The name s5p_usb_phy_init() suggests it is shared with S5Pv210 platform,
but it is not. It is specific to S3C64xx, so make it clear in the
name.

Actually, 'enum samsung_usb_phy_type' is unused; the 'type' parameter
has 'int' type. Anyway, there is no need to declare this enum in the
globally visible header. Squash the header.

The definitions have moved to include/linux/usb/samsung-usb-phy.h,
and plat/usb-phy.h is unavailable from drivers in a multiplatform
configuration.
Also fix up the plat/usb-phy.h header file to use the definitions
from the new header instead of providing a separate copy.

This patch moves header files from plat-s5p to plat-samsung to
remove plat-s5p directory to make one plat-samsung directory
for Samsung SoCs.
