Linus Torvalds 90489a72fb Merge branch 'perf-core-for-linus' of git://git.kernel.org/pub/scm/linux/kernel/git/tip/tip
Pull perf updates from Ingo Molnar:
 "The main kernel changes were:

   - add support for Intel's "adaptive PEBS v4" - which embedds LBS data
in PEBS records and can thus batch up and reduce the IRQ (NMI) rate
significantly - reducing overhead and making call-graph profiling
     less intrusive.

   - add Intel CPU core and uncore support updates for Tremont, Icelake,

   - extend the x86 PMU constraints scheduler with 'constraint ranges'
to better support Icelake hw constraints,

   - make x86 call-chain support work better with CONFIG_FRAME_POINTER=y

   - misc other changes

  Tooling changes:

   - updates to the main tools: 'perf record', 'perf trace', 'perf
     stat'

   - updated Intel and S/390 vendor events

   - libtraceevent updates

   - misc other updates and fixes"

Alexey Budankov 470530bbb8 perf record: Implement --mmap-flush=<number> option
Implement a --mmap-flush option that specifies minimal number of bytes
that is extracted from mmaped kernel buffer to store into a trace. The
default option value is 1 byte what means every time trace writing
thread finds some new data in the mmaped buffer the data is extracted,
possibly compressed and written to a trace.

  $ tools/perf/perf record --mmap-flush 1024 -e cycles -- matrix.gcc
  $ tools/perf/perf record --aio --mmap-flush 1K -e cycles -- matrix.gcc

The option is independent from -z setting, doesn't vary with compression
level and can serve two purposes.

The first purpose is to increase the compression ratio of a trace data.
Larger data chunks are compressed more effectively so the implemented
option allows specifying data chunk size to compress. Also at some cases
executing more write syscalls with smaller data size can take longer
than executing less write syscalls with bigger data size due to syscall
overhead so extracting bigger data chunks specified by the option value
could additionally decrease runtime overhead.

The second purpose is to avoid self monitoring live-lock issue in system
wide (-a) profiling mode. Profiling in system wide mode with compression
(-a -z) can additionally induce data into the kernel buffers along with
the data from monitored processes. If performance data rate and volume
from the monitored processes is high then trace streaming and
compression activity in the tool is also high. High tool process
activity can lead to subtle live-lock effect when compression of single
new byte from some of mmaped kernel buffer leads to generation of the
next single byte at some mmaped buffer. So perf tool process ends up in
endless self monitoring.

Implemented synch parameter is the mean to force data move independently
from the specified flush threshold value. Despite the provided flush
value the tool needs capability to unconditionally drain memory buffers,
at least in the end of the collection.

Committer testing:

Running with the default value, i.e. as soon as there is something to
read go on consuming, we first write the synthesized events, small
chunks of about 128 bytes:

  # perf trace -m 2048 --call-graph dwarf -e write -- perf record
  <SNIP>
     101.142 ( 0.004 ms): perf/25821 write(fd: 3</root/perf.data>, buf: 0x210db60, count: 120) = 120
                                         __libc_write (/usr/lib64/libpthread-2.28.so)
                                         ion (/home/acme/bin/perf)
                                         record__write (inlined)
                                         process_synthesized_event (/home/acme/bin/perf)
                                         perf_tool__process_synth_event (inlined)
                                         perf_event__synthesize_mmap_events (/home/acme/bin/perf)

Then we move to reading the mmap buffers consuming the events put there
by the kernel perf infrastructure:

     107.561 ( 0.005 ms): perf/25821 write(fd: 3</root/perf.data>, buf: 0x7f1befc02000, count: 336) = 336

                                         cmd_record (/home/acme/bin/perf)
     12919.953 ( 0.136 ms): perf/25821 write(fd: 3</root/perf.data>, buf: 0x7f1befc83150, count: 184984) = 184984
<SNIP same backtrace as in the 107.561 timestamp>
     12920.094 ( 0.155 ms): perf/25821 write(fd: 3</root/perf.data>, buf: 0x7f1befc02150, count: 261816) = 261816

<SNIP same backtrace as in the 107.561 timestamp>

If we limit it to write only when more than 16MB are available for
reading, it throttles that to a quarter of the --mmap-pages set for
'perf record', which by default get to 528384 bytes, found out using
'record -v':

  mmap flush: 132096
  mmap size 528384B

With that in place all the writes coming from
record__mmap_read_evlist(), i.e. from the mmap buffers setup by the
kernel perf infrastructure were at least 132096 bytes long.

Trying with a bigger mmap size:

   perf trace -e write perf record -v -m 2048 --mmap-flush 16M
   74982.928 ( 2.471 ms): perf/26500 write(fd: 3</root/perf.data>, buf: 0x7ff94a6cc000, count: 3580888) = 3580888
   74985.406 ( 2.353 ms): perf/26500 write(fd: 3</root/perf.data>, buf: 0x7ff949ecb000, count: 3453256) = 3453256
   74987.764 ( 2.629 ms): perf/26500 write(fd: 3</root/perf.data>, buf: 0x7ff9496ca000, count: 3859232) = 3859232
   74990.399 ( 2.341 ms): perf/26500 write(fd: 3</root/perf.data>, buf: 0x7ff948ec9000, count: 3769032) = 3769032
   74992.744 ( 2.064 ms): perf/26500 write(fd: 3</root/perf.data>, buf: 0x7ff9486c8000, count: 3310520) = 3310520
   74994.814 ( 2.619 ms): perf/26500 write(fd: 3</root/perf.data>, buf: 0x7ff947ec7000, count: 4194688) = 4194688
   74997.439 ( 2.787 ms): perf/26500 write(fd: 3</root/perf.data>, buf: 0x7ff9476c6000, count: 4029760) = 4029760

Was again limited to a quarter of the mmap size:

  mmap flush: 2098176
  mmap size 8392704B

A warning about that would be good to have but can be added later,
something like:

"max flush is a quarter of the mmap size, if wanting to bump the mmap
flush further, bump the mmap size as well using -m/--mmap-pages"

Also rename the 'sync' parameters to 'synch' to keep tools/perf building
with older glibcs:

cc1: warnings being treated as errors
  builtin-record.c: In function 'record__mmap_read_evlist':
builtin-record.c:775: warning: declaration of 'sync' shadows a global declaration
/usr/include/unistd.h:933: warning: shadowed declaration is here

Jiri Olsa 4e8a5c1551 perf evsel: Fix max perf_event_attr.precise_ip detection
After a discussion with Andi, move the perf_event_attr.precise_ip
detection for maximum precise config (via :P modifier or for default
cycles event) to perf_evsel__open().

The current detection in perf_event_attr__set_max_precise_ip() is
tricky, because precise_ip config is specific for given event and it
currently checks only hw cycles.

We now check for valid precise_ip value right after failing
sys_perf_event_open() for specific event, before any of the
perf_event_attr fallback code gets executed.

This way we get the proper config in perf_event_attr together with
allowed precise_ip settings.

We can see that code activity with -vv, like:

Song Liu 657ee55319 perf evlist: Introduce side band thread
This patch introduces side band thread that captures extended
information for events like PERF_RECORD_BPF_EVENT.

This new thread uses its own evlist that uses ring buffer with very low
watermark for lower latency.

To use side band thread, we need to:

1. add side band event(s) by calling perf_evlist__add_sb_event();
2. calls perf_evlist__start_sb_thread();
3. at the end of perf run, perf_evlist__stop_sb_thread().

In the next patch, we use this thread to handle PERF_RECORD_BPF_EVENT.

Committer notes:

Add fix by Jiri Olsa for when te sb_tread can't get started and then at
the end the stop_sb_thread() segfaults when joining the (non-existing)
thread.

That can happen when running 'perf top' or 'perf record' as a normal
user, for instance.

Further checks need to be done on top of this to more graciously handle
these possible failure scenarios.

Jin Yao 147c508f30 perf tools: Use target->per_thread and target->system_wide flags
Mathieu Poirier reports issue in commit ("73c0ca1eee3d perf thread_map:
Enumerate all threads from /proc") that it has negative impact on 'perf
record --per-thread'. It has the effect of creating a kernel event for
each thread in the system for 'perf record --per-thread'.

Mathieu Poirier's patch ("perf util: Do not reuse target->per_thread flag")
can fix this issue by creating a new target->all_threads flag.

This patch is based on Mathieu Poirier's patch but it doesn't use a new
target->all_threads flag. This patch just uses 'target->per_thread &&
target->system_wide' as a condition to check for all threads case.

Arnaldo Carvalho de Melo 640d5175a6 perf evlist: Set the correct idx when adding dummy events
The evsel->idx field is used mainly to access the right bucket in
per-event arrays such as the annotation ones, but also to set
evsel->tracking, that in turn will decide what of the events will ask
for PERF_RECORD_{MMAP,COMM,EXEC} to be generated, i.e. which
perf_event_attr will have its mmap, etc fields set.

When we were adding the "dummy" event using perf_evlist__add_dummy() we
were not setting it correctly, which could result in multiple tracking
events.

Now that I'll try using a dummy event to be the tracking one when using
'perf record --delay', i.e. when we process the --delay
setting we may already have the evlist set up, like with:

  perf record -e cycles,instructions --delay 1000 ./workload

We will need to add a "dummy" event, then reset evsel->tracking for the
first event, "cycles", and set it instead to the dummy one, and also
setting its attr.enable_on_exec, so that we get the PERF_RECORD_MMAP,
etc metadata events while waiting to enable the explicitely requested
events, so lets get this straight and set the right evsel->idx.

The event is allocated with cpus == 1, but freed with cpus == real number
When the evsel close function walks the file descriptors it exceeds the
fd xyarray boundaries and reads random memory.

v2:

Now that xyarrays save their original dimensions we can use these to
iterate the two dimensional fd arrays. Fix some users (close, ioctl) in
evsel.c to use these fields directly. This allows simplifying the code
and dropping quite a few function arguments. Adjust all callers by
removing the unneeded arguments.

The actual perf event reading still uses the original values from the
evsel list.

Mark Rutland 9f21b815be perf evlist: Only open events on CPUs an evsel permits
In systems with heterogeneous CPU PMUs, it's possible for each evsel to
cover a distinct set of CPUs, and hence the cpu_map associated with each
evsel may have a distinct idx<->id mapping. Any of these may be distinct
from the evlist's cpu map.

Events can be tied to the same fd so long as they use the same per-cpu
ringbuffer (i.e. so long as they are on the same CPU). To acquire the
correct FDs, we must compare the Linux logical IDs rather than the evsel
or evlist indices.

This path adds logic to perf_evlist__mmap_per_evsel to handle this,
translating IDs as required. As PMUs may cover a subset of CPUs from the
evlist, we skip the CPUs a PMU cannot handle.

Without this patch, perf record may try to mmap erroneous FDs on
heterogeneous systems, and will bail out early rather than running the
workload.

Wang Nan 54cc54decd perf evlist: Setup backward mmap state machine
Introduce a bkw_mmap_state state machine to evlist:

                     .________________(forbid)_____________.
                     | V
 NOTREADY --(0)--> RUNNING --(1)--> DATA_PENDING --(2)--> EMPTY
                     ^ ^ | ^ |
                     | |__(forbid)____/ |___(forbid)___/|
                     | |
                      \_________________(3)_______________/

NOTREADY : Backward ring buffers are not ready
RUNNING : Backward ring buffers are recording
DATA_PENDING : We are required to collect data from backward ring buffers
EMPTY : We have collected data from backward ring buffers.

 (0): Setup backward ring buffer
(1): Pause ring buffers for reading
 (2): Read from ring buffers
(3): Resume ring buffers for recording

We can't avoid this complexity. Since we deliberately drop records from
overwritable ring buffer, there's no way for us to check remaining from
ring buffer itself (by checking head and old pointers). Therefore, we
need DATA_PENDING and EMPTY state to help us recording what we have done
to the ring buffer.

In record__mmap_read_evlist(), drive this state machine from DATA_PENDING
to EMPTY.

In perf_evlist__mmap_per_evsel(), drive this state machine from NOTREADY
to RUNNING when creating backward mmap.

Currently, the evlist mmap related helpers and APIs accept evlist and

Following commits are going to introduce multiple mmap arrays to a

Arnaldo Carvalho de Melo c8b5f2c96d tools: Introduce str_error_r()
The tools so far have been using the strerror_r() GNU variant, that
returns a string, be it the buffer passed or something else.

But that, besides being tricky in cases where we expect that the
function using strerror_r() returns the error formatted in a provided
buffer (we have to check if it returned something else and copy that
instead), breaks the build on systems not using glibc, like Alpine
Linux, where musl libc is used.

So, introduce yet another wrapper, str_error_r(), that has the GNU
interface, but uses the portable XSI variant of strerror_r(), so that
users rest asured that the provided buffer is used and it is what is
returned.

Wang Nan b04b702375 perf evlist: Enforce ring buffer reading
Don't read broken data after 'head' pointer.

Following commits will feed perf_evlist__mmap_read() with some 'head'
pointers not maintained by kernel. If 'head' pointer breaks an event, we
should avoid reading from the broken event. This can happen in backward
ring buffer.

For example:

                              old head
                                | |
                                V V
     +---+------+----------+----+-----+--+
     |..E|D....D|C........C|B..B|A....|E.|
     +---+------+----------+----+-----+--+

'old' pointer points to the beginning of 'A' and trying read from it,
but 'A' has been overwritten. In this case, don't try to read from 'A',
simply return NULL.

Arnaldo Carvalho de Melo f5e7150cd9 perf evlist: Expose perf_event_mlock_kb_in_pages() helper
When the user doesn't set --mmap-pages, perf_evlist__mmap() will do it
by reading the maximum possible for a non-root user from the
/proc/sys/kernel/perf_event_mlock_kb file.

Expose that function so that 'perf trace' can, for root users, to bump
mmap-pages to a higher value for root, based on the contents of this
proc file.

Wang Nan fdf14720fb perf tools: Only set filter for tracepoints events
perf_evlist__set_filter() tries to set filter to every evsel linked in
the evlist. However, since filters can only be applied to tracepoints,
checking type of evsel before calling perf_evsel__set_filter() would be
better.

Arnaldo Carvalho de Melo d9aade7fd2 perf evlist: Handle -EINVAL for sample_freq > max_sample_rate in strerror_open()
When running the "code reading" test we get:

  ---- end ----
  Test object code reading: FAILED!
  #

And with -vv we get the errno value, -22, i.e. -EINVAL, but we can do
better and handle the case at hand, with this patch it becomes:

  # perf test -v "code reading" 2>&1 | tail -7
  perf_evlist__open() failed!
  Error: Invalid argument.
  Hint: Check /proc/sys/kernel/perf_event_max_sample_rate.

Adrian Hunter 05169df556 perf script: Fix segfault using --show-mmap-events
Patch "perf script: Don't assume evsel position of tracking events"
changed 'perf script' to use 'perf_evlist__id2evsel()'. That results
in a segfault if there is more than 1 event and there are
synthesized mmap events e.g.

	$ perf record -e cycles,instructions -p$$ sleep 1
	$ perf script --show-mmap-events
	Segmentation fault (core dumped)

That happens because these synthesized events have an 'id' of zero
which does not match any 'evsel'.

Currently, these synthesized events use the sample type of the first
evsel.

Change 'perf_evlist__id2evsel()' to reflect that which also makes
it consistent with 'perf_evlist__event2evsel()'.

Arnaldo Carvalho de Melo 94ad89bc8a perf evlist: Make perf_evlist__set_filter use perf_evsel__set_filter
Instead of calling perf_evsel__apply_filter straight away, so that
we can, in the next patches, expand the filter with more conditions
before actually calling the ioctl to pass the end result filter to
the kernel.

Now we need to call perf_evlist__apply_filters() after the filter
is completely setup, i.e. do the ioctl calls.

The perf_evlist__apply_filters() method was already in place, because
that is the model for the other tools that receives filters in the
command line: go on setting then in the evsel->filter and only at
the end, after parsing the whole command line, apply them.

We get, as a bonus, a more expressive message that states which
event, if any, failed to have the filter applied to, with an
error message stating what happened.

Arnaldo Carvalho de Melo f47805a2af perf evsel: Rename set_filter to apply_filter
We need to be able to go on constructing a complex filter in multiple
stages, since we can only set one filter per event.

For instance, we need to be able, in 'perf trace' to filter by the
'common_pid' field all the time, if only for the tracer itself, to
avoid a feedback loop, and, in addition, we may want to filter the
raw_syscalls:sys_{enter,exit} events by its 'id' filter, when using
'perf trace -e open,close' or 'perf trace -e !open,close', i.e. when
we are interested in just a subset of syscalls or when we are not
interested in it.

So we will have:

   perf_evsel__set_filter(evsel, char *filter)

       Replaces whatever is in evsel->filter.

perf_evsel__append_filter(evsel, const char *op, char *filter)

Appends, using op ("&&" or "||") with what is in evsel->filter.

   perf_evsel__apply_filter(evsel, filter):

That actually applies a filter, be it the one being
constructed in evsel->filter, or any other, for tools
with more specific ways to build the filter, issuing
the appropriate ioctl for all the evsel fds.

The same changes will be made to the evlist__{set,apply} variants to
keep everything consistent.

The problem seems to be a race condition, when the application has just
exited. Some/all fds associated with the perf-events (tracepoints) go
into a POLLHUP/ POLLERR state and the mmap region associated with those
events are unmapped (in perf_evlist__filter_pollfd()).

But we go back and do a perf_evlist__mmap_read() which assumes that the
mmaps are still valid and we hit the crash.

If the mapping for an event is released, its refcnt is 0 (and ->base
is NULL), so ensure we have non-zero refcount before accessing the map.

Note that perf-record has a similar logic but unlike perf-trace, the
record__mmap_read_all() checks the evlist->mmap[i].base before accessing
the map.

Adrian Hunter b72e74d1f1 perf evlist: Amend mmap ref counting for the AUX area mmap
Reference counting of the mmap buffer does not work correctly when there
is an AUX area mmap also.

In snapshot mode it is not easy to know if the AUX area mmap buffer
contains usefull information. Equally the evlist does not know if the
recording is in sanpshot mode anyway.

Consequently, for now just assume the AUX area mmap always has data,
which will just cause the mmap buffer to remain mmapped for the duration
of the recording.

This patch supports the addition to the kernel of AUX area buffers that
can be mmapped separately from the perf-events buffer.

The AUX buffer can be configured to contain hardware-produced trace

PT.

One auxtrace buffer is mmapped per perf-events buffer. If the requested
auxtrace buffer size is zero, which it will be until further support is
added, then no auxtrace mmapping is attempted.

Arnaldo Carvalho de Melo 0a2138b28f perf evlist: Fixup brown paper bag on "hint" for --mmap-pages cmdline arg
When failing due to asking for a number of mmap pages that is more than
the max, it was suggesting that an even bigger number of mmap pages
should be specified, doh, au contraire!

Before:

  [acme@ssdandy linux]$ trace -m 128 usleep 1
  Error:	Operation not permitted.
Hint:	Check /proc/sys/kernel/perf_event_mlock_kb (516 kB) setting.
  Hint:	Tried using 516 kB.
Hint:	Try using a bigger -m/--mmap-pages value.
  [acme@ssdandy linux]$

After:

  [acme@ssdandy linux]$ trace -m 128 usleep 1
  Error:	Operation not permitted.
Hint:	Check /proc/sys/kernel/perf_event_mlock_kb (516 kB) setting.
  Hint:	Tried using 516 kB.
Hint:	Try using a smaller -m/--mmap-pages value.
  [acme@ssdandy linux]$

And to (really) clarify what happens above, when what the user requests
is <= max and even then it fails, a changeset is being made to tell that
this is a per user limit, not per process (in the above example there
was another 'perf trace' running for this user, which was using all the
pages it could use).

Adrian Hunter 3c659eedad perf tools: Add id index
Add an index of the event identifiers, in preparation for Intel PT.

The event id (also called the sample id) is a unique number
allocated by the kernel to the event created by perf_event_open(). Events
can include the event id by having a sample type including PERF_SAMPLE_ID or
PERF_SAMPLE_IDENTIFIER.

Currently the main use of the event id is to match an event back to the
evsel to which it belongs i.e. perf_evlist__id2evsel()

The purpose of this patch is to make it possible to match an event back to
the mmap from which it was read. The reason that is useful is because the
mmap represents a time-ordered context (either for a cpu or for a thread).
Intel PT decodes trace information on that basis. In full-trace mode, that
information can be recorded when the Intel PT trace is read, but in
sample-mode the Intel PT trace data is embedded in a sample and it is in
that case that the "id index" is needed.

So the mmaps are numbered (idx) and the cpu and tid recorded against the id
by perf_evlist__set_sid_idx() which is called by perf_evlist__mmap_per_evsel().

That information is recorded on the perf.data file in the new "id index".
idx, cpu and tid are added to struct perf_sample_id (which is the node of
evlist's hash table to match ids to evsels). The information can be
retrieved using perf_evlist__id2sid(). Note however this all depends on
having a sample type including PERF_SAMPLE_ID or PERF_SAMPLE_IDENTIFIER,
otherwise ids are not recorded.

The "id index" is a synthesized event record which will be created when
Intel PT sampling is used by calling perf_event__synthesize_id_index().

Adrian Hunter bf8e8f4b83 perf evlist: Add 'system_wide' option
Add an option to cause a selected event to be opened always without a
pid when configured by perf_evsel__config().

This is needed when using the sched_switch tracepoint to follow object
code execution.

sched_switch occurs before the task switch and so it cannot record it in
a context limited to that task. Note that also means that sched_switch
is useless when capturing data per-thread, as is the 'context-switches'
software event for the same reason.

Arnaldo Carvalho de Melo 5f1c4225f6 perf evlist: Don't run workload if not told to
The perf_evlist__prepare_workload() method works by forking and then
waiting on a fd that must be written to to allow the workload to be
exec()ed.

But if the tool calling it fails to, say, set up the events with which
it wants to sample the workload for, it will not call
perf_evlist__start_workload(), but even in this case the workload ended
up running:

[acme@zoo linux]$ trace /bin/echo workload ends up running, it should not...
Couldn't mmap the events: Operation not permitted
workload ends up running, it should not...
  [acme@zoo linux]$

So check if at least one byte was written before letting exec() be
called.

Now the expected behaviour:

[acme@zoo linux]$ trace /bin/echo workload ends up running, it should not...
Couldn't mmap the events: Operation not permitted
  [acme@zoo linux]$

Stephane Eranian 8ad9219e08 perf stat: Fix memory corruption of xyarray when cpumask is used
This patch fixes a memory corruption problem with the xyarray when the
evsel fds get closed at the end of the run_perf_stat() call.

It could be triggered with:

 # perf stat -a -e power/energy-cores/ ls

When cpumask are used by events (.e.g, RAPL or uncores) then the evsel
fds are allocated based on the actual number of CPUs monitored. That
number can be smaller than the total number of CPUs on the system.

The problem arises at the end by perf stat closes the fds twice. When
fds are closed, their entry in the xyarray are set to -1.

The first close() on the evsel is made from __run_perf_stat() and it
uses the actual number of CPUS for the event which is how the xyarray
was allocated for.

The second is from perf_evlist_close() but that one is on the total
number of CPUs in the system, so it assume the xyarray was allocated to
cover it. However it was not, and some writes corrupt memory.

The fix is in perf_evlist_close() is to first try with the evsel->cpus
if present, if not use the evlist->cpus. That fixes the problem.

Move debugfs.* to api/fs/. We have a common tools/lib/api/ place where

For example, all the fs-related stuff goes to tools/lib/api/fs/ from

we link it into the tools which need it - in this case perf and

Arnaldo Carvalho de Melo 602ad878d4 perf target: Shorten perf_target__ to target__
Getting unwieldly long, for this app domain should be descriptive enough
and the use of __ to separate the class from the method names should
help with avoiding clashes with other code bases.

David Ahern 9639837e95 perf evlist: Round mmap pages to power 2 - v2
Currently perf requires the -m / --mmap_pages option to be a power of 2.

To be more user friendly perf should automatically round this up to the
next power of 2.

Currently:
  $ perf record -m 3 -a -- sleep 1
--mmap_pages/-m value must be a power of two.sleep: Terminated

With patch:
  $ perf record -m 3 -a -- sleep 1
rounding mmap pages size to 16384 (4 pages)
  ...

v2: Add bytes units to rounding message per Ingo's request. Other
suggestions (e.g., prefixing INFO) should be addressed by wrapping
    pr_info to catch all instances.

David Ahern 8973504be7 perf record: Fix segfault with --no-mmap-pages
Adrian reported a segfault when using --no-out-pages:

$ tools/perf/perf record -vv --no-out-pages uname
Segmentation fault (core dumped)

The same occurs with --no-mmap-pages. Fix by checking that str is
non-NULL before parsing it.

Arnaldo Carvalho de Melo ef503831d8 perf evsel: Remove idx parm from constructor
Most uses of the evsel constructor are followed by a call to
perf_evlist__add with an idex of evlist->nr_entries, so make rename
the current constructor to perf_evsel__new_idx and remove the need
for passing the constructor for the common case.

We still need the new_idx variant because the way groups are handled,
with evsel->nr_members holding the number of entries in an evlist,
partitioning the evlist into sublists inside a single linked list.

This asks for a clarifying refactoring, but for now simplify the non
parser cases, so that tool writers don't have to bother with evsel idx
setting.
