Keith Busch ddcb776286 NVMe: Fix divide-by-zero in nvme_trans_io_get_num_cmds
dev->max_hw_sectors may be zero to indicate the device has no limit on
the number of sectors.  nvme_trans_do_nvme_io() should use the software
limit, since this is guaranteed to be non-zero.

Keith Busch 4f5099af4f NVMe: IOCTL path RCU protect queue access
This adds rcu protected access to a queue in the nvme IOCTL path
to fix potential races between a surprise removal and queue usage in
nvme_submit_sync_cmd. The fix holds the rcu_read_lock() here to prevent
the nvme_queue from freeing while this path is executing so it can't
sleep, and so this path will no longer wait for a available command
id should they all be in use at the time a passthrough IOCTL request
is received.

Keith Busch 5a92e700af NVMe: RCU protected access to io queues
This adds rcu protected access to nvme_queue to fix a race between a
surprise removal freeing the queue and a thread with open reference on
a NVMe block device using that queue.

The queues do not need to be rcu protected during the initialization or
shutdown parts, so I've added a helper function for raw deferencing
to get around the sparse errors.

There is still a hole in the IOCTL path for the same problem, which is
fixed in a subsequent patch.

Keith Busch fb35e914b3 NVMe: Initialize device reference count earlier
If an NVMe device becomes ready but fails to create IO queues, the driver
creates a character device handle so the device can be managed. The
device reference count needs to be initialized before creating the
character device.

Rusty Russell fc4324b459 virtio-blk: base queue-depth on virtqueue ringsize or module param
Venkatash spake thus:

  virtio-blk set the default queue depth to 64 requests, which was
  insufficient for high-IOPS devices. Instead set the blk-queue depth to
  the device's virtqueue depth divided by two (each I/O requires at least
  two VQ entries).

But behold, Ted added a module parameter:

  Also allow the queue depth to be something which can be set at module
  load time or via a kernel boot-time parameter, for
  testing/benchmarking purposes.

And I rewrote it substantially, mainly to take
VIRTIO_RING_F_INDIRECT_DESC into account.

As QEMU sets the vq size for PCI to 128, Venkatash's patch wouldn't
have made a change.  This version does (since QEMU also offers
VIRTIO_RING_F_INDIRECT_DESC.

Jens Axboe 95363efde1 blk-mq: allow blk_mq_init_commands() to return failure
If drivers do dynamic allocation in the hardware command init
path, then we need to be able to handle and return failures.

And if they do allocations or mappings in the init command path,
then we need a cleanup function to free up that space at exit
time. So add blk_mq_free_commands() as the cleanup function.

This is required for the mtip32xx driver conversion to blk-mq.

Sam Bradshaw 5eb9291c36 mtip32xx: mtip_async_complete() bug fixes
This patch fixes 2 issues in the fast completion path:
1) Possible double completions / double dma_unmap_sg() calls due to lack
of atomicity in the check and subsequent dereference of the upper layer
callback function. Fixed with cmpxchg before unmap and callback.
2) Regression in unaligned IO constraining workaround for p420m devices.
Fixed by checking if IO is unaligned and using proper semaphore if so.

Alexander Gordeev 371ff93a72 cciss: Fallback to MSI rather than to INTx if MSI-X failed
Currently the driver falls back to INTx mode when MSI-X
initialization failed. This is a suboptimal behaviour
for chips that also support MSI. This update changes that
behaviour and falls back to MSI mode in case MSI-X mode
initialization failed.

Arnd Bergmann 106fd892bc swim3: fix interruptible_sleep_on race
interruptible_sleep_on is racy and going away. This replaces the one
caller in the swim3 driver with the equivalent race-free
wait_event_interruptible call. Since we're here already, this
also fixes the case where we get interrupted from atomic context,
which used to just spin in the loop.

Arnd Bergmann 7b8a3d22ba ataflop: fix sleep_on races
sleep_on() is inherently racy, and has been deprecated for a long time.
This fixes two instances in the atari floppy driver:

* fdc_wait/fdc_busy becomes an open-coded mutex. We cannot use the
  regular mutex since it gets released in interrupt context. The
  open-coded version using wait_event() and cmpxchg() is equivalent
  to the existing code but does the checks atomically, and we can
  now safely check the condition with irqs enabled.

* format_wait becomes a completion, which is the natural structure
  here. The format ioctl waits for the background task to either
  complete or abort.

This does not attempt to fix the preexisting bug of calling schedule
with local interrupts disabled.

Arnd Bergmann 9c552e1ddd DAC960: remove sleep_on usage
sleep_on and its variants are going away. The use of sleep_on() in
DAC960_V2_ExecuteUserCommand seems to be bogus because the command
by the time we get there, the command has completed already and
we just enter the timeout. Based on this interpretation, I concluded
that we can replace it with a simple msleep(1000) and rearrange the
code around it slightly.

The interruptible_sleep_on_timeout in DAC960_gam_ioctl seems equivalent
to the race-free version using wait_event_interruptible_timeout.
I left the driver to return -EINTR rather than -ERESTARTSYS to preserve
the timeout behavior.

Alexander Gordeev c94efe36e2 mtip32xx: Use pci_enable_msi() instead of pci_enable_msi_range()
Commit "mtip32xx: Use pci_enable_msix_range() instead of
pci_enable_msix()" was unnecessary, since pci_enable_msi()
function is not deprecated and is still preferable for
enabling the single MSI mode. This update reverts usage of
pci_enable_msi() function.

Besides, the changelog for that commit was bogus, since
mtip32xx driver uses MSI interrupt, not MSI-X.

Rusty Russell 5261b85e58 virtio_blk: don't crash, report error if virtqueue is broken.
A bad implementation of virtio might cause us to mark the virtqueue
broken: we'll dev_err() in that case, and the device is useless, but
let's not BUG_ON().

ENOMEM or ENOSPC implies the ring is full, and we should try again
later (-ENOMEM is documented to happen, but doesn't, as we fall
through to ENOSPC).

EIO means it's broken.

mtip_pci_probe() dumps the current CPU when loaded, but it does
so in a preemptible context. Hence smp_processor_id() correctly
warns:

Switch to raw_smp_processor_id(), since it's just informational
and persistent accuracy isn't important.

PREPARE_[DELAYED_]WORK() are being phased out.  They have few users
and a nasty surprise in terms of reentrancy guarantee as workqueue
considers work items to be different if they don't have the same work
function.

nvme_dev->reset_work is multiplexed with multiple work functions.
Introduce nvme_reset_workfn() which invokes nvme_dev->reset_workfn and
always use it as the work function and update the users to set the
->reset_workfn field instead of overriding the work function using
PREPARE_WORK().

It would probably be best to route this with other related updates
through the workqueue tree.

Compile tested.

PREPARE_[DELAYED_]WORK() are being phased out.  They have few users
and a nasty surprise in terms of reentrancy guarantee as workqueue
considers work items to be different if they don't have the same work
function.

floppy has been multiplexing floppy_work and fd_timer with multiple
work functions.  Introduce floppy_work_workfn() and fd_timer_workfn()
which invoke floppy_work_fn and fd_timer_fn respectively and always
use the two functions as the work functions and update the users to
set floppy_work_fn and fd_timer_fn instead of overriding work
functions using PREPARE_[DELAYED_]WORK().

It would probably be best to route this with other related updates
through the workqueue tree.

Lightly tested using qemu.

David Rientjes 668f9abbd4 mm: close PageTail race
Commit bf6bddf192 ("mm: introduce compaction and migration for
ballooned pages") introduces page_count(page) into memory compaction
which dereferences page->first_page if PageTail(page).

This results in a very rare NULL pointer dereference on the
aforementioned page_count(page).  Indeed, anything that does
compound_head(), including page_count() is susceptible to racing with
prep_compound_page() and seeing a NULL or dangling page->first_page
pointer.

This patch uses Andrea's implementation of compound_trans_head() that
deals with such a race and makes it the default compound_head()
implementation.  This includes a read memory barrier that ensures that
if PageTail(head) is true that we return a head page that is neither
NULL nor dangling.  The patch then adds a store memory barrier to
prep_compound_page() to ensure page->first_page is set.

This is the safest way to ensure we see the head page that we are
expecting, PageTail(page) is already in the unlikely() path and the
memory barriers are unfortunately required.

Hugetlbfs is the exception, we don't enforce a store memory barrier
during init since no race is possible.

Alexander Gordeev 46817769ed skd: Fix incomplete cleanup of MSI-X interrupt

the call to pci_disable_msix() is missed and the device is

Alexander Gordeev c5e3035c88 skd: Fix out of array boundary access
When enabling MSI-X, interrupts are requested for SKD_MAX_MSIX_COUNT
entries in skdev->msix_entries array, while the number of actually
allocated entries is skdev->msix_count. This might lead to an out of
boundary access in case number of allocated entries is less than
SKD_MAX_MSIX_COUNT. This update fixes the described misbehaviour.

Andreas Gruenbacher f597f6b8df drbd: Fix future possible NULL pointer dereference
Right now every resource has exactly one connection. But we are preparing
for dynamic connections. I.e. in the future thre can be resources without
connections.

However smatch points this out as 'variable dereferenced before check',
which is correct.

This issue was introduced in
drbd: get_one_status(): Iterate over resource->devices instead of connection->peer_devices

Andreas Gruenbacher 3b52beffc5 drbd: Turn drbd_printk() into a polymorphic macro
This allows drbd_alert(), drbd_err(), drbd_warn(), and drbd_info() to work for
a resource, device, or connection so that we don't have to introduce three
separate sets of macros for that.

The drbd_printk() macro itself is pretty ugly, but that problem is limited to
one place in the code.  Using drbd_printk() on an object type which it doesn't
understand results in an undefined drbd_printk_with_wrong_object_type symbol.

Andreas Gruenbacher d018017102 drbd: Remove the terrible DEV hack
DRBD was using dev_err() and similar all over the code; instead of having to
write dev_err(disk_to_dev(device->vdisk), ...) to convert a drbd_device into a
kernel device, a DEV macro was used which implicitly references the device
variable.  This is terrible; introduce separate drbd_err() and similar macros
with an explicit device parameter instead.

In a setup where a device (aka volume) can replicate to multiple peers and one
connection can be shared between multiple devices, we need separate objects to
represent devices on peer nodes and network connections.

Shlomo Pongratz d7790b928d block/null_blk: Fix completion processing from LIFO to FIFO
The completion queue is implemented using lockless list.

The llist_add is adds the events to the list head which is a push operation.
The processing of the completion elements is done by disconnecting all the
pushed elements and iterating over the disconnected list. The problem is
that the processing is done in reverse order w.r.t order of the insertion
i.e. LIFO processing. By reversing the disconnected list which is done in
linear time the desired FIFO processing is achieved.

David Vrabel 3661371701 xen-blkfront: handle backend CLOSED without CLOSING
Backend drivers shouldn't transistion to CLOSED unless the frontend is
CLOSED.  If a backend does transition to CLOSED too soon then the
frontend may not see the CLOSING state and will not properly shutdown.

So, treat an unexpected backend CLOSED state the same as CLOSING.

Roger Pau Monne 80bfa2f6e2 xen-blkif: drop struct blkif_request_segment_aligned
This was wrongly introduced in commit 402b27f9, the only difference
between blkif_request_segment_aligned and blkif_request_segment is
that the former has a named padding, while both share the same
memory layout.

Also correct a few minor glitches in the description, including for it
to no longer assume PAGE_SIZE == 4096.

Roger Pau Monne ef75341133 xen-blkback: fix memory leaks
I've at least identified two possible memory leaks in blkback, both
related to the shutdown path of a VBD:

- blkback doesn't wait for any pending purge work to finish before
  cleaning the list of free_pages. The purge work will call
  put_free_pages and thus we might end up with pages being added to
  the free_pages list after we have emptied it. Fix this by making
  sure there's no pending purge work before exiting
  xen_blkif_schedule, and moving the free_page cleanup code to
  xen_blkif_free.
- blkback doesn't wait for pending requests to end before cleaning
  persistent grants and the list of free_pages. Again this can add
  pages to the free_pages list or persistent grants to the
  persistent_gnts red-black tree. Fixed by moving the persistent
  grants and free_pages cleanup code to xen_blkif_free.

Also, add some checks in xen_blkif_free to make sure we are cleaning
everything.

Zoltan Kiss 08ece5bb23 xen/grant-table: Avoid m2p_override during mapping
The grant mapping API does m2p_override unnecessarily: only gntdev needs it,
for blkback and future netback patches it just cause a lock contention, as
those pages never go to userspace. Therefore this series does the following:
- the original functions were renamed to __gnttab_[un]map_refs, with a new
  parameter m2p_override
- based on m2p_override either they follow the original behaviour, or just set
  the private flag and call set_phys_to_machine
- gnttab_[un]map_refs are now a wrapper to call __gnttab_[un]map_refs with
  m2p_override false
- a new function gnttab_[un]map_refs_userspace provides the old behaviour

It also removes a stray space from page.h and change ret to 0 if
XENFEAT_auto_translated_physmap, as that is the only possible return value
there.

Minchan Kim e46e33152e zram: remove zram->lock in read path and change it with mutex
Finally, we separated zram->lock dependency from 32bit stat/ table
handling so there is no reason to use rw_semaphore between read and
write path so this patch removes the lock from read path totally and
changes rw_semaphore with mutex.  So, we could do

Now:

The below data proves mixed workload performs well 11 times and there is
also enhance on write-write path because current rw-semaphore doesn't

Write-related tests perform better (from 61% to 1058%) but read path has
good/bad(from -2.22% to 1.45%) but they are all marginal within stddev.

Minchan Kim f614a9f48d zram: remove workqueue for freeing removed pending slot
Commit a0c516cbfc ("zram: don't grab mutex in zram_slot_free_noity")
introduced free request pending code to avoid scheduling by mutex under
spinlock and it was a mess which made code lenghty and increased
overhead.

Now, we don't need zram->lock any more to free slot so this patch
reverts it and then, tb_lock should protect it.

Minchan Kim 92967471b6 zram: introduce zram->tb_lock
Currently, the zram table is protected by zram->lock but it's rather
coarse-grained lock and it makes hard for scalibility.

Let's use own rwlock instead of depending on zram->lock.  This patch
adds new locking so obviously, it would make slow but this patch is just
prepartion for removing coarse-grained rw_semaphore(ie, zram->lock)
which is hurdle about zram scalability.

Final patch in this patchset series will remove the lock from read-path
and change rw_semaphore with mutex in write path.  With bonus, we could
drop pending slot free mess in next patch.

Some of fields in zram->stats are protected by zram->lock which is
rather coarse-grained so let's use atomic operation without explict
locking.

This patch is ready for removing dependency of zram->lock in read path
which is very coarse-grained rw_semaphore.  Of course, this patch adds
new atomic operation so it might make slow but my 12CPU test couldn't
spot any regression.  All gain/lose is marginal within stddev.

Minchan Kim 9b353db16d zram: delay pending free request in read path
Sergey reported we don't need to handle pending free request every I/O
so that this patch removes it in read path while we remain it in write
path.

Let's consider below example.

Swap subsystem ask to zram "A" block free by swap_slot_free_notify but
zram had been pended it without real freeing.  Swap subsystem allocates
"A" block for new data but request pended for a long time just handled
and zram blindly free new data on the "A" block.  :(

That's why we couldn't remove handle pending free request right before
zram-write.

Minchan Kim da4a04126b zram: fix race between reset and flushing pending work
Dan and Sergey reported that there is a racy between reset and flushing
of pending work so that it could make oops by freeing zram->meta in
reset while zram_slot_free can access zram->meta if new request is
adding during the race window.

This patch moves flush after taking init_lock so it prevents new request
so that it closes the race.

Minchan Kim cd67e10ac6 zram: promote zram from staging
Zram has lived in staging for a LONG LONG time and have been
fixed/improved by many contributors so code is clean and stable now.  Of
course, there are lots of product using zram in real practice.

The major TV companys have used zram as swap since two years ago and
recently our production team released android smart phone with zram
which is used as swap, too and recently Android Kitkat start to use zram
for small memory smart phone.  And there was a report Google released
their ChromeOS with zram, too and cyanogenmod have been used zram long
time ago.  And I heard some disto have used zram block device for tmpfs.
In addition, I saw many report from many other peoples.  For example,
Lubuntu start to use it.

The benefit of zram is very clear.  With my experience, one of the
benefit was to remove jitter of video application with backgroud memory
pressure.  It would be effect of efficient memory usage by compression
but more issue is whether swap is there or not in the system.  Recent
mobile platforms have used JAVA so there are many anonymous pages.  But
embedded system normally are reluctant to use eMMC or SDCard as swap
because there is wear-leveling and latency issues so if we do not use
swap, it means we can't reclaim anoymous pages and at last, we could
encounter OOM kill.  :(

Although we have real storage as swap, it was a problem, too.  Because
it sometime ends up making system very unresponsible caused by slow swap
storage performance.

Other uses case is to use zram for block device.  Zram is block device
so anyone can format the block device and mount on it so some guys on
the internet start zram as /var/tmp.

Some users need more than 64 partitions per device.  Rather than simply
increasing the number of partitions, switch to the dynamic partition
allocation scheme.

This means that minor numbers are not stable across boots, but since major
numbers aren't either, I cannot see this being a significant problem.

Keith Busch 4d11542070 NVMe: Async IO queue deletion
This attempts to delete all IO queues at the same time asynchronously on
shutdown. This is necessary for a present device that is not responding;
a shutdown operation previously would take 2 minutes per queue-pair
to timeout before moving on to the next queue, making a device removal
appear to take a very long time or "hung" as reported by users.

In the previous worst case, a removal may be stuck forever until a kill
signal is given if there are more than 32 queue pairs since it would run
out of admin command IDs after over an hour of timed out sync commands
(admin queue depth is 64).

This patch will wait for the admin command timeout for all commands to
complete, so the worst case now for an unresponsive controller is 60
seconds, though that still seems like a long time.

Since this adds another way to take queues offline, some duplicate code
resulted so I moved these into more convienient functions.

Keith Busch 0e53d18051 NVMe: Surprise removal handling
This adds checks to see if the nvme pci device was removed. The check
reads the status register for the value of -1, which it should never be
unless the device is no longer present.

If a user performs a surprise removal on an nvme device, the driver will
be notified either by the pci driver remove callback if the platform's
slot is capable of this event, or via reading the device BAR status
register, which will indicate controller failure and trigger a reset.

Either way, the device is not present so all outstanding commands would
timeout. This will not send queue deletion commands to a drive that
isn't present and fail after ioremap, significantly speeding up surprise
removal; previously this took over 2 minutes per IO queue pair created,
but this will complete removing the device within a few seconds.

Ilya Dryomov 22116525ba libceph: start using oloc abstraction
Instead of relying on pool fields in ceph_file_layout (for mapping) and
ceph_pg (for enconding), start using ceph_object_locator (oloc)
abstraction.  Note that userspace oloc currently consists of pool, key,
nspace and hash fields, while this one contains only a pool.  This is
OK, because at this point we only send (i.e. encode) olocs and never
have to receive (i.e. decode) them.

This makes keeping a copy of ceph_file_layout in every osd request
unnecessary, so ceph_osd_request::r_file_layout field is nuked.

Sam Bradshaw 26d580575b mtip32xx: Correctly handle security locked condition
If power is removed during a secure erase, the drive will end up in a
security locked condition.  This patch causes the driver to identify,
log, and flag the security lock state.  IOs are prevented from
submission to the drive until the locked state is addressed with a
secure erase.

Bumped version number to reflect this capability.

Sam Bradshaw 188b9f49d4 mtip32xx: Make SGL container per-command to eliminate high order dma allocation
The mtip32xx driver makes a high order dma memory allocation to store a
command index table, some dedicated buffers, and a command header & SGL
blob.  This allocation can fail with a surprise insert under low &
fragmented memory conditions.

This patch breaks these regions up into separate low order allocations
and increases the maximum number of segments a single command SGL can
have.  We wanted to allow at least 256 segments for 1 MB direct IO.
Since the command header occupies the first 0x80 bytes of the SGL blob,
that meant we needed two 4k pages to contain the header and SGL.  The
two pages allow up to 504 SGL segments.

Jiri Kosina 7b7b68bba5 floppy: bail out in open() if drive is not responding to block0 read
In case reading of block 0 during open() fails, it is not the right thing
to let open() succeed.

Fix this by introducing FD_OPEN_SHOULD_FAIL_BIT flag, and setting it in
case the bio callback encounters an error while trying to read block 0.

As a bonus, this works around certain broken userspace (blkid), which is
not able to properly handle read()s returning IO errors. Hence be nice to
those, and bail out during open() already; if block 0 is not readable,
read()s are not going to provide any meaningful data anyway.

Ming Lei 518d00b749 block: null_blk: fix queue leak inside removing device
When queue_mode is NULL_Q_MQ and null_blk is being removed,
blk_cleanup_queue() isn't called to cleanup queue, so the queue
allocated won't be freed.

This patch calls blk_cleanup_queue() for MQ to drain all pending
requests first and release the reference counter of queue kobject, then
blk_mq_free_queue() will be called in queue kobject's release handler
when queue kobject's reference counter drops to zero.

   native environment).
 - if running in HVM, check if user wanted 'xen_emul_unplug=never',

Ilya Dryomov 7e513d4366 rbd: enable extended devt in single-major mode
If single-major device number allocation scheme is turned on, instead
of reserving 256 minors per device, which imposes a limit of 4096
images mapped at once, reserve 16 minors per device and enable extended
devt feature.  This results in a theoretical limit of 65536 images
mapped at once, and still allows to have more than 15 partititions:
partitions starting with 16th are mapped under major 259 (Block
Extended Major):

Ilya Dryomov 9b60e70b3b rbd: add support for single-major device number allocation scheme
Currently each rbd device is allocated its own major number, which
leads to a hard limit of 230-250 images mapped at once.  This commit
adds support for a new single-major device number allocation scheme,
which is hidden behind a new single_major boolean module parameter and
is disabled by default for backwards compatibility reasons.  (Old
userspace cannot correctly unmap images mapped under single-major
scheme and would essentially just unmap a random image, if that.)

The new limit is 4096 images mapped at once, and it comes from the fact
that, as before, 256 minor numbers are reserved for each mapping.
(A follow-up commit changes the number of minors reserved and the way
we deal with partitions over that number.)

If single_major is set to true, two new sysfs interfaces show up:
/sys/bus/rbd/{add,remove}_single_major.  These are to be used instead
of /sys/bus/rbd/{add,remove}, which are disabled for backwards
compatibility reasons outlined above.

Ilya Dryomov f8a22fc238 rbd: switch to ida for rbd id assignments
Currently rbd ids are allocated using an atomic variable that keeps
track of the highest id currently in use and each new id is simply one
more than the value of that variable.  That's nice and cheap, but it
does mean that rbd ids are allowed to grow boundlessly, and, more
importantly, it's completely unpredictable.  So, in preparation for
single-major device number allocation scheme, which is going to
establish and rely on a constant mapping between rbd ids and device
numbers, switch to ida for rbd id assignments.

Matias Bjørling fc1bc35443 null_blk: support submit_queues on use_per_node_hctx
In the case of both the submit_queues param and use_per_node_hctx param
are used. We limit the number af submit_queues to the number of online
nodes.

If the submit_queues is a multiple of nr_online_nodes, its trivial. Simply map
them to the nodes. For example: 8 submit queues are mapped as node0[0,1],
node1[2,3], ...
If uneven, we are left with an uneven number of submit_queues that must be
mapped. These are mapped toward the first node and onward. E.g. 5
submit queues mapped onto 4 nodes are mapped as node0[0,1], node1[2], ...

Keith Busch 9a6b94584d NVMe: Device resume error handling
Adds controller error handling on resume power management. If the device
fails to initialize, the device is queued for a reset. If the reset fails,
a thread is spawned to remove the pci device.

If the device resumes as "busy", the device is responding to admin
commands but will not create IO queues. In this case, we need to remove
the gendisks and free the IO queues since they can't be used and may be
holding bios in their lists.

From testing, the dma pools require a pci device so this had to change
the pci driver 'remove' to release the dma resources in line with that
call instead of after all references to the device are released.

Kent Overstreet 20d0189b10 block: Introduce new bio_split()
The new bio_split() can split arbitrary bios - it's not restricted to
single page bios, like the old bio_split() (previously renamed to
bio_pair_split()). It also has different semantics - it doesn't allocate
a struct bio_pair, leaving it up to the caller to handle completions.

Then convert the existing bio_pair_split() users to the new bio_split()
- and also nvme, which was open coding bio splitting.

(We have to take that BUG_ON() out of bio_integrity_trim() because this
bio_split() needs to use it, and there's no reason it has to be used on
bios marked as cloned; BIO_CLONED doesn't seem to have clearly
documented semantics anyways.)

Kent Overstreet 458b76ed2f block: Kill bio_segments()/bi_vcnt usage
When we start sharing biovecs, keeping bi_vcnt accurate for splits is
going to be error prone - and unnecessary, if we refactor some code.

So bio_segments() has to go - but most of the existing users just needed
to know if the bio had multiple segments, which is easier - add a
bio_multiple_segments() for them.

(Two of the current uses of bio_segments() are going to go away in a
couple patches, but the current implementation of bio_segments() is
unsafe as soon as we start doing driver conversions for immutable
biovecs - so implement a dumb version for bisectability, it'll go away
in a couple patches)

Kent Overstreet 4550dd6c6b block: Immutable bio vecs
This adds a mechanism by which we can advance a bio by an arbitrary
number of bytes without modifying the biovec: bio->bi_iter.bi_bvec_done
indicates the number of bytes completed in the current bvec.

Various driver code still needs to be updated to not refer to the bvec
directly before we can use this for interesting things, like efficient
bio splitting.

Kent Overstreet 7988613b0e block: Convert bio_for_each_segment() to bvec_iter
More prep work for immutable biovecs - with immutable bvecs drivers
won't be able to use the biovec directly, they'll need to use helpers
that take into account bio->bi_iter.bi_bvec_done.

This updates callers for the new usage without changing the
implementation yet.

Kent Overstreet a4ad39b1d1 block: Convert bio_iovec() to bvec_iter
For immutable biovecs, we'll be introducing a new bio_iovec() that uses
our new bvec iterator to construct a biovec, taking into account
bvec_iter->bi_bvec_done - this patch updates existing users for the new
usage.

Some of the existing users really do need a pointer into the bvec array
- those uses are all going to be removed, but we'll need the
functionality from immutable to remove them - so for now rename the
existing bio_iovec() -> __bio_iovec(), and it'll be removed in a couple
patches.
