The fst_open() function, after a successful try_module_get() may return
an error code if hdlc_open() returns it. However, it does not put the
module on this error path.
This patch adds the necessary module_put() call.
Found by Linux Driver Verification project (linuxtesting.org).

This reverts commit b1c43f82c5.
It was broken in so many ways, and results in random odd pty issues.
It re-introduced the buggy schedule_work() in flush_to_ldisc() that can
cause endless work-loops (see commit a5660b41af6a: "tty: fix endless
work loop when the buffer fills up").
It also used an "unsigned int" return value fo the ->receive_buf()
function, but then made multiple functions return a negative error code,
and didn't actually check for the error in the caller.
And it didn't actually work at all. BenH bisected down odd tty behavior
to it:
"It looks like the patch is causing some major malfunctions of the X
server for me, possibly related to PTYs. For example, cat'ing a
large file in a gnome terminal hangs the kernel for -minutes- in a
loop of what looks like flush_to_ldisc/workqueue code, (some ftrace
data in the quoted bits further down).
...
Some more data: It -looks- like what happens is that the
flush_to_ldisc work queue entry constantly re-queues itself (because
the PTY is full ?) and the workqueue thread will basically loop
forver calling it without ever scheduling, thus starving the consumer
process that could have emptied the PTY."
which is pretty much exactly the problem we fixed in a5660b41af.

Force dev_alloc_name() to be called from register_netdevice() by
dev_get_valid_name(). That allows to remove multiple explicit
dev_alloc_name() calls.

it makes it simpler to keep track of the amount of
bytes received and simplifies how flush_to_ldisc counts
the remaining bytes. It also fixes a bug of lost bytes
on n_tty when flushing too many bytes via the USB
serial gadget driver.

We don't actually need this and it causes problems for internal use of
this functionality. Currently there is a single use of the FILE * pointer.
That is the serial core which uses it to check tty_hung_up_p. However if
that is true then IO_ERROR is also already set so the check may be removed.

We must not wake the TX queue without free TX descriptors.
sca_xmit() expects at least one free descriptor and BUGs otherwise.

We register lapb when tty is created, but unregister it only when the
device is UP. So move the lapb_unregister to x25_asy_close_tty after
the device is down.
The old behaviour causes ldisc switching to fail each second attempt,
because we noted for us that the device is unused, so we use it the
second time, but labp layer still have it registered, so it fails
obviously.

The "skb" and "mem" arguments being passed here are DMA addresses
being programmed into the hardware registers, so pass them as the type

All these files use the big kernel lock in a trivial
way to serialize their private file operations,
typically resulting from an earlier semi-automatic
pushdown from VFS.
None of these drivers appears to want to lock against
other code, and they all use the BKL as the top-level
lock in their file operations, meaning that there
is no lock-order inversion problem.
Consequently, we can remove the BKL completely,
replacing it with a per-file mutex in every case.
Using a scripted approach means we can avoid
typos.

With port configured with PCR_FRM_SYNC_OUTPUT* and external clock, bringing
the hdlcX interface up and down without active clock supplied to the HSS
causes a TX lockup. We don't support channelized/partial interfaces so
FRaMe signals can't be used anyway, disabling them makes the lockup go away.
Changes to this logic will be required if we want to support channelized
HSS mode (this is most probably bug in NPE-A HSS firmware).

Now that core network takes care of trans_start updates, dont do it
in drivers themselves, if possible. Drivers can avoid one cache miss
(on dev->trans_start) in their start_xmit() handler.

tx_queue is used as a temporary queue when not allowed to queue skb
directly to the hw device driver (which may sleep). Most paths flush
it before returning, but ppp_start() currently cannot. Make sure we
don't leave skbs pointing to a non-existent device.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Slip and a few other drivers use the same ioctl numbers on
tty devices that are normally meant for sockets. This causes
problems with our compat_ioctl handling that tries to convert
the data structures in a different format.
Fortunately, these five drivers all use 32 bit compatible
data structures in the ioctl numbers, so we can just add
a trivial compat_ioctl conversion function to each of them.
SIOCSIFENCAP and SIOCGIFENCAP do not need to live in
fs/compat_ioctl.c after this any more, and they are not
used on any sockets.

Cisco HDLC uses keepalive packets and sequence numbers to determine link
state. In rare cases both ends could transmit keepalive packets at the same
time, causing the received sequence numbers to be treated as incorrect.
Now we accept our current sequence number as well as the previous one.

HSS usually uses external clocks, so it's not a big deal. Internal clock
is used for direct DTE-DTE connections and when the DCE doesn't provide
it's own clock.
This also depends on the oscillator frequency. Intel seems to have
calculated the clock register settings for 33.33 MHz (66.66 MHz timer
base). Their settings seem quite suboptimal both in terms of average
frequency (60 ppm is unacceptable for G.703 applications, their primary
intended usage(?)) and jitter.
Many (most?) platforms use a 33.333 MHz oscillator, a 10 ppm difference
from Intel's base.
Instead of creating static tables, I've created a procedure to program
the HSS clock register. The register consists of 3 parts (A, B, C).
The average frequency (= bit rate) is:
66.66x MHz / (A + (B + 1) / (C + 1))
The procedure aims at the closest average frequency, possibly at the
cost of increased jitter. Nobody would be able to directly drive an
unbufferred transmitter with a HSS anyway, and the frequency error is
what it really counts.
I've verified the above with an oscilloscope on IXP425. It seems IXP46x
and possibly IXP43x use a bit different clock generation algorithm - it
looks like the avg frequency is:
(on IXP465) 66.66x MHz / (A + B / (C + 1)).
Also they use much greater precomputed A and B - on IXP425 it would
simply result in more jitter, but I don't know how does it work on
IXP46x (perhaps 3 least significant bits aren't used?).
Anyway it looks that they were aiming for exactly +60 ppm or -60 ppm,
while <1 ppm is typically possible (with a synchronized clock, of
course).
The attached patch makes it possible to set almost any bit rate
(my IXP425 533 MHz quits at > 22 Mb/s if a single port is used, and the
minimum is ca. 65 Kb/s).
This is independent of MVIP (multi-E1/T1 on one HSS) mode.

PCI200SYN has its own PCI subsystem device ID for 3+ years, now it's
time to remove the generic PLX905[02] ID from the driver. Anyone with
old EEPROM data will have to run the upgrade.
Having the generic PLX905[02] (PCI-local bus bridge) ID is harmful
as the driver tries to handle other devices based on these bridges.

This is a brute force removal of the wierd slave interface done for
DLCI -> SDLA transmit. Before it was using non-standard return values
and freeing skb in caller. This changes it to using normal return
values, and freeing in the callee. Luckly only one driver pair was
doing this. Not tested on real hardware, in fact I wonder if this
driver pair is even being used by any users.

lines with multiple KERN_<level> prefixes are no longer emitted as
before the patch.
<level> is now included in the output on each additional use.

This patch converts the remaining occurences of raw return values to their
symbolic counterparts in ndo_start_xmit() functions that were missed by the
previous automatic conversion.
Additionally code that assumed the symbolic value of NETDEV_TX_OK to be zero
is changed to explicitly use NETDEV_TX_OK.

One point of contention in high network loads is the dst_release() performed
when a transmited skb is freed. This is because NIC tx completion calls
dev_kree_skb() long after original call to dev_queue_xmit(skb).
CPU cache is cold and the atomic op in dst_release() stalls. On SMP, this is
quite visible if one CPU is 100% handling softirqs for a network device,
since dst_clone() is done by other cpus, involving cache line ping pongs.
It seems right place to release dst is in dev_hard_start_xmit(), for most
devices but ones that are virtual, and some exceptions.
David Miller suggested to define a new device flag, set in alloc_netdev_mq()
(so that most devices set it at init time), and carefuly unset in devices
which dont want a NULL skb->dst in their ndo_start_xmit().
List of devices that must clear this flag is :
- loopback device, because it calls netif_rx() and quoting Patrick :
"ip_route_input() doesn't accept loopback addresses, so loopback packets
already need to have a dst_entry attached."
- appletalk/ipddp.c : needs skb->dst in its xmit function
- And all devices that call again dev_queue_xmit() from their xmit function
(as some classifiers need skb->dst) : bonding, vlan, macvlan, eql, ifb, hdlc_fr

Most fasync implementations do something like:
return fasync_helper(...);
But fasync_helper() will return a positive value at times - a feature used
in at least one place. Thus, a number of other drivers do:
err = fasync_helper(...);
if (err < 0)
return err;
return 0;
In the interests of consistency and more concise code, it makes sense to
map positive return values onto zero where ->fasync() is called.

When the napi api was changed to separate its 1:1 binding to the net_device

We have some reasons to kill netdev->priv:
1. netdev->priv is equal to netdev_priv().
2. netdev_priv() wraps the calculation of netdev->priv's offset, obviously
netdev_priv() is more flexible than netdev->priv.
But we cann't kill netdev->priv, because so many drivers reference to it
directly.
This patch is a safe convert for netdev->priv to netdev_priv(netdev).
Since all of the netdev->priv is only for read.
But it is too big to be sent in one mail.
I split it to 4 parts and make every part smaller than 100,000 bytes,
which is max size allowed by vger.

The generic packet receive code takes care of setting
netdev->last_rx when necessary, for the sake of the
bonding ARP monitor.
Drivers need not do it any more.
Some cases had to be skipped over because the drivers
were making use of the ->last_rx value themselves.

Use the newly introduced pci_ioremap_bar() function in drivers/net.
pci_ioremap_bar() just takes a pci device and a bar number, with the goal
of making it really hard to get wrong, while also having a central place
to stick sanity checks.

This merges branches irq/genirq, irq/sparseirq-v4, timers/hpet-percpu
and x86/uv.
The sparseirq branch is just preliminary groundwork: no sparse IRQs are
actually implemented by this tree anymore - just the new APIs are added
while keeping the old way intact as well (the new APIs map 1:1 to
irq_desc[]). The 'real' sparse IRQ support will then be a relatively
small patch ontop of this - with a v2.6.29 merge target.

Clean up the various different email addresses of mine listed in the code
to a single current and valid address. As Dave says his network merges
for 2.6.28 are now done this seems a good point to send them in where
they won't risk disrupting real changes.

Move the line disciplines towards a conventional ->ops arrangement. For
the moment the actual 'tty_ldisc' struct in the tty is kept as part of
the tty struct but this can then be changed if it turns out that when it
all settles down we want to refcount ldiscs separately to the tty.
Pull the ldisc code out of /proc and put it with our ldisc code.

Commit 4c13eb6657 ([ETH]: Make
eth_type_trans set skb->dev like the other *_type_trans) removed
skb->dev assignment from hdlc_fr.c:fr_rx(). Unfortunately it was also
needed for cases other than eth_type_trans().
Adding it back.
It's quite serious and may be a security risk as it causes a wrong
input interface indication (the physical hdlcX instead of logical
pvcX). Probably -stable class fix.

The syncppp layer wants a mid-level netdev private pointer.
It was using netdev->priv but that only worked by accident,
and thus this scheme was broken when the device private
allocation strategy changed.
Add a proper mid-layer private pointer for uses like this,
update syncppp and all users, and remove the HDLC_PPP broken
tag from drivers/net/wan/Kconfig

The local variable "prefix" is never used anymore, and the content of
this string appears a bit later, directly in a call to "alloc_netdev"
after doing exactly the same if/else test. So there seems to be no
point keeping those 4 lines anymore.

PPP support in generic HDLC in Linux 2.6.25 is broken and will cause
a kernel panic when a device configured in PPP mode is activated.
It will be replaced by new PPP implementation after Linux 2.6.25 is
released.
This affects only PPP support in generic HDLC (mostly Hitachi SCA
and SCA-II based drivers, wanxl, and few others). Standalone syncppp
and async PPP support are not affected.

We conciously make a change here - we permit mode and speed setting to
be done in things like SLIP mode. There isn't actually a technical
reason to disallow this. It's usually a silly thing to do but we can
do it and soemone might wish to do so.
