Linus Torvalds aa5ad10f6c SCSI misc on 20221213
Updates to the usual drivers (target, ufs, smartpqi, lpfc). There are
 some core changes, mostly around reworking some of our user context
 assumptions in device put and moving some code around. The remaining
 updates are bug fixes and minor changes.

Al Viro de4eda9de2 use less confusing names for iov_iter direction initializers
READ/WRITE proved to be actively confusing - the meanings are
"data destination, as used with read(2)" and "data source, as
used with write(2)", but people keep interpreting those as
"we read data from it" and "we write data to it", i.e. exactly
the wrong way.

Call them ITER_DEST and ITER_SOURCE - at least that is harder
to misinterpret...

Maurizio Lombardi fec1b2fa62 scsi: target: iscsi: Fix a race condition between login_work and the login thread
In case a malicious initiator sends some random data immediately after a
login PDU; the iscsi_target_sk_data_ready() callback will schedule the
login_work and, at the same time, the negotiation may end without clearing
the LOGIN_FLAGS_INITIAL_PDU flag (because no additional PDU exchanges are
required to complete the login).

The login has been completed but the login_work function will find the
LOGIN_FLAGS_INITIAL_PDU flag set and will never stop from rescheduling
itself; at this point, if the initiator drops the connection, the
iscsit_conn structure will be freed, login_work will dereference a released
socket structure and the kernel crashes.

BUG: kernel NULL pointer dereference, address: 0000000000000230

Jason A. Donenfeld a251c17aa5 treewide: use get_random_u32() when possible
The prandom_u32() function has been a deprecated inline wrapper around
get_random_u32() for several releases now, and compiles down to the
exact same code. Replace the deprecated wrapper with a direct call to
the real function. The same also applies to get_random_int(), which is
just a wrapper around get_random_u32(). This was done as a basic find
and replace.

Variable length SCSI commands are transferred over iSCSI via two CDB
buffers - in Basic Header Segment and in Additional Header Segment (AHS).
Since AHS is not supported yet, a target reads just BHS (48 byte) from TCP
and treats the remaining octets as a next new iSCSI PDU that causes
protocol errors.

Add support for the Extended CDB AHS type.

For all warnings, the format specifier is '%hu' which describes an unsigned
short. The resulting type of atomic_read is an int. The proposed fix is to
listen to Clang and swap the format specifier.

Guenter Roeck 998da772fd scsi: target: iscsi: Drop unnecessary container_of()
The structure pointer passed to container_of() is never NULL; that was
already checked. That means that the result of container_of() operations
on it is also never NULL, even though se_node_acl is the first element
of the structure embedding it. On top of that, it is misleading to perform
a NULL check on the result of container_of() because the position of the
contained element could change, which would make the test invalid.
Remove the unnecessary NULL check.

As it turns out, the container_of operation was only made for the purpose
of the NULL check. If the container_of is actually needed, it is repeated
later. Remove the container_of operation as well.

The NULL check was identified and removed with the following Coccinelle
script.

GFP_KERNEL is kept.

This will also be useful in subsequent patches where loop needs to do
target_submit_prep() from interrupt context to get a ref to the se_device,

Ayush Sawal 2355a6773a cxgb4/chtls/cxgbit: Keeping the max ofld immediate data size same in cxgb4 and ulds
The Max imm data size in cxgb4 is not similar to the max imm data size
in the chtls. This caused an mismatch in output of is_ofld_imm() of
cxgb4 and chtls. So fixed this by keeping the max wreq size of imm data
same in both chtls and cxgb4 as MAX_IMM_OFLD_TX_DATA_WR_LEN.

As cxgb4's max imm. data value for ofld packets is changed to
MAX_IMM_OFLD_TX_DATA_WR_LEN. Using the same in cxgbit also.

Sebastian Andrzej Siewior f88a10f80d scsi: target: iscsi: Redo iscsit_check_session_usage_count() return code
The return value of iscsit_check_session_usage_count() is only checked if
it was not allowed to sleep. If it returns `2' then a timer is prepared. If
it returns something else or if it was allowed to sleep then it is ignored.

Let iscsit_check_session_usage_count() return true if it needs to arm the
timer - otherwise false. This simplifies the code flow of the only caller.

Sebastian Andrzej Siewior efc9d73063 scsi: target: iscsi: Avoid in_interrupt() usage in iscsit_check_session_usage_count()
iscsit_check_session_usage_count() uses in_interrupt() to find out if it is
safe to invoke wait_for_completion().

The usage of in_interrupt() in drivers is phased out and Linus clearly
requested that code which changes behaviour depending on context should
either be separated or the context be conveyed in an argument passed by the
caller, which usually knows the context.

There is only one caller of iscsit_check_session_usage_count() which
already has an argument indicating if it is safe to sleep.

Extend iscsit_check_session_usage_count() by an argument indicating if it
may sleep.

Sebastian Andrzej Siewior 433675486a scsi: target: iscsi: Avoid in_interrupt() usage in iscsit_close_session()
iscsit_close_session() uses in_interrupt() to decide if it needs to check
the return value of iscsit_check_session_usage_count() if it was not able
to sleep.

The usage of in_interrupt() in drivers is phased out and Linus clearly
requested that code which changes behaviour depending on context should
either be separated or the context be conveyed in an argument passed by the
caller, which usually knows the context.

iscsit_close_session() has two callers:

 - iscsit_handle_time2retain_timeout()
   A timer_list callback.

 - iscsit_close_connection()
   Runs in preemptible context, acquires a mutex.

Add an argument to iscsit_close_session() indicating if sleeping is
possible.

Mike Christie f36199355c scsi: target: iscsi: Fix cmd abort fabric stop race
Maurizio found a race where the abort and cmd stop paths can race as
follows:

 1. thread1 runs iscsit_release_commands_from_conn and sets
    CMD_T_FABRIC_STOP.

 2. thread2 runs iscsit_aborted_task and then does __iscsit_free_cmd. It
    then returns from the aborted_task callout and we finish
    target_handle_abort and do:

    target_handle_abort -> transport_cmd_check_stop_to_fabric ->
	lio_check_stop_free -> target_put_sess_cmd

    The cmd is now freed.

 3. thread1 now finishes iscsit_release_commands_from_conn and runs
    iscsit_free_cmd while accessing a command we just released.

In __target_check_io_state we check for CMD_T_FABRIC_STOP and set the
CMD_T_ABORTED if the driver is not cleaning up the cmd because of a session
shutdown. However, iscsit_release_commands_from_conn only sets the
CMD_T_FABRIC_STOP and does not check to see if the abort path has claimed
completion ownership of the command.

This adds a check in iscsit_release_commands_from_conn so only the abort or
fabric stop path cleanup the command.

[<0>] iscsi_target_login_thread+0x2f/0x5a
[<0>] kthread+0xfa/0x130
[<0>] ret_from_fork+0x1f/0x30

This can be reproduced via the following steps:

1. Initiator A tries to log in to iqn1-tpg1 on port 3260. After finishing
   PDU exchange in the login thread and before the negotiation is finished
   the the network link goes down. At this point A has not finished login
   and tpg->np_login_sem is held.

2. Initiator B tries to log in to iqn2-tpg1 on port 3260. After finishing
   PDU exchange in the login thread the target expects to process remaining
   login PDUs in workqueue context.

3. Initiator A' tries to log in to iqn1-tpg1 on port 3260 from a new
   socket. A' will wait for tpg->np_login_sem with np->np_login_timer
   loaded to wait for at most 15 seconds. The lock is held by A so A'
   eventually times out.

4. Before A' got timeout initiator B gets negotiation failed and calls
   iscsi_target_login_drop()->iscsi_target_login_sess_out().  The
   np->np_login_timer is canceled and initiator A' will hang forever.
   Because A' is now in the login thread, no new login requests can be
   serviced.

Fix this by moving iscsi_stop_login_thread_timer() out of
iscsi_target_login_sess_out(). Also remove iscsi_np parameter from
iscsi_target_login_sess_out().

Varun Prakash 5528d03183 scsi: target: iscsi: Fix data digest calculation
Current code does not consider 'page_off' in data digest calculation. To
fix this, add a local variable 'first_sg' and set first_sg.offset to
sg->offset + page_off.

Gustavo A. R. Silva df561f6688 treewide: Use fallthrough pseudo-keyword
Replace the existing /* fall through */ comments and its variants with
the new pseudo-keyword macro fallthrough[1]. Also, remove unnecessary
fall-through markings when it is the case.
