qeth devices in layer3 mode need a separate handling of vipa and proxy-arp
addresses. vipa and proxy-arp addresses processed by qeth can be read from
userspace. Introduced with commit 5f78e29cee ("qeth: optimize IP handling
in rx_mode callback") the retrieval of vipa and proxy-arp addresses is
broken, if more than one vipa or proxy-arp address are set.
The qeth code used local variable "int i" for 2 different purposes. This
patch now spends 2 separate local variables of type "int".
While touching these functions hash_for_each_safe() is converted to
hash_for_each(), since there is no removal of hash entries.

STARTLAN needs to be the first IPA command after MPC initialization
completes.
So move the qeth_send_startlan() call from the layer disciplines
into the core path, right after the MPC handshake.
While at it, replace the magic LAN OFFLINE return code
with the existing enum.

When RX/TX checksum offloading is turned on and the adapter is
an OSA 3 card in layer 3 mode, the checksum offloading is only
performed when both peers use different adapters. If both peers
share an OSA 3 card, communication is a memory copy and
checksum offloading is not performed.
This patch adds a warning to inform the administrator.
OSA 3 in layer 2 mode does not offer the RX/TX checksum
offload feature.

Turning on receive and/or transmit checksum offload support
on the OSA card requires 2 commands:
1. start command which replies with available features
2. enable command to turn on selected features.
The current version does not check the reply of the start
command and simply uses the returned value to enable
offload features. When the start command returns zero, this
leads to a situation where no checksum offload
is turned on by the hardware. Even worse no error
indication is returned. The Linux kernel assumes
the OSA card performs RX/TX checksum offload, but the hardware
does not perform any checksum verification at all.
This patch checks the return of the start and enable
command responses from the hardware and turns off
checksum offloading if the commands fails or does not
respond with the correct bit setting.

driver. There are five improvements for zcrypt: the ID for the CEX6
cards is added, workload balancing and multi-domain support are

Then there are several patches in regard to inline assemblies. One
compile fix and several missing memory clobbers. As far as we can tell
the omitted memory clobbers have not caused any breakage.
A small change to the PCI arch code, the machine can tells us how big
the function measurement blocks are. The PCI function measurement will
be disabled for a device if the queried length is larger than the
allocated size for these blocks.
And two more patches to correct five printk messages.

It is unavoidable that zfcp_scsi_queuecommand() has to finish requests
with DID_IMM_RETRY (like fc_remote_port_chkready()) during the time
window when zfcp detected an unavailable rport but
fc_remote_port_delete(), which is asynchronous via
zfcp_scsi_schedule_rport_block(), has not yet blocked the rport.
However, for the case when the rport becomes available again, we should
prevent unblocking the rport too early. In contrast to other FCP LLDDs,
zfcp has to open each LUN with the FCP channel hardware before it can
send I/O to a LUN. So if a port already has LUNs attached and we
unblock the rport just after port recovery, recoveries of LUNs behind
this port can still be pending which in turn force
zfcp_scsi_queuecommand() to unnecessarily finish requests with
DID_IMM_RETRY.
This also opens a time window with unblocked rport (until the followup
LUN reopen recovery has finished). If a scsi_cmnd timeout occurs during
this time window fc_timed_out() cannot work as desired and such command
would indeed time out and trigger scsi_eh. This prevents a clean and
timely path failover. This should not happen if the path issue can be
recovered on FC transport layer such as path issues involving RSCNs.
Fix this by only calling zfcp_scsi_schedule_rport_register(), to
asynchronously trigger fc_remote_port_add(), after all LUN recoveries as
children of the rport have finished and no new recoveries of equal or
higher order were triggered meanwhile. Finished intentionally includes
any recovery result no matter if successful or failed (still unblock
rport so other successful LUNs work). For simplicity, we check after
each finished LUN recovery if there is another LUN recovery pending on
the same port and then do nothing. We handle the special case of a
successful recovery of a port without LUN children the same way without
changing this case's semantics.
For debugging we introduce 2 new trace records written if the rport
unblock attempt was aborted due to still unfinished or freshly triggered
recovery. The records are only written above the default trace level.
Benjamin noticed the important special case of new recovery that can be
triggered between having given up the erp_lock and before calling
zfcp_erp_action_cleanup() within zfcp_erp_strategy(). We must avoid the

Now the rport was erroneously unblocked while the zfcp_port is blocked.
This is another situation we want to avoid due to scsi_eh
potential. This state would at least remain until the new recovery from
the other context finished successfully, or potentially forever if it
failed. In order to close this race, we take the erp_lock inside
zfcp_erp_try_rport_unblock() when checking the status of zfcp_port or
LUN. With that, the possible corresponding rport state sequences would
be: (unblock[ERP thread],block[other context]) if the ERP thread gets
erp_lock first and still sees ((port->status & ...UNBLOCK) != 0),
(block[other context],NOP[ERP thread]) if the ERP thread gets erp_lock
after the other context has already cleard ...UNBLOCK from port->status.
Since checking fields of struct erp_action is unsafe because they could
have been overwritten (re-used for new recovery) meanwhile, we only
check status of zfcp_port and LUN since these are only changed under
erp_lock elsewhere. Regarding the check of the proper status flags (port
or port_forced are similar to the shown adapter recovery):

Since quite a while, Linux issues enough SCSI commands per scsi_device
which successfully return with FCP_RESID_UNDER, FSF_FCP_RSP_AVAILABLE,
and SAM_STAT_GOOD. This floods the HBA trace area and we cannot see
other and important HBA trace records long enough.
Therefore, do not trace HBA response errors for pure benign residual
under counts at the default trace level.
This excludes benign residual under count combined with other validity
bits set in FCP_RSP_IU, such as FCP_SNS_LEN_VAL. For all those other
cases, we still do want to see both the HBA record and the corresponding
SCSI record by default.

When SCSI EH invokes zFCP's callbacks for eh_device_reset_handler() and
eh_target_reset_handler(), it expects us to relent the ownership over
the given scsi_cmnd and all other scsi_cmnds within the same scope - LUN
or target - when returning with SUCCESS from the callback ('release'
them). SCSI EH can then reuse those commands.
We did not follow this rule to release commands upon SUCCESS; and if
later a reply arrived for one of those supposed to be released commands,
we would still make use of the scsi_cmnd in our ingress tasklet. This
will at least result in undefined behavior or a kernel panic because of
a wrong kernel pointer dereference.
To fix this, we NULLify all pointers to scsi_cmnds (struct zfcp_fsf_req
*)->data in the matching scope if a TMF was successful. This is done
under the locks (struct zfcp_adapter *)->abort_lock and (struct
zfcp_reqlist *)->lock to prevent the requests from being removed from
the request-hashtable, and the ingress tasklet from making use of the
scsi_cmnd-pointer in zfcp_fsf_fcp_cmnd_handler().
For cases where a reply arrives during SCSI EH, but before we get a
chance to NULLify the pointer - but before we return from the callback
-, we assume that the code is protected from races via the CAS operation
in blk_complete_request() that is called in scsi_done().

This patch introduces tracepoint definitions and tracepoint
event invocations for the s390 zcrypt device.
Currently there are just two tracepoint events defined.
An s390_zcrypt_req request event occurs as soon as the
request is recognized by the zcrypt ioctl function. This
event may act as some kind of request-processing-starts-now
indication.
As late as possible within the zcrypt ioctl function there
occurs the s390_zcrypt_rep event which may act as the point
in time where the request has been processed by the kernel
and the result is about to be transferred back to userspace.
The glue which binds together request and reply event is the
ptr parameter, which is the local buffer address where the
request from userspace has been stored by the ioctl function.
The main purpose of this zcrypt tracepoint patch is to get
some data for performance measurements together with
information about the kind of request and on which card and
queue the request has been processed. It is not an ffdc
interface as there is already code in the zcrypt device
driver to serve the s390 debug feature interface.

Rework the debug feature calls and initialization. There
are now two debug feature entries used by the zcrypt code.
The first is 'ap' with all the AP bus related stuff and the
second is 'zcrypt' with all the zcrypt and devices and
driver related entries. However, there isn't much traffic on
both debug features. The ap bus code emits only some debug
info and for zcrypt devices on appearance and disappearance
there is an entry written.
The new dbf invocations use the sprintf buffer layout,
whereas the old implementation used the ascii dbf buffer.
There are now 5*8=40 bytes used for each entry, resulting in
5 parameters per call. As the sprintf buffer needs a format
string the first parameter provides this and so up to 4 more
parameters can be used. Alltogehter the new layout should be
much more human readable for customers and test.

Add defines and switch case code to handle the two invalid
domain response codes better. Until now these two response
codes are handled via default resulting in -EAGAIN and
switching the processed queue to offline. So this kind of
malformed request bounced through all suitable queues and
switched them off. Now this kind of malformed request is
just rejected with EINVAL without switching off the queue.

According to the system architecture the current implementation
requires the presence of the N bit in GR2 in the TAPQ response
field to validate the max. number of domains (Nd).
Older machine types don't have this N bit, hence the max. domain
field was ignored.
Before the N bit was introduced the maximum number of domain was
a constant value of 15. So set this value in case of N bit absence.

For the older CEX2x and CEX3x cards the function bits returned
by TAPQ do not reflect the functions of the card. Instead the
functionality is implicit by the type of the card. The reworked
zcrypt requires to have the function bits set correct, so this
patch fixes this. The queue selection is not only based on these
function bits but also on function pointers set by the individual
drivers.

Currently the first eligible AP adapter respectively domain will be
selected to service requests. In case of sequential workload, the
very same adapter/domain will be used.
The adapter/domain selection algorithm now considers the completed
transactions per adaper/domain and therefore ensures a homogeneous
utilization.

Currently the ap infrastructure only supports one domain at a time.
This feature extends the generic cryptographic device driver to
support multiple cryptographic domains simultaneously.
There are now card and queue devices on the AP bus with independent
card and queue drivers. The new /sys layout is as follows:

Crypto requests are very different in complexity and thus runtime.
Also various crypto adapters are differ with regard to the execution
time. Crypto requests can be balanced much better when the request
type and eligible crypto adapters are rated in a more precise
granularity. Therefore, request weights and adapter speed rates for
dedicated requests will be introduced.

The poll thread of the AP bus is burning CPU while waiting for
crypto requests to complete. We can as well burn a few more cycles
in the poll thread to check if there are pending requests and
remove the atomic operations with the ap_poll_requests.
This improves the code if the machine has adapter interrupts.

Now that the message type modules are linked with the zcrypt_api
into a single module the zcrypt_ops_list is initialized by
the module init function of the zcyppt.ko module. After that
the list is static and all message types are present.
Drop the zcrypt_ops_list_lock spinlock and the module handling
in regard to the message types.

With this feature, the DASD device driver more robustly handles DASDs
that are attached via multiple channel paths and are subject to
constant Interface-Control-Checks (IFCCs) and Channel-Control-Checks
(CCCs) or loss of High-Performance-FICON (HPF) functionality on one or
more of these paths.
If a channel path does not work correctly, it is removed from normal
operation as long as other channel paths are available. All extended
error recovery states can be queried and reset via user space
interfaces.

The buffer for iucv_message_receive() needs to be below 2 GB. In
__iucv_message_receive(), the buffer address is casted to an u32, which
would result in either memory corruption or an addressing exception when
using addresses >= 2 GB.
Fix this by using GFP_DMA for the buffer allocation.

In order to be able to setup the cpu to node mappings early it is a
prerequisite to know which cpus are present. Therefore cpus must be
detected much earlier than before.
For sclp based cpu detection this requires yet another early sclp
call, since the system is not ready to use the regular interrupt and
memory allocations.

Switch the zcrypt bus from legacy suspend/resume callbacks to dev_pm_ops.
The conversion is straight forward with the help of SIMPLE_DEV_PM_OPS().
The new dev_pm_ops based version is functionally equivalent to the legacy
callbacks version.
This will allow to eventually remove support for legacy suspend/resume
callbacks from the kernel altogether.

Export fc_bsg_jobdone so drivers can use it directly instead of doing
the round-trip via struct fc_bsg_job::job_done() and use it in the
LLDDs. That way we can also unify the interfaces of fc_bsg_jobdone and
bsg_job_done.
As we've converted all LLDDs over to use fc_bsg_jobdone() directly, we
can remove the function pointer from struct fc_bsg_job as well.

Also note that MODULE_DEVICE_TABLE is a no-op for non-modular code.
We replace module.h with moduleparam.h since the file does declare
some module_param() and leaving that as-is is currently the easiest
way to remain compatible with existing boot arg use cases.

The Kconfig currently controlling compilation of this code is:
drivers/s390/char/Kconfig:config TN3215
drivers/s390/char/Kconfig: def_bool y
...meaning that it currently is not being built as a module by anyone.
Lets remove the modular code that is essentially orphaned, so that
when reading the driver there is no doubt it is builtin-only.
Since module_init translates to device_initcall in the non-modular
case, the init ordering remains unchanged with this commit.
We don't replace module.h with init.h since the file already has that.

The function dasd_ro_store() calls set_disk_ro() to set the device in
question read-only. Since set_disk_ro() might sleep, we can't call it
while holding a lock. However, we also can't simply check if the device,
block, and gdp references are valid before we call set_disk_ro() because
an offline processing might have been started in the meanwhile which
will destroy those references.
In order to reliably call set_disk_ro() we have to ensure several
things:
- Still check validity of the mentioned references but additionally
check if offline processing is running and bail out accordingly. Also,
do this while holding the device lock.
- To ensure that the block device is still safe after the lock, increase
the open_count while still holding the device lock.

The reference to a device in question may get lost when the extended
error reporting (EER) attribute is being enabled/disabled while the
device is set offline at the same time. This is due to missing
refcounting and incorrect locking. Fix this by the following:
- In dasd_eer_store() get the device directly and handle the refcount
accordingly.
- Move the lock in dasd_eer_enable() up so we can ensure safe
processing.
- Check if the device is being set offline and return with -EBUSY if so.
- While at it, change the return code from -EPERM to -EMEDIUMTYPE as
suggested by a FIXME, since that is what we're actually checking.

Before we set a device offline, the open_count for the block device is
checked and certain flags are checked and set as well.
However, this is all done without holding any lock. Potentially, if the
open_count was checked but the DASD_FLAG_OFFLINE wasn't set yet, a
different process might want to increase the open_count depending on
whether DASD_FLAG_OFFLINE is set or not in the meanwhile.
This is quite racy and can lead to the loss of the device for that
process and subsequently lead to a panic.
Fix this by checking the open_count and setting the offline flags while
holding the ccwdev lock.

When setting certain attributes, we actually set the according feature
flag. Do this by using dasd_set_feature() at a few occurrences and
remove duplicate code.
In dasd_set_feature() dasd_find_busid() is used to retrieve the devmap
for the device in question. Combined with the change above, this would
require the device to be set online at least once so that a devmap is
being created. Change that by using dasd_devmap_from_cdev() instead,
which uses dasd_find_busid() first and will create a devmap accordingly
if there is none yet.

Currently the block queue value max_segments is set to -1L, which
is then implicitly casted to unsigned short in blk_queue_max_segments.
This results in 65535 (64k) max_segments.
Even though the resulting value is correct, setting it implicitly using
-1L is rather confusing. Set the value explicitly using the USHRT_MAX
macro instead.

During IPL we register all chpids that are not in the unrecognized
state. This includes chpids that are not usable and chpids for which
the state could not be obtained.
Change that to only register chpids in the configured (usable) or
standby (usable after a configure operation) state. All other chpids

We accidentally overwrite the original saved value of "flags" so that we

High values are always wrong for netiucv's sysfs attribute "buffer".
But the current code does not detect values between 2**31 and 2**32
as invalid. Choosing type "unsigned int" for variable "bs1" and making
use of "kstrtouint()" improves the syntax checking for "buffer".

con3270 contains an optimisation that reduces the amount of data to be
transmitted to the 3270 terminal by putting a Repeat to Address (RA)
order into the data stream. The RA order itself takes up space, so
con3270 only uses it if there's enough space left in the line
buffer. Otherwise it just pads out the line manually.
For lines that were _just_ short enough that the RA order still fit in
the line buffer, the line was instead padded with an insufficient
amount of spaces. This was caused by examining the size of the
allocated line buffer rather than the length of the string to be
displayed.
For con3270_cline_end(), we just compare against the line length. For
con3270_update_string() however that isn't available anymore, so we
check whether the Repeat to Address order is present.

con3270 contains an optimisation that reduces the amount of data to be
transmitted to the 3270 terminal by putting a Repeat to Address (RA)
order into the data stream. The RA order itself takes up space, so
con3270 only uses it if there's enough space left in the line
buffer. Otherwise it just pads out the line manually.
For lines too long to include the RA order, one byte was left
uninitialised. This was caused by an off-by-one bug in the loop that
pads out the line. Since the buffer is allocated from a common pool,
the single byte left uninitialised contained some previous buffer
content. Usually this was just a space or some character (which can
result in clutter but is otherwise harmless). Sometimes, however, it
was a Repeat to Address order, messing up the entire screen layout and
causing the display to send the entire buffer content on every
keystroke.

Since commit 9f3d6d7 chsc_get_channel_measurement_chars is called with
interrupts disabled during resume from hibernate. Since this function
used spin_unlock_irq, interrupts have been enabled accidentally. Fix
this by using the irqsave variant.
Since we can't guarantee the IRQ-enablement state for all (future/
external) callers, change the locking in related functions to prevent
similar bugs in the future.

If the DASD device gets blocked for any reason, e.g. because it is reserved
somewhere, the host_access_count sysfs entry or the host_access_list
debugfs entry may sleep forever. Make it interruptible so that userspace
can use ^C to abort the operation.

A DASD device consists of the device itself and a discipline with a
corresponding private structure. These fields are set up during online
processing right after the device is created and before it is processed by
the state machine and made available for I/O.
During offline processing the discipline pointer and the private data gets
freed within the state machine and without protection of the existing
reference count. This might lead to a kernel panic because a function might
have taken a device reference and accesses the discipline pointer and/or
private data of the device while this is already freed.
Fix by freeing the discipline pointer and the private data after ensuring
that there is no reference to the device left.

Internal I/O is processed by the _sleep_on_function which might wait for a
device to get operational. During offline processing this will never happen
and therefore the refcount of the device will not drop to zero and the
offline processing blocks as well.
Fix by letting requests fail in the _sleep_on function during offline
processing. No further handling of the requests is necessary since this is
internal I/O and the device is thrown away afterwards.

The DASD device driver throws change events for the DASD blockdevice
after the online processing is done so that udev rules can take
actions after it.

commit 5f78e29cee ("qeth: optimize IP handling in rx_mode callback")
restructured the internal address handling.
This work broke setting a virtual IP address.
The command
echo 10.1.1.1 > /sys/bus/ccwgroup/devices/<device>/vipa/add4
fails with file exist error even if the IP address has not
been set before.
It turned out that the search result for the IP address
search is handled incorrectly in the VIPA case.
This patch fixes the setting of an virtual IP address.

According to recent performance measurements, turning on net_device
feature NETIF_F_SG only behaves well, but turning on feature
NETIF_F_GSO shows bad results. Since the kernel activates NETIF_F_GSO
automatically as soon as the driver configures feature NETIF_F_SG, qeth
should not activate feature NETIF_F_SG per default, until the qeth
problems with NETIF_F_GSO are solved.

To reduce the need of skb_linearize() calls, gso_max_segs of qeth
net_devices had been limited according to the maximum number of qdio SBAL
elements. But a gso segment cannot be larger than the mtu-size, while an
SBAL element can contain up to 4096 bytes. The gso_max_segs limitation
limits the maximum packet size given to the qeth driver. Performance
measurements with tso-enabled qeth network interfaces and mtu-size 1500
showed, that the disadvantage of smaller packets is much more severe than
the advantage of fewer skb_linearize() calls.
This patch gets rid of the gso_max_segs limitations in the qeth driver.

The qeth IP address mapping logic has been reworked recently. It
causes now problems to specify qeth sysfs attribute "hsuid" in DOWN
state, which is allowed. Postpone registering or deregistering of
IP-addresses in this case.

After device recovery, only a basic set of network device features is
enabled on the device. If features like checksum offloading or TSO were
enabled by the user before the recovery, this results in a mismatch
between the network device features, that the kernel assumes to be
enabled on the device, and the features actually enabled on the device.
This patch tries to restore previously set features, that require
changes on the device, after the recovery of a device. In case of an
error, the network device's features are changed to contain only the
features that are actually turned on.

The workqueue "appldata_wq" has been replaced with an ordered dedicated
workqueue.
WQ_MEM_RECLAIM has not been set since the workqueue is not being used on
a memory reclaim path.
The adapter->work_queue queues multiple work items viz
&adapter->scan_work, &port->rport_work, &adapter->ns_up_work,
&adapter->stat_work, adapter->work_queue, &adapter->events.work,
&port->gid_pn_work, &port->test_link_work. Hence, an ordered
dedicated workqueue has been used.
WQ_MEM_RECLAIM has been set to ensure forward progress under memory
pressure.

Many modules call misc_register and misc_deregister in its module init
and exit methods without any additional code. This ends up being
boilerplate. This patch adds helper macro module_misc_device(), that
replaces module_init()/ module_exit() with template functions.
This patch also converts drivers to use new macro.
Change since v1:
Add device.h include in miscdevice.h as module_driver macro was not
available from other include files in some architectures.

out of bounds for RSPN GS responses because u.rspn.rsp

we lost the N_Port-ID where an ELS response comes from.
With commit 7c7dc19681
("[SCSI] zfcp: Simplify handling of ct and els requests")
we lost the N_Port-ID where a CT response comes from.
It's especially useful if the request SAN trace record
with D_ID was already lost due to trace buffer wrap.
GS uses an open WKA port handle and ELS just a D_ID, and
only for ELS we could get D_ID from QTCB bottom via zfcp_fsf_req.
To cover both cases, add a new field to zfcp_fsf_ct_els
and fill it in on request to use in SAN response trace.
Strictly speaking the D_ID on SAN response is the FC frame's S_ID.
We don't need a field for the other end which is always us.

Since commit a54ca0f62f
("[SCSI] zfcp: Redesign of the debug tracing for HBA records.")
HBA records no longer contain WWPN, D_ID, or LUN
to reduce duplicate information which is already in REC records.
In contrast to "regular" target ports, we don't use recovery to open
WKA ports such as directory/nameserver, so we don't get REC records.
Therefore, introduce pseudo REC running records without any
actual recovery action but including D_ID of WKA port on open/close.

On a successful end of reopen port forced,
zfcp_erp_strategy_followup_success() re-uses the port erp_action
and the subsequent zfcp_erp_action_cleanup() now
sees ZFCP_ERP_SUCCEEDED with
erp_action->action==ZFCP_ERP_ACTION_REOPEN_PORT
instead of ZFCP_ERP_ACTION_REOPEN_PORT_FORCED
but must not perform zfcp_scsi_schedule_rport_register().
We can detect this because the fresh port reopen erp_action
is in its very first step ZFCP_ERP_STEP_UNINITIALIZED.
Otherwise this opens a time window with unblocked rport
(until the followup port reopen recovery would block it again).
If a scsi_cmnd timeout occurs during this time window
fc_timed_out() cannot work as desired and such command
would indeed time out and trigger scsi_eh. This prevents
a clean and timely path failover.
This should not happen if the path issue can be recovered
on FC transport layer such as path issues involving RSCNs.
Also, unnecessary and repeated DID_IMM_RETRY for pending and
undesired new requests occur because internally zfcp still
has its zfcp_port blocked.
As follow-on errors with scsi_eh, it can cause,
in the worst case, permanently lost paths due to one of:
sd <scsidev>: [<scsidisk>] Medium access timeout failure. Offlining disk!
sd <scsidev>: Device offlined - not ready after error recovery
For fix validation and to aid future debugging with other recoveries
we now also trace (un)blocking of rports.

In the hardware data router case, introduced with kernel 3.2
commit 86a9668a8d ("[SCSI] zfcp: support for hardware data router")
the ELS/GS request&response length needs to be initialized
as in the chained SBAL case.
Otherwise, the FCP channel rejects ELS requests with
FSF_REQUEST_SIZE_TOO_LARGE.
Such ELS requests can be issued by user space through BSG / HBA API,
or zfcp itself uses ADISC ELS for remote port link test on RSCN.
The latter can cause a short path outage due to
unnecessary remote target port recovery because the always
failing ADISC cannot detect extremely short path interruptions
beyond the local FCP channel.

For an NPIV-enabled FCP device, zfcp can erroneously show
"NPort (fabric via point-to-point)" instead of "NPIV VPORT"
for the port_type sysfs attribute of the corresponding
fc_host.
s390-tools that can be affected are dbginfo.sh and ziomon.
zfcp_fsf_exchange_config_evaluate() ignores
fsf_qtcb_bottom_config.connection_features indicating NPIV
and only sets fc_host_port_type to FC_PORTTYPE_NPORT if
fsf_qtcb_bottom_config.fc_topology is FSF_TOPO_FABRIC.
Only the independent zfcp_fsf_exchange_port_evaluate()
evaluates connection_features to overwrite fc_host_port_type
to FC_PORTTYPE_NPIV in case of NPIV.
Code was introduced with upstream kernel 2.6.30
commit 0282985da5
("[SCSI] zfcp: Report fc_host_port_type as NPIV").
This works during FCP device recovery (such as set online)
because it performs FSF_QTCB_EXCHANGE_CONFIG_DATA followed by
FSF_QTCB_EXCHANGE_PORT_DATA in sequence.
However, the zfcp-specific scsi host sysfs attributes
"requests", "megabytes", or "seconds_active" trigger only
zfcp_fsf_exchange_config_evaluate() resetting fc_host
port_type to FC_PORTTYPE_NPORT despite NPIV.
The zfcp-specific scsi host sysfs attribute "utilization"
triggers only zfcp_fsf_exchange_port_evaluate() correcting
the fc_host port_type again in case of NPIV.
Evaluate fsf_qtcb_bottom_config.connection_features
in zfcp_fsf_exchange_config_evaluate() where it belongs to.

On LPAR the read message buffer command should be executed on the path
it was received on otherwise there is a chance that the CUIR assignment
might be faulty and the wrong channel path is set online/offline.
Fix by setting the path mask accordingly.
On z/VM we might not be able to do I/O on this path but there it does
not matter on which path the read message buffer command is executed.
Therefor implement a retry with an open path mask.

When a device is in a status where CIO has killed all I/O by itself the
interrupt for a clear request may not contain an irb to determine the
clear function. Instead it contains an error pointer -EIO.
This was ignored by the DASD int_handler leading to a hanging device
waiting for a clear interrupt.
Handle -EIO error pointer correctly for requests that are clear pending and
treat the clear as successful.

There only ever have been two host implementations of the old
s390-virtio (pre-ccw) transport: the experimental kuli userspace,
and qemu. As qemu switched its default to ccw with 2.4 (with most
users having used ccw well before that) and removed the old transport
entirely in 2.6, s390-virtio probably hasn't been in active use for
quite some time and is therefore likely to bitrot.
Let's start the slow march towards removing the code by deprecating
it.
Note that this also deprecates the early virtio console code, which
has been causing trouble in the guest without being wired up in any
relevant hypervisor code.

During qdio_shutdown the queue tasklets are killed for all inbound and
outbound queues. The queue structures might be freed after
qdio_shutdown.

Prior to starting IO qdio checks for the internal state of the ccw
device. These checks happen without locking, so consistency between
state evaluation and starting of the IO is not guaranteed.
Since the internal state is checked during ccw_device_start it is
safe to get rid of these additional checks.

qdio sometimes checks return codes twice. First with the ccw device's
lock held and then a 2nd time after the lock is released. Simplify
the code by releasing the lock earlier and unify the return code
evaluation.

The subchannel id is a property of the subchannel. The additional
copy is not needed.
Internal users can obtain it from subchannel.schid - device drivers
can use ccw_device_get_schid().

- s390: support for trapping software breakpoints, nested virtualization
(vSIE), the STHYI opcode, initial extensions for CPU model support.
- MIPS: support for MIPS64 hosts (32-bit guests only) and lots of cleanups,
preliminary to this and the upcoming support for hardware virtualization
extensions.
- x86: support for execute-only mappings in nested EPT; reduced vmexit
latency for TSC deadline timer (by about 30%) on Intel hosts; support for
more than 255 vCPUs.
- PPC: bugfixes.
The ugly bit is the conflicts. A couple of them are simple conflicts due
to 4.7 fixes, but most of them are with other trees. There was definitely
too much reliance on Acked-by here. Some conflicts are for KVM patches
where _I_ gave my Acked-by, but the worst are for this pull request's
patches that touch files outside arch/*/kvm. KVM submaintainers should
probably learn to synchronize better with arch maintainers, with the
latter providing topic branches whenever possible instead of Acked-by.
This is what we do with arch/x86. And I should learn to refuse pull
requests when linux-next sends scary signals, even if that means that
submaintainers have to rebase their branches.
Anyhow, here's the list:
- arch/x86/kvm/vmx.c: handle_pcommit and EXIT_REASON_PCOMMIT was removed
by the nvdimm tree. This tree adds handle_preemption_timer and
EXIT_REASON_PREEMPTION_TIMER at the same place. In general all mentions
of pcommit have to go.
There is also a conflict between a stable fix and this patch, where the
stable fix removed the vmx_create_pml_buffer function and its call.
- virt/kvm/kvm_main.c: kvm_cpu_notifier was removed by the hotplug tree.
This tree adds kvm_io_bus_get_dev at the same place.
- virt/kvm/arm/vgic.c: a few final bugfixes went into 4.7 before the
file was completely removed for 4.8.
- include/linux/irqchip/arm-gic-v3.h: this one is entirely our fault;
this is a change that should have gone in through the irqchip tree and
pulled by kvm-arm. I think I would have rejected this kvm-arm pull
request. The KVM version is the right one, except that it lacks
GITS_BASER_PAGES_SHIFT.
- arch/powerpc: what a mess. For the idle_book3s.S conflict, the KVM
tree is the right one; everything else is trivial. In this case I am
not quite sure what went wrong. The commit that is causing the mess
(fd7bacbca4, "KVM: PPC: Book3S HV: Fix TB corruption in guest exit
path on HMI interrupt", 2016-05-15) touches both arch/powerpc/kernel/
and arch/powerpc/kvm/. It's large, but at 396 insertions/5 deletions
I guessed that it wasn't really possible to split it and that the 5
deletions wouldn't conflict. That wasn't the case.
- arch/s390: also messy. First is hypfs_diag.c where the KVM tree
moved some code and the s390 tree patched it. You have to reapply the
relevant part of commits 6c22c98637, plus all of e030c1125e, to
arch/s390/kernel/diag.c. Or pick the linux-next conflict
resolution from http://marc.info/?l=kvm&m=146717549531603&w=2.
Second, there is a conflict in gmap.c between a stable fix and 4.8.
The KVM version here is the correct one.

The device suspend call triggers all ap devices to fetch potentially
available response messages from the queues. Therefore the
corresponding zcrypt device, that is allocated asynchronously after
ap device probing, needs to be fully prepared. This race condition
could lead to uninitialized response buffers while trying to read
from the queues.
Introduce a new callback within the ap layer to get noticed when a
zcrypt device is fully prepared. Additional checks prevent reading
from devices that are not fully prepared.

We store requests for channel path configure operations in an array but
maintain an additional cfg_busy variable (indicating if we have requests
stored in said array). When 2 tasks request a channel path configure
operation cfg_busy could be set to 0 even if we still have unprocessed
requests. This would lead to the second task being woken up although its
request was not processed yet.
Fix that by getting rid of cfg_busy and use the chp_cfg_task array
in the wake up condition.

cfg_lock is never held long and we don't want to sleep while the
lock is being held. Thus it can be converted to a simple spinlock.
In addition we can now use the lock during the evaluation of a
wake_up condition.

- support for 2GB pages is added to the hugetlbfs backend.

- the oprofile hardware sampling support is dead code and is removed.
The oprofile user space uses the perf interface nowadays.
- the ETR clock synchronization is removed, this has been superseeded

Currently, presence of direct_access() in block_device_operations
indicates support of DAX on its block device. Because
block_device_operations is instantiated with 'const', this DAX
capablity may not be enabled conditinally.
In preparation for supporting DAX to device-mapper devices, add
QUEUE_FLAG_DAX to request_queue flags to advertise their DAX
support. This will allow to set the DAX capability based on how
mapped device is composed.

The workqueue "chp_wq" is involved in performing pending
configure tasks for channel paths.
It has a single work item(&cfg_work) and hence doesn't require
ordering. Also, it is not being used on a memory reclaim path.
Hence, the singlethreaded workqueue has been replaced with the use of
system_wq.
System workqueues have been able to handle high level of concurrency
for a long time now and hence it's not required to have a singlethreaded
workqueue just to gain concurrency. Unlike a dedicated per-cpu workqueue
created with create_singlethread_workqueue(), system_wq allows multiple
work items to overlap executions even on the same CPU; however, a
per-cpu workqueue doesn't have any CPU locality or global ordering
guarantee unless the target CPU is explicitly specified and thus the
increase of local concurrency shouldn't make any difference.

When we fetch channel path descriptors via chsc we use a suboptimal
struct chsc_scpd and adjust that by casting the response to a generic
chsc_response_struct. Simplify the code by improving struct chsc_scpd.

When fetching channel path descriptors we've only evaluated the rfmt
parameter which could lead us to trigger the chsc even though the
machine doesn't support the specific format or to not trigger the
chsc and report a failure to userspace even though the machine would've
supported it.
Improve these checks and change the parameters of the in-kernel
user to be less confusing.

Not all machines / hypervisors support the chsc commands to fetch
the fmt1 descriptor. When these commands fail the channel path would
currently not be available to linux.
Since users of these descriptors can already deal with invalid data
make fetching it optional. The only data that is mandatory for us is
the fmt0 channel path descriptor.
Also make the return code for missing facilities in
chsc_get_channel_measurement_chars consistent to other functions.

Prior to commit 1bc6664bdf a call to
enable_cmf for a device for which channel measurement was already
enabled resulted in a reset of the measurement data.
What looked like bugs at the time (a 2nd allocation was triggered
but failed, reset was called regardless of previous failures, and
errors have not been reported to userspace) was actually something
at least one userspace tool depended on. Restore that behavior in
a sane way.

Use the same code structure when determining preferred consoles for
Linux running as KVM guest as with Linux running in LPAR and z/VM
guest:
- Extend the console_mode variable to cover vt220 and hvc consoles
- Determine sensible console defaults in conmode_default()
- Remove KVM-special handling in set_preferred_console()
Ensure that the sclp line mode console is also registered when the
vt220 console was selected to not change existing behavior that
someone might be relying on.
As an externally visible change, KVM guest users can now select
the 3270 or 3215 console devices using the conmode= kernel parameter,
provided that support for the corresponding driver was compiled into
the kernel.

The __pmem address space was meant to annotate codepaths that touch
persistent memory and need to coordinate a call to wmb_pmem(). Now that
wmb_pmem() is gone, there is little need to keep this annotation.

A qeth_card contains a napi_struct linked to the net_device during
device probing. This struct must be deleted when removing the qeth
device, otherwise Panic on oops can occur when qeth devices are
repeatedly removed and added.

On HiperSockets only outbound queues 0 to 2 are available for unicast
packets. Current Priority Queuing implementation in the qeth driver puts
outgoing packets in outbound queues 0 to 3.
This puts outgoing unicast packets into outbound queue 2 instead of
outbound queue 3 when using Priority Queuing on a HiperSocket.
Additionally, the default outbound queue cannot be set to outbound queue 3
on HiperSockets.

The function set_features is called to configure network device features
on the hardware. If errors occur, the network device features should
reflect the changed hardware state and the function should return an
error in order to notify the user.
In case of an error, the current implementation does not necessarily
save the changed hardware state in the network device features before an
error is returned.
This patch improves error handling by saving features, that could be
changed, to the network device features before returning an error. If
the device is not running, an additional check in fix_features removes
features, that require hardware changes, before they are passed to
set_features. Thus, the corresponding check was removed in set_features.

Network device features indicate the capabilities of network devices (e.g.,
TX checksum offloading and TSO) and their configuration state. Additional
network device features (vlan_features) indicate for each network device,
which capabilities can be used on VLAN devices, that are configured on the
respective base network device.
In the current qeth implementation, network device features are only set
for the base network devices and not for the VLAN devices. Thus, features
like TX checksum offloading cannot be used on VLAN devices.
This patch adds network device features to vlan_features, so they can be
used by VLAN devices.

This patch introduces a common set of fix_features and set_features
functions for layer 2 and layer 3. The RX, TX and TSO offload
functionality on the OSA card is enabled using ethtool at user's
request and not at device initialization as done before.
For layer 3 the RX checksum offloading is disabled at device
initialization time.

In layer3 mode of the qeth driver, multicast IP addresses
from struct net_device and other type of IP addresses
from other sources require mapping to the OSA-card.
This patch simplifies the IP address mapping logic, and changes imple-
mentation of ndo_set_rx_mode callback and ip notifier events.
Addresses are stored in private hashtables instead of lists now.
It allows hardware registration/removal for new/deleted multicast
addresses only.

When skb data touches too many pages, skb_linearize() is called
opportunistically in the hope that less pages will be required
for a big linear buffer than for multiple fragments. This patch
intoduces a separate counter in ethtool statistics structure
representing _failed_ linearization attempts.

Set scatter/gather ON by default on OSA, for both layer 2 and
layer 3 modes. We always use fragmentation over QDIO anyway,
so let the upper layers of the stack take advantage of that.

The patch enables NETIF_F_SG flag for OSA in layer 2 mode.
It also adds performance accounting for fragmented sends,
adds a conditional skb_linearize() attempt if the skb had
too many fragments for QDIO SBAL, and fills netdevice->gso_*
attributes.

It is possible to specify a user offset for the TOD clock, e.g. +2 hours.
The TOD clock will carry this offset even if the clock is synchronized
with STP. This makes the time stamps acquired with get_sync_clock()
useless as another LPAR migth use a different TOD offset.
Use the PTFF instrution to get the TOD epoch difference and subtract
it from the TOD clock value to get a physical timestamp. As the epoch
difference contains the sync check delta as well the LPAR offset value
to the physical clock needs to be refreshed after each clock
synchronization.

The sync clock operation of the channel subsystem call for STP delivers
the TOD clock difference as a result. Use this TOD clock difference
instead of the difference between the TOD timestamps before and after
the sync clock operation.

Show the dynamic and static cpu mhz of each cpu. Since these values
are per cpu this requires a fundamental extension of the format of
/proc/cpuinfo.
Historically we had only a single line per cpu and a summary at the
top of the file. This format is hardly extendible if we want to add
more per cpu information.
Therefore this patch adds per cpu blocks at the end of /proc/cpuinfo:

sclp_ocf.c is the only way to get the cpc name, as it registers the
sole event handler for the ocf event. By creating a new global
function that copies that name, we make it accessible to the world
which longs to retrieve it.
Additionally we now also store the cpc name as EBCDIC, so we don't
have to convert it to and from ASCII if it is requested in native
encoding.

any device. This enables the use of DAX in the presence of these
errors by making all sector-aligned zeroing go through the driver.
- The driver (already) has the ability to clear errors on writes that
are sent through the block layer using 'DSMs' defined in ACPI 6.1.
Other misc changes:
- When mounting DAX filesystems, check to make sure the partition
is page aligned. This is a requirement for DAX, and previously, we
allowed such unaligned mounts to succeed, but subsequent reads/writes
would fail.
- Misc/cleanup fixes from Jan that remove unused code from DAX related to
zeroing, writeback, and some size checks.

The disconnect and later reconnect of a 3270 terminal does not do
a tty hangup. The current session is resumed which is a security
issue.

If an existing 3270 terminal disconnects and reconnects with a
different size, the 3270 driver still works with the old size.
If the new dimensions are larger the output merely looks funny,
if the new dimensions are smaller the terminal is unusable.
To fix this restart the size detection after a unit check has
been received.

If a 3270 terminal is disconnected while the tty view is active
the 3270 driver goes into an endless loop of failed I/O requests
until the terminal is connected again.
Add code to the raw3270 interrupt handler to check for unit checks
due to failed I/O requests and put the device to sleep with the
RAW3270_FLAGS_BUSY flag until a unsolicited device end interrupt
indicates that the device can be used again. while we are at it
simplify the 3270 irq handling and remove unnecessary code.

The tty3270_update function tries to optimize the output stream by
skipping the TO_SBA command to position the output offset if the
TO_RA command of the previous line ended at the same offset.
But some lines do not have the TO_RA at the end, namely those
with a length equal to the number of columns.
Make sure there is a TO_RA at the end of the previous line to avoid
funny looking output.

The tty3270_close function set tty->driver_data to NULL after the
open count dropped to zero but tty3270_cleanup still needs the
pointer to the tty3270 view to drop the reference count.
Move the clearing of tty->driver_data to tty3270_cleanup and
drop the unnecessary tty_port_tty_set calls in tty3270_install.

TTY_PORT_INITIALIZED bit in the tty_port::iflags field. Introduce helpers
tty_port_set_initialized() and tty_port_initialized() to abstract
atomic bit ops.
Note: the transforms for test_and_set_bit() and test_and_clear_bit()
are unnecessary as the state transitions are already mutually exclusive;
the tty lock prevents concurrent open/close/hangup.

The sclp_ctl_ioctl_sccb function uses two copy_from_user calls to
retrieve the sclp request from user space. The first copy_from_user
fetches the length of the request which is stored in the first two
bytes of the request. The second copy_from_user gets the complete
sclp request, but this copies the length field a second time.
A malicious user may have changed the length in the meantime.

Sort the sclp event type defines and use a macro to create the
corresponding event type masks. In addition to that one unused
type/mask pair is removed and another previously unused define
is used now (it was probably unused/unknown because it didn't
follow the EVTYP_X EVTYP_X_MASK convention).

With this feature, applications can query if a DASD volume is online

dcssblk_remove_store() holds the dcssblk_devices_sem semaphore while
calling device_unregister(), which in turn tries to acquire the kernfs
kn->dev_map rwsem for the device sysfs subtree. The same rwsem is also
acquired when using the per-device sysfs attributes in the device sub-tree,
and the attribute handlers then also acquire the dcssblk_devices_sem.
This can lead to a deadlock when removing a DCSS while concurrently
reading from / writing to one of its sysfs attributes. The following

On larger installations it is useful to disable automatic LUN scanning,
and only add the required LUNs via udev rules. This can speed up bootup
dramatically.
This patch introduces a new scan module parameter value 'manual', which
works like 'none', but can be overridden by setting the 'rescan' value
from scsi_scan_target to 'SCSI_SCAN_MANUAL'. And it updates all
relevant callers to set the 'rescan' value to 'SCSI_SCAN_MANUAL' if
invoked via the 'scan' option in sysfs.

Reorder lcu and device lock to get rid of the error-prone trylock
mechanism.
The locking order is lcu lock -> device lock.
This protects against changes to the lcu device lists and enables us
to iterate over the devices, take the cdev lock and make changes to
the device structures.
The complicated part is the summary unit check handler that gets an
interrupt on one device of the lcu that leads to structural changes of
the whole lcu itself. This work needs to be done even if devices on
the lcu disappear. So a device independent worker is used.
The old approach tried to update some lcu structures and set up the
lcu worker in the interrupt context with the device lock held.
But this forced the lock order "cdev lock -> lcu lock" that made it
hard to have the lcu lock held and iterate over all devices and change
them.
The new approach is to schedule a device specific worker that gets
out of the interrupt context and rid of the device lock for summary
unit checks. This worker is able to take the lcu lock and schedule the
lcu worker that updates all devices. The time between interrupt and
worker execution is no problem because the devices in the lcu reject
all I/O in this time with an appropriate error. The dasd driver can
deal with this situation and re-drive the I/O later on.

Prepare for new format checking function by renaming functions and
moving reusable code to separate functions:
- Move sanity checks into a new function and make it reusable.
- Move common format code to a new function called
dasd_eckd_format_process_data.
- Create the generic function dasd_eckd_format_build_ccw_req, which
itself will then decide what ccw request is being built according to
the input data. (with upcoming functionality).

Currently, dasd_format is calling the format logic of a DASD discipline
with PAV enabled. If that fails with an error code of -EAGAIN the value
of retries is decremented and the discipline function is called with PAV
turned off.
The loop is supposed to try this up to 255 times until success. However,
-EAGAIN can only occur once here and therefore the loop will never reach
the 255 retries.
So, replace the unnecessarily complicated loop logic and simply try again
without PAV enabled in case of an -EAGAIN error.

SET_IND takes as a payload the _address_ of the indicators, meaning
that we have one of the rare cases where kmalloc(sizeof(&...)) is
actually correct. Let's clarify that with a comment.
The count for the ccw, however, was only correct because the
indicators are 64 bit. Let's use the correct value.

git commit 1ec2772e0c ("s390/diag: add a statistic for diagnose
calls") added function calls to gather diagnose statistics.
In case of the dasd diag driver the function call was added between a
register asm statement which initialized register r2 and the inline
assembly itself. The function call clobbers the contents of register
r2 and therefore the diag 0x250 call behaves in a more or less random
way.
Fix this by extracting the function call into a separate function like
we do everywhere else.

The correct lock order for LCU lock and cdev lock is to take the cdev
lock first and afterwards the LCU lock. This is caused by the fact
that LCU functions are called in an interrupt context with the cdev
lock implicitly hold by CIO.
To assure the right locking order but also be able to iterate over
devices in a LCU introduce a trylock block that can be called with
the device lock for one device hold and then takes the LCU lock and
try to lock all devices accounted to this LCU. Afterwards all devices
and the LCU itself are locked.

Commit ca369d51b ("sd: Fix device-imposed transfer length limits")
introduced a new queue limit max_dev_sectors which limits the maximum
sectors for requests. The default value leads to small dasd requests
and therefor to a performance drop.
Set the max_dev_sectors value to the same value as the max_hw_sectors
to use the maximum available request size for DASD devices.

The channel checks the specified length and the provided amount of
data for CCWs and provides an incorrect length error if the size does
not match. Under z/VM with simulation activated the length may get
changed. Having the suppress length indication bit set is stated as
good CCW coding practice and avoids errors under z/VM.

The tty core no longer provides nor uses ASYNC_CLOSING; remove from
tty_port_close_start() and tty_port_close_end() as well as tty drivers
which open-code these state changes. Unfortunately, even though the
bit is masked from userspace, its inclusion in a uapi header precludes
removing the macro.

Per channel path measurement characteristics are obtained during channel
path registration. However if some properties of a channel path change
we don't update the measurement characteristics.
Make sure to update the characteristics when we change the properties of
a channel path or receive a notification from FW about such a change.

Make sure that in all cases where we could not obtain measurement
characteristics the associated fields are set to invalid values.
Note: without this change the "shared" capability of a channel path
for which we could not obtain the measurement characteristics was
incorrectly displayed as 0 (not shared). We will now correctly
report "unknown" in this case.

Currently, on card response failures a combination of card domain and
domain id is recorded in the kernel messages.
According to the message description only the card id will be recorded.
The domain id is not relevant, since the whole card including all domains
is set offline.

For the purpose of communicating the optional presence of a 'struct
page' for the pfn returned from ->direct_access(), introduce a type that
encapsulates a page-frame-number plus flags. These flags contain the
historical "page_link" encoding for a scatterlist entry, but can also
denote "device memory". Where "device memory" is a set of pfns that are
not part of the kernel's linear mapping by default, but are accessed via
the same memory controller as ram.
The motivation for this new type is large capacity persistent memory
that needs struct page entries in the 'memmap' to support 3rd party DMA
(i.e. O_DIRECT I/O with a persistent memory source/target). However,
we also need it in support of maintaining a list of mapped inodes which
need to be unmapped at driver teardown or freeze_bdev() time.

Pull misc vfs updates from Al Viro:
"All kinds of stuff. That probably should've been 5 or 6 separate
branches, but by the time I'd realized how large and mixed that bag
had become it had been too close to -final to play with rebasing.
Some fs/namei.c cleanups there, memdup_user_nul() introduction and
switching open-coded instances, burying long-dead code, whack-a-mole
of various kinds, several new helpers for ->llseek(), assorted
cleanups and fixes from various people, etc.
One piece probably deserves special mention - Neil's
lookup_one_len_unlocked(). Similar to lookup_one_len(), but gets
called without ->i_mutex and tries to avoid ever taking it. That, of
course, means that it's not useful for any directory modifications,
but things like getting inode attributes in nfds readdirplus are fine
with that. I really should've asked for moratorium on lookup-related
changes this cycle, but since I hadn't done that early enough... I
*am* asking for that for the coming cycle, though - I'm going to try
and get conversion of i_mutex to rwsem with ->lookup() done under lock
taken shared.
There will be a patch closer to the end of the window, along the lines
of the one Linus had posted last May - mechanical conversion of
->i_mutex accesses to inode_lock()/inode_unlock()/inode_trylock()/
inode_is_locked()/inode_lock_nested(). To quote Linus back then:

checkpatch.pl wants arrays of strings declared as follows:

Currently the find_vqs() function takes a const char *names[] argument
so passing checkpatch.pl's const char * const names[] results in a
compiler error due to losing the second const.
This patch adjusts the find_vqs() prototype and updates all virtio
transports. This makes it possible for virtio_balloon.c, virtio_input.c,
virtgpu_kms.c, and virtio_rpmsg_bus.c to use the checkpatch.pl-friendly
type.

sclp_sync_wait() disables all external interrupt classes except for
the service signal subclass.
The static mask used for that however is wrong. It clears a couple of
bits which shouldn't be cleared and on the other hand potentially does
not clear bits which should be cleared.
Fix this by using the same generic mask like we do it in our delay
implementation.

Enabling failfast should let request fail immediately if either an
error occurred or the device gets disconnected.
For disconnected devices new requests are not fetches from the block
queue and therefore failfast is not triggered.
Fix by letting the DASD driver fetch requests for disconnected devices
with failfast active.

Return value from kzalloc is not tested and
using a null pointer would lead to crash.
Even if this should not happen at this moment, we
may let the system decide if there is a better choice.

Adding tracepoints to inline functions adds tracepoint invocation code
for each instance where the function is inlined. The resulting increase
in kernel code size can have negative impact:
- More cache misses in instruction cache
- Reduced amount of DMA-capable memory
Therefore change all functions implementing I/O instructions from inline
to normal functions.

Replace the current semi-arbitrary distribution of inline assemblies:
- Inline assemblies used by CIO go into ioasm.h
- Data definitions used by inline assemblies go into cio.h
Beyond cleaning up the current structure this is also required for
use of tracepoints in inline assemblies introduced by a follow-on
patch.

The numeric representation of the xsch instruction was incorrectly
specified, resulting in reserved fields of the instruction opcode
potentially being set to a non-zero value. While this doesn't currently
cause any problem, a future architecture might make use of these fields
so that the current specification could result in an exception or
unwanted side-effects.
Fix this by using the xsch instruction code for which support in
binutils was added in 2003.
