On larger installations it is useful to disable automatic LUN scanning,
and only add the required LUNs via udev rules. This can speed up bootup
dramatically.
This patch introduces a new scan module parameter value 'manual', which
works like 'none', but can be overridden by setting the 'rescan' value
from scsi_scan_target to 'SCSI_SCAN_MANUAL'. And it updates all
relevant callers to set the 'rescan' value to 'SCSI_SCAN_MANUAL' if
invoked via the 'scan' option in sysfs.

Just remove the uncommented dead MTRR code then.

a) Take advantage of PAT when available
b) Help bury MTRR code away, MTRR is architecture specific and on

c) Help with the goal of eventually using _PAGE_CACHE_UC over

The I2O layer deals with a technology that to say the least didn't catch on
in the market.
The only relevant products are some of the AMI MegaRAID - which supported I2O
and its native mode (The native mode is faster and runs on Linux), an
obscure crypto ethernet card that's now so many years out of date nobody
would use it, the old DPT controllers, which speak their own dialect and
have their own driver - and ermm.. thats about it.
We also know the code isn't in good shape as recently a patch was proposed
and queried as buggy, which in turn showed the existing code was broken
already by prior "clean up" and nobody had noticed that either.
It's coding style robot code nothing more. Like some forgotten corridor
cleaned relentlessly by a lost Roomba but where no user has trodden in years.

Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth. For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.
Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway. The one other case looks
broken, but I've kept the behavior as-is for now.
Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.
Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

we do an atomic_inc_return early on to grab our slot in the queue,
and if necessary decrement it after finishing all checks.
Unlike the host and target busy counters this doesn't allow us to avoid the
queue_lock in the request_fn due to the way the interface works, but it'll
allow us to prepare for using the blk-mq code, which doesn't use the
queue_lock at all, and it at least avoids a queue_lock round trip in
scsi_device_unbusy, which is still important given how busy the queue_lock

Convert i2o_res_alloc() to use pci_bus_alloc_resource() rather than
pci_find_parent_resource() and allocate_resource(). We don't have a
resource to start with, so pci_find_parent_resource() can't do anything
useful: a bus may have several memory resources available, so there might
be several possible parents. This is more likely on root buses because
host bridges may have any number of apertures.
I'm pretty sure this didn't work in the first place because it passed
size == min == max to allocate_resource(). The min and max parameters are
constraints on the *addresses* of the resource, not on its size, so I think
it was impossible for allocate_resource() to succeed.

Refactor the PCI space allocation in i2o_iop_systab_set(). This might
improve readability slightly, but mainly it is to make the next patch
simpler.
No functional change.
Signed-off-by: Bjorn Helgaas <bhelgaas@google.com>
When i2o_iop_systab_set() allocates I/O port space, it specifies 1Mb
alignment required. This seems unlikely, since most platforms have only
64Kb of I/O space total. I think 4Kb is a more reasonable choice, since
that's the minimum alignment of a PCI-PCI bridge I/O window.
My guess is that this is a copy/paste error from the memory allocation
code, which specifies 1Mb alignment (which is the minimum alignment of a
PCI-PCI bridge memory window).
Signed-off-by: Bjorn Helgaas <bhelgaas@google.com>
When i2o_iop_systab_set() allocates I/O port space, it assigns the base of
the new I/O port region to sb->current_mem_base, not sb->current_io_base.
This looks like a copy/paste error, because we do use current_io_base, but
there's no other place that sets it.

i2o_cfg_compat_ioctl(I2OGETIOPS) locks i2o_cfg_mutex and then calls
i2o_cfg_ioctl(I2OGETIOPS) that locks i2o_cfg_mutex as well. A deadlock
is guaranteed.

Pull core block IO changes from Jens Axboe:
"The major piece in here is the immutable bio_ve series from Kent, the
rest is fairly minor. It was supposed to go in last round, but
various issues pushed it to this release instead. The pull request
contains:
- Various smaller blk-mq fixes from different folks. Nothing major
here, just minor fixes and cleanups.
- Fix for a memory leak in the error path in the block ioctl code

- Finally the immutable biovec changes from Kent Overstreet. This
enables some nice future work on making arbitrarily sized bios
possible, and splitting more efficient. Related fixes to immutable

- dm-cache immutable fixup from Mike Snitzer.
- btrfs immutable fixup from Muthu Kumar.

Race conditions are theoretically possible between the MPT PCI device
removal and the generic PCI bus rescan and device removal that can be
triggered via sysfs.
To avoid those race conditions make the MPT PCI code use

When we start sharing biovecs, keeping bi_vcnt accurate for splits is
going to be error prone - and unnecessary, if we refactor some code.
So bio_segments() has to go - but most of the existing users just needed
to know if the bio had multiple segments, which is easier - add a
bio_multiple_segments() for them.
(Two of the current uses of bio_segments() are going to go away in a
couple patches, but the current implementation of bio_segments() is
unsafe as soon as we start doing driver conversions for immutable
biovecs - so implement a dumb version for bisectability, it'll go away
in a couple patches)

For the workqueue creation interfaces that do not expect format strings,
make sure they cannot accidently be parsed that way. Additionally, clean
up calls made with a single parameter that would be handled as a format
string. Many callers are passing potentially dynamic string content, so
use "%s" in those cases to avoid any potential accidents.

Hardware with MCA bus is limited to 386 and 486 class machines
that are now 20+ years old and typically with less than 32MB
of memory. A quick search on the internet, and you see that
even the MCA hobbyist/enthusiast community has lost interest
in the early 2000 era and never really even moved ahead from
the 2.4 kernels to the 2.6 series.
This deletes anything remaining related to CONFIG_MCA from core
kernel code and from the x86 architecture. There is no point in
carrying this any further into the future.
One complication to watch for is inadvertently scooping up
stuff relating to machine check, since there is overlap in
the TLA name space (e.g. arch/x86/boot/mca.c).

It gets rid of:
- the net/tr.c which the drivers depended on
- the drivers/net component
- the Kbuild infrastructure around it
- any tokenring related CONFIG_ settings in any defconfigs
- the tokenring headers in the include/linux dir
- the firmware associated with the tokenring drivers.

Merge tag 'split-asm_system_h-for-linus-20120328' of git://git.kernel.org/pub/scm/linux/kernel/git/dhowells/linux-asm_system
Pull "Disintegrate and delete asm/system.h" from David Howells:
"Here are a bunch of patches to disintegrate asm/system.h into a set of
separate bits to relieve the problem of circular inclusion
dependencies.
I've built all the working defconfigs from all the arches that I can
and made sure that they don't break.
The reason for these patches is that I recently encountered a circular
dependency problem that came about when I produced some patches to
optimise get_order() by rewriting it to use ilog2().
This uses bitops - and on the SH arch asm/bitops.h drags in
asm-generic/get_order.h by a circuituous route involving asm/system.h.
The main difficulty seems to be asm/system.h. It holds a number of
low level bits with no/few dependencies that are commonly used (eg.
memory barriers) and a number of bits with more dependencies that
aren't used in many places (eg. switch_to()).
These patches break asm/system.h up into the following core pieces:
(1) asm/barrier.h
Move memory barriers here. This already done for MIPS and Alpha.
(2) asm/switch_to.h
Move switch_to() and related stuff here.
(3) asm/exec.h
Move arch_align_stack() here. Other process execution related bits
could perhaps go here from asm/processor.h.
(4) asm/cmpxchg.h
Move xchg() and cmpxchg() here as they're full word atomic ops and
frequently used by atomic_xchg() and atomic_cmpxchg().
(5) asm/bug.h
Move die() and related bits.
(6) asm/auxvec.h
Move AT_VECTOR_SIZE_ARCH here.
Other arch headers are created as needed on a per-arch basis."
Fixed up some conflicts from other header file cleanups and moving code
around that has happened in the meantime, so David's testing is somewhat
weakened by that. We'll find out anything that got broken and fix it..

At the same time get rid of the magic constant 50.
Actually, I can't see why copying the callback name is necessary
instead of simply storing a pointer to it (just like to the callback
function itself), but that goes beyond the scope of this fix.

[Resend patch as per Bernd Schubert comment ]
Issue:
Device goes offline while doing aggressive HBA reset
along with IO using some utility.
Root cause:
FW goes into bad state due to aggressive reset. Softreset does not
help to recover FW. And also aggressive reset open up the window for
Error handling thread to kicked off at the same time HBA will be in
constant RESET loop as part of aggressive reset test case can lead
Device to goes offline.
Changes:
1. Added extra check as below inside eh_timed_out call back as below.
if(ioc->ioc_reset_in_progress) Rc = EH_TIMER_RESET
2. Removed " DOORBELL_ACTIVE" check for SAS controller from task
management context. Since SAS controller uses high priority queue
for task management. This check is not required for SAS controller.

In-kernel disk event polling doesn't matter for legacy/fringe drivers
and may lead to infinite event loop if ->check_events() implementation
generates events on level condition instead of edge.
Now that block layer supports suppressing exporting unlisted events,
simply leaving disk->events cleared allows these drivers to keep the

There's a branch at the end of this function that
is supposed to normalize the return value with what
the mid-layer expects. In this one case, we get it wrong.
Also increase the verbosity of the INFO level printk
at the end of mptscsih_abort to include the actual return value
and the scmd->serial_number. The reason being success
or failure is actually determined by the state of
the internal tag list when a TMF is issued, and not the
return value of the TMF cmd. The serial_number is also
used in this decision, thus it's useful to know for debugging
purposes.

Needed to update to apply fixes for which the old branch was too
outdated.
Move the mid-layer's ->queuecommand() invocation from being locked
with the host lock to being unlocked to facilitate speeding up the
critical path for drivers who don't need this lock taken anyway.
The patch below presents a simple SCSI host lock push-down as an
equivalent transformation. No locking or other behavior should change
with this patch. All existing bugs and locking orders are preserved.
Additionally, add one parameter to queuecommand,
struct Scsi_Host *
and remove one parameter from queuecommand,
void (*done)(struct scsi_cmnd *)
Scsi_Host* is a convenient pointer that most host drivers need anyway,
and 'done' is redundant to struct scsi_cmnd->scsi_done.
Minimal code disturbance was attempted with this change. Most drivers
needed only two one-line modifications for their host lock push-down.

The block device drivers have all gained new lock_kernel
calls from a recent pushdown, and some of the drivers
were already using the BKL before.
This turns the BKL into a set of per-driver mutexes.

All these files use the big kernel lock in a trivial
way to serialize their private file operations,
typically resulting from an earlier semi-automatic
pushdown from VFS.
None of these drivers appears to want to lock against
other code, and they all use the BKL as the top-level
lock in their file operations, meaning that there
is no lock-order inversion problem.
Consequently, we can remove the BKL completely,
replacing it with a per-file mutex in every case.
Using a scripted approach means we can avoid
typos.

Fixed up pretty trivial conflicts in drivers/block/virtio_blk.c and
drivers/scsi/scsi_error.c as per Jens.
This restores the changes from "scsi/i2o_block: cleanup ioctl
handling", which accidentally got reverted.
Origignal changelog:
This fixes the ioctl function of the i2o_block driver, which
has multiple problems:
* The BLKI2OSRSTRAT and BLKI2OSWSTRAT commands always return
-ENOTTY on success, where they should return 0.
* Support for 32 bit compat is missing
* The driver should use the .ioctl function and because
.locked_ioctl is going away.
The use of the big kernel lock remains for now, but gets
made explictit in the ioctl function.

The open and release block_device_operations are currently
called with the BKL held. In order to change that, we must
first make sure that all drivers that currently rely
on this have no regressions.
This blindly pushes the BKL into all .open and .release
operations for all block drivers to prepare for the
next step. The drivers can subsequently replace the BKL
with their own locks or remove it completely when it can
be shown that it is not needed.
The functions blkdev_get and blkdev_put are the only
remaining users of the big kernel lock in the block
layer, besides a few uses in the ioctl code, none
of which need to serialize with blkdev_{get,put}.
Most of these two functions is also under the protection
of bdev->bd_mutex, including the actual calls to
->open and ->release, and the common code does not
access any global data structures that need the BKL.

Issue description:
In multipath topology, when device deletion is in transient state,
multipath driver can call blk_flush_queue() as part of path failure.
Before device get deleted from OS, Device may go OFFLINE as part of error
handling kicked off triggered from multipathing driver. Above condition hits
more frequently if device missing delay timer (which is LSI specific firmware
parameter) is non zero value.
root cause of this issue is Error handling thread is getting kicked off for
device which is not really present(in transient state of deleting).
This patch has solution for this issue. driver is now using eh_timed_out
callback. See below.
mptsas_transport_template->eh_timed_out = mptsas_eh_timed_out
Using mptsas_eh_timed_out function, driver can decide weather vdevice is
under Device missing delay or deleting state.
for either of those cases, there is BLK_EH_RESET_TIMER return to scsi mid
and error handling thread will not be kicked off for that particular scsi
command.

This patch fixes mptsas disk hot-removal processing. The
hot-removal processing doesn't complete because of this condition.
drivers/message/fusion/mptsas.c:
mptsas_taskmgmt_complete()
if ((mptsas_find_vtarget(ioc, channel, id)) && !ioc->fw_events_off)
mptsas_queue_device_delete(...);
mptsas_queue_device_delete(), which must be called for
hot-removal, never gets called because mptsas_find_vtarget()
always returns 0 here. At that time, the vtarget has already
been freed in mptsas_target_destroy(), and also the scsi_device
has been marked as SDEV_DEL.
As a result of the issue, port deletion functions won't get
called and the device ends up being in an incomplete state.
(Some data structures and sysfs entries, which should be
removed in hot-removal, remain.) One side effect of this is
that a hot-addition of the device (bringing the device back
on) fails.
This patch just removes mptsas_find_vtarget() from the if-state
condition.
