Just to silence sparse. Since zfcp only exists for s390 and
s390 is big endian, this has been working correctly without conversions
and all the new conversions are NOPs so no performance impact.
Nonetheless, use the conversion on the constant expression where possible.
NB: N_Port-IDs have always been handled with hton24 or ntoh24 conversions
because they also convert to / from character array.

This eases crash dump analysis by automatically dissecting these
protocol headers at least somewhat rather than getting a string
interpretation of large unstructured character array buffer fields.
Also, we can get rid of some unnecessary and error-prone type casts.
This change is possible since v2.6.33 commit 4318e08c84
("[SCSI] zfcp: Update FCP protocol related code").

v2.6.33 commit dbf5dfe9db ("[SCSI] zfcp: Use common code definitions for
FC CT structs") replaced own definitions with common code definitions.
While FC_BA_RJT_UNABLE happens to be defined with the same value 9 as
FC_FS_RJT_UNABL and thus also works, here we should use the latter from
fc_gs.h.
See also its use in libfc's fc_disc_gpn_ft_resp().

Complements debugging aspects of the otherwise functionally complete
v3.17 commit 9cb78c16f5 ("scsi: use 64-bit LUNs").
While I don't have access to a target exporting 3 or 4 level LUNs,
I did test it by explicitly attaching a non-existent fake 4 level LUN
by means of zfcp sysfs attribute "unit_add".
In order to see corresponding trace records of otherwise successful
events, we had to increase the trace level of area SCSI and HBA to 6.

At the default trace level, we only trace unsuccessful events including
FSF responses.
zfcp_dbf_hba_fsf_response() only used protocol status and FSF status to
decide on an unsuccessful response. However, this is only one of multiple
possible sources determining a failed struct zfcp_fsf_req.
An FSF request can also "fail" if its response runs into an ERP timeout
or if it gets dismissed because a higher level recovery was triggered
[trace tags "erscf_1" or "erscf_2" in zfcp_erp_strategy_check_fsfreq()].
FSF requests with ERP timeout are:
FSF_QTCB_EXCHANGE_CONFIG_DATA, FSF_QTCB_EXCHANGE_PORT_DATA,
FSF_QTCB_OPEN_PORT_WITH_DID or FSF_QTCB_CLOSE_PORT or
FSF_QTCB_CLOSE_PHYSICAL_PORT for target ports,
FSF_QTCB_OPEN_LUN, FSF_QTCB_CLOSE_LUN.
One example is slow queue processing which can cause follow-on errors,
e.g. FSF_PORT_ALREADY_OPEN after FSF_QTCB_OPEN_PORT_WITH_DID timed out.
In order to see the root cause, we need to see late responses even if the
channel presented them successfully with FSF_PROT_GOOD and FSF_GOOD.

In case of problems detecting that new responses are waiting on the input
queue, we sooner or later trigger adapter recovery due to an FSF request
timeout (trace tag "fsrth_1").
FSF requests with FSF request timeout are:
typically FSF_QTCB_ABORT_FCP_CMND; but theoretically also
FSF_QTCB_EXCHANGE_CONFIG_DATA or FSF_QTCB_EXCHANGE_PORT_DATA via sysfs,
FSF_QTCB_OPEN_PORT_WITH_DID or FSF_QTCB_CLOSE_PORT for WKA ports,
FSF_QTCB_FCP_CMND for task management function (LUN / target reset).
One or more pending requests can meanwhile have FSF_PROT_GOOD and FSF_GOOD
because the channel filled in the response via DMA into the request's QTCB.
In a theroretical case, inject code can create an erroneous FSF request
on purpose. If data router is enabled, it uses deferred error reporting.
A READ SCSI command can succeed with FSF_PROT_GOOD, FSF_GOOD, and
SAM_STAT_GOOD. But on writing the read data to host memory via DMA,
it can still fail, e.g. if an intentionally wrong scatter list does not
provide enough space. Rather than getting an unsuccessful response,
we get a QDIO activate check which in turn triggers adapter recovery.
One or more pending requests can meanwhile have FSF_PROT_GOOD and FSF_GOOD
because the channel filled in the response via DMA into the request's QTCB.

Only with luck in both above cases, we could see a follow-on trace record
of an unsuccesful event following a successful but late FSF response with
FSF_PROT_GOOD and FSF_GOOD. Typically this was the case for I/O requests
resulting in a SCSI trace record "rsl_err" with DID_TRANSPORT_DISRUPTED
[On ZFCP_STATUS_FSFREQ_DISMISSED, zfcp_fsf_protstatus_eval() sets
ZFCP_STATUS_FSFREQ_ERROR seen by the request handler functions as failure].
However, the reason for this follow-on trace was invisible because the
corresponding HBA trace record was missing at the default trace level
(by default hidden records with tags "fs_norm", "fs_qtcb", or "fs_open").
On adapter recovery, after we had shut down the QDIO queues, we perform
unsuccessful pseudo completions with flag ZFCP_STATUS_FSFREQ_DISMISSED
for each pending FSF request in zfcp_fsf_req_dismiss_all().
In order to find the root cause, we need to see all pseudo responses even
if the channel presented them successfully with FSF_PROT_GOOD and FSF_GOOD.
Therefore, check zfcp_fsf_req.status for ZFCP_STATUS_FSFREQ_DISMISSED
or ZFCP_STATUS_FSFREQ_ERROR and trace with a new tag "fs_rerr".
It does not matter that there are numerous places which set
ZFCP_STATUS_FSFREQ_ERROR after the location where we trace an FSF response
early. These cases are based on protocol status != FSF_PROT_GOOD or
== FSF_PROT_FSF_STATUS_PRESENTED and are thus already traced by default
as trace tag "fs_perr" or "fs_ferr" respectively.
NB: The trace record with tag "fssrh_1" for status read buffers on dismiss

If the FCP_RSP UI has optional parts (FCP_SNS_INFO or FCP_RSP_INFO) and
thus does not fit into the fsp_rsp field built into a SCSI trace record,
trace the full FCP_RSP UI with all optional parts as payload record
instead of just FCP_SNS_INFO as payload and
a 1 byte RSP_INFO_CODE part of FCP_RSP_INFO built into the SCSI record.
That way we would also get the full FCP_SNS_INFO in case a
target would ever send more than
min(SCSI_SENSE_BUFFERSIZE==96, ZFCP_DBF_PAY_MAX_REC==256)==96.
The mandatory part of FCP_RSP IU is only 24 bytes.
PAYload costs at least one full PAY record of 256 bytes anyway.
We cap to the hardware response size which is only FSF_FCP_RSP_SIZE==128.
So we can just put the whole FCP_RSP IU with any optional parts into
PAYload similarly as we do for SAN PAY since v4.9 commit aceeffbb59
("zfcp: trace full payload of all SAN records (req,resp,iels)").
This does not cause any additional trace records wasting memory.
Decoded trace records were confusing because they showed a hard-coded
sense data length of 96 even if the FCP_RSP_IU field FCP_SNS_LEN showed
actually less.
Since the same commit, we set pl_len for SAN traces to the full length of a
request/response even if we cap the corresponding trace.
In contrast, here for SCSI traces we set pl_len to the pre-computed
length of FCP_RSP IU considering SNS_LEN or RSP_LEN if valid.
Nonetheless we trace a hardcoded payload of length FSF_FCP_RSP_SIZE==128
if there were optional parts.
This makes it easier for the zfcpdbf tool to format only the relevant
part of the long FCP_RSP UI buffer. And any trailing information is still
available in the payload trace record just in case.
Rename the payload record tag from "fcp_sns" to "fcp_riu" to make the new
content explicit to zfcpdbf which can then pick a suitable field name such
as "FCP rsp IU all:" instead of "Sense info :"
Also, the same zfcpdbf can still be backwards compatible with "fcp_sns".

For problem determination we need to see that we were in scsi_eh
as well as whether and why we were successful or not.

Without this fix we get SCSI trace records on task management functions
which cannot be correlated to HBA trace records because all fields
related to the FSF request are empty (zero).
Also, the FCP_RSP_IU is missing as well as any sense data if available.
This was caused by v2.6.14 commit 8a36e4532e ("[SCSI] zfcp: enhancement
of zfcp debug features") introducing trace records for TMFs but
hard coding NULL for a possibly existing TMF FSF request.
The scsi_cmnd scribble is also zero or unrelated for the TMF request
so it also could not lookup a suitable FSF request from there.

There are some true cases where we really do not have an FSF request:
"rsl_fai" from zfcp_dbf_scsi_fail_send() called for early
returns / completions in zfcp_scsi_queuecommand(),
"abrt_or", "abrt_bl", "abrt_ru", "abrt_ar" from
zfcp_scsi_eh_abort_handler() where we did not get as far,
"lr_nres", "tr_nres" from zfcp_task_mgmt_function() where we're
successful and do not need to do anything because adapter stopped.
For these cases it's correct to pass NULL for fsf_req to _zfcp_dbf_scsi().

(req,resp,iels)") fixed trace data loss of 2.6.38 commit 2c55b750a8
("[SCSI] zfcp: Redesign of the debug tracing for SAN records.")
necessary for problem determination, e.g. to see the
currently active zone set during automatic port scan.
While it already saves space by not dumping any empty residual entries
of the large successful GPN_FT response (4 pages), there are seldom cases
where the GPN_FT response is unsuccessful and likely does not have
FC_NS_FID_LAST set in fp_flags so we did not cap the trace record.
We typically see such case for an initiator WWPN, which is not in any zone.
Cap unsuccessful responses to at least the actual basic CT_IU response
plus whatever fits the SAN trace record built-in "payload" buffer
just in case there's trailing information
of which we would at least see the existence and its beginning.
In order not to erroneously cap successful responses, we need to swap
calling the trace function and setting the CT / ELS status to success (0).

Up until now zfcp would just ignore the FCP_RESID_OVER flag in the FCP
response IU. When this flag is set, it is possible, in regards to the
FCP standard, that the storage-server processes the command normally, up
to the point where data is missing and simply ignores those.
In this case no CHECK CONDITION would be set, and because we ignored the
FCP_RESID_OVER flag we resulted in at least a data loss or even
-corruption as a follow-up error, depending on how the
applications/layers on top behave. To prevent this, we now set the
host-byte of the corresponding scsi_cmnd to DID_ERROR.
Other storage-behaviors, where the same condition results in a CHECK
CONDITION set in the answer, don't need to be changed as they are
handled in the mid-layer already.

Since commit db007fc5e2 ("[SCSI] Command protection operation"),
scsi_eh_prep_cmnd() saves scmd->prot_op and temporarily resets it to
SCSI_PROT_NORMAL.
Other FCP LLDDs such as qla2xxx and lpfc shield their queuecommand()
to only access any of scsi_prot_sg...() if
(scsi_get_prot_op(cmd) != SCSI_PROT_NORMAL).
Do the same thing for zfcp, which introduced DIX support with
commit ef3eb71d8b ("[SCSI] zfcp: Introduce experimental support for
DIF/DIX").
Otherwise, TUR SCSI commands as part of scsi_eh likely fail in zfcp,
because the regular SCSI command with DIX protection data, that scsi_eh
re-uses in scsi_send_eh_cmnd(), of course still has
(scsi_prot_sg_count() != 0) and so zfcp sends down bogus requests to the
FCP channel hardware.
This causes scsi_eh_test_devices() to have (finish_cmds == 0)
[not SCSI device is online or not scsi_eh_tur() failed]
so regular SCSI commands, that caused / were affected by scsi_eh,
are moved to work_q and scsi_eh_test_devices() itself returns false.
In turn, it unnecessarily escalates in our case in scsi_eh_ready_devs()
beyond host reset to finally scsi_eh_offline_sdevs()
which sets affected SCSI devices offline with the following kernel message:

Instead define the timeout behavior purely based on the host_template
eh_timed_out method and wire up the existing transport implementations
in the host templates. This also clears up the confusion that the
transport template method overrides the host template one, so some
drivers have to re-override the transport template one.

It is unavoidable that zfcp_scsi_queuecommand() has to finish requests
with DID_IMM_RETRY (like fc_remote_port_chkready()) during the time
window when zfcp detected an unavailable rport but
fc_remote_port_delete(), which is asynchronous via
zfcp_scsi_schedule_rport_block(), has not yet blocked the rport.
However, for the case when the rport becomes available again, we should
prevent unblocking the rport too early. In contrast to other FCP LLDDs,
zfcp has to open each LUN with the FCP channel hardware before it can
send I/O to a LUN. So if a port already has LUNs attached and we
unblock the rport just after port recovery, recoveries of LUNs behind
this port can still be pending which in turn force
zfcp_scsi_queuecommand() to unnecessarily finish requests with
DID_IMM_RETRY.
This also opens a time window with unblocked rport (until the followup
LUN reopen recovery has finished). If a scsi_cmnd timeout occurs during
this time window fc_timed_out() cannot work as desired and such command
would indeed time out and trigger scsi_eh. This prevents a clean and
timely path failover. This should not happen if the path issue can be
recovered on FC transport layer such as path issues involving RSCNs.
Fix this by only calling zfcp_scsi_schedule_rport_register(), to
asynchronously trigger fc_remote_port_add(), after all LUN recoveries as
children of the rport have finished and no new recoveries of equal or
higher order were triggered meanwhile. Finished intentionally includes
any recovery result no matter if successful or failed (still unblock
rport so other successful LUNs work). For simplicity, we check after
each finished LUN recovery if there is another LUN recovery pending on
the same port and then do nothing. We handle the special case of a
successful recovery of a port without LUN children the same way without
changing this case's semantics.
For debugging we introduce 2 new trace records written if the rport
unblock attempt was aborted due to still unfinished or freshly triggered
recovery. The records are only written above the default trace level.
Benjamin noticed the important special case of new recovery that can be
triggered between having given up the erp_lock and before calling
zfcp_erp_action_cleanup() within zfcp_erp_strategy(). We must avoid the

Now the rport was erroneously unblocked while the zfcp_port is blocked.
This is another situation we want to avoid due to scsi_eh
potential. This state would at least remain until the new recovery from
the other context finished successfully, or potentially forever if it
failed. In order to close this race, we take the erp_lock inside
zfcp_erp_try_rport_unblock() when checking the status of zfcp_port or
LUN. With that, the possible corresponding rport state sequences would
be: (unblock[ERP thread],block[other context]) if the ERP thread gets
erp_lock first and still sees ((port->status & ...UNBLOCK) != 0),
(block[other context],NOP[ERP thread]) if the ERP thread gets erp_lock
after the other context has already cleard ...UNBLOCK from port->status.
Since checking fields of struct erp_action is unsafe because they could
have been overwritten (re-used for new recovery) meanwhile, we only
check status of zfcp_port and LUN since these are only changed under
erp_lock elsewhere. Regarding the check of the proper status flags (port
or port_forced are similar to the shown adapter recovery):

Since quite a while, Linux issues enough SCSI commands per scsi_device
which successfully return with FCP_RESID_UNDER, FSF_FCP_RSP_AVAILABLE,
and SAM_STAT_GOOD. This floods the HBA trace area and we cannot see
other and important HBA trace records long enough.
Therefore, do not trace HBA response errors for pure benign residual
under counts at the default trace level.
This excludes benign residual under count combined with other validity
bits set in FCP_RSP_IU, such as FCP_SNS_LEN_VAL. For all those other
cases, we still do want to see both the HBA record and the corresponding
SCSI record by default.

When SCSI EH invokes zFCP's callbacks for eh_device_reset_handler() and
eh_target_reset_handler(), it expects us to relent the ownership over
the given scsi_cmnd and all other scsi_cmnds within the same scope - LUN
or target - when returning with SUCCESS from the callback ('release'
them). SCSI EH can then reuse those commands.
We did not follow this rule to release commands upon SUCCESS; and if
later a reply arrived for one of those supposed to be released commands,
we would still make use of the scsi_cmnd in our ingress tasklet. This
will at least result in undefined behavior or a kernel panic because of
a wrong kernel pointer dereference.
To fix this, we NULLify all pointers to scsi_cmnds (struct zfcp_fsf_req
*)->data in the matching scope if a TMF was successful. This is done
under the locks (struct zfcp_adapter *)->abort_lock and (struct
zfcp_reqlist *)->lock to prevent the requests from being removed from
the request-hashtable, and the ingress tasklet from making use of the
scsi_cmnd-pointer in zfcp_fsf_fcp_cmnd_handler().
For cases where a reply arrives during SCSI EH, but before we get a
chance to NULLify the pointer - but before we return from the callback
-, we assume that the code is protected from races via the CAS operation
in blk_complete_request() that is called in scsi_done().

Export fc_bsg_jobdone so drivers can use it directly instead of doing
the round-trip via struct fc_bsg_job::job_done() and use it in the
LLDDs. That way we can also unify the interfaces of fc_bsg_jobdone and
bsg_job_done.
As we've converted all LLDDs over to use fc_bsg_jobdone() directly, we
can remove the function pointer from struct fc_bsg_job as well.

The workqueue "appldata_wq" has been replaced with an ordered dedicated
workqueue.
WQ_MEM_RECLAIM has not been set since the workqueue is not being used on
a memory reclaim path.
The adapter->work_queue queues multiple work items viz
&adapter->scan_work, &port->rport_work, &adapter->ns_up_work,
&adapter->stat_work, adapter->work_queue, &adapter->events.work,
&port->gid_pn_work, &port->test_link_work. Hence, an ordered
dedicated workqueue has been used.
WQ_MEM_RECLAIM has been set to ensure forward progress under memory
pressure.

we lost the N_Port-ID where an ELS response comes from.
With commit 7c7dc19681
("[SCSI] zfcp: Simplify handling of ct and els requests")
we lost the N_Port-ID where a CT response comes from.
It's especially useful if the request SAN trace record
with D_ID was already lost due to trace buffer wrap.
GS uses an open WKA port handle and ELS just a D_ID, and
only for ELS we could get D_ID from QTCB bottom via zfcp_fsf_req.
To cover both cases, add a new field to zfcp_fsf_ct_els
and fill it in on request to use in SAN response trace.
Strictly speaking the D_ID on SAN response is the FC frame's S_ID.
We don't need a field for the other end which is always us.

Since commit a54ca0f62f
("[SCSI] zfcp: Redesign of the debug tracing for HBA records.")
HBA records no longer contain WWPN, D_ID, or LUN
to reduce duplicate information which is already in REC records.
In contrast to "regular" target ports, we don't use recovery to open
WKA ports such as directory/nameserver, so we don't get REC records.
Therefore, introduce pseudo REC running records without any
actual recovery action but including D_ID of WKA port on open/close.

On a successful end of reopen port forced,
zfcp_erp_strategy_followup_success() re-uses the port erp_action
and the subsequent zfcp_erp_action_cleanup() now
sees ZFCP_ERP_SUCCEEDED with
erp_action->action==ZFCP_ERP_ACTION_REOPEN_PORT
instead of ZFCP_ERP_ACTION_REOPEN_PORT_FORCED
but must not perform zfcp_scsi_schedule_rport_register().
We can detect this because the fresh port reopen erp_action
is in its very first step ZFCP_ERP_STEP_UNINITIALIZED.
Otherwise this opens a time window with unblocked rport
(until the followup port reopen recovery would block it again).
If a scsi_cmnd timeout occurs during this time window
fc_timed_out() cannot work as desired and such command
would indeed time out and trigger scsi_eh. This prevents
a clean and timely path failover.
This should not happen if the path issue can be recovered
on FC transport layer such as path issues involving RSCNs.
Also, unnecessary and repeated DID_IMM_RETRY for pending and
undesired new requests occur because internally zfcp still
has its zfcp_port blocked.
As follow-on errors with scsi_eh, it can cause,
in the worst case, permanently lost paths due to one of:
sd <scsidev>: [<scsidisk>] Medium access timeout failure. Offlining disk!
sd <scsidev>: Device offlined - not ready after error recovery
For fix validation and to aid future debugging with other recoveries
we now also trace (un)blocking of rports.

In the hardware data router case, introduced with kernel 3.2
commit 86a9668a8d ("[SCSI] zfcp: support for hardware data router")
the ELS/GS request&response length needs to be initialized
as in the chained SBAL case.
Otherwise, the FCP channel rejects ELS requests with
FSF_REQUEST_SIZE_TOO_LARGE.
Such ELS requests can be issued by user space through BSG / HBA API,
or zfcp itself uses ADISC ELS for remote port link test on RSCN.
The latter can cause a short path outage due to
unnecessary remote target port recovery because the always
failing ADISC cannot detect extremely short path interruptions
beyond the local FCP channel.

For an NPIV-enabled FCP device, zfcp can erroneously show
"NPort (fabric via point-to-point)" instead of "NPIV VPORT"
for the port_type sysfs attribute of the corresponding
fc_host.
s390-tools that can be affected are dbginfo.sh and ziomon.
zfcp_fsf_exchange_config_evaluate() ignores
fsf_qtcb_bottom_config.connection_features indicating NPIV
and only sets fc_host_port_type to FC_PORTTYPE_NPORT if
fsf_qtcb_bottom_config.fc_topology is FSF_TOPO_FABRIC.
Only the independent zfcp_fsf_exchange_port_evaluate()
evaluates connection_features to overwrite fc_host_port_type
to FC_PORTTYPE_NPIV in case of NPIV.
Code was introduced with upstream kernel 2.6.30
commit 0282985da5
("[SCSI] zfcp: Report fc_host_port_type as NPIV").
This works during FCP device recovery (such as set online)
because it performs FSF_QTCB_EXCHANGE_CONFIG_DATA followed by
FSF_QTCB_EXCHANGE_PORT_DATA in sequence.
However, the zfcp-specific scsi host sysfs attributes
"requests", "megabytes", or "seconds_active" trigger only
zfcp_fsf_exchange_config_evaluate() resetting fc_host
port_type to FC_PORTTYPE_NPORT despite NPIV.
The zfcp-specific scsi host sysfs attribute "utilization"
triggers only zfcp_fsf_exchange_port_evaluate() correcting
the fc_host port_type again in case of NPIV.
Evaluate fsf_qtcb_bottom_config.connection_features
in zfcp_fsf_exchange_config_evaluate() where it belongs to.

On larger installations it is useful to disable automatic LUN scanning,
and only add the required LUNs via udev rules. This can speed up bootup
dramatically.
This patch introduces a new scan module parameter value 'manual', which
works like 'none', but can be overridden by setting the 'rescan' value
from scsi_scan_target to 'SCSI_SCAN_MANUAL'. And it updates all
relevant callers to set the 'rescan' value to 'SCSI_SCAN_MANUAL' if
invoked via the 'scan' option in sysfs.

Allocating a large number of elements in atomic context could quickly
deplete memory reserves, so just disallow atomic resizing entirely.
Nothing currently uses mempool_resize() with anything other than
GFP_KERNEL, so convert existing callers to drop the gfp_mask.

All drivers use the implementation for ramping the queue up and down, so
instead of overloading the change_queue_depth method call the
implementation diretly if the driver opts into it by setting the
track_queue_depth flag in the host template.
Note that a few drivers validated the new queue depth in their
change_queue_depth method, but as we never go over the queue depth
set during slave_configure or the sysfs file this isn't nessecary
and can safely be removed.

This patch improves the Fibre Channel port scan behaviour of the zfcp lldd.
Without it the zfcp device driver may churn up the storage area network by
excessive scanning and scan bursts, particularly in big virtual server
environments, potentially resulting in interference of virtual servers and
reduced availability of storage connectivity.
The two main issues as to the zfcp device drivers automatic port scan in
virtual server environments are frequency and simultaneity.
On the one hand, there is no point in allowing lots of ports scans
in a row. It makes sense, though, to make sure that a scan is conducted
eventually if there has been any indication for potential SAN changes.
On the other hand, lots of virtual servers receiving the same indication
for a SAN change had better not attempt to conduct a scan instantly,
that is, at the same time.
Hence this patch has a two-fold approach for better port scanning:
the introduction of a rate limit to amend frequency issues, and the
introduction of a short random backoff to amend simultaneity issues.
Both approaches boil down to deferred port scans, with delays
comprising parts for both approaches.
The new port scan behaviour is summarised best by:

Implementation is straight-forward by converting an existing worker to
a delayed worker. But care is needed whenever that worker is going to be
flushed (in order to make sure work has been completed), since a flush
operation cancels the timer set up for deferred execution (see * above).
There is a small race window whenever a port scan work starts
running up to the point in time of storing the time stamp for that port
scan. The impact is negligible. Closing that gap isn't trivial, though, and
would the destroy the beauty of a simple work-to-delayed-work conversion.

Through sysfs attributes, zfcp unit objects
provide a trigger for manual LUN recovery
and export information for problem determination.
With commit

and when attaching SCSI devices through this new optional method,
no more zfcp unit objects are allocated for such SCSI devices.
Hence, the above-mentioned trigger and information were missing.

Hence, introduce the trigger and the information unconditionally
for all SCSI devices attached through zfcp.
We prefix the attribute names with 'zfcp_' to prevent collisions and
to avoid mix-ups such as with the common 'state' attribute.
Since some of the new attribute views do not need zfcp_port
in the ZFCP_DEFINE_SCSI_ATTR helper macro, remove zfcp_port
to avoid compiler warnings on unused variable.
It's easy to open code the conversion from zfcp_scsi_dev to zfcp_port
for the two already existing attributes hba_id and wwpn.

Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth. For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.
Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway. The one other case looks
broken, but I've kept the behavior as-is for now.
Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.
Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Unless we want to build a SPI tag message we should just check SCMD_TAGGED
instead of reverse engineering a tag type through the use of
scsi_populate_tag_msg.
Also rename the function to spi_populate_tag_msg, make it behave like the
other spi message helpers, and move it to the spi transport class.

The SCSI standard defines 64-bit values for LUNs, and large arrays
employing large or hierarchical LUN numbers become more and more
common.

Enabling the data router support by default
can increase performance in certain situations.
It is safe to do so and tolerated in LPAR and under z/VM

By popular demand, this patch brings back a couple of sysfs attributes
removed by commit 663e0890e3
"[SCSI] zfcp: remove access control tables interface".
The content has been irrelevant for years, but the files must be
there forever for whatever user space tools that may rely on them.
Since these files always return a constant value, a new stripped
down show-macro was required. Otherwise build warnings would have
been introduced.

Apparently, the ref_count for some scsi_device drops down to zero,
triggering device removal through execute_in_process_context(), while
the lldd error recovery thread iterates through a scsi device list.
Unfortunately, execute_in_process_context() decides to immediately
execute that device removal function, instead of scheduling asynchronous
execution, since it detects process context and thinks it is safe to do
so. But almost all calls to shost_for_each_device() in our lldd are
inside spin_lock_irq, even in thread context. Obviously, schedule()
inside spin_lock_irq sections is a bad idea.
Change the lldd to use the proper iterator function,
__shost_for_each_device(), in combination with required locking.
Occurences that need to be changed include all calls in zfcp_erp.c,
since those might be executed in zfcp error recovery thread context
with a lock held.
Other occurences of shost_for_each_device() in zfcp_fsf.c do not
need to be changed (no process context, no surrounding locking).
The problem was introduced in Linux 2.6.37 by commit
b62a8d9b45

This patch adds wait_event_interruptible_lock_irq_timeout(), which is a
straight-forward descendant of wait_event_interruptible_timeout() and
wait_event_interruptible_lock_irq().
The zfcp driver used to call wait_event_interruptible_timeout()
in combination with some intricate and error-prone locking. Using
wait_event_interruptible_lock_irq_timeout() as a replacement
nicely cleans up that locking.
This rework removes a situation that resulted in a locking imbalance
in zfcp_qdio_sbal_get():

It was introduced by commit c2af7545aa
"[SCSI] zfcp: Do not wait for SBALs on stopped queue", which had a new
code path related to ZFCP_STATUS_ADAPTER_QDIOUP that took an early exit
without a required lock being held. The problem occured when a
special, non-SCSI I/O request was being submitted in process context,
when the adapter's queues had been torn down. In this case the bug
surfaced when the Fibre Channel port connection for a well-known address
was closed during a concurrent adapter shut-down procedure, which is a
rare constellation.
This patch also fixes these warnings from the sparse tool (make C=1):

started using a value returned by the channel but only evaluated the value
if the fabric link is up.

introduced mempool resizings based on the above value.
On setting an FCP device online for the very first time since boot, a new
zeroed adapter object is allocated. If the link is down, the number of
status read requests remains zero. Since just the config data exchange is
incomplete, we proceed with adapter open recovery. However, we
unconditionally call mempool_resize with adapter->stat_read_buf_num == 0 in
this case.
This causes a kernel message "kernel BUG at mm/mempool.c:131!" in process
"zfcperp<FCP-device-bus-ID>" with last function mempool_resize in Krnl PSW
and zfcp_erp_thread in the Call Trace.
Don't evaluate channel values which are invalid on link down. The number of
status read requests is always valid, evaluated, and set to a positive
minimum greater than zero. The adapter open recovery can proceed and the
channel has status read buffers to inform us on a future link up event.
While we are not aware of any other code path that could result in mempool
resize attempts of size zero, we still also initialize the number of status
read buffers to be posted to a static minimum number on adapter object
allocation.

This patch removes an interface that was used to manage access control
tables within the HBA. The patch consequently removes the handling
for conditions related to those access control tables, too.
That initiator-based access control feature was only needed until the
introduction of NPIV and was withdrawn with z10 years ago.
It's time to cleanup the corresponding device driver code.

So far, we could only increase the s390dbf log level after an FCP
device has been initially set online for it to create the dbf entries
required to adjust the level.
Introduce zfcp.dbflevel as counterpart to the already existing
zfcp.dbfsize to enable debugging of e.g. setting an FCP device online.

"[SCSI] zfcp: support for hardware data router"
reduced the initial block queue limits in the scsi_host_template to the
absolute minimum and adjusted them later on. However, the adjustment was
too late for the BSG devices of Scsi_Host and fc_host.
Therefore, ioctl(..., SG_IO, ...) with request or response size > 4kB to a
BSG device of an fc_host or a Scsi_Host fails with EINVAL. As a result,
users of such ioctl such as HBA_SendCTPassThru() in libzfcphbaapi return
with error HBA_STATUS_ERROR.
Initialize the block queue limits in zfcp_scsi_host_template to the
greatest common denominator (GCD).
While we cannot exploit the slightly enlarged maximum request size with
data router, this should be neglectible. Doing so also avoids running into
trouble after live guest relocation (LGR) / migration from a data router
FCP device to an FCP device that does not support data router. In that
case, zfcp would figure out the new limits on adapter recovery, but the
fc_host and Scsi_Host (plus in fact all sdevs) still exist with the old and
now too large queue limits.
It should also OK, not to use half the size as in the DIX case, because
fc_host and Scsi_Host do not transport FCP requests including SCSI commands
using protection data.

FCP device remains in status ERP_FAILED when device is switched online
or adapter recovery is triggered while link to SAN is down.
When Exchange Configuration Data command returns the FSF status
FSF_EXCHANGE_CONFIG_DATA_INCOMPLETE it aborts the exchange process.
The only retries are done during the common error recovery procedure
(i.e. max. 3 retries with 8sec sleep between) and remains in status
ERP_FAILED with QDIO down.
This commit reverts the commit 0df138476c
(zfcp: Fix adapter activation on link down).
When FSF status FSF_EXCHANGE_CONFIG_DATA_INCOMPLETE is received the
adapter recovery will be finished without any retries. QDIO will be
up now and status changes such as LINK UP will be received now.

__scsi_remove_device (e.g. due to dev_loss_tmo) calls
zfcp_scsi_slave_destroy which in turn sends a close LUN FSF request to
the adapter. After 30 seconds without response,
zfcp_erp_timeout_handler kicks the ERP thread failing the close LUN
ERP action. zfcp_erp_wait in zfcp_erp_lun_shutdown_wait and thus
zfcp_scsi_slave_destroy returns and then scsi_device is no longer
valid. Sometime later the response to the close LUN FSF request may
finally come in. However, commit
b62a8d9b45
"[SCSI] zfcp: Use SCSI device data zfcp_scsi_dev instead of zfcp_unit"
introduced a number of attempts to unconditionally access struct
zfcp_scsi_dev through struct scsi_device causing a use-after-free.
This leads to an Oops due to kernel page fault in one of:
zfcp_fsf_abort_fcp_command_handler, zfcp_fsf_open_lun_handler,
zfcp_fsf_close_lun_handler, zfcp_fsf_req_trace,
zfcp_fsf_fcp_handler_common.
Move dereferencing of zfcp private data zfcp_scsi_dev allocated in
scsi_device via scsi_transport_reserve_device after the check for
potentially aborted FSF request and thus no longer valid scsi_device.
Only then assign sdev_to_zfcp(sdev) to the local auto variable struct
zfcp_scsi_dev *zfcp_sdev.

In FC fabrics with large zones, the automatic port_rescan on incoming ELS
and any adapter recovery can cause quite some traffic at the very same
time, especially if lots of Linux images share an HBA, which is common on
s390. This can cause trouble and failures. Fix this by making such port
rescans dependent on a user configurable module parameter.
The following unconditional automatic port rescans remain as is:
On setting an adapter online and
on manual user-triggered writes to the sysfs attribute port_rescan.

accidentally dropped a reference count check before tearing down
zfcp_ports that are potentially in use by zfcp_units.
Even remote ports in use can be removed causing
unreachable garbage objects zfcp_ports with zfcp_units.
Thus units won't come back even after a manual port_rescan.
The kref of zfcp_port->dev.kobj is already used by the driver core.
We cannot re-use it to track the number of zfcp_units.
Re-introduce our own counter for units per port
and check on port_remove.

If list_for_each_entry, etc complete a traversal of the list, the iterator
variable ends up pointing to an address at an offset from the list head,
and not a meaningful structure. Thus this value should not be used after
the end of the iterator. Replace port->adapter->scsi_host by
adapter->scsi_host.
This problem was found using Coccinelle (http://coccinelle.lip6.fr/).

If the mapping of FCP device bus ID and corresponding subchannel
is modified while the Linux image is suspended, the resume of FCP
devices can fail. During resume, zfcp gets callbacks from cio regarding
the modified subchannels but they can be arbitrarily mixed with the
restore/resume callback. Since the cio callbacks would trigger
adapter recovery, zfcp could wakeup before the resume callback.
Therefore, ignore the cio callbacks regarding subchannels while
being suspended. We can safely do so, since zfcp does not deal itself
with subchannels. For problem determination purposes, we still trace the
ignored callback events.
The following kernel messages could be seen on resume:

As part of adapter reopen recovery, zfcp performs auto port scanning
which can erroneously try to register new remote ports with
scsi_transport_fc and the device core code complains about the parent

In such cases, the adapter gave up recovery and remained blocked along
with its child objects: remote ports and LUNs/scsi devices. Even the
adapter shutdown as part of giving up recovery failed because the ccw
device state remained disconnected. Later, the corresponding remote
ports ran into dev_loss_tmo. As a result, the LUNs were erroneously
not available again after resume.
Even a manually triggered adapter recovery (e.g. sysfs attribute
failed, or device offline/online via sysfs) could not recover the
adapter due to the remaining disconnected state of the corresponding
ccw device.

The pl vector has scount elements, i.e. pl[scount-1] is the last valid
element. For maximum sized requests, payload->counter == scount after
the last loop iteration. Therefore, do bounds checking first (with
boolean shortcut) to not access the invalid element pl[scount].
Do not trust the maximum sbale->scount value from the HBA
but ensure we won't access the pl vector out of our allocated bounds.
While at it, clean up scoping and prevent unnecessary memset.

changed the semantics of FC_PORTSPEED defines to
FDMI port attributes of FC-HBA/SM-HBA
which is different from the previous bit reversed
Report Port Speed Capabilities (RPSC) ELS of FC-GS/FC-LS.
Zfcp showed "10 Gbit" instead of "4 Gbit" for supported_speeds.
It now uses explicit bit conversion as the other LLDs already
do, in order to be independent of the kernel bit semantics.

Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.
Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

The new is_compat_task() define for the !COMPAT case in
include/linux/compat.h conflicts with a similar define in
arch/s390/include/asm/compat.h.

zfcp_scsi_slave_destroy erroneously always tried to finish its task
even if the corresponding previous zfcp_scsi_slave_alloc returned
early. This can lead to kernel page faults on accessing uninitialized
fields of struct zfcp_scsi_dev in zfcp_erp_lun_shutdown_wait. Take the
port field of the struct to determine if slave_alloc returned early.
This zfcp bug is exposed by 4e6c82b (in turn fixing f7c9c6b to be
compatible with 21208ae) which can call slave_destroy for a
corresponding previous slave_alloc that did not finish.
This patch is based on James Bottomley's fix suggestion in

The qdio SBAL entry flag is made-up of four different values that are
independent of one another. Some of the bits are reserved by the
hardware and should not be changed by qdio. Currently all four values
are overwritten since the SBAL entry flag is defined as an u32.
Split the SBAL entry flag into four u8's as defined by the hardware
and don't touch the reserved bits.

Query the FC symbolic port name for reporting in the fc_host sysfs and
enable the symbolic_name attribute in the fc_host sysfs. When running
in NPIV mode, extend the symbolic port name with the devno and the
hostname. This allows better identification of Linux systems for SAN
and storage administrators.

The SCSI host and transport templates are the only members left in the
global zfcp_data struct. Move them out of zfcp_data and remove the
now unused zfcp_data struct. Also update the names of the register and
unregister functions to use the zfcp_scsi prefix.

For task management commands, only LUN and flags are required. The
regular FCP setup already sets the LUN in the fcp_cmnd. All is

Allocate the data for the GID_PN request through the new FC
kmem_cache. While updating the GID_PN code, also introduce a helper
function for initializing the CT header for FC nameserver requests.
Remove the "paranoia" check as well, the GID_PN request data does not
suddenly change.

A data buffer that is passed to the hardware must not cross a page
boundary. zfcp uses a series of kmem_caches to align the data to not
cross a page boundary. Introduce a new kmem_cache for the FC requests
sent from the zfcp driver and use it for the ELS ADISC data. The goal
is to migrate to the FC kmem_cache in later patches and remove the
request specific kmem_caches.

zfcp requires a mempool for the status read data blocks to resubmit
the "status read" requests at any time. Each status read data block
has the size of a page (4096 bytes) and needs to be placed in one
page.
Instead of having a kmem_cache for allocating page sized chunks, use
mempool_create_page_pool to create a mempool returning pages and
remove the zfcp kmem_cache.

Introduce a scan treshold for the qdio outbound queues. By setting the
threshold the driver can tell qdio after how much used SBALs qdio
should schedule the outbound tasklet that scans the queue for finished
SBALs. The threshold is specific by the drivers because a
Hipersockets device is much faster in utilizing outbound buffers than a
ZFCP or OSA device.
The default values after how many used SBALs the tasklet should run are:
OSA: > 31 SBALs
Hipersockets: > 7 SBALs
zfcp: > 55 SBALs

The zfcpdump tool requires a method to attach exactly one LUN. The
easiest way to achieve this is to add a new zfcp module parameter.
When allow_lun_scan is set to "false", zfcp only accepts LUNs that
have been configured through the unit_add sysfs interface.

Initialization of the qdio waitqueue should happen when the qdio data
is initialized and the QDIOUP flag should be handled in the qdio code
as well. Adjust the code accordingly and remove the superfluos

The tracing environment of the zfcp LLD has become very bulky and hard
to maintain. Small changes involve a large modification process which
is error-prone and not effective. This patch is the first of a set to
redesign the zfcp tracing to a more straight-forward and easy to
extend scheme. It removes all interpretation and visualization parts
and focuses on bare logging of the information.
This patch deals with all trace records of the zfcp error recovery.

Interrupting the connection to the FCP channel while I/O requests are
being issued can lead to this deadlock. scsi_dispatch_cmd already

can lead to an unpredictable race condition. Fix this by only

If the evaluation of GPN_FT requests wants to remove an invalid port
from the system the zfcp_erp_port_shutdown function is triggered.
Depending on the system status a superior action (e.g. adapter reopen)
is required. This can lead to an invalid mem access of the port struct
which might be freed at the time since the superior action is not
holding a reference of the port which triggered this ERP action.

corresponding handler is not consistent and leads to an unpredictable

Move the mid-layer's ->queuecommand() invocation from being locked
with the host lock to being unlocked to facilitate speeding up the
critical path for drivers who don't need this lock taken anyway.
The patch below presents a simple SCSI host lock push-down as an
equivalent transformation. No locking or other behavior should change
with this patch. All existing bugs and locking orders are preserved.
Additionally, add one parameter to queuecommand,
struct Scsi_Host *
and remove one parameter from queuecommand,
void (*done)(struct scsi_cmnd *)
Scsi_Host* is a convenient pointer that most host drivers need anyway,
and 'done' is redundant to struct scsi_cmnd->scsi_done.
Minimal code disturbance was attempted with this change. Most drivers
needed only two one-line modifications for their host lock push-down.

If an exchange config is executed while the local link is down, the
request succeeds but the returned data is incomplete. Proceeding with
the adapter activation is leading to an unpredictable behaviour (e.g.
kernel panic) caused by invalid values. In such a scenario the
recommended ERP is to retry the action and wait for a link up event.
If the issue persists the activation has to fail.

Fix up trivial conflicts due to __rcu sparse annotation in include/linux/genhd.h
All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.
The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.
New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time. Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.
The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.
Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

With the change that drivers have to explicitly request the serial
number for SCSI commands, this field should not be part of the zfcp
traces. It is not worth the effort to request the serial number only
for tracing purposes, so simply remove this field from the debug
traces.

To stop this warning, change the request queue lock to disable irqs,

Enable the LUN scanning mechanism in the SCSI midlayer:
- Do not set the disable_target_scan bit in the FC transport class.
- Set max_lun to 0xFFFFFFFF to allow the midlayer scan to include
the two-level hierachical LUNs (like 0x40XX40XX00000000, but in SCSI
midlayer LUN format).
- Set max_id to a high value to allow triggering the SCSI device
rescan from sysfs.
When running in NPIV mode, zfcp accepts all LUNs in slave_attach. When
running in non-NPIV mode, the list of zfcp_unit structs determines
which SCSI devices are allowed on the current system.

This is the large change to switch from using the data in
zfcp_unit to zfcp_scsi_dev. Keeping everything working requires doing
the switch in one piece. To ensure that no code keeps using the data
in zfcp_unit, this patch also removes the data from zfcp_unit that is
now being replaced with zfcp_scsi_dev.
For zfcp, the scsi_device together with zfcp_scsi_dev exist from the
call of slave_alloc to the call of slave_destroy. The data in
zfcp_scsi_dev is initialized in zfcp_scsi_slave_alloc and the LUN is
opened; the final shutdown for the LUN is run from slave_destroy.
Where the scsi_device or zfcp_scsi_dev is needed, the pointer to the
scsi_device is passed as function argument and inside the function
converted to the pointer to zfcp_scsi_dev; this avoids back and forth
conversion betweeen scsi_device and zfcp_scsi_dev.
While changing the function arguments from zfcp_unit to scsi_device,
the functions names are renamed form "unit" to "lun". This is to have
a seperation between zfcp_scsi_dev/LUN and the zfcp_unit; only code
referring to the remaining configuration information in zfcp_unit
struct uses "unit".

With the change for the LUN data to be part of the scsi_device, the
slave_destroy callback will be the final call to the
zfcp_erp_unit_shutdown function. The erp tries to acquire a reference
to run the action asynchronously and fail, if it cannot get the
reference. But calling scsi_device_get from slave_destroy will fail,
because the scsi_device is already in the state SDEV_DEL.
Introduce a new call into the zfcp erp to solve this: The function
zfcp_erp_unit_shutdown_wait will close the LUN and wait for the erp to
finish without acquiring an additional reference. The wait allows to
omit the reference; the caller waiting for the erp to finish already
has a reference that holds the struct in place.

When sending DIX integrity segments with an I/O request, the
restriction for the maximum number of segments is still the same for
the zfcp hardware. Report the new sg_prot_tablesize for the SCSI host,
so that the number of integrity segments plus the number of data
segments is not larger than the hardware limit. This results in using
half of the hardware segments for integrity data and the other half
for regular data.

Extend the qdio API to allow polling in the upper-layer driver. This
is needed by qeth to use NAPI.
To use the new interface the upper-layer driver must specify the
queue_start_poll(). This callback is used to signal the upper-layer
driver that is has initiative and must process the inbound queue by
calling qdio_get_next_buffers(). If the upper-layer driver wants to
stop polling it calls qdio_start_irq().
Since adapter interrupts are not completely stoppable qdio implements
a software bit QDIO_QUEUE_IRQS_DISABLED to safely disable interrupts for an
input queue.
The old interface is preserved and will be used as is by zfcp.

Exploit the cio siosl function to trigger logging in the FCP channel
on qdio error conditions. Add a helper function in zfcp_qdio to ensure
that tracing is only triggered once before calling qdio_shutdown.
Trigger in zfcp for hardware logs are:
- timeout for FSF requests to the FCP channel
- "no recommendation" status from FCP channel
- invalid FSF protocol status
- stalled outbound queue
- unknown request id on inbound queue
- QDIO_ERROR_SLSB_STATE
All of the above triggers run from the Linux qdio softirq context, so
no additional synchronization is necessary for the handling of the
ZFCP_STATUS_ADAPTER_SIOSL_ISSUED flag.

Introduce support for DIF/DIX in zfcp: Report the capabilities for the
Scsi_host, map the protection data when issuing I/O requests and
handle the new error codes. Also add the fsf data_direction field to
the hba trace, it is useful information for debugging in that area.
This is an EXPERIMENTAL feature for now.

A lot of functions require the amount of SBALs as one of their
parameter which is most times invariable. Therefore remove this
parameter and set the SBAL value explicitly if a non standard value is
required. In addition the warning message "oversized data" is
replaced with a BUG_ON() statement assuring the limits defined and
requested by zfcp.

Configuring a LUN in zfcp, also creates a SCSI device. For
consistency, it makes sense to remove the SCSI device when the LUN is
deconfigured. Replace the flush_work with the call to
scsi_remove_device: scsi_remove_device also takes the scan_mutex that
synchronizes itself with any long running device discovery.

When running in non-NPIV mode, the port_reopen in terminate_rport_io
might succeed even though the remote port is not available. If the
same port connection is held open from another operating system, the
reopen is only a virtual operation and might not hit the SAN. Fix this
by changing the call to forced_reopen that forces a logout/login
operation in the SAN.

When the successful return of an adisc is the final step to set the
port online, the registration of SCSI devices might be omitted. SCSI
devices that have been removed before (due to a short dev_loss_tmo
setting) might not be attached again.
The problem is that the registration of SCSI devices is done only
after erp has finished. The correct place would be after the call to
fc_remote_port_add to mimick the scan in the FC transport class.
Change the registration of SCSI devices to be triggered after the
fc_remote_port_add call. For the initial inquiry command to succeed,
the unit must also be open. If the unit reopen is still pending, the
inquiry command to the LUN will be deferred with DID_IMM_RETRY, so
there is no harm from this approach.

When the REOPEN_PORT_FORCED erp action succeeds, the port has been
closed. A REOPEN_PORT will try to open the port after the
REPORT_PORT_FORCED. The rport should only be unblocked after the
successful completion of the reopen port.

Trying to read the FC host statistics on an offline adapter results in
a 5 seconds wait. Reading the statistics tries to issue an exchange
port data request which first waits up to 5 seconds for an entry in
the request queue.
Change the strategy for getting a free SBAL to exit when the queue is
stopped. Reading the statistics will then fail without the wait.

Use nonseekable_open for a couple of s390 device drivers. This avoids

With debug kernels, the memory allocated with kmem_cache_alloc might
be initialized with the poison values 6b and a5. Use kmem_cache_zalloc
instead of kmem_cache_alloc to get zeroed memory and not send invalid
requests.

Instead of dealing with large segments in the scatter-gather lists in
zfcp_qdio.c, report the limits to the upper layers. With these limits
in place, the code for mapping large data blocks to multiple sbales
can be removed.

When a FSF request is returned with an error it should be reported
through blktrace for the ziomon tools, but the latency information
should not be read. Fix this by also calling zfcp_fsf_req_trace for
the error case, but skip reading the latencies inside the function.

The return code FAST_IO_FAIL from fc_block_scsi_eh indicates that the
pending I/O requests have been terminated as a result of the
fast_io_fail_tmo. Pass this return code back to the scsi eh to stop
the scsi eh in this case.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Move the qdio related structs and some helper functions to a new
zfcp_qdio.h header file. While doing this, rename the struct
zfcp_queue_req to zfcp_qdio_req to adhere to the naming scheme used in
zfcp. This allows a better seperation of the qdio code and inlining
the helper functions will save some function calls.

Move the decision which trace tag and trace level to use for the scsi

0 is a valid value for a LUN. It is slightly confusing to also see 0
in the trace entries relating to adapter and port. Change this to use
0xFFFFFFFFFFFFFFFF in the LUN field when the trace entry does not

On a link down, the adapter reopen is not strictly necessary, but it
helps flushing pending requests as quickly as possible. Add a comment
mentioning this.
qdio returning a problem on the response queue is an unlikely event.
The recovery mentioned in the comment might resolve it, so implement
it. This also has the advantage that it creates an entry in the
recovery trace to see if and when this is occurring.

Move the code for tracking FSF requests to new file to have this code
in one place. The functions for adding and removing requests on the
I/O path are already inline. The alloc and free functions are only
called once, so it does not hurt to inline them and add them to the
same file.

The status FC_CTELS_STATUS_REJECT for all FC BSG errors is not
appropriate. Instead, report -EIO in the result field if there was a
problem in zfcp with the FC BSG request. If the request is good from
our point of view, report result 0, status FC_CTELS_STATUS_OK and let
userspace read the Accept or Reject from the payload (as documented in
scsi_bsg_fc.h).

The hardware used with zfcp provides a timer for CT and ELS requests
instead of an abort capability for these commands. To correctly handle
the FC BSG timeouts, pass the timeout from the BSG requests to the
hardware.

Introduce a zfcp callback for timeouts triggered from FC BSG. With
zfcp, the underlying hardware cannot abort CT or ELS requests, so
there is nothing to do when the block layer timeout expires. To avoid
interference with the block layer timeout, simply indicate that the
block layer timer should be reset. The timer running in the hardware
for the pending CT or ELS request will return the request when it
expires.

Advance the correct pointer when inserting the linebreak for the HBA
trace. It was missing in the output since the pointer to the output
buffer was never advanced, and the linebreak character was overwritten
later.
