This solves a problem observed in kexec'ed kernel where 200ms timeout is
too short and bootconsole fails to initialize. Console did eventually
become workable but much later into the boot process.
Observed timeout was around 260ms, but I decided to make it a little bigger
for more reliability.
This has been tested on Power7 machine with Petitboot as a primary
bootloader and PowerNV firmware.

This will let us sort out a whole pile of tty related races. The
alternative would be to keep points and refcount the termios objects.
However
1. They are tiny anyway
2. Many devices don't use the stored copies
3. We can remove a pty special case

The driver already used refcounting. So we just switch it to tty_port
helpers. And switch to tty_port->lock for tty.

This adds a udbg and an hvc console backend for supporting a console
using the OPAL console interfaces.
On OPAL v1 we have hvc0 mapped to whatever console the system was
configured for (network or hvsi serial port) via the service
processor.
On OPAL v2 we have hvcN mapped to the Nth console provided by OPAL
which generally corresponds to:
hvc0 : network console (raw protocol)
hvc1 : serial port S1 (hvsi)
hvc2 : serial port S2 (hvsi)
Note: At this point, early debug console only works with OPAL v1
and shouldn't be enabled in a normal kernel.

A mix of think & mismerge on my side caused a problem where both the
new hvsi_lib and the old hvsi driver gets compiled and try to define
symbols with the same name.
This fixes it by renaming the hvsi_lib exported symbols.
