Due to bug history and code review, the node reference counting approach in
the driver isn't implemented consistently with how the scsi and nvme
transport perform registrations and unregistrations and their callbacks.
This resulted in many bad/stale node pointers.
Reword the driver so that reference handling is performed as follows:
- The initial node reference is taken on structure allocation
- Take a reference on any add/register call to the transport
- Remove a reference on any delete/unregister call to the transport
- After the node has fully removed from both the SCSI and NVMEe transports
(dev_loss_callbacks have called back) call the discovery engine
DEVICE_RM event which will remove the final reference and release the
node structure.
- Alter dev_loss handling when a vport or base port is unloading.
- Remove the put_node handling - no longer needed.
- Rewrite the vport_delete handling on reference counts. Part of this
effort was driven from the FDISC not registering with the transport and
disrupting the model for node reference counting.
- Deleted lpfc_nlp_remove. Pushed it's remaining ops into
lpfc_nlp_release.
- Several other small code cleanups.

The lpfc driver is calling get_device and put_device on scsi_fc_transport
device structure. When this code was removed, the driver triggered an oops
in "scsi_is_host_dev" when the first SCSI target was unregistered from the
transport.
The reason the calls were necessary is that the driver is calling
scsi_remove_host too early, before the target rports are unregistered and
the scsi devices disconnected from the scsi_host. The fc_host was torn
down during fc_remove_host.
Fix by moving the lpfc_pci_remove_one_s3/s4 calls to scsi_remove_host to
after the nodes are cleaned up. Remove the get_device and put_device calls
and the supporting code.

Now that the driver has gone to a normal ref interface (with no odd logic)
the discovery logic needs to be updated to reworked so that it properly
takes references when it should and give them up when it should.
Rework the driver for the following get/put model:
- Move gets to just before an I/O is issued. Add gets for places where an
I/O was issued without one.
- Ensure that failures from lpfc_nlp_get() are handled by the driver.
- Check and fix the placement of lpfc_nlp_puts relative to io completions.
Note: some of these paths may not release the reference on the exact io
completion as the reference is held as the code takes another step in
the discovery thread and which may cause another io to be issued.
- Rearrange some code for error processing and calling lpfc_nlp_put.
- Fix some places of incorrect reference freeing that was causing the
premature releasing of the structure.
- Nvmet plogi handling performs unreg_rpi's. The reference counts were
unbalanced resulting in premature node removal. In some cases this
caused loss of node discovery. Corrected the reftaking around nvmet
plogis.
Nodes that experience devloss now get released from the node list now that
there is a proper reference taking.

When a remote port is disconnected and disappears, its node structure
(ndlp) stays allocated and on a vport node list. While on the list it can
be matched, thus requires validation checks on state to be added in
numerous code paths. If the node comes back, its possible for there to be
multiple node structures for the same device on the vport node list. There
is no reason to keep the node structure around after it is no longer in
existence, and the current implementation creates problems for itself
(multiple nodes) and lots of unnecessary code for state validation.
Additionally, the reference taking on the node structure didn't follow the
normal model used by the kernel kref api. It included lots of odd logic to
match state with reference count. The combination of this odd logic plus
the way it was implicitly used in the discovery engine made its reference
taking implementation suspect and extremely hard to follow.
Change the driver such that the reference taking routines are now normal
ref increments/decrements and callout on refcount=0.
With this in place, the rework can be done such that the node structure is
fully removed and deallocated when the remote port no longer exists and all
references are removed. This removal logic, and the basic ref counting are
intrically tied, thus in a single patch.

Now that MIB support was registered with FDMI, the driver may receive CT
requests for MIB-related commands. At this time, no command is
supported. However, the driver needs to be graceful and reject the CT
request.
This patch adds identification of the requests as well as sending the
reject response.

Created new attribute lpfc_enable_mi, which by default is enabled.
Add command definition bits for SLI-4 parameters that recognize whether the
adapter has MIB information support and what revision of MIB data. Using
the adapter information, register vendor-specific MIB support with FDMI.
The registration will be done every link up.
During FDMI registration, encountered a couple of errors when reverting to
FDMI rev1. Code needed to exist once reverting. Fixed these.

The driver supports arbitrarily large scatter-gather lists and the current
value for max_sectors is limiting.
Change max_sectors to the largest value. This was actually done prior but
it only corrected one template and that template was later removed.
So change the remaining 2 templates. Other areas which hard-set the sectors
value should be inheriting what is in the template.

Commit 9816ef6ecb ("scsi: lpfc: Use after free in lpfc_rq_buf_free()")
was made to correct a use after free condition in lpfc_rq_buf_free().
Unfortunately, a subsequent patch cut on a tree without the fix
inadvertently reverted the fix.
Put the fix back: Move the freeing of the rqb_entry to after the print
function that references it.

With the conversion to blk_io_poll for better interrupt latency in normal
cases, it introduced this code path, executed when I/O aborts or logouts
are seen, which attempts to allocate memory for a mailbox command to be
issued. The allocation is GFP_KERNEL, thus it could attempt to sleep.
Fix by creating a work element that performs the event handling for the
remote port. This will have the mailbox commands and other items performed
in the work element, not the irq. A much better method as the "irq" routine
does not stall while performing all this deep handling code.
Ensure that allocation failures are handled and send LOGO on failure.
Additionally, enlarge the mailbox memory pool to reduce the possibility of
additional allocation in this path.

Three fixes: one in drivers (lpfc) and two for zoned block devices.
The latter also impinges on the block layer but only to introduce a
new block API for setting the zone model rather than fiddling with the
queue directly in the zoned block driver.

Initial FLOGIs are failing with the following message:
lpfc 0000:13:00.1: 1:(0):0820 FLOGI Failed (x300). BBCredit Not Supported
In a prior patch, the READ_SPARAM command was re-ordered to post after
CONFIG_LINK as the driver is expected to update the driver's copy of the
service parameters for the FLOGI payload. If the bb-credit recovery feature
is enabled, this is fine. But on adapters were bb-credit recovery isn't
enabled, it would cause the FLOGI to fail.
Fix by restoring the original command order (READ_SPARAM before
CONFIG_LINK), and after issuing CONFIG_LINK, detect bb-credit recovery
support and reissuing READ_SPARAM to obtain the updated service parameters
(effectively adding in the fix command order).

The driver is unable to successfully login with remote device. During pt2pt
login, the driver completes its FLOGI request with the remote device having
WWN precedence. The remote device issues its own (delayed) FLOGI after
accepting the driver's and, upon transmitting the FLOGI, immediately
recognizes it has already processed the driver's FLOGI thus it transitions
to sending a PLOGI before waiting for an ACC to its FLOGI.
In the driver, the FLOGI is received and an ACC sent, followed by the PLOGI
being received and an ACC sent. The issue is that the PLOGI reception
occurs before the response from the adapter from the FLOGI ACC is
received. Processing of the PLOGI sets state flags to perform the REG_RPI
mailbox command and proceed with the rest of discovery on the port. The
same completion routine used by both FLOGI and PLOGI is generic in
nature. One of the things it does is clear flags, and those flags happen to
drive the rest of discovery. So what happened was the PLOGI processing set
the flags, the FLOGI ACC completion cleared them, thus when the PLOGI ACC
completes it doesn't see the flags and stops.
Fix by modifying the generic completion routine to not clear the rest of
discovery flag (NLP_ACC_REGLOGIN) unless the completion is also associated
with performing a mailbox command as part of its handling. For things such
as FLOGI ACC, there isn't a subsequent action to perform with the adapter,
thus there is no mailbox cmd ptr. PLOGI ACC though will perform REG_RPI
upon completion, thus there is a mailbox cmd ptr.

On devices that support FCP sequence error recovery, which attempts to
preserve the devices login across link bounce, adisc is used for device
validation. Turns out the device fc4 type is cleared as part of the link
bounce, but the ADISC handling doesn't restore the FC4 support as it
normally would with a PRLI. This caused situations where the device wasn't
reregistered with the transport thus scan logic and LUN discovery never
kicked in.
In the ADISC completion handling, reset the fc4 type so that transport port
reregistration occurs with the remote port.

With port bounce/address swaps and timing between initiator GID queries vs
remote port FC4 support registrations, the driver may be in a situation
where it sends PRLIs for both FCP and NVME even though the target may not
support one of the protocols. In this case, the remote port will reject the
PRLI and usually indicate it does not support the request. However, the
driver currently ignores the status of the failure and immediately retries
the PRLI, which is pointless. In the case of this one remote port, the
reception of the PRLI retry caused it to decide to send a LOGO. The LOGO
restarted the process and the same results happened. It made the remote
port undiscoverable to either protocol.
Add logic to detect the non-support status and not attempt the retry
of the PRLI.

While mds diagnostic tests are running, if the driver is requested to be
unloaded, oops or hangs are observed. The driver doesn't terminate the
processing of diag frames when the unload is started. As such: oops may be
seen for __lpfc_sli_release_iocbq_s4 because ring memory is referenced that
was already freed; or hangs see in lpfc_nvme_wait_for_io_drain as ios no
longer complete.
If unloading, don't process diag frames. Just clean them up.

In configs with a large number of initiators in the same zone (>250), RSCN
timeouts are seen when creating or deleting vports:
lpfc 0000:07:00.1: 5:(0):0231 RSCN timeout Data: x0 x3
During RSCN processing driver issues GID_FT command to nameserver. A
counter for number of simultaneous GID_FT commands is maintained (an
unsigned value). The counter is incremented when the GID_FT is issued. If
the GID_FT command fails for some reason the driver retries the GID_FT from
the completion call back. But the counter was decremented before the retry
was issued. When the second GID_FT completes, the callback again tries to
decrement the counter, possibly wrapping to a very large non-zero value,
which causes the RSCN cleanup code to not execute. Thus the RSCN timeout
failure.
Do not decrement the counter on a retry. Also add defensive checks to
ensure the counter is not decremented if already zero.

Current Link speed was shown as "unknown" in sysfs for FCoE ports. In this
scenario, the port was working in 20G speed, which happens to not be a
speed handled by the driver.
Add support for all possible link speeds that could get reported from
port_speed field in link state ACQE.
Additionally, as supported_speeds can't be manipulated via the FCoE driver
on a converged ethernet port (it must be managed by the nic function),
don't fill out the supported_speeds field for the fc host object in sysfs.

We cannot wait on a completion object in the lpfc_nvme_targetport structure
in the _destroy_targetport() code path because the NVMe/fc transport will
free that structure immediately after the .targetport_delete() callback.
This results in a use-after-free, and a crash if slub_debug=FZPU is
enabled.
An earlier fix put put the completion on the stack, but commit 2a0fb340fc
("scsi: lpfc: Correct localport timeout duration error") subsequently
changed the code to reference the completion through a pointer in the
object rather than the local stack variable. Fix this by using the stack
variable directly.

nvme-fc allows the driver to specify a default devloss_tmo value when
registering the remote port. The lpfc driver is currently not doing so,
although it is implementing a driver internal node loss value of 30s which
also is used on the SCSI FC remote port. As no devloss_tmo is set, the
nvme-fc transport defaults to 60s. So there are competing timers.
Additionally, due to the competing timers, it is possible the NVMe rport is
removed but the SCSI rport remains. It is possible that the SCSI FC rport,
which was registered for the NVMe port even if it doesn't utilize the SCSI
protocol, had been tuned to not match either the 60s (nvme-fc default) or
30s (lldd default), it gets out of whack. The lldd will defer to the SCSI
FC rport as long as the SCSI FC rport has not had its devloss_tmo expire.
Correct the situation by specifying a default devloss_tmo to the nvme-fc
transport when registering the rport. Take the value from the SCSI FC
rport if it exists, otherwise use the driver default.

The expression start_idx - dbg_cnt is evaluated using unsigned int
arthithmetic (since these variables are unsigned ints) and hence can never
be less than zero, so the less than comparison is never true. Rewrite the
expression to check for start_idx being less than dbg_cnt.
After the logic was corrected, temp_idx wasn't working correctly. So fix it
as well.

Addresses-Coverity: ("Unsigned compared against 0")
With certain platforms its possible pci_alloc_irq_vectors() may affinitize
irq vectors to multiple (all?) CPUs. The driver is currently assuming
exclusivity and vectors being doled out to different CPUs and is assigning
primary ownership of each vector to the first CPU in the mask. The code
doesn't bother to check if the CPU already owns a vector and will
unconditionally overwrite the CPU to vector mapping. This causes the
relationships between eq's and cq's to get confused and gets worse when
CPUs start to offline. The net results are interrupts are skipped resulting
in mailbox timeouts and there are oopses in CPU offling flows.
Fix this changing up the primary vector assignment. Now assign the eq to a
CPU only if it is the CPU in the mask that does not have a prior
assignment. And once the primary ownership is assigned, break from the
loop. For CPUs that may have been set before but not the primary owner, the
lpfc_cpu_affinity_check() routine will balance the CPU to eq assignment.

The current logging methods typically end up requesting a reproduction with
a different logging level set to figure out what happened. This was mainly
by design to not clutter the kernel log messages with things that were
typically not interesting and the messages themselves could cause other
issues.
When looking to make a better system, it was seen that in many cases when
more data was wanted was when another message, usually at KERN_ERR level,
was logged. And in most cases, what the additional logging that was then
enabled was typically. Most of these areas fell into the discovery machine.
Based on this summary, the following design has been put in place: The
driver will maintain an internal log (256 elements of 256 bytes). The
"additional logging" messages that are usually enabled in a reproduction
will be changed to now log all the time to the internal log. A new logging
level is defined - LOG_TRACE_EVENT. When this level is set (it is not by
default) and a message marked as KERN_ERR is logged, all the messages in
the internal log will be dumped to the kernel log before the KERN_ERR
message is logged.
There is a timestamp on each message added to the internal log. However,
this timestamp is not converted to wall time when logged. The value of the
timestamp is solely to give a crude time reference for the messages.

Although the existing implementation is very good at high I/O load, on
tests involving light load, especially on only a few hardware queues,
latency was a little higher than it can be due to using workqueue
scheduling. Other tasks in the system can delay handling.
Change the lower level to use irq_poll by default which uses a softirq for
I/O completion. This gives better latency as variance in when the cq is
processed is reduced over the workqueue interface. However, as high load is
better served by not being in softirq when the CPU is loaded, work queues
are still used under high I/O load.

When the kdump kernel shuts down lpfc calls flush_work_queue on an
interrupt to schedule the cq handler. When there is only one CPU active on
the kdump kernel, it is possible for the work_on to get scheduled on a
non-active CPU causing it to never be scheduled.
When in the kdump environment, per-CPU affinity of cq's to cpus is not
necessary. In those cases, use a general queue_work rather than a
queue_work_on().

When vports are deleted, it is observed that there is memory/kthread
leakage as the vport isn't fully being released.
There is a shost reference taken in scsi_add_host_dma that is not released
during scsi_remove_host. It was noticed that other drivers resolve this by
doing a scsi_host_put after calling scsi_remove_host.
The vport_delete routine is taking two references one that corresponds to
an access to the scsi_host in the vport_delete routine and another that is
released after the adapter mailbox command completes that destroys the VPI
that corresponds to the vport.
Remove one of the references taken such that the second reference that is
put will complete the missing scsi_add_host_dma reference and the shost
will be terminated.

Call traces have been observed running different tests that involve aborts
and setting the rrq active flag. The lpfc_set_rrq_active routine is doing
a mempool_alloc under the soft_irq processing level. When the mempool needs
to get a new buffer from the free pool and has to wait for memory to become
free it will check the flags passed in on the alloc and dump the stack if
the thread is running in interrupt context.
Replace the GFP_KERNEL flag with GFP_ATOMIC so that the memory allocation
will not attempt to sleep if there is no mem available.

During driver unload/reload testing, the NVMe initiator would not
re-establish connectivity to NVMe controllers on reload.
The failing NVMe array supports concurrent FCP and NVMe operation via
different nport_id's. The array was repeatedly sending an ADISC every 2
seconds after PLOGI completed and while NVMe subsystems were executing
discovery. The target would continue this state for roughly 45 seconds.
The driver's current behavior on ADISC receipt is to validate a the ADISC
vs the device and issue a RESUME_RPI to restore transmission. The receipt
of the ADISC effectively caused a driver to take actions similar to a
logout and login for the remote port, causing the deregistration of the
nvme rport and a subsequent re-registration. This caused a constant reset
and re-connect of the NVMe controller while this 45s window occurred. There
was no need for the state changes as ADISC does not change login state.
This patch corrects this behavior by validating if the remoteport is
already logged in (MAPPED) and when true, avoids the call to set the ndlp
state to MAPPED, which triggers the unreg/re-reg. Thus ADISC does not
change the login state of the node.

Coverity reported the following error:
Assigned value that is never used may represent unnecessary computation.
The rc variable was initially assigned a value but in several cases, when
an error case is detected, it is reassigned a new value. The initial value
had little use.
In code-reviewing this routine, it could use some cleanup:
- Setting the initialization value to -ENODEV is a much better choice and
lessens code in the routine.
- The wasn't tracking logic errors vs no error and mailbox failure.
Better to resolve by adding a status to track the mailbox failure
and merge it with the logic error when the routine returns.

The axchg structure is a structure allocated early in the
lpfc_nvme_unsol_ls_handler() to represent the newly received exchange.
Upon error, the out_fail path in the routine unconditionally frees the
pointer, yet subsequently passes the pointer to the abort routine.
Additionally, the abort routine, lpfc_nvme_unsol_ls_issue_abort(), also
has a failure path that will attempt to delete the pointer on error.
Fix these errors by:
- Removing the unconditional free so that it stays valid if passed
to the abort routine.
- Revise the abort routine to not free the pointer. Instead, return
a success/failure status. Note: if success, the later completion of
the abort frees the structure.
- Back in the unsol_ls_handler() error path, if the abort routine was
skipped (thus no possible reference) or the abort routine returned
error, free the pointer.

In order to create or activate a new node, lpfc_els_unsol_buffer() invokes
lpfc_nlp_init() or lpfc_enable_node() or lpfc_nlp_get(), all of them will
return a reference of the specified lpfc_nodelist object to "ndlp" with
increased refcnt.
When lpfc_els_unsol_buffer() returns, local variable "ndlp" becomes
invalid, so the refcount should be decreased to keep refcount balanced.
The reference counting issue happens in one exception handling path of
lpfc_els_unsol_buffer(). When "ndlp" in DEV_LOSS, the function forgets to
decrease the refcnt increased by lpfc_nlp_init() or lpfc_enable_node() or
lpfc_nlp_get(), causing a refcnt leak.
Fix this issue by calling lpfc_nlp_put() when "ndlp" in DEV_LOSS.

As the nvmet layer does not have the concept of a remoteport object, which
can be used to identify the entity on the other end of the fabric that is
to receive an LS, the hosthandle was introduced. The driver passes the
hosthandle, a value representative of the remote port, with a ls request
receive. The LS request will create the association. The transport will
remember the hosthandle for the association, and if there is a need to
initiate a LS request to the remote port for the association, the
hosthandle will be used. When the driver loses connectivity with the
remote port, it needs to notify the transport that the hosthandle is no
longer valid, allowing the transport to terminate associations related to
the hosthandle.
This patch adds support to the driver for the hosthandle. The driver will
use the ndlp pointer of the remote port for the hosthandle in calls to
nvmet_fc_rcv_ls_req(). The discovery engine is updated to invalidate the
hosthandle whenever connectivity with the remote port is lost.

nvmefc_tgt_ls_req -> nvmefc_ls_rsp
nvmefc_tgt_ls_req.nvmet_fc_private -> nvmefc_ls_rsp.nvme_fc_private
Change nvmet_fc_rcv_ls_req() calling sequence to provide handle that
can be used by transport in later LS request sequences for an association.

In an audit of lockdep calls in the driver, there are multiple lockdep
checks in successive calling layers. E.g. a routine checks, and then calls
a lower routine that also checks, and so on. Calling sequences result in
many redundant checks.
Refine the code to remove lower-level lockdep checks. Update comments on
the lock, correcting a few places where lock object in comment was

By default, the driver attempts to allocate a hdwq per logical cpu in order
to provide good cpu affinity. Some systems have extremely high cpu counts
and this can significantly raise memory consumption.
In testing on x86 platforms (non-AMD) it is found that sharing of a hdwq by
a physical cpu and its HT cpu can occur with little performance
degredation. By sharing, the hdwq count can be halved, significantly
reducing the memory overhead.
Change the default behavior of the driver on non-AMD x86 platforms to
share a hdwq by the cpu and its HT cpu.

The lldd rebinds the ndlp with rport during a nvme rport registration (via
nvme_fc_register_remoteport). If rport & ndlp pointers are same as the
previous one, the lldd will re-use the ndlp and rport association without
re-initialization. This assumption is incorrect. The lldd should be
ignorant of whether the returned rport pointer is new or not, and should
always assume it is new.
Remove the re-binding code, always assumes that rport pointer received from
transport is a new pointer.

The original patch was to resolve the lldd being able to be unloaded
while being used to talk to the boot device of the system. However, the
end result of the original patch is that any driver unload while a nvme
controller is live via the lldd is now being prohibited. Given the module
reference, the module teardown routine can't be called, thus there's no
way, other than manual actions to terminate the controllers.

The cpu io statistics were capped by a hard define limit of 128. This
effectively was a max number of CPUs, not an actual CPU count, nor actual
CPU numbers which can be even larger than both of those values. This made
stats off/misleading and on large CPU count systems, wrong.
Fix the stats so that all CPUs can have a stats struct. Fix the looping
such that it loops by hdwq, finds CPUs that used the hdwq, and sum the
stats, then display.

The AER interfaces to clear error status registers were a confusing mess:
- pci_cleanup_aer_uncorrect_error_status() cleared non-fatal errors
from the Uncorrectable Error Status register.
- pci_aer_clear_fatal_status() cleared fatal errors from the
Uncorrectable Error Status register.
- pci_cleanup_aer_error_status_regs() cleared the Root Error Status
register (for Root Ports), the Uncorrectable Error Status register,
and the Correctable Error Status register.
Rename them to make them consistent:

pci_aer_clear_fatal_status() pci_aer_clear_fatal_status()
pci_cleanup_aer_error_status_regs() pci_aer_clear_status()

The lpfc_sli4_wq_release() routine iterates for each interim value when
updating the wq consuemr index. This wastes cycles and possibly confuses
things as thevalue itterates (and the modulo logic is being applied).
There's no reason for this. Just set it to the value from the hw.

Injecting EEH on a 32GB card is causing kernel oops
The pci error handler is doing an IO flush and the offline code is also
doing an IO flush. When the 1st flush is complete the hdwq is destroyed
(freed), yet the second flush accesses the hdwq and crashes.
Added a check in lpfc_sli4_fush_io_rings to check both the HBA_IOQ_FLUSH

freed.

SCSI layer sends driver IOs with more s/g segments than driver can handle.
This results in "Too many sg segments from dma_map_sg. Config 64, seg_cnt
219" error messages from the lpfc_scsi_prep_dma_buf_s3() routine.
The was due to use the driver using individual templates for pport and
vport, host reset enabled or not, nvme vs scsi, etc. In the end, there was
a combination for a vport that didn't match the pport.
Rather than enumerating more templates and more discretionary assignments,
revert to a base template that is copied to a template specific to the
pport/vport. Then, based on role, attributes and sli type, modify the
fields that are different for that port. Added a log message to
lpfc_create_port to validate values.

In lpfc_nvmet_prep_fcp_wqe() the line "rsp->sg_cnt = 0" is modifying the
transport's data structure. This may result in the transport believing the
s/g list was already freed, thus may not unmap/free it properly. Lpfc
driver should not modify the transport data structure.
The zeroing of the sg_cnt is to avoid use of the transport's sgl in a
subsequent loop where the driver builds the necessary requests for the
adapter firmware to complete the IO.
Change LLDD to use a local copy of the transport sg_cnt when building
requests to be passed to the adapter fw.

When processing the completion of a "Reg Rpi" login mailbox command in
lpfc_sli_def_mbox_cmpl, a call may be made to lpfc_unreg_login. The vpi is
extracted from the completing mailbox context and passed as an input for
the next. However, the vpi stored in the mailbox command context is an
absolute vpi, which for SLI4 represents both base + offset. When used with
a non-zero base component, (function id > 0) this results in an
out-of-range access beyond the allocated phba->vpi_ids array.
Fix by subtracting the function's base value to get an accurate vpi number.

This patch modifies lpfc to register for Link Integrity events via the use
of an RDF ELS and to perform Link Integrity FPIN logging.
Specifically, the driver was modified to:
- Format and issue the RDF ELS immediately following SCR registration.
This registers the ability of the driver to receive FPIN ELS.
- Adds decoding of the FPIN els into the received descriptors, with
logging of the Link Integrity event information. After decoding, the ELS
is delivered to the scsi fc transport to be delivered to any user-space
applications.
- To aid in logging, simple helpers were added to create enum to name
string lookup functions that utilize the initialization helpers from the
fc_els.h header.
- Note: base header definitions for the ELS's don't populate the
descriptor payloads. As such, lpfc creates it's own version of the
structures, using the base definitions (mostly headers) and additionally
declaring the descriptors that will complete the population of the ELS.

There was report of an odd "Fix me..." log message, which was tracked down
to the lpfc_els_rcv_rps() routine. This was in handling of a very old and
obsolete ELS - Read Port Status. The RPS ELS was defined in FC-LS-1, but
deprecated in FC-LS-2, and removed from all later FC-LS revisions. It was
replaced by the Read Diagnostic Parameters (RDP) ELS and the Link Error

When driver is set to enable bb credit recovery, the switch displayed the
setting as inactive. If the link bounces, it switches to Active.
During link up processing, the driver currently does a MBX_READ_SPARAM
followed by a MBX_CONFIG_LINK. These mbox commands are queued to be
executed, one at a time and the completion is processed by the worker
thread. Since the MBX_READ_SPARAM is done BEFORE the MBX_CONFIG_LINK, the
BB_SC_N bit is never set the the returned values. BB Credit recovery status
only gets set after the driver requests the feature in CONFIG_LINK, which
is done after the link up. Thus the ordering of READ_SPARAM needs to follow
the CONFIG_LINK.
Fix by reordering so that READ_SPARAM is done after CONFIG_LINK. Added a
HBA_DEFER_FLOGI flag so that any FLOGI handling waits until after the
READ_SPARAM is done so that the proper BB credit value is set in the FLOGI
payload.

The driver is occasionally seeing the following SLI Port error, requiring
reset and reinit:
Port Status Event: ... error 1=0x52004a01, error 2=0x218
The failure means an RQ timeout. That is, the adapter had received
asynchronous receive frames, ran out of buffer slots to place the frames,
and the driver did not replenish the buffer slots before a timeout
occurred. The driver should not be so slow in replenishing buffers that a
timeout can occur.
When the driver received all the frames of a sequence, it allocates an IOCB
to put the frames in. In a situation where there was no IOCB available for
the frame of a sequence, the RQ buffer corresponding to the first frame of
the sequence was not returned to the FW. Eventually, with enough traffic
encountering the situation, the timeout occurred.
Fix by releasing the buffer back to firmware whenever there is no IOCB for
the first frame.

Overall this would be a reduction both in complexity and line count, but

2. Actually moving handlers out of block/compat_ioctl.c and
block/scsi_ioctl.c into drivers, mixed in with cleanup
patches
3. Document how to do this right. I keep getting asked about this,
and it helps to point to some documentation file.
The branch is based on another one that fixes a couple of bugs found
during the creation of this series.

the host with a maximum delay of few hundreds of microseconds. If the
switch doesn't get response frames within this time frame, it fails the
test.
Test is failing as the lpfc-wq workqueue was overwhelmed by the packet rate
and in some cases, the work element yielded to other kernel elements.
To resolve, reduce the outstanding load allowed by the adapter. This
ensures the driver spends a reasonable amount of time doing loopback and
can do so such that latency values can be met. Load is managed by reducing
the number of receive buffers posted such that the link can be
backpressured to reduce load.

When unattaching, the driver did not unmap the DPP bar. This caused the
next load of the driver, which attempts to enable wc, to not work correctly
and wc to be disabled due to an address mapping overlap.
Fix by unmapping on unattach.

The order of the flags/checks for adapters where FC-AL is supported
erroneously excluded lpe35000 adapter models. Also noted that the G7 flags
for Loop and Persistent topology are incorrect. They should follow the
rules as G6.
Rework the logic to enable LPe35000 FC-AL support. Collapse G7 support
logic to the same rules as G6.

/sys/kernel/debug/lpfc/fn0/ras_log always shows the same ras_log even if
there are link bounce events triggered via issue_lip
Dynamic FW logging had logic that prematurely breaks from the buffer
filling loop.
Fix the check for buffer overrun by looking before copying and restricting
copy length to the remaining buffer. When copying, ensure space for NULL
character is left in the buffer. While in the routine - ensure the buffer
is cleared before adding elements.

There are reports of multiple ports on the same system displaying different
hostnames in fabric FDMI displays.
Currently, the driver registers the hostname at initialization and obtains
the hostname via init_utsname()->nodename queried at the time the FC link
comes up. Unfortunately, if the machine hostname is updated after
initialization, such as via DHCP or admin command, the value registered
initially will be incorrect.
Fix by having the driver save the hostname that was registered with FDMI.
The driver then runs a heartbeat action that will check the hostname. If
the name changes, reregister the FMDI data.
The hostname is used in RSNN_NN, FDMI RPA and FDMI RHBA.

When the WriteObject mailbox response has change_status set to is 0x2
(Firmware Reset) or 0x04 (Port Migration Reset), the CSF field should also
be checked to see if a fw reset is sufficient to enable all new features in
the updated firmware image. If not, a fw reset would start the new
firmware, but with a feature level equal to existing firmware. To enable
the new features, a chip reset/pci slot reset would be required.
Check the CSF bit when change_status is 0x2 or 0x4 to know whether to
perform a pci bus reset or fw reset.

This patch reworks the fdmi symbolic node name data for the following two
issues:
- Correcting extraneous periods following the DV and HN fdmi data fields.
- Avoiding buffer overflow issues when formatting the data.
The fix to the fist issue is to just remove the characters.
The fix to the second issue has all data being staged in temporary storage
before being moved to the real buffer.
