using the internal thermal
sensor directly.

We only add/remove crtcs at driver load, you cannot remove when
the GPU is running a CS packet since the fd is open, when
GPU hotplugging on radeons actually is needed all this locking
needs a review and I've started re-working kms core locking to deal
with this better. But for now avoid long delays in CS processing when
hotplug detect is happening in a different thread.
this fixes a regression introduced with hotplug detection.

This fixes a regression caused by b2ea4aa67b
due to the way shared ddc with multiple digital connectors was handled.
You generally have two cases where DDC lines are shared:
- HDMI + VGA
- HDMI + DVI-D
HDMI + VGA is easy to deal with because you can check the EDID for the
to see if the attached monitor is digital. A shared DDC line with two
digital connectors is more complex. You can't use the hdmi bits in the
EDID since they may not be there with DVI<->HDMI adapters. In this case
all we can do is check the HPD pins to see which is connected as we have
no way of knowing using the EDID.

The asics in question have the following requirements with regard to
their gart setups:
1. The GART aperture size has to be in the form of 2^X bytes, where X is from 25 to 31
2. The GART aperture MC base has to be aligned to a boundary equal to the size of the
aperture.
3. The GART page table has to be aligned to the boundary equal to the size of the table.
4. The GART page table size is: table_entry_size * (aperture_size / page_size)
5. The GART page table has to be allocated in non-paged, non-cached, contiguous system
memory.
This patch takes care 2. The rest should already be handled properly.

Some RV100 cards with 2 VGA ports show up with DVI+VGA, however
some boards with DVI+VGA have the same subsystem ids. Better
to have a VGA port show up as DVI than having a non-useable
DVI port.

There is a regression from 2.6.34 related to the recent radeon power
management changes, caused by attempting to cancel a delayed work
item that's never been scheduled. However, the code as is has some
other issues potentially leading to visible problems.
First, the mutex around cancel_delayed_work() in radeon_pm_suspend()
doesn't really serve any purpose, because cancel_delayed_work() only
tries to delete the work's timer. Moreover, it doesn't prevent the
work handler from running, so the handler can do some wrong things if
it wins the race and in that case it will rearm itself to do some
more wrong things going forward. So, I think it's better to wait for
the handler to return in case it's already been queued up for
execution. Also, it should be prevented from rearming itself in that
case.
Second, in radeon_set_pm_method() the cancel_delayed_work() is not
sufficient to prevent the work handler from running and queing up
itself for the next run (the failure scenario is that
cancel_delayed_work() returns 0, so the handler is run, it waits on
the mutex and then rearms itself after the mutex has been released),
so again the work handler should be prevented from rearming itself in
that case..
Finally, there's a potential deadlock in radeon_pm_fini(), because
cancel_delayed_work_sync() is called under rdev->pm.mutex, but the
work handler tries to acquire the same mutex (if it wins the race).
Fix the issues described above.

Some IGP systems specify the system memory clock in the Firmware
table rather than the IGP info table. Check both and make sure
we have a value system memory clock value.

For some reason on resume, executing the BIOS scripts locks up the whole chipset, by avoiding the dynclk table the machine resumes properly and seems to function okay.

HDP non surface should cover the whole VRAM but we were misscomputing
the size and we endup in some case not covering the VRAM at all (if
VRAM size were > 1G). Covering more than the VRAM size shouldn't be
an issue.

Code did not handle projected 2d and depth coordinates, meaning potentially
set 3d or cube special handling might stick.
(Not sure what depth coord actually does, but I guess handling it

Installing 2.6.34 on a Power5/rn50 combo machine, X showed buggy sw rendering,
enabling tiling in the DDX fixed it. Investigation showed that a further /16
was needed in the untiled case on this chipset. Need further investigations
on what other chips this could affect, possibly rv100->rv280.

So when we added output polling, we'd suddenly use this code more often, and the fact that it always takes over crtc2 and messes with it during probing isn't what we really want to be happening. A more complete fix would to change it to use whatever crtc was free at the time, but for now lets stay simple and just don't poll if crtc2 is already in use.
Although a more correct fix was found I suspect we should do this as well, until we get a chance to readdres the tv out polling issues.

We really don't want to be polling tv-out but since we weren't forcing the
i2c lines to invalid (tv-out has no DDC), we were adding tv connectors to the
polling setup and this was causing blinking on secondary displays.
This fixes the regression Torsten reported.

The vddc value in the power tables is not an actual voltage
like on discrete r6xx/r7xx/evergreen systems, but instead has
a symbolic meaning (e.g., NONE, LOW, HIGH, etc.). See atombios.h
Most RS780/RS880 vbioses don't have a SetVoltage table anyway,
so it shouldn't be doing anything to the hardware at the moment.
I need to figure out how voltage is supposed to work on the newer
IGPs; until then, disable it.

This adds an additional profile, mid, to the pm profile
code which takes the place of the old low profile. The default
behavior remains the same, e.g., auto profile now selects between
mid and high profiles based on power source, however, you can now
manually force the low profile which was previously only available
as a dpms off state. Enabling the low profile when the displays
are on has been known to cause display corruption in some cases.

To make sure the default power state matches the boot up state.

This is needed to enable accel in the ddx. However,
due to a bug in older versions of the ddx, it relies
on accel being disabled in order to load properly on
evergreen chips. To maintain compatility, we add a new
get accel param and call that from the ddx. The old one
always returns false for evergreen cards.

PM attemps to unmap objects that aren't actually mapped into userspace ever,
so just don't bother unmapping them at this point, since all you are doing
is nothing. We should be making sure all access to these objects are locked in
kernel space instead. In theory the VRAM gart table is already done, and both
the shaders and stolen vga memory blocks are never accessed at runtime.

The default power state does not always match the default clocks and voltage
for a particular card. The information in the firmware info table is correct
and should be used in preference to the info the default power state.

list reservation was too optimistic about ttm object reservation
and could think that an object reserved by some other process
as reserved by the list reservation which was false. Thus when
unreserving the list it might unreserve object that it didn't
reserved in the list. Sorry if it's hard to follow but this
kind of things are just causing headheck.

Some LVDS connectors don't have a ddc bus, so reset the
ddc bus to invalid before parsing the next connector
to avoid using stale ddc bus data. Should fix
fdo bug 28164.

This patch is a combination of the previous two profile
patches, but without the index bugs. It cleans up and
fixes some issues with pm profile setup on r6xx chips.
Some tables have different orderings for the power states,
also, r600 only has 1 clock mode per power state. On
desktop cards there are no battery modes, so the low and high
power states are the same. For the low profile case, choose
the lower clock mode.

- Separate dynpm and profile based power management methods. You can select the pm method
by echoing the selected method ("dynpm" or "profile") to power_method in sysfs.
- Expose basic 4 profile in profile method
"default" - default clocks
"auto" - select between low and high based on ac/dc state
"low" - DC, low power mode
"high" - AC, performance mode
The current base profile is "default", but it should switched to "auto" once we've tested
on more systems. Switching the state is a matter of echoing the requested profile to
power_profile in sysfs. The lowest power states are selected automatically when dpms turns
the monitors off in all states but default.
- Remove dynamic fence-based reclocking for the moment. We can revisit this later once we
have basic pm in.
- Move pm init/fini to modesetting path. pm is tightly coupled with display state. Make sure
display side is initialized before pm.
- Add pm suspend/resume functions to make sure pm state is properly reinitialized on resume.
- Remove dynpm module option. It's now selectable via sysfs.

Touching vram while the card is reclocking can lead to lockups. Unmap
any pages that could be touched by the CPU and block any accesses to
vram until the reclocking is complete.

Add two new sysfs attributes:
- dynpm
- power_state
Echoing 0/1 to dynpm disables/enables dynamic power management.
The driver scales the sclk dynamically based on the number of
queued fences. dynpm only scales sclk dynamically in single head
mode.
Echoing x.y to power_state selects a static power state (x) and clock
mode (y). This allows you to statically select a power state and clock
mode. Selecting a static clock mode will disable dynpm.

need to wait for the GUI engine (2D, 3D, CP, etc.) to be
idle before changing clocks or adjusting engine parameters.

The BSD ringbuffer support that is landing in this branch
significantly conflicts with the Ironlake PIPE_CONTROL fix on master,
and requires it to be tested successfully anyway.

Selecting the wrong or no CONFIG_AGP_* chipset can cause a NULL pointer
dereference when combined with CONFIG_DRM_RADEON_KMS and an old system
with a R100 AGP card (should effect other cards too). The agp field
will be set to NULL if no suitable AGP chipset driver is loaded,
drm_agp_acquire already preforms a suitable NULL check so it can be used
directly.

drm/radeon/kms/evergreen: fix LUT setup
Previous reset code leaded to computer hard lockup (need to unplug
the power too reboot the computer) on various configuration. This
patch change the reset code to avoid hard lockup. The GPU reset
is failing most of the time but at least user can log in remotely
or properly shutdown the computer.
Two issues were leading to hard lockup :
- Writting to the scratch register lead to hard lockup most likely
because the write back mecanism is in fuzy state after GPU lockup.
- Resetting the GPU memory controller and not reinitializing it
after leaded to hard lockup. We did only reinitialize in case of
successfull reset thus unsuccessfull reset quickly leaded to hard
lockup.

All TTM driver have been converted to new io_mem_reserve/free
interface which allow driver to choose and return proper io
base, offset to core TTM for ioremapping if necessary. This
patch remove what is now deadcode.

On systems with the tv dac shared between DVI and TV,
we can only use the dac for one of the connectors.
However, when using a digital monitor on the DVI port,
you can use the dac for the TV connector just fine.
Check the use_digital status when resolving the conflict.

On X startup we were getting a flicker where there shouldn't have been one.
the X DDX calls the kernel to set the properties to the same values (yes
it could be smarter), however the kernel was doing a pointless modeset then,
making my nice smooth boot ugly.

There is case where we want to be able to wait only for the
GPU while not waiting for other buffer to be unreserved. This
patch split the no_wait argument all the way down in the whole
ttm path so that upper level can decide on what to wait on or
not.

This patch is against the drm-fbdevfix1 branch. It removes the
drm_fb_helper_setcolreg function. The reason is that fb_setcolreg is only
used in the case where fb_setcmap is called and no fb_ops->fb_setcmap is
used. In the drm case we always need a fb_setcmap hook to handle multiple
crtcs so we don't need a fb_setcolreg hook. Please apply.

a) slow work is always used now for any fbcon hotplug, as its not
a fast task and is more suited to being ran under slow work.
b) attempt to not do any fbdev changes when X is running as we'll
just mess it up. This hooks set_par to hopefully do the changes
once X hands control to fbdev.
This also adds the nouveau/intel hotplug support.

When we are running in a headless environment we have no idea what
output the user might plug in later, we only have hotplug detect
from the digital outputs. So if we detect no connected outputs at
initialisation, start a slow work operation to poll every 5 seconds
for an output.
this is only hooked up for radeon so far, on hw where we have full
hotplug detection there is no need for this.

This breaks the connection between the core drm connector list
and the fbdev connector usage, and allows them to become disjoint
in the future. It also removes the untype void* that was in the
connector struct to support this.
All connectors are added to the fbdev now but this could be
changed in the future.

This move to using the list of crtcs in the fb helper and cleans up the
whole picking code, now we store the crtc/connectors we want directly
into the modeset and we use the modeset directly to set the mode.

The fbdev layer in the kms code should act like a consumer of the kms services and avoid having relying on information being store in the kms core structures in order for it to work.
This patch
a) removes the info pointer/psuedo palette from the core drm_framebuffer structure and moves it to the fbdev helper layer, it also removes the core drm keeping a list of kernel kms fbdevs.
b) migrated all the fb helper functions out of the crtc helper file into the fb helper file.
c) pushed the fb probing/hotplug control into the driver
d) makes the surface sizes into a structure for ease of passing
This changes the intel/radeon/nouveau drivers to use the new helper.

This simplify and improve GPU reset for R1XX-R6XX hw, it's
not 100% reliable here are result:
- R1XX/R2XX works bunch of time in a row, sometimes it
seems it can work indifinitly
- R3XX/R3XX the most unreliable one, sometimes you will be
able to reset few times, sometimes not even once
- R5XX more reliable than previous hw, seems to work most
of the times but once in a while it fails for no obvious
reasons (same status than previous reset just no same
happy ending)
- R6XX/R7XX are lot more reliable with this patch, still
it seems that it can fail after a bunch (reset every
2sec for 3hour bring down the GPU & computer)
This have been tested on various hw, for some odd reasons
i wasn't able to lockup RS480/RS690 (while they use to
love locking up).
Note that on R1XX-R5XX the cursor will disapear after
lockup haven't checked why, switch to console and back
to X will restore cursor.
Next step is to record the bogus command that leaded to
the lockup.

module, use the gpu_lockup boolean to avoid entering
inifinite waiting loop on fence while reiniting the GPU

This patch cleanup the fence code, it drops the timeout field of
fence as the time to complete each IB is unpredictable and shouldn't
be bound.
The fence cleanup lead to GPU lockup detection improvement, this
patch introduce a callback, allowing to do asic specific test for
lockup detection. In this patch the CP is use as a first indicator
of GPU lockup. If CP doesn't make progress during 1second we assume
we are facing a GPU lockup.
To avoid overhead of testing GPU lockup frequently due to fence
taking time to be signaled we query the lockup callback every
500msec. There is plenty code comment explaining the design & choise
inside the code.
This have been tested mostly on R3XX/R5XX hw, in normal running
destkop (compiz firefox, quake3 running) the lockup callback wasn't
call once (1 hour session). Also tested with forcing GPU lockup and
lockup was reported after the 1s CP activity timeout.
V2 switch to 500ms timeout so GPU lockup get call at least 2 times
in less than 2sec.
V3 store last jiffies in fence struct so on ERESTART, EBUSY we keep
track of how long we already wait for a given fence
V4 make sure we got up to date cp read pointer so we don't have
false positive

If we resume in a bad way, we'll get 0xffffffff in wptr, and then
oops with no console. This just adds a sanity check so that we can
avoid the oops and hopefully get more details out of people's systems.

Turn off hw i2c by default except for mm i2c which
is hw only until we sort out the remaining prescale
issues on older chips. hw i2c can be enabled with
hw_i2c=1.

This allows us to remove the internal bit algo bus used by
the radeon i2c algo. We now register a radeon algo adapter
if the gpio line is hw capable and the hw inplementation is
available, otherwise we register a bit algo adapter.

Some systems have LCD* rather than DFP* device tags in the bios
for eDP connectors; notably the new apple iMac. This fixes

- Drop some more safe regs taht userspace shouldn't hit
- Constant base regs need relocs. This allows us to use
constant buffers rather than the constant register file.
Also we don't want userspace to be able to set arbitrary
mc base values for the const caches.
- Track SQ_CONFIG so we know whether userspace is using
the cfile or constant buffers.

When there is allocation failure in radeon_cs_parser_relocs parser->nrelocs
is not cleaned. This causes NULL pointer defeference in radeon_cs_parser_fini
when clean up code is trying to loop over the relocation array and free the
objects.
Fix adds a check for a possible NULL pointer in clean up code.

radeon_gart_fini might call GART unbind callback function which
might try to access GART table but if gart_disable is call first

RS4xx+ IGP chips use an internal gart, however,
some of them have the agp cap bits set in their pci

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

AMD says in section 2.5.4 (GFX MSI Enable) of #43291 (AMD 780G Family
Register Programming Requirements):
The SBIOS must enable internal graphics MSI capability in GCCFG by
setting the following: NBCFG.NB_CNTL.STRAP_MSI_ENABLE='1'
Quite a few BIOS writers misinterpret this sentence and think that
enabling MSI is an optional feature. However, clearing that bit just
prevents delivery of MSI messages but does not remove the MSI PCI
capabilities registers, and so leaves these devices unusable for any
driver that attempts to use MSI.
Setting that bit is not possible after the BIOS has locked down the
configuration registers, so we have to manually disable MSI for the
affected devices.
This fixes the codec communication errors in the HDA driver when
accessing the HDMI audio device, and allows us to get rid of the
overcautious quirk in radeon_irq_kms.c.

We tried to implement interruptible waiting with timeout (it was broken
anyway) which was not a good idea as explained by Andrew. It's possible
to avoid using additional variable but actually it inroduces using more
complex in-kernel tools. So simply add one variable for condition.

R6xx+ cards need to use the 3D engine to blit data which requires
quite a bit of hw state setup. Rather than pull the whole 3D driver
(which normally generates the 3D state) into the DRM, we opt to use
statically generated state tables. The regsiter state and shaders
were hand generated to support blitting functionality. See the 3D
driver or documentation for descriptions of the registers and
shader instructions.

Older GPUs are little different, HDMI blocks are not hard-wired, but routable.
We should just find some free HDMI block and route it to choosen encoder. In
case of RS6x0 there is only one HDMI block, we don't enable HDMI on RS6x00 yet
however.

We almost always used first HDMI block for first encoder and second for sencod.
Exception was KLDSCP_LVTMA. Analyzing code picking DIG encoder shows the same
behaviour. It shows HDMI block are related to DIGs, which relation we now use.

In somecase the atombios code might lead to infinite loop because
the GPU is in broken state, this patch track the jump history and
will abort atombios execution if we are stuck executing the same
jump for more than 1sec. Note that otherwise in some case we might
enter an infinite loop in the kernel context which is bad.

Many new laptops now come with 2 gpus, one to be used for low power
modes and one for gaming/on-ac applications. These GPUs are typically
wired to the laptop panel and VGA ports via a multiplexer unit which
is controlled via ACPI methods.
4 combinations of systems typically exist - with 2 ACPI methods.
Intel/ATI - Lenovo W500/T500 - use ATPX ACPI method
ATI/ATI - some ASUS - use ATPX ACPI Method
Intel/Nvidia - - use _DSM ACPI method
Nvidia/Nvidia - - use _DSM ACPI method.

This patch adds support for the ATPX method and initial bits

This commit "disabled" audio on RV710 and RV740 only, leaving RV770 and RV730.
The order is: CHIP_RV770 < CHIP_RV730 < CHIP_RV710 < CHIP_RV740.
It is not needed anway, as we do not even try to enable audio on RV770 and
newer. We call initializing function in r600.c only, not in rv770.c.
If there is something causing green tinges, it's HDMI mode setting for encoder
and I will try to debug that.

radeon's have a special ability to passthrough writes in their internal
memory space directly to PCI, this ability means that if some of the internal
surfaces like the depth buffer point at 0x0, any writes to these will
go directly to RAM at 0x0 via PCI busmastering.
Now mesa used to always emit clears after emitting state, since the
radeon mesa driver was refactored a year or more ago, it was found it
could generate a clear request without ever sending any setup state to the
card. So the clear would attempt to clear the depth buffer at 0x0, which
would overwrite main memory at this point. fs corruption ensues.
Also once one app did this correctly, it would never get set back to 0
making this messy to reproduce.
The kernel should block this from happening as mesa runs without privs,
though it does require the user be connected to the current running X session.
This patch implements a check to make sure the depth offset has been set
before a depth clear occurs and if it finds one it prints a warning and
ignores the depth clear request. There is also a mesa fix to avoid sending
the badness going into mesa.
This only affects r100/r200 GPUs in user modesetting mode.

This patch adds a check on avivo chips to see if we are in the VBL
region for the active crtcs when we trigger the engine change.
I appear to have glitches locally on pm transistion (not sure all
fixes are in yet) and this at least seems to be correct here,
maybe others can test on systems with no glitches.
Allocation of single large block of memory may fail under memory
presure. drm_buffer object can hold one large block of data in
multiple independ pages which preents alloation failures.
This patch converts all access to command stream to use drm_buffer
interface. All direct access to array has to go tough drm_buffer
functions to get correct pointer.
Outputting the command stream to ring buffer needs to be awear of
the split nature of drm_buffer. The output operation requires the
new OUT_RING_DRM_BUFFER.

Previous code did associate fence to bo before the fence was emited
and it also didn't lock protected access to ttm sync_obj member.
Both of this flaw leads to possible race between different code
path. This patch fix this by associating fence only once the fence
is emitted and properly lock protect access to sync_obj member of
ttm.

There is 3 different distinct states for an indirect buffer (IB) :
1- free with no fence
2- free with a fence
3- non free (fence doesn't matter)
Previous code mixed case 2 & 3 in a single one leading to possible
catastrophique failure. This patch rework the handling and properly
separate each case. So when you get ib we set the ib as non free and
fence status doesn't matter. Fence become active (ie has a meaning
for the ib code) once the ib is scheduled or free. This patch also
get rid of the alloc bitmap as it was overkill, we know go through
IB pool list like in a ring buffer as the oldest IB is the first
one the will be free.

For usec delays use udelay instead of scheduling, this should
allow reclocking to happen faster. This also was the cause
of reported 33s delays at bootup on certain systems.

Get rid of _location and use _start/_end also simplify the
computation of vram_start|end & gtt_start|end. For R1XX-R2XX
we place VRAM at the same address of PCI aperture, those GPU
shouldn't have much memory and seems to behave better when
setup that way. For R3XX and newer we place VRAM at 0. For
R6XX-R7XX AGP we place VRAM before or after AGP aperture this
might limit to limit the VRAM size but it's very unlikely.
For IGP we don't change the VRAM placement.
Tested on (compiz,quake3,suspend/resume):
PCI/PCIE:RV280,R420,RV515,RV570,RV610,RV710
AGP:RV100,RV280,R420,RV350,RV620(RPB*),RV730

r200 cards have dma engine which can be used to tranfer data
between vram and system memory.
r300 dma engine registers match r200 dma engine. Enabling
dma copy for r200 is simple as hooking r200 asic to already
existing function r300_copy_dma.
Rename r300_dma_copy to r200_dma_copyto reflect that supports
starts from r200 cards.

r300_asic_pcie is just copy from r300_asic with gart tlb
functions replaced with pcie versions.

Previous code did associate fence to bo before the fence was emited
and it also didn't lock protected access to ttm sync_obj member.
Both of this flaw leads to possible race between different code
path. This patch fix this by associating fence only once the fence
is emitted and properly lock protect access to sync_obj member of
ttm.

There is 3 different distinct states for an indirect buffer (IB) :
1- free with no fence
2- free with a fence
3- non free (fence doesn't matter)
Previous code mixed case 2 & 3 in a single one leading to possible
catastrophique failure. This patch rework the handling and properly
separate each case. So when you get ib we set the ib as non free and
fence status doesn't matter. Fence become active (ie has a meaning
for the ib code) once the ib is scheduled or free. This patch also
get rid of the alloc bitmap as it was overkill, we know go through
IB pool list like in a ring buffer as the oldest IB is the first
one the will be free.

V2 remove the scheduled list, it's useless now, fix free ib scanning

Noticed on a DEC Alpha.
Start up into console mode caused 15 unaligned accesses, and starting X
caused another 48.

and "shouldn't" in comments"). According to Dave Airlie:
"Can we drop any cleanups to atombios.h? This file comes from
AMD and we normally drop it into place, so they just get overwritten
next time."
