All the virtualized platforms (KVM, lguest and Xen) have persistent
wallclocks that have more than one second of precision.
read_persistent_wallclock() and update_persistent_wallclock() allow
for nanosecond precision but their implementation on x86 with
x86_platform.get/set_wallclock() only allows for one second precision.
This means guests may see a wallclock time that is off by up to 1
second.
Make set_wallclock() and get_wallclock() take a struct timespec
parameter (which allows for nanosecond precision) so KVM and Xen
guests may start with a more accurate wallclock time and a Xen dom0
can maintain a more accurate wallclock for guests.

Pull x86 mm changes from Peter Anvin:
"This is a huge set of several partly interrelated (and concurrently
developed) changes, which is why the branch history is messier than
one would like.
The *really* big items are two humonguous patchsets mostly developed
by Yinghai Lu at my request, which completely revamps the way we
create initial page tables. In particular, rather than estimating how
much memory we will need for page tables and then build them into that
memory -- a calculation that has shown to be incredibly fragile -- we
now build them (on 64 bits) with the aid of a "pseudo-linear mode" --
a #PF handler which creates temporary page tables on demand.
This has several advantages:
1. It makes it much easier to support things that need access to data
very early (a followon patchset uses this to load microcode way
early in the kernel startup).
2. It allows the kernel and all the kernel data objects to be invoked
from above the 4 GB limit. This allows kdump to work on very large
systems.
3. It greatly reduces the difference between Xen and native (Xen's
equivalent of the #PF handler are the temporary page tables created
by the domain builder), eliminating a bunch of fragile hooks.
The patch series also gets us a bit closer to W^X.
Additional work in this pull is the 64-bit get_user() work which you
were also involved with, and a bunch of cleanups/speedups to
__phys_addr()/__pa()."

Abstract TSC's save/restore sched_clock_state functions and use

When kvm guest uses kvmclock, it may hang on vcpu hot-plug.
This is caused by an overflow in pvclock_get_nsec_offset,
u64 delta = tsc - shadow->tsc_timestamp;
which in turn is caused by an undefined values from percpu
hv_clock that hasn't been initialized yet.
Uninitialized clock on being booted cpu is accessed from
start_secondary
-> smp_callin
-> smp_store_cpu_info
-> identify_secondary_cpu
-> mtrr_ap_init
-> mtrr_restore
-> stop_machine_from_inactive_cpu
-> queue_stop_cpus_work
...
-> sched_clock
-> kvm_clock_read
which is well before x86_cpuinit.setup_percpu_clockev call in
start_secondary, where percpu clock is initialized.
This patch introduces a hook that allows to setup/initialize
per_cpu clock early and avoid overflow due to reading
- undefined values
- old values if cpu was offlined and then onlined again
Another possible early user of this clock source is ftrace that
accesses it to get timestamps for ring buffer entries. So if
mtrr_ap_init is moved from identify_secondary_cpu to past
x86_cpuinit.setup_percpu_clockev in start_secondary, ftrace
may cause the same overflow/hang on cpu hot-plug anyway.

Introduce a new x86_init hook called pagetable_reserve that at the end
of init_memory_mapping is used to reserve a range of memory addresses for
the kernel pagetable pages we used and free the other ones.
On native it just calls memblock_x86_reserve_range while on xen it also
takes care of setting the spare memory previously allocated
for kernel pagetable pages from RO to RW, so that it can be used for
other purposes.
A detailed explanation of the reason why this hook is needed follows.
As a consequence of the commit:
commit 4b239f458c
Author: Yinghai Lu <yinghai@kernel.org>
Date: Fri Dec 17 16:58:28 2010 -0800
x86-64, mm: Put early page table high
at some point init_memory_mapping is going to reach the pagetable pages
area and map those pages too (mapping them as normal memory that falls
in the range of addresses passed to init_memory_mapping as argument).
Some of those pages are already pagetable pages (they are in the range
pgt_buf_start-pgt_buf_end) therefore they are going to be mapped RO and
everything is fine.
Some of these pages are not pagetable pages yet (they fall in the range
pgt_buf_end-pgt_buf_top; for example the page at pgt_buf_end) so they
are going to be mapped RW. When these pages become pagetable pages and
are hooked into the pagetable, xen will find that the guest has already
a RW mapping of them somewhere and fail the operation.
The reason Xen requires pagetables to be RO is that the hypervisor needs
to verify that the pagetables are valid before using them. The validation
operations are called "pinning" (more details in arch/x86/xen/mmu.c).
In order to fix the issue we mark all the pages in the entire range
pgt_buf_start-pgt_buf_top as RO, however when the pagetable allocation
is completed only the range pgt_buf_start-pgt_buf_end is reserved by
init_memory_mapping. Hence the kernel is going to crash as soon as one
of the pages in the range pgt_buf_end-pgt_buf_top is reused (b/c those
ranges are RO).
For this reason we need a hook to reserve the kernel pagetable pages we
used and free the other ones so that they can be reused for other
purposes.
On native it just means calling memblock_x86_reserve_range, on Xen it
also means marking RW the pagetable pages that we allocated before but
that haven't been used before.
Another way to fix this is without using the hook is by adding a 'if
(xen_pv_domain)' in the 'init_memory_mapping' code and calling the Xen
counterpart, but that is just nasty.

The PCI initialization in pci_subsys_init() is a mess. pci_numaq_init,
pci_acpi_init, pci_visws_init and pci_legacy_init are called and each
implementation checks and eventually modifies the global variable
pcibios_scanned.
x86_init functions allow us to do this more elegant. The pci.init
function pointer is preset to pci_legacy_init. numaq, acpi and visws
can modify the pointer in their early setup functions. The functions
return 0 when they did the full initialization including bus scan. A
non zero return value indicates that pci_legacy_init needs to be
called either because the selected function failed or wants the
generic bus scan in pci_legacy_init to happen (e.g. visws).

Fix up conflicts (added both iommu_shutdown and is_untracked_pat_range)
to 'struct x86_platform_ops') in
arch/x86/include/asm/x86_init.h
arch/x86/kernel/x86_init.c
Move the find_smp_config() call to before bootmem is initialized.
Use reserve_early() instead of reserve_bootmem() in it.
This simplifies the code, we only need to call find_smp_config()
once and can remove the now unneeded reserve parameter from
x86_init_mpparse::find_smp_config.
We thus also reduce x86's dependency on bootmem allocations.

- Change is_untracked_pat_range() to return bool.
- Clean up the initialization of is_untracked_pat_range() -- by default,
we simply point it at is_ISA_range() directly.
- Move is_untracked_pat_range to the end of struct x86_platform, since
it is the newest field.

GRU space is always mapped as WB in the page table. There is
no need to track the mappings in the PAT. This also eliminates
the "freeing invalid memtype" messages when the GRU space is
unmapped.

We call the detections functions of all the IOMMUs then all
their initialization functions. The latter is pointless since we
don't detect multiple different IOMMUs. What we need to do is
calling the initialization function of the detected IOMMU.
This adds iommu_init hook to x86_init_ops so if an IOMMU
detection function can set its initialization function to the
hook.

This patch cleans up pci_iommu_shutdown() a bit to use
x86_platform (similar to how IA64 initializes an IOMMU driver).
This adds iommu_shutdown() to x86_platform to avoid calling
every IOMMUs' shutdown functions in pci_iommu_shutdown() in
order. The IOMMU shutdown functions are platform specific (we
don't have multiple different IOMMU hardware) so the current way
is pointless.
An IOMMU driver sets x86_platform.iommu_shutdown to the shutdown
function if necessary.

get/set_wallclock() have already a set of platform dependent
implementations (default, EFI, paravirt). MRST will add another
variant.
Moving them to platform ops simplifies the existing code and minimizes
the effort to integrate new variants.

Platforms like Moorestown require early setup and want to avoid the
call to reserve_ebda_region. The x86_init override is too late when
the MRST detection happens in setup_arch. Move the default i386
x86_init overrides and the call to reserve_ebda_region into a separate
function which is called as the default of a switch case depending on
the hardware_subarch id in boot params. This allows us to add a case
for MRST and let MRST have its own early setup function.

TSC calibration is modified by the vmware hypervisor and paravirt by
separate means. Moorestown wants to add its own calibration routine as
well. So make calibrate_tsc a proper x86_init_ops function and
override it by paravirt or by the early setup of the vmware
hypervisor.

The timer init code is convoluted with several quirks and the paravirt
timer chooser. Figuring out which code path is actually taken is not
for the faint hearted.
Move the numaq TSC quirk to tsc_pre_init x86_init_ops function and
replace the paravirt time chooser and the remaining x86 quirk with a
simple x86_init_ops function.

paravirt overrides the setup of the default apic timers as per cpu
timers. Moorestown needs to override that as well.
Move it to x86_init_ops setup and create a separate x86_cpuinit struct
which holds the function for the secondary evtl. hotplugabble CPUs.

Replace another obscure paravirt magic and move it to
x86_init_ops. Such a hook is also useful for embedded and special
hardware.

ARCH_SETUP is a horrible leftover from the old arch/i386 mach support
code. It still has a lonely user in xen. Move it to x86_init_ops.
