Peter Xu 64019a2e46 mm/gup: remove task_struct pointer for all gup code
After the cleanup of page fault accounting, gup does not need to pass
task_struct around any more.  Remove that parameter in the whole gup
stack.

Vitaly Kuznetsov e8c22266e6 KVM: async_pf: change kvm_setup_async_pf()/kvm_arch_setup_async_pf() return type to bool
Unlike normal 'int' functions returning '0' on success, kvm_setup_async_pf()/
kvm_arch_setup_async_pf() return '1' when a job to handle page fault
asynchronously was scheduled and '0' otherwise. To avoid the confusion
change return type to 'bool'.

No functional change intended.

Vitaly Kuznetsov 2a18b7e7cd KVM: async_pf: Inject 'page ready' event only if 'page not present' was previously injected
'Page not present' event may or may not get injected depending on
guest's state. If the event wasn't injected, there is no need to
inject the corresponding 'page ready' event as the guest may get
confused. E.g. Linux thinks that the corresponding 'page not present'
event wasn't delivered *yet* and allocates a 'dummy entry' for it.
This entry is never freed.

Note, 'wakeup all' events have no corresponding 'page not present'
event and always get injected.

s390 seems to always be able to inject 'page not present', the
change is effectively a nop.

Vitaly Kuznetsov 7863e346e1 KVM: async_pf: Cleanup kvm_setup_async_pf()
schedule_work() returns 'false' only when the work is already on the queue
and this can't happen as kvm_setup_async_pf() always allocates a new one.
Also, to avoid potential race, it makes sense to to schedule_work() at the
very end after we've added it to the queue.

While on it, do some minor cleanup. gfn_to_pfn_async() mentioned in a
comment does not currently exist and, moreover, we can check
kvm_is_error_hva() at the very beginning, before we try to allocate work so
'retry_sync' label can go away completely.

Vitaly Kuznetsov 557a961abb KVM: x86: acknowledgment mechanism for async pf page ready notifications
If two page ready notifications happen back to back the second one is not
delivered and the only mechanism we currently have is
kvm_check_async_pf_completion() check in vcpu_run() loop. The check will
only be performed with the next vmexit when it happens and in some cases
it may take a while. With interrupt based page ready notification delivery
the situation is even worse: unlike exceptions, interrupts are not handled
immediately so we must check if the slot is empty. This is slow and
unnecessary. Introduce dedicated MSR_KVM_ASYNC_PF_ACK MSR to communicate
the fact that the slot is free and host should check its notification
queue. Mandate using it for interrupt based 'page ready' APF event
delivery.

As kvm_check_async_pf_completion() is going away from vcpu_run() we need
a way to communicate the fact that vcpu->async_pf.done queue has
transitioned from empty to non-empty state. Introduce
kvm_arch_async_page_present_queued() and KVM_REQ_APF_READY to do the job.

Vitaly Kuznetsov 7c0ade6c90 KVM: rename kvm_arch_can_inject_async_page_present() to kvm_arch_can_dequeue_async_page_present()
An innocent reader of the following x86 KVM code:

may get very confused: if APF mechanism is not enabled, why do we report
that we 'can inject async page present'? In reality, upon injection
kvm_arch_async_page_present() will check the same condition again and,
in case APF is disabled, will just drop the item. This is fine as the
guest which deliberately disabled APF doesn't expect to get any APF
notifications.

Rename kvm_arch_can_inject_async_page_present() to
kvm_arch_can_dequeue_async_page_present() to make it clear what we are
checking: if the item can be dequeued (meaning either injected or just
dropped).

On s390 kvm_arch_can_inject_async_page_present() always returns 'true' so
the rename doesn't matter much.

Davidlohr Bueso da4ad88cab kvm: Replace vcpu->swait with rcuwait
The use of any sort of waitqueue (simple or regular) for
wait/waking vcpus has always been an overkill and semantically
wrong. Because this is per-vcpu (which is blocked) there is
only ever a single waiting vcpu, thus no need for any sort of
queue.

As such, make use of the rcuwait primitive, with the following
considerations:

  - rcuwait already provides the proper barriers that serialize
  concurrent waiter and waker.

  - Task wakeup is done in rcu read critical region, with a
  stable task pointer.

  - Because there is no concurrency among waiters, we need
  not worry about rcuwait_wait_event() calls corrupting
  the wait->task. As a consequence, this saves the locking
  done in swait when modifying the queue. This also applies
  to per-vcore wait for powerpc kvm-hv.

The x86 tscdeadline_latency test mentioned in 8577370fb0
("KVM: Use simple waitqueue for vcpu->wq") shows that, on avg,
latency is reduced by around 15-20% with this change.

Sean Christopherson 736c291c9f KVM: x86: Use gpa_t for cr2/gpa to fix TDP support on 32-bit KVM
Convert a plethora of parameters and variables in the MMU and page fault
flows from type gva_t to gpa_t to properly handle TDP on 32-bit KVM.

Thanks to PSE and PAE paging, 32-bit kernels can access 64-bit physical
addresses.  When TDP is enabled, the fault address is a guest physical
address and thus can be a 64-bit value, even when both KVM and its guest
are using 32-bit virtual addressing, e.g. VMX's VMCS.GUEST_PHYSICAL is a
64-bit field, not a natural width field.

Using a gva_t for the fault address means KVM will incorrectly drop the
upper 32-bits of the GPA.  Ditto for gva_to_gpa() when it is used to
translate L2 GPAs to L1 GPAs.

Opportunistically rename variables and parameters to better reflect the
dual address modes, e.g. use "cr2_or_gpa" for fault addresses and plain
"addr" instead of "vaddr" when the address may be either a GVA or an L2
GPA.  Similarly, use "gpa" in the nonpaging_page_fault() flows to avoid
a confusing "gpa_t gva" declaration; this also sets the stage for a
future patch to combing nonpaging_page_fault() and tdp_page_fault() with
minimal churn.

Sprinkle in a few comments to document flows where an address is known
to be a GVA and thus can be safely truncated to a 32-bit value.  Add
WARNs in kvm_handle_page_fault() and FNAME(gva_to_gpa_nested)() to help
document such cases and detect bugs.

Ingo Molnar 6e84f31522 sched/headers: Prepare for new header dependencies before moving code to <linux/sched/mm.h>
We are going to split <linux/sched/mm.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.

Create a trivial placeholder <linux/sched/mm.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.

The APIs that are going to be moved first are:

   mm_alloc()
   __mmdrop()
   mmdrop()
   mmdrop_async_fn()
   mmdrop_async()
   mmget_not_zero()
   mmput()
   mmput_async()
   get_task_mm()

Lorenzo Stoakes 8b7457ef9a mm: unexport __get_user_pages_unlocked()
Unexport the low-level __get_user_pages_unlocked() function and replaces
invocations with calls to more appropriate higher-level functions.

In hva_to_pfn_slow() we are able to replace __get_user_pages_unlocked()
with get_user_pages_unlocked() since we can now pass gup_flags.

In async_pf_execute() and process_vm_rw_single_vec() we need to pass
different tsk, mm arguments so get_user_pages_remote() is the sane
replacement in these cases (having added manual acquisition and release
of mmap_sem.)

Additionally get_user_pages_remote() reintroduces use of the FOLL_TOUCH
flag.  However, this flag was originally silently dropped by commit
1e9877902d ("mm/gup: Introduce get_user_pages_remote()"), so this
appears to have been unintentional and reintroducing it is therefore not
an issue.

The reason is that kvm_put_kvm is causing the destruction of the VM, but
the page fault is still on the ->queue list.  The ->queue list is owned
by the VCPU, not by the work items, so we cannot just add list_del to
the work item.

Instead, use work->vcpu to note async page faults that have been resolved
and will be processed through the done list.  There is no need to flush
those.

  user-controllable permission masks in the pte.  So instead of having a
  fixed protection mask in the pte (which needs a system call to change
  and works on a per page basis), the user can map a (handful of)
  protection mask variants and can change the masks runtime relatively
  cheaply, without having to change every single page in the affected
  virtual memory range.

  This allows the dynamic switching of the protection bits of large
  amounts of virtual memory, via user-space instructions.  It also
  allows more precise control of MMU permission bits: for example the
  executable bit is separate from the read bit (see more about that
  below).

  This tree adds the MM infrastructure and low level x86 glue needed for
  that, plus it adds a high level API to make use of protection keys -
  if a user-space application calls:

        mmap(..., PROT_EXEC);

  or

        mprotect(ptr, sz, PROT_EXEC);

  (note PROT_EXEC-only, without PROT_READ/WRITE), the kernel will notice
  this special case, and will set a special protection key on this
  memory range.  It also sets the appropriate bits in the Protection
  Keys User Rights (PKRU) register so that the memory becomes unreadable
  and unwritable.

  So using protection keys the kernel is able to implement 'true'
  PROT_EXEC on x86 CPUs: without protection keys PROT_EXEC implies
  PROT_READ as well.  Unreadable executable mappings have security
  advantages: they cannot be read via information leaks to figure out
  ASLR details, nor can they be scanned for ROP gadgets - and they
  cannot be used by exploits for data purposes either.

  We know about no user-space code that relies on pure PROT_EXEC
  mappings today, but binary loaders could start making use of this new
  feature to map binaries and libraries in a more secure fashion.

  There is other pending pkeys work that offers more high level system
  call APIs to manage protection keys - but those are not part of this
  pull request.

  Right now there's a Kconfig that controls this feature
  (CONFIG_X86_INTEL_MEMORY_PROTECTION_KEYS) that is default enabled
  (like most x86 CPU feature enablement code that has no runtime
  overhead), but it's not user-configurable at the moment.  If there's

Marcelo Tosatti 8577370fb0 KVM: Use simple waitqueue for vcpu->wq
The problem:

On -rt, an emulated LAPIC timer instances has the following path:

1) hard interrupt
2) ksoftirqd is scheduled
3) ksoftirqd wakes up vcpu thread
4) vcpu thread is scheduled

This extra context switch introduces unnecessary latency in the
LAPIC path for a KVM guest.

The solution:

Allow waking up vcpu thread from hardirq context,
thus avoiding the need for ksoftirqd to be scheduled.

Normal waitqueues make use of spinlocks, which on -RT
are sleepable locks. Therefore, waking up a waitqueue
waiter involves locking a sleeping lock, which
is not allowed from hard interrupt context.

cyclictest command line:

This patch reduces the average latency in my tests from 14us to 11us.

Daniel writes:
Paolo asked for numbers from kvm-unit-tests/tscdeadline_latency
benchmark on mainline. The test was run 1000 times on
tip/sched/core 4.4.0-rc8-01134-g0905f04:

  ./x86-run x86/tscdeadline_latency.flat -cpu host

with idle=poll.

The test seems not to deliver really stable numbers though most of
them are smaller. Paolo write:

Dave Hansen 1e9877902d mm/gup: Introduce get_user_pages_remote()
For protection keys, we need to understand whether protections
should be enforced in software or not.  In general, we enforce
protections when working on our own task, but not when on others.
We call these "current" and "remote" operations.

This patch introduces a new get_user_pages() variant:

        get_user_pages_remote()

Which is a replacement for when get_user_pages() is called on
non-current tsk/mm.

We also introduce a new gup flag: FOLL_REMOTE which can be used
for the "__" gup variants to get this new behavior.

The uprobes is_trap_at_addr() location holds mmap_sem and
calls get_user_pages(current->mm) on an instruction address.  This
makes it a pretty unique gup caller.  Being an instruction access
and also really originating from the kernel (vs. the app), I opted
to consider this a 'remote' access where protection keys will not
be enforced.

Without protection keys, this patch should not change any behavior.

Andres Lagar-Cavilla 234b239bea kvm: Faults which trigger IO release the mmap_sem
When KVM handles a tdp fault it uses FOLL_NOWAIT. If the guest memory
has been swapped out or is behind a filemap, this will trigger async
readahead and return immediately. The rationale is that KVM will kick
back the guest with an "async page fault" and allow for some other
guest process to take over.

If async PFs are enabled the fault is retried asap from an async
workqueue. If not, it's retried immediately in the same code path. In
either case the retry will not relinquish the mmap semaphore and will
block on the IO. This is a bad thing, as other mmap semaphore users
now stall as a function of swap or filemap latency.

This patch ensures both the regular and async PF path re-enter the
fault allowing for the mmap semaphore to be relinquished in the case
of IO wait.

async_pf_execute() has no reasons to adopt apf->mm, gup(current, mm)
should work just fine even if current has another or NULL ->mm.

Recently kvm_async_page_present_sync() was added insedie the "use_mm"
section, but it seems that it doesn't need current->mm too.

Oleg Nesterov 41c22f6262 KVM: async_pf: mm->mm_users can not pin apf->mm
get_user_pages(mm) is simply wrong if mm->mm_users == 0 and exit_mmap/etc
was already called (or is in progress), mm->mm_count can only pin mm->pgd
and mm_struct itself.

Change kvm_setup_async_pf/async_pf_execute to inc/dec mm->mm_users.

kvm_create_vm/kvm_destroy_vm play with ->mm_count too but this case looks
fine at first glance, it seems that this ->mm is only used to verify that
current->mm == kvm->mm.

Dominik Dingel e0ead41a6d KVM: async_pf: Provide additional direct page notification
By setting a Kconfig option, the architecture can control when
guest notifications will be presented by the apf backend.
There is the default batch mechanism, working as before, where the vcpu
thread should pull in this information.
Opposite to this, there is now the direct mechanism, that will push the
information to the guest.
This way s390 can use an already existing architecture interface.

Still the vcpu thread should call check_completion to cleanup leftovers.

When we cancel 'async_pf_execute()', we should behave as if the work was
never scheduled in 'kvm_setup_async_pf()'.
Fixes a bug when we can't unload module because the vm wasn't destroyed.

Gleb Natapov 7c90705bf2 KVM: Inject asynchronous page fault into a PV guest if page is swapped out.
Send async page fault to a PV guest if it accesses swapped out memory.
Guest will choose another task to run upon receiving the fault.

Allow async page fault injection only when guest is in user mode since
otherwise guest may be in non-sleepable context and will not be able
to reschedule.

Vcpu will be halted if guest will fault on the same page again or if
vcpu executes kernel code.
