Drivers should not assume that interface descriptors have been parsed in
any particular order so use the interface number to look up the second
alternate setting. That number is also what the driver later use to
switch setting.
Note that although the driver could end up verifying the existence of
the expected endpoints on the wrong interface, a later sanity check in
usb_wwan_port_probe() would have caught this before it could cause any
real damage.

Now that the SPDX tag is in all USB files, that identifies the license
in a specific and legally-defined manner. So the extra GPL text wording
can be removed as it is no longer needed at all.
This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.
No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.
Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

The qcserial driver fails to expose the .tiocmget and .tiocmset methods
available from usb_wwan. These methods are required by ioctl commands
dealing with the modem control signals DTR, RTS, etc.
With these methods not set ioctl calls intended to control the DTR state
will fail. For example, pppd drops and raises DTR in preparation to
dialing the modem, which handles the case of the modem already being
connected by making it hang up and return to command mode. DTR control
being unavailable will lead to a protracted failure to connect as the
modem will be stuck in a state not responsive to command.
I have tested that with this patch the described case is handled
successfully. There is an analogous method for .ioctl available from
usb_wwan (as used in option.c) but I conservatively omitted that for
lack of familiarity.

Lenovo has decided to use new USB device IDs for the wwan modules in
their 2017 laptops. The actual hardware is still the Sierra Wireless
EM7455 or EM7430, depending on region.

The Dell DW5570 is a re-branded Sierra Wireless MC8805 which will by
default boot with vid 0x413c and pid 0x81a3. When triggered QDL download
mode, the device switches to pid 0x81a6 and provides the standard TTY
used for firmware upgrade.

The DEVICE_HWI type was added under the faulty assumption that Huawei
devices based on Qualcomm chipsets and firmware use the static USB
interface numbering known from Gobi devices. But this model does
not apply to Huawei devices like the HP branded lt4112 (Huawei me906e).
Huawei firmwares will dynamically assign interface numbers. Functions
are renumbered when the firmware is reconfigured.
Fix by changing the DEVICE_HWI type to use a simplified version
of Huawei's subclass + protocol scheme: Blacklisting known network
interface combinations and assuming the rest are serial.

Three Sierra Wireless modems have been found to require the CDC ACM
SET_CONTROL_LINE_STATE request on the AT port in order to receive
unsolicited response codes, most recently the Dell Wireless 5808e
(which is a re-branded Sierra Wireless EM7355). On the other hand,
the Sierra Wireless MC7710 does not seem to need this request, but
it was found to work either way.
Use this request on the AT port of devices with the Sierra Wireless
layout in the qcserial driver. The other modems that were moved to
the option driver to work around this can now be moved back.

Tests with a Sierra Wireless MC7355 have shown that 1199:9041 devices
also require the option_send_setup() code to be used on the USB
interface for the AT port to make unsolicited response codes work
correctly. Move these devices from the qcserial driver to the option
driver like it has been done for the 1199:68c0 devices in commit
d80c0d1418 ("USB: qcserial/option: make
AT URCs work for Sierra Wireless MC73xx").

option_send_setup() code to be used on the USB interface for the AT port

the qcserial driver where they have been added by commit
70a3615fc0 ("usb: qcserial: add Sierra Wireless
MC73xx") to the option driver and add a MC73xx-specific blacklist
to ensure that
1. the sendsetup code is not used for the DIAG/DM and NMEA interfaces
2. the option driver does not attach to the QMI/network interfaces

Matching on interface numbers was not such a good idea
for multi-function serial devices after all. It is much
better do create well defined device layouts, allowing
a single match entry per device.

All the "non Gobi" Qualcomm based devices handled by this
driver share a common standard Sierra Wireless specific
layout. Adding code specifically for this layout allow
us to reduce the number of match entries per device from
three to one.
This change will result in a penalty wrt stable backports,
but simplifies new Sierra device addtitions in the long
term.

Fix port-data memory leak in usb-serial probe error path by moving port
data allocation to port_probe.
Since commit a1028f0abf ("usb: usb_wwan: replace release and disconnect
with a port_remove hook") port data is deallocated in port_remove. This
leaves a possibility for memory leaks if usb-serial probe fails after
attach but before the port in question has been successfully registered.
Note that this patch also fixes two additional memory leaks in the error
path of attach should port initialisation fail for any port as the urbs
were never freed and neither was the data of any of the successfully
initialised ports.
Compile-only tested.
