On some EHCI usb debug controllers, the EHCI debug device will fail to
be seen after a port reset, after a warm reset. Two options exist to
get the device to initialize correctly.
Option 1 is to unplug and plug in the device.
Option 2 is to use the EHCI port test to get the usb debug device to
start talking again. At that point the debug controller port reset
will succeed.

If the EHCI debug port is initialized and in use, the EHCI host
controller driver must follow two rules.
1) If the EHCI host driver issues a controller reset, the debug
controller driver re-initialization must get called after the reset
is completed.
2) The EHCI host driver should ignore any requests to the physical
EHCI debug port when the EHCI debug port is in use.
The code to check for the debug port was moved from ehci_pci_reinit()
to ehci_pci_setup because it must get called prior to ehci_reset()
which will clear the debug port registers.

This patch implements several changes:
1) Improve the capability to debug the dbgp driver
The dbgp_ehci_status() was added in a number of places to report
the critical ehci registers to diagnose the cause of a failure of
the ehci-dbgp driver.
2) Capability to survive the host controller initialization
The dbgp_external_startup(), dbgp_not_safe, and dbgp_phys_port were
added so as to allow the ehci-dbgp to re-initialize after the ehci
host controller is reset by the standard host controller driver.
This same routine is common for the early startup or
re-initialization.
This resulted in the need to move some of the initialization code
out of the __init section because the ehci driver has the
possibility to be loaded later on as a kernel module.
3) Stability improvements for device initialization
The device enumeration from 0 to 127 has the possibility to fail
the first time after a warm reset on some older EHCI debug
controllers. The enumeration will be tried up to 3 times to
account for this failure case.
The dbg_wait_until_complete() was changed to wait up to 250 ms
before failing which only comes into play during device
initialization. The maximum delay will never get hit during the
course of normal operation of the driver, unless the device got
unplugged or there was a ehci controller failure, in which case the
dbgp device driver will shut itself down.

When do_output_char() attempts to write a carriage return/line feed sequence,
it first checks to see how much buffer room is available. If there are at least
two characters free, it will write the carriage return/line feed with two calls
to tty_put_char(). It calls the tty_operation functions write() for devices that
don't support the tty_operations function put_char(). If the USB generic serial
device's write URB is not in use, it will return the buffer size when asked how
much room is available. The write() of the carriage return will cause it to mark
the write URB busy, so the subsequent write() of the line feed will be ignored.
This patch uses the kfifo infrastructure to implement a write FIFO that
accurately returns the amount of space available in the buffer.

Usbnet framework assumes USB hardware doesn't handle zero length
packets, but SMSC LAN95xx requires these to be sent for correct
operation.
This patch fixes an easily reproducible tx lockup when sending a frame
that results in exactly 512 bytes in a USB transmission (e.g. a UDP
frame with 458 data bytes, due to IP headers and our USB headers). It
adds an extra flag to usbnet for the hardware driver to indicate that
it can handle and requires the zero length packets.

The USB 3.0 bus specification added an "Endpoint Companion" descriptor that is
supposed to follow all SuperSpeed Endpoint descriptors. This descriptor is used
to extend the bus protocol to allow more packets to be sent to an endpoint per
"microframe". The word microframe was removed from the USB 3.0 specification
because the host controller does not send Start Of Frame (SOF) symbols down the
USB 3.0 wires.
The descriptor defines a bMaxBurst field, which indicates the number of packets
of wMaxPacketSize that a SuperSpeed device can send or recieve in a service
interval. All non-control endpoints may set this value as high as 16 packets
(bMaxBurst = 15).
The descriptor also allows isochronous endpoints to further specify that they
can send and receive multiple bursts per service interval. The bmAttributes
allows them to specify a "Mult" of up to 3 (bmAttributes = 2).
Bulk endpoints use bmAttributes to report the number of "Streams" they support.
This was an extension of the endpoint pipe concept to allow multiple mass
storage device commands to be outstanding for one bulk endpoint at a time. This
should allow USB 3.0 mass storage devices to support SCSI command queueing.
Bulk endpoints can say they support up to 2^16 (65,536) streams.
The information in the endpoint companion descriptor must be stored with the
other device, config, interface, and endpoint descriptors because the host
controller needs to access them quickly, and we need to install some default
values if a SuperSpeed device doesn't provide an endpoint companion descriptor.

Modify the USB core to handle the new USB 3.0 speed, "SuperSpeed". This
is 5.0 Gbps (wire speed). There are probably more places that check for
speed that I've missed.
SuperSpeed devices have a 512 byte endpoint 0 max packet size. This shows
up as a bMaxPacketSize0 set to 0x09 (see table 9-8 of the USB 3.0 bus
spec).
xHCI spec says that the xHC can handle intervals up to 2^15 microframes. That
might change when real silicon becomes available.
Add FIXME note for SuperSpeed isochronous endpoints. They can transmit up
to 16 packets in one "burst" before they wait for an acknowledgment of the
packets. They can do up to 3 bursts per microframe (determined by the
mult value in the endpoint companion descriptor). The xHCI driver doesn't
have support for isoc yet, so fix this later.

This patch (as1254) splits up the shutdown method of usb_serial_driver
into a disconnect and a release method.
The problem is that the usb-serial core was calling shutdown during
disconnect handling, but drivers didn't expect it to be called until
after all the open file references had been closed. The result was an
oops when the close method tried to use memory that had been
deallocated by shutdown.

The usb_debug driver was modified to implement serial break handling
by using a "magic" data packet comprised of the sequence:
0x00 0xff 0x01 0xfe 0x00 0xfe 0x01 0xff
When the tty layer requests a serial break the usb_debug driver sends
the magic packet. On the receiving side the magic packet is thrown
away or a sysrq is activated depending on what kernel .config options
have been set.
The generic serial driver was modified as well as the usb serial
headers to generically implement sysrq processing in the same way the
non usb uart based drivers implement the sysrq handling. This will
allow other usb serial devices to implement sysrq handling as desired.
The new usb serial functions are named similarly and implemented
similarly to the uart functions as follows:
usb_serial_handle_break <-> uart_handle_break
usb_serial_handle_sysrq_char <-> uart_handle_sysrq_char

The usb_debug driver, when used as the console, will always fail to
insert the carriage return and new line sequence as well as randomly
drop console output. This is a result of only having the single
write_urb and that the tty layer will have a lock that prevents the
processing of the back to back urb requests.
The solution is to allow more than one urb to be outstanding and have
a slightly deeper transmit queue. The idea and some code is borrowed
from the ftdi_sio usb driver.
The generic usb serial driver was modified so as to allow the classic
method of 1 write urb, or a multi write urb scheme with N allowed
outstanding urbs where N is controlled by max_in_flight_urbs. When
max_in_flight_urbs in a "struct usb_serial_driver" is non zero the
multi write urb scheme will be used.
The size of 4000 was selected for the usb_debug driver so that the
driver lowers possibility of losing the queued console messages during
the kernel startup.

This introduces a CDC Ethernet Emulation Model (EEM) host side
driver to support USB EEM devices.
EEM is different from the Ethernet Control Model (ECM) currently
supported by the "CDC Ethernet" driver. One key difference is
that it doesn't require of USB interface alternate settings to
manage interface state; some maldesigned hardware can't handle
that part of USB. It also avoids a separate USB interface for
control and status updates.

USB serial likes to use port->tty back pointers for the real work it does and
to do so without any actual locking. Unfortunately when you consider hangup
events, hangup/parallel reopen or even worse hangup followed by parallel close
events the tty->port and port->tty pointers are not guaranteed to be the same
as port->tty is the active tty while tty->port is the port the tty may or
may not still be attached to.
So rework the entire API to pass the tty struct. For console cases we need
to pass both for now. This shows up multiple drivers that immediately crash
with USB console some of which have been fixed in the process.
Longer term we need a proper tty as console abstraction

Add <linux/usb/composite.h> interfaces for composite gadget drivers, and
basic implementation support behind it:
- struct usb_function ... groups one or more interfaces into a function
managed as one unit within a configuration, to which it's added by
usb_add_function().
- struct usb_configuration ... groups one or more such functions into
a configuration managed as one unit by a driver, to which it's added
by usb_add_config(). These operate at either high or full/low speeds
and at a given bMaxPower.
- struct usb_composite_driver ... groups one or more such configurations
into a gadget driver, which may be registered or unregistered.
- struct usb_composite_dev ... a usb_composite_driver manages this; it
wraps the usb_gadget exposed by the controller driver.
This also includes some basic kerneldoc.
How to use it (the short version): provide a usb_composite_driver with a
bind() that calls usb_add_config() for each of the needed configurations.
The configurations in turn have bind() calls, which will usb_add_function()
for each function required. Each function's bind() allocates resources
needed to perform its tasks, like endpoints; sometimes configurations will
allocate resources too.
Separate patches will convert most gadget drivers to this infrastructure.

Define three new descriptor manipulation utilities, for use when
setting up functions that may have multiple instances:
usb_copy_descriptors() to copy a vector of descriptors
usb_free_descriptors() to free the copy
usb_find_endpoint() to find a copied version
These will be used as follows. Functions will continue to have static
tables of descriptors they update, now used as __initdata templates.
When a function creates a new instance, it patches those tables with
relevant interface and string IDs, plus endpoint assignments. Then it
copies those morphed descriptors, associates the copies with the new
function instance, and records the endpoint descriptors to use when
activating the endpoints. When initialization is done, only the copies
remain in memory. The copies are freed on driver removal.
This ensures that each instance has descriptors which hold the right
instance-specific data. Two instances in the same configuration will
obviously never share the same interface IDs or use the same endpoints.
Instances in different configurations won't do so either, which means
this is slightly less memory-efficient in some cases.
This also includes a bugfix to the epautoconf code that shows up with
this usage model. It must replace the previous endpoint number when
updating the template descriptors, not just mask in a few more bits.

There's a new PM-related change notice for the USB 2.0 specification
called "Link Power Management" (LPM). It defines a new "L1 Suspend"

useful for runtime PM, even though it doesn't mandate reduced power
draw from VBUS.

When a usb serial adapter is used as console, the usb serial console
driver bumps the open_count on the port struct used but doesn't attach
a real tty to it (only a fake one temporaly). If this port is opened later
using the regular character device interface, the open method won't
initialize the port, which is the expected, and will receive a brand new
tty struct created by tty layer, which will be stored in port->tty.
When the last close is issued, open_count won't be 0 because of the
console usage and the port->tty will still contain the old tty value. This
is the last ttyUSB<n> close so the allocated tty will be freed by the
tty layer. The usb_serial and usb_serial_port are still in use by the
console, so port_free() won't be called (serial_close() ->
usb_serial_put() -> destroy_serial() -> port_free()), so the scheduled
work (port->work, usb_serial_port_work()) will still run. And
usb_serial_port_work() does:
(...)
tty = port->tty;
if (!tty)
return;
tty_wakeup(tty);

This patch (as995) cleans up the remains of the former NO_AUTOSUSPEND
quirk. Since autosuspend is disabled by default, we will let
userspace worry about which devices can safely be suspended. Thus the
lengthy series of quirk entries is no longer needed, and neither is
the quirk ID. I suppose someone might eventually run across a hub
that can't be suspended; let's ignore the possibility for now.
The patch also cleans up the hasty way in which autosuspend gets
disabled. Setting udev->autosuspend_delay to -1 wasn't quite right,
because the value is always supposed to be a multiple of HZ. It's
better to leave the delay value alone and set autosuspend_disabled,
which is what the quirk routine used to do.

I added two fields to struct usb_serial_port to keep track of the
throttle state. Other usb-serial drivers typically use private data for
such things, but the generic driver can not really do that because some
of its code is also used by other drivers (which may have their own
private data needs).
As it is, I am not sure that this patch is useful in all scenarios.
It is certainly helpful for low-bandwidth devices that can hold their
data in response to throttling. But for devices that pump data in
real-time as fast as possible (webcam, A/D converter, etc), throttling
may actually cause more data loss.

Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.
The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around. On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).
Where appropriate, an arch may override the generic storage facility and do
something different with the variable. On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.
Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions. Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller. A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.
I've build this code with allyesconfig for x86_64 and i386. I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.
This will affect all archs. Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:
struct pt_regs *old_regs = set_irq_regs(regs);
And put the old one back at the end:
set_irq_regs(old_regs);
Don't pass regs through to generic_handle_irq() or __do_IRQ().
In timer_interrupt(), this sort of change will be necessary:
- update_process_times(user_mode(regs));
- profile_tick(CPU_PROFILING, regs);
+ update_process_times(user_mode(get_irq_regs()));
+ profile_tick(CPU_PROFILING);
I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().
Some notes on the interrupt handling in the drivers:
(*) input_dev() is now gone entirely. The regs pointer is no longer stored in
the input_dev struct.
(*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking. It does
something different depending on whether it's been supplied with a regs
pointer or not.
(*) Various IRQ handler function pointers have been moved to type
irq_handler_t.

* Bug fixes to endpoint handling.
* Workaround for USB_REQ_SET_CONFIGURATION handling,

* Added SND and SND_RAWMIDI dependencies in Kconfig.
* Moved usb_audio.h and usb_midi.h to usb/*.h

* Removed some unneeded stuff inherited from zero.c, more to go.
* Provide DECLARE_* macros for the variable-length structs.

USB serial outside of the kernel tree can not build properly due to
usb-serial.h being buried down in the source tree. This patch moves the
location of the file to include/linux/usb and fixes up all of the usb
serial drivers to handle the move properly.

use them in prism54usb (it is not merged yet, but I plan to do it soon).
