Gavin Shan dbbceee12f powerpc/eeh: Move stats to PE
The patch removes the eeh related statistics for eeh device since
they have been maintained by the corresponding eeh PE. Also, the
flags used to trace the state of eeh device and PE have been reworked
for a little bit.

Gavin Shan 120dc49661 powerpc/eeh: Make EEH handler PE sensitive
Once eeh error is found, eeh event will be created and put it into
the global linked list. At the mean while, kernel thread will be
started to process it. The handler for the kernel thread originally
was eeh device sensitive.

The patch reworks the handler of the kernel thread so that it's PE
sensitive.

Gavin Shan 371a395d2f powerpc/eeh: Make EEH operations based on PE
Originally, all the EEH operations were implemented based on OF node.

Actually, it explicitly breaks the rules that the operation target
is PE instead of device. Therefore, the patch makes all the operations
based on PE instead of device.

Unfortunately, the backend for config space has to be kept as original
because it doesn't depend on PE.

Gavin Shan 66523d9f2d powerpc/eeh: Trace error based on PE from beginning
There're 2 conditions to trigger EEH error detection: invalid value
returned from reading I/O or config space. On each case, the function
eeh_dn_check_failure will be called to initialize EEH event and put
it into the poll for further processing.

The patch changes the function for a little bit so that the EEH error
will be traced based on PE instead of EEH device any more. Also, the
function eeh_find_device_pe() has been removed since the eeh device
is tracing the PE by struct eeh_dev::pe.

Gavin Shan 5b66352944 powerpc/eeh: Trace EEH state based on PE
Since we've introduced dedicated struct to trace individual PEs,
it's reasonable to trace its state through the dedicated struct
instead of using "eeh_dev" any more.

The patches implements the state tracing based on PE. It's notable
that the PE state will be applied to the specified PE as well as
its child PEs. That complies with the rule that problematic parent
PE will prevent those child PEs from working properly.

Gavin Shan 82e8882f7f powerpc/eeh: Remove PE at appropriate time
During PCI hotplug and EEH recovery, the PE hierarchy tree might be
changed due to the PCI topology changes. At later point when the
PCI device is added, the PE will be created dynamically again.

The patch introduces new function to remove EEH devices from the
associated PE. That also can cause that the parent PE is removed
from the PE tree if the parent PE doesn't include valid EEH devices
and child PEs.

Gavin Shan 9b84348c92 powerpc/eeh: Create PEs duing EEH initialization
The patch creates PEs and associated the newly created PEs with
it parent/silbing as well as EEH devices. It would become more
straight to trace EEH errors and recover them accordingly.

Once the EEH functionality on one PCI IOA has been enabled, we
tries to create PE against it. If there's existing PE, to which
the current PCI IOA should be attached, the existing PE will be
converted from "device" type to "bus" type accordingly.

The patch implements searching PE based on the following
requirements:

 * Search PE according to PE address, which is traditional
   PE address that is composed of PCI bus/device/function
   number, or unified PE address assigned by firmware or
   platform.
 * Search parent PE according to the given EEH device. It's
   useful when creating new PE and put it into right position.

Gavin Shan 55037d1761 powerpc/eeh: Create PEs for PHBs
For one particular PE, it's only meaningful in the ancestor PHB
domain. Therefore, each PHB should have its own PE hierarchy tree
to trace those PEs created against the PHB.

The patch creates PEs for the PHBs and put those PEs into the
global link list traced by "eeh_phb_pe". The link list of PEs
would be first level of overall PE hierarchy tree across the
system.

Gavin Shan 968f968f9b powerpc/eeh: Introduce eeh_pe struct
As defined in PAPR 2.4, Partitionable Endpoint (PE) is an I/O subtree
that can be treated as a unit for the purposes of partitioning and error
recovery. Therefore, eeh core should be aware of PE. With eeh_pe struct,
we can support PE explicitly. Further more, it makes all the stuff much
more data centralized. Another important reason is for eeh core to support
multiple platforms. Some of them like pSeries figures out PEs through
OF nodes while others like powernv have to do that through PCI bus/device
tree. With explicit PE support, eeh core will be implemented based on
the centrialized data and platform dependent implementations figure it
out by their feasible ways.

When the struct is designed, following factors are taken in account:
  * Reflecting the relationships of PEs. PE might have parent
    as well children.
  * Reflecting the association of PE and (eeh) devices.
  * PEs have PHB boundary.
  * PE should have unique address assigned in the corresponding
    PHB domain.

Gavin Shan 7e4bbaf0bf powerpc/eeh: Use slab to allocate eeh devices
The EEH initialization functions have been postponed until slab/slub
are ready. So we use slab/slub to allocate the memory chunks for newly
creatd EEH devices. That would save lots of memory.

The patch also does cleanup to replace "kmalloc" with "kzalloc" so
that we needn't clear the allocated memory chunk explicitly.

Gavin Shan 35e5cfe27e powerpc/eeh: Move EEH initialization around
Currently, we have 3 phases for EEH initialization on pSeries platform.
All of them are done through builtin functions: platform initialization,
EEH device creation, and EEH subsystem enablement. All of them are done
no later than ppc_md.setup_arch. That means that the slab/slub isn't ready
yet, so we have to allocate memory chunks on basis of PAGE_SIZE for those
dynamically created EEH devices. That's pretty expensive.

In order to utilize slab/slub for memory allocation, we have to move the EEH
initialization functions around, but all of them should be called after slab
is ready.

Michael Ellerman 407821a34f powerpc: Initialise paca.data_offset with poison
It's possible for the cpu_possible_mask to change between the time we
initialise the pacas and the time we setup per_cpu areas.

Obviously impossible cpus shouldn't ever be running, but stranger things
have happened. So be paranoid and initialise data_offset with a poison
value in case we don't set it up later.

The reason is that mpc512x_shared.c has a couple token #ifdef
on FB_FSL_DIU/FB_FSL_DIU_MODULE, but they don't come close to
masking all the DIU dependencies, as the above fail shows.

Rather than sprinkle more pointless #ifdef in this file, just
remove the existing two, and make FB_FSL_DIU part of the
dependency.  The mpc512x_defconfig already has the line
"CONFIG_FB_FSL_DIU=y" so this change should be zero impact
on real world configs.

Suzuki Poulose 4bc77a5ed2 powerpc: Export memory limit via device tree
The powerpc kernel doesn't export the memory limit enforced by 'mem='
kernel parameter. This is required for building the ELF header in
kexec-tools to limit the vmcore to capture only the used memory. On
powerpc the kexec-tools depends on the device-tree for memory related
information, unlike /proc/iomem on the x86.

Without this information, the kexec-tools assumes the entire System
RAM and vmcore creates an unnecessarily larger dump.

This patch exports the memory limit, if present, via
chosen/linux,memory-limit
property, so that the vmcore can be limited to the memory limit.

The prom_init seems to export this value in the same node. But doesn't
really
appear there.  Also the memory_limit gets adjusted with the processing of
crashkernel= parameter. This patch makes sure we get the actual limit.

The kexec-tools will use the value to limit the 'end' of the memory
regions.

Tested this patch on ppc64 and ppc32(ppc440) with a kexec-tools
patch by Mahesh.

Suzuki Poulose a84fcd4687 powerpc: Change memory_limit from phys_addr_t to unsigned long long
There are some device-tree nodes, whose values are of type phys_addr_t.
The phys_addr_t is variable sized based on the CONFIG_PHSY_T_64BIT.

Change these to a fixed unsigned long long for consistency.

This patch does the change only for memory_limit.

The following is a list of such variables which need the change:

 1) kernel_end, crashk_size - in arch/powerpc/kernel/machine_kexec.c

 2) (struct resource *)crashk_res.start - We could export a local static
    variable from machine_kexec.c.

Changing the above values might break the kexec-tools. So, I will
fix kexec-tools first to handle the different sized values and then change
 the above.

Carl E. Love adbf115d66 powerpc/oprofile: Fix marked events support on Power7+ not set.
Starting with Power 7+ we need to check for marked events if the SIAR
register is valid, i.e. it contains the correct address of the instruction
at the time the performance counter overflowed.  The mmcra register on
Power 7+, contains a new bit to indicate that the contents of the SIAR
is valid. If the event is not marked, then the sample is recorded
independently of the SIAR valid bit setting.  For older processors, there
is no SIAR valid bit to check so the samples are always recorded.  This is
done by forcing the cntr_marked_events bit mask to zero.  The code will
always record the sample in this case since the bit mask says the event is
not a marked event even if it really is a marked event.

Anton Blanchard 752f5216f1 powerpc/pseries: Round up MSI-X requests
The pseries firmware currently refuses any non power of two MSI-X
request. Unfortunately most network drivers end up asking for that
because they want a power of two for RX queues and one or two extra
for everything else.

This patch rounds up the firmware request to the next power of two
if the quota allows it. If this fails we fall back to using the
original request size.

Gavin Shan cf1a4cf875 powerpc/pci: Save P2P bridge resource if possible
When PCI probe flag PCI_REASSIGN_ALL_RSRC has been passed into PCI
core, it's hoped that all resources to be reassigned by PCI core.
As to particular P2P (PCI-to-PCI) bridge, the size of the corresponding
BAR (I/O, MMIO, prefetchable MMIO) is calculated by the resources
required by the PCI devices behind the P2P bridge. That means that
the information like start/end address retrieved from the hardware
registers of the P2P bridge is meainingless in the case. However,
we still count that in and the BARs might have been configured by
firmware with non-zero size. That leads to space waste.

The patch explicitly sets the size of P2P bridge BARs to zero in
case that resource reassignment is expected with PCI probe flag
PCI_REASSIGN_ALL_RSRC. In the result, it will save overall resource
required by the system without waste.

Benjamin Herrenschmidt 636802ef96 powerpc: Don't use __put_user() in patch_instruction
patch_instruction() can be called very early on ppc32, when the kernel
isn't yet running at it's linked address. That can cause the !
is_kernel_addr() test in __put_user() to trip and call might_sleep()
which is very bad at that point during boot.

Paul Mackerras 9fb1b36ca1 powerpc: Make sure IPI handlers see data written by IPI senders
We have been observing hangs, both of KVM guest vcpu tasks and more
generally, where a process that is woken doesn't properly wake up and
continue to run, but instead sticks in TASK_WAKING state.  This
happens because the update of rq->wake_list in ttwu_queue_remote()
is not ordered with the update of ipi_message in
smp_muxed_ipi_message_pass(), and the reading of rq->wake_list in
scheduler_ipi() is not ordered with the reading of ipi_message in
smp_ipi_demux().  Thus it is possible for the IPI receiver not to see
the updated rq->wake_list and therefore conclude that there is nothing
for it to do.

In order to make sure that anything done before smp_send_reschedule()
is ordered before anything done in the resulting call to scheduler_ipi(),
this adds barriers in smp_muxed_message_pass() and smp_ipi_demux().
The barrier in smp_muxed_message_pass() is a full barrier to ensure that
there is a full ordering between the smp_send_reschedule() caller and
scheduler_ipi().  In smp_ipi_demux(), we use xchg() rather than
xchg_local() because xchg() includes release and acquire barriers.
Using xchg() rather than xchg_local() makes sense given that
ipi_message is not just accessed locally.

This moves the barrier between setting the message and calling the
cause_ipi() function into the individual cause_ipi implementations.
Most of them -- those that used outb, out_8 or similar -- already had
a full barrier because out_8 etc. include a sync before the MMIO
store.  This adds an explicit barrier in the two remaining cases.

These changes made no measurable difference to the speed of IPIs as
measured using a simple ping-pong latency test across two CPUs on
different cores of a POWER7 machine.

The analysis of the reason why processes were not waking up properly
is due to Milton Miller.

Anton Blanchard 714332858b powerpc: Restore correct DSCR in context switch
During a context switch we always restore the per thread DSCR value.
If we aren't doing explicit DSCR management
(ie thread.dscr_inherit == 0) and the default DSCR changed while
the process has been sleeping we end up with the wrong value.

Check thread.dscr_inherit and select the default DSCR or per thread
DSCR as required.

This was found with the following test case, when running with
more threads than CPUs (ie forcing context switching):

Anton Blanchard 1021cb268b powerpc: Fix DSCR inheritance in copy_thread()
If the default DSCR is non zero we set thread.dscr_inherit in
copy_thread() meaning the new thread and all its children will ignore
future updates to the default DSCR. This is not intended and is
a change in behaviour that a number of our users have hit.

We just need to inherit thread.dscr and thread.dscr_inherit from
the parent which ends up being much simpler.

Anton Blanchard 00ca0de02f powerpc: Keep thread.dscr and thread.dscr_inherit in sync
When we update the DSCR either via emulation of mtspr(DSCR) or via
a change to dscr_default in sysfs we don't update thread.dscr.
We will eventually update it at context switch time but there is
a period where thread.dscr is incorrect.

If we fork at this point we will copy the old value of thread.dscr
into the child. To avoid this, always keep thread.dscr in sync with
reality.

Paul Mackerras 375f561a41 powerpc/powernv: Always go into nap mode when CPU is offline
The CPU hotplug code for the powernv platform currently only puts
offline CPUs into nap mode if the powersave_nap variable is set.
However, HV-style KVM on this platform requires secondary CPU threads
to be offline and in nap mode.  Since we know nap mode works just
fine on all POWER7 machines, and the only machines that support the
powernv platform are POWER7 machines, this changes the code to
always put offline CPUs into nap mode, regardless of powersave_nap.
Powersave_nap still controls whether or not CPUs go into nap mode
when idle, as before.

Paul Mackerras dabe859ec6 powerpc: Give hypervisor decrementer interrupts their own handler
At the moment the handler for hypervisor decrementer interrupts is
the same as for decrementer interrupts, i.e. timer_interrupt().
This is bogus; if we ever do get a hypervisor decrementer interrupt
it won't have anything to do with the next timer event.  In fact
the only time we get hypervisor decrementer interrupts is when one
is left pending on exit from a KVM guest.

When we get a hypervisor decrementer interrupt we don't need to do
anything special to clear it, since they are edge-triggered on the
transition of HDEC from 0 to -1.  Thus this adds an empty handler
function for them.  We don't need to have them masked when interrupts
are soft-disabled, so we use STD_EXCEPTION_HV instead of
MASKABLE_EXCEPTION_HV.

Mihai Caraman 8b64a9dfb0 powerpc/booke64: Use SPRG0/3 scratch for bolted TLB miss & crit int
Embedded.Hypervisor category defines GSPRG0..3 physical registers for guests.
Avoid SPRG4-7 usage as scratch in host exception handlers, otherwise guest
SPRG4-7 registers will be clobbered.
For bolted TLB miss exception handlers, which is the version currently
supported by KVM, use SPRN_SPRG_GEN_SCRATCH aka SPRG0 instead of
SPRN_SPRG_TLB_SCRATCH aka SPRG6. Keep using TLB PACA slots to fit in one
64-byte cache line.
For critical exception handlers use SPRG3 instead of SPRG7. Provide a routine
to store and restore user-visible SPRGs. This will be subsequently used
to restore VDSO information in SPRG3. Add EX_R13 to paca slots to free up
SPRG3 and change the critical exception epilog to use it.

Mihai Caraman 79b5c8dbaa powerpc/booke64: Eemove mfspr srr1 duplicate in exception prolog
Refactor exception prolog to get rid of mfspr srr1 duplicate. This was
introduced by KVM integration, with DO_KVM macro logic expecting srr1 value
earlier in r11.
Reserve r11 to hold srr1's value also required at the end of the prolog and
free up r10 to serve as spare in addition macros.
For syscalls case this change does not add any performance penalty. For irq
soft-disabled case the change adds a store/load of conditional register value
to/from a paca slot. Paca slots fit in one 64-byte cache line so these
additional operations have little impact on performance.

We have an old FIXME in reg.h which points out that we should standardise
on PVR_foo for our PVR #defines. Currently we use PVR_ on 32-bit and PV_
on 64-bit.

So do that rename and remove the FIXME.

Seeing as we're touching all but one usage of __is_processor(), rename it
to something less ugly and more indicative of what it does, which is
simply to check the PVR version.

In commit f5339277 "powerpc: Remove FW_FEATURE ISERIES from arch code", we
removed the bulk of the iSeries code, but missed a few bits.

Remove the mschunks bits, these were only ever used on iSeries as far as I
know, and are definitely not used anymore.

Make it even clearer that phys_to_abs() is a nop, by making it a macro. We
still have a few users of this, but should clean those up.

Gavin Shan 66a03505a7 KVM: PPC: book3s: fix build error caused by gfn_to_hva_memslot()
The build error was caused by that builtin functions are calling
the functions implemented in modules. This error was introduced by
commit 4d8b81abc4 ("KVM: introduce readonly memslot").

The patch fixes the build error by moving function __gfn_to_hva_memslot()
from kvm_main.c to kvm_host.h and making that "inline" so that the
builtin function (kvmppc_h_enter) can use that.

Sukadev Bhattiprolu 813312110b powerpc/perf: Use pmc_overflow() to detect rolled back events
For certain speculative events on Power7, 'perf stat' reports far higher
event count than 'perf record' for the same event.

As described in following commit, a performance monitor exception is raised
even when the the performance events are rolled back.

perf_event_interrupt() records an event only when an overflow occurs. But
this check for overflow is a simple 'if (val < 0)'.

Because the events are rolled back, this check for overflow fails and the
event is not recorded. perf_event_interrupt() later uses pmc_overflow() to
detect the overflow and resets the counters and the events are lost completely.

To properly detect the overflow of rolled back events, use pmc_overflow()
even when recording events.

To reproduce:

Jiri Kosina 7256a5d2da powerpc: Fix personality handling in ppc64_personality()
Directly comparing current->personality against PER_LINUX32 doesn't work
in cases when any of the personality flags stored in the top three bytes
are used.

Directly forcefully setting personality to PER_LINUX32 or PER_LINUX
discards any flags stored in the top three bytes

Use personality() macro to compare only PER_MASK bytes and make sure that
we are setting only the bits that should be set, instead of overwriting
the whole value.

Checking for device mask to cover the whole IOMMU table is too strict.
IOMMU allocators should handle mask constraint properly for each
allocation.

The patch enables to use old AirPort Extreme cards on PowerMacs with
more than 1GB of memory; without the patch the driver init fails with:

Tiejun Chen 5f630401f9 powerpc/kgdb: Restore current_thread_info properly
For powerpc BooKE and e200, singlestep is handled on the critical/dbg
exception stack. This causes current_thread_info() to fail for kgdb
internal, so previously We work around this issue by copying
the thread_info from the kernel stack before calling kgdb_handle_exception,
and copying it back afterwards.

But actually we don't do this properly. We should backup current_thread_info
then restore that when exit.

Tiejun Chen 572b411cb4 powerpc/kgdb: Do not set kgdb_single_step on ppc
The kgdb_single_step flag has the possibility to indefinitely
hang the system on an SMP system.

The x86 arch have the same problem, and that problem was fixed by
commit 8097551d9ab9b9e3630(kgdb,x86: do not set kgdb_single_step
on x86). This patch does the same behaviors as x86's patch.

Frederic Weisbecker baa36046d0 cputime: Consolidate vtime handling on context switch
The archs that implement virtual cputime accounting all
flush the cputime of a task when it gets descheduled
and sometimes set up some ground initialization for the
next task to account its cputime.

These archs all put their own hooks in their context
switch callbacks and handle the off-case themselves.

Consolidate this by creating a new account_switch_vtime()
callback called in generic code right after a context switch
and that these archs must implement to flush the prev task
cputime and initialize the next task cputime related state.

Paul Mackerras 04f995a544 KVM: PPC: Book3S HV: Fix incorrect branch in H_CEDE code
In handling the H_CEDE hypercall, if this vcpu has already been
prodded (with the H_PROD hypercall, which Linux guests don't in fact
use), we branch to a numeric label '1f'.  Unfortunately there is
another '1:' label before the one that we want to jump to.  This fixes
the problem by using a textual label, 'kvm_cede_prodded'.  It also
changes the label for another longish branch from '2:' to
'kvm_cede_exit' to avoid a possible future problem if code modifications
add another numeric '2:' label in between.

 - x86 can integrate rmap and rmap_pde and remove heuristics in

Alexander Graf ad36cb0d1d powerpc/kvm/book3s_32: Fix MTMSR_EERI macro
Commit b38c77d82e moved the MTMSR_EERI macro from the KVM code to generic
ppc_asm.h code. However, while adding it in the headers for the ppc32 case,
it missed out to remove the former definition in the KVM code.

This patch fixes compilation on server type PPC32 targets with CONFIG_KVM
enabled.

Will Deacon c1d7e01d78 ipc: use Kconfig options for __ARCH_WANT_[COMPAT_]IPC_PARSE_VERSION
Rather than #define the options manually in the architecture code, add
Kconfig options for them and select them there instead.  This also allows
us to select the compat IPC version parsing automatically for platforms
using the old compat IPC interface.

Marek Szyprowski 64ccc9c033 common: dma-mapping: add support for generic dma_mmap_* calls
Commit 9adc5374 ('common: dma-mapping: introduce mmap method') added a
generic method for implementing mmap user call to dma_map_ops structure.

This patch converts ARM and PowerPC architectures (the only providers of
dma_mmap_coherent/dma_mmap_writecombine calls) to use this generic
dma_map_ops based call and adds a generic cross architecture
definition for dma_mmap_attrs, dma_mmap_coherent, dma_mmap_writecombine
functions.

The generic mmap virt_to_page-based fallback implementation is provided for
architectures which don't provide their own implementation for mmap method.

Steven Rostedt bac821a6e3 powerpc/ftrace: Trace function graph entry before updating index
As Colin Cross ported my x86 change to ARM, he also pointed out that
powerpc is also behind in this fix.

The commit 722b3c7469 "ftrace/graph: Trace function entry before
updating index" fixes an issue with function graph tracing for x86,
where if the called entry function decides not to trace interrupts, it
can fail the check if an interrupt comes in just after the
curr_ret_stack is updated.

The solution is to call the entry function first, then update the
curr_ret_stack if the entry function wants to be traced.

(which works much better nowadays, in fact the latter crashes on modern

from the firmware offb by nouveau doesn't work properly (and leads to
unexplained black screens for some users).

Timur Tabi 896c01cb4b powerpc/85xx: p1022ds: fix DIU/LBC switching with NAND enabled
In order for indirect mode on the PIXIS to work properly, both chip selects
need to be set to GPCM mode, otherwise writes to the chip select base
addresses will not actually post to the local bus -- they'll go to the
NAND controller instead.  Therefore, we need to set BR0 and BR1 to GPCM
mode before switching to indirect mode.

Timur Tabi 6269f2584a powerpc/85xx: p1022ds: disable the NAND flash node if video is enabled
The Freescale P1022 has a unique pin muxing "feature" where the DIU video
controller's video signals are muxed with 24 of the local bus address signals.
When the DIU is enabled, the bulk of the local bus is disabled, preventing
access to memory-mapped devices like NAND flash and the pixis FPGA.

Therefore, if the DIU is going to be enabled, then memory-mapped devices on
the localbus, like NAND flash, need to be disabled.

This patch is similar to "powerpc/85xx: p1022ds: disable the NOR flash node
if video is enabled", except that it disables the NAND flash node instead.
This PIXIS node needs to remain enabled because it is used by platform code
to switch into indirect mode.

Claudiu Manoil 163f22dc96 powerpc/85xx: Fix sram_offset parameter type
The sram_offset parameter represents a physical address and should be of
type phys_addr_t. As part of this fix, the extraction of sram_params is
being cleaned-up and fixed.

This patch fixes now the case when the offset value of 0xfff00000 was being
rejected by the driver (returning -EINVAL), although this is a valid offset
value.

Shaohui Xie e1bd5d8bc1 powerpc/85xx: P3041DS - change espi input-clock from 40MHz to 35MHz
Default CoreNet Coherency Bus (CCB) frequency on P3041 is 750MHz, but espi
cannot work at 40MHz with this CCB frequency, so we need to slow down the
clock rate of espi to 35MHz to make it work stable at the CCB frequency.

   - the first of two really major architecture changes - death to open
     intents.

     The former is finally there; it was very long in making, but with
     Miklos getting through really hard and messy final push in
     fs/namei.c, we finally have it.  Unlike his variant, this one
     doesn't introduce struct opendata; what we have instead is
     ->atomic_open() taking preallocated struct file * and passing
     everything via its fields.

     Instead of returning struct file *, it returns -E...  on error, 0
     on success and 1 in "deal with it yourself" case (e.g.  symlink
     found on server, etc.).

     See comments before fs/namei.c:atomic_open().  That made a lot of
     goodies finally possible and quite a few are in that pile:
     ->lookup(), ->d_revalidate() and ->create() do not get struct
     nameidata * anymore; ->lookup() and ->d_revalidate() get lookup
     flags instead, ->create() gets "do we want it exclusive" flag.

     With the introduction of new helper (kern_path_locked()) we are rid
     of all struct nameidata instances outside of fs/namei.c; it's still
     visible in namei.h, but not for long.  Come the next cycle,
     declaration will move either to fs/internal.h or to fs/namei.c
     itself.  [me, miklos, hch]

   - The second major change: behaviour of final fput().  Now we have
     __fput() done without any locks held by caller *and* not from deep
     in call stack.

     That obviously lifts a lot of constraints on the locking in there.
     Moreover, it's legal now to call fput() from atomic contexts (which
     has immediately simplified life for aio.c).  We also don't need
     anti-recursion logics in __scm_destroy() anymore.

     There is a price, though - the damn thing has become partially
     asynchronous.  For fput() from normal process we are guaranteed
     that pending __fput() will be done before the caller returns to
     userland, exits or gets stopped for ptrace.

     For kernel threads and atomic contexts it's done via
     schedule_work(), so theoretically we might need a way to make sure
     it's finished; so far only one such place had been found, but there
     might be more.

     There's flush_delayed_fput() (do all pending __fput()) and there's
     __fput_sync() (fput() analog doing __fput() immediately).  I hope
     we won't need them often; see warnings in fs/file_table.c for
     details.  [me, based on task_work series from Oleg merged last
     cycle]

   - sync series from Jan

   - large part of "death to sync_supers()" work from Artem; the only
     bits missing here are exofs and ext4 ones.  As far as I understand,
     those are going via the exofs and ext4 trees resp.; once they are
     in, we can put ->write_super() to the rest, along with the thread
     calling it.

   - preparatory bits from unionmount series (from dhowells).

   - assorted cleanups and fixes all over the place, as usual.

  This is not the last pile for this cycle; there's at least jlayton's
  ESTALE work and fsfreeze series (the latter - in dire need of fixes,
  so I'm not sure it'll make the cut this cycle).  I'll probably throw
  symlink/hardlink restrictions stuff from Kees into the next pile, too.

Benjamin Herrenschmidt 574ce79cea powerpc/mpic: Create a revmap with enough entries for IPIs and timers
The current mpic code creates a linear revmap just big enough for all
the sources, which happens to miss the IPIs and timers on some machines.

This will in turn break when the irqdomain code loses the fallback of
doing a linear search when the revmap fails (and really slows down IPIs
otherwise).

This happens for example on the U4 based Apple machines such as the
dual core PowerMac G5s.

Takuya Yoshikawa b3ae209697 KVM: Introduce kvm_unmap_hva_range() for kvm_mmu_notifier_invalidate_range_start()
When we tested KVM under memory pressure, with THP enabled on the host,
we noticed that MMU notifier took a long time to invalidate huge pages.

Since the invalidation was done with mmu_lock held, it not only wasted
the CPU but also made the host harder to respond.

This patch mitigates this by using kvm_handle_hva_range().

Takuya Yoshikawa 84504ef386 KVM: MMU: Make kvm_handle_hva() handle range of addresses
When guest's memory is backed by THP pages, MMU notifier needs to call
kvm_unmap_hva(), which in turn leads to kvm_handle_hva(), in a loop to
invalidate a range of pages which constitute one huge page:

  for each page
    for each memslot
      if page is in memslot
        unmap using rmap

This means although every page in that range is expected to be found in
the same memslot, we are forced to check unrelated memslots many times.
If the guest has more memslots, the situation will become worse.

Furthermore, if the range does not include any pages in the guest's
memory, the loop over the pages will just consume extra time.

This patch, together with the following patches, solves this problem by
introducing kvm_handle_hva_range() which makes the loop look like this:

  for each memslot
    for each page in memslot
      unmap using rmap

In this new processing, the actual work is converted to a loop over rmap
which is much more cache friendly than before.

Kleber Sacilotto de Souza 10db8d2128 powerpc/eeh: Check handle_eeh_events() return value
Function eeh_event_handler() dereferences the pointer returned by
handle_eeh_events() without checking, causing a crash if NULL was
returned, which is expected in some situations.

This patch fixes this bug by checking for the value returned by
handle_eeh_events() before dereferencing it.

Jia Hongtao b915341b4b powerpc/85xx: Add phy nodes in SGMII mode for MPC8536/44/72DS & P2020DS
In SGMII riser card different PHY chip are used with different external
IRQ from eTSEC. To support PHY link state auto detect in SGMII mode we
should add another group of PHY nodes for SGMII mode.

For MPC8572DS IRQ6 is used for PHY0~PHY1, IRQ7 is used for PHY2~PHY3.
For MPC8544DS and MPC8536DS IRQ6 is used for PHY0~PHY1.
For P2020DS IRQ5 is used for PHY1~PHY2.

map is available, but the current code ignores it and relies upon the
slow path to insert revmap records.  This patch adds revmap updating

Scott Wood 9653018b61 powerpc/e500: add paravirt QEMU platform
This gives the kernel a paravirtualized machine to target, without
requiring both sides to pretend to be targeting a specific board
that likely has little to do with the host in KVM scenarios.  This
avoids the need to add new boards to QEMU just to be able to
run KVM on new CPUs.

As this is the first platform that can run with either e500v2 or
e500mc, CONFIG_PPC_E500MC is now a legitimately user configurable
option, so add a help text.

Scott Wood 07e4f8014f powerpc/fsl-pci: get PCI init out of board files
As an alternative incremental starting point to Jia Hongtao's patchset,
get the FSL PCI init out of the board files, but do not yet convert to a
platform driver.

Rather than having each board supply a magic register offset for
determining the "primary" bus, we look for which PCI host bridge
contains an ISA node within its subtree.  If there is no ISA node,
normally that would mean there is no primary bus, but until certain
bugs are fixed we arbitrarily designate a primary in this case.

Conversion to a platform driver and related improvements can happen
after this, as the ordering issues are sorted out.

Haren Myneni e1612de9e4 powerpc: Disable /dev/port interface on systems without an ISA bridge
Some power systems do not have legacy ISA devices. So, /dev/port is not
a valid interface on these systems. User level tools such as kbdrate is
trying to access the device using this interface which is causing the
system crash.

This patch will fix this issue by not creating this interface on these
powerpc systems.

Anton Blanchard 18ad51dd34 powerpc: Add VDSO version of getcpu
We have a request for a fast method of getting CPU and NUMA node IDs
from userspace. This patch implements a getcpu VDSO function,
similar to x86.

Ben suggested we use SPRG3 which is userspace readable. SPRG3 can be
modified by a KVM guest, so we save the SPRG3 value in the paca and
restore it when transitioning from the guest to the host.

I have a glibc patch that implements sched_getcpu on top of this.
Testing on a POWER7:

baseline: 538 cycles
vdso:      30 cycles

Deepthi Dharwar 852d8cb1ff powerpc/cpuidle: Fixes for pseries_idle hotplug notifier
Currently the call to pseries_notify_cpuidle_add_cpu(), that takes
action on the cpuidle front when a cpu is added/removed
is being made from smp_xics_setup_cpu().
This caused lockdep issues as

On addition of each cpu,
resources were cleared and re-allocated each time, all in critical
section as part of start_secondary() call were interrupts are disabled.
To resolve this issue, the pseries_notify_cpuidle_add_cpu() call is
is being replaced by a hotplug notifier which
would prevent cpuidle resources from being
released and allocated each time cpu is onlined in the critical code path.
It was fixed in https://lkml.org/lkml/2012/5/18/174.

Also it is essential to call cpuidle_enable/disable_device
between  cpuidle_pause_and_lock()  and
cpuidle_resume_and_unlock()  when used externally
to avoid race conditions. Add support for CPU_ONLINE_FROZEN
and CPU_DEAD_FROZEN as part of hotplug notify event for
pseries_idle  and unregister hotplug notifier
while exiting out. The above mentioned issues
are fixed as part of this patch.

Benjamin Herrenschmidt 8bf8385b9c powerpc: Fixup oddity in entry_32.S
When I "fixed" the CONFIG_TRACE_IRQFLAGS case on interrupt entry,
I screwed up a little bit with the test for user space vs. kernel.

The code is fine, there's just some dead code around it. I basically
removed the test and always create the added stack frame whether
coming from user or kernel since in any case we do need to save
a bunch of volatile registers or bad things would happen (we can
take page faults in the kernel for example).

Shawn Guo 6d4ae2ddfe powerpc: select PPC_CLOCK unconditionally for FSL_SOC
Freescale PowerPC SoCs share a number of IP blocks with Freescale
ARM/IMX SoCs, FlexCAN, SSI, FEC, eSDHC, USB, etc.  There are some
effort consolidating those drivers to make them work for both
architectures.

One outstanding difference between two architectures is ARM/IMX will
turn off module clocks during platform initialization for power saving
and expects drivers manage clocks using clk API, while PowerPC
mostly does not do that, and thus does not always build in clk API.

Listing all those driver Kconfig options in "select PPC_CLOCK if" seems
not scalable for long term maintenance, and could easily introduce
Kconfig recursive dependency.  This patch chooses to select PPC_CLOCK
unconditionally for FSL_SOC to always build clk API for PowerPC in.

Timur Tabi 6bd825f029 powerpc/85xx: use the BRx registers to enable indirect mode on the P1022DS
In order to enable the DIU video controller on the P1022DS, the FPGA needs
to be switched to "indirect mode", where the localbus is disabled and
the FPGA is accessed via writes to localbus chip select signals CS0 and CS1.

To obtain the address of CS0 and CS1, the platform driver uses an "indirect
pixis mode" device tree node.  This node assumes that the localbus 'ranges'
property is sorted in chip-select order.  That is, reg value 0 maps to
CS0, reg value 1 maps to CS1, etc.  This is how the 'ranges' property is
supposed to be arranged.

Unfortunately, the 'ranges' property is often mis-arranged, and not just on
the P1022DS.  Linux normally does not care, since it does not program the
localbus.  But the indirect-mode code on the P1022DS does care.

The "proper" fix is to have U-Boot fix the 'ranges' property, but this would
be too cumbersome.  The names and 'reg' properties of all the localbus
devices would also need to be updated, and determining which localbus device
maps to which chip select is board-specific.

Instead, we determine the CS0/CS1 base addresses the same way that U-boot
does -- by reading the BRx registers directly and mapping them to physical
addresses.  This code is simpler and more reliable, and it does not require
a U-boot or device tree change.

Since the indirect pixis device tree node is no longer needed, the node is
deleted from the DTS.

Paul Gortmaker b048b4e17c powerpc: remove Wind River SBC8560 support
This reference board dates back to 2004, and is largely a legacy
EOL product.  The MPC8560 is a pre e500v2 CPU.  The SBC8548 is
a more modern, better e500v2 target for people to use as a
reference board with today's kernels, should they require one.

Removing support for it will also allow us to remove some
sbc8560 specific quirk handling in 8250 UART code, and some
MTD mapping support.
