GPNID is triggered by RSCN. For multiple RSCNs of the same affected
NPORT ID, serialize the GPNID to prevent confusion.

When NPort Handle is in use, driver needs to mark the handle as used and
pick another. Instead, the code clears the handle and re-pick the same
handle.

Fix co-existence between Block MQ and Target Mode. Block MQ and
initiator mode requires midlayer queue mapping to check for IRQ to be

While testing "sd: Micro-optimize READ / WRITE CDB encoding" patches it
was helpful to check various code paths associated with READ/WRITE 6, 10
and 16 byte cdb variants. There seems to be no user space "knobs" to
twiddle use_10_for_rw and friends in the scsi_device structure. So add
a parameter to scsi_debug called "cdb_len" for this purpose.

Since scsi_get_device_flags_keyed() callers do not check whether or not
the returned value is an error code, change that function such that it
returns a flags value even if the 'key' argument is invalid. Note:
since commit 28a0bc4120 ("scsi: sd: Implement blacklist option for
WRITE SAME w/ UNMAP") bit 31 is a valid device information flag so
checking whether bit 31 is set in the return value is not sufficient to
tell the difference between an error code and a flags value.

If scsi_eh_scmd_add() is called concurrently with
scsi_host_queue_ready() while shost->host_blocked > 0 then it can
happen that neither function wakes up the SCSI error handler. Fix
this by making every function that decreases the host_busy counter
wake up the error handler if necessary and by protecting the
host_failed checks with the SCSI host lock.

We are testing if there is a match with the ses device in a loop by
calling ses_match_to_enclosure(), which will issue scsi receive
diagnostics commands to the ses device for every device on the same
host. On one of our boxes with 840 disks, it takes a long time to load
the driver:

There is no need to go through an intermediate timespec to convert to
ktime_t when we just want a simple multiplication. This gets rid of one

The logic for sg_seg_cnt is a bit convoluted. This patch tries to clean
up a couple of areas, especially around the +2 and +1 logic.
This patch:
- Cleans up the lpfc_sg_seg_cnt attribute to specify a real minimum
rather than making the minimum be whatever the default is.
- Removes the hardcoding of +2 (for the number of elements we use in a
sgl for cmd iu and rsp iu) and +1 (an additional entry to compensate
for nvme's reduction of io size based on a possible partial page)
logic in sg list initialization. In the case where the +1 logic is
referenced in host and target io checks, use the values set in the
transport template as that value was properly set.
There can certainly be more done in this area and it will be addressed
in combined host/target driver effort.

During driver unload, the driver may crash due to NULL pointers. The
NULL pointers were due to the driver not protecting itself sufficiently
during some of the teardown paths. Additionally, the driver was not
waiting for and cleanup up nvme io resources. As such, the driver wasn't
making the callbacks to the transport, stalling the transports
association teardown.
This patch waits for io clean up before tearding down and adds checks
for possible NULL pointers.

When the driver is unloading, the nvme transport could be in the process
of submitting new requests, will send abort requests to terminate
associations, or may make LS-related requests. The driver's abort and
request entry points currently is ignorant of the unloading state and is
starting the requests even though the infrastructure to complete them
continues to teardown.
Change the entry points for new requests to check whether unloading and
if so, reject the requests. Abort routines check unloading, and if so,
noop the request. An abort is noop'd as the teardown paths are already
aborting/terminating the io outstanding at the time the teardown
initiated.

The driver's interaction with the host nvme transport has been incorrect
for a while. The driver did not wait for the unregister callbacks
(waited only 5 jiffies). Thus the driver may remove objects that may be
referenced by subsequent abort commands from the transport, and the
actual unregister callback was effectively a noop. This was especially
problematic if the driver was unloaded.
The driver now waits for the unregister callbacks, as it should, before
continuing with teardown.

During RSCN storms, the driver does not rediscover some targets. The
driver marks some RSCN as to be handled after the ones it's working
on. The driver missed processing some deferred RSCN.
Move where the driver checks for deferred RSCNs and initiate deferred
RSCN handling if the flag was set. Also revise nport state within the
RSCN confirm routine. Add some state data to a possible debug print to
aid future debugging.

The current default for async hw receive queues is 1, which presents
issues under heavy load as number of queues influence the available
async receive buffer limits.
Raise the default to the either the current hw limit (16) or the number
of hw qs configured (io channel value).
Revise the attribute definition for mrq to better reflect what we do for
hw queues. E.g. 0 means default to optimal (# of cpus), non-zero
specifies a specific limit. Before this change, mrq=0 meant target mode
was disabled. As 0 now has a different meaning, rework the if tests to
use the better nvmet_support check.

Performing an LS abort results in the following message being seen:
0603 Invalid CQ subtype 6: 00000300 22000002 ffff0016 d0050000
and the associated exchange is not properly freed.
The code did not recognize the exchange type that was aborted, thus it
was not properly handled.
Correct by adding the NVME LS ELS type to the exchange types that are
recognized.

In test cases where an instance of the driver is detached and
reattached, the driver will crash on reattachment. There is a compound
if statement that will skip over the bar setup if the pci_resource_start
call is not successful. The driver erroneously returns success to its
bar setup in this scenario even though the bars aren't properly
configured.
Rework the offending code segment for proper initialization steps. If
the pci_resource_start call fails, -ENOMEM is now returned.

XRI_ABORTED_CQE completions were not being handled in the fast path.
They were being queued and deferred to the lpfc worker thread for
processing. This is an artifact of the driver design prior to moving
queue processing out of the isr and into a workq element. Now that queue
processing is already in a deferred context, remove this artifact and
process them directly.

Hardware queues are a fast staging area to push commands into the
adapter. The adapter should drain them extremely quickly. However,
under heavy io load, the host cpu is pushing commands faster than the
drain rate of the adapter causing the driver to resource busy commands.
Enlarge the hardware queue (wq & cq) to support a larger number of queue
entries (4x the prior size) before backpressure. Enlarging the queue
requires larger contiguous buffers (16k) per logical page for the
hardware. This changed calling sequences that were expecting 4K page
sizes that now must pass a parameter with the page sizes. It also
required use of a new version of an adapter command that can vary the
page size values.

There is no need to #define the license of the driver, just put it in
the MODULE_LICENSE() line directly as a text string.
This allows tools that check that the module license matches the source
code license to work properly, as there is no need to unwind the
unneeded dereference, especially when the string is defined in a .h file
far away from the .c file it is used in.

The TW_IOCTL_GET_LOCK ioctl uses do_gettimeofday() to check whether a
lock has expired. This can misbehave due to a concurrent settimeofday()
call, as it is based on 'real' time, and it will overflow in y2038 on
32-bit architectures, producing unexpected results when used across the
overflow time.
This changes it to using monotonic time, using ktime_get() to simplify
the code.

The calculation of the number of seconds since Sunday 00:00:00 overflows
in 2106, meaning that we instead will return the seconds since Wednesday
06:28:16 afterwards.
Using 64-bit time stamps avoids this slight inconsistency, and the
deprecated do_gettimeofday(), replacing it with the simpler
ktime_get_real_seconds().

twl_aen_queue_event/twa_aen_queue_event, we use do_gettimeofday() to
read the lower 32 bits of the current time in seconds, to pass them to
the TW_IOCTL_GET_NEXT_EVENT ioctl or the 3ware_aen_read sysfs file.
This will overflow on all architectures in year 2106, there is not much
we can do about that without breaking the ABI. User space has 90 years
to learn to deal with it, so it's probably ok.
I'm changing it to use ktime_get_real_seconds() with a comment to
document what happens when.

bfa_aen_entry_s is passed through a netlink socket that can be read by
either 32-bit or 64-bit processes, but the data format is different
between the two on current implementations.
Originally, this was using a 'struct timeval', which also suffers from
getting redefined with a new libc implementation.
With this patch, the layout gets fixed to having two 64-bit members for
the time, making it the same on 32-bit kernels and 64-bit kernels
running either compat or native user space including x32.
Provided that the new header file gets used to recompile any 32-bit
application binaries, this will fix running those on a 64-bit kernel
(with or without this patch) e.g. in a container environment, and it
will make binaries work that will be built against a future 32-bit glibc
that uses a 64-bit time_t, and avoid the y2038 overflow there.
However, this also breaks compatibility with any existing 32-bit binary
running on a native 32-bit kernel, those must be recompiled against the
new header, which in turn makes them incompatible with older kernels
unless the same change gets applied there.
Obviously this patch should only be applied when the benefits outweigh
the possible breakage. I'm posting it under the assumption that there
are no open-source tools using the netlink interface, and that users of
the binaries provided by qlogic for SLES10/11 and RHEL5/6 are not
actually being used on new future systems with 32-bit x86 kernels.

bfa_aen_entry_s is passed to user space in a netlink message, but is
defined using a 'struct timeval' and an 'enum' that are not only
different between architectures, but also between 32-bit user space and
64-bit kernels they may run on, as well as depending on the particular C
library that defines timeval.
This changes the in-kernel definition to no longer use the timeval type
directly but instead use two open-coded 'unsigned long' members. This
keeps the existing ABI, but making the variable unsigned also helps make
it work after y2038, until it overflows in 2106.
Since the macro becomes overly complex at this point, I'm changing it to
an inline function for readability.
I'm not changing the 32-bit user-space ABI at this point, to keep the
changes separate, I deally this would be defined using the same binary
layout for all architectures.

The bfa_get_log_time() returns a 64-bit timestamp that does not suffer
from the y2038 overflow on 64-bit systems. However, on 32-bit
architectures the timestamp will jump from 0x000000007fffffff to
0xffffffff80000000 in y2038 and produce wrong results.
The ktime_get_real_seconds() function does the same thing as
bfa_get_log_time() without that problem, so we can simply remove the
former use ktime_get_real_seconds() instead.

io_profile_start_time() gets read using do_gettimeofday() and passed
down as a 32-bit value through multiple functions. This will overflow in
y2038 or y2106, depending on whether it gets interpreted as unsigned in
the end.
This changes do_gettimeofday() to ktime_get_real_seconds() and pushes
the point at which it overflows to where we actually assign it to the
bfa_fcpim_del_itn_stats_s structure, with an appropriate comment.

In bfa_ioc_send_enable, we use the deprecated do_gettimeofday() function
to read the current time. This is not a problem, since the firmware
interface is already limited to 32-bit timestamps, but it's better to
use ktime_get_seconds() and document what the limitation is.
I noticed that I did the same change in commit a5af839253 ("bna: avoid
writing uninitialized data into hw registers") for the ethernet
driver. That commit also changed the "disable" funtion to initialize the
data we pass to the firmware properly, so I'm doing the same thing here.

We use the deprecated do_gettimeofday() function to read the current
time when resetting the statistics in both bfa_port and bfa_svc. This
works fine because overflow is handled correctly, but we want to get rid
of do_gettimeofday() and using a non-monotonic time suffers from
concurrent settimeofday calls and other problems.
This uses the ktime_get_seconds() function instead, which does what we
need here.

BFA_TRC_TS() calculates a 32-bit microsecond timestamp using the
deprecated do_gettimeofday() function. This overflows roughly every 71
minutes, so it's obviously not used as an absolute time stamp, but it
seems wrong to use a time base for it that will jump during
settimeofday() calls, leap seconds, or the y2038 overflow.
This converts it to ktime_get_ts64(), which has none of those problems
but is not synchronized to wall-clock time.

This mechanically converts all remaining cases of ancient open-coded timer
setup with the old setup_timer() API, which is the first step in timer
conversions. This has no behavioral changes, since it ultimately just
changes the order of assignment to fields of struct timer_list when
finding variations of:

If a cleanup task is not responded to while we are in bnx2fc_abts_cleanup, it
will hang the SCSI error handler since we use wait_for_completion instead of
wait_for_completion_timeout. So, use wait_for_completion_timeout so that we
don't hang the SCSI error handler thread forever.

The contexts from which a SCSI device can be quiesced or resumed are:
* Writing into /sys/class/scsi_device/*/device/state.
* SCSI parallel (SPI) domain validation.
* The SCSI device power management methods. See also scsi_bus_pm_ops.
It is essential during suspend and resume that neither the filesystem
state nor the filesystem metadata in RAM changes. This is why while
the hibernation image is being written or restored that SCSI devices
are quiesced. The SCSI core quiesces devices through scsi_device_quiesce()
and scsi_device_resume(). In the SDEV_QUIESCE state execution of
non-preempt requests is deferred. This is realized by returning
BLKPREP_DEFER from inside scsi_prep_state_check() for quiesced SCSI
devices. Avoid that a full queue prevents power management requests
to be submitted by deferring allocation of non-preempt requests for
devices in the quiesced state. This patch has been tested by running
the following commands and by verifying that after each resume the
fio job was still running:

System crashed due to a hard lockup at lpfc_els_timeout_handler+0x128.
The els ring's txcmplq list is corrupted: the last element in the list
does not point back the the head causing a loop. Issue is the els
processing path for sli4 hbas are using the hbalock instead of the
ring_lock for removing elements from the txcmplq list.
Use the adapter SLI_REV to determine which lock should be used for
removing iocbqs from the els rings txcmplq.
note: the future refactoring will address this so that we don't have
this ugly type-based lock code.

The indenting wasn't right, because the last two prints weren't indented
far enough. Also it used pr_info() where it was supposed to use
pr_cont(). I reversed the if statement and pulled the code in one tab
and did a couple other minor cleanups.

aacraid passes the current time to the firmware in one of two ways,
either as year/month/day/... or as 32-bit unsigned seconds.
The first one is broken on 32-bit architectures as it cannot go past
year 2038. Using timespec64 here makes it behave properly on both 32-bit
and 64-bit architectures, and avoids relying on signed integer overflow
to pass times into the second interface.
The interface used in aac_send_hosttime() however is still problematic
in year 2106 when 32-bit seconds overflow. Hopefully we don't have to
worry about aacraid by that time.

This commit added a call to sysfs_notify() from within
scsi_device_set_state(), which in turn turns out to make libata very
unhappy, because ata_eh_detach_dev() does

So now we called sysfs_notify() from within a spinlocked region, which
really doesn't work. The 0day robot reported this as:
BUG: sleeping function called from invalid context at kernel/locking/mutex.c:238
because sysfs_notify() ends up calling kernfs_find_and_get_ns() which
then does mutex_lock(&kernfs_mutex)..
The pollability of the device state isn't critical, so revert this all
for now, and maybe we'll do it differently in the future.

After taking a closer look, I found that the problem is that the new
code mixes up pointers and dma_addr_t values unnecessarily.
This changes it to use the correct types consistently, which lets us get
rid of a lot of type casts in the process. I'm also renaming some
variables to avoid confusion between physical and dma address spaces
that are often distinct.

When there are multiple disks attached to the same SCSI controller, the
host may send several VSTOR_OPERATION_REMOVE_DEVICE or
VSTOR_OPERATION_ENUMERATE_BUS messages in a row, to indicate there is a
change on the SCSI controller. In response, storvsc rescans the SCSI
host.
There is no need to do multiple scans on the same host. Fix the code to
do only one scan.

It is enough to just check if we can get the budget via .get_budget().
And we don't need to deal with device state change in .get_budget().
For SCSI, one issue to be fixed is that we have to call
scsi_mq_uninit_cmd() to free allocated ressources if SCSI device fails
to handle the request. And it isn't enough to simply call
blk_mq_end_request() to do that if this request is marked as
RQF_DONTPREP.

It is very expensive to atomic_inc/atomic_dec the host wide counter of
host->busy_count, and it should have been avoided via blk-mq's mechanism
of getting driver tag, which uses the more efficient way of sbitmap queue.
Also we don't check atomic_read(&sdev->device_busy) in scsi_mq_get_budget()
and don't run queue if the counter becomes zero, so IO hang may be caused
if all requests are completed just before the current SCSI device
is added to shost->starved_list.

PCIe switches similar to existing SAS events.
* NVMe device detection, addition and removal are reported by the
firmware through PCIe Topology Change list events.
* The PCIe device state change events are sent when the firmware
detects any abnormal conditions with a NVMe device or switch.
* The enumeration event are sent when the firmware starts PCIe device
enumeration and stops.

* Added support for translating the SGLs associated with incoming
commands either to IEE SGL or NVMe PRPs for NVMe devices.
* The hardware translation of IEEE SGL to NVMe PRPs has limitations
and if a command cannot be translated by hardware then it will go to
firmware and the firmware needs to translate it. This will have a
performance impact. To avoid that, the driver proactively checks
whether the translation will be done in hardware or not. If not, then
driver translates.

When running multipath on a VM if all available paths go down the driver
can schedule large amounts of storvsc_remove_lun work items to the same
lun. In response to the failing paths typically storvsc responds by
taking host->scan_mutex and issuing a TUR per lun. If there has been
heavy IO to the failed device all the failed IOs are returned from the
host. A remove lun work item is issued per failed IO. If the outstanding
TURs have not been completed in a timely manner the scan_mutex is never
released or released too late. Consequently the many remove lun work
items are not completed as scsi_remove_device also tries to take
host->scan_mutex. This results in dragging the VM down and sometimes
completely.
This patch only allows one remove lun to be issued to a particular lun
while it is an instantiated member of the scsi stack.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

This patch is based on work done by Thomas Gleixner and Kate Stewart
and Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset
of the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to
license had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied
to a file was done in a spreadsheet of side by side results from of
the output of two independent scanners (ScanCode & Windriver)
producing SPDX tag:value files created by Philippe Ombredanne.
Philippe prepared the base worksheet, and did an initial spot review
of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537
files assessed. Kate Stewart did a file by file comparison of the
scanner results in the spreadsheet to determine which SPDX license
identifier(s) to be applied to the file. She confirmed any
determination that was not immediately clear with lawyers working with
the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained
>5 lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.

- when the two scanners agreed on the detected license(s), that
became the concluded license(s).
- when there was disagreement between the two scanners (one detected
a license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply
(and which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases,
confirmation by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.
The Windriver scanner is based on an older version of FOSSology in
part, so they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot
checks in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect
the correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial
patch version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch
license was not GPL-2.0 WITH Linux-syscall-note to ensure that the
applied SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

After the cdrom cleanup, I get randconfig warnings for some configurations:

This adds an explicit BLK_DEV dependency for both drivers. The other
drivers that select 'CDROM' already have this and don't need a change.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

Instead of referring from inside drivers/cdrom/Makefile to all the
drivers that use this driver, let these drivers select the cdrom
driver. This change makes the cdrom build code follow the approach
that is used for most other drivers, namely refer from the higher
layers to the lower layer instead of from the lower layer to the
higher layers.

We need to tell blk-mq to reserve resources before queuing one request,
so implement these two callbacks. Then blk-mq can avoid to dequeue
request too early, and IO merging can be improved a lot.

The scsi_debug driver incorrectly suggests there is an error with the
SCSI WRITE SAME command when the number_of_logical_blocks is greater
than 1. It will also suggest there is an error when NDOB
(no data-out buffer) is set and the number_of_logical_blocks is
greater than 0. Both are valid, fix.

The 5380 drivers only support asynchronous transfers and the 5380
controllers only have narrow busses. Hence, the core driver will reject
any SDTR and WDTR messages from target devices. Don't log this, it's
expected behaviour. Also, fix the off-by-one array indices in the
arguments to scmd_printk().

The cxlflash driver tracks process IDs alongside contexts to validate
context ownership. Currently, the process IDs are derived by directly
accessing values from the 'current' task pointer. While this method of
access is fine for the current process, it is incorrect when the parent
process ID is needed as the access requires serialization.
To address the incorrect issue and provide a consistent means of
deriving the process ID within the cxlflash driver, use the task
accessors defined linux/sched.h.

Currently, all adapters that cxlflash supports must have WWPN VPD
keywords to complete configuration. This was required as cards with
external FC ports needed to be programmed with WWPNs.
Newer supported cards do not have an external FC interface and therefore
do not require WWPN. To support backwards compatibility, these devices
have included 'dummy' WWPN VPD with WWPN values of zero. This however
places a dependency that all future cards have WWPN VPD, which may not
always be the case.
Allow for cards to not have WWPN, designating which cards are expected
to have it in order to configure properly.

The existing write same routine within the cxlflash driver uses a
statically defined value for the maximum write same transfer length.
While this is close to the value reflected by the original device that
was supported by cxlflash, newer devices are capable of much larger
lengths. Supporting what the device is capable of offers substantial
performance improvement as the scrub routine within cxlflash operates on
'chunk size' units (256MB with a 4K sector size).
Instead of a #define, use the write same maximum length that is stored
in the block layer in units of 512 byte sectors. This value is initially
determined from the block limits VPD page during device discovery and
can also be manipulated from sysfs. As a general cleanup, designate the
timeout used when executing the write same command as constant.

The CQ event is handled in tasklet context, and it could be delayed if
the system loading is high.
It is possible to run into some problems when executing a host reset
when cq_tasklet_vx_hw() is being executed.

We should check register PHY_STATE when getting the bitmap of a
wideport, as, if the PHY is not ready, the value of register
PHY_PORT_NUM_MA is not valid.

On error, kthread_create() returns an errno-encoded pointer, not NULL.
The routine qla2x00_probe_one() detects the error case and jumps to
probe_failed, but has already assigned the return value from
kthread_create() to ha->dpc_thread. Then probe_failed checks to see if
ha->dpc_thread is not NULL before doing cleanup on it. Since in the
error case this is also not NULL, it ends up trying to access an invalid
task pointer.
Solution is to assign NULL to ha->dpc_thread in the error path to avoid
kthread cleanup in that case.

The function hisi_sas_slot_task_free() is used to free the slot and do
tidy-up of LLDD resources. The LLDD generally should know the state of
a slot and decide when to free it, and it should only be done once.
For some scenarios, we really don't know the state, like when TMF
timeout. In this case, we check task->lldd_task before calling
hisi_sas_slot_task_free().
However, we may miss some scenarios when we should also check
task->lldd_task, and it is not SMP safe to check task->lldd_task as we
don't protect it within spin lock.
This patch is to fix this risk of freeing slot twice, as follows:
1. Check task->lldd_task in the hisi_sas_slot_task_free(), and give
up freeing of this time if task->lldd_task is NULL.
2. Set slot->buf to NULL after it is freed.

When a PHY_FUNC_LINK_RESET is issued, we need to fill the transport
identify_frame to SAS controller before the PHYs are enabled.
Without this, we may find that if a PHY which belonged to a wideport
before the reset may generate a new port id.

Currently the size of memory we allocate for SATA breakpoint buffer is
incorrect.
The breakpoint memory size should be as follows: 32 (NCQ tags) * 128 *
2048 (max #devs) = 8MB
Currently we only allocate 0.5MB, but get away with it as we never
have SATA device index > 128 typically.
To conserve precious DMA memory (8MB may not be even available), limit
the number of devices per HBA to 1024, which means 4MB of memory
required for SATA breakpoint.
The 1024 device limit applied to all HW versions. For v3 hw, we need
to configure this value.

For several reasons, it is desirable to use {READ,WRITE}_ONCE() in
preference to ACCESS_ONCE(), and new code is expected to use one of the
former. So far, there's been no reason to change most existing uses of
ACCESS_ONCE(), as these aren't harmful, and changing them results in
churn.
However, for some features, the read/write distinction is critical to
correct operation. To distinguish these cases, separate read/write
accessors must be used. This patch migrates (most) remaining

The latest MegaRAID Firmware (for Invader series) has support for 64bit
DMA for both streaming and consistent DMA buffers. All Ventura series
controller FW always support 64 bit consistent DMA. Also, on a few
architectures 32bit DMA is not supported.
Current driver always prefers 32bit for consistent DMA and 64bit for
streaming DMA. This behavior was unintentional and carried forwarded
from legacy controller FW. Need to enhance the driver to support 64bit
consistent DMA buffers based on the firmware capability.
Below is the DMA setting strategy in driver with this patch. For
Ventura series, always try to set 64bit DMA mask. If it fails fall back
to 32bit DMA mask. For Invader series and earlier generation
controllers, first try to set to 32bit consistent DMA mask irrespective
of FW capability. This is needed to ensure firmware downgrades do not
break. If 32bit DMA setting fails, check FW capability and try seting to
64bit DMA mask.
There are certain restrictions in the hardware for having all sense
buffers and all reply descriptors to be in the same 4GB memory region.
This limitation is h/w dependent and can not be changed in firmware.
This limitation needs to be taken care in driver while allocating the

Driver load fails if memory allocation for request frame pool fails due
to the higher queue_depth requirement. The driver now allows dynamically
reducing queue_depth if memory allocations fail rather than failing
load. With this, there is no need to limit queue_depth to 1K.

In certain cases, the host memory is limited and with FW supporting
higher queue depths there are increasing chances of IO request frame
allocation failures that we are seeing. In case of request frame
allocation failures, retry allocation with reduced queue depth (in steps
of 64) to continue to configure the controller with a reduced
performance rather than failing load.

cmd->frame->dcmd.opcode will be valid only for MFI_CMD_DCMD
IOCTL frames. Currently driver check for cmd->frame->dcmd.opcode without
checking cmd type. Ensure we check dcmd opcode only for MFI_CMD_DCMD
commands. Separate handling of MFI_CMD_SMP/STP commands from
MFI_CMD_DCMD in completion path.

Older firmware version unconditionally pulls 4k frame for IOC INIT MFA
frame. But driver allocates 1k or 4k max_chain_frame_sz based on FW
capability. During boot time, this results in DMA read errors.
Workaround fix in driver by allocating separate ioc_init frame of 4k
size to support older firmware.
