Some UFS devices (and may be host) have issues if LCC is
enabled. So we are setting PA_Local_TX_LCC_Enable to 0
before link startup which will make sure that both host
and device TX LCC are disabled once link startup is
completed.

We put the UFS device in sleep state & UFS link in hibern8 state during
runtime suspend. After this we put all the UFS rails in low power
modes immediately but it seems some devices may still draw more than
sleep current from UFS rails (especially from VCCQ rail) at-least for
500us.
To avoid this situation, this change adds 2ms delay before putting
these UFS rails in LPM mode.

Currently when we try to put the link in off/disabled state during
suspend, it seems link is not being kept in low power mode.
This patch fixes the issue by putting the link in hibern8 first
(so device also puts the link in low power mode) and then stop the
host controller.

Optimal values of local UniPro parameters like PA_Hibern8Time &
PA_TActivate can help reduce the hibern8 exit latency. If both host and
device supports UniPro ver1.6 or later, these parameters will be
automatically tuned during link startup itself. But if either host or
device doesn't support UniPro ver 1.6 or later, we have to manually
tune them. But to keep manual tuning logic simple, we will only do
manual tuning if local unipro version doesn't support ver1.6 or later.

events even when BKOPS status doesn't indicate performace impacted or
critical. Handle these device by determining their urgent bkops status

Query commands have 100ms timeout and it may timeout if they are
issued in parallel to ongoing read/write SCSI commands, this change
adds the retry (max: 10) in case command timeouts.

Some vendor's UFS device sends back to back NACs for the DL data frames
causing the host controller to raise the DFES error status. Sometimes
such UFS devices send back to back NAC without waiting for new
retransmitted DL frame from the host and in such cases it might be
possible the Host UniPro goes into bad state without raising the DFES
error interrupt. If this happens then all the pending commands would
timeout only after respective SW command (which is generally too
large).
This change workarounds such device behaviour like this:
- As soon as SW sees the DL NAC error, it would schedule the error
handler
- Error handler would sleep for 50ms to see if there any fatal errors
raised by UFS controller.
- If there are fatal errors then SW does normal error recovery.
- If there are no fatal errors then SW sends the NOP command to
device to check if link is alive.
- If NOP command times out, SW does normal error recovery
- If NOP command succeed, skip the error handling.
If DL NAC error is seen multiple times with some vendor's UFS devices
then enable this quirk to initiate quick error recovery and also
silence related error logs to reduce spamming of kernel logs.

UFS driver's error handler forcefully tries to clear all the pending
requests. For each pending request in the queue, it waits 1 sec for it
to get cleared. If we have multiple requests in the queue then it's
possible that we might end up waiting for those many seconds before
resetting the host. But note that resetting host would any way clear
all the pending requests from the hardware. Hence this change skips
the forceful clear of the pending requests if we are anyway going to
reset the host (for fatal errors).

Sometimes due to hw issues it takes some time to the
host controller register to update. In order to verify the register
has updated, a polling is done until its value is set.
In addition the functions ufshcd_hba_stop() and
ufshcd_wait_for_register() was updated with an additional input
parameter, indicating the timeout between reads will
be done by sleeping or spinning the cpu.

A race condition exists between request requeueing and scsi layer
error handling:
When UFS driver queuecommand returns a busy status for a request,
it will be requeued and its tag will be freed and set to -1.
At the same time it is possible that the request will timeout and
scsi layer will start error handling for it. The scsi layer reuses
the request and its tag to send error related commands to the device,
however its tag is no longer valid.
As this request was never really sent to the device, there is no
point to start error handling with the device.
Implement the scsi error handling timeout callback and bypass SCSI
error handling for request that were not actually sent to the device.
For such requests simply reset the block layer timer. Otherwise, let
SCSI layer perform the usual error handling.

When control reaches to Linux UFS driver during UFS boot mode, UFS host
controller interrupt status/enable registers may have left over
settings.
In order to avoid any spurious interrupts due to these left overs,
it's important to clear these interrupt status/enable registers before
enabling UFS interrupt handling.

One of the strange things that the original sg driver did was let the
user provide both a data-out buffer (it followed the sg_header+cdb)
_and_ specify a reply length greater than zero. What happened was that
the user data-out buffer was copied into some kernel buffers and then
the mid level was told a read type operation would take place with the
data from the device overwriting the same kernel buffers. The user would
then read those kernel buffers back into the user space.
From what I can tell, the above action was broken by commit fad7f01e61
("sg: set dxferp to NULL for READ with the older SG interface") in 2008
and syzkaller found that out recently.
Make sure that a user space pointer is passed through when data follows
the sg_header structure and command. Fix the abnormal case when a
non-zero reply_len is also given.

The iBFT table only specifies a prefix length, not a netmask.
And the netmask is pretty much pointless for IPv6.
So introduce a new attribute 'prefix-len'.
Some older user-space code might rely on the netmask attribute
being present, so we should always display it.

There are few MFI adapters which do not support MR_DCMD_PD_LIST_QUERY so
if MFI adapters fail this DCMD, it should not be considered as FATAL and
driver should not issue kill adapter and set per controller's instance
variable- pd_list_not_supported so that same variable can be used inside
functions- slave_alloc and slave_configure to allow firmware scan.
Killing adapter because of DCMD failure when this DCMD is not supported
causes driver's probe getting failed. This issue got introduced by
commit 6d40afbc7d ("megaraid_sas: MFI IO timeout handling").
Killing adapter in case of this DCMD failure should be limited to Fusion
adapters only. Per controller's instance variable allow_fw_scan is
removed as pd_list_not_supported better reflect the purpose.

Calling synchronize_irq() right before free_irq() is quite useless. On
one hand the IRQ can easily fire again before free_irq() is entered, on
the other hand free_irq() itself calls synchronize_irq() internally (in
a race condition free way), before any state associated with the IRQ is
freed.

To read the adapter VPD, drivers can't rely on pci config APIs, as it

With the current value of cmd_per_lun at 16, the throughput
over a single adapter is limited to around 150kIOPS.
Increase the value of cmd_per_lun to 256 to improve
throughput. With this change a single adapter is able to
attain close to the maximum throughput (380kIOPS).
Also change the number of RRQ entries that can be queued.

When switching to the internal LUN defined on the
IBM CXL flash adapter, there is an unnecessary
scan occurring on the second port. This scan leads
to the following extra lines in the log:

By definition, both of the internal LUNs are on the first port/channel.
When the lun_mode is switched to internal LUN the
same value for host->max_channel is retained. This
causes an unnecessary scan over the second port/channel.
This fix alters the host->max_channel to 0 (1 port), if internal
LUNs are configured and switches it back to 1 (2 ports) while
going back to external LUNs.

In order to support cxlflash in the PowerVM environment, underlying
hypervisor APIs have imposed a kernel API ordering change.
For the superpipe access to LUN, user applications need a context.
The cxlflash module creates this context by making a sequence of
cxl calls. In the current code, a context is initialized via
cxl_dev_context_init() followed by cxl_process_element(), a function
that obtains the process element id. Finally, cxl_start_work()
is called to attach the process element.
In the PowerVM environment, a process element id cannot be obtained
from the hypervisor until the process element is attached. The
cxlflash module is unable to create contexts without a valid
process element id.
To fix this problem, cxl_start_work() is called before obtaining
the process element id.

The cxlflash_disk_attach() routine currently uses a cascading error
gate strategy for its error cleanup path. While this strategy is
commonly used to handle cleanup scenarios, it is too restrictive when
function callouts need to be restructured. Problems range from
inserting error path bugs in previously 'good' code to the cleanup
path imposing design changes to how the normal path is structured.
A less restrictive approach is needed to support ordering changes
that come about when operating in different environments.
To overcome this restriction, the error cleanup path is modified to
have a single entrypoint and use conditional logic to cleanup where
necessary. Entities that require multiple cleanup steps must be
carefully vetted to ensure their APIs support state. In cases where
they do not (none as of this commit) additional local variables can
be used to maintain state on their behalf.

Presently, context information structures are allocated and
initialized in the same routine, create_context(). This imposes
an ordering restriction such that all pieces of information needed
to initialize a context must be known before the context is even
allocated.
This design point is not flexible when the order of context
creation needs to be modified. Specifically, this can lead to
problems when members of the context information structure are
a part of an ordering dependency (i.e. - the 'work' structure
embedded within the context).
To remedy, the allocation is left as-is, inside of the existing
create_context() routine and the initialization is transitioned
to a new void routine, init_context(). At the same time, in
anticipation of these routines not being called in sequence, a
state boolean is added to the context information structure to
track when the context has been initilized. The context teardown
routine, destroy_context(), is modified to support being called
with a non-initialized context.

When operating in the PowerVM environment, the cxlflash module can
receive an error from the hypervisor indicating that there are
existing mappings in the page table for the process MMIO space.
This issue exists because term_afu() currently invokes term_mc()
before stop_afu(), allowing for the master context to be detached
first and the problem state area to be unmapped second.
To resolve this issue, stop_afu() should be called before term_mc().

The calls to pci_request_regions(), pci_resource_start(),
pci_set_dma_mask(), pci_set_master() and pci_save_state() are all
unnecessary for the IBM CXL flash adapter since data buffers
are not required to be mapped to the device's memory.
The use of services such as pci_set_dma_mask() are problematic on
hypervisor managed systems as the IBM CXL flash adapter is operating
under a virtual PCI Host Bridge (virtual PHB) which does not support
these services.

The resolution is to simplify init_pci(), to a point where it does the
bare minimum (pci_enable_device). Similarly, remove the call the
pci_release_regions() from cxlflash_remove().

Calling synchronize_irq() right before free_irq() is quite useless. On one
hand the IRQ can easily fire again before free_irq() is entered, on the
other hand free_irq() itself calls synchronize_irq() internally (in a race
condition free way), before any state associated with the IRQ is freed.

'device_add' will be evaluating the 'is_visible' callback when creating
the sysfs attributes. As by this time the device handler has not been
attached the 'access_state' attribute will never be visible.
This patch moves the code around so that the device handler is present
by the time 'is_visible' is evaluated to correctly display the
'access_state' attribute.

The snic version number is expected to be 4 decimals in the form like a
netmask string with each number stored in an element in array v.
However, there is an off-by-one check on the number of elements in v
allowing one to pass a 5 decimal version number causing v[4] to be
referenced, causing a buffer overrun. Fix the off-by-one error by
comparing to i > 3 rather than 4.

-1.
Found using Coccinelle. A simplified version of the semantic patch

This bug causes the wrong command to have its sense pointer overwritten,
which sometimes leads to a NULL pointer deref. Fix this by checking which
command is being requeued before restoring the scsi_eh_save data.
It turns out that some targets will disconnect a REQUEST SENSE command.
The autosense algorithm doesn't anticipate this. Hence multiple commands
can end up undergoing autosense simultaneously, and they will all try to
use the same scsi_eh_save struct, which won't work. Defer autosense when
the scsi_eh_save storage is in use by another command.

The list structures and related logic used in the NCR5380 driver mean that
a command cannot be queued twice (i.e. can't appear on more than one queue
and can't appear on the same queue more than once).
The abort handler must forget the command so that the mid-layer can re-use
it. E.g. the ML may send it back to the LLD via via scsi_eh_get_sense().
Fix this and also fix two error paths, so that commands get forgotten iff
completed.

Calling NCR5380_select() from the abort handler causes various problems.
Firstly, it means potentially re-entering NCR5380_select(). Secondly, it
means that the lock is released, which permits the EH handlers to be
re-entered. The combination results in crashes. Don't do it.

The calls to NCR5380_transfer_pio() for DATA IN and DATA OUT phases will
modify cmd->SCp.this_residual, cmd->SCp.ptr and cmd->SCp.buffer. That
works as long as EH does not intervene, which became possible in
atari_NCR5380.c when I changed the locking to bring it closer to
NCR5380.c.
If error recovery aborts the command, the scsi_cmnd in question and its
buffer will be returned to the mid-layer. So the transfer has to cease,
but it can't be stopped by the initiator because the target controls the
bus phase.
The problem does not arise if the lock is not released. That was fine for
atari_scsi, because it implements DMA. For the other drivers, we have to
release the lock and re-enable interrupts for long PIO data transfers.
The solution is to split the transfer into small chunks. In between chunks
the main loop releases the lock and re-enables interrupts. Thus interrupts
can be serviced and eh_bus_reset_handler can intervene if need be.
This fixes an oops in NCR5380_transfer_pio() that can happen when the EH
abort handler is invoked during DATA IN or DATA OUT phase.

Commands subject to exception handling are to be returned to the scsi
mid-layer. Make sure that the various command pointers and command lists
in the low-level driver are correctly cleansed of affected commands.
This fixes some bugs that I accidentally introduced in v4.5-rc1 including
the removal of INIT_LIST_HEAD for the 'autosense' and 'disconnected'
command lists, and the possible NULL pointer dereference in
NCR5380_bus_reset() that was reported by Dan Carpenter.
hostdata->sensing may also point to an affected command so this pointer
also has to be cleared. The abort handler calls complete_cmd() to take
care of this; let's have the bus reset handler do the same.
The issue queue may also contain an affected command. If so, remove it.
This also follows the abort handler logic.

This adds the missing error check and path for if the call to the
function hwi_init_controller fails as this error path was clearly missed
when writing beiscsi_eeh_resume and thus we must add it now in order to
be able to handle this nonrecoverable failing function call gracefully

Do not reset fip selection time for every advertisement
in fcoe_ctlr_recv_adv() but set it only once for the first
validated FCF. Otherwise FCF selection won't happen when the
advertisements consistently arrive with sub FCOE_CTLR_START_DELAY
periodicity.

In high-datarate aging tests, it is found that the
SCSI framework can periodically issue lu resets as
some commands timeout.
Response TASK SET FULL and SAS_QUEUE_FULL may be
returned many times for the same command, causing the
timeouts.
The SAS_QUEUE_FULL errors come from
TRANS_TX_CREDIT_TIMEOUT_ERR, TRANS_TX_CLOSE_NORMAL_ERR,
and TRANS_TX_ERR_FRAME_TXED errors. They do not mean
that the queue is full in the host, but rather it is
equivalent to meaning the queue is full for the sdev.
To overcome this, the queue depth for the sdev is
reduced to 64 (from 256, set in sas_slave_configure()).
Normally error code SAS_QUEUE_FULL will result in the
sdev queue depth falling, but it falls too slowly during
high-datarate tests and commands timeout before it
has fallen to an adequete level from original value.

Add a function to abort a slot (task) in the target
device and then cleanup and complete the task.
The function is called from work queue context as
it cannot be called from the context where it is
triggered (interrupt).
Flag hisi_sas_slot.abort is added as the flag used
in the slot error handler to indicate whether the
slot needs to be aborted in the sdev prior to
cleanup and finish.

In hisi_sas_exec_internal_tmf_task(), the check for
SAM_STAT_GOOD is replaced with
TMF_RESP_FUNC_COMPLETE, which is a genuine tmf
response code.
SAM_STAT_GOOD and TMF_RESP_FUNC_COMPLETE have the
same value, so this is why it worked before.

Commit d63c7dd5bc ("ipr: Fix out-of-bounds null overwrite") removed
the end of line handling when storing the update_fw sysfs attribute.
This changed the userpace API because it started refusing writes
terminated by a line feed, which broke the update tools we already have.
This patch re-adds that handling, so both a write terminated by a line
feed or not can make it through with the update.

struct timeval will overflow on 32-bit systems in y2038 and is being
removed from the kernel. Replace the use of struct timeval and
do_gettimeofday() with ktime_get_real_seconds() which provides a 64-bit
seconds value and is y2038 safe.
gdth driver requires changes in two areas:
1) gdth_store_event() loads two u32 timestamp fields for ioctl GDTIOCTL_EVENT
These timestamp fields are part of struct gdth_evt_str used for passing
event data to userspace. At the first instance of an event we do
(first_stamp=last_stamp="current time"). If that same event repeats,
we do (last_stamp="current time") AND increment same_count to indicate
how many times the event has repeated since first_stamp.
This patch replaces the use of timeval and do_gettimeofday() with
ktime_get_real_seconds() cast to u32 to extend the timestamp fields
to y2106.
Beyond y2106, the userspace tools (ie. RAID controller monitors) can
work around the time rollover and this driver would still not need to
change.
Alternative: The alternative approach is to introduce a new ioctl in gdth
with the u32 time fields defined as u64. This would require userspace
changes now, but not in y2106.
2) gdth_show_info() calculates elapsed time using u32 first_stamp
It is adding events with timestamps to a seq_file. Timestamps are
calculated as the "current time" minus the first_stamp.
This patch replaces the use of timeval and do_gettimeofday() with
ktime_get_real_seconds() cast to u32 to calculate the timestamp.
This elapsed time calculation is safe even when the time wraps (beyond
y2106) due to how unsigned subtraction works. A comment has been added
to the code to indicate this safety.
Alternative: This piece itself doesn't warrant an alternative, but
if we do introduce a new structure & ioctl with u64 timestamps, this
would change accordingly.

1. Add hotplug support. Pegasus support surprise removal. To this end, I
use return_abnormal_state function to return DID_NO_CONNECT for all
commands which sent to driver.
2. Remove stex_hba_stop in stex_remove because we cannot send command to
device after hotplug.
3. Add new device status: MU_STATE_STOP, MU_STATE_NOCONNECT,
MU_STATE_STOP. MU_STATE_STOP is currently not referenced.
MU_STATE_NOCONNECT represent that device is plugged out from the
host.
4. Use return_abnormal_function() to substitute part of code in
stex_do_reset.

An oops can occur when submitting ioaccel2 commands when the phys_disk
pointer is NULL in hpsa_scsi_ioaccel_raid_map. Happens when there are
configuration changes during I/O operations.
If the phys_disk pointer is NULL, send the command down the RAID path.

If a device needs to be rescanned the device_handler might need
to be rechecked, too.
So add a 'rescan' callback to the device handler and call it
upon scsi_rescan_device(). The rescan callback will be invoked
from the Unit Attention handling of ASC/ASCQ 3F 03
(INQUIRY DATA HAS CHANGED).

Sending a 'REPORT TARGET PORT GROUP' command is a costly operation,
as the array has to gather information about all ports.
So instead of using RTPG to poll for a status update when a port
is in transitioning we should be sending a TEST UNIT READY, and
wait for the sense code to report success.

When we receive a unit attention code of 'ALUA state changed'
we should recheck the state, as it might be due to an implicit
ALUA state transition. This allows us to return NEEDS_RETRY
instead of ADD_TO_MLQUEUE, allowing to terminate the retries
after a certain time.
At the same time a workqueue item might already be queued, which
should be started immediately to avoid any delays.

The current ALUA device_handler has two drawbacks:
- We're sending a 'SET TARGET PORT GROUP' command to every LUN,
disregarding the fact that several LUNs might be in a port group
and will be automatically switched whenever _any_ LUN within
that port group receives the command.
- Whenever a LUN is in 'transitioning' mode we cannot block I/O
to that LUN, instead the controller has to abort the command.
This leads to increased traffic across the wire and heavy load
on the controller during switchover.
With this patch the RTPG handling is moved to a per-portgroup
workqueue. This reduces the number of 'REPORT TARGET PORT GROUP'
and 'SET TARGET PORT GROUPS' sent to the controller as we're sending
them now per port group, and not per device as previously.
It also allows us to block I/O to any LUN / port group found to be
in 'transitioning' ALUA mode, as the workqueue item will be requeued
until the controller moves out of transitioning.

When the optimize_stpg module option is set we should just set it
once during port_group allocation. Doing so allows us to override
it later with device specific settings.

All commands are issued synchronously, so no need to open-code
scsi_execute_req_flags() anymore. And we can get rid of the
static sense code structure element. scsi_execute_req_flags()
will be setting REQ_QUIET and REQ_PREEMPT, but that is
perfectly fine as we're evaluating and logging any errors
ourselves and we really need to send the command even if
the device is quiesced.

When logging async events the scsi_id, wwpn, and node_name values are
used directly from the CRQ struct which are of type __be64. This can be
confusing to someone looking through the log on a LE system. Instead
byteswap these values to host endian prior to logging.

The values returned by the show functions for the host os_type,
mad_version, and partition_number attributes get their values directly
from the madapter_info struct whose associated fields are __be32
typed. Added endian conversion to ensure these values are sane on LE
platforms.

A VIOSRP_HOST_CONFIG_TYPE management datagram (MAD) has existed in the
code for some time. From what information I've gathered from Brian King
this was likely implemented on the host side in a SLES 9 based VIOS,
which is no longer supported anywhere. Further, it is not defined in
PAPR or supported by any AIX based VIOS.
Treating as bit rot and removing the associated host config code. The
config attribute and its show function are left as not to break
userspace. The behavior remains the same returning nothing.

The PAPR defines four valid header values for the first byte of a CRQ
message. Namely, an unused/empty message (0x00), a valid
command/response entry (0x80), a valid initialization entry (0xC0), and
a valid transport event (0xFF). Further, initialization responses have
two formats namely initialize (0x01) and initialize complete
(0x02). Define these values as enums and use them in the code in
place of their magic number equivalents.

The enum values for VIOSRP_LINUX_FORMAT and VIOSRP_INLINE_FORMAT are off
by one. They are currently defined as 0x06 and 0x07 respetively. These
values are defined in PAPR correctly as 0x05 and 0x06. This
inconsistency has gone unnoticed as neither enum is currently used. The
possible future support of PING messages between the VIOS and client
adapter relies on VIOSRP_INLINE_FORMAT crq messages. Corrected these
enum values to match PAPR definitions.

Several functions in lpfc have comments stating that the function must
be called with the hbalock (or hostlock, or ringlock) held. Add
lockdep_assert_held() annotations to these functions, so one can
actually verify the locks are held.

__bfa_trc() compiles to 115 bytes of machine code.
With this .config: http://busybox.net/~vda/kernel_config
there are 1494 calls of __bfa_trc().
__bfa_trc32() is very similar, so it is uninlined too.
However, it appears to be unused, therefore this patch
ifdefs it out.
Change in code size is about 130,000 bytes:

Sometimes queries from the device might return a failure so it is
recommended to retry sending the query, before giving up. This change
adds a wrapper to retry sending a query attribute, in cases where we
need to wait longer, before we continue, or before reporting a failure.

Add a write memory barrier to make sure descriptors prepared are
actually written to memory before ringing the doorbell. We have also
added the write memory barrier after ringing the doorbell register so
that controller sees the new request immediately.

Performing several writes to UFS host controller registers has no
guarantee of ordering, so we must make sure register writes to setup
request list base address etc. are performed before the run/stop
register is enabled. In addition, when setting up a task request, we
must make sure the updating of descriptors takes places before ringing
the doorbell, similarly to setting up a transfer request.

DME commands such as Hibern8 enter/exit and gear switch generate 2
completion interrupts, one for confirmation that command is received by
local UniPro and 2nd one is the final confirmation after communication
with remote UniPro. Currently both of these completions are registered
as interrupt events which is not quite necessary and instead we can just
wait for the interrupt of 2nd completion, this should reduce the number
of interrupts and could reduce the unnecessary CPU wakeups to handle
extra interrupts.

UFS flag query requests may fail sometimes due to timeouts etc. Add a
wrapper function to retry up to 10 times in case of such failure,
similar to retries being made for attribute queries.

Hibern8 exit can be called from 3 different contexts:
- ufshcd_hibern8_exit_work
- ufshcd_ungate_work
- runtime/system resume
If hibern8 exit fails for some reason then we try to bring the link to
active state by link startup but this recovery mechanism results into
deadlock or errors from first 2 context listed above. This change fixes
the recovery by adding proper error handling mechanism.

If hibern8 enter command fails then UFS link state may be unknown which
may result into timeout of all the commands issued after failure.
This change does 2 things (for pre-defined number of retry counts) after
hibern8 enter failure:
1. Recovers the UFS link to active state
2. If link is recovered to active state, tries to put the UFS link in
hibern8 enter again until retry count expires.

The dme_peer get/set attribute commands are prone to errors, therefore
we add three retries for the UIC command sending. Error code returned
from ufshcd_send_uic_cmd() is checked, and unless it was successful or
the retries have finished, another command will be sent.

If device raises the exception event in the response to the commands
sent during the runtime/system PM callbacks, exception event handler
might run in parallel with PM callbacks and may see unclocked register
accesses. This change fixes this issue by not scheduling the exception
event handler while PM callbacks are running.

fDeviceInit query response time for some devices is too long that
default query request timeout of 100ms may not be enough. Experiments
show that fDeviceInit response sometimes takes 500ms so to be on safer
side this change sets the timeout to 600ms. Without this change, we
might unnecessarily have to retry fDeviceInit query requests multiple
times and each query request timeout prints one error message.

When sending a query to the device returns with a timeout error, we
clear the corresponding bit in the DOORBELL register but we don't clear
the outstanding_request field as we should. This patch fixes this bug.

A race condition appear to exist between request completion when
scsi_done() is called to end the request and set the tag back to -1 (at
blk_queue_end_tag() scsi_end_request), and scsi layer error handling
which aborts the command and reuses it to request sense data. Sending
the request sense is done with tag which was set to -1 and so it is
invalid. Assert command tag passed from scsi layer is valid.

Some of the data structures (like response UPIU) and/or its elements
(unused fields) should be cleared before sending out the respective
command to UFS device.
This change clears the UPIU response data structure for query commands
and NOP command before sending out the command. We also initialize the
PRDT table length to zero which should take care of commands which
doesn't have any data associated with it. We are also clearing the
unused fields in request UPIU for NOP command.

Clear the UFS data structures before sending new request.
The SCSI command is sent to the device within the UFS UPIU request.
As part of the transfer UPIU preparation, the SCSI command is copied
to the UPIU structure according to the SCSI command size.
As different SCSI commands differ in size from each other, we need
to clear the whole SCSI command field to prevent sending uninitialized
data to the device.
The UPIU response doesn't always include the sense data and can differ
in size.
Hence, the UPIU response should also be cleared before the transfer.

When the scsi_dh core was moved into the scsi core module,
CONFIG_SCSI_DH became a 'bool' option, and now anything depending on it
can be built-in even when CONFIG_SCSI=m. This of course cannot link
successfully:

Unfortunately, I don't see any helpful way to add proper error handling
for this case, and the failure scenario for rd_reg seems rather obscure,

The fdomain SCSI host driver is one of the last remaining drivers that
manually search the PCI bus using pci_get_device rather than registering
a pci_driver instance.
This means the module device table is unused when the driver is
built-in, and we get a warning about it:

This removes a redundant code block that will either be executed if the
ENABLE_FCP_RING_POLLING flag is set in phba->cfg_poll or not. The code
is just duplicated in both cases, hence we unify it again.
This probably is a left over from some sort of refactoring.

Module parameter to enable/disable configuring affinity hint for msix
vector. SMP affinity feature can be enabled/disabled by setting module
parameter "smp_affinity_enable" to 1/0. By default this feature is

Never block the SEP device (i.e. Never invoke the
scsi_internal_device_block() API for SEP device) even for the delay not
responding events. Blocking the SEP device will create a deadlock while
adding any device to the OS.

As driver was using MPI SGL while framing the SMP Passthrough request
message due to which firmware unable to post the Reply Data in the host
memory and timeout is observed for this SMP Passthrough request message
and so unable to perform phy disable operation.

Change the MTMKPART operation of the MTIOCTOP ioctl so that it works
also with current drives (LTO-5/6, etc.). Send a separate FORMAT MEDIUM
command if the partition mode page indicates that this is required. Use
LOAD to position the tape at the beginning of tape.
The operation is extended so that if the argument is negative, its
absolute value specifies the size of partition 0, which is the
physically first partition of the tape.

Driver assumes that VFs always have peers present whenever they have
same LD IDs. But this is not the case. This patch handles the above
mentioned by explicitly checking for a peer before making HA/non-HA path
decision.

The hisi_sas driver is required to support both device tree and
ACPI. The scanning of the device properties now uses the Unified Device
Properties API, which serves both OF and ACPI.
Since syscon is not supported by ACPI, syscon is only used in the driver
when device tree is used.

FW got into UE after running IO stress test
With kernel change to split session lock in frwd_lock and back_lock for
tx and rx path correspondingly, in the IO path, common resource used in
driver such as WRB was left unprotected.
Add wrb_lock spinlock to protect allocation and freeing of WRB.

Connection resets observed from some targets when NOP-Out with wrong
ExpStatSn is sent.
FW keeps track of StatSn and fills up ExpStatSn accordingly. The header
filled up by the stack needs to be modified by driver to clear
ExpStatSn. If the field is not cleared, FW recalculates ExpStatSn and
wrong offset'ed ExpStatSn is seen in the wire trace.

WARN_ON(atomic_read(&mccq->used) >= mccq->len) seen when FW gets into
UE.
MCCQ overflow is happening because driver discards any new request and
frees up the tag. The tag allocation controls the number of MCC WRB
posted. It is being replenished but WRBs are not hence the WARN_ON.
Allocation and freeing of WRB and tags for MCC is now done in one place.
This helps to achieve proper accounting of WRB indices and MCC tags.

returned. MCC WRB allocated for the operation is not freed.

be_mcc_compl_poll waits till 'used' count of MCC WRBQ is zero. This is
to determine the completion of an MCC sent.
Change function to poll for the tag of MCC sent, instead, and wait till
its tag_state is cleared.

Rename mcc_numtag to mcc_tag_status. MCC CQE status is processed using
macros already defined in be_cmds.h.

mcc_cq_lock spin_lock is used only in beiscsi_process_mcc which is
called only when all interrupts are disabled from mgmt_epfw_cleanup
during unloading of driver. There is no other context where there can be
contention for the processing of CQ.

BFA_MFG_NAME is "QLogic" which is only 7 bytes, but we are copying 8
bytes. It's harmless because the badding byte is likely zero but it
makes static checkers complain.

During EEH PCI hotplug activity kernel unloads and loads the driver,
causing character device to be unregistered(aac_remove_one).When the
driver is loaded back using aac_probe_one the character device needs
to be registered again for the AIF management tools to work.
Fixed by adding code to register character device in aac_probe_one if
it is unregistered in aac_remove_one.

while driver removal is in progress or PCI shutdown is invoked, driver
kills AIF aacraid thread, but IOCTL requests from the management tools
re-start AIF thread leading to IOP_RESET.
Fixed by setting adapter_shutdown flag when PCI shutdown is invoked.

aac_mutex was used to create protect the ioctl path for only the compat
path, it would be make more sense to place mutex in aac_do_ioctl, which
is the main ioctl function call that handles all ioctl commands.
Created new mutex ioctl_mutex in struct aac_dev to protect switch case
in aac_do_ioctl and removed aac_mutex from aac_cfg_ioctl and
aac_compat_do_ioctl

During EEH recovery number of online CPU's might change thereby changing
the number of MSIx vectors. Since each fib is allocated to a vector,
changes in the number of vectors causes fib to be sent thru invalid
vectors.In addition the correct number of MSIx vectors is not updated in
the INIT struct sent to the controller, when it is reinitialized.
Fixed by reassigning vectors to fibs based on the updated number of MSIx
vectors and updating the INIT structure before sending to controller.

aac_fib_map_free() calls pci_free_consistent() without checking that
dev->hw_fib_va is not NULL and dev->max_fib_size is not zero.If they are
indeed NULL/0, this will result in a hang as pci_free_consistent() will
attempt to invalidate cache for the entire 64-bit address space
(which would take a very long time).
Fixed by adding a check to make sure that dev->hw_fib_va and
dev->max_fib_size are not NULL and 0 respectively.

The driver utilizes an array of atomic variables to keep track of IO
submissions to each vector. To submit an IO multiple threads iterate
through the array to find a vector which has empty slots to send an
IO. The reading and updating of the variable is not atomic, causing race
conditions when a thread uses a full vector to submit an IO.
Fixed by mapping each FIB to a vector, the submission path then uses
said vector to submit IO thereby removing the possibly of a race
condition.The vector assignment is started from 1 since vector 0 is
reserved for the use of AIF management FIBS.If the number of MSIx
vectors is 1 (MSI or INTx mode) then all the fibs are allocated to
vector 0.

The method to allocate and free FIB's in the present code utilizes
spinlocks. Multiple IO's have to wait on the spinlock to acquire or free
fibs creating a performance bottleneck.
An alternative solution would be to use block layer tags to keep track
of the fibs allocated and freed. To this end aac_fib_alloc_tag was
created to utilize the blk layer tags to plug into the Fib pool.These
functions are used exclusively in the IO path. 8 fibs are reserved for
the use of AIF management software and utilize the previous spinlock
based implementations.

Add code to initialise the hardware.
Support is also added to deal with the "am-max-transmissions" (amt)
limitation in hip06 controller #1. This is how many connection requests
we can send on the system bus before waiting for a response. Due to
chip bus design, controller #1 is limited to 32 amt, while, by design, a
controller supports 64. The default value for the nibbles in the
relevant registers is 0x40; these need to be programmed with 0x20.

Since v2 hw only supports 2048 itct entries, as opposed to 4096 for v1
hw, set the max itct entries to the lower of the two.
It is not anticipated that any device with v1 will ever require to
connect > 2048 devices.

Update the SCSI hosts module to use ida to manage its host_no index
instead of an atomic integer. This means that the SCSI host number will
now be reclaimable.
Use the ida "simple" mechanism, since there should be no need for a
separate spin lock current usage. Ida was chosen over idr because the
hosts module already has its own instance and locking mechanisms that
aren't easily changed.

This patch will add capability in driver to tell firmware that it can
throttle IOs in case controller's queue depth is downgraded post OFU
(online firmware upgrade). This feature will ensure firmware can be
downgraded from higher queue depth to lower queue depth without needing
system reboot. Added throttling code in IO path of driver, in case OS
tries to send more IOs than post OFU firmware's queue depth.

1. For iMR controllers, firmware will report two queue depths:
- Controller-wide queue depth
- LDIO queue depth (240)
Controller-wide queue depth will be greater among the two. Using this
new feature, iMR can provide larger Queue depth(QD) for JBOD and limited
QD for Virtual Disk(VD).
2. megaraid_sas driver will throttle read/write LDIOs based on "LDIO
Queue Depth".
3. Dual queue depth can be enabled/disabled via module parameter. It is
enabled by default if the firmware supports it. Only specific firmware
builds will enable the feature.
4. Added sysfs parameter "ldio_outstanding" which permits querying the
number of outstanding LDIO requests at runtime.

This patch will create a reply queue pool for each MSI-X index and will
provide an array of base addresses instead of the single address of
legacy mode. Using this new interface the driver can support higher
queue depths through scattered DMA pools.
If array mode is not supported driver will fall back to the legacy
method of reply pool allocation. This limits controller queue depth to
1K max. To enable a queue depth of more than 1K driver requires firmware
to support array mode and scratch_pad3 will provide the new queue depth
value.
When RDPQ is used, downgrading to an older firmware release should not
be permitted. This may cause firmware fault and is not supported.

Firmware will fill out per-LD data to tell driver whether a particular
LD supports region lock bypass. If yes, then driver will send non-FP
LDIO to region lock bypass FIFO. With this change in driver, firmware
will optimize certain code to improve performance.

This patch will update device Queue depth based on interface type(SAS,
SATA..) for sysPDs. For Virtual disks(VDs), there will be no change in
queue depth (will remain 256). To fetch interface type (SAS or SATA or
FC..) of syspD, driver will send DCMD MR_DCMD_PD_GET_INFO.

This patch adds task management for SCSI commands. Added functions are
task abort and target reset.
1. Currently, megaraid_sas driver performs controller reset when any IO
times out. With task management support added, task abort and target
reset will be tried to recover timed out IO. If task management fails,
then controller reset will be performaned. If the task management
request times out, fail the request and escalate to the next
level (controller reset).
2. mr_device_priv_data will be allocated for all generations of
controller, but is_tm_capable flag will never be set for
controllers (prior to Invader series) as firmware support is not
available for task management.
3. Task management capable firmware will set is_tm_capable flag in
firmware API.

This patch will do proper error handling for DCMD timeout failure cases
for Fusion adapters:
1. For MFI adapters, in case of DCMD timeout (DCMD which must return
SUCCESS) driver will call kill adapter.
2. What action needs to be taken in case of DCMD timeout is decided by
function dcmd_timeout_ocr_possible(). DCMD timeout causing OCR is
applicable to the following commands:
MR_DCMD_PD_LIST_QUERY
MR_DCMD_LD_GET_LIST
MR_DCMD_LD_LIST_QUERY
MR_DCMD_CTRL_SET_CRASH_DUMP_PARAMS

3. If DCMD fails from driver init path there are certain DCMDs which
must return SUCCESS. If those DCMDs fail, driver bails out. For optional
DCMDs like pd_info etc., driver continues without executing certain

This patch will do synhronization between OCR function and AEN function
using "reset_mutex" lock. reset_mutex will be acquired only in the
first half of the AEN function which issues a DCMD. Second half of the
function which calls SCSI API (scsi_add_device/scsi_remove_device)
should be out of reset_mutex to avoid deadlock between scsi_eh thread
and driver.
During chip reset (inside OCR function), there should not be any PCI
access and AEN function (which is called in delayed context) may be
firing DCMDs (doing PCI writes) when chip reset is happening in parallel
which will cause FW fault. This patch will solve the problem by making
AEN thread and OCR thread mutually exclusive.

The hpsa driver uses this function to cleanup inquiry data. Our new pqi
driver will also use this function. This function was copied into both
drivers.
This patch exports sanitize_inquiry_string so the hpsa and the pqi
drivers can use this function directly.

If we fail to queue an srb for an async login we should set the
relogin flag so it will be retried as the reason for the queuing
failure was most likely transient. Failure to do this can lead to
failed paths as login is never retried if the relogin flag is not
set.

hwi_write_buffer() does not check if mapping dma memory succeed. The
patch adds the check and failure handling.
Found by Linux Driver Verification project (linuxtesting.org).

Login/Logout loop was hanging after few hours. /var/log/message showed
that alloc_wrb_handle() function was not able to allocate any new WRB.

Driver allocates WRB to pass login negotiated parameters information to FW
in beiscsi_offload_connection(). This allocated WRB was not freed so there

Use only port_link_status only to determine link state change. Only bit
0 is used to get the state. Remove code for processing port_fault.
Fixed get_nic_conf structure definition. Removed rsvd[23] field in
be_cmd_get_nic_conf_resp.
Moved definitions of struct field values below the field.

Async link event provides port_speed info. Cache the port_speed info
and use the same to report in ISCSI_HOST_PARAM_PORT_SPEED query.
Removed link status query IOCTL used to do the same.

OS not responding when running 2 port traffic on 72 CPUs system.
be2iscsi IRQs gets affined to CPU0 when irqbalancer is disabled.
be_iopoll processing completions in BLOCK_IOPOLL_SOFTIRQ hogged CPU0.
1. Use budget to exit the polling loop. beiscsi_process_cq didn't honour
it.
2. Rearming of EQ is done only after iopoll completes.

Log messages for misconfigured transceivers reported by FW.
Register async events that driver handles using MCC_CREATE_EXT ioctl.
Errors messages for faulted/uncertified/unqualified optics are logged.
Added IOCTL to get port_name to be displayed in error message.
