Mike Rapoport 782de70c42 mm: consolidate pgtable_cache_init() and pgd_cache_init()
Both pgtable_cache_init() and pgd_cache_init() are used to initialize kmem
cache for page table allocations on several architectures that do not use
PAGE_SIZE tables for one or more levels of the page table hierarchy.

Most architectures do not implement these functions and use __weak default
NOP implementation of pgd_cache_init().  Since there is no such default
for pgtable_cache_init(), its empty stub is duplicated among most
architectures.

Rename the definitions of pgd_cache_init() to pgtable_cache_init() and
drop empty stubs of pgtable_cache_init().

Nicholas Piggin 13224794cb mm: remove quicklist page table caches
Patch series "mm: remove quicklist page table caches".

A while ago Nicholas proposed to remove quicklist page table caches [1].

I've rebased his patch on the curren upstream and switched ia64 and sh to
use generic versions of PTE allocation.

This patch (of 3):

Remove page table allocator "quicklists".  These have been around for a
long time, but have not got much traction in the last decade and are only
used on ia64 and sh architectures.

The numbers in the initial commit look interesting but probably don't
apply anymore.  If anybody wants to resurrect this it's in the git
history, but it's unhelpful to have this code and divergent allocator
behaviour for minor archs.

Also it might be better to instead make more general improvements to page
allocator if this is still so slow.

Aneesh Kumar K.V faa6d21153 powerpc/nvdimm: use H_SCM_QUERY hcall on H_OVERLAP error
Right now we force an unbind of SCM memory at drcindex on H_OVERLAP error.
This really slows down operations like kexec where we get the H_OVERLAP
error because we don't go through a full hypervisor re init.

H_OVERLAP error for a H_SCM_BIND_MEM hcall indicates that SCM memory at
drc index is already bound. Since we don't specify a logical memory
address for bind hcall, we can use the H_SCM_QUERY hcall to query
the already bound logical address.

Boot time difference with and without patch is:

Aneesh Kumar K.V cf387d9644 libnvdimm/altmap: Track namespace boundaries in altmap
With PFN_MODE_PMEM namespace, the memmap area is allocated from the device
area. Some architectures map the memmap area with large page size. On
architectures like ppc64, 16MB page for memap mapping can map 262144 pfns.
This maps a namespace size of 16G.

When populating memmap region with 16MB page from the device area,
make sure the allocated space is not used to map resources outside this
namespace. Such usage of device area will prevent a namespace destroy.

Add resource end pnf in altmap and use that to check if the memmap area
allocation can map pfn outside the namespace. On ppc64 in such case we fallback
to allocation from memory.

Aneesh Kumar K.V 047e6575ae powerpc/mm: Fixup tlbie vs mtpidr/mtlpidr ordering issue on POWER9
On POWER9, under some circumstances, a broadcast TLB invalidation will
fail to invalidate the ERAT cache on some threads when there are
parallel mtpidr/mtlpidr happening on other threads of the same core.
This can cause stores to continue to go to a page after it's unmapped.

The workaround is to force an ERAT flush using PID=0 or LPID=0 tlbie
flush. This additional TLB flush will cause the ERAT cache
invalidation. Since we are using PID=0 or LPID=0, we don't get
filtered out by the TLB snoop filtering logic.

We need to still follow this up with another tlbie to take care of
store vs tlbie ordering issue explained in commit:
a5d4b5891c ("powerpc/mm: Fixup tlbie vs store ordering issue on
POWER9"). The presence of ERAT cache implies we can still get new
stores and they may miss store queue marking flush.

Aneesh Kumar K.V 677733e296 powerpc/book3s64/mm: Don't do tlbie fixup for some hardware revisions
The store ordering vs tlbie issue mentioned in commit
a5d4b5891c ("powerpc/mm: Fixup tlbie vs store ordering issue on
POWER9") is fixed for Nimbus 2.3 and Cumulus 1.3 revisions. We don't
need to apply the fixup if we are running on them

We can only do this on PowerNV. On pseries guest with KVM we still
don't support redoing the feature fixup after migration. So we should
be enabling all the workarounds needed, because whe can possibly
migrate between DD 2.3 and DD 2.2

Laurent Dufour 59545ebe33 powerpc/pseries: Call H_BLOCK_REMOVE when supported
Depending on the hardware and the hypervisor, the hcall H_BLOCK_REMOVE
may not be able to process all the page sizes for a segment base page
size, as reported by the TLB Invalidate Characteristics.

For each pair of base segment page size and actual page size, this
characteristic tells us the size of the block the hcall supports.

In the case, the hcall is not supporting a pair of base segment page
size, actual page size, it is returning H_PARAM which leads to a panic
like this:

The call to H_BLOCK_REMOVE should only be made for the supported pair
of base segment page size, actual page size and using the correct
maximum block size.

Due to the required complexity in do_block_remove() and
call_block_remove(), and the fact that currently a block size of 8 is
returned by the hypervisor, we are only supporting 8 size block to the
H_BLOCK_REMOVE hcall.

In order to identify this limitation easily in the code, a local
define HBLKR_SUPPORTED_SIZE defining the currently supported block
size, and a dedicated checking helper is_supported_hlbkr() are
introduced.

For regular pages and hugetlb, the assumption is made that the page
size is equal to the base page size. For THP the page size is assumed
to be 16M.

Laurent Dufour 1211ee61b4 powerpc/pseries: Read TLB Block Invalidate Characteristics
The PAPR document specifies the TLB Block Invalidate Characteristics
which tells for each pair of segment base page size, actual page size,
the size of the block the hcall H_BLOCK_REMOVE supports.

These characteristics are loaded at boot time in a new table
hblkr_size. The table is separate from the mmu_psize_def because this
is specific to the pseries platform.

A new init function, pseries_lpar_read_hblkrm_characteristics() is
added to read the characteristics. It is called from
pSeries_setup_arch().

On a 2-socket Power9 system with 32 cores/128 threads (SMT4) and 1TB
of memory running the following guest configs:

  guest A:
    - 224GB of memory
    - 56 VCPUs (sockets=1,cores=28,threads=2), where:
      VCPUs 0-1 are pinned to CPUs 0-3,
      VCPUs 2-3 are pinned to CPUs 4-7,
      ...
      VCPUs 54-55 are pinned to CPUs 108-111

  guest B:
    - 4GB of memory
    - 4 VCPUs (sockets=1,cores=4,threads=1)

with the following workloads (with KSM and THP enabled in all):

  guest A:
    stress --cpu 40 --io 20 --vm 20 --vm-bytes 512M

It was subsequently found that ipi_message[PPC_MSG_CALL_FUNCTION]
was set for CPU 42 by at least 1 of the CPUs waiting in
smp_call_function_many(), but somehow the corresponding
call_single_queue entries were never processed by CPU 42, causing the
callers to spin in csd_lock_wait() indefinitely.

Nick Piggin suggested something similar to the following sequence as
a possible explanation (interleaving of CALL_FUNCTION/RESCHEDULE
IPI messages seems to be most common, but any mix of CALL_FUNCTION and
!CALL_FUNCTION messages could trigger it):

Fixing this scenario would require an smp_mb() *after* clearing
host_ipi flag in kvmppc_set_host_ipi() to order the store vs.
subsequent processing of IPI messages.

To handle both cases, this patch splits kvmppc_set_host_ipi() into
separate set/clear functions, where we execute smp_mb() prior to
setting host_ipi flag, and after clearing host_ipi flag. These
functions pair with each other to synchronize the sender and receiver
sides.

With that change in place the above workload ran for 20 hours without
triggering any lock-ups.

Don't map the early shadow page read-only yet when creating the new
page tables for the real shadow memory, otherwise the memblock
allocations that immediately follows to create the real shadow pages
that are about to replace the early shadow page trigger a page fault
if they fall into the region being worked on at the moment.

Jordan Niethe 13c7bb3c57 powerpc/64s: Set reserved PCR bits
Currently the reserved bits of the Processor Compatibility
Register (PCR) are cleared as per the Programming Note in Section
1.3.3 of version 3.0B of the Power ISA. This causes all new
architecture features to be made available when running on newer
processors with new architecture features added to the PCR as bits
must be set to disable a given feature.

For example to disable new features added as part of Version 2.07 of
the ISA the corresponding bit in the PCR needs to be set.

As new processor features generally require explicit kernel support
they should be disabled until such support is implemented. Therefore
kernels should set all unknown/reserved bits in the PCR such that any
new architecture features which the kernel does not currently know
about get disabled.

An update is planned to the ISA to clarify that the PCR is an
exception to the Programming Note on reserved bits in Section 1.3.3.

Alistair Popple c6fadabb28 powerpc: Fix definition of PCR bits to work with old binutils
Commit 388cc6e133 ("KVM: PPC: Book3S HV: Support POWER6
compatibility mode on POWER7") introduced new macros defining the PCR
bits. When used from assembly files these definitions lead to build
errors using older versions of binutils that don't support the 'ul'
suffix. This fixes the build errors by updating the definitions to use
the __MASK() macro which selects the appropriate suffix.

Aneesh Kumar K.V d9101bfa6a powerpc/mm/mce: Keep irqs disabled during lockless page table walk
__find_linux_mm_pte() returns a page table entry pointer after walking
the page table without holding locks. To make it safe against a THP
split and/or collapse, we disable interrupts around the lockless page
table walk. However we need to keep interrupts disabled as long as we
use the page table entry pointer that is returned.

Fix addr_to_pfn() to do that.

David Howells d2e0981c3b vfs: Convert spufs to use the new mount API
Convert the spufs filesystem to the new internal mount API as the old
one will be obsoleted and removed.  This allows greater flexibility in
communication of mount parameters between userspace, the VFS and the
filesystem.

 Two major chunks of code are moving out of the tree and into the staging
 directory, uwb and wusb (wireless USB support), because there are no
 devices that actually use this protocol anymore, and what we have today
 probably doesn't work at all given that the maintainers left many many
 years ago.  So move it to staging where it will be removed in a few
 releases if no one screams.
 
 Other than that, lots of little things.  The usual gadget and xhci and
 usb serial driver updates, along with a bunch of sysfs file cleanups due
 to the driver core changes to support that.  Nothing really major, just
 constant forward progress.
 
 All of these have been in linux-next for a while with no reported

Ganesh Goudar e7ca44ed3b powerpc: dump kernel log before carrying out fadump or kdump
Since commit 4388c9b3a6 ("powerpc: Do not send system reset request
through the oops path"), pstore dmesg file is not updated when dump is
triggered from HMC. This commit modified system reset (sreset) handler
to invoke fadump or kdump (if configured), without pushing dmesg to
pstore. This leaves pstore to have old dmesg data which won't be much
of a help if kdump fails to capture the dump. This patch fixes that by
calling kmsg_dump() before heading to fadump ot kdump.

Cédric Le Goater 855d9140a3 powerpc/xmon: Fix output of XIVE IPI
When dumping the XIVE state of an CPU IPI, xmon does not check if the
CPU is started or not which can cause an error. Add a check for that
and change the output to be on one line just as the XIVE interrupts of
the machine.

Hari Bathini 7b1b3b4825 powerpc/fadump: consider f/w load area
OPAL loads kernel & initrd at 512MB offset (256MB size), also exported
as ibm,opal/dump/fw-load-area. So, if boot memory size of FADump is
less than 768MB, kernel memory to be exported as '/proc/vmcore' would
be overwritten by f/w while loading kernel & initrd. To avoid such a
scenario, enforce a minimum boot memory size of 768MB on OPAL platform
and skip using FADump if a newer F/W version loads kernel & initrd
above 768MB.

Also, irrespective of RMA size, set the minimum boot memory size
expected on pseries platform at 320MB. This is to avoid inflating the
minimum memory requirements on systems with 512M/1024M RMA size.

Hari Bathini bec53196ad powerpc/fadump: add support to preserve crash data on FADUMP disabled kernel
Add a new kernel config option, CONFIG_PRESERVE_FA_DUMP that ensures
that crash data, from previously crash'ed kernel, is preserved. This
helps in cases where FADump is not enabled but the subsequent memory
preserving kernel boot is likely to process this crash data. One
typical usecase for this config option is petitboot kernel.

As OPAL allows registering address with it in the first kernel and
retrieving it after MPIPL, use it to store the top of boot memory.
A kernel that intends to preserve crash data retrieves it and avoids
using memory beyond this address.

Move arch_reserved_kernel_pages() function as it is needed for both
FA_DUMP and PRESERVE_FA_DUMP configurations.

Hari Bathini 51bba8edef powerpc/fadump: support copying multiple kernel boot memory regions
Firmware uses a 32-bit field for size while copying/backing-up memory
during MPIPL. So, the maximum value that could be represented with
a PAGE_SIZE aligned 32-bit field will be the maximum copy size for a
region but FADump capture kernel usually needs more memory than that
to be preserved to avoid running into out of memory errors.

So, request firmware to copy multiple kernel boot memory regions
instead of just one (which worked fine for pseries as 64-bit field
was used for size there).

Currently, the O= build shows the absolute path to the script:

With this commit, it is always a relative path to the timestamp file:

Michael Ellerman 0cb0837f9d powerpc/kvm: Move kvm_tmp into .text, shrink to 64K
In some configurations of KVM, guests binary patch themselves to
avoid/reduce trapping into the hypervisor. For some instructions this
requires replacing one instruction with a sequence of instructions.

For those cases we need to write the sequence of instructions
somewhere and then patch the location of the original instruction to
branch to the sequence. That requires that the location of the
sequence be within 32MB of the original instruction.

The current solution for this is that we create a 1MB array in BSS,
write sequences into there, and then free the remainder of the array.

This has a few problems:
 - it confuses kmemleak.
 - it confuses lockdep.
 - it requires mapping kvm_tmp executable, which can cause adjacent
   areas to also be mapped executable if we're using 16M pages for the
   linear mapping.
 - the 32MB limit can be exceeded if the kernel is big enough,
   especially with STRICT_KERNEL_RWX enabled, which then prevents the
   patching from working at all.

We can fix all those problems by making kvm_tmp just a region of
regular .text. However currently it's 1MB in size, and we don't want
to waste 1MB of text. In practice however I only see ~30KB of kvm_tmp
being used even for an allyes_config. So shrink kvm_tmp to 64K, which
ought to be enough for everyone, and move it into .text.

Greg Kurz 6ccb4ac2bf powerpc/xive: Fix bogus error code returned by OPAL
There's a bug in skiboot that causes the OPAL_XIVE_ALLOCATE_IRQ call
to return the 32-bit value 0xffffffff when OPAL has run out of IRQs.
Unfortunatelty, OPAL return values are signed 64-bit entities and
errors are supposed to be negative. If that happens, the linux code
confusingly treats 0xffffffff as a valid IRQ number and panics at some
point.

A fix was recently merged in skiboot:

e97391ae2bb5 ("xive: fix return value of opal_xive_allocate_irq()")

but we need a workaround anyway to support older skiboots already
in the field.

Internally convert 0xffffffff to OPAL_RESOURCE which is the usual error
returned upon resource exhaustion.

Nathan Lynch 92c94dfb69 powerpc/pseries: correctly track irq state in default idle
prep_irq_for_idle() is intended to be called before entering
H_CEDE (and it is used by the pseries cpuidle driver). However the
default pseries idle routine does not call it, leading to mismanaged
lazy irq state when the cpuidle driver isn't in use. Manifestations of
this include:

* Dropped IPIs in the time immediately after a cpu comes
  online (before it has installed the cpuidle handler), making the
  online operation block indefinitely waiting for the new cpu to
  respond.

* Hitting this WARN_ON in arch_local_irq_restore():

Ravi Bangoria bc01bdf6c5 powerpc/watchpoint: Disable watchpoint hit by larx/stcx instructions
If watchpoint exception is generated by larx/stcx instructions, the
reservation created by larx gets lost while handling exception, and
thus stcx instruction always fails. Generally these instructions are
used in a while(1) loop, for example spinlocks. And because stcx
never succeeds, it loops forever and ultimately hangs the system.

Note that ptrace anyway works in one-shot mode and thus for ptrace
we don't change the behaviour. It's up to ptrace user to take care
of this.

Actual bug here is that, we have two loops with potentially different
loop counts. That is, in imc_get_mem_addr_nest(), loop count is
obtained from the dt entries. But in case of export_imc_mode_and_cmd(),
loop was based on for_each_nid() count. Patch fixes the loop count in
latter based on the struct mem_info. Ideally it would be better to
have array size in struct imc_pmu.

Nicholas Piggin 2275d7b575 powerpc/64s/radix: introduce options to disable use of the tlbie instruction
Introduce two options to control the use of the tlbie instruction. A
boot time option which completely disables the kernel using the
instruction, this is currently incompatible with HASH MMU, KVM, and
coherent accelerators.

And a debugfs option can be switched at runtime and avoids using tlbie
for invalidating CPU TLBs for normal process and kernel address
mappings. Coherent accelerators are still managed with tlbie, as will
KVM partition scope translations.

Cross-CPU TLB flushing is implemented with IPIs and tlbiel. This is a
basic implementation which does not attempt to make any optimisation
beyond the tlbie implementation.

This is useful for performance testing among other things. For example
in certain situations on large systems, using IPIs may be faster than
tlbie as they can be directed rather than broadcast. Later we may also
take advantage of the IPIs to do more interesting things such as trim
the mm cpumask more aggressively.

tlbiel_all, make these flushes conditional on CPU_FTR_HVMODE. Process
scope translations are the only type visible to the guest.

Nicholas Piggin 99161de3a2 powerpc/64s/radix: tidy up TLB flushing code
There should be no functional changes.

- Use calls to existing radix_tlb.c functions in flush_partition.

- Rename radix__flush_tlb_lpid to radix__flush_all_lpid and similar,
  because they flush everything, matching flush_all_mm rather than
  flush_tlb_mm for the lpid.

- Remove some unused radix_tlb.c flush primitives.
