Greg Ungerer cbd5b982f6 m68k: report correct FPU type on ColdFire MMU platforms
Not all ColdFire SoC parts that have an MMU also have an FPU - so set
an FPU type (via m68k_fputype) appropriate for the configured platform.

With this set correctly /proc/cpuinfo will report FPU "none" on devices
that don't have one. And kernel code paths that initialize FPU hardware
will now only execute if an FPU is actually present.

Greg Ungerer 81d33350ba m68k: set appropriate machine type for m5411x SoC platforms
Create a new machine type for platforms based around the ColdFire 5441x
SoC family. Set that machine type on startup when building for this
platform type.

Currently the ColdFire head.S hard codes a M54xx machine type at startup -
since that is the only platform type currently supported with MMU enabled.
The m5441x has an MMU and this change forms part of the support required
to run it with the MMU enabled.

Greg Ungerer e5f8d1f0a1 m68k: move CONFIG_FPU set to per-CPU configuration
Move the selection of CONFIG_FPU to each CPU type configuration.

Currently for m68k we have a global set of CONFIG_FPU based on if CONFIG_MMU
is enabled or not. There is at least one CPU family we support (m5441x)
that has an MMU but has no FPU hardware. So we need to be able to have
CONFIG_MMU set and CONFIG_FPU not set.

Whether we build for a CPU with MMU enabled or not doesn't change the
fact that it has FPU hardware support. Our current non-MMU builds have
never had CONIG_FPU enabled - and in fact the kernel will not compile
with that set and CONFIG_MMU not set at the moment. It is easy enough
to fix this - but it would involve a structure change to sigcontext.h,
and that is a user space exported header (so ABI change).

This change makes no configuration visible changes, and all configs
end up with the same configuration settings as before.

Greg Ungerer 36d050d913 m68knommu: fix IO write size in nettel pin set
The pin write code that supports the UART signals is not using he correct
word write IO access method. It correctly reads the correct 16 bit
registrer, it should also write the new value back with a 16 bit write.
Fix it to use writew().

Greg Ungerer 41b39ea190 m68knommu: switch to using IO access methods in WildFire board code
Most ColdFire support code has switched to using IO memory access
methods (readb/writeb/etc) when reading and writing internal peripheral
device registers. The WildFire board specific halt code was missed.

As it is now the WildFire code is broken, since all register definitions
were changed to be register addresses only some time ago.

Fix the WildFire board code to use the appropriate IO access functions.

Greg Ungerer bc065e4784 m68knommu: fix early setup to not access variables
The early setup code for the ColdFire 53xx platform accesses variables
before the RAM and other system initialization steps may have taken place.
Currently it has 2 global variables that will end up in the bss section
that are accessed during this early setup. There is a special static RAM
stack setup at this time, but not necessarily the RAM where kernel data
sections will end up.

Even on system setups where RAM is setup by a boot loader the access
to the early setup variables is before the BSS section has been initialized.
This can potentially corrupt a ram loaded root filesystem that sits in that
memory area before it has been moved.

These 2 variables are not used at all after being set, and can just be
removed.

Greg Ungerer 0b98027122 m68knommu: fix user a5 register being overwritten
On no-MMU systems the application a5 register can be overwitten with the
address of the process data segment when processing application signals.
For flat format applications compiled with full absolute relocation this
effectively corrupts the a5 register on signal processing - and this very
quickly leads to process crash and often takes out the whole system with
a panic as well.

This has no effect on flat format applications compiled with the more
common PIC methods (such as -msep-data). These format applications reserve
a5 for the pointer to the data segment anyway - so it doesn't change it.

A long time ago the a5 register was used in the code packed into the user
stack to enable signal return processing. And so it had to be restored on
end of signal cleanup processing back to the original a5 user value. This
was historically done by saving away a5 in the sigcontext structure. At
some point (a long time back it seems) the a5 restore process was changed
and it was hard coded to put the user data segment address directly into a5.
Which is ok for the common PIC compiled application case, but breaks the
full relocation application code.

We no longer use this type of signal handling mechanism and so we don't
need to do anything special to save and restore a5 at all now. So remove the
code that hard codes a5 to the address of the user data segment.

Krzysztof Kozlowski 00085f1efa dma-mapping: use unsigned long for dma_attrs
The dma-mapping core and the implementations do not change the DMA
attributes passed by pointer.  Thus the pointer can point to const data.
However the attributes do not have to be a bitfield.  Instead unsigned
long will do fine:

1. This is just simpler.  Both in terms of reading the code and setting
   attributes.  Instead of initializing local attributes on the stack
   and passing pointer to it to dma_set_attr(), just set the bits.

2. It brings safeness and checking for const correctness because the
   attributes are passed by value.

Nicolas Pitre 002d2f01f1 m68k: enable binfmt_flat on systems with an MMU
Now that the generic changes are in place, this can be enabled on m68k
with the use of proper user space accessors in the flat_get_addr_from_rp()
and flat_put_addr_at_rp() handlers as rp actually holds a user space
address.

Greg Ungerer 89e6ffd9f0 m68k: fix bFLT executable running on MMU enabled systems
Even after recent changes to support running flat format executables on
MMU enabled systems (by nicolas.pitre@linaro.org) they still failed to
run on m68k/ColdFire MMU enabled systems. On trying to run a flat format
binary the application would immediately crash with a SIGSEGV.

Code to setup the D5 register with the base of the application data
region was only in the non-MMU code path, so it was not being set for
the MMU enabled case. Flat binaries on m68k/ColdFire use this to support
GOT/PIC flat built application code.

Fix this so that D5 is always setup when loading/running a bFLT executable
on m68k systems.

basically only rebased it on top of 4.7-rc1 tree and dropped "dm: get
rid of superfluous gfp flags" which went through dm tree.  I am sending
it now because it is tree wide and chances for conflicts are reduced
considerably when we want to target rc2.  I plan to send the next step
and rename the flag and move to a better semantic later during this
release cycle so we will have a new semantic ready for 4.8 merge window
hopefully.

Motivation:

While working on something unrelated I've checked the current usage of
__GFP_REPEAT in the tree.  It seems that a majority of the usage is and
always has been bogus because __GFP_REPEAT has always been about costly
high order allocations while we are using it for order-0 or very small
orders very often.  It seems that a big pile of them is just a
copy&paste when a code has been adopted from one arch to another.

I think it makes some sense to get rid of them because they are just
making the semantic more unclear.  Please note that GFP_REPEAT is
documented as

* __GFP_REPEAT: Try hard to allocate the memory, but the allocation attempt

* _might_ fail.  This depends upon the particular VM implementation.
  while !costly requests have basically nofail semantic.  So one could
  reasonably expect that order-0 request with __GFP_REPEAT will not loop
  for ever.  This is not implemented right now though.

I would like to move on with __GFP_REPEAT and define a better semantic
for it.

So we are down to the third after this patch series.  The remaining
places really seem to be relying on __GFP_REPEAT due to large allocation
requests.  This still needs some double checking which I will do later
after all the simple ones are sorted out.

I am touching a lot of arch specific code here and I hope I got it right
but as a matter of fact I even didn't compile test for some archs as I
do not have cross compiler for them.  Patches should be quite trivial to
review for stupid compile mistakes though.  The tricky parts are usually
hidden by macro definitions and thats where I would appreciate help from
arch maintainers.

This patch (of 19):

__GFP_REPEAT has a rather weak semantic but since it has been introduced
around 2.6.12 it has been ignored for low order allocations.  Yet we
have the full kernel tree with its usage for apparently order-0
allocations.  This is really confusing because __GFP_REPEAT is
explicitly documented to allow allocation failures which is a weaker
semantic than the current order-0 has (basically nofail).

Let's simply drop __GFP_REPEAT from those places.  This would allow to
identify place which really need allocator to retry harder and formulate
a more specific semantic for what the flag is supposed to do actually.

Peter Zijlstra e39d88ea3c locking/atomic, arch/m68k: Implement atomic_fetch_{add,sub,and,or,xor}()
Implement FETCH-OP atomic primitives, these are very similar to the
existing OP-RETURN primitives we already have, except they return the
value of the atomic variable _before_ modification.

This is especially useful for irreversible operations -- such as
bitops (because it becomes impossible to reconstruct the state prior
to modification).

Arnd Bergmann 084b3600e2 char/genrtc: remove m68k support
The asm/rtc.h header is only used for the old gen_rtc driver
that has been replaced by rtc-generic. According to Geert
Uytterhoeven, nobody has used the old driver on m68k for
a long time, so we can now just remove the header file
and disallow the driver in Kconfig.

All files that used to include asm/rtc.h are now changed so
they include the headers that were used implicitly through
asm/rtc.h.

Arnd Bergmann 598e8a1fb5 rtc: m68k: provide ioctl for q40
The q40 platform is the only machine in the kernel that provides
RTC_PLL_GET/RTC_PLL_SET ioctl commands in its rtc through the
mach_get_rtc_pll/mach_set_rtc_pll callbacks.

However, this currenctly works only in the old-style genrtc
driver, not the (somewhat) modern rtc-generic driver replacing
it. This adds an ioctl implementation to the m68k generic_rtc_ops
in order to let both drivers provide the same API.

After this, we should be able to remove support for genrtc
from the m68k architecture.

   - Avoids 64-bit multiplies in hash_64() on 32-bit platforms.  Two
     32-bit multiplies will do well enough.

   - Rids the world of the bad hash multipliers in hash_32.

     This finishes the job started in commit 689de1d6ca ("Minimal
     fix-up of bad hashing behavior of hash_64()")

     The vast majority of Linux architectures have hardware support for
     32x32-bit multiply and so derive no benefit from "simplified"
     multipliers.

     The few processors that do not (68000, h8/300 and some models of
     Microblaze) have arch-specific implementations added.  Those
     patches are last in the series.

   - Overhauls the dcache hash mixing.

     The patch in commit 0fed3ac866 ("namei: Improve hash mixing if
     CONFIG_DCACHE_WORD_ACCESS") was an off-the-cuff suggestion.
     Replaced with a much more careful design that's simultaneously
     faster and better.  (My own invention, as there was noting suitable
     in the literature I could find.  Comments welcome!)

   - Modify the hash_name() loop to skip the initial HASH_MIX().  This
     would let us salt the hash if we ever wanted to.

   - Sort out partial_name_hash().

     The hash function is declared as using a long state, even though
     it's truncated to 32 bits at the end and the extra internal state
     contributes nothing to the result.  And some callers do odd things:

      - fs/hfs/string.c only allocates 32 bits of state
      - fs/hfsplus/unicode.c uses it to hash 16-bit unicode symbols not bytes

   - Modify bytemask_from_count to handle inputs of 1..sizeof(long)
     rather than 0..sizeof(long)-1.  This would simplify users other
     than full_name_hash"

  Special thanks to Bruce Fields for testing and finding bugs in v1.  (I
  learned some humbling lessons about "obviously correct" code.)

  On the arch-specific front, the m68k assembly has been tested in a
  standalone test harness, I've been in contact with the Microblaze
  maintainers who mostly don't care, as the hardware multiplier is never
  omitted in real-world applications, and I haven't heard anything from

Zhaoxiu Zeng fff7fb0b2d lib/GCD.c: use binary GCD algorithm instead of Euclidean
The binary GCD algorithm is based on the following facts:
	1. If a and b are all evens, then gcd(a,b) = 2 * gcd(a/2, b/2)
	2. If a is even and b is odd, then gcd(a,b) = gcd(a/2, b)
	3. If a and b are all odds, then gcd(a,b) = gcd((a-b)/2, b) = gcd((a+b)/2, b)

Even on x86 machines with reasonable division hardware, the binary
algorithm runs about 25% faster (80% the execution time) than the
division-based Euclidian algorithm.

On platforms like Alpha and ARMv6 where division is a function call to
emulation code, it's even more significant.

There are two variants of the code here, depending on whether a fast
__ffs (find least significant set bit) instruction is available.  This
allows the unpredictable branches in the bit-at-a-time shifting loop to
be eliminated.

If fast __ffs is not available, the "even/odd" GCD variant is used.

Greg Ungerer 2763ee6448 m68k/gpio: remove arch specific sysfs bus device
The ColdFire architecture specific gpio support code registers a sysfs
bus device named "gpio". This clashes with the new generic API device
added in commit 3c702e99 ("gpio: add a userspace chardev ABI for GPIOs").

The old ColdFire sysfs gpio device was never used for anything specific,
and no links or other nodes were created under it. The new API sysfs gpio
device has all the same default sysfs links (device, drivers, etc) and
they are properly populated.

Remove the old ColdFire sysfs gpio registration.

Core changes:
 
 - The gpio_chip is now a *real device*. Until now the gpio chips
   were just piggybacking the parent device or (gasp) floating in
   space outside of the device model. We now finally make GPIO chips
   devices. The gpio_chip will create a gpio_device which contains
   a struct device, and this gpio_device struct is kept private.
   Anything that needs to be kept private from the rest of the kernel
   will gradually be moved over to the gpio_device.
 
 - As a result of making the gpio_device a real device, we have added
   resource management, so devm_gpiochip_add_data() will cut down on
   overhead and reduce code lines. A huge slew of patches convert
   almost all drivers in the subsystem to use this.
 
 - Building on making the GPIO a real device, we add the first step
   of a new userspace ABI: the GPIO character device. We take small
   steps here, so we first add a pure *information* ABI and the tool
   "lsgpio" that will list all GPIO devices on the system and all
   lines on these devices. We can now discover GPIOs properly from
   userspace. We still have not come up with a way to actually *use*
   GPIOs from userspace.
 
 - To encourage people to use the character device for the future,
   we have it always-enabled when using GPIO. The old sysfs ABI is
   still opt-in (and can be used in parallel), but is marked as
   deprecated. We will keep it around for the foreseeable future,
   but it will not be extended to cover ever more use cases.
 
 Cleanup:
 
 - Bjorn Helgaas removed a whole slew of per-architecture <asm/gpio.h>
   includes. This dates back to when GPIO was an opt-in feature and
   no shared library even existed: just a header file with proper
   prototypes was provided and all semantics were up to the arch to
   implement. These patches make the GPIO chip even more a proper
   device and cleans out leftovers of the old in-kernel API here
   and there. Still some cruft is left but it's very little now.
 
 - There is still some clamping of return values for .get() going
   on, but we now return sane values in the vast majority of drivers
   and the errorpath is sanitized. Some patches for powerpc, blackfin
   and unicore still drop in.

Alexander Duyck 1e94082963 ipv6: Pass proto to csum_ipv6_magic as __u8 instead of unsigned short
This patch updates csum_ipv6_magic so that it correctly recognizes that
protocol is a unsigned 8 bit value.

This will allow us to better understand what limitations may or may not be
present in how we handle the data.  For example there are a number of
places that call htonl on the protocol value.  This is likely not necessary
and can be replaced with a multiplication by ntohl(1) which will be
converted to a shift by the compiler.

Christoph Hellwig bc4b024a8b PCI: Move pci_dma_* helpers to common code
For a long time all architectures implement the pci_dma_* functions using
the generic DMA API, and they all use the same header to do so.

Move this header, pci-dma-compat.h, to include/linux and include it from
the generic pci.h instead of having each arch duplicate this include.

Greg Ungerer ca6fafd183 m68knommu: fix FEC platform device registration when driver is modular
The FEC (Fast Ethernet Crontroller) module on many ColdFire parts can
be compiled into the kernel, or as a module. Therefore the platform device
support for it is required whenever the driver is enabled - not just when
built into the kernel. Use IS_ENABLED(CONFIG_FEC) instead of a conditional
check on only the driver being built into the kernel.

Greg Ungerer a95517992a m68k: Use conventional function parameters for do_sigreturn
Create conventional stack parameters for the calls to do_sigreturn and
do_rt_sigreturn. The current C code for do_sigreturn and do_rt_sigreturn
dig into the stack to create local pointers to the saved switch stack
and the pt_regs structs.

The motivation for this change is a problem with non-MMU targets that
have broken signal return paths on newer versions of gcc. It appears as
though gcc has determined that the pointers into the saved stack structs,
and the saved structs themselves, are function parameters and updates to
them will be lost on function return, so they are optimized away. This
results in large parts of restore_sigcontext() and mangle_kernel_stack()
functions being removed. Of course this results in non-functional code
causing kernel oops. This problem has been observed with gcc version
5.2 and 5.3, and probably exists in earlier versions as well.

Using conventional stack parameter pointers passed to these functions has
the advantage of the code here not needing to know the exact details of
how the underlying entry handler layed these structs out on the stack.
So the rather ugly pointer setup casting and arg referencing can be
removed.

The resulting code after this change is a few bytes larger (due to the
overhead of creating the stack args and their tear down). Not being hot
paths I don't think this is too much of a problem here.

An alternative solution is to put a barrier() in the do_sigreturn() code,
but this doesn't feel quite as clean as this solution.

This change has been compile tested on all defconfigs, and run tested on
Atari (through aranym), ColdFire with MMU (M5407EVB) and ColdFire with
no-MMU (QEMU and M5208EVB).

  and get conversion of i_mutex to rwsem with ->lookup() done under lock
  taken shared.

  There will be a patch closer to the end of the window, along the lines

Andreas Schwab 8474ba7419 m68k: Define asmlinkage_protect
Make sure the compiler does not modify arguments of syscall functions.
This can happen if the compiler generates a tailcall to another
function.  For example, without asmlinkage_protect sys_openat is compiled
into this function:

sys_openat:

	move.l %d0,16(%sp)
	jbra do_sys_open

Note how the fourth argument is modified in place, modifying the register
%d4 that gets restored from this stack slot when the function returns to
user-space.  The caller may expect the register to be unmodified across
system calls.

Peter Zijlstra 62e8a3258b atomic, arch: Audit atomic_{read,set}()
This patch makes sure that atomic_{read,set}() are at least
{READ,WRITE}_ONCE().

We already had the 'requirement' that atomic_read() should use
ACCESS_ONCE(), and most archs had this, but a few were lacking.
All are now converted to use READ_ONCE().

And, by a symmetry and general paranoia argument, upgrade atomic_set()
to use WRITE_ONCE().

Dave Young 2965faa5e0 kexec: split kexec_load syscall from kexec core code
There are two kexec load syscalls, kexec_load another and kexec_file_load.
 kexec_file_load has been splited as kernel/kexec_file.c.  In this patch I
split kexec_load syscall code to kernel/kexec.c.

And add a new kconfig option KEXEC_CORE, so we can disable kexec_load and
use kexec_file_load only, or vice verse.

The original requirement is from Ted Ts'o, he want kexec kernel signature
being checked with CONFIG_KEXEC_VERIFY_SIG enabled.  But kexec-tools use
kexec_load syscall can bypass the checking.

Vivek Goyal proposed to create a common kconfig option so user can compile
in only one syscall for loading kexec kernel.  KEXEC/KEXEC_FILE selects
KEXEC_CORE so that old config files still work.

Because there's general code need CONFIG_KEXEC_CORE, so I updated all the
architecture Kconfig with a new option KEXEC_CORE, and let KEXEC selects
KEXEC_CORE in arch Kconfig.  Also updated general kernel code with to
kexec_load syscall.

Viresh Kumar 5bbc08fb0f m68k/coldfire/pit: Migrate to new 'set-state' interface
Migrate m68k driver to the new 'set-state' interface provided by
clockevents core, the earlier 'set-mode' interface is marked obsolete
now.

This also enables us to implement callbacks for new states of clockevent
devices, for example: ONESHOT_STOPPED.

We weren't doing anything in ->set_mode(RESUME) and so tick_resume()
isn't implemented.

Christoph Hellwig 4246a0b63b block: add a bi_error field to struct bio
Currently we have two different ways to signal an I/O error on a BIO:

 (1) by clearing the BIO_UPTODATE flag
 (2) by returning a Linux errno value to the bi_end_io callback

The first one has the drawback of only communicating a single possible
error (-EIO), and the second one has the drawback of not beeing persistent
when bios are queued up, and are not passed along from child to parent
bio in the ever more popular chaining scenario.  Having both mechanisms
available has the additional drawback of utterly confusing driver authors
and introducing bugs where various I/O submitters only deal with one of
them, and the others have to add boilerplate code to deal with both kinds
of error returns.

So add a new bi_error field to store an errno value directly in struct
bio and remove the existing mechanisms to clean all this up.

Luis R. Rodriguez 4c73e89266 arch/*/io.h: Add ioremap_uc() to all architectures
This adds ioremap_uc() only for architectures that do not
include asm-generic.h/io.h as that already provides a default
definition for them for both cases where you have CONFIG_MMU
and you do not, and because of this, the number of architectures
this patch address is less than the architectures that the
ioremap_wt() patch addressed, "arch/*/io.h: Add ioremap_wt() to
all architectures").

In order to reduce the number of architectures we have to
modify by adding new architecture IO APIs we'll have to review
the architectures in this patch, see why they can't add
asm-generic.h/io.h or issues that would be created by doing
so and then spread a consistent inclusion of this header
towards the end of their own header. For instance arch/metag
includes the asm-generic/io.h *before* the ioremap*()
definitions, this should be the other way around but only
once we have guard wrappers for the non-MMU case also for
asm-generic/io.h.

Laurent Dufour f2abeef9fd mm: clean up per architecture MM hook header files
Commit 2ae416b142 ("mm: new mm hook framework") introduced an empty
header file (mm-arch-hooks.h) for every architecture, even those which
doesn't need to define mm hooks.

As suggested by Geert Uytterhoeven, this could be cleaned through the use
of a generic header file included via each per architecture
asm/include/Kbuild file.

The PowerPC architecture is not impacted here since this architecture has
to defined the arch_remap MM hook.

Greg Ungerer fa95a1dd08 m68knommu: make ColdFire SoC selection a choice
It would be nice if we could support multiple ColdFire SoC types in a
single binary - but currently the code simply does not support it.
Change the SoC selection config options to be a choice instead of
individual selectable entries.

This fixes problems with building allnoconfig, and means that a sane
linux kernel is generated for a single ColdFire SoC type.

Greg Ungerer 15c2ca4e98 m68knommu: improve the clock configuration defaults
Create some intelligent default settings for each ColdFire SoC type
in the configuration entry for CONFIG_CLOCK_FREQ.

The ColdFire clock frequency is configurable at build time. There is a
lot of variation in the frequency of operation on specific ColdFire based
boards. But we can choose a default that matches the maximum frequency
of clock operation for a particular ColdFire part. That is typically
the most common clock setting.

Laurent Dufour 2ae416b142 mm: new mm hook framework
CRIU is recreating the process memory layout by remapping the checkpointee
memory area on top of the current process (criu).  This includes remapping
the vDSO to the place it has at checkpoint time.

However some architectures like powerpc are keeping a reference to the
vDSO base address to build the signal return stack frame by calling the
vDSO sigreturn service.  So once the vDSO has been moved, this reference
is no more valid and the signal frame built later are not usable.

This patch serie is introducing a new mm hook framework, and a new
arch_remap hook which is called when mremap is done and the mm lock still
hold.  The next patch is adding the vDSO remap and unmap tracking to the
powerpc architecture.

This patch (of 3):

This patch introduces a new set of header file to manage mm hooks:
- per architecture empty header file (arch/x/include/asm/mm-arch-hooks.h)
- a generic header (include/linux/mm-arch-hooks.h)

The architecture which need to overwrite a hook as to redefine it in its
header file, while architecture which doesn't need have nothing to do.

The default hooks are defined in the generic header and are used in the
case the architecture is not defining it.

In a next step, mm hooks defined in include/asm-generic/mm_hooks.h should
be moved here.

       This is an important RAS feature which adds hardware support for

       form of a deferred error.  It is the OS's responsibility then to
       take proper recovery action and thus prolonge system lifetime as

Toshi Kani 556269c138 arch/*/io.h: Add ioremap_wt() to all architectures
Add ioremap_wt() to all arch-specific asm/io.h headers which
define ioremap_wc() locally. These headers do not include
<asm-generic/iomap.h>. Some of them include <asm-generic/io.h>,
but ioremap_wt() is defined for consistency since they define
all ioremap_xxx locally.

In all architectures without Write-Through support, ioremap_wt()
is defined indentical to ioremap_nocache().

frv and m68k already have ioremap_writethrough(). On those we
add ioremap_wt() indetical to ioremap_writethrough() and defines
ARCH_HAS_IOREMAP_WT in both architectures.

The ioremap_wt() interface is exported to drivers.

Akinobu Mita 1214c52548 m68k: Use for_each_sg()
This replaces the plain loop over the sglist array with for_each_sg()
macro which consists of sg_next() function calls.  Since m68k doesn't
select ARCH_HAS_SG_CHAIN, it is not necessary to use for_each_sg() in
order to loop over each sg element.  But this can help find problems
with drivers that do not properly initialize their sg tables when
CONFIG_DEBUG_SG is enabled.

David Hildenbrand 70ffdb9393 mm/fault, arch: Use pagefault_disable() to check for disabled pagefaults in the handler
Introduce faulthandler_disabled() and use it to check for irq context and
disabled pagefaults (via pagefault_disable()) in the pagefault handlers.

Please note that we keep the in_atomic() checks in place - to detect
whether in irq context (in which case preemption is always properly
disabled).

In contrast, preempt_disable() should never be used to disable pagefaults.
With !CONFIG_PREEMPT_COUNT, preempt_disable() doesn't modify the preempt
counter, and therefore the result of in_atomic() differs.
We validate that condition by using might_fault() checks when calling
might_sleep().

Therefore, add a comment to faulthandler_disabled(), describing why this
is needed.

faulthandler_disabled() and pagefault_disable() are defined in
linux/uaccess.h, so let's properly add that include to all relevant files.

Frederic Weisbecker 92cf211874 sched/preempt: Merge preempt_mask.h into preempt.h
preempt_mask.h defines all the preempt_count semantics and related
symbols: preempt, softirq, hardirq, nmi, preempt active, need resched,
etc...

preempt.h defines the accessors and mutators of preempt_count.

But there is a messy dependency game around those two header files:

	* preempt_mask.h includes preempt.h in order to access preempt_count()

	* preempt_mask.h defines all preempt_count semantic and symbols
	  except PREEMPT_NEED_RESCHED that is needed by asm/preempt.h
	  Thus we need to define it from preempt.h, right before including
	  asm/preempt.h, instead of defining it to preempt_mask.h with the
	  other preempt_count symbols. Therefore the preempt_count semantics
	  happen to be spread out.

	* We plan to introduce preempt_active_[enter,exit]() to consolidate
	  preempt_schedule*() code. But we'll need to access both preempt_count
	  mutators (preempt_count_add()) and preempt_count symbols
	  (PREEMPT_ACTIVE, PREEMPT_OFFSET). The usual place to define preempt
	  operations is in preempt.h but then we'll need symbols in
	  preempt_mask.h which already includes preempt.h. So we end up with
	  a ressource circle dependency.

Lets merge preempt_mask.h into preempt.h to solve these dependency issues.
This way we gather semantic symbols and operation definition of
preempt_count in a single file.

This is a dumb copy-paste merge. Further merge re-arrangments are
performed in a subsequent patch to ease review.

Valentin Rothberg 507224aa88 serial: 8250: remove Kconfig indirection
Remove CONFIG_SERIAL_DETECT_IRQ and CONFIG_SERIAL_MANY_PORTS, and
substitute all references to the proper 8250 Kconfig options.  Now, the
actual Kconfig dependencies are not hidden when reading the code and

Kirill A. Shutemov 980d5b7387 m68k: mark PMD folded and expose number of page table levels
We would want to use number of page table level to define mm_struct.
Let's expose it as CONFIG_PGTABLE_LEVELS.

Core mm expects __PAGETABLE_{PUD,PMD}_FOLDED to be defined if these page
table levels folded.
