Fix 2 printk format warnings (this driver is currently only used by
arch/sh/) by using "%pap" instead of "%lx".
Fixes these build warnings:

Since (a few releases before) v2.6.0 there have been checks for
CONFIG_MTD_SUPERH_RESERVE. One check is still present. But a Kconfig
symbol MTD_SUPERH_RESERVE has never been added. So a few lines of dead
code can be removed.

The first 3 arguments of 'mtd_device_parse_register()' are pointers,
but many callers pass '0' instead of 'NULL'. Fix this globally. Thanks
to coccinelle for making it easy to do with the following semantic patch:

Replace custom invocations of parse_mtd_partitions and mtd_device_register
with common mtd_device_parse_register call. This would bring: standard
handling of all errors, fallback to default partitions, etc.

The older add_mtd_device()/add_mtd_partitions() and their removal
counterparts will soon be gone. Replace uses with mtd_device_register()
and mtd_device_unregister().

Once upon a time, the MTD repository was using CVS.
This patch therefore removes all usages of the no longer updated CVS
keywords from the MTD code.
This also includes code that printed them to the user.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
