If a crash happens on cpu3 and all interrupts are binding on cpu0, the
bad irq routing will cause a crash kernel which can't receive any irq.
Because crash kernel won't clean up all harts' PLIC enable bits in
enable registers. This patch is similar to 9141a003a4 ("ARM: 7316/1:
kexec: EOI active and mask all interrupts in kexec crash path") and
78fd584cde ("arm64: kdump: implement machine_crash_shutdown()"), and
PowerPC also has the same mechanism.

The pointer to buffer loading kernel binaries is in kernel space for
kexec_fil mode, When copy_from_user copies data from pointer to a block
of memory, it checkes that the pointer is in the user space range, on
RISCV-V that is:

and TASK_SIZE is 0x4000000000 for 64-bits, which now causes
copy_from_user to reject the access of the field 'buf' of struct
kexec_segment that is in range [CONFIG_PAGE_OFFSET - VMALLOC_SIZE,
CONFIG_PAGE_OFFSET), is invalid user space pointer.
This patch fixes this issue by skipping access_ok(), use mempcy() instead.

raw_smp_processor_id() doesn't return the hart id as stated in
arch/riscv/include/asm/smp.h, use smp_processor_id() instead
to get the cpu id, and cpuid_to_hartid_map() to pass the hart id
to the next kernel. This fixes kexec on HiFive Unleashed/Unmatched
where cpu ids and hart ids don't match (on qemu-virt they match).

We have a lot of variables that are used to hold kernel mapping addresses,
offsets between physical and virtual mappings and some others used for XIP
kernels: they are all defined at different places in mm/init.c, so group
them into a single structure with, for some of them, more explicit and concise
names.

This patch adds support for kdump, the kernel will reserve a
region for the crash kernel and jump there on panic. In order
for userspace tools (kexec-tools) to prepare the crash kernel
kexec image, we also need to expose some information on
/proc/iomem for the memory regions used by the kernel and for
the region reserved for crash kernel. Note that on userspace
the device tree is used to determine the system's memory
layout so the "System RAM" on /proc/iomem is ignored.
I tested this on riscv64 qemu and works as expected, you may
test it by triggering a crash through /proc/sysrq_trigger:

This patch adds support for kexec on RISC-V. On SMP systems it depends
on HOTPLUG_CPU in order to be able to bring up all harts after kexec.
It also needs a recent OpenSBI version that supports the HSM extension.
I tested it on riscv64 QEMU on both an smp and a non-smp system.
