Trond Myklebust dc9dc2febb NFS: Fix use-after-free issues in nfs_pageio_add_request()
We need to ensure that we create the mirror requests before calling
nfs_pageio_add_request_mirror() on the request we are adding.
Otherwise, we can end up with a use-after-free if the call to
nfs_pageio_add_request_mirror() triggers I/O.

Trond Myklebust 1de3af9883 NFS: Remove unused FLUSH_SYNC support in nfs_initiate_pgio()
If the FLUSH_SYNC flag is set, nfs_initiate_pgio() will currently
wait for completion, and then return the status of the I/O operation.
What we actually want to report in nfs_pageio_doio() is whether or
not the RPC call was launched successfully, whereas actual I/O
status is intended handled in the reply callbacks.

Since FLUSH_SYNC is never set by any of the callers anyway, let's
just remove that code altogether.

Trond Myklebust 17d8c5d145 NFS: Fix initialisation of I/O result struct in nfs_pgio_rpcsetup
Initialise the result count to 0 rather than initialising it to the
argument count. The reason is that we want to ensure we record the
I/O stats correctly in the case where an error is returned (for
instance in the layoutstats).

Thomas Gleixner 457c899653 treewide: Add SPDX license identifier for missed files
Add SPDX license identifiers to all files which:

 - Have no license information of any form

 - Have EXPORT_.*_SYMBOL_GPL inside which was used in the
   initial scan/conversion to ignore the file

These files fall under the project license, GPL v2 only. The resulting SPDX
license identifier is:

Trond Myklebust 33344e0f7e pNFS: Add tracking to limit the number of pNFS retries
When the client is reading or writing using pNFS, and hits an error
on the DS, then it typically sends a LAYOUTERROR and/or LAYOUTRETURN
to the MDS, before redirtying the failed pages, and going for a new
round of reads/writebacks. The problem is that if the server has no
way to fix the DS, then we may need a way to interrupt this loop
after a set number of attempts have been made.
This patch adds an optional module parameter that allows the admin
to specify how many times to retry the read/writeback process before
failing with a fatal error.
The default behaviour is to retry forever.

Trond Myklebust f57dcf4c72 NFS: Fix I/O request leakages
When we fail to add the request to the I/O queue, we currently leave it
to the caller to free the failed request. However since some of the
requests that fail are actually created by nfs_pageio_add_request()
itself, and are not passed back the caller, this leads to a leakage
issue, which can again cause page locks to leak.

This commit addresses the leakage by freeing the created requests on
error, using desc->pg_completion_ops->error_cleanup()

NeilBrown a52458b48a NFS/NFSD/SUNRPC: replace generic creds with 'struct cred'.
SUNRPC has two sorts of credentials, both of which appear as
"struct rpc_cred".
There are "generic credentials" which are supplied by clients
such as NFS and passed in 'struct rpc_message' to indicate
which user should be used to authorize the request, and there
are low-level credentials such as AUTH_NULL, AUTH_UNIX, AUTH_GSS
which describe the credential to be sent over the wires.

This patch replaces all the generic credentials by 'struct cred'
pointers - the credential structure used throughout Linux.

For machine credentials, there is a special 'struct cred *' pointer
which is statically allocated and recognized where needed as
having a special meaning.  A look-up of a low-level cred will
map this to a machine credential.

NeilBrown bf4b490597 NFS: various changes relating to reporting IO errors.
1/ remove 'start' and 'end' args from nfs_file_fsync_commit().
   They aren't used.

2/ Make nfs_context_set_write_error() a "static inline" in internal.h
   so we can...

3/ Use nfs_context_set_write_error() instead of mapping_set_error()
   if nfs_pageio_add_request() fails before sending any request.
   NFS generally keeps errors in the open_context, not the mapping,
   so this is more consistent.

4/ If filemap_write_and_write_range() reports any error, still
   check ctx->error.  The value in ctx->error is likely to be
   more useful.  As part of this, NFS_CONTEXT_ERROR_WRITE is
   cleared slightly earlier, before nfs_file_fsync_commit() is called,
   rather than at the start of that function.

The writeback code wants to send a commit after processing the pages,
which is why we want to delay releasing the struct path until after
that's done.

Also, the layout code expects that we do not free the inode before
we've put the layout segments in pnfs_writehdr_free() and
pnfs_readhdr_free()

Trond Myklebust 14abcb0bf5 NFSv4: Fix up mirror allocation
There are a number of callers of nfs_pageio_complete() that want to
continue using the nfs_pageio_descriptor without needing to call
nfs_pageio_init() again. Examples include nfs_pageio_resend() and
nfs_pageio_cond_complete().

The problem is that nfs_pageio_complete() also calls
nfs_pageio_cleanup_mirroring(), which frees up the array of mirrors.
This can lead to writeback errors, in the next call to
nfs_pageio_setup_mirroring().

Fix by simply moving the allocation of the mirrors to
nfs_pageio_setup_mirroring().

Trond Myklebust b4f937cffa NFS: Don't run wake_up_bit() when nobody is waiting...
"perf lock" shows fairly heavy contention for the bit waitqueue locks
when doing an I/O heavy workload.
Use a bit to tell whether or not there has been contention for a lock
so that we can optimise away the bit waitqueue options in those cases.

Commit 8ef9b0b9e1 open-coded nfs_pgarray_set(), and left out the
initialization of the nfs_page_array's npages.  This mistake didn't show up
until testing with block layouts, and there shows that all pNFS reads
return -EIO.

Trond Myklebust 919e3bd9a8 NFS: Ensure we commit after writeback is complete
If the page cache is being flushed, then we want to ensure that we
do start a commit once the pages are done being flushed.
If we just wait until all I/O is done to that file, we can end up
livelocking until the balance_dirty_pages() mechanism puts its
foot down and forces I/O to stop.
So instead we do more or less the same thing that O_DIRECT does,
and set up a counter to tell us when the flush is done,

Benjamin Coddington 7d6ddf88c4 NFS: Add an iocounter wait function for async RPC tasks
By sleeping on a new NFS Unlock-On-Close waitqueue, rpc tasks may wait for
a lock context's iocounter to reach zero.  The rpc waitqueue is only woken
when the open_context has the NFS_CONTEXT_UNLOCK flag set in order to
mitigate spurious wake-ups for any iocounter reaching zero.

Benjamin Coddington 43b7d964ed NFS: Fix missing pg_cleanup after nfs_pageio_cond_complete()
Commit a7d42ddb30 ("nfs: add mirroring
support to pgio layer") moved pg_cleanup out of the path when there was
non-sequental I/O that needed to be flushed.  The result is that for
layouts that have more than one layout segment per file, the pg_lseg is not
cleared, so we can end up hitting the WARN_ON_ONCE(req_start >= seg_end) in
pnfs_generic_pg_test since the pg_lseg will be pointing to that
previously-flushed layout segment.

NeilBrown b184b5c38e NFS: remove l_pid field from nfs_lockowner
this field is not used in any important way and probably should
have been removed by

Commit: 8003d3c4aa ("nfs4: treat lock owners as opaque values")

which removed the pid argument from nfs4_get_lock_state.

Except in unusual and uninteresting cases, two threads with the same
->tgid will have the same ->files pointer, so keeping them both
for comparison brings no benefit.
