Michal Hocko 390511e147 mm, memory_hotplug: drop arch_free_nodedata
Prior to "mm: handle uninitialized numa nodes gracefully" memory hotplug
used to allocate pgdat when memory has been added to a node
(hotadd_init_pgdat) arch_free_nodedata has been only used in the failure
path because once the pgdat is exported (to be visible by NODA_DATA(nid))
it cannot really be freed because there is no synchronization available
for that.

pgdat is allocated for each possible nodes now so the memory hotplug
doesn't need to do the ever use arch_free_nodedata so drop it.

This patch doesn't introduce any functional change.

Michal Hocko 09f49dca57 mm: handle uninitialized numa nodes gracefully
We have had several reports [1][2][3] that page allocator blows up when an
allocation from a possible node is requested.  The underlying reason is
that NODE_DATA for the specific node is not allocated.

NUMA specific initialization is arch specific and it can vary a lot.  E.g.
x86 tries to initialize all nodes that have some cpu affinity (see
init_cpu_to_node) but this can be insufficient because the node might be
cpuless for example.

One way to address this problem would be to check for !node_online nodes
when trying to get a zonelist and silently fall back to another node.
That is unfortunately adding a branch into allocator hot path and it
doesn't handle any other potential NODE_DATA users.

This patch takes a different approach (following a lead of [3]) and it pre
allocates pgdat for all possible nodes in an arch indipendent code -
free_area_init.  All uninitialized nodes are treated as memoryless nodes.
node_state of the node is not changed because that would lead to other
side effects - e.g.  sysfs representation of such a node and from past
discussions [4] it is known that some tools might have problems digesting
that.

Newly allocated pgdat only gets a minimal initialization and the rest of
the work is expected to be done by the memory hotplug - hotadd_new_pgdat
(renamed to hotadd_init_pgdat).

generic_alloc_nodedata is changed to use the memblock allocator because
neither page nor slab allocators are available at the stage when all
pgdats are allocated.  Hotplug doesn't allocate pgdat anymore so we can
use the early boot allocator.  The only arch specific implementation is
ia64 and that is changed to use the early allocator as well.

Michal Hocko e930d99971 mm, memory_hotplug: make arch_alloc_nodedata independent on CONFIG_MEMORY_HOTPLUG
Patch series "mm, memory_hotplug: handle unitialized numa node gracefully".

The core of the fix is patch 2 which also links existing bug reports.  The
high level goal is to have all possible numa nodes have their pgdat
allocated and initialized so

	for_each_possible_node(nid)
		NODE_DATA(nid)

will never return garbage.  This has proven to be problem in several
places when an offline numa node is used for an allocation just to realize
that node_data and therefore allocation fallback zonelists are not
initialized and such an allocation request blows up.

There were attempts to address that by checking node_online in several
places including the page allocator.  This patchset approaches the problem
from a different perspective and instead of special casing, which just
adds a runtime overhead, it allocates pglist_data for each possible node.
This can add some memory overhead for platforms with high number of
possible nodes if they do not contain any memory.  This should be a rather
rare configuration though.

Patches 1 and 3-6 are mostly cleanups.  The patchset has been reviewed by
Rafael (thanks!) and the core fix tested by Rafael and Alexey (thanks to
both).  David has tested as per instructions above and hasn't found any
fallouts in the memory hotplug scenarios.

This is a preparatory patch and it doesn't introduce any functional
change.  It merely pulls out arch_alloc_nodedata (and co) outside of
CONFIG_MEMORY_HOTPLUG because the following patch will need to call this
from the generic MM code.

Fix IA64 discontig.c Section mismatch warnings.

When CONFIG_SPARSEMEM=y and CONFIG_MEMORY_HOTPLUG=y, the functions
computer_pernodesize() and scatter_node_data() should not be marked as
__meminit because they are needed after init, on any memory hotplug
event.  Also, early_nr_cpus_node() is called by compute_pernodesize(),
so early_nr_cpus_node() cannot be __meminit either.

Mike Rapoport 1f11212997 ia64: split virtual map initialization out of paging_init()
For both FLATMEM and DISCONTIGMEM/SPARSEMEM the virtual map initialization
is spread over paging_init() for no good reason.

Split out the bits related to virtual map initialization to a helper
functions, one for FLATMEM and another for !FLATMEM configurations.

Mike Rapoport b90b554768 ia64: discontig: paging_init(): remove local max_pfn calculation
The maximal PFN in the system is calculated during find_memory() time and
it is stored at max_low_pfn then.

Use this value in paging_init() and remove the redundant detection of
max_pfn in that function.

Mike Rapoport c89ab04feb mm/sparse: cleanup the code surrounding memory_present()
After removal of CONFIG_HAVE_MEMBLOCK_NODE_MAP we have two equivalent
functions that call memory_present() for each region in memblock.memory:
sparse_memory_present_with_active_regions() and membocks_present().

Moreover, all architectures have a call to either of these functions
preceding the call to sparse_init() and in the most cases they are called
one after the other.

Mark the regions from memblock.memory as present during sparce_init() by
making sparse_init() call memblocks_present(), make memblocks_present()
and memory_present() functions static and remove redundant
sparse_memory_present_with_active_regions() function.

Also remove no longer required HAVE_MEMORY_PRESENT configuration option.

Anshuman Khandual 1d9cfee753 mm/sparsemem: enable vmem_altmap support in vmemmap_populate_basepages()
Patch series "arm64: Enable vmemmap mapping from device memory", v4.

This series enables vmemmap backing memory allocation from device memory
ranges on arm64.  But before that, it enables vmemmap_populate_basepages()
and vmemmap_alloc_block_buf() to accommodate struct vmem_altmap based
alocation requests.

This patch (of 3):

vmemmap_populate_basepages() is used across platforms to allocate backing
memory for vmemmap mapping.  This is used as a standard default choice or
as a fallback when intended huge pages allocation fails.  This just
creates entire vmemmap mapping with base pages (PAGE_SIZE).

On arm64 platforms, vmemmap_populate_basepages() is called instead of the
platform specific vmemmap_populate() when ARM64_SWAPPER_USES_SECTION_MAPS
is not enabled as in case for ARM64_16K_PAGES and ARM64_64K_PAGES configs.

At present vmemmap_populate_basepages() does not support allocating from
driver defined struct vmem_altmap while trying to create vmemmap mapping
for a device memory range.  It prevents ARM64_16K_PAGES and
ARM64_64K_PAGES configs on arm64 from supporting device memory with
vmemap_altmap request.

This enables vmem_altmap support in vmemmap_populate_basepages() unlocking
device memory allocation for vmemap mapping on arm64 platforms with 16K or
64K base page configs.

Each architecture should evaluate and decide on subscribing device memory
based base page allocation through vmemmap_populate_basepages().  Hence
lets keep it disabled on all archs in order to preserve the existing
semantics.  A subsequent patch enables it on arm64.

Mike Rapoport ca15ca406f mm: remove unneeded includes of <asm/pgalloc.h>
Patch series "mm: cleanup usage of <asm/pgalloc.h>"

Most architectures have very similar versions of pXd_alloc_one() and
pXd_free_one() for intermediate levels of page table.  These patches add
generic versions of these functions in <asm-generic/pgalloc.h> and enable
use of the generic functions where appropriate.

In addition, functions declared and defined in <asm/pgalloc.h> headers are
used mostly by core mm and early mm initialization in arch and there is no
actual reason to have the <asm/pgalloc.h> included all over the place.
The first patch in this series removes unneeded includes of
<asm/pgalloc.h>

In the end it didn't work out as neatly as I hoped and moving
pXd_alloc_track() definitions to <asm-generic/pgalloc.h> would require
unnecessary changes to arches that have custom page table allocations, so
I've decided to move lib/ioremap.c to mm/ and make pgalloc-track.h local
to mm/.

This patch (of 8):

In most cases <asm/pgalloc.h> header is required only for allocations of
page table memory.  Most of the .c files that include that header do not
use symbols declared in <asm/pgalloc.h> and do not require that header.

As for the other header files that used to include <asm/pgalloc.h>, it is
possible to move that include into the .c file that actually uses symbols
from <asm/pgalloc.h> and drop the include from the header file.

The process was somewhat automated using

	sed -i -E '/[<"]asm\/pgalloc\.h/d' \
                $(grep -L -w -f /tmp/xx \
                        $(git grep -E -l '[<"]asm/pgalloc\.h'))

where /tmp/xx contains all the symbols defined in
arch/*/include/asm/pgalloc.h.

Kees Cook 3f649ab728 treewide: Remove uninitialized_var() usage
Using uninitialized_var() is dangerous as it papers over real bugs[1]
(or can in the future), and suppresses unrelated compiler warnings
(e.g. "unused variable"). If the compiler thinks it is uninitialized,
either simply initialize the variable or make compiler changes.

In preparation for removing[2] the[3] macro[4], remove all remaining
needless uses with the following script:

for x86_64, i386, arm64, arm, powerpc, powerpc64le, s390x, mips, sparc64,
alpha, and m68k.

Mike Rapoport 9691a071aa mm: use free_area_init() instead of free_area_init_nodes()
free_area_init() has effectively became a wrapper for
free_area_init_nodes() and there is no point of keeping it.  Still
free_area_init() name is shorter and more general as it does not imply
necessity to initialize multiple nodes.

Rename free_area_init_nodes() to free_area_init(), update the callers and
drop old version of free_area_init().

Mike Rapoport d80db5c1ed ia64: add checks for the return value of memblock_alloc*()
Add panic() calls if memblock_alloc*() returns NULL.

Most of the changes are simply addition of

		panic();

statements after the calls to memblock_alloc*() variants.

This patch (of 2):

At present there are multiple places where invalid node number is
encoded as -1.  Even though implicitly understood it is always better to
have macros in there.  Replace these open encodings for an invalid node
number with the global macro NUMA_NO_NODE.  This helps remove NUMA
related assumptions like 'invalid node' from various places redirecting
them to a common definition.

Mike Rapoport f62800992e ia64: switch to NO_BOOTMEM
Since ia64 already uses memblock to register available physical memory it
is only required to move the calls to register_active_ranges() that wrap
memblock_add_node() earlier and replace bootmem memory reservations with
memblock_reserve(). Of course, all the code that find the place to put the
bootmem bitmap is removed.

When EFI memory map is traversed to determine the extents of each node, the
minimal and maximal PFNs are stored in the bootmem_data structures. The

Christoph Hellwig d5c23ebf1b ia64: replace ZONE_DMA with ZONE_DMA32
ia64 uses ZONE_DMA for allocations below 32-bits.  These days we
name the zone for that ZONE_DMA32, which will allow to use the

Greg Kroah-Hartman b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   SPDX license identifier                            # files

   and resulted in the first patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

Mel Gorman aec6a8889a mm, show_mem: remove SHOW_MEM_FILTER_PAGE_COUNT
Commit 4b59e6c473 ("mm, show_mem: suppress page counts in
non-blockable contexts") introduced SHOW_MEM_FILTER_PAGE_COUNT to
suppress PFN walks on large memory machines.  Commit c78e93630d ("mm:
do not walk all of system memory during show_mem") avoided a PFN walk in
the generic show_mem helper which removes the requirement for
SHOW_MEM_FILTER_PAGE_COUNT in that case.

This patch removes PFN walkers from the arch-specific implementations
that report on a per-node or per-zone granularity.  ARM and unicore32
still do a PFN walk as they report memory usage on each bank which is a
much finer granularity where the debugging information may still be of
use.  As the remaining arches doing PFN walks have relatively small
amounts of memory, this patch simply removes SHOW_MEM_FILTER_PAGE_COUNT.

Paul Gortmaker ccce9bb83e [IA64] Delete __cpuinit usage from all ia64 users
The __cpuinit type of throwaway sections might have made sense
some time ago when RAM was more constrained, but now the savings
do not offset the cost and complications.  For example, the fix in
commit 5e427ec2d0 ("x86: Fix bit corruption at CPU resume time")
is a good example of the nasty type of bugs that can be created
with improper use of the various __init prefixes.

After a discussion on LKML[1] it was decided that cpuinit should go
the way of devinit and be phased out.  Once all the users are gone,
we can then finally remove the macros themselves from linux/init.h.

This removes all the ia64 uses of the __cpuinit macros.

Johannes Weiner 0aad818b2d sparse-vmemmap: specify vmemmap population range in bytes
The sparse code, when asking the architecture to populate the vmemmap,
specifies the section range as a starting page and a number of pages.

This is an awkward interface, because none of the arch-specific code
actually thinks of the range in terms of 'struct page' units and always
translates it to bytes first.

In addition, later patches mix huge page and regular page backing for
the vmemmap.  For this, they need to call vmemmap_populate_basepages()
on sub-section ranges with PAGE_SIZE and PMD_SIZE in mind.  But these
are not necessarily multiples of the 'struct page' size and so this unit
is too coarse.

Just translate the section range into bytes once in the generic sparse
code, then pass byte ranges down the stack.

David Rientjes 4b59e6c473 mm, show_mem: suppress page counts in non-blockable contexts
On large systems with a lot of memory, walking all RAM to determine page
types may take a half second or even more.

In non-blockable contexts, the page allocator will emit a page allocation
failure warning unless __GFP_NOWARN is specified.  In such contexts, irqs
are typically disabled and such a lengthy delay may even result in NMI
watchdog timeouts.

To fix this, suppress the page walk in such contexts when printing the
page allocation failure warning.

Tang Chen 0197518cd3 memory-hotplug: remove memmap of sparse-vmemmap

Yasuaki Ishimatsu 46723bfa54 memory-hotplug: implement register_page_bootmem_info_section of sparse-vmemmap
For removing memmap region of sparse-vmemmap which is allocated bootmem,
memmap region of sparse-vmemmap needs to be registered by
get_page_bootmem().  So the patch searches pages of virtual mapping and
registers the pages by get_page_bootmem().

NOTE: register_page_bootmem_memmap() is not implemented for ia64,
      ppc, s390, and sparc.  So introduce CONFIG_HAVE_BOOTMEM_INFO_NODE
      and revert register_page_bootmem_info_node() when platform doesn't
      support it.

      It's implemented by adding a new Kconfig option named
      CONFIG_HAVE_BOOTMEM_INFO_NODE, which will be automatically selected
      by memory-hotplug feature fully supported archs(currently only on
      x86_64).

      Since we have 2 config options called MEMORY_HOTPLUG and
      MEMORY_HOTREMOVE used for memory hot-add and hot-remove separately,
      and codes in function register_page_bootmem_info_node() are only
      used for collecting infomation for hot-remove, so reside it under
      MEMORY_HOTREMOVE.

      Besides page_isolation.c selected by MEMORY_ISOLATION under
      MEMORY_HOTPLUG is also such case, move it too.

David Rientjes 7bf02ea22c arch, mm: filter disallowed nodes from arch specific show_mem functions
Architectures that implement their own show_mem() function did not pass
the filter argument to show_free_areas() to appropriately avoid emitting
the state of nodes that are disallowed in the current context.  This patch
now passes the filter argument to show_free_areas() so those nodes are now
avoided.

This patch also removes the show_free_areas() wrapper around
__show_free_areas() and converts existing callers to pass an empty filter.

ia64 emits additional information for each node, so skip_free_areas_zone()
must be made global to filter disallowed nodes and it is converted to use
a nid argument rather than a zone for this use case.

David Rientjes b2b755b5f1 lib, arch: add filter argument to show_mem and fix private implementations
Commit ddd588b5dd ("oom: suppress nodes that are not allowed from
meminfo on oom kill") moved lib/show_mem.o out of lib/lib.a, which
resulted in build warnings on all architectures that implement their own
versions of show_mem():

	lib/lib.a(show_mem.o): In function `show_mem':
	show_mem.c:(.text+0x1f4): multiple definition of `show_mem'
	arch/sparc/mm/built-in.o:(.text+0xd70): first defined here

The fix is to remove __show_mem() and add its argument to show_mem() in
all implementations to prevent this breakage.

Architectures that implement their own show_mem() actually don't do
anything with the argument yet, but they could be made to filter nodes
that aren't allowed in the current context in the future just like the
generic implementation.

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Rusty Russell dd17c8f729 percpu: remove per_cpu__ prefix.
Now that the return from alloc_percpu is compatible with the address
of per-cpu vars, it makes sense to hand around the address of per-cpu
variables.  To make this sane, we remove the per_cpu__ prefix we used
created to stop people accidentally using these vars directly.

Now we have sparse, we can use that (next patch).

Tejun Heo 877105cc49 percpu: make percpu symbols in ia64 unique
This patch updates percpu related symbols in ia64 such that percpu
symbols are unique and don't clash with local symbols.  This serves
two purposes of decreasing the possibility of global percpu symbol
collision and allowing dropping per_cpu__ prefix from percpu symbols.

Partly based on Rusty Russell's "alloc_percpu: rename percpu vars

Tejun Heo 52594762a3 ia64: convert to dynamic percpu allocator
Unlike other archs, ia64 reserves space for percpu areas during early
memory initialization.  These areas occupy a contiguous region indexed
by cpu number on contiguous memory model or are grouped by node on
discontiguous memory model.

As allocation and initialization are done by the arch code, all that
setup_per_cpu_areas() needs to do is communicating the determined
layout to the percpu allocator.  This patch implements
setup_per_cpu_areas() for both contig and discontig memory models and
drops HAVE_LEGACY_PER_CPU_AREA.

Please note that for contig model, the allocation itself is modified
only to allocate for possible cpus instead of NR_CPUS.  As dynamic
percpu allocator can handle non-direct mapping, there's no reason to
allocate memory for cpus which aren't possible.

Tejun Heo 36886478f5 ia64: allocate percpu area for cpu0 like percpu areas for other cpus
cpu0 used special percpu area reserved by the linker, __cpu0_per_cpu,
which is set up early in boot by head.S.  However, this doesn't
guarantee that the area will be on the same node as cpu0 and the
percpu area for cpu0 ends up very far away from percpu areas for other
cpus which cause problems for congruent percpu allocator.

This patch makes percpu area initialization allocate percpu area for
cpu0 like any other cpus and copy it from __cpu0_per_cpu which now
resides in the __init area.  This means that for cpu0, percpu area is
first setup at __cpu0_per_cpu early by head.S and then moved to an
area in the linear mapping during memory initialization and it's not
allowed to take a pointer to percpu variables between head.S and
memory initialization.

Tejun Heo 126b3fcdec ia64: don't alias VMALLOC_END to vmalloc_end
If CONFIG_VIRTUAL_MEM_MAP is enabled, ia64 defines macro VMALLOC_END
as unsigned long variable vmalloc_end which is adjusted to prepare
room for vmemmap.  This becomes probnlematic if a local variables
vmalloc_end is defined in some function (not very unlikely) and
VMALLOC_END is used in the function - the function thinks its
referencing the global VMALLOC_END value but would be referencing its
own local vmalloc_end variable.

There's no reason VMALLOC_END should be a macro.  Just define it as an
unsigned long variable if CONFIG_VIRTUAL_MEM_MAP is set to avoid nasty
surprises.

Ken'ichi Ohmichi aca14f3310 [IA64] fix the difference between node_mem_map and node_start_pfn
makedumpfile[1] cannot run on ia64 discontigmem kernel, because the member
node_mem_map of struct pgdat_list has invalid value.  This patch fixes it.

node_start_pfn shows the start pfn of each node, and node_mem_map should
point 'struct page' of each node's node_start_pfn.  On my machine, node0's
node_start_pfn shows 0x400 and its node_mem_map points 0xa0007fffbf000000.
 This address is the same as vmem_map, so the node_mem_map points 'struct
page' of pfn 0, even if its node_start_pfn shows 0x400.

The cause is due to the round down of min_pfn in count_node_pages() and
node0's node_mem_map points 'struct page' of inactive pfn (0x0).  This
patch fixes it.
